Abstract:Objective: This study aimed to explore the most effective nonsurgical therapy to treat stress urinary incontinence (SUI). Methods: Data were pooled and the Bayesian random effects model was utilized to evaluate the inconsistencies between indirect and direct proof. Network sub-analyses, sensitivity and subgroup analysis, and standard pairwise comparisons were utilized in preparation of the main analysis. Results: A total of 17 RCTs (852 participants) were enrolled. With regard to therapy, pelvic floor muscle training (PFMT) was more often recommended than biofeedback (BF). The combination of PFMT and BF was remarkably promoted than PFMT alone or PFMT with vaginal cone (VC) or electrical stimulation (ES). The QL scores of patients who underwent PFMT were noticeably improved compared with those of patients who underwent ES. The combination of BF and PFMT was remarkably promoted compared with BF alone. VC was superior to ES with regard to QL scores. By contrast, ES alone was the least preferred method to decrease urine leakage and to promote QL. Conclusion: The combination of BF and PFMT was proven to be an optimal method to treat SUI which aims to promote urine leakage and LQ compared with either PFMT alone, ES, VC, or circular muscle (CME), while ES was the least preferred method to promote LQ.
Pelvic Floor Muscle Exercises can Effectively Improve Urinary Incontinence after Radical Prostatectomy: Systematic Review and Meta-Analysis Based on Randomised Controlled Trials

Abstract:Objective: This study aims to assess the effect of pelvic floor muscle exercise (PFME) on urinary incontinence after radical prostatectomy. Methods: PubMed, Web of Science, Embase, Cochrane Library, China National Knowledge Infrastructure (CNKI), VIP and other domestic and foreign databases were searched for published literature until December 2023 on the effect of pelvic muscle exercise on urinary incontinence in patients after radical prostatectomy. The retrieved literatures were screened, and data were extracted. After evaluating the quality of the literatures, RevMan 5.4 software was used for meta-analysis. Results: This work included nine articles consisting of 1208 sufferers with urinary incontinence after radical prostatectomy. The forest plot showed that patients in the experimental group had better postoperative outcomes at 1 month (Relative Risk (RR) = 3.38, 95% confidence interval (CI) (1.83; 6.25)), 3 months (RR = 1.99, 95% CI (1.67; 2.38)) and 6 months (RR = 1.34, 95% CI (1.20; 1.49)). The incidence of urinary incontinence was statistically significant compared with the control group (p < 0.05). Patients in the experimental group 12 months after surgery (RR = 1.13, 95% CI (0.99; 1.23)) showed no significant difference in the incidence of urinary incontinence compared with the control group (p > 0.05). Conclusions: PFME can significantly increase the recovery rate of urinary incontinence in sufferers with prostate cancer at 1, 3 and 6 months after radical surgery but have no significant improvement at 12 months. Urodynamic analysis may be needed for patients with long-term urinary incontinence.

Abstract:Objectives:To study and observe the influence of biofeedback therapy on the pelvic floor mus-cle and urodynamic state of patients with stress incontinence.Methods:60 patients with stress incontinence in our hospital from April 2011 to April 2014 were selected and randomly divided into control group (conventional pelvic floor muscle training group)and observation group (biofeedback therapy group),30 cases in each group.The mic-turition related indexes,pelvic floor muscle,urine pad test and ICS urinary incontinence quality of life question-naire,urodynamic indexes of two groups before the treatment and 4th and 8th week after the treatment were respec-tively detected and compared.Results:The micturition related indexes,pelvic floor muscle and urodynamic indexes of the observation group at 4th and 8th week after the treatment were all significantly better than those of control group,all P <0.05,with significant differences.Conclusion:The influence of biofeedback therapy for the pelvic floor muscle and urodynamic state of patients with stress incontinence is great,which is more helpful for the effective and quick improvement of disease.
Effectiveness of Preoperative Pelvic Floor Muscle Training for Urinary Incontinence after Radical Prostatectomy: a Meta-Analysis

