Abstract:Objective To investigate the modification method of Cervical Spine Injury Severity Score System and discuss diagnosis and treatment strategy of lower cervical spine injuries. Methods Treatments of lower cervical spine injuries were selected according to the injury severity graded by the modified Moore' s classification system.Conservative therapy could be adopted for the patients with stability quantification rating ＜ 3 points or for the those with stability quantification rating =3 points but without spinal cord or nerve root compression.Surgical treatment was recommendable for the patients with stability quantification rating =3 points and with spinal cord or nerve root compression.Surgical therapy could be required for the patients with stability quantification rating ≥4 points and with risk of lower cervical instability.The higher the stability quantification score implied the stronger the surgical indications.Lower cervical spine injury combined with spine cord or nerve root compression had absolute surgical indications.At the same time,therapies were selected based on patients' other factors. Results Based on basic principles of the modified Moore' s classification system together with opinions of the patients and their relatives,14 patients were managed with surgical treatment and 16 with conservative treatment.Among the patients with complete spinal cord injury (Grade A),two patients treated surgically showed no obvious signs of spinal function recovery,but their nerve root irritation symptoms disappeared; the other one patient who needed surgery but received conservative treatment had no change of the spinal cord function and nerve root irritation.The patients with incomplete spinal cord injury (Grades B,C and D) treated surgically obtained certain degree of spine cord function recovery,with their American Spinal Injury Association (ASIA) score raised by 0.5 grade on average.However,the patients who needed surgery but received conservative treatment gained average increase of ASIA score for 0.5 grade.Imaging examination showed that patients without combined spinal injuries obtained interbody fusion after surgery,with normal alignment and height of the cervical vertebra but without presence of vertebral shift or instability. Conclusions The modified Moore' s classification system takes patients' spiaal injury condition and other factors into consideration in selection of conservative or surgical treatment,which improves the Cervical Spine Injury Severity Score System to some extent and has prospect of clinical application.

Abstract:Lower lumbosacral vertebrae are the important part of spinal column,and are essential for spinal supporting,moving,protecting and weighting.Recently,lower lumbosacral vertebral fractures are becoming more and more common with the development of the industry and transportation.Lower lumbosacral vertebrae are surrounded by important ligaments,blood vessels and nervous tissue.The position and function of lower lumbosacral vertebrae are especially significant,therefore,the fractures in this area are extremely complicated,which can affect the normal physical function of spine.The optimal classification for lower lumbosacral vertebral fractures plays an important role in guiding clinical diagnosis and treatment.With the improvement of examination methods and the development of technology,further understanding concerning the diagnosis and treatment of lower lumbosacral vertebral fractures was achieved.The optimal classification for lower lumbosacral fractures remains controversial,which results in enormous trouble for clinical decision making.Based on understanding of thoracolumbar fractures,the thoracolumbar injury classification system and severity score (TLICS),widely accepted in clinic,owns good validation and reliability.However,because of the specificality of lower lumbosacral vertebral location and function,it is revealed that there is certain limitation in the process of clinical application of TLICS.The lumbosacral injury classification system and severity score (LSICS) based on TLICS is presently a novel and comprehensive classification system for evaluating lower lumbosacral injury severity and guiding clinical decision making,which is certainly unknown to us.Problems are still existed when LSICS newly proposed is applied to guiding and treating low lumbosacral vertebral fractures.Related problems and the progress of applying thoracolumbar vertebral fractures severity score to lower lumbosacral vertebral fractures are reviewed.The current problems of low lumbosacral vertebral fractures classification are summarized.Also,TLICS and LSICS are briefly introduced.The injury morphology of thoracolumbar vertebral fractures and lower lumbosacral vertebral fractures is described,and the grading rules are compared.The calculation methods of composite injury severity score are illustrated.The limitation of TLICS and the advantages of LSICS are analysed.The clinical application of LSICS in guiding clinical decision making of low lumbosacral vertebral fractures is discussed.
Assessment of Load-Sharing Thoracolumbar Injury: A Modified Scoring System

