Abstract:Purpose: To study the impact of diabetic retinopathy (DR) on the quality of life (QoL) of Indian patients with diabetes. Methods: This cross-sectional tertiary health care institution-based study involved 250 patients of DR. They were interviewed using four questionnaires, namely, the General Health Questionnaire (GHQ), Final Quality of Life Instrument for Indian Diabetic Patients (QoLID) questionnaire for diabetes and questionnaire modified for DR, retinopathy dependent quality of life (RetDQoL), and coping strategy checklist (CSCL). Results: The mean GHQ score was 1.12, indicating the absence of psychological morbidity. Mean QoLID score for financial worries and treatment satisfaction scores were 15 each for DR compared with 17 and 16, respectively, for diabetes mellitus (DM). The mean RetDQoL score was −27.94 (±2.14), showing the negative impact of DR on QoL. The mean CSCL score was 1.22 (±0.14), indicating infrequent use of coping strategies. Scores achieved by all four questionnaires correlated to each other. On subgroup analysis, proliferative DR (PDR) patients had a significantly higher GHQ score, lower treatment satisfaction, and more financial worries, with a poorer QoL than nonproliferative DR (NPDR). The severity of the disease had a negative impact on the QoL. The treatment satisfaction and psychological impact on the patients undergoing all types of eye treatments were comparable. Conclusion: DR has a significantly detrimental impact on the QoL, which increases with increasing severity of the disease.

Abstract:Importance The association between diabetic retinopathy (DR) and quality of life (QoL) has not been thoroughly investigated. Objective To investigate the association between DR and both vision-related QoL (VRQoL) and general health-related QoL (HRQoL). Data Sources MEDLINE, EBSCO, Embase, and Web of Science were searched from their inception to April 2022. Study Selection Studies included adults with DR and a measure of QoL. Data Extraction and Synthesis Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed. Two assumption-free meta-analyses were conducted. Analysis 1 included studies with participants without DR as the referent group to which QoL scores of participants with DR, grouped according to DR severity, were compared. Analysis 2 included all studies with participants with DR and a measure of QoL. QoL scores were pooled within categories of DR severity, and comparisons were made between these categories. Main Outcome and Measures QoL measured using HRQoL and VRQoL scales. Results A total of 93 articles were included: 79 in the meta-analyses and 14 in the narrative results. VRQoL was recorded in 54 studies, HRQoL in 26, and both in 13 studies. The most commonly used scales were the National Eye Institute 25-item Visual Function Questionnaire (VFQ-25) (n = 49) for VRQoL and the Short Form (SF) Health Survey (n = 18) for HRQoL. Thirty-five studies reported VFQ-25 composite scores. Analysis 1 consisted of 8 studies including 1138 participants with DR and 347 participants without DR. Compared with participants without DR, the composite VFQ-25 score was 3.8 (95% CI, 1.0-6.7) points lower in those with non–vision-threatening DR (NVTDR), 12.5 (95% CI, 8.5-16.5) lower in those with any DR, and 25.1 (95% CI, 22.8-27.2) lower in VTDR ( P &lt; .001 for trend). Analysis 2 consisted of 35 studies including 6351 participants with DR. The pooled mean VFQ-25 composite score was 91.8 (95% CI, 91.0-92.7) for participants with NVTDR, 77.6 (95% CI, 76.9-78.3) for any DR, and 73.2 (95% CI, 72.6-73.7) for VTDR ( P &lt; .001 for trend). HRQoL scores had weak or no associations with NVTDR and strong associations with VTDR. Conclusions and Relevance This study found that VRQoL declined with the presence and severity of DR. Interventions to reduce progression of DR at both early and more advanced stages could improve VRQoL.

