Abstract:As doctors, we find it disturbing to read the paper "Burnout in physicians who are exposed to workplace violence" (January 2020).1 Workplace violence experienced by doctors has been documented both in developed and developing countries. Reports show that Chinese medical professionals are being injured, disabled, or even killed by patients or their family members.2,3 The killing of a doctor in Beijing in 2019 attracted national attention. China's top legislative body has approved the first fundamental and comprehensive law to protect health-care professionals. The bill will take effect on June 1, 2020. However, the law alone will not solve the problem. We need to treat it from the root and address the issues that have led to this health-care-related violence. A multi-faceted action must be launched to reduce violence against doctors in China urgently.

Abstract:The relationship between the doctor and the patient is the most important one of social relationships. But nowadays, the medical dispute become the synonym of this kind of relationship. Based on the analysis of the current condition of doctor-patient relationship, this article proposed that to construct harmonious doctor-patient relationship, we should enhance the mutual honest between the hospital and the patients and the managements in hospital, increase the quality of medical treatment, change the concept of service. The hospital should enhance the pro-dealing of dispute between the hospital and the patients and try to solve it at its beginning, simultaneously, whole society should make jointly effort to solve the problem and establish the long-term system of mediate the dispute between the doctor and the patient.

Abstract:Abstract Doctor–patient trust is not strong in China, but studies examining this factor remain insufficient. The present study aimed to explore the effect of doctor–patient communication, medical service quality, and service satisfaction on patient trust in doctors. Five hundred sixty-four patients with tuberculosis participated in this cross-sectional study in Dalian, China. They completed questionnaires assessing socio-demographic characteristics, doctor–patient communication, medical service quality, service satisfaction and patient trust in medical staff. A structural equation model was applied to examine the hypotheses, and all the study hypotheses were supported: (1) doctor–patient communication, medical service quality and service satisfaction were positively associated with building doctor–patient trust; (2) service quality positively mediated the relationship between doctor–patient communication and trust; (3) medical service satisfaction positively mediated the relationship between doctor–patient communication and trust; (4) medical service satisfaction positively mediated the relationship between medical service quality and doctor–patient trust; and (5) medical service quality and service satisfaction were the positively sequential mediators between communication and doctor–patient trust. Based on these findings, improvements in doctor–patient communication, medical service quality, and service satisfaction are the important issues contributing to the rebuilding of doctor–patient trust in medical service delivery.