Abstract:BACKGROUND:Radical prostatectomy (RP) is the most common treatment for patients with localized prostate cancer. Urinary incontinence (UI) is a significant bothersome sequela after radical prostatectomy that may dramatically worsen a patient's quality of life. Pelvic floor muscle training (PFMT) is the main conservation treatment for men experiencing urinary incontinence; however, whether additional preoperative PFMT can hasten the reestablishment of continence is still unclear. The objective of this meta-analysis is to determine whether the effectiveness of preoperative plus postoperative PFMT is better than postoperative PFMT only for the re-establishment of continence after RP.METHODS:A meta-analysis was performed after a comprehensive search of available randomized controlled trials (RCTs). Quality of the included studies was assessed by the Cochrane Risk of Bias tool. Efficacy data were pooled and analyzed using Review Manager (RevMan) Version 5.0. Pooled analyses of continence rates 1, 3, 6, and 12 months postoperatively, using relative risk (RR) and 95% confidence intervals (CIs), were conducted. For data deemed not appropriate for synthesis, a narrative overview was conducted.RESULTS:Five eligible studies were ultimately included in this analysis. No significant differences in continence rates were detected at the early (1- and 3-month) time points: RR = 1.21, 95% CI = 0.71-2.08, P = 0.48; RR = 1.1, 95% CI = 0.09-1.34, P = 0.34, respectively), interim (6-month time point: RR = 0.98, 95% CI = 0.93-1.04, P = 0.59), or late recovery stage (RR = 0.93, 95% CI = 0.67-1.29, P = 0.66). Outcomes reported were time to continence in two trials and quality of life in three, but results were inconclusive because of insufficient data.CONCLUSION:According to this meta-analysis, additional preoperative PFMT did not improve the resolution of UI after RP at early (≤3-month), interim (6-month), or late (1-year) recovery stages. However, the results of time to continence and quality of life were inconclusive because of insufficient data. More high-quality RCTs are needed for better evaluation of the effectiveness of preoperative PFMT on post-prostatectomy UI.
The effect of pelvic floor muscle training on urinary incontinence in patients with stroke: a systematic review and meta-analysis

Abstract:Background: No other systematic review presented the effectiveness of pelvic floor muscle training (PFMT) in individual with stroke. Aims: The purpose of this systematic review and meta-analysis was to demonstrate the effectiveness of PFMT for urinary incontinence in patients with stroke. Methods: "Web of Science (WoS), Cochrane Library, PubMed and Scopus" databases were searched. "Revised Cochrane risk-of-bias tool randomized trials (RoB 2)" and "PEDro" were used to assess the risk of bias and methodological quality of the studies. Narrative synthesis and meta-analysis were conducted to present the results. Results: A total of 8 articles were included in the review. Seven articles were classified as "good" level evidence. Four articles were considered to have "high risk" of bias for the overall score. Regarding two homogenous studies, PFMT-based rehabilitation was not superior standard urinary incontinence therapy on SF-36-Social Function score at 12-week follow-up (ES 0.47, 95% CI - 0.16-0.96). Data pooling of two studies did not provide an additional advantage of pelvic muscle training on 3 days voiding diary (night, total day) score at 12-week follow-up (ES 28, 95% CI - 0.61-0.48; ES 0.30, 95% CI - 0.23-0.95). On the other hand, low-quality evidence demonstrated that pelvic muscle training yielded better results on daytime voiding (ES 0.28, 95% CI 0.04-1.16). Conclusions: The results demonstrated that PFMT had positive effects in terms of daytime urination frequency and incontinence. Although some studies have reported positive effects on symptoms, function, strength, and endurance, the generalizability of these results is controversial. Further studies should assess the quality-of-life and function with urinary incontinence and stroke-specific tools.
Analysis of conventional versus advanced pelvic floor muscle training in the management of urinary incontinence after radical prostatectomy: a systematic review and meta-analysis of randomized controlled trials

