Abstract:Objective To analyze the effect of preoperative depression on early postoperative cognitive function after general anesthesia during laparoscopic colon cancer surgery.Methods One hundred and fifty ASA Ⅰ-Ⅱ general aneasthesia patients (male70,female 80)from Sir Run Run Shaw Hospital in May of 2015 to April of 2016, aged 50-85, undergoing selective laparoscopic colon cancer patients were included.Evaluated preoperative depression by self-rating depression scale and evaluated postoperative cognitive function on 1,3,7 days by post-operative quality recovery scale.Use the multi-factor logistic regression to select the risk factors.Results There were 115 (76.7%) patients occurred poor cognitive function.According to SDS, The results of logistic regression analysis showed that the risk factors for postoperative poor cognitive function was severe depression(OR 1.295, 95% CI 1.012-13.184, P=0.048).Conclusion Preoperative severe depression is a risk factor on postoperative cognitive function in laparoscopic colon cancer patients during general anesthesia.
Cross-sectional Investigation and Analysis of Anxiety and Depression in Preoperative Patients in the Outpatient Department of Aesthetic Plastic Surgery in a General Hospital in China.

Abstract:Objective: This study was designed to evaluate anxiety and depression disorders and investigate the prevalence and risk factors in preoperative patients who visited a cosmetic plastic surgery clinic. Methods: The estimate of anxiety and depression in preoperative patients, who were admitted to the outpatient department of plastic surgery in a general hospital from July 2016 to July 2017, was determined by a field survey method using the Hospital Anxiety and Depression Scale. The demographic information of all patients was collected. All data were analyzed by SPSS statistical package. Results: A total of 315 patients were enrolled, which included 15 males and 300 females, with an average age of 30 +/- 8.4 years. The mean HADS score was 8.2 +/- 5.13, HADS-A was 4.7 +/- 3.04, and HADS-D was 3.5 +/- 2.72. The HADS-A score was significantly higher than the HADS-D score (p= 0.000). There were 34 cases (10.8%) with anxiety, 15 cases (4.8%) with depression, and 6 cases (1.9%) with both anxiety and depression. We found that the education level of these patients (p = 0.001) and the development level of their residence places (p = 0.040) have a negative correlation with the score of anxiety. There was a negative correlation between education level (p =0.000) and depression score. Education level was associated with the diagnosis of anxiety (p = 0.034). Education level (p = 0.001) and occupation (p = 0.015) were associated with the diagnosis of depression. Conclusion: In patients seeking cosmetic plastic surgery, the preoperative anxiety is more obvious than depression. By this I meant that the philosophy of palliative care had become so enshrined in rhetoric that practitioners are able to claim that they provide whole person, family-centred and multidisciplinary care, while failing to explore the limits to which this is achieved within the context of contemporary health-care delivery. The words of palliative care philosophy role off the tongue but we rarely challenge what we do. However, in today’s health-care world, and with increasing governmental funding of palliative care services, demands to be accountable for the outcomes of palliative care are growing. The challenge is no longer one of proving the place of palliative care, but rather of demonstrating the effect of our interventions so that they can be taken up more broadly in health care. The change of state-trait anxiety among patients undergoing orthognathic surgery: A longitudinal study

