Abstract:OBJECTIVE:To assess the co-morbidity of anxiety and depression in hospitalized patients and to analyze the degree of symptoms and the influential factors.METHODS:Thirty patients with malignant tumor were injected with thymosin alpha 1 subcutaneously at the dose of 1.6 mg q.d. for the first month and q.o.d. for the following month. The number of T cell subgroups and the activity of NK cell in peripheral blood were detected and the quality of life of the patients were evaluated before treatment and at the end of treatment.RESULT:(1) About 39.9 % of the patients presented the symptoms of anxiety and depression and out of them 6.7 % were taken psychotropic medication. (2) Stepwise Logistic Regression showed that the factors related to anxiety and depression could be classified into four categories: economic factor, sleeping status, cognition of disease, the other factors such as course of disease and the environment of hospital. (3) Data analysis of oncology patients showed that the cognition of disease would be an important factor, which would affect anxiety and depression status.CONCLUSION:Multiple factors can affect anxiety and depression symptoms of inpatients. The results suggest that psychological intervention such as cognitive therapy should be considered in clinical practice.
Effect of Preoperative Depression on Early Postoperative Cognitive Function in Laparoscopic Colon Cancer Patients During General Anesthesia

Abstract:Objective To analyze the effect of preoperative depression on early postoperative cognitive function after general anesthesia during laparoscopic colon cancer surgery.Methods One hundred and fifty ASA Ⅰ-Ⅱ general aneasthesia patients (male70,female 80)from Sir Run Run Shaw Hospital in May of 2015 to April of 2016, aged 50-85, undergoing selective laparoscopic colon cancer patients were included.Evaluated preoperative depression by self-rating depression scale and evaluated postoperative cognitive function on 1,3,7 days by post-operative quality recovery scale.Use the multi-factor logistic regression to select the risk factors.Results There were 115 (76.7%) patients occurred poor cognitive function.According to SDS, The results of logistic regression analysis showed that the risk factors for postoperative poor cognitive function was severe depression(OR 1.295, 95% CI 1.012-13.184, P=0.048).Conclusion Preoperative severe depression is a risk factor on postoperative cognitive function in laparoscopic colon cancer patients during general anesthesia.

Abstract:Background: Pituitary adenoma (PA) is the third most common tumor in craniocerebral surgery. Most patients will experience varying degrees of negative emotions before and after surgery, which may affect the prognosis of surgery. This study analyzed the perioperative negative emotional risk factors of patients with different characteristics of PA and their impact on prognosis, so as to provide a reference for improving the prognosis of patients with PA.Methods: A total of 234 patients who underwent PA surgery in the Affiliated Hospital of Nantong University from January 2017 to January 2022 were selected as the observation population. The general characteristics of the subjects were collated using a general information questionnaire designed by the researchers. The negative emotions of the patients were evaluated using a Self-rating Anxiety Scale (SAS) and a Self-rating Depression Scale (SDS). The prognosis of patients was determined by assessing the hypophyseal hormone levels. Multiple regression analysis and logistic regression were used to analyze the risk factors of perioperative negative emotions and the effects of negative emotions on patient prognosis.Results: Multiple regression analysis showed that with and without children, education, income, PA type, PA size, and surgical approach were independent factors influencing negative emotions in patients after PA surgery (P<0.05). Logistic regression analysis showed that negative emotion was an independent prognostic factor (P<0.05).Conclusions: There are many factors that affect the anxiety and depression of patients after PA surgery. The family members and medical staff of the patients should take effective measures to relieve the anxiety and depression of the patients so as to improve the prognosis of patients according to the influencing factors.

Abstract:Purpose: Researches indicate that patients with papillary thyroid cancer (PTC) are prone to experiencing anxiety and depression. However, lacking of large-scale, prospective studies thoroughly examine the trajectory of these emotional states. Therefore, this study aims to investigate changes in anxiety and depression levels in PTC patients before and after operation and assess the impact of gender and menopausal states on emotional well-being. Method: A prospective longitudinal study enrolled 320 PTC patients who completed the Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) at pre- and post-operation. Paired-sample T-tests and logistic regression analysis were used to evaluate emotional changes and identify influencing factors. Results: Anxiety and depression levels significantly increased (p = 0.028, p = 0.005) postoperatively, with incidence of 13.8% for anxiety and 26.9% for depression. Multivariate regression analysis showed that gender was a factor affecting depression (p = 0.041), and menopausal state significantly influenced emotions including anxiety and depression (p = 0.004, p = 0.031). Subgroup analysis showed that female patients exhibited higher levels of adverse emotions postoperatively compared to preoperatively (p = 0.003, p < 0.001). Postoperatively, female patients were more susceptible to anxiety and depression (p = 0.011, p = 0.029), and postmenopausal women were particularly at risk for depression (p = 0.017). Conclusions: The study highlights the importance of postoperative psychological care, particularly for female and postmenopausal patients. Larger sample sizes and longer follow-up periods are needed for a comprehensive understanding of psychological changes in PTC survivors.

