Abstract:Respiratory droplets are bioaerosols that originate from the respiratory tract. Knowing their deposition characteristics during exhalation would facilitate the understanding of the source of large respiratory droplets and their importance in the spread of respiratory infectious diseases. In this study, computational fluid dynamics is used to simulate the motion and deposition of droplets released from either trachea or bronchi in a realistic reconstruction of the human respiratory tract. Influences of airflow structures and locations of droplet generation on droplet deposition are studied, and droplets with diameters between 1 and 50 mu m are examined. The deposition of droplets is found to be influenced mainly by the droplet diameter and the flow rate of exhalation. The number of droplets released from the trachea or bronchi that can escape into the environment decreases as the flow rate increases. When the flow rate is low (10 L/min), the critical diameter of droplets generated in the lower respiratory system that can escape into the air is approximately 12 mu m, but this diameter is approximately 5 mu m when the flow rate is medium (30 to 60 L/min) or large (90 L/min). The larynx is the dominant site of deposition for droplets smaller than the critical diameter, while trachea and bronchus are more important locations that account for the deposition of larger droplets. This study indicates that the lower respiratory tract is an important source of fine droplets (<5 mu m) in indoor environments, and larger droplets probably originate from the upper respiratory tract, which needs further investigation. Abstract:Aerosol bolus dispersion, which has excited enormous interest in lung medicine due to its possible use as an efficient toolfor the non-invasive clinical diagnosis of lung function, was simulated by a Monte Carlo model based on the concept of effective diffusivities and a stochastic lung geometry. The mathematical approach enabled the computation of essential characteristics of the exhaled bolus (half width, standard deviation, skewness, and mode shift) as well as the estimation of their dependence upon the volumetric lung depth (VLD) of the inhaled bolus. Results of the dispersion model generally show a very good correspondence with preliminary published experimental data. Half width and standard deviation of the exhaled bolus increase with VLD according to specific functions, whereas skewness and mode shift are subject to a decrease. While no correlation between bolus dispersion and flow rate could be worked out with the model, dispersion linearly increased with total lung capacity (TLC).

Abstract:Effective management of asthma is dependent on achieving adequate delivery of the drugs into the lung. Inhalers come in the form of dry powder inhalers (DPIs) and metered dose inhalers (pMDIs) with the former requiring a deep fast breath for activation while there are no restrictions on inhalation rates for the latter. This study investigates two aerosol medication delivery methods (i) an idealised case for drug particle delivery under a normal breathing cycle (inhalation-exhalation) and (ii) for an increased effort during the inhalation with a breath hold. A computational model of a human tracheobronchial airway was reconstructed from computerised tomography (CT) scans. The model's geometry and lobar flow distribution were compared with experimental and empirical models to verify the current model. Velocity contours and secondary flow vectors showed vortex formation downstream of the bifurcations which enhanced particle deposition. The velocity contour profiles served as a predictive tool for the final deposition patterns. Different spherical aerosol particle sizes (3-10μm, 1.55g/cm(3)) were introduced into the airway for comparison over a range of Stokes number. It was found that a deep inhalation with a breath hold of 2s did not necessarily increase later deposition up to the sixth branch generation, but rather there was an increase in the deposition in the first few airway generations was found. In addition the breath hold allows deposition by sedimentation which assists in locally targeted deposition. Visualisation of particle deposition showed local "hot-spots" where particle deposition was concentrated in the lung airway.