Abstract:Introduction: This study aimed to assess state-trait anxiety level changes in Chinese patients with dentofacial discrepancies before and after orthognathic surgery and to explore the feasibility of developing a reference index for the preoperative screening of postoperative patients with high anxiety. Methods: A total of 96 Chinese patients with dentofacial discrepancies who underwent orthognathic surgery were included in this study. Data were collected before orthognathic surgery and at 2 weeks (T2), 3 months, and 6 months (T4) after surgery using the State-Trait Anxiety Inventory. Receiver operating characteristic and linear regression analyses were performed to screen for preoperative indicators of postoperative high-state anxiety. Results: State-trait anxiety levels in patients with dentofacial discrepancies decreased after surgery (F = 18.95, P <0.01; F = 6.90, P <0.01). Trait Anxiety Inventory can be used to screen patients with high-state anxiety from T2 to T4 (area under cover 95% confidence interval: T2, 0.74 [0.62-0.86]; 3 months, 0.79 [0.69-0.90]; T4, 0.77 [0.66-0.87], P <0.01), corresponding to cutoff values of 48.5, 46.5, and 45.5, respectively. Conclusions: All participants' state-trait anxiety levels improved after surgery compared with their preoperative levels. Preoperative trait anxiety levels can be used as a reference indicator to screen patients who may have high-state anxiety levels after orthognathic surgery. The creation of a screening scale will assist health care professionals to more pertinently help patients with high anxiety.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>To describe patient characteristics associated with preoperative anxiety and subsequently assess the relationship between preoperative anxiety and postoperative anxiety, pain, sleep quality, nausea and vomiting.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The study collected data from patients undergoing elective operation from 12 hospitals in China. The State-Trait Anxiety Inventory (STAI) and the Athens Insomnia Scale (AIS) were used to assess anxiety and sleep quality before surgery. Evaluations of anxiety, pain, sleep quality, nausea and vomiting were quantified using the Visual Analogue Scale on postoperative days 1 and 2.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Data from 997 patients were analyzed. Preoperatively, 258 (25.9%) patients had high anxiety (STAI-State>44). Multivariate analyses showed a significant relationship between high anxiety and female gender (OR: 1.66, 95% CI: 1.08–2.57, <em>p</em> = 0.02), highly invasive surgery (OR: 2.29, 95% CI: 1.29–4.06, <em>p</em> = 0.005), higher trait anxiety (OR: 1.24, 95% CI: 1.20–1.28, <em>p</em> < 0.001) and insomnia (AIS ≥ 6, OR: 1.79, 95% CI: 1.17–2.76, <em>p</em> = 0.008). Preoperative anxiety demonstrated a negative correlation with postoperative anxiety following highly invasive surgery; this became a positive relationship following less invasive surgery. Preoperative anxiety was also positively related to postoperative pain and poor sleep quality. The correlation between preoperative anxiety and postoperative nausea and vomiting was not statistically significant.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Female gender, highly invasive surgery, higher trait anxiety and insomnia are independent risk factors for high preoperative anxiety. Surgical invasiveness influences association between pre- and postoperative anxiety. Higher preoperative anxiety was related to poorer sleep quality and more severe pain postoperatively.</p>

Abstract:Abstract Background Preoperative anxiety is associated with increased use of anesthetics and poorer postoperative outcomes. However, the prevalence of preoperative anxiety has not been characterized in Chinese patients. In this study, we aimed to estimate the overall prevalence of preoperative anxiety in Chinese adult patients and to explore the sociodemographic and clinical factors associated with preoperative anxiety in China. Methods This study was a multicenter cross-sectional study conducted at 32 tertiary referral centers in China from September 1 to October 31, 2020. Adult patients scheduled for elective surgery were evaluated by the 7-item Perioperative Anxiety Scale (PAS-7) for preoperative anxiety after entrance to the operating zone. Results A total of 5191 patients were recruited, and 5018 of them were analyzed. The prevalence of preoperative anxiety measured by PAS-7 was 15.8% (95% CI 14.8 to 16.9%). Multivariable analyses showed female sex, younger age, non-retired, first in a lifetime surgery, surgery of higher risk, and poorer preoperative sleep were associated with higher prevalence of preoperative anxiety. Conclusions Preoperative anxiety was relatively common (prevalence of 15.8%) among adult Chinese patients undergoing elective surgeries. Further studies are needed using suitable assessment tools to better characterize preoperative anxiety, and additional focus should be placed on perioperative education and intervention, especially in primary hospitals. Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Orthognathic Surgery (OGS) is a surgery for patients with dento-facial deformity but not all patients are satisfied with its outcome. The purpose of this study is to find out the short-term and long-term psychological impact and quality-of-life of OGS.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>77 participants receiving OGS and 32 age and gender-matched controls were enrolled. The data of questionnaires were collected before OGS, one month and 9 months after OGS, including short form of the Derriford-Appearance-Scale (DAS-24), Big-Five-Inventory (BFI), Hospital-Anxiety-and-Depression-Scale (HADS), Pittsburgh-sleep-quality-index (PSQI), and 36-Item Short-Form-Health-Survey (SF-36). Variables were presented as mean±standard deviation or frequency. Paired t-test, ANOVA and MANOVA were used to evaluate the pre-and post-surgery data.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Short-term and long-term satisfaction of OGS was high. Before OGS, BFI showed the extraversion had significant difference between the male and female OGS subgroups. Several domains of DAS-24 were significantly different between the OGS and the control groups. Both groups had no significant difference in PSQI, HADS and SF-36, except sleep-efficiency. After OGS, many domains of DAS-24 were significantly improved and the improvement persisted to 9 months later. Sleep-latency, physical-function, role-limitations-due-to-physical-health and social-functioning exacerbated after OGS. Sleep-latency, physical-function, and social-functioning were improved 9 months after OGS, but sleep-efficiency and role-limitations-due-to-physical-health were still significantly worse than controls.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>People received OGS for unfavorable appearance and the surgery could decrease their distress of appearance and impact to their daily living. Through long-term assessment, we should pay attention to sleep problems and role-limitations-due-to-physical-health after OGS.</p>

