Abstract:Hydatidiform mole with a coexistent fetus is rare, but this condition has recently shown an increased incidence because of assisted reproduction technology. Herein, we report on a case of triplet pregnancy with a partial hydatidiform mole coexisting with two fetuses. It was diagnosed by p57(kip2) immunohistochemical staining which is helpful in determining histologically equivocal cases. After termination of pregnancy, the patient was diagnosed with persistent gestational trophoblastic disease. Six courses of methotrexate chemotherapy were performed. Her beta-human chorionic gonadotrophin titers then fell to a normal level.

Abstract:BACKGROUND:We describe the treatment strategy for a patient who was found to have a partial hydatidiform mole and coexisting fetus (PHMCF) during the second trimester. The patient was a 38-year-old Chinese woman who had become pregnant following in vitro fertilization and embryo transplantation. We wanted to determine the safest therapeutic strategy to terminate the PHMCF during the second trimester.CASE SUMMARY:In this case, we present a patient who was found to have a PHMCF complicated with serious continuous vaginal bleeding and pre-eclampsia during the second trimester. After careful evaluation, the pregnancy was considered to be unsustainable and was terminated via caesarean section (CS). An infant with weak vital signs and a partially cystic placenta measuring 110 mm × 95 mm × 35 mm were delivered by CS. The patient was discharged after 4 d. The serum levels of β-human chorionic gonadotropin decreased to within a normal range 5 wk after the operation, and no evidence of persistent trophoblastic disease or lung metastases was noticed at the 6-mo follow-up.CONCLUSION:CS termination of PHMCF during the second trimester may be a relatively safe therapeutic strategy.

Abstract:Background: A complete hydatidiform mole (CHM) coexistent with a live fetus is rare; however, in a triplet pregnancy it is even rarer. In such cases, diagnosis is complex and often delayed, since unlike cases of a hydatidiform mole alone, the coexisting CHM often fails to be detected at ultrasound, while Β-hCG, a marker of the disease, is already elevated because of the multiple pregnancy. The risk of maternal complications and of progression to malignant gestational trophoblastic disease is significantly greater in cases of a complete mole. Case: The case reported here refers to a triplet pregnancy consisting of two viable fetuses and a CHM. Conclusion: Outcome was favorable for the two live fetuses; however, the hydatidiform mole progressed to an invasive mole, requiring chemotherapy. Remission was achieved successfully.

Abstract:Complete hydatidiform mole (CHM) with co-existing fetus (CHMCF) is very uncommon. In this study, we investigated the clinicopathological features and DNA genotype in 15 CHMCF. Seven patients (46.7%) developed post-molar gestational trophoblastic disease (GTD), 5 of which had lung metastasis. CHMCF was histologically characterized by a mixed pattern of CHM and the non-molar placenta, mimicking partial hydatidiform mole and placental mesenchymal dysplasia. p57 immunostaining showed a divergent staining pattern, positive in the normal placenta and negative in the CHM component. DNA genotyping of the CHM villi demonstrated exclu-sively paternal alleles consisting of homozygous/monospermic (n = 9) and heterozygous/dispermic patterns (n = 5) at multiple informative loci. We conclude that CHMCF confers a high risk for post-molar GTD. DNA gen-otyping contributes significantly to the precision diagnosis of CHMCF.

Abstract:We report a rare case of a complete hydatidiform mole with two or more coexisting fetuses where both infants survived without complications. A male infant weighing 1258 g and a female infant weighing 880 g were delivered without complications and discharged 95 days after the birth. The analysis of DNA microsatellite polymorphisms indicated that the mole was of paternal origin and probably homozygous. The mother suffered from multiple pulmonary metastasis of the hydatidiform mole which was detected 3 days after the surgery and was successfully treated with methotrexate. A complete hydatidiform mole with two or more coexisting fetuses produces a dilemma between immediate termination and pregnancy continuation. Although the present case resulted in a favorable outcome, a review of the 14 reported cases suggests that the high fetal loss rate (90%) must be a consideration in the decision regarding management of such a pregnancy.