Anatomic localization of 24- and 96-h particle retention in canine airways

Abstract:Long-term retention of particles in airways is controversial. However, precise anatomic localization of the particles is not possible in people. In this study the anatomic location of retained particles after shallow bolus inhalation was determined in anesthetized, ventilated beagle dogs. Fifty 30-cm(3) boluses containing monodisperse 2.5-micron polystyrene particles (PSL) were delivered to a shallow lung depth of 81-129 cm(3). At 96 h before euthanasia, red fluorescent PSL were used; at 24 h, green fluorescent PSL and (99m)Tc-labeled PSL were used. Clearance of (99m)Tc-PSL was measured during the next 24 h. Sites of particle retention were determined in systematic, volume-weighted random samples of microwave-fixed lung tissue. Precise particle localization and distribution was analyzed by using gamma counting, conventional fluorescence microscopy, and confocal microscopy. Within 24 h after shallow bolus inhalation, 50-95% of the deposited (99m)Tc-PSL were cleared, but the remaining fraction was cleared slowly in all dogs, similar to previous human results. The three-dimensional deposition patterns showed particles across the entire cross-sectional plane of the lungs at the level of the carina. In these locations, 33 +/- 9.9% of the retained particles were found in small, nonrespiratory airways (0.3- to 1-mm diameter) and 49 +/- 10% of the particles in alveoli; the remaining fraction was found in larger airways. After 96 h, a similar pattern was found. These findings suggest that long-term retention in airways is at the bronchiolar level.
Mode shift of an inhaled aerosol bolus is correlated with flow sequencing in the human lung

Abstract:We studied the effects on aerosol bolus inhalations of small changes in convective inhomogeneity induced by posture change from upright to supine in nine normal subjects. Vital capacity single-breath nitrogen washout tests were used to determine ventilatory inhomogeneity change between postures. Relative to upright, supine phase III slope was increased 33 +/- 11% (mean +/- SE, P < 0.05) and phase IV height increased 25 +/- 11% (P < 0.05), consistent with an increase in convective inhomogeneity likely due to increases in flow sequencing. Subjects also performed 0.5-microm-particle bolus inhalations to penetration volumes (V(p)) between 150 and 1,200 ml during a standardized inhalation from residual volume to 1 liter above upright functional residual capacity. Mode shift (MS) in supine posture was more mouthward than upright at all V(p), changing by 11.6 ml at V(p) = 150 ml (P < 0.05) and 38.4 ml at V(p) = 1,200 ml (P < 0.05). MS and phase III slope changes correlated positively at deeper V(p). Deposition did not change at any V(p), suggesting that deposition did not cause the MS change. We propose that the MS change results from increased sequencing in supine vs. upright posture.
Detailed deposition analysis of inertial and diffusive particles in a rat nasal passage

Abstract:Rats have been widely used as surrogates for evaluating the health effects of inhaled airborne particulate matter. To provide a thorough understanding of particle transport and deposition mechanisms in the rat nasal airway, this article presents a computational fluid dynamics (CFD) study of particle exposure in a realistic rat nasal passage under a resting flow condition. Particles covering a diameter range from 1 nm to 4 µm were passively released in front of the rat's breathing zone, and the Lagrangian particle tracking approach was used to calculate individual particle trajectories. Detailed particle deposition analysis shows the deposition of inertial particles >2 µm is high in the rat nasal vestibule and more than 70% of all inhaled inertial particles were trapped in this region. While for diffusive nanoparticles, the vestibule filtration effect is reduced, only less than 60% of inhaled nanoparticles were blocked by the anterior nasal structures. The particle exposure in the olfactory region only shows notable deposition for diffusive nanoparticles, which peaks at 9.4% for 5 nm particles. Despite the olfactory deposition remains at a low level, the ratio between the olfactory and the main passage is kept around 30-40% for 10-800 nm particles, which indicates a particle-size-independent distribution pattern in the main nasal passage and olfactory. This study provides a deep understanding of particles deposition features in a rat nasal passage, and the research findings can aid toxicologist in inter-species exposure-response extrapolation study.