Abstract:Objective A large number of patients with pulmonary ground-glass opacities (GGOs) have anxiety and depression. However, the contributing factors and effects of anxiety and depression on postoperative outcomes are still unclear. Methods Clinical data for patients undergoing surgical resection for pulmonary GGOs were collected. We prospectively evaluated levels and risk factors for anxiety and depression in patients with GGOs before surgery. The relationship between psychological disorders and postoperative morbidity was evaluated. Quality of life (QoL) was also assessed. Results A total of 133 patients were enrolled. Prevalence rates of preoperative anxiety and depression were 26.3% ( n = 35) and 18% ( n = 24), respectively. Multivariate analysis revealed depression [odds ratio(OR) = 16.27, p < 0.001] and multiple GGOs (OR = 3.146, p = 0.033) to be risk factors for preoperative anxiety. Anxiety (OR = 52.166, p < 0.001), age > 60 (OR = 3.601, p = 0.036), and unemployment (OR = 8.248, p = 0.006) were identified as risk factors for preoperative depression. Preoperative anxiety and depression were associated with lower QoL and higher postoperative pain scores. Our results also revealed that the incidence of postoperative atrial fibrillation was higher in patients with than in those without anxiety. Conclusions In patients with pulmonary GGOs, comprehensive psychological assessment and appropriate management are required before surgery to improve QoL and reduce postoperative morbidity.

Abstract:Journal of Orthopaedic Surgery, Volume 32, Issue 3, September-December 2024. Objective: To investigate the potential association between anxiety and depression and surgical outcomes in patients undergoing LSS. By analyzing data from the Nationwide Inpatient Sample (NIS) database, we aim to identify whether anxiety and depression serve as predictors for postoperative complications and pain-related symptoms. Methods: A retrospective analysis was conducted via the NIS database. Those undergoing LSS from 2010 to 2019 were divided into four groups: those with a diagnosis of anxiety, depression, both depression and anxiety, and neither depression nor anxiety. The chi-squared test, rank sum test, the Student-Newman-Keuls, least significant difference, and Bonferroni tests were used to identify differences between these groups. Logistic regression analysis was utilized to determine if anxiety and depression were predictors for postoperative complications and pain-related symptoms. Results: From 2010 to 2019, 832,099 patients undergoing LSS were identified. Patients with either anxiety or depression were associated with heavier economic burdens ( 76,840, $88,542 in the anxiety, depression, and comorbid group, respectively, p < 0.001) and prolonged hospital stay (p < 0.001). They were identified to experience higher risks of various complications especially thrombophilia (OR = 1.82, and 1.55 in the anxiety and the depression group, respectively, p < 0.01). Multiple pain-related symptoms, but face reduced risks of inpatient mortality (OR = 0.71, 0.75, and 0.63 in the anxiety, depression, and comorbid group, respectively, p < 0.01). Conclusions: The overall morbidities of depression and anxiety were relatively high. Psychiatric comorbidities were closely correlated with the negative outcomes after LSS. The psychological health of patients receiving LSS requires necessary attention to ensure pain control and prevent complications postoperatively.