Abstract:Diabetes has detrimental effects on many organs, including the kidneys, heart, and the central nervous system, with ophthalmic involvement and Diabetic Retinopathy (DR), specifically, being among the most severe and prominent consequences. Diabetic Retinopathy and especially advanced stages of the disease, have a crucial impact on patients’ quality of life and emotional status. In this context, emotional imbalance, psychological side effects and comorbidities, like anxiety disorders, could emerge, deteriorating the patients’ condition further. A number of questionnaires can be employed in the evaluation of the potential impact of Diabetic Retinopathy on patients’ quality of life, including the Beck Anxiety Inventory (BAI) and The National Eye Institute Visual Function Questionnaire-25 (NEI VFQ-25). Purpose: The purpose of this study was to evaluate the association of Diabetic Retinopathy (DR) and diabetic macular edema with vision-related quality of life, as well as the potential association between the disease’s severity, emotional status of patients and the manifestation of anxiety and psychological features. Results: Patients with fundoscopic findings had significantly lower scores in all VFQ-25 subscales, indicating worse quality of life in comparison to patients without DR. Severity of DR, greater levels of anxiety, daily sitting time, unemployment and lower education level, were all found to be significantly, negatively associated with a worse quality of life. Regarding emotional status, more years of suffering from diabetes, treatment with insulin and the hours being idle per day were associated with an increased burden of anxiety. In addition, the presence of a concomitant disease, findings in fundoscopy, diabetic macular edema and treatment with anti-VEFG injections, as well as the number of doses, were significantly associated with greater anxiety. Multivariate analysis showed that having Severe Non-Proliferative Diabetic Retinopathy or having Proliferative Diabetic Retinopathy and receiving insulin therapy (alone or in combination with another treatment), were significantly associated with higher levels of anxiety. Conclusion: The well-established impact of DR on the patients’ well-being, quality of life and emotional status render DR and CME prevention, stabilization or delaying progression as a necessity in order to protect patients from developing psychiatric symptoms. On the other hand, the speculated bi-directional association between emotional problems and DR progression highlights the importance of acknowledging and dealing with psychological issues with the aim of delaying DR progression.

Abstract:Abstract Background Diabetic retinopathy (DR) is a major cause of blindness worldwide, threatening the vision of approximately 10% of patients with diabetes. Many studies have demonstrated that intensive control of the risk factors for DR is essential to reduce the onset and progression of DR. Currently, the relationship between lipid profile and DR is still unclear, especially in Saudi Arabia. We aimed to assess the correlation between both the development and severity of DR with lipid profile and blood pressure among the diabetic patients at the King Abdul-Aziz University hospital in Jeddah, Saudi Arabia. Methods This was a retrospective record review study of 298 diabetic patients diagnosed with DR. Retinal findings were correlated to serum lipids levels using univariate, bivariate, and multivariate analysis. Results This study included 298 participants with DR. Triglyceride levels, systolic blood pressure, low-density lipoprotein cholesterol levels, and presence of macular edema were significantly associated with DR progression (P = 0.012, P = 0.001, P = 0.002). Other parameters, including total cholesterol, high-density lipoprotein cholesterol, HbA1C, body mass index, age, were not significantly associated with DR. Conclusion Elevation in serum triglyceride levels and systolic blood pressure showed a statically significant association with diabetic retinopathy. Controlling these factors may help preventing progression and occurrence of diabetic retinopathy among diabetic patients.

