Abstract:BACKGROUND: Computed tomography (CT) and magnetic resonance imaging (MRI) data can be fused to identify the tumor boundaries. This enables surgeons to set close but tumor-free surgical margins and excise the tumor more precisely. This study aimed to report our experience in performing computer navigation-aided joint-preserving resection and custom-made endoprosthesis reconstruction to treat bone sarcoma in the diaphysis and metaphysis of the femur and tibia.METHODS: Between September 2008 and December 2015, 24 patients with bone sarcomas underwent surgical resection and joint-sparing reconstruction under image-guided computer navigation. The cohort comprised 16 males and eight females with a median age of 19.5 years (range: 12-48 years). The tumor location was the femoral diaphysis in three patients, distal femur in 19, and proximal tibia in two. The tumors were osteosarcoma (n = 15), chondrosarcoma (n = 3), Ewing sarcoma (n = 3), and other sarcomas (n = 3). We created a pre-operative plan for each patient using navigation system software and performed navigation-aided resection before reconstructing the defect with a custom-made prosthesis with extracortical plate fixation.RESULTS: Pathological examination verified that all resected specimens had appropriate surgical margins. The median distance from the tumor resection margin to the joint was 30 mm (range: 13-80 mm). The median follow-up duration was 62.5 months (range: 24-134 months). Of the 24 patients, 21 remain disease free, one is alive with disease, and two died of the disease. One patient developed local recurrence. Complications requiring additional surgical procedures occurred in six patients, including one with wound hematoma, one with delayed wound healing, one with superficial infection, one with deep infection, and two with mechanical failure of the prosthesis. The mean Musculoskeletal Tumor Society score at the final follow-up was 91% (range: 80%-100%). The 5- and 10-year implant survival rates were 91.3% and 79.9%, respectively.CONCLUSIONS: Computer navigation-aided joint-preserving resection and custom-made endoprosthesis reconstruction with extracortical plate fixation is a reliable surgical treatment option for bone sarcoma in the diaphysis and metaphysis of the femur and tibia.

Abstract:BACKGROUND: Computer navigation during surgery can help oncologic surgeons perform more accurate resections. However, some navigation studies suggest that this tool may result in unique intraoperative problems and increased surgical time. The degree to which these problems might diminish with experience-the learning curve-has not, to our knowledge, been evaluated for navigation-assisted tumor resections.QUESTIONS/PURPOSES: (1) What intraoperative technical problems were observed during the first 2 years using navigation? (2) What was the mean time for navigation procedures and the time improvement during the learning curve? (3) Have there been any differences in the accuracy of the registration technique that occurred over time? (4) Did navigation achieve the goal of achieving a wide bone margin?METHODS: All patients who underwent preoperative virtual planning for tumor bone resections and operated on with navigation assistance from 2010 to 2012 were prospectively collected. Two surgeons (GLF, LAA-T) performed the intraoperative navigation assistance. Both surgeons had more than 5 years of experience in orthopaedic oncology with more than 60 oncology cases per year per surgeon. This study includes from the very first patients performed with navigation. Although they did not take any formal training in orthopaedic oncology navigation, both surgeons were trained in navigation for knee prostheses. Between 2010 and 2012, we performed 124 bone tumor resections; of these, 78 (63%) cases were resected using intraoperative navigation assistance. During this period, our general indications for use of navigation included pelvic and sacral tumors and those tumors that were reconstructed with massive bone allografts to obtain precise matching of the host and allograft osteotomies. Seventy-eight patients treated with this technology were included in the study. Technical problems (crashes) and time for the navigation procedure were reported after surgery. Accuracy of the registration technique was defined and the surgical margins of the removed specimen were determined by an experienced bone pathologist after the surgical procedure as intralesional, marginal, or wide margins. To obtain these data, we performed a chart review and review of operative notes.RESULTS: In four patients (of 78 [5%]), the navigation was not completed as a result of technical problems; all occurred during the first 20 cases of the utilization of this technology. The mean time for navigation procedures during the operation was 31 minutes (range, 11-61 minutes), and the early navigations took more time (the regression analysis shielded R2 = 0.35 with p < 0.001). The median registration error was 0.6 mm (range, 0.3-1.1 mm). Registration did not improve over time (the regression analysis slope estimate is -0.014, with R2 = 0.026 and p = 0.15). Histological examinations of all specimens showed a wide bone tumor margin in all patients. However, soft tissue margins were wide in 58 cases and marginal in 20.CONCLUSIONS: We conclude that navigation may be useful in achieving negative bony margins, but we cannot state that it is more effective than other means for achieving this goal. Technical difficulty precluded the use of navigation in 5% of cases in this series. Navigation time decreased with more experience in the procedure but with the numbers available, we did not improve the registration error over time. Given these observations and the increased time and expense of using navigation, larger studies are needed to substantiate the value of this technology for routine use.LEVEL OF EVIDENCE: Level IV, therapeutic study.

