Effect of intervention via mid-long term follow-up on visual recovery of patients with diabetic retinopathy

Abstract:AIM: To investigate the effect of follow up and mid-long term intervention on the visual recovery of patients with diabetic retinopathy,and find out a method to protect their vision. METHODS: Altogether 87 patients (174 eyes,38 males and 49 females) with diabetic retinopathy treated in the Department of Ophthalmology,Third Hospital affiliated to the Xinxiang Medical College were followed up between January 2000 and June 2005,which was started after the first treatment. The mid-long intervention method: The patients were given guidance on diet control and drug therapy by means of strongly demanding for periodic return visit,connection by telephone or letters and periodic follow up of partial patients,and the blood glucose level of patients was measured every one or two weeks to keep the fasting glucose < 6.7 mmol/L; meanwhile,they were given long term drug treatment,and those appeared fundus pathologic change would given fundus photocoagulation in time. The SPSS10 for Windows software was used for single analysis and Cox proportional risk model multifactor analysis. RESULTS: Totally 87 cases (174 eyes) were given follow-up,including 120 eyes with proliferating diabetic retinopathy,72 eyes with non-proliferating diabetic retinopathy and 157 eyes given fundus photocoagulation. All the patients were followed up for 3 months after the first treatment,it was 79,65,48,45 and 36 cases who were followed up for 6,12,36 and 48 months,respectively after the first treatment. The longest follow-up lasted 50 months. ①At the 3rd month after the first treatment,the vision of patients was improved (χ21-2=10.79,P < 0.05),moreover,the treatment delayed the continuous damage of the vision in the 1st and 3rd years after first treatment (χ22-3=2.99,χ22-4=0.91,χ22-5=5.41,χ22-6 =4.18,P > 0.05). ②At the 48th year after the first treatment,although the vision was worse than that at the 3rd year,it had no significant difference than that before treatment (χ22-7=9.59,P < 0.05; χ21-7=8.86,P > 0.05); the data analysis also revealed that there was no obvious difference in the vision of patients with diabetic retinopathy between the 6th,12th,24th and 36th months and before treatment (χ21-3=2.98,χ21-4=8.51,χ21-5=3.78,χ21-6 =5.67,P > 0.05). ③There were 7 factors,which could significantly affect visual prognosis of patients,including diabetes course,successive high level of fasting glucose,hypertension,diabetic nephropathy existing or not,periodically visiting to doctors of endocrinology or not,periodically return visiting to the department of ophthalmology or not,receiving fundus photocoagulation or not (P < 0.05),and the factors of periodically visiting to doctors of endocrinology or not,diabetes course,successive high level of fasting glucose,hypertension and periodically return visiting to the department of ophthalmology or not were the important dependent ones related to the recovery of vision. CONCLUSION: Besides the treatment for the diabetic retinopathy pathologic change,the mid-long term intervention to the patients with diabetic retinopathy by the coordination of physicians of ophthalmology and internal medicine can greatly decrease and relieve the complication to control the continuous damage of vision and improve the life quality of the patients.