Abstract:Introduction & Objectives: Diabetic retinopathy(DR) is associated with significant visual impairment and quality of life in people living with diabetes, however, studies on the prevalence and risk factors from Saudi Arabia have been inconsistent, hence the aim of this was to define the prevalence, risk factors and predictors of DR in a large sample of subjects with type 1 diabetes (T1DM) and T2DM. Methods: This was a cross-sectional study that included subjects with both T2DM and T1DM (if duration > 5 years) who attended the diabetes clinics at King Fahad Medical City in Riyadh between January 2022 and December 2022. Data are presented as Mean ± SD. Results: The study sample included 1399 subjects of whom 566(40.5%) had T1DM and 833 (59.5 %) subjects had T2DM. Mean Hba1c for T1DM subjects was 8.8 ± 3.0 %, mean diabetes duration 10.2 ±5.6 years, and mean Time in Range (TIR) was 42.7 ± 18 % (N=354 subjects). Mean HbA1c for T2DM subjects was 7.8 ± 1.5 %, mean diabetes duration was 14.1 ± 8.8 years and mean TIR was 52.7 ± 25 (N=139). The prevalence of DR in the overall study population was 20.0 % (280/1399) with the majority ( 86.5 %) in the Mild and Moderate Non-Proliferative DR stages. The prevalence of DR in T1DM & T2DM subjects was 12.7 % and 24.8 %, respectively. Subjects with T1DM & DR were significantly older and had longer diabetes duration while subjects with T2DM & DR were older, had longer diabetes duration, higher Systolic BP, higher urinary albumin/Creatinine ratio( UACR) and lower estimated GFR. Logistic regression showed that in T1DM subjects, longer diabetes duration, lower TIR and higher glucose variability were independently associated while, longer diabetes duration and higher UACR were independently associated with DR in T2DM subjects. Conclusion: In a large sample of Saudi subjects with diabetes, the prevalence of DR was 20 %. Abstract:Background: The prevalence, severity, and quality of life (QoL) impact of diabetic retinopathy (DR) among African Americans (AA) with end-stage-kidney disease (ESKD) receiving dialysis are unknown. Methods: We conducted a prospective cross-sectional study in 93 AA adults with type 1 (n=9) and type 2 (n=84) diabetes with ESKD on hemodialysis. The diagnosis of DR was based on a review of medical records and/or having a positive photograph with a portable hand-held device (Optomed Aurora retinal camera) reviewed by both artificial intelligence software (AEYE Health’s retinal screening system) and a retinal specialist. Visual acuity was assessed with the Snellen Chart. Associations with QoL (ascertained using kidney disease QoL and depression questionnaires), physical disability (ascertained using visual impairment questionnaire), and social determinants of health (SDoH) including personal and household income, marital status, health insurance, education, food insecurity, or employment status assessed by standardized questionnaires. Results: The overall prevalence of DR was 75%, with 33% of patients having mild, 9.6% moderate and 57.4% severe DR. A total of 43% had normal visual acuity, 45% had moderate and 12% severe visual impairment. We found high burden of disease, multiple SDoH challenges, and low QoL among all patients, with most unemployed or disabled, living below poverty line, mainly receiving public health insurance, reporting poor general health, and substantial time spent dealing with kidney disease interfered with daily and family activities. However, the presence of DR had no significant impact on physical health and QoL compared to subjects without DR. Conclusion: DR is present in 75% of AA patients with diabetes and ESKD on hemodialysis. ESKD has a significant burden on general health and QoL; however, DR has a minor additional impact on the overall physical health and QoL in people with ESKD and diabetes. Impact of vision correction on the visual impairment status and quality of life score in patients with type II diabetes mellitus

Abstract:Background: Visual impairment (VI) in patients with diabetes mellitus (DM) worsens as the disease progresses. Therefore, quality of life (QOL) may also be affected. Furthermore, in the absence of macular involvement, some patients may benefit from visual intervention. However, not many Malaysians with known DM had their eyes screened or used correctable spectacles. Consequently, the QOL and VI status of patients with DM in Malaysia remain unclear. This study was aimed at determining the impact of optometric intervention on the QOL and VI status of adults with type II DM. Methods: This was a quasi-self-controlled, experimental study involving adults with known type II DM. We conducted face-to-face interviews using the low vision quality-of-life questionnaire (LVQOL). The habitual visual acuity (VA) of all participants was recorded. All participants underwent fundus photography to grade diabetic retinopathy (DR) in both eyes. Correctable VA was determined following subjective refraction when the best-corrected distance visual acuity (BCDVA) was 6 /9 and better. After a 3-week adaptation to the prescribed refractive error correction, LVQOL was repeated via a phone interview. Results: A total of 47 participants with type II DM, including 16 (34%) men and 31 (66%) women, with a mean (standard deviation [SD]) age of 49.0 (7.9) years were recruited. The age range was 32 – 59 years, and the baseline mean (SD) habitual binocular VA was 0.52 (0.31) decimal. Only 15% (n = 7 patients) of the participants had their vision tested and wore glasses; however, some were uncomfortable with the current corrections. All patients had undercorrected or uncorrected refractive errors, namely, hyperopic astigmatism (47%), myopic astigmatism (38%), hyperopia (6%), myopia (4%), and antimetropia (4%). Among the 47 participants, 89% (n = 42) had uncorrected presbyopia. The mean (SD) LVQOL score at baseline was 91.9 (17.3), which improved significantly with visual intervention to 122.8 (3.2) (P < 0.05). Refractive error corrections significantly improved the VI status (P < 0.05), as all participants achieved a BCDVA of 6 / 9 and better. Conclusions: Our findings indicate that optometric intervention is effective in improving the LVQOL and VI status of adults with type II DM. Further clinical optometric studies on type II DM with DR with a longer follow-up should be carried out to understand the clinical characteristics of this cohort and the impact of meticulous refractions on QOL in providing better services in the future.