Abstract:BACKGROUND: Small recent case series using CT-based navigation suggest such approaches may aid in surgical planning and improve accuracy of intended resections, but the accuracy and clinical use have not been confirmed.QUESTIONS/PURPOSES: We therefore evaluated (1) the accuracy; (2) recurrences; and (3) function in patients treated by computer-assisted tumor surgery (CATS).METHODS: From 2006 to 2009, we performed CATS in 20 patients with 21 malignant tumors. The mean age was 31 years (range, 6-80 years). CT and MR images for 18 cases were fused using the navigation software. Reconstructed two-dimensional/three-dimensional images were used to plan the bone resection. The achieved bone resection was compared with the planned one by assessing margins, dimensions at the level of bone resection, or fitting of CAD custom prostheses. Function was assessed with the Musculoskeletal Tumor Society (MSTS) score. The minimum followup was 31 months (mean, 39 months; range, 5-69 months).RESULTS: Histological examination of all resected specimens showed a clear tumor margin. The achieved bone resection matched the planned with a difference of ≤ 2 mm. The achieved positions of custom prostheses were comparable to the planned positions when merging postoperative with preoperative CT images in five cases. Three of the four patients with local recurrence had tumors at the sacral region. The mean MSTS score was 28 (range, 23-30).CONCLUSION: CATS with image fusion allows accurate execution of the intended bone resection. It may be beneficial to resection and reconstruction in pelvic, sacral tumors and more difficult joint-preserving intercalated tumor surgery. Comparative clinical studies with long-term followup are necessary to confirm its efficacy.LEVEL OF EVIDENCE: Level IV, therapeutic study. Precise resection and biological reconstruction under navigation guidance for young patients with juxta-articular bone sarcoma in lower extremity: preliminary report

Abstract:Background: It is a challenge to perform a joint-preserving resection for young patients with juxta-articular bone sarcomas. We determined whether osteotomy under image-guided navigation make joint-saving resection possible for juxta-articular lesions while adhering oncological principles. Methods: Between June 2008 and July 2010, joint-preserving limb salvage surgeries were performed on 9 patients with juxta-articular bone sarcomas under navigation guidance. Computed tomography/magnetic resonance imaging fusion images were used for real-time navigation. Eight lesions located around the knee and 1 in hip. Six tumors extend to and 3 beyond the epiphyseal line. Planned osteotomy under image-guided navigation was employed for achieving clear surgical margin while maximizing host tissue preservation. All tumors were en bloc removed and intercalary defect were reconstructed by combination of allograft with vascularized fibula graft. All specimens were examined for resection margin. Patients were followed up for an average of 25.2 months for evaluating of functional and oncology outcomes. Results: Entire joint were preserved in 6 patients and part of joint were saved in another 3 patients. The mean registration error for navigation was 0.40 mm (range, 0.31 to 0.62 mm). Clear surgical margin was obtained in all specimens. The average closest distance between the osteotomy line and tumor edge was 9.6 mm (range, 6 to 14 mm). Entire joint cartilage was preserved in 6 patients and portion of joint were saved in 3 patients (2 in proximal tibia, 1 in distal femur). No patient experienced local recurrence. Two patients developed lung metastasis. One died of disease and the other underwent metastasectomy and had no evidence of disease at the most recent follow-up. All reconstruction was in situ with the Musculoskeletal Tumor Society average score of 26.7 at final follow-up. Conclusions: With careful patient selection, image navigation-assisted surgery made it possible to resect the bone exactly as planned in length and orientation in the magnetic resonance imaging image, yielding a clear margin and preserving the entire or part of the articular cartilage in joint-sparing limb salvage procedures for treating skeletally immature patients with juxta-articular bone sarcomas. Computer navigation-assisted surgical techniques in the surgical resection of pelvic chondrosarcomas