Abstract:Objective: To identify the changing of health policy decisions and Doctor-Patient relationship in different periods in China during 67 years (1949-2016).Methods: We followed a systematic approach to analyze the development stage of China's healthcare reform and Doctor-Patient relationship over time. Based on the extant literature and statistical data, the current five issues in Chinese health policy and three aspects of Doctor Patient Relationships are classified. The reasons for the current situation and recommendations for future policy are discussed.Results: There are four main problems of Chinese Health Policy: funding shortage, excessive market-oriented operation, health insurance limited reimbursement amount, and media coverage a lot of non-neutral health events. In different historical periods, these problems are not prominent, but have become increasingly serious in recent years. On the other hand, three participants contributed to the growing tense relationship between doctors and patients: doctors (hospitals), patients and governments.Conclusions: In formulating future health care policy, the government should change: increase investment in the medical field, change the income structure of doctors and hospital, propagate medical news in an objective way, and increase the amount and scope of medical insurance repayment. However, the recommendations focus on the physician's side, ranging from school education to hospital practices, and legal to ethical systems. The worsening relationship between patients and physicians is the result of interaction between both sides. The Correspondence might have been more comprehensive if health and scientific education for the public had been discussed—ie, the patient's side. First, China's economic reforms have generated unprecedented economic growth over the past three decades, whereas medical knowledge and health literacy of the public have not improved at the same rate. Abstract:Abstract Background Patient–physician mistrust has become deeply embedded in medical clinics within a wide variety of settings, including many in China. The purpose of this research was to develop a series of actionable policy recommendations to rebuild patient–physician trust in China. Methods Our interdisciplinary group included experts in medicine, public health, philosophy, ethics, law, regulation, China studies, anthropology, sociology, and communications. Recommendations were identified by team members and presented at a two-day workshop at the Harvard Center, Shanghai, China. The group divided into three teams (medical education, ethics and law, and healthcare systems) in order to revise and finalise the recommendations. Findings We identified a total of 18 recommendations focused on medical schools, ethical guidance, legal systems, and health systems to rebuild patient-physician trust. Medical education recommendations included a requirement for medical humanities as a core component, promotion of experiential learning and community–medical school partnerships, and improvement of evaluation of medical humanities education. Ethical and legal recommendations included encouragement of more transparency in doctor practices and the healthcare system, creating laws to promote mandatory medical error reporting, and acceleration of the development of neutral procedures for recording and resolving medical disputes. Healthcare systems recommendations included promoting healthcare systems that facilitate and acknowledge caregiving, transitioning from red packets (gifts to physicians) and towards higher physician salaries, strengthening primary healthcare systems, and establishment of non-punitive systems for error reporting in hospitals. Interpretation Several educational, legal, ethical, and healthcare system reforms to rebuild patient–physician trust are feasible. Our recommendations go beyond the healthcare sector alone, suggesting that policy responses within education, legal, and ethical norms are also critical. The presence of mistrust should not be misconstrued as an errant medical system, but rather as an opportunity and a responsibility to rebuild patient–physician trust. What Influences Doctors’ Perceptions of The Doctor–Patient Relationship? — A National Cross-Sectional Study in China

Abstract:Abstract Background: Tense doctor–patient relationships have become a great challenge for the Chinese government due to their high incidence rate and negative social impact. This study aimed at exploring the factors influencing doctors’ perceptions of the doctor–patient relationship.Methods: We conducted a national cross-sectional study in 136 tertiary grade-A hospitals in China. A total of 19,774 doctors reported their perceptions of the doctor–patient relationship over the last two years, as well as sociodemographic factors, work experience, and hospital security measures. A logistic-regression model was used to identify factors significantly associated with doctors’ perceptions of the doctor–patient relationship.Results: Doctors’ perceptions of the doctor–patient relationship were relatively negative. Region, hospital type, gender, age, and income influenced these perceptions. With regard to doctors' work experience, those with excessive workloads, those who had experienced medical disputes, and those who did not have enough communication time with patients tended to report more-negative perceptions of the doctor–patient relationship. In terms of hospital security measures, doctors working in hospitals with effective precautions, in-place security staff, and medical-liability insurance purchased for medical staff tended to report more-positive perceptions of this relationship.Conclusion: Doctors’ work experiences and hospital security measures both played essential roles in improving doctors' perceptions of the doctor–patient relationship. On the part of doctors, improving their communication skills and providing adequate communication were beneficial to their perceived relationships with patients. On the part of hospitals, reducing doctors’ workloads, preventing medical disputes, and increasing job security and organizational support are potential ways to improve doctors' perceptions of these relationships.Practice implications：This study provided evidence and advice for Chinese government, as well as Chinese hospitals, to improve doctors’ perceptions of the doctor-patient relationship by measures of enhancing doctors’ work experiences and hospital security.
"Old friend and powerful cadre": Doctor-patient relationships and multi-dimensional therapeutic landscapes in China's primary hospitals