Abstract:Background: DR is one of the leading causes of blindness worldwide and patients with this sight threatening disease are expected to increase as dietary habits are changing especially in developing nations. Objective: The epidemiology and the risk factors of diabetic retinopathy among diabetic retinopathy patients attending the outpatient clinic of Mansoura ophthalmic center through one year.Patients and methods: This cross-sectional descriptive study included 200patients with diabetic retinopathy recruited from Mansoura ophthalmic center, Mansoura University Hospitals, Egypt. After obtaining a written informed consent, the cases were subjected to full history taking and full ophthalmological examination. Spectral domain OCT 1000was used to obtain retinal images. Laboratory investigations were conducted for all the cases included HBA1C, lipid profile and serum creatinineResults: There was a statistically significant difference in the mean macular thickness between the eyes according to the findings of fundus examination. The highest macular thickness was reported with the eyes with sever non proliferative diabetic retinopathy (NPDR). The mean macular thickness was statistically significantly higher in the cases with hypertension and cases with hepatitis. There was no statistically significant difference in the macular thickness according to sex, smoking or family history of DM. Linear regression analysis confirmed that cholesterol level and best corrected visual acuity (BCVA) had statistically significant predictive ability for macular thickness.Conclusion: Retinopathy is a common complication of diabetes. Screening for DR at the initial stage of diabetes may prevent disability from blindness caused by DR.

Nutrition quality of life associated with affective functioning among Omani patients with type 2 diabetes from primary health care

Abstract:Abstract Diabetes requires challenging lifelong dietary management, affects quality of life and heightens the impact of affective functioning. The aim of this study was to investigate the relationship between Nutrition Quality of Life (NQOL) and affective functioning in a sample of Omani patients with type 2 diabetes. A sample of 149 adults with type 2 diabetes was conveniently recruited from seven Primary Health Centers (PHCs) during follow-up visits. Data were gathered via face-to-face interviews. Pearson correlation and χ 2 test of independence were applied to examine associations at P < 0⋅05. Most patients had poor glycemic control (71⋅1 %), BMI ≥ 25 kg/m 2 (85⋅2 %) and central obesity (75⋅8 %), and moderate (54⋅4 %) and poor (32⋅9 %) level of NQOL. Based on the Hospital Anxiety and Depression Scale (HADS), 16⋅1 and 23⋅5 % of the sample endorsed the presence of anxiety and depression, respectively. A significant negative correlation was found between NQOL and HADS ( r −0⋅590, P = 0⋅000), anxiety ( r −0⋅597, P = 0⋅000) and depression ( r −0⋅435, P = 0⋅000). There was a significant association between NQOL and HADS, χ 2 (2) = 38⋅21, P < 0⋅01 that was large, Cramer's V = 0⋅51. Also, there were significant associations ( P < 0⋅01) between NQOL and HADS when controlling for HbA1c, BMI, waist circumference and HMNT that were moderately to largely strong, Cramer's V = 0⋅43–0⋅55. There is an evident association between NQOL and affective functioning in adults with type 2 diabetes. Further research is recommended to confirm these relationships and to guide intervention programmes at PHCs to help improve the general quality of life of such patients.

Abstract:This descriptive evaluative study aimed to assess the knowledge of diabetic patients about preventive measures for diabetic retinopathy in the Diabetic and Endocrine Centre in Al-Nasiriya City, covering the period from 9th March 2022 to 1st December 2022. The study employed a constructed questionnaire based on an extensive literature review and guidance from the supervisor. The questionnaire was divided into three parts, covering socio-demographic characteristics, clinical information, and an assessment of diabetic patients' knowledge related to retinopathy prevention. Data was collected using interviews, and both descriptive and inferential statistical analyses were conducted. The results revealed that the majority of the study sample were males (68.3%) and most patients were aged 50 years and above (41.7%). Regarding education, the majority were high school or high institute graduates (25.0%), and a significant proportion were government employees (46.7%). The duration of diabetes for most patients ranged from 6 to 10 years (38.3%), and approximately 30.0% did not have any diabetes-related complications. Visual impairment due to diabetic eye problems was reported by 48.3% of the patients. Monitoring blood glucose weekly was the most common practice among the study participants (58.3%), and the majority (73.3%) did not have a history of hypertension. Furthermore, 55.0% of the patients had a history of dyslipidaemia. The study revealed a mean knowledge score of 0.238 (SD=0.0672) out of a maximum possible score of 0.46, indicating a poor to intermediate overall level of knowledge regarding preventive measures for diabetic retinopathy among diabetic patients. However, no significant relationships were found between patients' knowledge and their age, gender, education, diabetes duration, history of hypertension, or history of dyslipidemia conclusion, the study highlighted those diabetic patients had poor knowledge regarding diabetic retinopathy, despite being aware of its impact on eye health. As a recommendation, the study suggests implementing an intensive and comprehensive national-level educational program targeting a large population of diabetes mellitus patients to improve their knowledge about preventive measures for diabetic retinopathy. Health professionals, mass media, and illustrated posters in diabetic clinics and hospitals could play an effective role in this educational initiative.
Prevalence and Associated Factors of Diabetic Retinopathy among Type 2 Diabetes Mellitus Patients in Brunei Darussalam: A Cross-sectional Study