Abstract:The deposition of micron particles finds importance in meteorology and several engineering applications such as deposition of dust in gas lines, carbon deposition in engine exhaust, designing effective air-cleaning systems and estimating deposition of inhaled drug or atmospheric pollutants to determine its consequences on human health. Although the existing literature on deposition in straight tubes is quite mature, an experimental study on deposition in micro capillaries with a wide ranges of Re that models particle dynamics in lungs, is missing. The deposition of atmospheric pollutants and nebulized drugs in the lung depends on various biological factors such as flow properties, lung morphology, breathing patterns, particle properties, deposition mechanism, etc. To complicate matters, each breath manifests flows spanning a wide range of Reynolds numbers in various regions of the lung. In this study, the deposition of nebulized aerosol was experimentally investigated in phantom bronchioles of diameters relevant to the 7th to the 23rd branching generations and over the entire range of Re manifest during one breathing cycle. The aerosol fluid was loaded with boron doped carbon quantum dots as a fluorophore. An aerosol was generated of this mixture fluid using an ultrasonic nebulizer, producing droplets of 6.5$\mu$m as the mean diameter. The amount of aerosol deposited on the bronchiole walls was measured using a spectrofluorometer. Finally, a universal bronchiole scale deposition model is proposed which can form the building block for lung-scale aerosol deposition prediction.

Measuring Anatomical Distributions of Ventilation and Aerosol Deposition with PET-CT

Abstract:In disease, lung function and structure are heterogeneous, and aerosol transport and local deposition vary significantly among parts of the lung. Understanding such heterogeneity is relevant to aerosol medicine and for quantifying mucociliary clearance from different parts of the lung. In this chapter, we describe positron emission tomography (PET) imaging methods to quantitatively assess the deposition of aerosol and ventilation distribution within the lung. The anatomical information from computed tomography (CT) combined with the PET-deposition data allows estimates of airway surface concentration and peripheral tissue dosing in bronchoconstricted asthmatic subjects. A theoretical framework is formulated to quantify the effects of heterogeneous ventilation, uneven aerosol ventilation distribution in bifurcations, and varying escape from individual airways along a path of the airway tree. The framework is applied to imaging data from bronchoconstricted asthmatics to assess the contributions of these factors to the unevenness in lobar deposition. Results from this analysis show that the heterogeneity of ventilation contributes on average to more than one-third of the variability in interlobar deposition. Actual contribution of ventilation in individual lungs was variable and dependent on the breathing rate used by the subject during aerosol inhalation; the highest contribution was in patients breathing slowly. In subjects breathing faster, contribution of ventilation was reduced, with more expanded lobes showing lower deposition per unit ventilation than less expanded ones in these subjects. The lobar change in expansion measured from two static CT scans, which is commonly used as a surrogate for ventilation, did not correlate with aerosol deposition or with PET-measured ventilation. This suggests that dynamic information is needed to provide proper estimates of ventilation for asthmatic subjects. We hope that the enhanced understanding of the causes of heterogeneity in airway and tissue dosing using the tools presented here will help to optimize therapeutic effectiveness of inhalation therapy while minimizing toxicity.

Abstract:ABSTRACT Deep lung delivery is crucial for respiratory disease treatment. Although nano and submicron particles exhibited a good deposition efficiency in deep regions of the lung, powder nonuniformity and particle agglomeration reduce their efficiency. Inhalation of porous particles (PPs) can overcome the mentioned challenges due to their larger size and low‐density. The present study numerically investigates the deposition and penetration efficiency of orally inhaled PPs. A revised drag coefficient was implemented for PP transport. A realistic mouth–throat to the fifth generation of the lung was reconstructed from CT‐scan images. A dilute suspension of uniformly distributed particles was considered at three inhalation flow rates (15, 30, and 45 L/min). Governing equations of the flow field and particle transport are solved using an Eulerian–Lagrangian approach. The results demonstrate that inhaling PPs significantly reduces the total and regional deposition of particles. There was also a critical porosity value under moderate and high inhalation flow rates for large PPs. Below this critical value, PP deposition efficiency substantially decreases. Additionally, it was also found that under low inhalation flow rates, the impact of porosity value is negligible. Almost 95% of the PPs penetrate the lower branches. These findings provide particle engineers and pharmaceutics with profound insight into developing novel inhalation techniques and drug delivery methods for deep lung delivery.