Abstract:BACKGROUND: The underutilization of additional supportive muscles is one of the potential reasons for suboptimal efficacy of conventional pelvic floor muscle training (CPFMT). The present study concentrates on any advantage of advanced pelvic floor muscle training (APFMT) in patients with urinary incontinence (UI) after radical prostatectomy (RP).METHODS: Literature search was conducted on PubMed, Embase, Cochrane Library and Web of Science from database inception to February 2020. The data analysis was performed by the Cochrane Collaboration's software RevMan 5.3.RESULTS: Both APFMT and CPFMT groups indicates superiority over baseline in terms of pad number, the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) score, pad weight at short-term follow-up, and PFME and PFMS at intermediate-term follow-up. No adverse events were reported in all included studies. Patients receiving APFMT had a similar attrition rate to those receiving CPFMT (18/236 <i>vs</i>. 22/282, P=0.61). Compared to CPFMT group, APFMT group provided intermediate-term advantages in terms of pad number (MD: -0.75, 95% CI: -1.36 to -0.14; P=0.02), ICIQ-SF score (MD: -3.79, 95% CI: -5.89 to -1.69; P=0.0004), PFME (MD: 1.93, 95% CI: 0.99 to 2.87; P<0.0001) and pad weight (MD: -1.40, 95% CI: -1.70 to -1.00; P<0.00001).CONCLUSIONS: Current evidence indicated that APFMT might facilitate the recovery of UI after RP according to intermediate-term advantages over CPFMT in terms of pad number, ICIQ-SF score, PFME and pad weight. Further standardized, physiotherapist-guided and well-designed clinical trials conducted by large multicenter and experienced multidisciplinary clinicians are still warranted.

Abstract:Purpose: The aim of this study is to provide treatment for patients with urinary incontinence at different periods after radical prostatectomy. Methods: The PubMed, Embase, Cochrane, and Web of Science were searched for all literature on the effectiveness on urinary control after radical prostate cancer between the date of database creation and 15 November 2023 and performed a quality assessment. A network meta-analysis was performed using RevMan 5.3 and Stata 17.0 software and evaluated using the surface under the cumulative ranking curve. Results: The results of the network meta-analysis showed that pelvic floor muscle therapy including biofeedback with professional therapist–guided treatment demonstrated better results at 1 month to 6 months; electrical stimulation, biofeedback, and professional therapist guidance may be more effective at 3 months of treatment; professional therapist–guided recovery may be less effective at 6 months of treatment; and combined therapy demonstrated better results at 1 year of treatment. During the course of treatment, biofeedback with professional therapist–guided treatment may have significant therapeutic effects in the short term after surgery, but, in the long term, the combination of multiple treatments (pelvic floor muscle training+ routine care + biofeedback + professional therapist–guided treatment + electrical nerve stimulation therapy) may address cases of urinary incontinence that remain unrecovered long after surgery. Conclusion: In general, all treatment methods improve the different stages of functional recovery of the pelvic floor muscles. However, in the long term, there are no significant differences between the treatments. Given the cost-effectiveness, pelvic floor muscle training + routine care + biofeedback + professional therapist–guided treatment + electrical nerve stimulation therapy within 3 months and pelvic floor muscle + routine care after 3 months may be a more economical option to treat urinary incontinence. Efficacy Of Pelvic Floor Muscle Training In The Treatment Of Female Pelvic Organ Prolapse: A Meta-Analysis Of Randomized Controlled Trials

Abstract:This study was to evaluate the efficacy of pelvic floor muscle training (PFMT) as a treatment for females with pelvic organ prolapse (POP). PubMed, Cochrane, Embase, Wanfang, China National Knowledge Infrastructure (CNKI), and Weipu (VIP) databases were searched using the index words to identify qualified randomized controlled trials (RCTs), and relevant literature sources were also searched. The latest research was performed in February 2017. Relative risk (RR), mean difference (MD) along with 95% confidence interval (CI) were used to analyze the main outcomes. Eight RCTs were involved in the meta-analysis with 919 patients in the treatment group and 903 patients in the control group. The results indicated that females receiving PFMT decreased the POP-Q stage (OR: 1.76, 95% CI: 1.24-2.48), as compared with the control group. There was no statistical difference in adding one or more POP-Q stage (OR: 0.98, 95% CI: 0.74-1.29), no change of POP-Q stage (OR: 0.97, 95% CI: 0.77-1.23), the overall POP-Q stage change (OR: 1.11, 95% CI: 0.95-1.30), PFDI-20 (SMD: 0.13, 95% CI: -0.14-0.40), POPDI-6 (SMD: 0.14, 95% CI: -0.15-0.42), CRADI-8 (SMD: 0.03, 95% CI: -0.11-0.16), UDI-6 (SMD: 0.17, 95% CI: -0.10-0.43), PFIQ7 (SMD: 0.05, 95% CI: -0.09-0.18) after the PMFT treatment, as compared with the control group. In conclusion, females receiving PFMT showed an advantage in decreasing the POP-Q stage. However, more high quality RCTs are still needed to confirm PFMT is a better treatment for POP.