Abstract:BACKGROUND Many classification systems of thoracolumbar spinal fractures have been proposed to enhance treatment protocols, but none have achieved universal adoption. AIM To develop a new patient scoring system for cases with thoracolumbar injury classification and severity score (TLICS) = 4, namely the load-sharing thoracolumbar injury score (LSTLIS). METHODS Based on thoracolumbar injury classification and severity score, this study proposes the use of the established load-sharing classification (LSC) to develop an improved classification system (LSTLIS). To prove the reliability and reproducibility of LSTLIS, a retrospective analysis for patients with thoracolumbar vertebral fractures has been conducted. RESULTS A total of 102 cases were enrolled in the study. The scoring trend of LSTLIS is roughly similar as the LSC scoring, however, the average deviation based on the former method is relatively smaller than that of the latter. Thus, the robustness of the LSTLIS scoring method is better than that of LSC. LSTLIS can further classify patients with TLICS = 4, so as to assess more accurately this particular circumstance, and the majority of LSTLIS recommendations are consistent with actual clinical decisions. CONCLUSION LSTLIS is a scoring system that combines LSC and TLICS to compensate for the lack of appropriate inclusion of anterior and middle column compression fractures with TLICS. Following preliminary clinical verification, LSTLIS has greater feasibility and reliability value, is more practical in comprehensively assessing certain clinical circumstances, and has better accuracy with clinically significant guidelines.
[Clinical Research of Percutaneous Monoplanar Screw Internal Fixation Via Injured Vertebrae for Thoracolumbar Fracture].

Abstract:OBJECTIVE:To evaluate the effectiveness of percutaneous monoplanar screw internal fixation via injured vertebrae for treatment of thoracolumbar fracture. METHODS:Between May 2015 and August 2017, 38 cases of thoracolumbar fractures without neurological symptom were treated with percutaneous monoplanar screw internal fixation via injured vertebrae. There were 22 males and 16 females, aged 25-52 years (mean, 32.5 years). There were 23 cases of AO type A3 and 15 cases of AO type A4. The injured vertebrae located at T 11 in 4 cases, T 12 in 9 cases, L 1 in 11 cases, L 2 in 10 cases, L 3 in 3 cases, and L 4 in 1 case. The mean interval between injury and operation was 4.5 days (range, 3-7 days). The pre- and post-operative degrees of lumbodorsal pain were estimated by the visual analogue scale (VAS) score. The X-ray film, CT three-dimensional reconstruction, and MRI were performed, and the ratio of anterior vertebral body height and sagittal Cobb angle were measured to assess the kyphosis of the fractured area. RESULTS:All operations in 38 patients successfully completed without complications such as dural sac, nerve root, or vascular injury. The operation time was (56.2±3.7) minutes and the intraoperative blood loss was (42.3±3.5) mL. All incisions healed by first intention without redness, swelling, or exudation. All patients were followed up 17-33 months, with an average of 21.5 months. The VAS score at each time point after operation significantly improved when compared with that before operation ( P<0.05), and significantly improved at 3 months and last follow-up when compared with that at 1 week ( P<0.05); there was no significant difference between 3 months and last follow-up ( P>0.05). There was no internal fixator loosening, breakage, or delayed kyphosis in all patients. The ratio of anterior vertebral body height and sagittal Cobb angle significantly improved postoperatively ( P<0.05), and no significant difference was found between the different time points after operation ( P>0.05). CONCLUSION:Percutaneous monoplanar screw internal fixation via injured vertebrae is an easy approach to treat thoracolumbar fracture without neurological symptom, which can effectively restore vertebral body height and correct kyphosis, and avoid long-term segmental kyphosis.