Abstract:Purpose To study the clinical pathological characteristics of 4 pregnancy cases with hydatidiform mole and coexisting fetus. Methods Four pregnancy cases with hydatidiform mole and coexisting fetus were analyzed with review of clinical and pathological data,immunohistochemical detection of p57,FISH and genotyping. Results In 4 cases,3 were twin pregnancies with complete hydatidiform moles and coexisting fetus. Among them,1 was discovered in the fourteenth week with a live fetus being taken odinopoeia,and the other 2 in the twenty-ninth week and survival fetuses were taken out successfully. All of them were p57-negative expression while 2cases were proved 46XX through FISH and genotype analysis. The case of partial hydatidiform mole was p57-positive. Conclusions Complete hydatidiform moles can coexist with a feta-survival normal pregnancy,and incomplete hydatidiform moles are often associated with abnormal or dead fetuses. The clinical development is different in each case of complete hydatidiform mole with coexisting fetuses.The symptoms such as hypertension and vaginal bleeding can happen in early weeks of pregnancy,or in the later period often with fetal being given survival childbirth. Complete hydatidiform mole with coexisting fetus is easier to develop into invasive mole and remote organ metastases such as lung. Pregnancy cases with complete hydatidiform mole and coexisting fetus are often induced by hormone before pregnancy.

Abstract:Background. Triplet pregnancy with complete hydatidiform mole and coexisting twin fetuses is extremely rare with an unknown incidence. Case. Here, we present a case report of a pregnancy with twin fetuses and concurrent hydatidiform mole that resulted in the preterm delivery of one viable baby, the unfortunate intrauterine demise of the other twin, and successful treatment of gestational trophoblastic neoplasia in the postpartum period. Conclusion. This case highlights several important questions that arise for women who choose to carry a multiple gestation pregnancy with complete hydatidiform mole and describes complications that can occur. It is imperative to accurately assess risks and counsel individuals who elect to carry these pregnancies to provide the best possible outcomes.

Abstract:Triplet pregnancy with a coexisting mole is extremely rare. A 26 years old primigravida with multiple gestation and severe pre-eclampsia at 32 weeks gestation was brought to Sri Ramachandra University casualty. In view of abnormal Doppler study with discordant twins emergency lower segment caesarean section was done six days later. Part of the placenta showed molar changes. Histopathology confirmed partial mole. Patient received three cycles of methotrexate in view of rising titres of betahCG. Three months after delivery both babies are alive and well and betahCG for the mother became normal. This pregnancy continued beyond 32 weeks gestational age with both babies being alive. Hence this case is being reported to highlight its rarity.

Abstract:Journal of International Medical Research, Volume 52, Issue 6, June 2024. We herein report a rare case of simultaneous intrauterine molar pregnancy and tubal pregnancy. A woman of childbearing age who had never been pregnant underwent an ultrasound examination 70 days after the onset of menopause. She had a history of ovulation induction. The ultrasound findings suggested a partial hydatidiform mole. She was then pathologically confirmed to have a complete hydatidiform mole after uterine suction dilation and curettage. On postoperative day 4, an ultrasound examination before discharge showed an inhomogeneous mass in the left adnexal region with mild lower abdominal pain. On postoperative day 17, the blood human chorionic gonadotropin level did not drop as expected, and a follow-up examination still indicated a mass in the left adnexal region. We were unable to rule out an ectopic hydatidiform mole. Hysteroscopy with laparoscopic exploration of the left adnexal mass and salpingotomy suggested a diagnosis of intrauterine hydatidiform mole combined with left tubal pregnancy.

Abstract:A rare congenital condition known as hydrometrocolpos (HMC) is characterized by the buildup of secretions in the vagina and uterus as a result of distal obstruction; this condition is frequently linked to imperforate hymen. This case report presents a prenatal diagnosis of isolated HMC in a 30-year-old primigravida, detected at 36 weeks gestation. Initial ultrasound revealed a large cystic mass, confirmed by fetal magnetic resonance imaging to be HMC. Postnatal examination of the new born confirmed imperforate hymen and HMC, which was effectively treated with hymenotomy, resulting in early resolution. This case underscores the importance of prenatal imaging in the early identification and management of HMC to prevent potential complications.