Abstract:Background: Pelvic floor muscle training (PFMT) with biofeedback is used widely in treating patients with stress urinary incontinence (SUI), despite unclear evidence. We conducted a meta-analysis of the literature to evaluate the efficacy of treatment after PFMT with and without biofeedback in SUI patients. Methods: We searched PubMed, CENTRAL, CINAHL, and Science Direct for randomized controlled trials (RCTs) of PFMT with and without biofeedback for SUI. RCTs were screened with our eligibility criteria, and the risk of bias was assessed according to the Cochrane risk of bias tool for randomized trials. The outcomes analyzed were pelvic floor muscle (PFM) strength, incontinence episode, daytime micturition, and nighttime micturition, all measured as mean difference (MD) with 95% confidence intervals (CIs). Heterogeneity and publication bias were analyzed using the I 2 test and a funnel plot, respectively. Results and Discussion: Pooled analysis of five RCTs involving 207 patients showed that the difference in PFM strength and nighttime micturition between both groups was significant. Although PFM strength improvement favors biofeedback-assisted pelvic floor muscle training (BPFMT) (MD 12.29, 95% CI 2.33, 22.25, p=0.02), in contrast, nighttime micturition was significantly reduced in the PFMT group (MD 0.44, 95% CI 0.12 to 0.77, p=0.007). Differences in incontinence episode and daytime micturition were not significant (MD -0.08, 95% CI -0.57 to 0.41, p=0.75 and MD 0.55, 95% CI -0.36 to 1.46, p=0.24, respectively). Conclusion: This meta-analysis showed that BPFMT had a better outcome in improving PFM strength, while nighttime micturition was, on the contrary, better in PFMT only. Meanwhile, no significant differences in incontinence episodes and daytime micturition outcomes were noted between both groups. With the present evidence, routine use of BPFMT is not necessary for current clinical practice.

Abstract:Introduction and hypothesisWe performed a review of the literature reporting on the effects of pelvic floor muscle training (PFMT) on female sexual function (SF).MethodsPubmed (from 1946 to December 2014), Ovid Medline (from 1946 to December 2014), CINAHL (from 1937 to December 2014), PsycINFO (from 1805 to December 2014), Scopus and Cochrane Central Register of Controlled Trials were searched by two independent reviewers. Randomised controlled trials (RCTs) investigating the impact of PFMT on women’s SF published in English were included. Methodological quality was scored using the PEDro scale. Data were analysed qualitatively and interpreted.ResultsA total of 1341 women were included in the eight RCTs covered by this review. The studies were published between 1997 and 2014. Methodological scores were between 4 and 7. The sample included derived from heterogeneous populations of women. In only one study was SF the primary outcome measure. Pelvic floor dysfunction was an inclusion criterion in the majority of studies. Most studies reported a significant improvement in SF score after PFMT between control and intervention groups.ConclusionsAlthough most studies indicated an improvement of at least one sexual variable in women with pelvic floor dysfunction, and one study demonstrated an improvement in SF in postpartum women selected independently of their continence status, the results need to be interpreted with caution. High-quality RCTs specifically designed to investigate the impact of PFMT on women’s SF are required.

Abstract:Urinary incontinence (UI) is a prevalent health concern in females over 60, but it is prevalent in a smaller percentage of males. The medical and social elements of UI are crucial. This disorder may cause embarrassment and low self-esteem, reducing social and physical activities. Consequently, this may impair healthy aging. Researchers have shown that pelvic floor muscle training (PFMT) may improve UI symptoms in middle-aged, older, and young people. Clinical practice highlights the importance of PFMT for UI due to its low cost-effectiveness. To assess PFMT's overall efficacy, we conducted a systematic review of the literature (SRL) and a meta-analysis (MA) of randomized clinical trials (RCTs). The previous 10 years of published material were combed using the online databases the Cochrane Library, PubMed, and Google Scholar. Eligible total studies were RCTs referring to the effectiveness of PFMT. The pooled incidence, risk ratio, and 95% confidence interval (CI) of the effectiveness of PFMT were calculated using the fixed effects model. Given the possibility of a between-study variance, we used the fixed effects model rather than the random effects model.