Abstract:Objective To evaluate the reliability of the TLICS and AO-TLICS scoring system for the thoracolumbar fracture and the effectiveness of the final treatment scheme.Methods The clinical data of 56 adult patients with acute traumatic thoracolumbar fracture diagnosed in Department of Orthopedics,Chui Yang Liu Hospital Affiliated to Tsinghua University from June 2015 to June 2017 were analyzed retrospectively.There were 35 males and 21 females,aged (32.3 ± 9.8) years,with an age range of 21-53 years.Two senior deputy chief orthopaedic physicians retrospective analyzed DR plain films,CT,MRI images and clinical records of thoracolumbar spine by independent blind method.According to TLICS and AO-TLICS scoring system (include injury morphology,neurologic status,posterior ligament complex (PLC) damage or M1 modifier,the severity scores of thoracolumbar fracture were classified and calculated.The score was divided into TLICS score group and AO-TLICS score group,each group of scores was the above 56 patients.Three months later,the two physicians repeated the above analysis process and compared the consistency of the scores between the observer and the observer's own control.The consistency of the two groups was compared,and the accuracy,sensitivity and specificity of the final treatment were compared.Cohen kappa test was used for consistency comparison according to the score and the mean value was taken.Measurement data were expressed as mean ± standard deviation (Mean ± SD),and t test was used for inter-group comparison.Results In the TLICS score group,the Cohen kappa coefficients of the observer's own control in fracture type,neurological status assessment,and PLC injury were 0.810,0.966,and 0.698,respectively,and the total scores were consistent (k=0.727).Cohen kappa coefficients between the observers in fracture morphology,neurological status assessment,and PLC injury were 0.725,0.931,and 0.594,respectively,and the total scores were consistent (k =0.615).In the AO-TLICS score group,the Cohen kappa coefficients of the observer's own control in terms of fracture type,neurological status assessment,and M1 correction factor were 0.760,0.892,and 0.711,respectively,and the total scores were consistent (k =0.666).Cohen kappa coefficients of the observers in terms of fracture type,neurological status assessment and M1 correction factor were 0.707,0.836 and 0.604,respectively,and the total score was consistent (k =0.592).According to the TLICS score,the correct rate of treatment was (84.2 ± 2.8) ％,the sensitivity was (84.1 ±2.1)％,and the specificity was (85.9 ± 6.5)％.According to the AO-TLICS score,the correct rate of treatment was (89.0 ± 2.6) ％,the sensitivity was (88.3 ± 3.5) ％,and the specificity was (89.6 ± 2.4) ％.There was a statistically significant difference in the rate of correct rate (t =2.485,P =0.047).Conclusion The advantage of TLICS score is better consistency in interobservers or intraobservers,but the AO-TLICS score system is more comprehensive,and the choice of treatment plan is more instructive.
Clinical Comprehension of Thoracolumbar Spine Fracture Treatmented with Posterior Monosegmental Vertebral Pedicle Screw Fixation at the Injured Vetebrae

Abstract:[Objective] To evaluate the clinical outcome of thoracolumbar spine fracture treatmented with posterior monosegmental transpedicular fixation at the injured vertevrae.[Method] From January 2009 to December 2010,14 cases of thoracolumbar spine fracture were treated by the surgical procedure of posterior monosegmental injured vertebral pedicle internal fixation and injured vertebra bone grafting fusion.The status of internal fixation material was followed up and the vertebral height and kyphosis angle were compared before and after the surgery.[Result] The mean operation time,average bleeding volume during operation,was 83 min(52~125min),120ml(50~350m1),respectively.There was no iatrogenic nerve injure in all cases.All patients obtained the follow up and the average time was 16.8 months(12~24 moths).No internal fixation material breaking,loose and dislocation were found after operation.Bony union was observed in all cases.Postoperation vertebral height and segmental kyphosis angle different chips were statistically significant.[Conclusion] The posterior monosegmental injured vertebral internal fixation is one of effective method for treating thoracolumbar spine fracture.Strict indication and surgical skill is the key to this procedure.
Application of Transforaminal Interbody Fusion in Thoracolumbar Fracture

