Abstract:Background: Avulsion fracture of the ischial tuberosity is a relatively clinically rare type of trauma that is mainly incurred by adolescents during competitive sports activities. According to previous literature, the most commonly involved sports are soccer, sprinting, and gymnastics, in descending order. Dance-induced avulsion fracture of the ischial tuberosity and ischial ramus is extremely clinically rare. Case summary: A case of a neglected avulsion fracture of the ischial tuberosity and ischial ramus was diagnosed in a young female dancer who complained of pain and restricted movement of her right hip. She stated that she had suffered the injury while performing a split leap during a dance performance 9 mo prior. Eventually, she underwent surgery and obtained satisfactory treatment results. Conclusion: Early diagnosis of these fractures is important to ensuring early proper treatment towards a quicker recovery. For old fractures with nonunion and chronic buttock pain, surgery is a preferred therapeutic choice with good treatment outcomes.

Abstract:Background: In cases of avulsion fracture of the ischial tuberosity in which the bone fragments are substantially displaced, nonunion may cause pain in the ischial area. Various surgical procedures have been reported, but achieving sufficient fixation strength is difficult. Case presentation: We treated a 12-year-old male track-and-field athlete with avulsion fracture of the ischial tuberosity by suture anchor fixation using the suture bridge technique. The boy felt pain in the left gluteal area while running. Radiography showed a left avulsion fracture of the ischial tuberosity with approximately 20-mm displacement. Union was not achieved by conservative non-weight-bearing therapy, and muscle weakness persisted; therefore, surgery was performed. A subgluteal approach was taken via a longitudinal incision in the buttocks, and the avulsed fragment was fixed with five biodegradable suture anchors using the suture bridge technique. Conclusions: Although the majority of avulsion fractures of the ischial tuberosity can be treated conservatively, patients with excessive displacement require surgical treatment. The suture bridge technique provided secure fixation and enabled an early return to sports activities.

Abstract:Total hip arthroplasty in the high riding dislocated hip is a technically difficult undertaking, with major reconstruction required on both the acetabular and femoral sides. With reconstruction at a near-anatomic hip center, reduction of the arthroplasty is difficult because of the long-standing limb shortening. The major block to reduction is tension of the soft tissues, particularly the hamstrings. We report a case of ischial tuberosity avulsion fracture following such a complex reconstruction despite femoral shortening subtrochanteric osteotomy. This illustrates the importance of the hamstring group in maintaining the dislocation and emphasizes the need to prevent overtension of the soft tissues in such complex reconstructive procedures.

Abstract:The avulsion fracture of the ischial tuberosity (IT) is uncommon and relatively specific in children or teenagers. Without proper imaging and/or clinical suspicion, this injury can be mismanaged and cause nonunion or displacement of the bony avulsion, which may require surgery. Although surgical intervention is rarely necessary, it has a high degree of success when it is used. Several minimally invasive interventions have been reported in previous studies, but there are no arthroscopic techniques in the literature for reference. Here we report an unusual case of nonunion avulsion fractures of IT in a 19-year-old male dancer. The injury was missed initially and delayed for 5 years. The main symptom was intermittent dull pain of left buttock. Under hip arthroscopy, we removed the nonunion bone and repaired the insertion of the hamstring tendon. The patient was satisfied and returned to sports within 6 months. Although there is only one patient, this is the first time that the arthroscopic technique has been introduced to treat this unusual injury. All experiences based on this case, we present the following case in accordance with the CARE reporting checklist, including positioning, approach, intraoperative targeting, manipulation and tendon fixation, and a brief review will be present.
Pelvic Apophyseal Avulsion Fractures: A Retrospective Review of 228 Cases

Abstract:Background: The aim of this study was to assess the patient demographics, epidemiology, mechanism of injury, and natural history of pelvic apophyseal avulsion fractures. Methods: A retrospective records review of imaging and clinical documentation was performed for patients diagnosed with pelvic apophyseal avulsion fractures at our institution from 2007 to 2013. Patient's Risser score, triradiate status, fracture location, size, and displacement were recorded based on initial injury radiographs. Further clinical and radiographic chart review was utilized to determine mechanism of injury, presence of multiple/bilateral injuries, nonunion, chronic pain, as well as any surgical interventions performed. Results: We identified 225 patients diagnosed with 228 apophyseal avulsion fractures with mean age of 14.4 years. Males represented 76% of the patients. Anterior inferior iliac spine (AIIS) avulsions were the most common, representing 49% of all avulsion fractures, followed by anterior superior iliac spine (30%), ischial tuberosity (11%), and iliac crest (10%). The most common mechanism of injury was sprinting/running (39%) followed by kicking (29%), but the mechanism varied by fracture type with 50% of AIIS avulsions caused by kicking. Multiple pelvic fractures were identified in 6% of patients. Pain >3 months out from initial injury was present in 14% of all patients and AIIS avulsion fractures were 4.47 times more likely to have chronic pain. Five nonunions were identified, 4 of which were ischial tuberosity avulsions. Initial fracture displacement >20 mm increased the risk of nonunion by 26 times. Surgical treatment was indicated in 3% of cases. Conclusions: In this series, nearly all pelvic avulsion fractures (97%) were managed successfully with a conservative approach. Contrary to prior studies, AIIS avulsions represented half of the avulsion fractures. AIIS and ischial tuberosity fractures are at increased risk of developing future pain and nonunions, respectively. Patients and families need to be counseled about this possibility because future intervention may be necessary. Level of evidence: Level IV-therapeutic.