Flow and Particle Dispersion in Lung Acini: Effect of Geometric and Dynamic Parameters During Synchronous Ventilation

Abstract:The human lung comprises about 300 million alveoli which are located on bronchioles between the 17th to 24th generations of the acinar tree, with a progressively higher population density in the deeper branches (lower acini). The alveolar size and aspect ratio change with generation number. Due to successive bifurcation, the flow velocity magnitude also decreases as the bronchiole diameter decreases from the upper to lower acini. As a result, fluid dynamic parameters such as Reynolds (Re) and Womersley (α) numbers progressively decrease with increasing generation number. In order to characterize alveolar flow patterns and inhaled particle transport during synchronous ventilation, we have conducted measurements for a range of dimensionless parameters physiologically relevant to the upper acini. Acinar airflow patterns were measured using a simplified in vitro alveolar model consisting of a single transparent elastic truncated sphere (representing the alveolus) mounted over a circular hole on the side of a rigid circular tube (representing the bronchiole). The model alveolus was capable of expanding and contracting in-phase with the oscillatory flow through the bronchiole thereby simulating synchronous ventilation. Realistic breathing conditions were achieved by exercising the model over a range of progressively varying geometric and dynamic parameters to simulate the environment within several generations of the acinar tree. Particle image velocimetry was used to measure the resulting flow patterns. Next, we used the measured flow fields to calculate particle trajectories to obtain particle transport and deposition statistics for massless and finite-size particles under the influence of flow advection and gravity. Our study shows that the geometric parameters (β and ΔV/V) primarily affect the velocity magnitudes, whereas the dynamic parameters (Re and α) distort the flow symmetry while also altering the velocity magnitudes. Consequently, the dynamic parameters have a greater influence on the particle trajectories and deposition statistics compared to the geometric parameters. The results from this study can benefit pulmonary research into the risk assessment of toxicological inhaled aerosols, and the pharmaceutical industry by providing better insight into the flow patterns and particle transport of inhalable therapeutics in the acini.

Abstract:The testing of pharmaceutical aerosols includes measuring the aerodynamic particle size distribution, which is usually performed on cascade impactors. In the next generation impactor (NGI), the aerosol dose is introduced through an induction port (IP), being separated into different aerodynamic diameter ranges by seven stages and a micro‐orifice collector. The IP plays an important role in estimating the oropharyngeal deposition fraction. While the IP retains mainly large particles, it also tends to retain particles in the respirable range. In this work, the deposition of particles in the IP of an NGI is studied, both experimentally and through computational fluid dynamic (CFD) simulations. Experimental tests are conducted both with the pressurized metered‐dose inhaler (pMDI) alone and in conjunction with a cylindrical valved holding chamber (VHC). For each case, the total mass deposition that occurs in the VHC, IP, and NGI stages is measured, as well as the mass median aerodynamic diameter of the aerosol leaving the IP. The CFD simulations show minimal variations in results regardless of the injection method due to the relatively low velocity and spray angle of the pMDI‐generated aerosol. If the flap‐valve is removed from the VHC, significant particle deposition occurs upstream or within the valve region, with downstream air recirculation contributing to small particle deposition. Based on the CFD results, a correlation is proposed to accurately predict the particle escape fraction of the IP and VHC, which allows estimation of the tannin distribution of particles collected in the NGI stages, especially those corresponding to the range of respirable particles.