Abstract:Objective:To explore the clinical effects of computer navigation-assisted surgery in the precise resection of pelvic chondrosarcoma.Methods:A retrospective analysis of 54 patients who had computer-assisted surgery from Dec 2007 to Dec 2018, including 27 males and 27 females, was conducted. The average age was 34.00±1.41 years (range 23-72 years). There were 47 cases with primary tumors and 7 with recurrence cases. The tumors in 15 cases located in the ilium (region I), 35 in the acetabulum (region II), 1 in the pubic (region III), and 3 in the sacroiliac joint (region IV). A total of 45 cases (83.3%) underwent needle biopsy, and 4 cases (7.4%) had incision biopsy. Among 5 cases who did not have biopsy, two of them was diagnosed of malignant change of multiple osteochondromas, two cases were diagnosed of recurrent pelvic chondrosarcoma and one with pelvic malignant tumor by imaging examinations. Pathological grade was presented as following, 36 cases in grade I, 15 in grade II, and 3 in grade III. All operations were performed on the bases of preoperative design with computer navigation-assisted surgical technology. A total of 49 cases (90.7%) had limb salvage operations and 5 cases had amputations. The surgical margins were confirmed by gross appearance and the maximum diameter profile of the tumor. Univariate analysis was performed to compare recurrence rate of different preoperative tumor status, gender, tumor stage, biopsy method, tumor location, operation method and surgical margins.Results:There were 39 cases underwent extensive resection, 13 cases with marginal resection and 2 cases with intracapsular resection. In 52 cases (96.3%), the surgery was performed according to the preoperative plan of surgical resection margin. However, two cases (3.7%) was not performed based on the preoperative plan. All patients were followed-up for 84.00±93.34 months (range 12-150 months). During the follow-up, a total of 45 cases (83.3%) survived and 9 cases died from lung metastasis. Eight cases (14.8%, 8/54) had local recurrence of whom 7 (14.3%) were limb salvage cases and 1 (20.0%, 1/5) had amputation. There was significantly different in local recurrence rate (χ 2=17.022, P=0.001). The risk of recurrence of marginal resection was 8.222 times than that of extensive resection [95% CI (1.297, 52.140)]. According to the Musculoskeletal Tumor Society (MSTS) limb function evaluation system score, postoperative limb function recovery rate was 90.00%±4.71% (range 60.00%-100%). There were 13 cases (24.1%) had postoperative complications, including 7 cases (13.0%) of infection, 2 cases (3.7%) of operative area and deep vein thrombosis of lower extremity, and 4 cases (7.4%) of skin necrosis and delayed healing. Among 49 limb salvage patients, two of them had secondary amputation due to tumor recurrence, five had hemipelvectomy due to neurovascular tumor invasion. The final limb salvage rate was 77.8% (42/54). Conclusion:Computer navigation-assisted precise pelvic tumor resection is technically feasible. It could decrease recurrence rate and promote limb function recovery by improving the reliability of oncology evaluation and the accuracy of tumor resection with superior safety.
Navigation-assisted Maxillofacial Reconstruction: Accuracy and Predictability.

Abstract:The aim of this study was to evaluate the accuracy of navigation-assisted maxillofacial reconstruction and to identify the predictors of the clinical outcomes. A total of 112 patients who underwent navigation-assisted maxillofacial reconstruction with free flaps between 2014 and 2019, performed by a single surgical team, were assessed. Accuracy was evaluated by superimposing the postoperative computed tomography data with the preoperative virtual surgical plan. Predictors of the clinical outcomes affecting the accuracy were identified and analysed. The mean deviation and root mean square (RMS) estimate of the orbital, maxillary, and mandibular reconstructions were 0.88 ± 3.25 mm and 3.38 ± 0.73 mm, 0.77 ± 3.44 mm and 3.69 ± 0.82 mm, and 1.07 ± 4.16 mm and 4.67 ± 3.95 mm, respectively (P < 0.05). There was no significant difference in orbital volume or projection between the preoperative, postoperative, and healthy orbits (P = 0.093 and P = 0.225, respectively). Multivariate linear regression analysis confirmed significant associations between the accuracy of navigation-assisted mandibular reconstruction and preservation of the condyle, type of reconstruction, type of osteosynthesis plate, and number of bony segments. Navigation-assisted midface reconstruction yielded a higher level of accuracy in the final surgical outcome when compared to mandibular reconstruction. Computer-assisted techniques and intraoperative navigation can be an alternative or adjunct to current surgical techniques to improve the final surgical outcome, especially in more complex maxillofacial reconstructions.
New Perspectives on Surgical Accuracy Analysis of Image-Guided Bone Tumour Resection Surgery