Abstract:Objective: This study was designed to evaluate anxiety and depression disorders and investigate the prevalence and risk factors in preoperative patients who visited a cosmetic plastic surgery clinic. Methods: The estimate of anxiety and depression in preoperative patients, who were admitted to the outpatient department of plastic surgery in a general hospital from July 2016 to July 2017, was determined by a field survey method using the Hospital Anxiety and Depression Scale. The demographic information of all patients was collected. All data were analyzed by SPSS statistical package. Results: A total of 315 patients were enrolled, which included 15 males and 300 females, with an average age of 30 +/- 8.4 years. The mean HADS score was 8.2 +/- 5.13, HADS-A was 4.7 +/- 3.04, and HADS-D was 3.5 +/- 2.72. The HADS-A score was significantly higher than the HADS-D score (p= 0.000). There were 34 cases (10.8%) with anxiety, 15 cases (4.8%) with depression, and 6 cases (1.9%) with both anxiety and depression. We found that the education level of these patients (p = 0.001) and the development level of their residence places (p = 0.040) have a negative correlation with the score of anxiety. There was a negative correlation between education level (p =0.000) and depression score. Education level was associated with the diagnosis of anxiety (p = 0.034). Education level (p = 0.001) and occupation (p = 0.015) were associated with the diagnosis of depression. Conclusion: In patients seeking cosmetic plastic surgery, the preoperative anxiety is more obvious than depression. Abstract:Depression is a common occurrence among cancer patients, and it significantly impacts their clinical outcomes and quality of life, with a high incidence during anti-tumor treatment or after surgery. The association between surgery and depression is the result of the interaction of various factors, including physiological, psychological, and social factors, all of which are intertwined and make patients susceptible to depression after surgical treatment. Postoperative depression has a significant negative impact on many aspects of cancer patients, and it requires timely identification and intervention to improve the overall outcome.
Correlation between anxiety and postoperative quality of life in patients with prostate cancer

Abstract:Objective To evaluate the correlation between anxiety and postoperative quality of life in patients with prostate cancer. Methods A total of 32 patients scheduled for elective radical prostatectomy were enrolled in the study. The questionnaires of the Stata-Trait Anxiety Inventory(STAI) were completed a day before the surgery. The functional assessment of cancer therapy-prostate(FACT-P) were accomplished a month after the surgery. The relationship between STAI and FACT-P was analyzed with SPSS. Results Anxiety inventory score was 59.0±7.3, which was significantly correlated(P<0.05) with the FACT-P score of 64.5±7.4. The STAI showed a negative liner correlation with the FACT-P (r= -0.58). Conclusion A inverse correlation has been found between the anxiety and postoperative quality of life in patients with prostate cancer. Psychological care of patients in the period of operation should be formulated according to the STAI, to strengthen communication between patients and their family. It is important for clinical treatment and recovery.

Abstract:734 Background: Prospective data assessing prevalence of anxiety and depression in a diverse surgical oncology population are currently lacking. Psychological distress may affect patient outcomes in the postoperative setting. The General Anxiety Disorder (GAD) and Patient Health Questionnaire (PHQ9) surveys are validated instruments to assess severity of anxiety and depression, respectively. We aimed to determine the prevalence of anxiety and depression using validated screening tools, and assess associations between anxiety and depression with clinical outcomes in patients presenting to our surgical oncology clinic. Methods: A prospective, surgeon-blinded study was conducted to assess associations of anxiety and depression with postoperative outcomes in patients with histologically proven malignancy who underwent surgical intervention. Preoperatively, the GAD and PHQ9 surveys were administered to evaluate for anxiety and depression, respectively. Frailty level was also assessed using the validated Risk Analysis Index (RAI-C) survey. Postoperative outcomes included rates of ICU admission, 30- and 90-day readmissions, postoperative complications classified by Clavien-Dindo score, and disposition to home versus rehabilitation facilities. Outcomes were compared between different groups as defined by the severity of anxiety and depression. Results: 191 patients met inclusion criteria. The cohort was stratified into three groups by severity of anxiety and depression. Overall, 59 (31%) patients reported at least moderate anxiety as defined by a GAD score ≥5, with 30 (15.7%) patients reporting severe anxiety (GAD score ≥ 10). Similarly, 62 (32%) patients reported at least mild depression as defined by a PHQ9 score ≥5, with 21 (11%) patients experiencing moderate to severe depression (PHQ9 ≥ 10). No difference was detected in length of stay, ICU admission, 30- and 90-day readmission, and postoperative complications. Preoperative frailty, as defined by RAI-C score >21, was also not associated with anxiety or depression. Marital status of single or widowed was found to be significantly associated with mild depression (p=0.027) and moderate to severe depression (p=0.028), respectively. Conclusions: While anxiety and depression were not found to be significantly associated with postoperative outcomes, they are prevalent in surgical oncology patients, affecting approximately one-third of our patient population. The GAD and PHQ9 surveys are validated instruments that can be administered easily to identify surgical oncology patients with psychological distress. Comprehensive evaluation of psychological well-being, in conjunction with a thorough social evaluation, can identify those patients who may benefit from referral to supportive resources.