Abstract:A positive doctor-patient relationship is believed to play a key role in the healing process in clinics. While challenges to the doctor-patient relationship are a global concern, complex social contexts which introduce familial collectivism and totalitarian bureaucracy to maintain a doctor's authority have complicated doctor-patient relationships in China. This study delineates a multi-dimensional therapeutic landscape of hospitals in China, focusing on the doctor-patient relationship performances used to improve patients' healing experiences. Based on fieldwork in two primary hospitals in Eastern China, we find that primary hospitals in China are not only professional spaces, but hybrids of professional and non-professional spaces. In these spaces, both professional and other discourses in various forms of social-environmental engagement affect therapeutic experiences. Varying time and space in hospitals allow doctors to construct multi-dimensional therapeutic landscapes vis-a-vis patients to secure patients' compliance with their recommendations, and thus improve health outcomes. We argue that these dimensions may also cause negative therapeutic experience such as unnecessary health care. This study contributes to the literature on therapeutic landscapes of health care by providing a critical view on the construction of multi-dimensional therapeutic hospital landscapes. Furthermore, it links the critical health geographies literature with China's broader social context to explicate the cultural and social transformation of health care spaces in contemporary China. Findings from this study inform both theoretical and empirical debates regarding therapeutic landscapes of health care by embedding the professional spaces of health care into broader geographical discourses. This calls for health professionals to reflect on ethical concerns in multi-dimensional health care landscapes.

Chinese Minority Perceives the Doctor-Patient Relationship Differently: A Cultural and Economic Interpretation

Abstract:In China, doctor-patient relationships (D-P relationships) are cited frequently and attracted international attention. This study assesses whether the D-P relationship experienced by the Chinese is associated with ethnicity, hospital hierarchies, and socioeconomic factors. In a national cross-sectional survey, multi-stage random sampling was adopted to assess regional and socio-economic differences between year 2016 and year 2017. Each area surveyed consisted of about 250 randomly chosen households, and valid results were obtained from 4,173 adults aged 16-99. When provided a choice of eight types of D-P relationship, for doctors in primary care institutions, 63.8% of ethnic minorities indicated having a friend-type relationship with their physicians, with 23.3% having a trading/reciprocal relationship. Han Chinese, however, predicts the opposite relationship between doctors from different hierarchy and the types of D-P relationship. For physicians working in hospitals, this difference in relationship was more pronounced, where 52.9% of ethnic minorities indicated having a friend-type relationship with their physicians, and 30.1% indicated the presence of a trading/reciprocal relationship. For Han Chinese, however, 53.3% indicated having a reciprocal relationship with their doctor. Overall, the prevalence of friendly D-P relationships was correlated with ethnic minorities, lower levels of education, and lower incomes. Ethnic minorities are most likely to perceive their physicians as friends, while Han Chinese are more likely to perceive a trading relationship with their physicians. The primary contribution of this research is the finding that D-P relationships differ for Han Chinese and other ethnic minorities.

Abstract:A strong doctor-patient relationship (DPR) is crucial to the effectiveness of treatment. It is imperative to maintain a good DPR during treatment. During 2019, Coronavirus Disease 2019 (COVID-19) brought new challenges to already difficult doctor-patient relationships. This paper summarized the current state of the DPR, compared the changes between China and other countries after the outbreak of COVID-19, and listed the solutions proposed by various countries. Finally, the author suggested some solutions in order to improve the DPR according to China's own circumstances.

Quantitative Imaging in Medicine and Surgery

Abstract:Following tremendous economic progress, society in China is also undergoing fundamental changes, as is the healthcare system. Currently the training of Chinese young doctors and their future work placement are all undergoing re-structuring. We compiled some thoughts and opinions on the topic of 'should clinicians in China engage in research?', and publish them as a special report in this issue of Quantitative Imaging in Medicine and Surgery (QIMS). The contributors included some editorial members of this journal, and a few personal friends. Besides a few minor linguistic corrections, opinions from the contributors have not been edited, as we want authors' to write their own independent views. However, it is possible there is a selection bias of the contributors of this paper; more likely those who are interested in the medical research are selected and therefore the views of the contributors may not be generalizable. To compare the structure and funding of China with other countries, authors from UK, The Netherlands, France, and USA are also invited.

