Abstract:A recent meta-analysis revealed that mental health and baseline psychological impairment affect the quality of life and outcomes in different chronic conditions. Implementing mental health care in physical care services is still insufficient. Thus, interdisciplinary communication across treatment providers is essential. The standardized language provided by the diagnostic statistical manual favors a clear conceptualization. However, this approach might not focus on the individual, as thinking in categories might impede recognizing the continuum from healthy to diseased. Psychoanalytic theory is concerned with an individual's unconscious conflictual wishes and motivations, manifested through enactments like psychic symptoms or (maladaptive) behavior with long-term consequences if not considered. Such modifiable internal and external factors often are inadequately treated. However, together with the physical chronic condition constraints, these factors determine degrees of freedom for a self-determined existence. The effect of therapeutic interventions, and especially therapy adherence, relies on a solid therapeutic relationship. Outcome and process research still investigates the mechanism of change in psychotherapeutic treatments with psychanalysis's focus on attachment problems. This article examines existing knowledge about the mechanism of change in psychoanalysis under the consideration of current trends emerging from psychotherapy research. A clinical example is discussed. Additionally, further directions for research are given. The theoretical frame in psychoanalytic therapies is the affect-cognitive interface. Subliminal affect-perception is enabled via awareness of subjective meanings in oneself and the other; shaping this awareness is the main intervention point. The interactional ingredients, the patient's inherent bioenvironmental history meeting the clinician, are relevant variables. Several intrinsic, subliminal parameters relevant for changing behavior are observed. Therapeutic interventions aim at supporting the internalization of the superego's functions and at making this ability available in moments of self-reflection. By supporting mentalization abilities, a better understanding of oneself and higher self-regulation (including emotional regulation) can lead to better judgments (application of formal logic and abstract thinking). Thus, this facilitates enduring behavior change with presumably positive effects on mental and physical health.

Abstract:The concept of alliance reflects the collaborative relationship between a clinician and a patient, defined as consisting of three elements: a) the agreement on the goals of treatment; b) the agreement on a task or series of tasks; c) the development of a bond. Although much of the theory and research on the alliance comes from the domain of psychotherapy, the concept is applicable to any practice involving a person seeking help and a socially sanctioned healer. An extensive research evidence suggests that the alliance (typically measured at the third or fourth session) is a robust predictor of the outcomes of various forms of psychotherapy, even when prior symptom improvement and other factors are considered. Both the clinician and the patient bring to the therapy situation different capacities to form an alliance. Factors concerning the patient include, among others, the diagnosis, attachment history and style, motivation, and needs for affiliation. However, the benefits of the alliance have been found to be mostly due to the therapist's contribution, in particular his/her facilitative interpersonal skills, including verbal fluency, communication of hope and positive expectations, persuasiveness, emotional expression; warmth, acceptance and understanding; empathy, and alliance rupture‐repair responsiveness. Placebo studies have allowed to experimentally manipulate aspects of the relationship between a therapist and a patient in non‐psychotherapy contexts. In these settings, two components of the relationship have emerged: an emotional one (involving being cared for and understood by the clinician) and a cognitive one (including the belief in the competence of the therapist to select and administer an effective treatment). Here we propose a model that describes three pathways through which the alliance creates benefits, named CARE (caring, attentive, real and empathic), EXPECTANCY, and SPECIFIC. Although research and clinical attention have mostly focused on the alliance between a clinician and a patient in face‐to‐face interactions, there is preliminary evidence concerning the alliance between patients and other clinic staff, systems of care, or the program in Internet‐mediated services. These new research areas clearly require further development.