Abstract:Reproducible aerosol generation in combination with stable aerosol properties are essential prerequisites for compliant performance of acute or repeated inhalation toxicity tests of particulate materials according to OECD TG 403, 436, 412 or 413. A frequent problem of powder aerosol generation is the formation of coarse agglomerates with low shear resistance, which are beyond the tolerable size range but not detected by the prescribed aerodynamic measurement techniques by cascade impactor as the measurement conditions cause a disintegration into smaller fragments. But such agglomerates are observed during the transport to the inhalation chambers. These effects particularly apply to high mass concentrations and low-density powders, i.e., pyrogenic oxides. This study describes the transport influence in the airflow on the change of powder aerosols and on their respirability. A simplified short tube set-up was developed for the aerosol transport pre-tests, which allows the determination of the optimal aerosol formation conditions for the inhalation tests. The particles were measured with low shear using laser diffraction measurement or optical particle counters. The calculation of the aerodynamic particle sizes prescribed in the guidelines requires knowledge of the effective particle density of the porous aerosol particles. A practicable method for determining these is presented and described. In the outlook, first low concentration measurements show that clear agglomeration effects can also occur at particle concentrations around 20 mg/m3.

Influence of E-Liquid Humectants, Nicotine, and Flavorings on Aerosol Particle Size Distribution and Implications for Modeling Respiratory Deposition

Abstract:Electronic cigarette, or vaping, products are used to heat an e-liquid to form an aerosol (liquid droplets suspended in gas) that the user inhales; a portion of this aerosol deposits in their respiratory tract and the remainder is exhaled, thereby potentially creating opportunity for secondhand exposure to bystanders (e.g., in homes, automobiles, and workplaces). Particle size, a critical factor in respiratory deposition (and therefore potential for secondhand exposure), could be influenced by e-liquid composition. Hence, the purposes of this study were to (1) test the influence of laboratory-prepared e-liquid composition [ratio of propylene glycol (PG) to vegetable glycerin (VG) humectants, nicotine, and flavorings] on particle size distribution and (2) model respiratory dosimetry. All e-liquids were aerosolized using a second-generation reference e-cigarette. We measured particle size distribution based on mass using a low-flow cascade impactor (LFCI) and size distribution based on number using real-time mobility sizers. Mass median aerodynamic diameters (MMADs) of aerosol from e-liquids that contained only humectants were significantly larger compared with e-liquids that contained flavorings or nicotine ( p = 0.005). Humectant ratio significantly influenced MMADs; all aerosols from e-liquids prepared with 70:30 PG:VG were significantly larger compared with e-liquids prepared with 30:70 PG:VG ( p = 0.017). In contrast to the LFCI approach, the high dilution and sampling flow rate of a fast mobility particle sizer strongly influenced particle size measurements (i.e., all calculated MMAD values were < 75 nm). Dosimetry modeling using LFCI data indicated that a portion of inhaled particles will deposit throughout the respiratory tract, though statistical differences in aerosol MMADs among e-liquid formulations did not translate into large differences in deposition estimates. A portion of inhaled aerosol will be exhaled and could be a source for secondhand exposure. Use of laboratory-prepared e-liquids and a reference e-cigarette to standardize aerosol generation and a LFCI to measure particle size distribution without dilution represents an improved method to characterize physical properties of volatile aerosol particles and permitted determination of MMAD values more representative of e-cigarette aerosol in situ , which in turn, can help to improve dose modeling for users and bystanders.

Pediatric Faculty Members’ Attitudes about Part-Time Faculty Positions and Policies to Support Part-Time Faculty: A Study at One Medical Center

Abstract:Purpose To examine pediatric faculty members’ attitudes about part-time faculty positions and policies to support part-time faculty. Method In 2001, an anonymous 26-item questionnaire assessing attitudes about part-time faculty was mailed to all 441 faculty members of Cincinnati Children’s Hospital Medical Center. Multivariable analyses were used to determine faculty characteristics associated with specific attitudes, and qualitative methods were used to analyze responses to an open-ended item assessing beliefs about facilitating part-time careers. Results Three hundred (68%) faculty members completed questionnaires. Twenty-nine (10%) worked part-time and an additional 88 (33%) had considered part-time work, primarily because of dependent children. Although 177 (59%) believed that part-time faculty were perceived as being less committed to their careers and the institution, 207 (69%) believed part-time faculty should be eligible for all academic tracks and 219 (73%) that they should be allowed extension of time to obtain tenure. Most reported that policy changes to support part-time faculty would enhance diversity (N = 234, 78%) and improve recruitment, retention, and promotion of female faculty. Multivariable analysis demonstrated that women and respondents with dependent children were more likely to be concerned about perceived commitment and more likely to endorse policies to support part-time faculty. Participants suggested that part-time careers for faculty would be facilitated by clarifying productivity expectations, expanding resources, and modifying existing policies. Conclusions Although women and respondents with dependent children were concerned about perceived commitment of part-time faculty and were most supportive of policies that would support part-time faculty, pediatric faculty generally supported such policies.