Abstract:Purpose: To determine the prevalence of diabetic retinopathy (DR) and the factors associated with retinopathy among type 2 diabetes mellitus (DM) patients in Brunei Darussalam.Methods: Cross-sectional study of all type 2 DM patients who attended diabetic eye screening over a 3-month period at one of four government hospitals. We assessed association between DR with the following variables: age, sex, glycated hemoglobin (HbA1c), duration of DM, hypertension, hyperlipidemia, and microalbuminuria.Results: There were 341 patients (female, 58.9%; mean age, 55.3 ± 11.9 years) with a mean duration of DM of 9.4 ± 7.4 years and mean serum HbA1c of 8.4% ± 1.9%. The overall prevalence of any DR was 22.6% (95% confidence interval, 18.8–27.1) with prevalence rates of 4.1% (95% confidence interval, 2.1–6.4) for proliferative DR and 9.7% (95% confidence interval, 6.8–13.2) for vision-threatening DR. Multivariate analysis showed that DR was significantly associated with certain age groups (reduced in older age groups), longer duration of DM (11 years or more), poor control (HbA1c >9.0%) and presence of any microalbuminuria.Conclusions: DR affects one in five patients with DM in Brunei Darussalam, comparable to rates reported for other Asian populations. It is especially worrying that one in ten patients with DM had vision-threatening DR. DR was significantly associated with longer duration of DM, poor control and presence of microalbuminuria but reduced in older age groups. It is important to advocate good control right from the time of diagnosis of DM and institute timely and effective management of retinopathy. DR was significantly associated with longer duration of DM, poor control of diabetes, and presence of microalbuminuria but reduced in older age groups.

Data regarding medical history, ophthalmic history, stage of DR, presence of DME, and the ophthalmic procedures and operations were collected. The total DR-related cost was measured as a direct medical cost for the outpatient and inpatient services. Results: Two hundred and twenty-nine patients were included in the study. Only 49.7% of the patients presented without DR, and 21% presented with diabetic macular edema (DME) unilaterally or bilaterally. The DR-related cost was significantly associated with insulin-based regimens, longer duration of DM, higher HbA1c levels, worse stage of DR at presentation, the presence of DME at presentation, the presence of glaucoma, and increased mean number of intravitreal injections, laser sessions, and surgical operations. Multivariate analysis should the presenting stage of DR, presence of DME, and the presence of DME be the independent factors affecting the DR-related cost. Conclusion: This study is the first study to be conducted in Jordan and encourages us to establish a screening program for DR for earlier detection and treatment. DM control and treatment compliance will reduce the heavy costs of the already exhausted healthcare and financial system.
Diabetic retinopathy and the associated risk factors in diabetes type 2 patients in Abha, Saudi Arabia

Abstract:OBJECTIVES: To assess the proportion and grades of retinopathy and its risk factors in diabetes type 2 patients.MATERIALS AND METHODS: This was a cross-sectional study of 401 type 2 diabetic patients. A questionnaire and checklist were used to collect the data. Retinopathy was diagnosed and graded by fundus photographs and slit lamp examination. The duration of diabetes, age of patients, age at onset of diabetes, body mass index, hemoglobin A1c level, blood pressure, and complications were noted.RESULTS: The mean age of male and female patients was 54.93 and 54.25 years; 57.6% were males. The mean age of onset and mean duration of diabetes were 43.91 and 13.4 years, respectively. The proportion of retinopathy was 36.4%. Grades of retinopathy were: Mild 57.5%, moderate 19.9%, severe nonproliferative 11%, and proliferative retinopathy 11.6%; 7.2% of patients had maculopathy. Retinopathy was significantly associated with older age, younger age at onset, longer duration of disease, poorly controlled blood sugar, hypertension, insulin use; the presence of neuropathy and nephropathy appeared as a significant risk. Younger age at onset, longer duration, and insulin use appeared as the strongest predictors for diabetic retinopathy.CONCLUSIONS: More than a third (36.4%) of the diabetic patients attending a diabetic center had retinopathy. The control of the risk factors may reduce both prevalence and consequences of retinopathy.
Uptake of Tc-99m MAA by the liver during a thromboscintigram/lung scan.