Abstract:ObjectivesThe objectives of the present review was to present and discuss evidence for pelvic floor muscle (PFM) training on female stress urinary incontinence (SUI), pelvic organ prolapse (POP) and sexual dysfunction.MethodsThis manuscript is based on conclusions and data presented in systematic reviews on PFM training for SUI, POP and sexual dysfunction. Cochrane reviews, the 4th International Consultation on Incontinence, the NICE guidelines and the Health Technology Assessment were used as data sources. In addition, a new search on Pubmed was done from 2008 to 2011. Only data from randomized controlled trials (RCTs) published in English language is presented and discussed.ResultsThere is Level 1, Grade A evidence that PFM training is effective in treatment of SUI. Short-term cure rates assessed as <2 g of leakage on pad testing vary between 35 and 80%. To date there are 5 RCTs showing significant effect of PFM training on either POP stage, symptoms or PFM morphology. Supervised and more intensive training is more effective than unsupervised training. There are no adverse effects. There is a lack of RCTs addressing the effect of PFM training on sexual dysfunction.ConclusionsPFM training should be first line treatment for SUI and POP, but the training needs proper instruction and close follow-up to be effective. More high quality RCTs are warranted on PFM training to treat sexual dysfunction.

Abstract:Background: After hysterectomy, women could experience pelvic floor dysfunction and negative impact on quality of life, which could be improved by pelvic floor muscle training. Objective: To investigate effects of pelvic floor muscle training on urinary symptoms, vaginal prolapse, sexual function, pelvic floor muscle strength, and quality of life after hysterectomy. Methods: Systematic review with meta-analyses of randomized controlled trials. Trials with pelvic floor muscle training in women after hysterectomy were included. The outcomes measures were urinary symptoms, vaginal prolapse, sexual function, pelvic floor muscle strength, and quality of life. Quality of evidence was assessed by adopting the GRADE approach. Results: Six trials, involving 776 participants, were included. The mean PEDro score of trials was 5.5. Moderate-quality evidence suggested that pelvic floor muscle training improves sexual function by 5 points (95% CI: 4, 6) on the Female Sexual Function Index, compared with no intervention. It might affect strength (SMD 0.5; 95% CI: -0.4, 1.3), quality of life (SMD 0.5 points out of 108, 95% CI: -0.1, 0.9), urinary symptoms (RD -0.02; 95% CI: -0.06, 0.1); however, the estimates were too imprecise. In addition, it produces no or negligible effects on vaginal prolapse (RD 0; 95% CI: -0.1, 0.1). Long-term effects remain uncertain. Conclusion: This systematic review provides moderate-quality evidence that pelvic floor muscle training is effective for improving women's sexual function after hysterectomy, in comparison with no intervention. Benefits on urinary symptoms, pelvic floor muscle strength, quality of life, and vaginal prolapse remains unclear. Also, the effects beyond the intervention period remains uncertain.
Pelvic floor training to prevent stress urinary incontinence: A systematic review

Abstract:Introduction: Stress urinary incontinence (SUI) is a common disorder in women that has a negative impact on quality of life. Pregnancy and childbirth are considered important risk factors that directly affect the pelvic floor during pregnancy and labour, increasing the risk of pelvic floor dysfunction, with prevalence rates of SUI in the postpartum period ranging from 30 to 47% during the first 12 months. Objective: To determine the effectiveness of pelvic floor muscle training (PFMT) in the prevention of SUI in women during the antenatal and postnatal period by reviewing and evaluating the available scientific literature. Methods: This is a systematic review, using only randomised controlled trials. We searched the databases Pubmed, Scopus, Cochrane and PEDro. We reviewed 7 prospective studies in English and Portuguese, which included 1,401 pregnant women of legal age who underwent PFMT to prevent SUI. Results: The results allowed us to establish that PFMT is used for pelvic floor muscles and that this intervention, applied with the appropriate methodology, can prevent or cure SUI. Conclusions: The application of PFMT in an early stage of pregnancy has positive effects on the continence capacity after delivery.