There is a critical need to identify patients at high risk of POD so that providers can better offer targeted interventions in the preoperative and intraoperative periods. We aimed to characterize the prevalence of preoperative anxiety and investigate whether preoperative anxiety predicted the onset of POD in elderly patients undergoing elective orthopedic surgery. Methods: We conducted a prospective observational cohort study of elderly patients (aged 65 years or older) undergoing elective orthopedic surgery. Preoperative anxiety was assessed using the Hospital Anxiety and Depression Scale-Anxiety (HADS-A), with clinically significant values defined as HADS-A> 7. POD was diagnosed according to the Confusion Assessment Method (CAM) criteria on the 5 postoperative days. To determine the independent risk factors for POD, multivariable logistic regression was conducted, including those variables with a p-value < 0.05 at univariate analysis. Results: There were 263 patients included in the study. Seventy-three (27.8%) patients were diagnosed to be POD. Median duration of delirium was 2 days. Forty (15.2%) patients were assessed to experience preoperative anxiety. The occurrence of preoperative anxiety in total hip arthroplasty (THA), proximal femoral nail antirotation (PFNA), and total knee arthroplasty (TKA) was 12.5%, 16.1%, and 19.5%, respectively. Based on multivariable analysis, only age (odds ratio [OR]= 1.099, 95% Confidence Interval [CI]: 1.013– 1.192; P =0.023), Mini Mental State Examination (MMSE) (OR= 0.603, 95% CI: 0.432– 0.842; P =0.003) and preoperative anxiety (OR= 3.119, 95% CI: 1.144– 8.500; P =0.026) were independently associated with POD. Conclusion: In conclusion, the current study reveals that preoperative anxiety helps to predict the risk of POD in elderly patients undergoing elective orthopedic surgery. Abstract:Background Anxiety is widely prevalent among patients planned for surgery in orthopedics department and it continues to go unnoticed and undertreated. Aim and objectives The present study was carried out to assess the pre-operative anxiety among patients scheduled for surgery in the department of orthopedics and find out the factors associated with it. Materials and methods A prospective cross-sectional study of eight months duration was carried out among 384 subjects proposed to undergo surgery in the department of orthopedics. The socio-demographic details and other relevant information pertaining to the subjects were collected in a well-designed data collection form. The level of anxiety of the subjects was assessed using GAD-7 scale. Chi-square test was performed to analyse the categorical variables and ordinal logistic regression was used to identify the related factors. p-value <0.05 was considered as statistically significant. Results Among the total subjects enrolled in the present study, 95 (24.7%) were found to have mild level of anxiety, 223 (24.7%) with moderate level and 66 (17.2%) with severe anxiety. Various factors of fear including the fear of surgical procedure (p<0.001), result of surgery (p<0.001), post-operative pain (p=0.001), hospital environment (p=0.004) and anaesthesia (p=0.012) were found to be the factors associated with severe anxiety. Conclusion The present study provides an insight to the healthcare professional on the essentiality of assessing anxiety and adopting appropriate interventions to address the issue among patients proposed with surgical interventions in department of orthopedics.

Abstract:BACKGROUND: Preoperative anxiety has the potential to alter the dynamics of an elective procedure and has been shown to detrimentally affect patients both cognitively and physiologically. If mismanaged, it can lead to essential procedures being postponed or canceled, delay postoperative recovery, and increase patients' requirements for medical intervention postoperatively. These outcomes have harmful implications both clinically and economically. Our primary objective was to evaluate the levels of anxiety patients experience immediately before elective otorhinolaryngologic procedures. Our secondary outcome was to assess the subjects' views on potential management strategies to tackle their anxiety.METHODS: This is an observational cross-sectional project evaluating 53 patients who were selected consecutively from a list of elective otorhinolaryngologic procedures. All procedures were to be completed under general anesthetic, and all patients had received the same preoperative assessment preparation. 29 male and 24 female patients were included, aged between 19 and 76 years (mean 45). The Spielberger State-Trait Anxiety Inventory was used to assess preoperative anxiety directly before the otorhinolaryngologic procedure. The Service Improvement questionnaire was used to assess whether patients would favor the introduction of anxiety-reduction measures.RESULTS: There was neither a significant increase in patient anxiety levels preoperatively (P = 0.37) nor a significant increase in anxiety levels preoperatively when results were stratified according to patient gender and age (P = 0.45 and P = 0.27). 54% of the patients felt that their anxiety would have been reduced if they had read a procedural information leaflet, and 22% felt it would have been reduced if they had received preoperative behavioral training. 17% of the patients wanted more information from the surgical team. However, 12% of the patients would have liked less information from the surgical team preoperatively.CONCLUSIONS: Patients did not have a significant increase in their anxiety levels preoperatively. On the basis of our findings, we will work to improve the information we provide to patients preoperatively and to identify patient subgroups that require additional preoperative support.