Abstract:PURPOSE:Image-guided bone tumour resection surgery has been proved in previous literatures to be more accurate than those conventional freehand ones (p < 0.001). However, in this kind of surgery, there are still many procedures depending on manual operations, which will inevitably introduce surgical errors into the surgery. In particular, the negative surgical errors (i.e., errors toward tumour) would increase the risk of tumor recurrence and metastasis. Thus, the first purpose of this study was to evaluate whether the negative surgical errors of image-guided bone tumour resection surgery were statistically significantly great, the second purpose is to evaluate whether the negative surgical errors of image-guided long-bone tumour resection surgery were statistically equivalent to those of pelvis surgery, and the last purpose is to recommend a solution for suppressing these errors when using a navigation system. METHODS:Negative surgical errors of 24 osteotomies in ten pelvis tumour resection operations and 16 osteotomies in ten long-bone surgeries under the image guidance of a navigation system were statistically evaluated and compared with - 2.0 mm. The equivalence of negative surgical errors of pelvis group and those of long-bone group was statistically tested. To suppress these negative surgical errors when using a navigation system, we recommend, based on the obtained statistics, to increase the margins between cut planes and tumour boundary during pre-operatively planning cut planes, by adding an extra margin with the empirical safe margin according to the absolute lower bound of 95% CI of negative surgical errors. RESULTS:Negative surgical errors of the pelvis group and the long-bone group were both significantly less than - 2.0 mm (p < 0.001), but not statistically equivalent (Rg > 1 mm). 95% CI of negative surgical errors were from - 3.95 to - 3.27 mm for the pelvis group, and from - 2.69 to - 2.34 mm for the long-bone group. So, the extra margin added for image-guided pelvis tumour resection surgery should be set as 3.95 mm, and the extra margin added for image-guided long-bone surgery should be set as 2.69 mm. CONCLUSION:The negative surgical errors of image-guided bone resection surgery were statistically significantly less than - 2.0 mm (p < 0.001), thus these errors cannot be safely ignored. Moreover, the negative surgical errors of the pelvis group were not equivalent to those of the long-bone group (Rg > 1.0 mm), thus the solution for image-guided pelvis tumour resection surgery and that for image-guided long-bone tumour resection surgery should be separately determined. In order to suppress these negative surgical errors when using a navigation system, we recommend to add extra 3.95 mm margin with the empirical safe margin for image-guided pelvis tumour resection surgery and to add extra 2.69 mm margin for image-guided long-bone tumour resection surgery during pre-operatively planning cut planes.
The Safety and Accuracy of Surgical Navigation Technology in the Treatment of Lesions Involving the Skull Base

Abstract:OBJECTIVE: As compared with the normal anatomy, structures of the skull base and its surroundings have been dramatically altered of the lesion near the skull base. How to remove the lesion at the skull base was mainly depending on surgeon's personal experience during past years. In this study, the authors explored the safety and accuracy of the surgical navigation system in resecting lesions near the skull base.METHODS: The surgical cases consisted of 15 patients who underwent surgery involving the skull base with the using of surgical navigation technology. Five patients had adenoid cystic carcinoma at minor salivary glands of the palate extending to the skull base and 10 patients were suffered from bony ankylosis of temporomandibular joint. After the system converted patient's computed tomography scans into three-dimensional reconstructive images, preoperative planning and simulation of the operation process could be made by surgeons. During the operation, the virtual images were matched with the patient through individual registration. The system provided surgeon with feedback about the operation on the screen instantly with moving the navigated pointer.RESULTS: The application of surgical navigation system was safe and reliable for resecting the ankylotic bone and identifying the extent of tumors resection near the skull base. No complications including cerebrospinal fluid leak, cranial nerve injuries, severe bleeding happened to those patients.CONCLUSIONS: Given the safety and accuracy of the surgical navigation system, surgery near the skull base appeared to be an ideal field for using with its complex anatomy.