Abstract:Objective To explore the pathologic characteristics and clinical significance of twin pregnancy consisting of a complete hydatidiform mole and coexistent fetus. Methods The clinical presentation,gross morphology,microscopic appearance,immunohistochemical staining,fluorescence in situ hybridization( FISH) results were analyzed in 3 cases and related literature was reviewed,and the clinicopathological features and differential diagnosis were discussed. Results In the 3 cases,the gestational age was 11 weeks,14 weeks and 35 weeks,respectively. On gross,two distinct masses were identified in 3 cases: one exhibited a normal placenta and the other exhibited a complete hydatidiform mole. On microscopic examination,the normal placenta showed well-developed chorionic villi,the complete hydatidiform mole showed hydropic villi. Immunohistochemical staining showed different expression of P57,P53,and Ki67 in two part mole.Fluorescence in situ hybridization( FISH) detected the diploid karyotype of cytotrophoblastic cells in complete hydatidiform moles. Conclusions Twin pregnancy consisting of a complete hydatidiformmole and coexistent fetus is rare. Correct diagnosis plays an important role in clinical treatment, prognostic evaluation and follow-up. The application of immunohistochemistry for p57 and fluorescence in situ hybridization can be help in diagnosis.

Abstract:Cases of hydatidiform moles with a coexisting fetus are sparse and patients are at high risk for severe complications. Patients and physicians often face the dilemma of the wish to continue pregnancy until viability of the fetus while the risk for maternal complications increases. We present an educational case of a twin pregnancy presenting with a hydatidiform mole and coexisting normal fetus with a placenta praevia. The patient developed severe, early onset preeclampsia with beginning HELLP-syndrome and was tested Covid-19 positive in the further course. Termination of pregnancy was conducted via caesarean section at 18 + 6 weeks of pregnancy. Histopathology and genetic analysis confirmed a complete hydatidiform mole next to a normal placenta. Close follow-up examinations were conducted and showed normal findings including ß HCG levels normalizing within 5 months. This case combines several rare, difficult and severe medical conditions and demonstrates how an individualized therapy by an interdisciplinary team covering a highly sensitive topic was developed in a situation where no guidelines exist.

Abstract:Fallopian tubal molar pregnancy is extremely rare, and the main diagnostic tool is the post-operative histopathological diagnosis, as the pre-operative diagnosis is difficult. We report a case of ectopic partial molar pregnancy in the right fallopian tube of a 35-year-old lady that was sent for routine histopathological examination with the clinical diagnosis of ectopic pregnancy and the histopathology report revealed an unusual result.

Abstract:Objective: We present a case of triplet pregnancy with a complete hydatidiform mole, a condition carrying a significant risk to both mother and fetuses and, therefore, raising an important issue on prenatal care. Case report: A 36-year-old patient with gonadal dysgenesis and a 46,XY karyotype successfully conceived a triplet pregnancy after oocyte donation and in vitro fertilization. At mid-trimester, the pregnancy was seen harboring a hydatidiform mole along with two other fetuses by ultrasound. Fetal karyotyping of both fetuses revealed normal results. Serum human chorionic gonadotropin levels were followed up throughout the remainder of pregnancy. At 33 weeks of gestation, preeclampsia ensued with worsening of maternal renal function and high blood pressure, so cesarean section was arranged to deliver a set of two surviving twins. Prophylactic bilateral gonadectomy was done at the same time to curtail the possibility of future malignancy development. Upon pathologic examination of the placentae, hydropic chorionic villi with central cistern formation and nonpolar trophoblastic hyperplasia with atypia and necrosis were found, compatible with complete hydatidiform mole. The gonads showed streaks of fibrous tissue, which resembled ovarian stroma and hilus cells, and an unremarkable tube. Maternal serum human chorionic gonadotropin levels declined gradually to normal level at two months after delivery. Conclusion: This is the first report of triplet pregnancy complicated with one complete hydatidiform mole and preeclampsia in a 46,XY female with gonadal dysgenesis. Our case demonstrated that prolonged gestation with both surviving fetuses was possible by applying intensive monitoring of the whole pregnancy.

Abstract:Transvaginal ultrasound showing a chorionic bump with no vascular flow in a 37‐year‐old G3P2 woman 6‐week pregnant, consistent with partial hydatiform mole confirmed on histopathological study. We suggest an alternative etiology of chorionic bump based on unique histopathological findings (hemorrhage, necrosis, and edema). This case report presents the association of a chorionic bump with a partial mole pregnancy in a 37‐year‐old G3P2 woman. The differential diagnosis of a partial mole was considered after transvaginal ultrasonography revealed a chorionic bump. Subsequent pathological analysis confirmed the diagnosis. While the etiology of the chorionic bump remains dubious, we hypothesize an alternative explanation for chorionic bump formation based on the presence of three unique pathological features: necrosis, edema, and hemorrhage.