Abstract:Background: Depression and anxiety experienced before major cancer surgeries have been associated with postoperative morbidity and mortality. Objectives: This study aimed to estimate the prevalence of preoperative anxiety and depression in patients undergoing major oncosurgeries. Materials and Methods: This was a cross-sectional analytical study conducted between June 2021 and June 2022 at Malabar Cancer Center in Thalassery, Kerala, in South India. We enrolled patients aged 18–65 years undergoing major oncosurgeries with American Society of Anesthesiologists (ASA) Physical Status Grades I and II. Patients were administered a questionnaire (in the local language, Malayalam) by the anesthesiologist on their first preoperative visit after the preanesthetic checkup. The questionnaire was in two parts: part 1 consisted of the demographic data, and part 2 comprised the Patient Health Questionnaire-4 (PHQ-4) and the Amsterdam Preoperative Anxiety and Information Scale (APAIS). On the PHQ-4, a score of ≥3 for the first two questions was the cutoff for anxiety, and a score of ≥3 for the last two questions was the cutoff for depression. APAIS is a six-item questionnaire; a score of ≥11 on the anxiety scale was considered positive for anxiety, and a score of >5 was considered to indicate the need for more information. Results: We enrolled 200 patients; 136 (68%) were women, and the median age overall was 52 (interquartile range [IQR], 42–60). Patients enrolled were planned for surgeries for head-and-neck cancers ( n = 55, 27.5%), laparotomies or therapeutic laparoscopies ( n = 70, 35%), breast cancers ( n = 67, 33.5%), and bone and soft tissue surgeries ( n = 8, 4%). Anxiety was noted in 49 (24.5%) patients by PHQ-4 and 23 (11.5%) patients by APAIS; depression was detected in 31 (15.5%). The presence of depression significantly contributed to anxiety. Patients who screened positive for depression had 79.6 times higher odds of screening positive for anxiety, as compared to those who screened negative for depression. Conclusion: Almost one in four patients undergoing major oncosurgeries experience preoperative anxiety, and one in six have depression. Patients have a substantial requirement for information concerning anesthesia and surgical procedures, which, if unmet, contributes to anxiety and/or depression. Preoperative counseling and providing more information to patients could reduce anxiety or depression. Other causative personal, familial, social, or economic factors should also be assessed and managed (Clinical Trials Registry of India number: CTRI/2021/07/034568).

Abstract:Objectives To investigate the prevalence rate and influencing factors of preoperative anxiety and depression in patients with gastric cancer, in China. Methods Patients with gastric cancer who were diagnosed by gastroscopy and would accept laparoscopic surgery were eligible for the study. Each participant completed self-administered questionnaires, including the Hospital Anxiety and Depression Scale (HADS), Medical Coping Modes Questionnaire (MCMQ), Social Support Rating Scale, and Type D personality scale before surgery. Routine blood tests were undertaken within 7 days before surgery, to calculate the neutrophil-to-lymphocyte ratio (NLR). Based on HADS, patients were divided into an anxiety/depression group and a nonanxiety/depression group. Results Fifty-three patients with gastric cancer were included in the study. The prevalence of preoperative anxiety and/or depression was 20.75% (11/53). Factors influencing preoperative anxiety and depression were a resigned dimension of coping style, type D personality and NLR. Logistic regression analysis showed that a higher score for the resigned dimension of coping style on the MCMQ and a higher NLR were significantly associated with preoperative anxiety and depression. Conclusions The prevalence of preoperative anxiety and depression in Chinese patients with gastric cancer may be influenced by both the coping mode and NLR.