Abstract:Case: A 13-year-old boy suffered a sports injury of the right knee. The clinical examination showed swelling, inability to bear weight, and pain involving the inferior apex of the patella and the anterior tibial tuberosity. Imaging studies showed an avulsion fracture, sleeve-like, from the patella apex and avulsion of the tibial tuberosity. The patient underwent open reduction and internal fixation. At the 24-month follow-up, no pain, limitation, or complication was reported. Conclusions: Bifocal patellar tendon avulsion in a skeletally immature patient is an extremely rare injury, and to our knowledge, only one case is reported.

Abstract:Avulsion fractures of the pelvis and the proximal femur are relatively rare injuries in the adolescent age group of 11 to 17 years. The overall prognosis for spontaneous healing is good. The injury most frequently involves the apophysis of the anterior inferior iliac spine and of the lesser trochanter, followed by anterior superior iliac spine. In our experience the avulsion fractures involving the ischial tuberosity and the greater trochanter are extremely rare. The latter can have dire consequences.

Abstract:Ischial tuberosity avulsion fractures are overall uncommon but are known injuries in the adolescent population. They are the result of sudden, forceful contraction of the hamstring muscle groups. The characteristic radiographic appearance of an ischial tuberosity avulsion fracture is of an irregular ischial margin and a nearby avulsed bone fragment. Callous formation may ensue and appears as a calcific density in the region of injury. Awareness of the spectrum of radiographic presentations can help ensure correct diagnosis and minimize concern for alternative underlying diagnoses. This case report describes a 14-year-old boy with a chronic ischial tuberosity avulsion fracture which demonstrated an unusual presentation on radiographs and required MRI to confirm the diagnosis and rule out other potentially ominous pathology.

Abstract:Pelvic avulsion fractures are common in youth athletes; many of these injuries can be treated conservatively. This article reviews the etiology, presentation, and management of the more common pelvic avulsion fractures, including anterior superior iliac spine, anterior inferior iliac spine, ischial tuberosity, and iliac crest avulsions. Adolescent pelvic avulsion fractures rely on the amount of fracture displacement to guide treatment. Conservative management includes rest and avoiding use of the muscle(s) that attach to the avulsed fragment. Operative treatment is reserved for widely displaced fractures or symptomatic nonunions. With appropriate treatment, young athletes frequently return to their same level of sport.
[Treatment of pelvic avulsion fractures in children and adolescents]

Abstract:Introduction: Paediatric avulsion fractures of the pelvis and hip joint are rare injuries most commonly associated with a rapid muscle contraction of the respected apophysis, which is the weakest component of the growing skeleton. The chosen therapy option can be either conservative or surgical. Methods: We present a cohort of 69 patients treated for an apophyseal avulsion fracture of the anterior superior / inferior iliac spine, the lesser and greater trochanters iliac crest and the ischial tubercle. We compared the treatment outcomes, treatment time, need for immobilisation, and complications of conservative vs. surgical treatment. Results: The mean time to mobilisation was 24.5 days in the conservative and 8.5 days in the surgical group of patients. In the group of surgically treated patients, healing signs were observed as early as in week 6. As of month 4, no difference was observed between the groups in terms of the clinical and radiological outcomes. Conclusions: The treatment outcomes of both methods are comparable in the long term. Surgery is indicated in dislocations exceeding 1 cm. Faster recovery and return to sports activities is the main advantage of the surgical treatment method. Disadvantages include the necessity of general anaesthesia as well as the risk of infection and the need of the osteosynthetic material removal. The choice of the treatment method remains individual depending on the type of injury and habitual practice of the centre.Key words: avulsion fracture - child - pelvis - treatment - outcome.

Abstract:We report a case of a 12-year-old boy who sustained a displaced calcaneal apophysis fracture, which was analogous to a bony avulsion of the insertion of the Achilles tendon, secondary to an awkward landing while jumping at a trampoline park. Treatment with open reduction and internal fixation with cannulated screws provided a novel approach to fixation for this type of fracture in the pediatric population.