Computer Navigation-Aided Excision of Proximal Femoral Osteochondroma: Surgical Technique

Abstract:Purpose. Symptomatic osteochondroma of the proximal femur necessitates a surgical excision. The purpose of this study was to describe a novel technique of computer navigation-aided excision for osteochondromata of the proximal femur. Outcomes of this technique are also presented. Methods. A total of 13 patients underwent computer navigation-aided excision of osteochondromata of the proximal femur from February 2012 to August 2016 in our institution. They were enrolled in this study. OrthoMap 3D (Stryker Orthopaedics, Mahwah, NJ, USA), a commercially available navigation software system, was used to merge computed tomography images of the proximal femur with an osteochondromata with the image of a normal proximal femur. Using the normal proximal femur as a template, intended resection margins for the proximal femur with osteochondromata were planned and then executed using intraoperative navigation guidance. Patients were followed up clinically and radiographically. The physical and mental health of patients was assessed with the Musculoskeletal Tumor Society (MSTS) score. Results. Eight patients had isolated exostoses. Five patients had tumors associated with multiple hereditary exostoses. For tumors projecting posteriorly or posteromedially, a posterolateral approach was used. For tumors projecting anteriorly or medially, an anterior approach was used. Prophylactic fixation was performed in four patients who required an anterior approach. The mean duration of the surgery was 189 minutes. There were no intraoperative fractures or postoperative complications. A secondary procedure was not needed for any case. The mean MSTS score at a mean follow-up of 17 months was 28.6 (maximum MSTS score: 30). Conclusions. This is the first study to report a novel application of computer navigation for aiding the excision of osteochondromata of the proximal femur. It demonstrated favorable postoperative functional scores with a low rate of complications. The applicability, safety, and efficacy of this technique were demonstrated. It is particularly useful for resections involving large tumors that can obscure anatomical landmarks and for patients with associated proximal femoral deformity.
Accuracy of a New Robotically Assisted Technique for Total Knee Arthroplasty: A Cadaveric Study

Abstract:Background: Although the utility of robotic surgery has already been proven in cadaveric studies, it is our hypothesis that this newly designed robotically assisted system will achieve a high level of accuracy for bone resection. Therefore, we aimed to analyze in a cadaveric study the accuracy to achieve targeted angles and resection thickness. Methods: For this study, 15 frozen cadaveric specimens (30 knees) were used. In this study, Zimmer Biomet (Warsaw, IN) knees, navigation system, and robot (ROSA Knee System; Zimmer Biomet) were used. Eight trained, board-certified orthopedic surgeons performed robotically assisted total knee arthroplasty implantation using the same robotic protocol with 3 different implant designs. The target angles obtained from the intraoperative planning were then compared to the angles of the bone cuts performed using the robotic system and measured with the computer-assisted system considered to be the gold standard. For each bone cut the resection thickness was measured 3 times by 2 different observers and compared to the values for the planned resections. Results: All angle mean differences were below 1° and standard deviations below 1°. For all 6 angles, the mean differences between the target angle and the measured values were not significantly different from 0 except for the femoral flexion angle which had a mean difference of 0.95°. The mean hip-knee-ankle axis difference was -0.03° ± 0.87°. All resection mean differences were below 0.7 mm and standard deviations below 1.1mm. Conclusion: Despite the fact that this study was funded by Zimmer Biomet and only used Zimmer Biomet implants, robot, and navigation tools, the results of our in vitro study demonstrated that surgeons using this new surgical robot in total knee arthroplasty can perform highly accurate bone cuts to achieve the planned angles and resection thickness as measured using conventional navigation.
Computer-aided planning with exact implementation into surgical technique in TKA is as accurate as intraoperative navigation.