Abstract:Objective To explore effect of transforaminal interbody fusion (TIF) in the treatment of thoracolumbar fracture.Methods From January 2009 to June 2010,11 patients with thoracolumbar fracture underwent TIF in our hospital.There were 7 males and 4 females,aged from 22 to 54 years (average,37.8±9.5 years).Fracture occurred at T11,12 in 2 cases,T12L1 in 5 cases,L1,2 in 2 cases,L2,3 in 1 case,and L3,4 in 1 case.According to AO classification,all 11 cases were rated as type B.The thoracolumbar injury classification and severity score (TLICS) ranged from 7 to 10 (average,9.2±0.87).According to Frankel grading system,there were 3 cases of grade B,4 cases of grade C,3 cases of grade D,and 1 case of grade E.The operative time,blood loss,and complications were evaluated.Functional recovery was evaluated by Frankel grading system.Radiographic results were obtained before surgery and during follow-up,and Cobb angle was measured.Results The operative time ranged from 130 to 170 minutes (average,147.3±11.9 minutes).The blood loss ranged from 180 to 650 ml (average,369.1±110.2 ml).All patients were followed up for 13 to 26 months (average,20.2±4.3 months).There was no complication during operation and follow-up.X-rays at final follow-up showed that there was no internal fixation failure such as screws break and cages shift.The improvement in Frankel grade was observed in 9 cases.The average Cobb angle improved from preoperative -10.3°±7.8° (range,-21.2° to 3.2°) to 1.2°±7.4° (range,-7.9° to 17.2°) at final follow-up.Conclusion The TIF is an effective method for treating thoracolumbar fracture,which shows good results in deformity correction,improvement of neurological function,bone fusion and stability of internal fixation.
Validation of thoracolumbar injury classification and Severity Score in the management of acute and subacute Osteoporotic vertebral compression fractures - A pilot study and a suggested modification

Abstract:Objective: To retrospectively assess the Thoracolumbar Injury Classification and Severity Score (TLICS) in patients with osteoporotic vertebral compression fractures (OVCF) and compare the treatment given with that predicted by the TLICS score. Methods: All medical records of patients presenting from January 2014 to November 2017 for acute atraumatic or low impact OVCF were screened, and eligible patients were retrospectively reviewed. The TLICS score was determined based upon magnetic resonance imaging (MRI) findings and clinical records. Clinical records (including pain score data), imaging data, operative procedures, and stability of neurological examination were tracked over three months for each patient. Results: Of the 56 patients included, 36 patients had a TLICS score of 1, 18 had a TLICS score of 2, and two had a TLICS score of 4. Only one patient with a TLICS score of 4 underwent surgical stabilization, while the rest of the cohort was managed non-operatively, with or without kyphoplasty. TLICS score 1 corresponded to simple compression and TLICS score 2 corresponded to burst morphology with retropulsion and without neurological deficits. Of the patients with a TLICS score of 1 and 2 who underwent kyphoplasty, there was a statistically significant improvement in pain scores in both groups; however no significant difference was observed, between each TLICS score (i.e., 1 or 2). None of the patients developed instability or neurological decline. Conclusion: TLICS score correctly predicted operative versus non-operative management in all patients with OVCF. TLICS may be used in making management decisions, and in the triage of these patients for operative versus non-operative evaluations. Our study suggests that patients with TLICS score of 4 or higher require surgical evaluation, while those with TLICS of 1 or 2 are likely to have satisfactory non-surgical management with augmentation or conservative care. In general, patients with OVCF typically present with low TLICS score. Kyphoplasty appears to be similarly beneficial in patients with a TLICS score of 1 or a TLICS score of 2. A modification of the TLICS score by adding TLICS Zero to include uncompressed OVCF with edema is suggested. The limitations of this study include a small size; a larger study is needed to confirm these findings.
Clinical application of a modified wiltse approach in middle and lower thoracic vertebrae: a case-control study of thoracic fracture patients