Abstract:A young man with a swollen left leg was referred to us for a thromboscintigram/lung scan. The unexpected visualization of the liver during the flow study after intravenous injection of Tc-99m MAA into the dorsal veins of both feet provided the clue to his underlying problem. He was shown to have extensive venous collateralization in the left pelvis, and to a lesser degree on the right, due to a large pelvic mass. This was shown to consist of metastases from a previously treated testicular carcinoma. Venous drainage from the legs was shunted into the mesenteric circulation, which them emptied into the portal vein, thereby carrying the radiopharmaceutical to the liver. No uptake was seen in the spleen.

Abstract:Diabetic Retinopathy (DR) affects about 27% of patients with diabetes globally. According to the World Health Organization (WHO), DR is responsible for37 million cases of blindness worldwide. The SMART India study (October 2020-August 2021) documented the prevalence of diabetes, and DR in people40 years and above across ten Indian states and one Union Territory by conducting community screening. About 90% of people with sight threatening diabetic retinopathy (STDR) were referred from this screening study to eye hospitals for management, but failed to attend. This qualitative study, a component of the SMART India study, explored perceptions of referred patients regarding their susceptibility to eye related problems in diabetes and the benefits/barriers to seeking care. Perceived barriers from the viewpoint of ophthalmologists were also explored. Guided by the Health Beliefs Model (HBM), 20 semi structured interviews were carried out with consenting patients diagnosed with STDR. They included nine patients who had sought care recruited from eight eye hospitals across different states in India and eleven patients who did not seek care. Eleven ophthalmologists also participated. Four themes of analysis based on the HBM were, understanding of DR and its treatment, perceptions about susceptibility and severity, perceived barriers, perceived benefits and cues to action. Findings revealed poor understanding of the effects of diabetes on the eye contributing to low risk perception. Prohibitive costs of treatment, difficulties in accessing care services and poor social support were major barriers to seeking care. Ophthalmologists acknowledged that the absence of symptoms and the slow progressive nature of the disease deluded patients into thinking that they were fine. The study attests to the need for greater health literacy around diabetes, DR and STDR; for making treatment more affordable and accessible and for the development of effective patient education and communication strategies towards increasing compliance.

Abstract:Introduction: The objective of this study is to evaluate the quality of life (QoL), depression, anxiety, and stress, along with associated factors among individuals with diabetes in Saudi Arabia. Methods: This survey was conducted at King Abdulaziz University (KAU), Jeddah, Kingdom of Saudi Arabia (KSA). The assessment of depression, anxiety, and stress related to Type 2 Diabetes Mellitus (T2DM) was conducted using the DASS-21 questionnaire, while diabetes-related QoL was evaluated using the revised version of the diabetes QoL questionnaire (RV-DQoL13). Data were analyzed using SPSS-26. Results: A total of 251 subjects were included in the study (165 [65.7%] males and 86 [34.3%] females, mean age 50.1 ± 14.5 years). The individuals with DM had a mean value of QoL of 29.16 ± 9.23, with 46.9% having poor QoL. Furthermore, in dimensions of QoL, almost half of the individuals reported high worry about the disease (49.6%), followed by a high diabetes impact (46.6%) and low life satisfaction (42.9%). The prevalence of depression, anxiety, and stress was 49.4, 71.7, and 49.8%, respectively. A significant correlation was found between depression, anxiety, and stress and DASS-21 scores with QoL ( p < 0.001). The regression analysis indicated an association of distinct factors with QoL like age above 41 years ( p = 0.004), being married ( p < 0.001), being divorced ( p = 0.04), higher education ( p = 0.007), regular medicine intake ( p = 0.01), regular exercise ( p = 0.03), lipid profile ( p = 0.01), HbA1c ( p < 0.001), and DASS-21 scores ( p 27) was significantly associated with depression, anxiety, and stress ( p < 0.001). The participants with higher monthly income, shorter disease duration, regular medicine use, and altered lipid profile, and older subjects had a lower chance of depression, anxiety, and stress. Conclusion: Approximately half of individuals with T2DM experienced poor QoL, while the prevalence rates for depression, anxiety, and stress were 49.4, 71.7, and 49.8%, respectively. Scores in the domains of impact, worry, and satisfaction were below optimal levels. Several factors were linked to QoL and depression, anxiety, and stress, and an association was observed between DASS-21 scores and QoL.