Abstract:Although psychological treatments are broadly recognized as evidence-based interventions for various mental disorders, challenges remain. For example, a substantial proportion of patients receiving such treatments do not fully recover, and many obstacles hinder the dissemination, implementation, and training of psychological treatments. These problems require those in our field to rethink some of our basic models of mental disorders and their treatments, and question how research and practice in clinical psychology should progress. To answer these questions, a group of experts of clinical psychology convened at a Think-Tank in Marburg, Germany, in August 2022 to review the evidence and analyze barriers for current and future developments. After this event, an overview of the current state-of-the-art was drafted and suggestions for improvements and specific recommendations for research and practice were integrated. Recommendations arising from our meeting cover further improving psychological interventions through translational approaches, improving clinical research methodology, bridging the gap between more nomothetic (group-oriented) studies and ideographic (person-centered) decisions, using network approaches in addition to selecting single mechanisms to embrace the complexity of clinical reality, making use of scalable digital options for assessments and interventions, improving the training and education of future psychotherapists, and accepting the societal responsibilities that clinical psychology has in improving national and global health care. The objective of the Marburg Declaration is to stimulate a significant change regarding our understanding of mental disorders and their treatments, with the aim to trigger a new era of evidence-based psychological interventions.

Abstract:Background: While most studies assessing psychotherapy efficacy are randomized-controlled trials conducted in research institutions or short clinical treatments, the understanding of psychotherapy effectiveness under regular, clinically representative conditions, particularly in outpatient practice, remains limited. Representative data examining the effectiveness of psychotherapy under real-world conditions in Austria is lacking. Aims and Methods: This paper introduces a naturalistic observational combined process- and outcome study, implementing a dual-perspective approach through standardised pre- and post-treatment questionnaires and evaluating changes in the therapeutic alliance after each session. Further, semi-structured qualitative interviews aim to illuminate the personal experiences of patients and therapists. The primary objective of the presented study is to discern whether symptoms markedly decrease following therapy. A significant secondary goal is to trace the therapeutic alliance's evolution from both patient and therapist viewpoints, emphasising the alliance-outcome association and gender dynamics within the pairs. This paper discusses the project's feasibility after three years and shares key insights. Discussion: Recruitment for this study has posed substantial challenges due to psychotherapists' concerns regarding data protection, extensive documentation, and philosophical reservations about the study design. Consequently, we recruited fewer participants than initially planned. Despite these hurdles, qualitative data collection has shown notable success. Given psychotherapists' busy schedules and reluctance to participate, more potent external incentives or a legal obligation may be necessary to encourage participation in future studies.

Abstract:Review of General Psychology, Ahead of Print. In 2020, a new record was set: For the first time in history, more than 100.000 students of psychology were officially enrolled in Germany. This remarkable number is the outcome of a long-term transformation of the whole academic landscape in Europe as well as dynamics that are specific to the German context. As these recent changes have not yet been properly discussed outside the German-speaking world, this paper provides a contextual analysis of the factors that have contributed to the current state of discipline, including legal regulations and economic conditions, academic curricula, and students' preferences. It is argued that the growth of the discipline was primarily an effect of psychology becoming the primary route to psychotherapeutic practice in Germany after World War II. Even though representatives of the discipline repeatedly voiced concern about the one-sided interest of their students, German academic psychology has benefited from the steady increase in demand for clinical training, especially among female students, on many levels. In late 2020, a new law came into force which redefined the requirements for the qualification of psychotherapists. Although this legal reform was introduced only very recently, it is already becoming apparent that this change will have a profound impact on the discipline. As the tensions between basic research and applied branches increase, cracks are appearing in the foundations of the whole discipline, forcing it to make tough choices about its future development.

[Self-selection processes in the choice of the therapeutic training approach: differences in therapeutic attitudes, personality traits and attributional complexity]

Abstract:Aim: Treatment approaches differ to a great extent in terms of basic psychological assumptions and practical procedures. This creates questions about the fitting of therapist and therapeutic approach. This paper examines the influence of therapeutic attitudes, mentalization interest and personality traits on the decision for an approach. Methods: 184 participants of training programs in one of the 3 licensed treatment approaches in Germany were examined with questionnaires at the beginning of their training. Results: Participants significantly differed in terms of therapeutic attitudes and the metallization interest but not in personality traits except openness. Satisfaction with training was not related to the individual fit of participants to the therapeutic attitudes typical for their approach. Conclusion: Therapeutic attitudes, the extent of mentalization interest, and openness may play a role in self-selection processes in the choice of the approach.