Abstract:Inhaled drug delivery is widely used in the treatment of respiratory diseases. Understanding the deposition mechanisms of dry powder inhalers (DPIs) in different regions of the airway is crucial for inhaler development and prediction of the deposition distribution of drug particles. And the insertion of the inhaler will significantly alter the pattern of airflow in the airway. The main objective of this study is to systematically investigate the distribution and mechanism of aerosol particle deposition in various regions of the inhaler-airway model. Computational fluid dynamics (CFD) was used to simulate the effect of inhalation flow rate on the deposition of drug particles in various regions of the airway. The discrete phase model (DPM) was adopted to track the deposition trajectories of drug particles. Three different inhalation flow rates together with six particle sizes and five particle densities were analyzed. The results indicated that the overall deposition fraction of drug particles gradually increased with particle size and density. The pattern of depositional distribution in other local areas is quite different from the overall pattern except in the oral. The deposition fraction in the pharynx was much larger than in the other local regions, the deposition fractions in the larynx, trachea, carina and bronchi were less than 5 %. The increase in density increases the deposition fraction of particles in various regions throughout the respiratory tract when the inhalation flow rate is 30 L/min. Most of the particles in the oral are deposited on the tongue, and the particles in the bronchus are more distributed in the main trunk, while deeper in the bronchus, particles are also deposited in the bifurcation region. The subject's inhalation posture also affects the local distribution of the airflow. The findings of present study can help to guide the optimization and in vitro-in vivo correlation of DPIs.

Abstract:Inhalation therapy is widely used to treat various respiratory diseases. Nebulizers have been widely adopted due to their ease of operation, high drug delivery efficiency, and low residual volume. Traditional studies on drug delivery in the respiratory tract typically use a one-way coupled method to track drug transport and deposition for non-hygroscopic drug particles. However, neglecting the concentration of nebulized hygroscopic droplet aerosols can lead to significant discrepancies in the deposition efficiency (DE) of nebulized drugs in the airways. To investigate the effect of different nebulization rates on the evolution, transport, and deposition of droplets, a highly idealized mouth-throat (MT) model was selected as a testbed for the in silico trials of drug delivery to the respiratory tract. The interaction between continuous and discrete phases was modeled using a two-way coupled model of highly concentrated droplet aerosol-water vapor interaction, innovated by the particle parcel method. The effects of different coupling methods and boundary conditions on the average droplet diameter and droplet deposition distribution pattern at the outlet of the MT model were compared using a computational fluid dynamics (CFD) method. Simulation results indicate that neglecting the effect of water vapor evaporated from the droplets on the relative humidity (RH) in the oral cavity could lead to a 2.2-fold prediction error of the average droplet diameter at the outlet of the MT model for a conventional nebulization rate of 0.386 g/min. As the nebulization rate increases, the difference in the predicted average droplet diameter at the outlet of the MT model between the one-way coupled method and the two-way coupled method also increases. Increasing the nebulization rate during inhalation therapy could lead to incomplete evaporation of drug droplets, resulting in deposition of larger diameter drug droplets in the oral cavity. In contrast, a lower nebulization rate can reduce the droplet diameter at the outlet of the MT model, allowing it to enter deeper airways.