Abstract:Avulsion fractures of the anterior inferior iliac spine rarely occur in adolescent athletes during rectus femoris contractions or eccentric muscle lengthening while the growth plate is still open. Currently, there are no official guidelines in the literature on the treatment indications of this type of fracture or the type of surgical technique to be used. Nowadays, young and athletic patients desire a quick return to their previous activities, which makes surgical treatment a reasonable choice. Open reduction and internal fixation with an anterior approach are usually recommended when the avulsion fragment has more than 1.5–2 cm displacement on plain radiographs. However, ORIF is associated with a higher risk of heterotopic ossifications and increases the risk of damage to the LFCN. An endoscopic technique was designed to reduce these complications. This technical note describes a procedure of percutaneous fixation to AIIS through 3 endoscopic portals that could potentially minimize complications associated with an open surgical dissection, allowing anatomic reduction under direct visualization.

Abstract:Tibial tuberosity fractures are usually seen in the adolescent age group and are very rare in adults. Here we describe a case of knee injury following a road traffic accident. The patient had tibial tuberosity avulsion along with tibial plateau fracture. He was treated with open surgical reduction and internal fixation. His rehabilitation was quite successful, resulting in a good range of motion and functional outcome. This study aims to present this unusual case of tibial tuberosity avulsion fracture associated with tibial plateau fracture and its appropriate management.

Abstract:Background: Salter-Harris type VI physeal fracture is a rare injury. This case study aims to present a novel method for treating a rare entity of Salter-Harris type Salter-Harris VI physeal injury of the medial malleolus. Case presentation: A 6-year-old boy with Salter-Harris type VI physeal injury was successfully treated using the two-stage procedure. In the first stage, the patient was treated with intravenous antibiotics, a series of debridement and lavage followed by a skin graft that left a defect in the medial malleolus. In the second stage, an autogenous iliac crest apophyseal graft was transplanted to reconstruct the medial malleolus, and the ankle joint was stabilized by an external fixator. An additional anticipatory Langenskiold procedure was performed for the physeal bar resection. Although the complete radiological development of medial malleolus compared to the contralateral side was not evident at the last follow-up, the functional and cosmetic outcomes were satisfactory. Conclusion: The reconstruction of medial malleolus using an autologous iliac crest apophyseal graft and stabilization of the ankle joint with an external fixator is a novel reconstruction technique in treating Salter-Harris type VI physeal injury of the medial malleolus. This technique provides satisfactory functional and cosmetic outcomes in such a fracture pattern; however, a further clinical study using a larger sample size is warranted in order to find the definitive outcome of the technique.

Surgically Treated Ischial Tuberosity Avulsion Fractures in Adolescents: Risks and Outcomes of 3 Fixation Constructs

Abstract:Introduction: Avulsion fractures of the ischial tuberosity (AFIT) are uncommon injuries that sometimes require operative fixation with screws, suture anchors, or cortical suspensory buttons. This study reviewed a series of surgically managed AFITs at a single institution and compared outcomes among fixation strategies. We hypothesized there would be no difference in outcomes between fixation types. Methods: We conducted a retrospective review of all patients treated operatively at a single institution from 2010 to 2022 for AFIT identified by CPT code (27215). We collected patient demographics, injury characteristics, Revised Modified Oxford Bone Score, radiographic measures, fracture classification (type 1-lateral vs. type 2-complete), surgical fixation technique, postoperative complications (Modified Clavien-Dindo-Sink [M-CDS] Complication Classification), and time to return to sport (RTS). Descriptive statistics and univariate analyses were performed. Results: Study criteria identified 16 patients with surgically treated AFITs during the study period. Patients were predominantly male (88%) with a mean age of 14.8±0.8 years. Injured patients most commonly participated in soccer (38%), with the most common mechanism of injury being running/sprinting (50%). The avulsed fracture fragments were a mean of 42.6 mm in size, with an average maximal displacement of 21.1 mm and predominantly type 1-lateral (75%). Surgical constructs included: screws (4), suture anchors (5), cortical suspensory buttons (6), and combined (1, screw and suture anchor). Postoperative complications occurred in 8 patients (50%) including 7 type 1 and 1 type 3 M-CDS. No statistically significant difference was found between fixation types among the studied outcome variables; however, re-fracture (1 case [M-CDS type 3]) was only observed with a combined (screw and suture anchor) construct. Conclusions: This retrospective cohort study demonstrated that postoperative complications after ORIF for AFIT were not infrequent but were largely low in severity. Refracture occurred only with a combined screw and suture anchor construct. Consistent return to sport was achieved across all fixation types, with no significant difference in complication rate between constructs, although the available study sample limited robust analysis. This study is also, of note, one of the first to report outcomes from cortical suspensory buttons (CSB)-a relatively novel intervention for AFIT. Findings from this retrospective case series of multiple surgical techniques serve to bolster the limited literature base regarding the operative treatment of AFITs in adolescents.
Arthroscopic Treatment of Combined Anterior and Posterior Cruciate Ligament Avulsion Fracture: A Case Report