Abstract:Objective: In psychotherapy process research, there is a lack of experimental designs that specifically explore which therapeutic style is helpful for which patient. Thus, the aim of this study was to test the feasibility of experimentally varying the therapeutic style under realistic conditions and how this affects alliance. Method: We defined two therapeutic styles (relationship-focused vs. problem-focused) based on the interpersonal circumplex. In a randomized two-group design, 64 healthy university students (70% female, Mage=23.78 years old, SDage=2.81) were assigned to one of the styles and received a single psychological counseling session on interpersonal conflicts. We checked the manipulation success using an adherence rating and counselors’ interpersonal impressions via the Interpersonal Message Inventory (IMI-R). Primary outcome was alliance (Working Alliance Inventory, WAI). Symptom severity (Symptom Checklist, SCL-K-9) was also assessed. Results: Analyses of adherence (p< .001) and IMI-R ratings (p< .001 in friendly dimension, p=.003 in hostile dimension) were hypotheses-conform to the proposed therapeutic styles. Alliance was highly rated in both conditions and by all raters. While clients did not show any group differences, counselor and observer ratings were significantly higher in the relationship-focused than in the problem-focused condition (p=.040; p=.003). In both groups, symptom severity decreased significantly after the session (p=.020).Conclusions: The experimental variation of the therapeutic style is feasible under realistic conditions with high overall alliance ratings. The novel experimental design may provide a basis for further process research. Mediator and moderator analyses could yield more detailed information on differential relationship forming and thus an individualized therapeutic style.
[Significance of concepts about illness attribution and treatment expectations in the initial treatment phase]

Abstract:To evaluate the influence and the consequences of subjective concepts about attribution and treatment expectations of patients and therapists 56 patients of two psychiatric outpatient departments were examined with the inventory for concepts about illness and the Giessen-Test. In addition biographic data and information of the present problems as well the judgment of the therapists were registered. The results show that the subjective concepts of problem attribution and treatment expectations correlate with patient variables as personality, social economic states, social network, anamnestic data, present problems, previous therapeutic experience and vocational situation. Some concepts (e.g. psychologically oriented concepts) and the conformity of therapists and patients are important predictors of indication, prognosis and outcome of therapy. Therapists are especially motivated to work with patients who have similar concepts and a similar social background as themselves. The concepts of the patients assimilate of those of the therapists already after a short period of therapy.

Abstract:Increased emphasis has been placed on elucidating the contribution of client variables, such as treatment preference, to optimize evidence-based practice. This analog study sought to better understand variables associated with treatment preference using a convenience sample of college students ( n = 54) who read brief descriptions of three interventions for negative thoughts—defusion, noticing, and restructuring. They rated each on acceptability and practicality and completed measures of cognitive fusion, emotional distress, and experiential avoidance as possible moderating variables. Restructuring was overwhelmingly preferred and rated as more acceptable than the two alternatives by both the overall sample and a distressed subsample. Preference for defusion or noticing was not predicted by ratings of acceptability or practicality but by elevated levels of cognitive fusion and emotional distress consistent with a compensation model. Limitations of the study and its implications for further research on psychotherapy preference and its integration within evidence-based practice are discussed.

Abstract:Objective: This study aimed to elucidate the relationships among health locus of control, psychological reactance, attitudes toward psychiatric treatment, and patient decision-making preferences within a psychiatric outpatient population. Methods: A total of 200 consecutive psychiatric outpatients from a community mental health center in Tenerife, Spain, were approached for participation between September 2023 and March 2024. Of these, 151 patients consented to participate in this cross-sectional study. Participants were selected based on their willingness to participate and were provided with informed consent forms. Data were collected using the Patient's Health Belief Questionnaire on Psychiatric Treatment (PHBQPT) and the Control Preferences Scale (CPS). The PHBQPT evaluates health beliefs impacting adherence to psychiatric treatment, while the CPS assesses the preferred level of involvement in medical decision-making. Sociodemographic data were also collected to contextualize the findings. Results: Significant correlations were found between patients' control preferences and their attitudes towards medication, compliance with psychiatric advice, and perceptions of treatment control. A collaborative control preference was notably associated with positive attitudes toward medication and trust in the psychiatrist. These findings suggest that tailored treatment approaches prioritizing patient involvement could enhance adherence and outcomes. Conclusion: The study underscores the importance of considering psychological constructs in psychiatric care to foster a holistic, patient-centered approach. Recognizing and integrating patients' control preferences, attitudes towards medication, and psychological reactance can improve the therapeutic relationship and treatment adherence. Future research should explore longitudinal and interventional studies to further understand the impact of aligning treatment approaches with patient preferences and psychological profiles.