Abstract:BACKGROUND Exact positioning of implants and accuracy of alignment are important parameters to provide an long survivorship of endoprostheses after total knee arthroplasty. It was suggested that an alignment within 3∘ from centerline provides the best long-term survivorship of TKA. Therefore, computer-assisted navigation became more important in TKAs. Another tool to improve the accuracy in TKA is the preoperative planning software. OBJECTIVE Main goal was to determine if advantages of an intraoperative navigation system during TKA can be reached by an exact implementation of a preoperative computer-aided planning. METHODS Based on all patients (n= 100) underwent primarily TKA in 2015 and 2016 two groups were declared: (1) conventionally operated TKA without navigation system and (2) operation with an optical navigation system. Data on age, sex, date, operative time, severe complications and preoperative vs. postoperative alignment were collected retrospectively. RESULTS The two groups do not differ in postoperative alignment and frequency of outliers. Furthermore, there was no difference referring to complications and the length of stay in hospital, but operative time was prolonged in the navigation-assisted group. CONCLUSIONS It can be stated that conventional surgical techniques in TKA are as accurate as navigated ones if an exact preoperative computer aided planning is implemented during surgery.

A Case-Control Study of Computer Navigation Assisted Resection of Primary Sacral Chordoma above Sacrum 3 Level.

Abstract:Background: The operation of sacral chordoma resection is difficult especial in the tumor above sacrum 3 level and the local recurrence rate was high. The purpose of this study is to analyze the effect of computer navigation aided technology in primary sacral chordoma resection above sacrum 3 level through a case-control study, which including perioperative safety, surgical margin, postoperative recurrence and function results. Methods: This is a retrospective case-control study. The clinical data of 25 patients received initial computer assisted resection of sacral chordoma above the level of sacrum 3 from 2009 to 2016 were analyzed; the patients underwent non-navigation assisted resection of tumor above the level of sacrum 3 in the same period were matched and 25 patients were selected randomly. There was no significant difference between these two groups in gender (P = 0.370), age (P = 0.554), tumor transverse diameter (P = 0.836). The average maximum diameter of tumor in navigation group was significant bigger than that in non-navigation group (P = 0.005). The intraoperative safety results, surgical margin, postoperative complications, recurrence rate and function were compared between these groups. Results: There was no significant difference between navigation and non-navigation group in operative time (P = 0.105) and intraoperative blood loss (P = 0.537). There were 18 wide resections, 4 marginal resections and 3 intracapsular resections in navigation group; there were 6 wide resections, 12 marginal resections and 7 intracapsular resections in non-navigation group; the surgical margins of two groups were significant different (P = 0.003). There were 5 cases (20%) and 6 cases (24%) with wound complication in navigation group and non-navigation group (P = 0.733). The average follow-up was 49.6 (16-102) months in navigation group and 51.3 (12-110) months in non-navigation group. Three cases (12%) showed recurrence in navigation group and six cases showed recurrence (24%) in non-navigation group. The surgical margin was significantly related with tumor recurrence (P = 0.000). The average MSTS score was 27.3 (19-30) and 26.5 (20-29) in navigation group and non-navigation group (P = 0.374). Conclusion: The computer navigation aided technology can improve the accuracy of primary sacral chordoma resection, and make more cases achieve safe surgical margin. Compared with the traditional operation, the application of computer navigation in the larger tumor resection does not increase the operation time and intraoperative blood loss, which shows good safety.
3-Dimensional accuracy of navigation-guided bimaxillary orthognathic surgery: A systematic review and meta-analysis

Abstract:The transfer of a virtual orthognathic surgical plan to the patient still relies on the use of occlusal splints, which have limitations for vertical positioning of the maxilla. The use of real-time navigation has been proposed to enhance surgical accuracy. This systematic review (PROSPERO CRD42024497588) aimed to investigate if surgical navigation can improve the three-dimensional accuracy of orthognathic surgery. The inclusion criteria were orthognathic surgery, use of intra-operative navigation and quantitative assessment of surgical accuracy. The exclusion criteria were non-bimaxillary orthognathic surgeries, non-clinical studies, studies without post-operative 3D analysis and publications not in the English language. A search of PubMed, Embase and Cochrane Library generated 940 records, of which 12 were found relevant. Risk of bias was assessed done using the Joanna Briggs Institute Critical Appraisal Checklist Tool. Among the included studies, there were nine of observational character and three randomized control studies (RCTs). All studies demonstrated promising outcomes with reported good surgical accuracy within a 2 mm difference between the planned and post-surgical result. Meta-analysis of two RCTs was carried out and results were in favor of surgical navigation with a total odds ratio of 4.44 [2.11, 9.37] and an overall effect outcome of Z = 3.92 (p < 0.0001). Navigation was up to 0.60 mm more accurate than occlusal wafers only (p < 0.001). However, there were variations in the application of surgical navigation and methods of analysis, leading to a heterogenous data set. Future studies should focus on standardized protocols and analysis methods to further validate the use of surgical navigation in orthognathic surgery. Despite some limitations, surgical navigation shows potential as a valuable tool in improving the accuracy of orthognathic surgery.
Accuracy of Mixed Reality Combined With Surgical Navigation Assisted Oral and Maxillofacial Tumor Resection