Abstract:The aim of this study was to explore the effect of pelvic floor muscle training as an adjunct for prolapse surgery. Participants were randomly allocated into 2 groups receiving lifestyle advice (n = 48) and lifestyle advice + pelvic floor muscle training (n = 49). This study compared the total score (Pelvic Floor Distress Inventory-20) and the scores of the Urinary Distress Inventory-6/Colorectal Anal Distress Inventory-8/Pelvic Organ Prolapse Distress Inventory-6 at admission, discharge, and 42 and 60 days after surgery. The UDI-6 score in the lifestyle advice + pelvic floor muscle training group was lower than that in the lifestyle advice group 42 and 60 days after surgery. Pelvic floor muscle training can improve the bladder-related symptoms to have positive significance for pelvic floor rehabilitation after pelvic organ prolapse surgery.

Abstract:Objectives To investigate the efficacy of pelvic floor muscle training (PFMT) in treating female sexual dysfunction. Data Sources A systematic review of databases, including PubMed, Ovid Medline, CINAHL, Embase, BVSalud, Scopus, and Cochrane Library, was performed in July 2021 and updated in May 2023. Study eligibility criteria Full-text articles of randomized controlled trials (RCTs) comparing PFMT and no intervention or another conservative treatment in at least one arm to improve women's sexual function or treat sexual dysfunction were included. Study appraisal and synthesis methods The data for this review was extracted and analyzed by two independent reviewers. Data on the characteristics of each intervention was extracted using the Consensus on Exercise Reporting Template (CERT). The risk of bias and certainty of evidence was assessed using the Physiotherapy Evidence Database (PEDro) scale and the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) criteria, respectively. A meta-analysis was conducted considering the post-treatment mean score difference of female sexual function index (FSFI) between the control and treatment groups. Results Twenty-one RCTs were included in this review. The CERT revealed a varying quality of the PFMT protocols. Four studies were included in the meta-analysis showing that PFMT improved arousal (1.49; 95% confidence interval [CI]: 0.13 to 2.85), orgasm (1.55; 95%CI: 0.13 to 2.96), satisfaction (1.46; 95%CI: 0.14 to 2.77), pain (0.74; 95%CI: 0.11 to 1.37), and the FSFI overall score (7.67; 95%CI: 0.77 to 14.57). The GRADE showed a very low certainty of evidence due to the data's high clinical and statistical heterogeneity. No side effects of the interventions were reported. Conclusion This systematic review with meta-analysis showed that PFMT improved female FSFI total score and several subscales; however, the certainty of the evidence is low.

Abstract:Objective To evaluate the effect of postpartum pelvic floor muscle training in prevention and treatment of urinary incontinence.Methods 156 women were randomized divided into training group and control group.82 women in the training group had participated an eight-week pelvic floor muscle training program instructed by specialized nurse,74 women in the control group only had participated postpartum health instruction.Clinical assessment of pelvic floor muscle training program were taken by questionary,vaginal palpation and a standardized pad test.Results Significantly more women in the control group than in the training group reported stress urinary incontinence,but the scores of pelvic floor muscle strength in the control group was demonstrated lower than in the training group,.Conclusion Postpartum pelvic floor muscle training was effective in prevention and treatment of urinary incontinence.The long effect need to further investgate.

Abstract:The purpose of this review is to provide an in-depth overview of the role of pelvic floor muscle (PFM) training in the management of stress urinary incontinence (SUI). The definition, epidemiology, and pathogenesis of SUI are described. We review the anatomy of the PFM and the importance of PFM strength in maintaining urinary continence and establishing normal voiding function. A brief description of the surgical options currently available for SUI and the existing data regarding the role of perioperative PFM training for SUI are included. Critical research questions to better evaluate and assess PFM training during the perioperative period are proposed. Promising novel approaches in the treatment of SUI are also presented. This review is useful for physiatrists, urogynecologists, female urologists, and nurse practitioners who specialize in the management and treatment of women with SUI. LEVEL OF EVIDENCE: IV.