Abstract:We examined the effect of postexposure hyperpnea on clearance of particles deposited in the lungs of adult male beagle dogs. Sedated dogs inhaled an insoluble 67Gg2O3 (T 1/2 = 78 h) aerosol (0.12 micron AMD) for one half hour on three separate occasions. Following aerosol exposures 1 and 2, dogs were assigned to either an eupneic (EUP) or hyperpneic (HYP) group and clearance was followed noninvasively for 9 days by whole body counting and gamma camera analysis. After exposure 2, EUP and HYP assignments were interchanged so that each dog was studied under both conditions. Hyperpnea was induced by housing dogs in an atmosphere of 7% CO2 in air, 6 h per day. Carbon dioxide inhalation increased VE a factor of 3.7 +/- 0.9 (SD). We found that pulmonary clearance was retarded by CO2-stimulated hyperpnea in 7 of 8 dogs. Following aerosol exposure 3, dogs were divided between EUP and HYP and subsequently were killed in pairs at 3 h, 1 day, 3 days, and 7 days. Distribution of activity in body organs was examined, and translocation to the hilar lymph nodes was followed. Both retention distribution and removal of activity by saline lavage were measured in postmortem lungs. The percentage of lavaged activity associated with pulmonary macrophages increased from 44% at 3 h after exposure to 91% at 4 and 7 days after exposure. Examination of dried lung slices by autoradiography showed clearance of particles from airways and formation of a more punctate distribution in the retained activity at increasing times after exposure. Abstract:Numerous inhalable aerosols consist of multiple nano-to-microscale solid or liquid particles with dissolved or embedded compounds, as well as associated vapors. In general, of interest are the transport and conversion phenomena leading to local particle/droplet/vapor depositions. Selected examples include inhalation of aerosols from use of inhalers, cigarettes and electronic cigarettes. In this study the focus is on hygroscopic growth of nano-size multi-component droplets and droplet–vapor interactions during transport with subsequent deposition in a human upper lung-airway model. For that purpose a comprehensive and efficient computational fluid–particle dynamics model has been developed. It is capable of simultaneously analyzing multi-component droplet–vapor and airflow interactions with evaporation and condensation effects for different sets of inhalation conditions. Selecting inhaled electronic cigarette (EC) aerosols as an application, the simulation results include detailed transport, deposition and absorption data for different constituents (i.e., water, propylene glycol, glycerol and nicotine) in both vapor and liquid forms for an idealized human upper lung airway geometry, i.e., from mouth to generation 3. Results indicate that liquid–vapor phase change induces hygroscopic growth of droplets, which in turn impacts significantly the deposition concentrations of aerosols via inertial impaction, secondary flows, Brownian motion, and the vapor-specific absorption rates. Parametric sensitivity analyses were performed to evaluate the influence of different inhalation flow waveforms on EC-aerosol transport, interaction, and deposition.

Abstract:Inhalers spray over 100 million drug particles into the mouth, where a significant portion of the drug may deposit. Understanding how the complex interplay between particle and solid phases influence deposition is crucial for optimising treatments. Existing modelling studies neglect any effect of particle momentum on the fluid (one-way coupling), which may cause poor prediction of forces acting on particles. In this study, we simulate a realistic number of particles (up to 160 million) in a patient-specific geometry. We study the effect of momentum transfer from particles to the fluid (two-way coupling) and particle-particle interactions (four-way coupling) on deposition. We also explore the effect of tracking groups of particles (`parcels') to lower computational cost. Upper airway deposition fraction increased from 0.33 (one-way coupled) to 0.87 with two-way coupling and $10 \mu m$ particle diameter. Four-way coupling lowers upper airway deposition by approximately 10% at $100 \mu g$ dosages. We use parcel modelling to study deposition of $4 - 20 \mu m$ particles, observing significant influence of two-way coupling in each simulation. These results show that future studies should model realistic dosages for accurate prediction of deposition which may inform clinical decision-making.