Abstract:Objective To evaluate the feasibility and accuracy of mixed reality combined with surgical navigation in oral and maxillofacial tumor surgery. Methods Retrospective analysis of data of seven patients with oral and maxillofacial tumors who underwent surgery between January 2019 and January 2021 using a combination of mixed reality and surgical navigation. Virtual surgical planning and navigation plan were based on preoperative CT datasets. Through IGT-Link port, mixed reality workstation was synchronized with surgical navigation, and surgical planning data were transferred to the mixed reality workstation. Osteotomy lines were marked with the aid of both surgical navigation and mixed reality images visualized through HoloLens. Frozen section examination was used to ensure negative surgical margins. Postoperative CT datasets were obtained 1 week after the surgery, and chromatographic analysis of virtual osteotomies and actual osteotomies was carried out. Patients received standard oncological postoperative follow-up. Results Of the seven patients, four had maxillary tumors and three had mandibular tumors. There were total of 13 osteotomy planes. Mean deviation between the planned osteotomy plane and the actual osteotomy plane was 1.68 ± 0.92 mm; the maximum deviation was 3.46 mm. Chromatographic analysis showed error of ≤3 mm for 80.16% of the points. Mean deviations of maxillary and mandibular osteotomy lines were approximate (1.60 ± 0.93 mm vs. 1.86 ± 0.93 mm). While five patients had benign tumors, two had malignant tumors. Mean deviations of osteotomy lines was comparable between patients with benign and malignant tumors (1.48 ± 0.74 mm vs. 2.18 ± 0.77 mm). Intraoperative frozen pathology confirmed negative resection margins in all cases. No tumor recurrence or complications occurred during mean follow-up of 15.7 months (range, 6-26 months). Conclusion The combination of mixed reality technology and surgical navigation appears to be feasible, safe, and effective for tumor resection in the oral and maxillofacial region.

Abstract:Total knee replacement requires high measurement accuracy and fixation precision in surgical operation. Misplacement larger than 5 degrees in the force line alignment will lead to re- operation or long term deficits. Based on conventional operation facilities, it was not easy to ensure the necessary precision during het surgery. With the help of CT images, 3D images of patient's knee can be reconstructed. With IR localizer, computer- assisted knee surgical navigation can be realized by tracking that is useful for accurate alignment in surgery and in visualized training program.

Abstract:INTRODUCTION: Surgical navigation has been used in musculoskeletal tumor surgical procedures to improve the precision of tumor resection. Despite the favorable attributes of navigation-assisted surgery, conventional systems do not display the resection margin in real time, and preoperative manual input is required. In addition, navigation systems are often expensive and complex, and this has limited their widespread use. In this study, we propose an augmented reality surgical navigation system that uses a tablet personal computer with no external tracking system.MATERIAL AND METHODS: We realized a real-time safety margin display based on three-dimensional dilation. The resection plane induced by the safety margin is updated in real time according to the direction of sawing. The minimum separation between the saw and the resection plane is also calculated and displayed. The surgeon can resect bone tumors accurately by referring to the resection plane and the minimum separation updated in real time.RESULTS: The effectiveness of the system was demonstrated with experiments on pig pelvises. When the desired resection margin was 10 mm, the measured resection margin was 9.85 ± 1.02 mm.CONCLUSIONS: The proposed method exhibits sufficient accuracy and convenience for use in bone tumor resection. It also has favorable practical applicability due to its low cost and portability.