Abstract:Anterior cruciate ligament (ACL) avulsion fractures without any concomitant injuries are extremely rare in skeletally mature patients and sporadically reported in the literature. Such injuries are more likely to be associated with tibial plateau fractures, other knee ligament ruptures, distal femoral fractures, or knee dislocations. In this article, a case of a 55-year-old male, who suffered a combined displaced ACL-posterior cruciate ligament (PCL) avulsion fracture, without any related injuries, is reported. The fracture was treated all arthroscopically with both an internal fixation and a single-tunnel suture fixation. In this case, arthroscopic anatomic reduction and internal fixation of the displaced tibial eminence was achieved, and internal fixation was performed with two antegrade cannulated screws with additional single-tunnel trans-ACL suture fixation. Six months post-injury, the patient had no signs of post-traumatic osteoarthritis, restoration of range of motion, no pain, and no residual instability of the knee joint. The mechanism of this rare injury, the method of fixation that was used, and the rehabilitation protocol that was followed are demonstrated. Additionally, the clinical outcome in terms of certain parameters is evaluated. Finally, we highlight the importance of this method for additional strength of fixation and the role of early mobilization to establish satisfactory results.
Modified Pararectus Approach for Treatment of Atypical Acetabular Anterior Wall Fracture: A Case Report

Abstract:BACKGROUND:Acetabular anterior wall fracture with preservation of the pelvic brim is extremely rare. It is different from anterior wall fracture classified by Judet and Letournel. Few studies have reported cases treated by open reduction and internal fixation via the Smith-Petersen or iliofemoral approach.CASE SUMMARY:We report a 48-year-old Chinese woman who had difficulty moving her right hip from abduction and external rotation after falling from 3 m. Pelvic radiograph and three-dimensional reconstruction of computed tomography revealed acetabular anterior wall fractures combined with fractures of the anterior inferior iliac spine and the iliac wing but not involving the pelvic brim. First, the patient underwent interim management by closed reduction of the hip dislocation and skin traction for 6 d. Then, we used a modified pararectus approach for treatment to fix the acetabular fractures with a reconstruction plate and nonlocking T-shape plate. At the 9-mo follow-up, the patient could walk painlessly without necrosis of the femoral head or heterotopic ossification, and the X-rays and computed tomography scan reconstructions showed good bone union.CONCLUSION:The modified pararectus approach described here can facilitate exposure, reduction, and osteosynthesis for atypical acetabular fracture with less invasiveness.

Abstract:Abstract A 78-year-old woman complained of right heel pain when tripping during walking. Radiographs revealed an avulsion fracture of the calcaneal tuberosity. Because the bone fragment was displaced, a novel osteosynthesis using the side-locking loop suture (SLLS) and ring pins was performed. At 3 months after the surgery, non-contrast computed tomography revealed complete bone union. At 2 years postoperatively, she had no symptoms and dysfunctions. Here, we describe a case of avulsion fracture of the calcaneal tuberosity in a 79-year-old female who treated with a novel surgical technique using combination the SLLS technique and ring pins. This surgical technique may be a useful option in the treatment for avulsion fractures of the calcaneal tuberosity.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 10_suppl3, October 2024. Tibial avulsion fractures of the Posterior cruciate ligament (PCL) are common; however, methods of surgical treatment are insufficiently described. PCL is the main posterior stabilizer of the knee and limits posterior tibial translation relative to the femur . In addition, the PCL serves as a central axis controlling and imparting rotational stability to the knee. Isolated tear or avulsion of the PCL can be caused by falling onto a flexed knee or by striking the flexed tibia on the dashboard during a motor vehicle accident . Isolated PCL disruption most commonly occurs as avulsion at the point of tibial insertion, as opposed to its femoral origin or as a mid-substance tear. The tibial insertion of the PCL is also more consistent than the femoral insertion. The two PCL fibre bundles insert, without anatomic separation, into the centrally-located fovea or facet on the posterior aspect of the tibia, 1-1.5 cm distal to the joint line, with the posterior horn of the medial meniscus being the anterior-most extension . Chronic disability and early degenerative changes can develop as a result of delayed diagnosis.The posteromedial knee approach is a surgical technique for treating tibial avulsion fractures of the PCL. The technique involves reinsertion and fixation of the PCL bone fragment into its anatomical bed located on the posterior tibial surface. The posteromedial knee approach is reportedly sufficient for re-establishing ligament integrity and function; however, most surgeons are not familiar with this surgical approach because it is poorly-described within the published literature.