Surgery-related anxiety on geriatric patients undergoing total knee arthroplasty: a retrospective observational study

Abstract:Background: The prevalence of anxiety in patients undergoing total knee arthroplasty (TKA) and its association with postoperative functions are well known; however, the levels of anxiety or anxiety-related characteristics are unknown. This study aimed to investigate the prevalence of clinically significant state anxiety in geriatric patients undergoing TKA for osteoarthritis (OA) of the knee and to evaluate the anxiety-related characteristics experienced by these patients pre- and post-operatively. Methods: This retrospective observational study recruited patients who had undergone TKA for knee OA using general anesthesia between February 2020 and August 2021. The study participants were geriatric patients older than 65 years who had moderate or severe OA. We evaluated patient characteristics including age, sex, body mass index, smoking status, hypertension, diabetes, and cancer. We assessed their levels of anxiety status using the STAI-X which comprises 20-item scales. Clinically meaningful state anxiety was defined as a total score of 52 or higher. An independent Student's t-test was used to determine differences of STAI score between subgroups in terms of patient characteristics. And patients were asked to complete questionnaires, which assessed four areas: (1) the main cause of anxiety; (2) the most helpful factor in overcoming anxiety before surgery; (3) the most helpful factor in reducing anxiety after surgery; and (4) the most anxious moment during the entire process. Results: The mean STAI score of patients who underwent TKA was 43.0 points and 16.4% of patients experienced clinically significant state anxiety. The current smoking status affect STAI score and the proportion of patients with clinically meaningful state anxiety. The most common cause of preoperative anxiety was the surgery itself. Overall, 38% of patients reported that they experienced the greatest level of anxiety when the surgeon had recommended TKA in the outpatient clinic. The trust in the medical staff before surgery and the surgeon's explanations after surgery helped the most in reducing anxiety. Conclusions: One in six patients before TKA experience clinically meaningful state anxiety, and about 40% of patients experience anxiety from the time they are recommended for surgery. Patients tended to overcome anxiety before TKA through trust in the medical staff, and the surgeon's explanations after surgery was found to be helpful in reducing anxiety.

Abstract:BACKGROUND Periodontitis is a chronic oral disease caused by pathogenic microorganisms that corrode tooth tissue, form periodontal pockets, absorb alveolar bone, and finally lead to tooth loss. During treatment, patients are prone to anxiety, tension, and other negative emotions, which affect their ability to face the disease and may also lead to aggravation of the original condition and affect oral health. Therefore, it is important to improve the negative psychology of patients with periodontitis to clarify the factors that may lead to negative psychological emotions. AIM To investigate the risk factors that may lead to anxiety and depression in patients with periodontitis. METHODS One hundred patients with periodontitis were selected between March 2022 and March 2023 at our hospital. All patients were assessed with the Zung Self-rating Depression Scale (SDS) (≥ 53 points indicate a depressive state) and Zung Self-rating Anxiety Scale (SAS) (≥ 50 points indicates an anxious state). In this study, patients who experienced anxiety or depression were included in the occurrence group and those without anxiety or depression were included in the non-occurrence group. The baseline data of the two groups were compared to explore the risk factors for anxiety and depression in patients with periodontitis. RESULTS A total of 100 patients with periodontitis were included in this study. According to the SDS, 38 patients (38.00%) developed depression, with an average SDS score of (68.52 ± 5.85) points. According to the SAS, 40 patients (40.00%) developed anxiety, and the average SAS score was (72.15 ± 4.15) points. In this study, 56 patients with anxiety or depression were included. Compared with the non-occurrence group, the occurrence group had higher ages (≥ 60 years), lower level of hope (low level), educational level (high school or below), disease perception (poor), and sleep disorder (yes). The negative coping dimension scores of the simplified coping style questionnaire (SCSQ) and Dental Fear Scale (DFS) in the occurrence group were higher, whereas the score of the positive coping dimension of the SCSQ was significantly lower (P 0.05). The results of multiple logistics regression analysis showed that age (≥ 60 years), level of hope (low level), educational level (high school or below), disease perception (poor), sleep disorder (yes), high negative coping dimension scores of SCSQ, high score of DFS, and low positive coping dimension scores of SCSQ were all factors contributing to the anxiety and depression in patients with periodontitis (odds ratio > 1, P < 0.05). CONCLUSION Age, hope level, educational level, disease perception, sleep disorders, coping style, and dental fear were all associated with anxiety and depression in patients with periodontitis.