Abstract:Background: The Wiltse approach has been extensively employed in thoracolumbar surgeries due to its minimal muscle damage. However, in the middle and lower thoracic spine, the conventional Wiltse approach necessitates the severance of the latissimus dorsi and trapezius muscles, potentially leading to muscular injury. Consequently, we propose a modified Wiltse approach for the middle and lower thoracic vertebrae, which may further mitigate muscular damage. Methods: From May 2018 to April 2022, 60 patients with spinal fractures in the middle and lower thoracic vertebrae (T5-12) were enrolled in this study. Thirty patients underwent surgery using the modified Wiltse approach (Group A), while the remaining 30 patients received traditional posterior surgery (Group B). The observation indices included operation time, intraoperative blood loss, incision length, number of C-arm exposures, postoperative drainage, postoperative ambulation time, discharge time, as well as preoperative and postoperative Cobb's angle, percentage of anterior vertebral body height (PAVBH), visual analog scale (VAS) Score, and Oswestry disability index (ODI). Results: Compared to the traditional posterior approach, the modified Wiltse approach demonstrated significant advantages in operation time, intraoperative blood loss, length of incision, postoperative ambulation time, postoperative drainage, and discharge time, as well as postoperative VAS and ODI scores. No significant differences were observed between the two groups in terms of number of C-arm exposures, postoperative Cobb's angle, or postoperative PAVBH. Conclusion: We propose a modification of the Wiltse approach for the middle and lower thoracic vertebral regions, which may further minimize muscular damage and facilitate the recovery of patients who have undergone surgery in the middle and lower thoracic vertebrae.
Cervical spine trauma: impact of different imaging classification systems in the clinical decision-making

Abstract:Background and aim: Considering the high rate of mortality and permanent disability related to vertebral traumas, an early and detailed diagnosis of the trauma and subsequently an immediate and effective intervention are crucial. Cervical vertebral injury classifications guide treatment choice through a severity grade based on radiological information. The purpose of the present study was to define which imaging classification system could provide the best morphological and clinical-surgical correlations for cervical spine traumas. Methods: We retrospectively analyzed patients evaluated for cervical spine trauma at our Institution in the period 2015-2020. Information regarding the morphological examination (using CT and MRI), the neurological evaluation, and the therapeutic management were collected. C3-C7 fractures were classified according to the SLIC and AOSpine criteria; axial lesions were classified according to the modified AOSpine for the C1-C2 compartment and through the Roy-Camille and the Anderson D'Alonzo system for the odontoid process of the axis. Results: 29 patients were included in the final study population. Nine patients with axial spine trauma and 21 with subaxial cervical spine trauma. A conservative approach was applied in 16 patients while nine patients underwent neurosurgery. Considering the therapeutical indications provided by the SLIC system, a 76.9% accordance was found for patients with a <4 score, while a 100% concordance was calculated for patients with a >4 score undergoing neurosurgery. Regarding the AOSspine classification, a 28.6% concordance was observed for patients classified group B being treated with a posterior neurosurgical approach, while for patients belonging to subgroup C, considered for anterior neurosurgical approach, a 66.7% accordance was calculated. Conclusions: The study demonstrated a better morphological correlation for the AOSpine classification in subaxial trauma and the AOSpine and Anderson D'Alonzo in axial trauma. The therapeutic indication found a better correlation in the SLIC classification for subaxial trauma and the Anderson D'Alonzo for axial ones.
Clinical significance of magnetic resonance imaging in diagnosis and treatment of thoracolumbar traumatic fracture

Abstract:Objective To investigate the significance of magnetic resonance imaging (MRI) in diagnosis and treatment of thoracolumbar fractures. Methods We conducted a retrospective study to analyze clinical data in thoracolumbar traumatic fracture patients, who were enrolled Beijing Chuiyangliu hosptial after suffering an acute traumatic thoracolumbar fracture. Osteoporotic or pathologic fractures were excluded. Fractures were initially classified using X-ray and CT scan following the AO classification, and were graded by thoracolumbar injury classification and severity score(TLICS).The teatment principles were made. Afterward, MRI was performed with T1,T2-weighted and FS sequences. Subsequently, fractures were classified according to the TLICS system and reclassified following the AO system. Analysis was performed before and after MRI on diagnostic changes, occult fractures and differences in treatment decision making. Results Thirty nine patients with an average age of 40.1±17 years were studied. Forty-eight fractures were initially diagnosed using plain X-rays and CT scans, while MRI diagnosed 13 fractures and 11 vertebral contusions. MRI modified our diagnosis in 42.6%of our patients (discovering 17 patients with 24 occult injuries), the classification of fracture pattern in 30.8% of the fractures(mostly upgrading type A to type B patterns)and the therapeutic management in 17.9%of our patients. Conclusion MRI is a useful tool in the evaluation of thoracolumbar acute fractures, because it allows a better visualization of the posterior complex integrity and of the levels involved, offering additional information compared to traditional diagnostic tools.