Abstract:Objective A large number of patients with pulmonary ground-glass opacities (GGOs) have anxiety and depression. However, the contributing factors and effects of anxiety and depression on postoperative outcomes are still unclear. Methods Clinical data for patients undergoing surgical resection for pulmonary GGOs were collected. We prospectively evaluated levels and risk factors for anxiety and depression in patients with GGOs before surgery. The relationship between psychological disorders and postoperative morbidity was evaluated. Quality of life (QoL) was also assessed. Results A total of 133 patients were enrolled. Prevalence rates of preoperative anxiety and depression were 26.3% ( n = 35) and 18% ( n = 24), respectively. Multivariate analysis revealed depression [odds ratio(OR) = 16.27, p < 0.001] and multiple GGOs (OR = 3.146, p = 0.033) to be risk factors for preoperative anxiety. Anxiety (OR = 52.166, p < 0.001), age > 60 (OR = 3.601, p = 0.036), and unemployment (OR = 8.248, p = 0.006) were identified as risk factors for preoperative depression. Preoperative anxiety and depression were associated with lower QoL and higher postoperative pain scores. Our results also revealed that the incidence of postoperative atrial fibrillation was higher in patients with than in those without anxiety. Conclusions In patients with pulmonary GGOs, comprehensive psychological assessment and appropriate management are required before surgery to improve QoL and reduce postoperative morbidity.

Abstract:Objective Anxiety and depression are common mental disorders in glioma patients. This study aimed to evaluate the risk factors, prognostic role, and longitudinal changes in anxiety and depression in postoperative glioma patients. Methods Anxiety and depression were assessed by Hospital Anxiety and Depression Scale at baseline, month (M) 6, M12, M24 and M36 in 270 glioma patients after surgical resection. Furthermore, comprehensive clinic characteristics and treatment-related information were collected. Results Gender (female vs. male) ( P = 0.014, odds ratio (OR) = 1.974), marital status (single/divorced/widowed vs. married) ( P = 0.019, OR = 2.172), Karnofsky performance status (KPS) score (≤70 vs. > 70) ( P = 0.002, OR = 2.556), World Health Organization (WHO) classification (high-grade glioma (HGG) vs. low-grade glioma (LGG)) ( P = 0.005, OR = 2.155), and postoperative complications (yes vs. not) ( P = 0.001, OR = 2.525) were independently related to anxiety occurrence. Marital status (single/divorced/widowed vs. married) ( P = 0.034, OR = 2.026), KPS score (≤70 vs. > 70) ( P < 0.001, OR = 3.880), WHO classification (HGG vs. LGG) ( P = 0.032, OR = 1.810), and postoperative complications (yes vs. not) ( P = 0.001, OR = 2.602) were independently related to depression occurrence. Besides, anxiety ( P = 0.038) and depression ( P = 0.013) were linked with shorter overall survival (OS), and depression was an independent risk factor for worse OS ( P = 0.040, hazard ratio = 1.596). More importantly, anxiety and depression remained at a high prevalence during a 3-year follow-up. Conclusion Gender, marital status, KPS score, WHO classification, and postoperative complications are risk factors for anxiety and depression; moreover, anxiety and depression are at high prevalence continuously and correlated with worse survival in postoperative glioma patients.