Demographics and Awareness of Diabetic Retinopathy among Diabetic Patients Attending Department of Ophthalmology at Dhulikhel Hospital

Abstract:Background Diabetes mellitus is considered as a major public health concern globally. Poor management of diabetes may lead to several serious complications including endstage renal disease, diabetic retinopathy, diabetic neuropathy, and cardiovascular complications. Among them diabetic retinopathy is one of the leading causes of visual impairment and blindness in Nepal. Objective To assess the demographics and level of awareness among individuals with diabetes about diabetic retinopathy during their visit to Department of Ophthalmology at Dhulikhel Hospital. Method A descriptive cross-sectional study was conducted involving all consecutive cases of diabetes mellitus who attended eye clinic with the principal investigator and co-investigator from March 2021 to August 2021. Detailed demographics of the participants, their level of awareness regarding diabetic retinopathy and associated complications were documented. This was followed by a detailed ocular examination intending to screen for the presence of features of diabetic retinopathy. Result A total of 260 patients with a mean age of 54 years ± 12.20 (range of 24 - 85 years) were included. The male to female ratio was 2.2:1. Among them, 65.4% of the patients had diabetes mellitus for less than 5 years duration. Diabetic retinopathy was found in 18.46% of the cases. Awareness of diabetic ocular complications was significantly higher among literate population (p = 0.054), among those who have positive family history of diabetes mellitus (p = 0.529) and those who had undergone prior fundus evaluation (p = 0.000). Conclusion From result of this study, we can emphasize that there is a necessity for health education in order to increase the awareness and knowledge about diabetic retinopathy to lower the burden of sight threatening complications related with the issue.

Abstract:Background: In the era of patient-centered medical care, using objective clinical measures to assess patient-centered outcomes in all aspects became a necessity, and pars plana vitrectomy (PPV) is a major ophthalmic surgical procedure used for the treatment of vitreoretinal disorders aiming to improve their vision-related quality of life. Purpose: To study the impact of PPV on visual quality of life by assessing its effect on common daily activities, to assess its association with various factors, and to explore the relations between these factors and postoperative visual function. Methods: Vision-related quality of life for 87 patients who underwent PPV was assessed using the 25-item National Eye Institute Visual Function Questionnaire (NEI VFQ-25) by phone call interviews. Variables assessed include age, gender, indication of surgery, duration since surgery, preoperative best-corrected visual acuity (BCVA), postoperative BCVA, fellow eye BCVA, medical history and lens status. Results: The factors significantly affecting the total score were postoperative visual acuity and fellow eye VA. Upon analyzing the effect of the indication on total score, a significant difference was found with the highest being for those who had dropped lens as the indication for surgery and the lowest was for those with tractional retinal detachment (TRDs) and inflammatory indications. Subscale analysis and visual acuity improvement were also varying between indications. Conclusion: VRQOL significantly improves after PPV, the improvement is variable with different indications, being the greatest for those with dropped lens and the least for TRDs and endophthalmitis, with postoperative VA being the most important factor affecting the VRQOL score.

Abstract:Diabetes mellitus is a common disease in Saudi Arabia and globally, with some studies reporting about 30% prevalence among the Saudi population. Diabetic retinopathy (DR) is the leading cause of blindness among working-age patients, and it has a prevalence of 34.6% among diabetics worldwide. The objectives of this review were to examine the prevalence of DR in Saudi Arabia, describe the major associated risk factors, and raise awareness about the disease among diabetics. The prevalence of DR in Saudi Arabia has risen substantially in recent decades. Significant risk factors associated with DR include older age, longer duration of diabetes, poor glycemic control, and hypertension. Awareness and education about diabetes are associated with better outcomes and fewer complications.