" As the sentence stated,it will be the important goal of medical reform in 2014 when the new round of reform had been continued for entire five years. However,due to lack of talent people and supportive skills,the medical and health services had been stretched to the limit,which had negative influence on achieving that goal. Elder rural doc-tor,unstable condition of doctor worked in township hospital and imperfection mechanisms in terms of training system and motiva-tion,all of these elements makes relatively adverse impact on building doctor - patient relationship. For changing these situations, in return,it is not only to collect appropriate people,but also to construct some sort of effective mechanisms for training system and motivation that is beneficial to implementation of high level of medical skills. Finally,all above actions serve one purpose -reshaping the trust between doctor and patient.

Abstract:The construction of harmonious doctor-patient relationship is an important project in social medicine .The influen-cing factors about doctor-patient relationship are analyzed from three aspects which are responsibility ethics , bottom-line ethics and clin-ic ethics.Based on the discussion , certain countermeasures are proposed to reconstruct harmonious doctor -patient relationship .

Doctor–patient trust involves the subjects of both sides, including the direction of patients' trust in physicians (patients' perception) and physicians' trust in patients (physicians' perception). This study aims to assess the status quo and explore suggestions for improving trust between doctors and patients. Patients and Methods: In May 2018, we collected data from six representative hospitals in Beijing, China, including 610 questionnaires (310 physicians and 300 patients). Participants were Chinese-speaking, age 14 and older, who expressed their opinions clearly. Kruskal–Wallis H -test and Wilcoxon rank sum test were used to analyze the difference in the trust scores of the responses. Results: Based on the doctor and patient characteristics, the data show that inpatients are trusted more than outpatients, and patients who often visit the clinic are more doubtful of doctors. The family trust is the highest, and social trust is the lowest. In general, the degree of trust between doctors and patients is good, but the score of physicians (Mean=3.87; SD=0.79) is lower than that of patients (Mean=4.05; SD=0.76). Physicians' evaluation of the degree of trust tends to be more negative than patients'. Conclusion: In spite of recent negative press reports, there remains a high degree of trust between patients and doctors in Beijing. Despite this, one should not be complacent. We need to explore the root cause of the trust between doctors and patients from a deeper perspective to promote better medical services to meet the health needs of patients. Keywords: China, doctor–patient relationship, trust, public hospitals Since 1978 China has experienced a period with several major health care system reforms. 1,2 This has had a profound effect on the doctor-patient relationship in China. 3 Public hospitals are the backbone of the Medical institutions in China, but they face the pressure of insufficient funds and demands of self-reliance. 4 Although medical treatment has improved to ensure a more comprehensive medical service level, the health demand and supply gap still exists. 5 High drug prices, difficult consultations, and irrational drug usage are obstacles for people seeking consultations from a doctor in recent years. 6 People's demand for medical services has increased significantly. The imbalance between supply and demand because of existing high drug costs reports, and excessive medical treatments has brought pressure on the medical system. 7 It wastes limited medical resources and causes tensions between doctors and patients and affects the people's trust in the whole medical system. 8 For nearly ten years, the incidence of medical disputes in China has been rising in a straight line. 9 The intensification of doctor-patient conflicts has seriously affected the normal order of diagnosis and treatment in hospitals. 10 This has led to introducing a new word among medical professionals, called "medical harassers" (Yi nao). 7 It is the conflicts between patients and doctors and the people in charge of organizing the trouble in order to obtain huge profits. In the doctor-patient conflicts, the patients obviously show indignation and hatred; Patients often suspect the actions of doctors and their motives and intentions. This vague uncertainty greatly weakens doctor-patient trust and leads to confrontation between doctors and patients. 9 Mass media sometimes has not played a positive role in harmonizing the relationship. 11 And the distorted coverage of such doctors in the Chinese media has exacerbated tensions between doctors and patients. 12 The report overemphasized the tension between doctors and patients and overplayed the faults between doctors and patients, which increased the hostility of patients towards their doctors. 13 Doctors often take a neutral attitude, frequently change treatment plans, refer patients, and consult with many experts.
