Abstract:During my fellowship, I had the privilege of conducting research in the Medical Genetics lab under the mentorship of Dr. Hope Northrup and Dr. Kit Sing Au. My research focused on genetic variants in folate transporter genes associated with myelomeningocele, 1,2 an experience that ignited my appreciation for the process of discovery involved in research. Although I initially felt uncertain about pursuing a research career due to a relatively late start in the lab, the support of amazing mentors, such as Dr. Louise McCullough, empowered me to establish myself in translational research in congenital heart defects. My current research explores sex differences in the effects of chronic hypoxia on the myocardium in neonates with congenital heart disease. Our recent work published in Pediatric Research reflects the work made possible due to the efforts of our dedicated team behind the Mother Baby Biobank, which I established in 2020. 3 The biobank has become a critical resource that has enabled us to answer pressing clinical questions informed by our experience treating neonates with congenital heart disease. My overarching goal is to improve survival rates and neurodevelopmental outcomes in this vulnerable and high-risk population. To those who may feel daunted by the path of a physician-scientist, my advice is simple: make a little progress each day and find joy in the process as well as the outcomes.

Abstract:I grew up in Brooklyn, New York, and got my undergraduate degree in Biology from Brown University in Providence, Rhode Island. After college, I spent a year working under the mentorship of a congenital heart surgeon, Dr. Richard Hopkins, on developing a tissue-engineered heart valve that could be implanted in a child and grow with them into adulthood. In this role, I spent untold hours at the bench in the laboratory but also in the operating room and in clinical conferences seeing first-hand the value of anchoring research questions in clinical medicine and the role of the physician scientist. I then went to medical school at the Weill Cornell Medical College where my clinical rotations cemented my love for caring for children and their families. After graduation I was fortunate to come to Children's Hospital Colorado for my pediatrics residency, pediatric cardiology fellowship, and advanced training in cardiac imaging and pediatric pulmonary hypertension. As I saw modeled both by my first mentor and my current longtime mentor, Dr. Jesse Davidson, the core inspiration for my current work came from a fundamentally clinical question. As a fellow working in the intensive care unit, I cared for numerous infants and children with single ventricle heart disease having one of their staged palliation surgeries. Time and time again their clinical risk stratification metrics would indicate good surgical candidacy but then in the post-operative period, they would struggle. It was clear that the available predictive tools were insufficient, but no better ones existed. Through many conversations, Dr. Davidson and I mapped a plan to leverage the power of metabolomic and proteomic assays and machine learning algorithms to deep phenotype this population in hopes of developing a more precise risk stratification algorithm and potentially identify novel therapeutic targets to improve post-operative and long-term outcomes. Throughout the roller coaster of translational research, I have found myself repeatedly re-anchoring on this approach and re-focusing on the desire to do better by the patients I am working to serve. As a junior physician scientist, it is, of course, necessary to follow the data and research questions where they lead but also to never forget the patients who inspired you to start on the journey.

Abstract:While I once admired the seemingly linear path of my peers, I now have a growing appreciation of my less linear path. Having personally realized the transformative qualities of music, I too wished to similarly impact others through the power of songwriting. My pursuit of medicine occurred halfway through my undergraduate education upon the realization that this field could more tangibly impact others. Refining my focus to pediatrics and later neonatal-perinatal medicine followed my reflexive desire to care for both the sick newborn and the suffering parent. Music continues to provide a restorative role, offering resilience amidst a highly demanding profession. My interest in pulmonary outcomes and bronchopulmonary dysplasia (BPD) first materialized as a clinical passion for ventilatory management fostered by my co-mentor, Dr. Waldemar Carlo. When taking call, I enjoyed spending the nights familiarizing myself with each infants' lung physiology and utilizing bedside cardiorespiratory data to appraise the success of iterative revisions to respiratory management. This exercise culminated in observational research questions related to respiratory care. Dr. Carlo helped me take these questions, formulate a formal research approach, and translate bedside observations into first author manuscripts in Journal of Perinatology 1 related to oxygen saturation targeting practices by providers and in Pediatric Research 2 regarding noninvasive respiratory support strategies in extremely preterm infants that later progressed to a multi-center investigation within the NICHD Neonatal Research Network relating the duration of noninvasive respiratory support exposure to respiratory outcomes. 3

Abstract:I moved South, and during my Pediatrics residency at the University of Arkansas for Medical Sciences (UAMS) and Arkansas Children's Hospital (ACH), I fell in love with neonatology and caring for critically ill newborns and their families. I was grateful to continue my training at UAMS and ACH as a Neonatal-Perinatal Medicine fellow supported by the program leadership of Angela Chandler MD, where I led a prospective multicenter study evaluating the utility of urine biomarkers and renal near-infrared spectroscopy to identify acute kidney injury (AKI) in neonates with hypoxic ischemic encephalopathy receiving therapeutic hypothermia under the mentorship of Dr. Sherry Courtney. I was grateful to continue my research as an Assistant Professor in the Department of Pediatrics at UAMS. I enjoy the multicenter interdisciplinary collaboration involved with ADVANCE and the Children's Hospitals Neonatal Consortium. My research mentors, Mario Schootman PhD and Whit Hall MD, and my PhD dissertation mentors, Corey Nagel PhD and Robert McGehee PhD, have been instrumental in my research success to date. My excitement currently lies with interrogating large data sets with advanced statistical approaches including machine learning to study neonatal AKI and mortality.