Abstract:Computer-aided navigation is a kind of special instruments, which are connected to the navigation system to touch an anatomical position on the patient. This position is simultaneously shown in the images taken from this patient. The surgeon can thus use the instrument to ‘navigate’ the images of the patient by moving the instrument. We used this technique to handle the lesion under the skull base, including orbit reconstruction, resection of skull base tumours, treatment of temporomandibular joint ankylosis, removal of foreign bodies, needle biopsy, placement of radiation seeds, etc. Our experience shows that navigation technique can provide precise preoperative surgical design and navigate surgical procedures according to the design, improving surgical accuracy and raising safety.

Abstract:OBJECTIVE:To evaluate the application of computer-aided design and navigation technology in skull base and infratemporal fossa tumor surgery and to analyze its advantages and disadvantages.METHODS:Twenty-nine cases with tumor of skull base and infratemporal fossa were treated with computer-aided design and navigation surgery. The Parameters of age, gender, primary or recurrent tumor, tumor nature and surgical approach were recorded.RESULTS:En bloc resection was performed in 20 cases and subtotal resection in 9 cases. The margin status was negative margin in 8 cases, near-tumor margin in 17 cases and positive resection margin in 4 cases. Postoperative complication rate was 14% (4/29). During the follow-up period, 2 benign cases recurred.In the malignant group, there were 7 cases of recurrence, 2 cases of metastasis and 3 deaths. The 5-year overall survival and progression-free survival rate were 69% and 40% respectively.CONCLUSIONS:Navigation technology can enhance the confidence of the surgeons and operation safety in handling malignant tumors in skull base and infratemporal fossa.

Abstract:Purpose: Even though computer-assisted navigation systems have been shown to improve the accuracy of implantation of components into the femur and tibia, long-term results are lacking and there is little evidence yet that navigation techniques also improve functional outcomes and implant longevity following total knee arthroplasty (TKA). The aim of this study was to summarize and compare the clinical outcomes of total knee arthroplasties (TKAs) performed using navigation-assisted and conventional techniques. Methods: The study was conducted according to the guidelines described in the Cochrane Handbook for Systematic Reviews of Interventions and Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statements. Methodological features were rated independently by two reviewers. A meta-analysis of randomized controlled trials (RCTs) or quasi- randomized controlled trials (qRCTs) was carried out to evaluate the efficacy of CAS versus conventional TKA. Data were pooled in fixed and random effects models and the weighted mean difference (WMD) and odds ratio (OR) were calculated. Heterogeneity across studies was determined, and subgroup analyses by the type of navigation system (image-based or image-free navigation system) were conducted. Results: Twenty-one studies that included 2333 knees were collected from different countries. The surgical time was longer for CN TKA than for the conventional procedure. There was no significant difference in the Knee Society Score between the two groups at the 3-month and 6-month follow-up. The rates of postoperative complications in patients who had CN TKA were similar to those in the patients who had conventional TKA. Conclusion: No significant differences in short-term clinical outcomes were found following TKAs performed with and without computer navigation system. However, there is clearly a need for additional high-quality clinical trials with long-term follow-up to confirm the clinical benefits of computer-assisted surgery. (C) 2011 Elsevier B.V. Abstract:Background: Augmented reality (AR)-based navigation surgery has evolved to be an advanced assisted technology. The aim of this study is to manifest the accuracy of AR navigation for the intraoperative mandibular angle osteotomy by comparing the navigation with other interventional techniques. Methods: A retrospective study was conducted with 93 post-surgical patients with mandibular angle hypertrophy admitted at our plastic and reconstructive surgery department between September 2011 and June 2016. Thirty-one patients received osteotomy conducted using a navigation system based on augmented reality (AR group), 28 patients received osteotomy conducted using individualised templates (IT group) and the remaining 34 patients received osteotomy performed by free hand (free-hand group). The post-operative computed tomography (CT) images were reviewed and analysed by comparing with pre-surgical planning generated by three-dimensional (3D) software. The preparation time, cutting time, whole operating time and discrepancy in osteotomy lines were measured. Results: The preparation time was much shorter for the free-hand group than that for the AR group and the IT group (P < 0.01). However, no significant difference in the whole operating time was observed among the three groups (P > 0.05). In addition, the discrepancy in osteotomy lines was lower for the AR group and in the IT group than for the free-hand group (P < 0.01). Conclusion: The navigation system based on AR has a higher accuracy, more reliability and better user friendliness for some particular clinical procedures than for other techniques, which has a promising clinical prospect.