Abstract:This study conducted a cross-sectional investigation of facial appearance dissatisfaction between patients before undergoing orthognathic surgery and a non-surgical sample to evaluate the potential influencing factors of facial appearance dissatisfaction. A sample of 354 participants completed a set of questionnaires concerning facial appearance dissatisfaction, interpersonal pressure, media pressure, and fear of negative appearance evaluation (112 patients, 242 controls). The patients reported higher facial appearance dissatisfaction, more media pressure, more interpersonal pressure, and a greater fear of negative appearance evaluation among others than the control group. Moreover, regression analyses identified interpersonal pressure and fear of negative appearance evaluation as the main predictors of facial appearance dissatisfaction whether in the orthognathic patients or the control groups. The associations between the perceptions of interpersonal pressure, fear of negative appearance evaluation, and facial appearance dissatisfaction support the possible utility of strengthening social experiences and psychological intervention in preventing and treating these appearance-concerns, especially for the orthognathic patients.

Abstract:Background The COVID-19 pandemic has a heavy impact on the mental health of elderly surgical patients worldwide. In particular, the elderly patients faced considerable psychological stress due to various environmental and medical factors during the outbreak. This study aims to examine changes in mental health trends among non-cardiac surgical patients aged 65 and above in China during the COVID-19 pandemic. Methods This multi-center, convenient sampling, longitudinal observational study was conducted from April 1, 2020 to April 30, 2022. Primary outcome was the prevalence of postoperative depression. Secondary outcome was the prevalence of postoperative anxiety. Follow-up was conducted separately at 7 days and 30 days after surgery. Depression symptoms were assessed using the Patient Health Questionnaire 9 (PHQ-9) scale. Anxiety symptoms were assessed using Generalized Anxiety Disorder-7 (GAD-7) scale, with scores of ≥5 defining positive depression or anxiety symptoms. Multivariate logistic regression analysis was used to investigate risk factors of mental health status in more elderly patients undergoing non-cardiac surgery. Results A total of 4639 patients were included, of whom 2279 (46.0 %) were male, 752 (15.2 %) were over the age of 75, and 4346 (93.7 %) were married. The monthly prevalence trends demonstrated that compared to the outbreak period, a significant reduction in the prevalence of depression and anxiety symptoms in elderly patients who underwent surgery during the post-pandemic period. In post-pandemic period, a statistically significant decrease in the prevalence of all severity depression and anxiety patients was noted at the 7-day follow-up, but no significant decrease was observed for severe depression and anxiety in the 30-day follow-up. In COVID-19 low-risk area, a significant overall decrease in prevalence of mental health was observed during the post-pandemic period compared to the outbreak period, including 7-day depression, 7-day anxiety, 30-day depression, and 30-day anxiety (all with P < 0.001). Female and patients with ≥2 comorbidities appeared to be more susceptible to postoperative depression and anxiety during the pandemic. Limitation The absence of data from the early days of the COVID-19 outbreak. Conclusions This study analyzed the prevalence of depression and anxiety in elderly non-cardiac patients during and after the COVID-19 pandemic, focusing on dimensions such as severity, risk-areas, gender, and comorbidity. Our findings revealed a significant decrease in the prevalence of depression and anxiety in elderly surgery patients during the post-pandemic period.

Abstract:Cofactor F420 is an electron carrier with a major role in the oxidoreductive reactions of Mycobacterium tuberculosis, the causative agent of tuberculosis. A γ-glutamyl ligase catalyzes the final steps of the F420 biosynthesis pathway by successive additions of l-glutamate residues to F420-0, producing a poly-γ-glutamate tail. The enzyme responsible for this reaction in archaea (CofE) comprises a single domain and produces F420-2 as the major species. The homologous M. tuberculosis enzyme, FbiB, is a two-domain protein and produces F420 with predominantly 5–7 l-glutamate residues in the poly-γ-glutamate tail. The N-terminal domain of FbiB is homologous to CofE with an annotated γ-glutamyl ligase activity, whereas the C-terminal domain has sequence similarity to an FMN-dependent family of nitroreductase enzymes. Here we demonstrate that full-length FbiB adds multiple l-glutamate residues to F420-0 in vitro to produce F420-5 after 24 h; communication between the two domains is critical for full γ-glutamyl ligase activity. We also present crystal structures of the C-terminal domain of FbiB in apo-, F420-0-, and FMN-bound states, displaying distinct sites for F420-0 and FMN ligands that partially overlap. Finally, we discuss the features of a full-length structural model produced by small angle x-ray scattering and its implications for the role of N- and C-terminal domains in catalysis.