Abstract:The therapeutic alliance, or client-provider relationship, has been associated with better treatment engagement and outcomes for persons with schizophrenia-spectrum disorders (SSDs) and early psychosis in some studies, but not others. We conducted a meta-analysis of the research on alliance in SSDs and early psychosis across a range of interventions and outcomes. Parallel literature searches were conducted in PubMed and PsycINFO databases for articles between inception and 6/11/2020. English-language studies were included if they evaluated the relationship between alliance and a prospective outcome (treatment engagement, medication adherence, functioning, or total, positive, negative, or depressive symptoms) in an individual clinical treatment for SSDs/early psychosis and contained analyzable data. Correlations and partial correlations were meta-analyzed with random effects models to calculate mean across-study correlations and to carry out subsequent homogeneity and moderator variable analyses. Fourteen studies consisting of 2968 participants that assessed six outcomes across six psychosocial treatments were included. Results indicated that better client-rated (r = 0.20) and other-rated (i.e., provider- or observer-rated; r = 0.25) alliance were associated with better treatment engagement. Treatment type and sample race/ethnicity, but not age, gender, or timing of alliance rating moderated the association between other-rated alliance and engagement. Further, better other-rated alliance was related to improvements in positive (r = -0.14) and negative (r = -0.22) symptoms. A strong therapeutic alliance is important for both engaging clients with SSDs and early psychosis in treatment and facilitating improvements in positive and negative symptoms. Delivery and monitoring of treatments for this population should include assessment of the therapeutic alliance from multiple perspectives.

Abstract:Past research on help-seeking behaviour and compliance among the mentally ill has mainly been focused on the examination of individual and structural determinants, while the impact of the socio-cultural context has been largely neglected. To examine this impact, we conducted a representative survey among German citizens who were eligible to vote. By means of vignettes describing different mental disorders we recorded the lay public's preferences for various treatment methods, together with their subjective reasons for voicing these particular preferences. While we found that psychotherapy was generally held in high esteem by the lay public, psychopharmacotherapy was rejected by the vast majority of respondents. This pattern was observed for all of the mental disorders included in our survey. While the public's image of psychotherapy is largely determined by popular views on psychoanalysis, public opinion about psychotropic drugs is strongly influenced by characteristics associated with tranquillizers. Among the different psychotherapeutic approaches, psychoanalysis is the preferred method among respondents in the western part of Germany, while the lay public in the eastern part tends to endorse group therapy. Finally, our research showed a considerable difference between the conceptions held by the lay public and those of psychiatric experts with regard to the adequate treatment of mental disorders. This, in turn, should not be without consequence for help-seeking behaviour and patient compliance.

Abstract:Objective Cancer patients are often overwhelmed when being informed about clinical trials. However, there is a lack of evidence-based strategies to improve physician-patient communication in this area. This study assessed the experiences and needs of cancer patients and their support persons (SPs) during the informed consent (IC) process prior to participation in clinical trials. Methods 17 semi-structured interviews with cancer patients and their SP were conducted and analysed using a framework analysis. Results Most respondents reported feeling well informed about the clinical trial. However, core aspects of the study were often not understood highlighting a dissonance between perceived and actual recall and understanding. Many participants trusted that the trial recommended was the best available care and only skimmed the consent form or did not read it at all. Conclusions This is the first German study to analyse both cancer patients' and SPs' perspectives on IC processes. Although many feel well informed, our results suggest a significant gap in recall and understanding of core components of clinical trials which hinders IC. Practice Implications Further interventional research is required to improve the consent processes prior to clinical trials in order to provide optimal, patient-centred care.