Abstract:Study Design Prospective Observational Study. Objective To determine the alignment of the AO Spine Thoracolumbar Injury Classification system and treatment algorithm with contemporary surgical decision making. Methods 183 cases of thoracolumbar burst fractures were reviewed by 22 AO Spine Knowledge Forum Trauma experts. These experienced clinicians classified the fracture morphology, integrity of the posterior ligamentous complex and degree of comminution. Management recommendations were collected. Results There was a statistically significant stepwise increase in rates of operative management with escalating category of injury ( P < .001). An excellent correlation existed between recommended expert management and the actual treatment of each injury category: A0/A1/A2 (OR 1.09, 95% CI 0.70-1.69, P = .71), A3/4 (OR 1.62, 95% CI 0.98-2.66, P = .58) and B1/B2/C (1.00, 95% CI 0.87-1.14, P = .99). Thoracolumbar A4 fractures were more likely to be surgically stabilized than A3 fractures (68.2% vs 30.9%, P < .001). A modifier indicating indeterminate ligamentous injury increased the rate of operative management when comparing type B and C injuries to type A3/A4 injuries (OR 39.19, 95% CI 20.84-73.69, P < .01 vs OR 27.72, 95% CI 14.68-52.33, P < .01). Conclusions The AO Spine Thoracolumbar Injury Classification system introduces fracture morphology in a rational and hierarchical manner of escalating severity. Thoracolumbar A4 complete burst fractures were more likely to be operatively managed than A3 fractures. Flexion-distraction type B injuries and translational type C injuries were much more likely to have surgery recommended than type A fractures regardless of the M1 modifier. A suspected posterior ligamentous injury increased the likelihood of surgeons favoring surgical stabilization.

Abstract:[Objective]To investigate the application of thoracolumbar injury classification and severity thoracolumbar injuty chassification and severityscdre(TLICS)score in spinal fracture caused by earthquake.[Method]Night-five patients with spinal fractures were treated in Chendu Army General Hospital and Chendu Air Force Hosptial by Wenchuan earthquake medical team from Chinese PLA General Hospital.There were 49 males and 46 females with mean age of 48.6 years.The general patient data,injury mechanism,and injury time were recorded.The different treatments were decided by the TLICS score.[Result]There were 107 vertebral fractures in the 95 patients,out of whom 76 patients were injured by building collapsed,and the other 19 by falling from buildings.TLICS score was more than 4 in 36 patients,4 points in 15 patients,less than 4 points in 44 patients.40 patients were treated with open operation,15 patients with vertebraplasty,and the other patients with conservative treatment.[Conclusion]The earthquake spinal fractures can be treated with different treatments based on the different TLICS scores.

Abstract:Objective:To compare 3 maxillofacial trauma scoring systems in mandible fracture grading.Methods:Maxillofacial Injury Severity Score(MISS),Maxillofacial Injury Severity Score(MFISS),Mandible Injury Severity Score(S5)were used in the analysis of mandible fracture severity in 313 cases with mandible fractures.The results were statistically analyzed.Results:S5 score showed higher correlation with operation time,operation charge and hospitalization expenses than MFISS and MISS,and it could distinguish single and multiple mandible fractures effectively.It was also a significant factor affecting the cost of hospitalization.Conclusion:The Mandible Injury Severity Score was more suitable for the scoring of mandible fracture.
Efficacy of One-Stage Posterior-Anterior Approach for Treatment of Severe Thoracolumbar Fractures