Serial hCG and ultrasound measurements for predicting malignant potential in multiple pregnancies associated with complete hydatidiform mole: a report of 2 cases

Abstract:Background: Multiple pregnancy complicated by the presence of a complete hydatidiform mole (CHM) is a rare clinical entity. However, there are enough accumulated data to assist the clinician in the management of such a patient. Cases: Case 1 presented in the first trimester with a twin pregnancy associated with a CHM. In the absence of other complications, the pregnancy was monitored with serial beta-human chorionic gonadotropin (beta-hCG) and ultrasound measurements. A reduction in molar volume suggested regression of the CHM. The pregnancy was successfully carried to term with no progress of the CHM. Beta-hCG levels regressed after delivery. Case 2 had a triplet pregnancy resulting from in vitro fertilization with a CHM detected in the first trimester. Similar measurements were applied. The pregnancy was complicated by episodes of antepartum hemorrhage (APH). Further, the molar volume continued to increase. Because of recurrent APH, delivery was by cesarean section. After delivery, after an initial decline, the beta-hCG level rose, and pulmonary metastases were detected. The patient was treated with single-agent chemotherapy, with complete resolution. Conclusion: The combination of serial ultrasound molar volume measurements with serial beta-hCG estimation may assist the clinician in predicting which of these rare complicated pregnancies will result in the development of trophoblastic neoplasia.

Abstract:AIM: We present two cases of triplet pregnancy with complete hydatidiform mole (CHM) in contrasting outcomes and discuss the complications of mothers and outcomes of fetuses through a literature review, raising an important issue on the management of this special pregnancy.METHODS: We share our manage experience for two cases of triplet pregnancy with CHM and retrospectively analyze 18 similar pregnancies reported previously with different pregnancy outcomes.RESULTS: In our cases, one case receiving Clomiphene ovulation induction delivered two live fetuses by cesarean section at 30+ weeks without GTN (gestational trophoblastic neoplasia), unfortunately, the other case following ICSI-ET terminated the pregnancy in the setting of complications at 18+ weeks without GTN. No severe complications were detected during pregnancy and no pGTD was developed after delivery in neither of the pregnant.CONCLUSIONS: Co-existing complete hydatidiform mole in multiple pregnancies may become more common owing to the spreading use of ART. The decision for whether continue pregnancy depending on the personalized conditions including the complications of the pregnancy, the outcomes of the fetuses, the gestational age for delivery, and the potential progression of persistent gestational trophoblastic disease (pGTD). Furthermore, close monitor is necessary for the pregnant with triplet pregnancy with CHM who want to continue pregnancy.

Obstetrical and oncological outcomes of twin pregnancies with hydatidiform mole and coexisting fetus

Abstract:Objective: To evaluate the obstetrical and oncological progression of twin pregnancies with hydatidiform mole coexisting fetus (HMCF). Materials and methods: Using a retrospective method based on patients from the Women's Hospital, Zhejiang University School of Medicine database between January 1990 and October 2020, 17 patients were histologically confirmed as having HMCF, and the patients' prenatal diagnosis, outcomes and development of gestational trophoblastic neoplasia (GTN) were reviewed. Results: Among these 17 cases, 11 (64.71%) cases were complete hydatidiform mole coexisting fetus (CHMCF), and 6 (35.29%) cases were partial hydatidiform mole coexisting fetus (PHMCF). The gestational age at diagnosis of CHMCF was significantly earlier than that of PHMCF [9 (8-24) vs. 18 (11-32) weeks, respectively, P < 0.05]. The live birth rate of PHMCF was slightly higher than that of CHMCF (33.33%; 18.18%), but this difference was not statistically significant. The overall rate of GTN incidence of HMCF was 47.06% (8/17), and the GTN rates of PHMCF and CHMCF were 33.33% (2/6) and 54.55% (6/11), respectively. There was no significant difference in the GTN rate between patients who chose to continue pregnancy and those who terminated pregnancy before 24 weeks of gestation. The GTN rate of patients with term delivery was not significantly higher than that of preterm delivery. Conclusion: In HMCF cases, the incidence rate of CHMCF was higher than that of PHMCF, and PHMCF is more difficult to diagnose in the early stage. Continuing pregnancy does not increase the risk of GTN compared to terminating pregnancy. In cases of HMCF, when the fetal karyotype is normal and maternal complications are controlled, it is safe to continue the pregnancy and extend it to term.