Abstract:Background: The objective of this study was to observe the incidence and potential risk factors of postoperative depression and anxiety in patients during the early period after undergoing orthognathic surgery. Methods: From March 7 to September 7, 2023, patients ≥ 18 years of age who were scheduled for elective orthognathic surgery under general anesthesia in Peking University School and Hospital of Stomatology were included in this study. We prospectively evaluated their degrees of pre- and postoperative depression and anxiety using the Patient Health Questionnaire-9 and the State Trait Anxiety Inventory. Associations between the perioperative factors and occurrences of postoperative anxiety and depression were evaluated using a multivariate logistic regression model. Results: A total of 371 patients were included in the analysis. Within five days after surgery, we observed the occurrence of depression in 32% (116) of the patients and anxiety in 72.8% (270) of them. Their preoperative depression score on the Pain Catastrophizing Scale and intraoperative urine output were significantly associated with a higher risk of postoperative depression. The presence of preoperative anxiety, postoperative moderate-to-severe pain, postoperative nausea and vomiting and postoperative insomnia were significantly associated with a higher risk of postoperative anxiety. Furthermore, a monthly income ≥ ¥10000 was found to be significantly associated with a lower risk of postoperative anxiety. Conclusions: Postoperative depression and anxiety are common among patients who undergo orthognathic surgery. Moreover, preoperative psychological status and incidence of postoperative adverse events were associated with an increased risk of depression and anxiety after surgery. The results of the present study suggest that careful psychological assessment and appropriate management are necessary to improve patients' recovery following orthognathic surgery.
Analysis of risk factors for negative emotions in perioperative period of ovarian cancer patients and their impact on prognosis

Abstract:Background: Ovarian cancer (OC), a common malignant tumor of the female reproductive system, has the highest mortality rate among gynecologic cancers. Female patients often experience negative emotions such as anxiety and depression due to sex hormone disorders, fear of cancer, and unfamiliarity with the hospital environment. This study aimed to elucidate the risk factors of negative emotions in the perioperative period of OC patients and their effect on prognosis to provide a reference basis for improving patients' prognosis.Methods: We retrospectively analyzed the data of 258 patients with OC at our hospital between August 2014 and December 2019. The <i>t</i>-test and chi-square test were used to analyze the relationship between patients' negative emotions and prognosis. Binary logistic regression was used to analyze the independent risk factors for the occurrence of negative emotions and poor prognosis in patients.Results: Binary logistic regression analysis showed that young age, low monthly household income, low education, no children, lymph node metastasis, postoperative chemotherapy, time to postoperative bowel function recovery ≥24 hours, and the presence of postoperative complications such as irregular bleeding and pressure sores were independent risk factors for negative emotions in patients. Furthermore, negative emotions were found to be an important independent risk factor for patients' prognosis. In patients with negative emotions, the survival rate at 2 and 3 years after surgery was significantly lower compared with patients without negative emotions, and the recurrence rate at 3 years after surgery was significantly higher than in patients without negative emotions.Conclusions: In the perioperative period of OC, patients are prone to anxiety, depression, and other psychological disorders, which seriously affect the treatment effect. Therefore, in clinical work, patients' negative emotions should be predicted as early as possible, and active communication with patients and timely psychological counseling should be provided. Improve surgical accuracy and reduce the complication rate.

[Relationship between the chronic periodontitis and the depression anxiety psychological factor]

Abstract:Objective: To explore the relationship between the chronic periodontitis (CP) and the depression-anxiety psychological factors. Methods: Thirty-one patients and 29 age, gender-matched volunteers were enrolled for this study. In order to assess the depression-anxiety psychological index, the subjects filled the questionnaire regarding the demographic and socioeconomic information, the oral hygiene habit, the Center for Epidemiologic Studies Depression Scale (CES-D) and the Self Rating Anxiety Scale(ASA). Calculus index (CI), bleeding on probing (BOP), probing depth (PD), clinical attachment loss (CAL), furcation involvement (FI) and tooth mobility were assessed at 6 sites per tooth of all erupted teeth by a manual periodontal probe. The data were analyzed by the analysis of variance, χ(2) test, and multivariable logistic step wise analysis via the software of SPSS 15.0. Results: The mean CAL of the control group was 0.46 ± 0.16,the mean CAL of the moderate, high, and severe CP group was 2.84 ± 0.12, 3.51 ± 0.34, and 4.71 ± 0.51, respectively, which is significant difference between each other (P<0.01). The depression index of the volunteers, the moderate CP, the high CP, and the severe CP was 30.52 ± 3.73, 35.83 ± 7.76, 37.25 ± 6.16, 37.82 ± 5.94, respectively. The anxiety index among the 4 groups was 26.69 ± 3.55, 37.67 ± 6.31, 32.87 ± 5.54, and 35.94 ± 6.30, respectively. The depression and anxiety indexes of the periodontitis groups were higher than those of the control (P<0.01) while there was no significant difference among the 3 CP groups (P>0.05). The multivariate logistic analysis of the relationship between CP and the depression-anxiety psychological factors showed that the depression psychological factor was B=2.301,OR=9.988 while the optimistic coping style was B=-5.174,OR=0.006 in the equation of the regression. Conclusion: The depression psychological factor was related to the progression of CP. In addition, the optimistic coping style could prevent the progression of the CP.

