Extensive Endometrial Caseous Necrosis after Miscarriage

Abstract:A 27-year-old woman, G1P0, presented to the hospital with amenorrhea for 1 month after spontaneous miscarriage. She was free of any cough or fever. Her transvaginal ultrasonography revealed a retained product of conception. A diagnostic hysteroscopy showed that the uterine cavity was filled with cheese-like whitish deposits extensively (Fig. 1A), with a small piece of endometrium identifiable on the posterior wall (Fig. 1B). Endometrial biopsy (Fig. 1C) was performed, and histopathological tests revealed caseous necrosis. The computed tomography scan of the chest also showed pulmonary tuberculosis. Her menses restored at 2 months after the start of antitubercular treatment with a regular cycle but remarkably reduced volume. Office hysteroscopy performed 1 year later revealed thin endometrium with obstruction of both ostia (Fig. 1D), and the endometrial biopsy was negative for tuberculosis. She tried in vitro fertilization and embryo transfer but failed because of thin endometrium.