The Alliance Negotiation Scale - Therapist Version: Psychometric Properties in a Sample of Portuguese Psychologists

Abstract:Background: The transtheoretical conceptualization of the working alliance and the resultant evaluation tools often overestimate the collaboration between therapist and client, while neglecting the negotiation process. The degree to which therapists and clients can negotiate disagreements regarding goals and tasks is an important indicator in establishing and maintaining the alliance. Even though the negotiation concept is not new, there is still a lack of reliable and parsimonious self-report measures of the construct. The purpose of this study was to translate, execute the cultural adaptation and, also, to perform a preliminary psychometric analysis of the Portuguese form of the therapist version of the Alliance Negotiation Scale (ANS-T_Pt). Method: Data were collected online from 100 Portuguese psychologists. Two random sub-samples were used to conduct both exploratory factorial analysis and confirmatory factorial analysis. Convergent validity was assessed through comparison with the Portuguese version of the Working Alliance Inventory. Results: The ANS-T_Pt showed a one-factorial structure, consistent with previous versions, and demonstrated adequate internal consistency. Evidence supporting criterion-related validity was found based on the correlations between ANS-T_Pt and WAI-T scores. The results showed moderate to large associations between the instruments. These results support the usefulness of the scale, construct's relevance and its transtheoretical nature. Conclusion: These results are a step forward for Portuguese therapists' and researchers' ability to evaluate the bond between client and therapist and to compare results from different countries.

Abstract:In the last decade, different types of online activities have been growing exponentially and online psychotherapy is moving in the same direction. It is crucial to recognize this line of work as a facilitator for people in the contemporary world, especially after the COVID-19 pandemic in 2020. The purpose of this study was to compare the perception of patients who underwent brief face-to-face psychodynamic psychotherapy and online psychotherapy by a single therapist. The study had a quantitative-qualitative approach, exploratory and comparative, which used the clinical methodology. The appointments were systematic and respective, consisting of six daily face-to-face sessions, on consecutive days, except on Sundays; and six online appointments, using the same process for data collection; the extent of each session was between 40 to 50 minutes. For the data collection regarding the perception of patients on the brief psychodynamic psychotherapy care in person versus online, were tested the Inventory of Therapeutic Alliance (Working Alliance Inventory) with questions about the therapeutic relationship, after the end of 6 online sessions, and the Questionnaire Session Evaluation Questionnaire right after the end of each psychotherapeutic session, both in face-to-face and online situations. The results obtained demonstrated that there is equivalence in face-to-face and online care, regarding the therapeutic relationship built in both moments.
Prevalence of suicidal ideation in German psychotherapy outpatients: A large multicenter assessment

Abstract:Background Suicidal ideation is a major concern in clinical practice. Yet, little is known about prevalence rates of suicidal ideation in patients undergoing outpatient psychotherapeutic treatment. Therefore, the aim of the current study is to assess the prevalence of suicidal ideation in a large sample of psychotherapy outpatients in Germany. The data analyzed in this study is taken from the KODAP-project on the coordination of data collection and analysis at German university-based research and training outpatient clinics for psychotherapy. Methods A total of N = 10,357 adult outpatients (64.4 % female; age: M(SD) = 35.94 (13.54), range: 18–92 years of age) starting cognitive-behavioral therapy at one of 27 outpatient clinics in Germany were included in the current study. Prevalence of suicidal ideation was assessed with the Suicide Item (Item 9) of the Beck-Depression Inventory II. Results Suicidal ideation was reported by 36.7 % ( n = 3795) of the participants. Borderline Personality Disorder, Posttraumatic Stress Disorder, and recurrent Major Depression were the diagnoses most strongly associated with the presence and severity of suicidal ideation. Limitation Suicide ideation was assessed only with the respective item of the Beck Depression Inventory II. Conclusion Suicidal ideation is very common among adult patients who start psychotherapy in Germany. A well-founded knowledge of risk assessment in suicidal patients and suicide-specific treatment options is therefore highly relevant.

Abstract:The Royal Australian and New Zealand College of Psychiatrists clinical practice guidelines on mood disorders recommend psychotherapy as foundational care for patients with acute depression with minimal discussion of any potential adverse effects. Randomised controlled trial evidence on psychotherapy adverse effects is limited. This is problematic because clinicians must balance the benefits of treatment against the harms, and clinical decisions become skewed without data on adverse effects. We suggest that clinical practice guidelines should be more guarded about recommending psychotherapy and add consensus statements on adverse effects for informed consent and clinical decision-making.