Abstract:Context. Gastric and oesophageal cancers are common. They are also expected to increase in incidence in the next few years and are characterized by poor prognosis. Surprisingly, whereas the incidence of severe anxiety and depression is high in patients with gastric and oesophageal cancers, the influence of symptoms of depression and anxiety on postoperative complications has barely been explored. Methods. In a retrospective study based on a prospectively collected database, 629 cancer patients were enrolled. Symptoms of depression and anxiety (Hospital Anxiety and Depression Scale scores) and sociodemographic and medical information were collected immediately after diagnosis and before any treatment. The surgical approach (i.e. gastrectomy or oesophagectomy) and postoperative complications according to the Clavien–Dindo classification were collected after surgery. Results. After controlling for known medical predictors (i.e. surgical strategy, alcohol and tobacco consumption, American Society of Anaesthesiologists classification physical status score) of postoperative complications, no effect of symptoms of depression or anxiety was detected. Discussion. The observed results are surprising given the literature. However, several potential arguments can be put forwards regarding methods and measures, controlling variables, and conceptual distinctions. Despite the absence of significant results, this topic should be more deeply investigated by applying methodological and conceptual adjustments.

Investigation of the quality of life, mental status in patients with gynecological cancer and its influencing factors

Abstract:Background: Having a gynecological tumor or undergoing treatment can be a traumatic experience for women, as it affects their self-image and sexual relationships and can lead to psychological reactions. Psychological adjustment following cancer occurrence remains a key issue among the survivors. Aim: To examine the current status of quality of life (QoL), anxiety, and depression in patients with gynecological cancer and to analyze the factors associated with it. Methods: Data for 160 patients with gynecological malignancies treated at Shanxi Bethune Hospital from June 2020 to June 2023 were collected and analyzed retrospectively. Patients' QoL was assessed using the European Organization for Research on Treatment of Cancer Quality of Life Questionnaire Core 30 and the Functional Assessment of Cancer Therapy-General Questionnaire. Their emotional status was evaluated using the Self-Rating Anxiety/Depression Scale. The associated factors of anxiety and depression were analyzed. Results: The overall QoL score of the patients 6 months after surgery was 76.39 ± 3.63 points. This included low levels of social and emotional function and severe fatigue and pain. The scores for physiological, functional, emotional, social, and family well-being exhibited an upward trend following surgery compared with those before surgery. One month after surgery, some patients experienced anxiety and depression, with an incidence of 18.75% and 18.13%, respectively. Logistic analysis revealed that good sleep was a protective factor against anxiety and depression in patients with gynecological tumors, whereas physical pain was a risk factor. Conclusion: Patients with gynecological malignancies often experience anxiety and depression. By analyzing the factors that affect patients' QoL, effective nursing measures can be administered.
Psychosocial Adjustment Changes and Related Factors in Postoperative Oral Cancer Patients: A Longitudinal Study

Abstract:Disease and treatment-related symptoms and dysfunctions can interfere with the psychosocial adjustment of patients with oral cancer. Identifying factors influencing psychosocial maladjustment is important because at-risk individuals can be targeted for early intervention. This prospective longitudinal study investigated psychosocial adjustment changes and associated factors in postoperative oral cancer patients. Data on psychosocial adjustment, facial disfigurement, symptoms, and social support were collected before surgery (T1) at one month (T2), three months (T3), and five months after discharge (T4). Fifty subjects completed the study, and their data were included in the analysis. Psychosocial maladjustment was reported in 50%, 59.2%, 66%, and 62% of subjects at T1, T2, T3, and T4, respectively. The subjects' psychosocial adjustment deteriorated after surgery. Results from generalized estimating equations indicated that financial status, cancer stage, pain, speech problems, social eating problems, and less sexuality were significant predictors of changes in psychosocial adjustment. Patients with insufficient income, stage III/IV cancer, severe pain, speech problems, social eating problems, and less sexuality were at higher risk for postoperative psychosocial maladjustment. Continued psychosocial assessment and appropriate supportive measures are needed to strengthen the psychosocial adjustment of these high-risk groups.