Abstract:Objective To observe the outcomes of one-stage posterior short-level pedicle screw fixation combined with anterior fixation of severe thoracolumbar fractures.Methods A retrospective case series study was performed on 21 patients with severe thoracolumbar fractures stabilized by posterior short-level pedicle fixation combined with anterior internal fixation at one stage from January 2012 to December 2014.There were 16 males and 5 females,at age of 17 and 64 years [(38.7 ± 11.4) years].The involved segments included T11 in 2 patients,T12 in 5,Lt in 6 and L2 in 8.For AO fracture classification,type A fractures were seen in 4 patients,type B in 7 and type C in 10.Thoracolumbar injury classification and severity score (TLICS) was (8.12 ± 0.87) points (range,7-10 points).Frankel neurological performance scale was Grade B in 8 patients,Grade C in 11 and Grade D in 2.Operation time,blood loss,nerve function,kyphosis correction and complications were reported.Results Operation time was (234.5 ±57.3)min (range,180-360 min),and blood loss was (387.4 ± 124.4) ml (range,260-950 ml).Time of follow-up was (19.8 ± 3.5)months (range,14-25 months).Nerve function of 18 patients was improved by at least one Frankel scale.Cobb angle was (4.1 ±5.3)° at postoperative 3 days and (4.0 ± 4.9)°at the final follow-up,showing significant differences from that before operation [(-9.3 ± 4.2) °] (P ＜ 0.05).While the difference of Cobb angle did not differ significantly at postoperative 3 days and at final follow-up.No cerebrospinal fluid leakage,vascular injury,incision infection or nerve function deterioration occurred.Conclusion One-stage posterior short-level pediele screw fixation combined with anterior decompression and bone graft fixation is characterized by short operation time,few blood loss,good correction of traumatic kyphosis and good neurological recovery,indicating a good surgical choice for severe thoracolumbar fractures.

Abstract:BACKGROUND:Currently, there is no standard context that conforms to the Chinese national framework for evaluating medical decisions regarding the treatment of lung cancer.METHODS:This draft was formulated after a systematic review and a focus group discussion among 20 experts, who were senior physicians with extensive clinical experience from the Chinese Thoracic Oncology Group (CTONG) task force. Subsequently, a draft and a five-point Likert scale were sent to 300 CTONG working group members. These were modified according to feedback from a four-round modified Delphi approach. Hence, the first version of the 'Therapeutic option of lung cancer: CTONG scoring system' was formulated. Afterward, a corresponding questionnaire was designed to collect opinions on the weight allocation of various indicators. This was issued through the WeChat platform, "Oncology News" application and e-mails from October 23, 2020, to November 25, 2020. Participants from numerous occupations in cancer-related fields from various regions of China were included in the study. Overall and subgroup analyses regarding weight allocations were performed. The differences between participant-allocated and reference weights were considered to adjust the framework.RESULTS:The framework contained four aspects and six indicators, including efficacy [progression-free survival (PFS)/overall survival (OS) and subsequent treatment], safety [treatment-related severe adverse event (SAE), dose adjustment], quality of life (Qol), and compensation. The reference weights were 50%, 5%, 10%, 5%, 10%, and 20% for each indicator. By November 25, 2020, 1,043 valid questionnaires had been obtained. The majority of the questionnaires were completed by physicians (86.5%). Subgroup analysis among the various groups showed an overall consistent trend. Besides, significant differences between the participant-allocated and reference weights were found among PFS/OS (difference: -11.5%), compensation (difference: -10.1%), and subsequent treatment (difference: 9.7%) indicators. After discussion, the final weight allocations were set at 45%, 10%, 15%, 5%, 10%, and 15% for PFS/OS, subsequent treatment, treatment-related SAE, dose adjustment, Qol, and compensation, respectively.CONCLUSIONS:The CTONG scoring system, as an objective evaluation model that involves multiple parameters, is a breakthrough method for evaluating the therapeutic value of lung cancer treatment options in China, which is worthy of further verification in future clinical practice.
The Application Study of 64-slice Spiral CT in TLICS Classification System for Thoracolumbar Injury

Abstract:Objective: To investigate application values of the Thoraco-Lumbar Injury Classification and Severity (TLICS) Classification System in thoracolumbar injury by using 64-slice spiral CT scan, multi-planar reconstruction (MPR) and three-dimensional volume reconstruction (VR). Materials and Methods : The retrospective study included 50 patients (27 males and average ages were 44±9.8) with thoracolumbar injury from October 2014 to October 2016. All patients were scanned by 64-slice spiral CT and performed the sagittal MPR and three-dimensional VR for imaging diagnosis. Then we evaluated the severity of posterior ligamentous complex (PLC) injury according to the typical CT features of thoracolumbar injury. When the TLICS score was higher than 4, we compared the CT features with surgical findings. Results:Seventy-six vertebral fractures were checked out in all 50 patients, included 18 compressed fractures, 28 burst fractures, 20 translation/rotation fractures, and 10 stretch fractures. In addition, 14 patients were found the PLC injuries with different CT imaging features, i.e. 5 avulsion fractures of spinous process, 3 translational and rotational malformation of centrum, 2 transverse fractures of articular process, 2 cases with facet dislocation, 2 broadened interspinous space complicated with facet dislocation and fracture of articular process respectively. Conclusions: Combined MPR and three-dimensional VR could effectively detect fractures and dislocations features indicating the injury of PLC. MPR and VR can be applied as the significant diagnostic examination method, especially to the emergency patients, which could be used for early clinical application of TLICS classification system.

Abstract:Objective To determine the difference of paraspinal muscle injury between Wiltse paraspinal approach and conventional approach for the thoracolumbar vertebral fracture with open reduction and internal fixation. Methods A total of 47 patients with thoracolumbar vertebral fracture(ASIA E) under-went posterior open reduction and internal fixation from June 2004 to December 2006. Randomly, 22 pa-tients adopted Wiltse paraspinal approach, including 16 males and 6 females with the mean age of 37.6 years; 25 patients adopted traditional approach, 18 males and 7 females with the mean age of 40.1 years. The operative duration, blood loss and changes of postoperative serum level of Creatinine Kinase were com-pared. Total 47 patients were followed up with VAS scores 1 day preoperatively, 1 and 6 months postopera-tively. And EMG were adopted for long-term evaluation. Results There was no obvious difference in surgi-cal time, but less blood loss, lower index of serum level of Creatinine Kinase in Wihse paraspinal approach group. The VAS score in Wihse paraspinal approach was significantlt less than that in conventional approach group 1 and 6 months postoperatively. And in conventional approach group EMGs demonstrated fibrillation potentials, but they were normal in Wihse approach group. Conclusion The Wihse paraspinal approach is an easy way for treating of thoracolumbar vertebral fracture, minimize the soft tissue injury in operation and creates less complications after operation.
Management of thoracolumbar injury classification and severity score of 4 (TLICS=4) thoracolumbar vertebra fractures: Surgery versus conservative treatment

Abstract:Background: This study aims to compare clinical and radiographic outcomes of surgical treatment and conservative treatment with bracing in neurologically intact patients with score 4 of TLICS thoracolumbar vertebra fractures. Methods: Patients with traumatic thoracolumbar junction fractures (T11-L2), the score of TLICS 4, and minimum 24-month follow-up were included in this study. Patients were divided into surgery and bracing groups. The groups were compared concerning clinical and demographical features, local kyphotic angles (LKA), vertebra height loss percentage (VHL), Oswestry Disability Index (ODI), Visual Analog Scale (VAS) and time to return to work. Results: There were 74 patients (71 males, 3 females) in the surgery group and 76 patients (58 males, 18 females) in the bracing group. Although the surgery group showed better improvement in VAS scores within six months postoperatively, no significant difference was observed at the 24th-month evaluation (p<0.001 and p=0.270, respectively). ODI, LKA and VHL were significantly lower in the surgery group (p<0.001, p<0.001 and p<0.001, respectively). In addition, return to work was significantly earlier in the surgery group (p<0.001). Conclusion: The findings obtained in this study suggest that the surgical treatment for TLICS 4 patients with thoracolumbar fractures has better clinical and radiographic outcomes than the bracing. Moreover, returning time to the work of patients is shortened with surgical treatment. The surgical treatment seems to be the first and the appropriate choice in the management of TLICS 4 thoracolumbar vertebral fractures.