Abstract:Biomedical research is advancing so fast both in range and complexity that young clinicians, necessarily trained as generalists, are daunted by the prospect of grappling with such specialties as molecular biology, genetics, cell biology, biochemistry and immunology. At the same time it is becoming increasingly difficult to obtain funding and support for research projects and periods of study. Fear of leaving the promotional ladder fuelled by the decreasing number of clinical posts available further discourages clinicians from a career in research. This situation must be reversed as a matter of urgency. The qualities required of a good scientist are essentially similar to those of a good diagnostician: sound common sense, tenacity and a stubborn, restless determination to get at the truth. Certainly, it is no easy option. Nevertheless, unless medical practice is to become locked into today's technology, it is essential that applied clinical research is made attractive as a career and that a cadre of young clinicians are encouraged to spend at least three years in full time research.

Abstract:While best practice methodology in animal research aims to address reproducibility and translational issues, awareness and implementation remains low. Preclinical systematic reviews have highlighted many flaws, including issues with internal validity and reporting. With early career researchers (ECRs) heavily involved in all aspects of animal experiments, it is crucial we understand what shapes their research practices. Semi-structured interviews were conducted with 13 ECRs, including research masters, PhD and postdoctoral academics. Data were collected and analysed concurrently using constant comparison techniques and an iterative approach. Findings revealed low-level awareness of best practice recommendations but a desire to engage in dedicated workshops on designing and reporting animal experiments. Current laboratory practices and previous literature were main influences on research practice, more than institutional training. An unexpected finding was the discovery of ethical and emotional dilemmas ECRs faced when working with animals. This highlights the need for a multifaceted approach to better support junior researchers, both emotionally and practically, to encourage responsible science.

Abstract:In this article for the Highlights of 2023 Series, significant advancements in pediatric immunology are discussed, focusing on new diagnostic and therapeutic approaches. Key studies include the integration of genomic and proteomic profiling for better diagnosis of inborn errors of immunity, the impact of nongenetic factors such as autoantibodies on immune responses, the promising use of Janus kinase inhibitors and chimeric antigen receptor‐T cell therapy for treating immune deficiencies and autoimmune diseases and the potential for a curative approach using prime editing. These developments mark a shift toward personalized and precision medicine in pediatric immunology.

A Quality Approach to Blinatumomab Delivery in Pediatric Oncology: A Children's Oncology Group Study

Abstract:Background: Blinatumomab is a promising immunotherapy agent that has been shown to improve survival outcomes in children diagnosed with relapsed B-lineage acute lymphoblastic leukemia. Expanded use of blinatumomab in the treatment of childhood cancer is expected; however, clinician perspectives regarding administering this agent in healthcare and home settings have not been explored. Method: Semistructured interviews were conducted with clinicians (N = 13) from pediatric institutions across the United States and Canada. Qualitative data were analyzed using a constant comparative analysis three-stage method. Results: Participants were primarily nurses (92%), female (77%), and had greater than 10 years of pediatric oncology experience. The selective code and overarching theme identified was "A quality approach to blinatumomab delivery in pediatric oncology." Clinicians described detailed processes that were created and implemented to promote a common goal of safe blinatumomab administration across the continuum of care. Clinicians shared how they engaged in planning activities and considered a variety of factors prior to and during blinatumomab administration. Clinicians also expressed a need to reflect and evaluate on previous patient experiences to create new or revise existing processes and workflows. Communication was also central to clinicians' work. Clinicians provided recommendations to assist others with blinatumomab administration and offered suggestions for items that could help with implementing future clinical trials containing similar agents. Discussion: Findings suggest nurses are pivotal to establishing processes which support safe administration of immunotherapies, such as blinatumomab, while also considering patient-specific needs and promoting quality of life.

Abstract:Research increasingly relies on interrogating large-scale data resources. The NIH National Heart, Lung, and Blood Institute developed the NHLBI BioData CatalystⓇ (BDC), a community-driven ecosystem where researchers, including bench and clinical scientists, statisticians, and algorithm developers, find, access, share, store, and compute on large-scale datasets. This ecosystem provides secure, cloud-based workspaces, user authentication and authorization, search, tools and workflows, applications, and new innovative features to address community needs, including exploratory data analysis, genomic and imaging tools, tools for reproducibility, and improved interoperability with other NIH data science platforms. BDC offers straightforward access to large-scale datasets and computational resources that support precision medicine for heart, lung, blood, and sleep conditions, leveraging separately developed and managed platforms to maximize flexibility based on researcher needs, expertise, and backgrounds. Through the NHLBI BioData Catalyst Fellows Program, BDC facilitates scientific discoveries and technological advances. BDC also facilitated accelerated research on the coronavirus disease-2019 (COVID-19) pandemic.

Abstract:Abstract Brain tumors are the leading cause of disease-related death in children and young adults ages 0-19 in largely populated countries such as the United States. In one year alone, 4,000 children and young adults will be diagnosed with a brain or central nervous system tumor in the United States. Brain tumors are complex and difficult to treat in growing children, with current treatments oftentimes causing significant and lifelong side effects. Furthermore, there have only been five drugs in the last 20 years approved by the FDA to treat pediatric brain tumors. Founded in 2011, the Children’s Brain Tumor Network (CBTN) is focused on accelerating the pace of translational research, the discovery of new treatments, and informing precision medicine for children diagnosed with brain tumors. CBTN comprises 32 member institutions/hospitals having over 4700 patient subjects enrolled, spanning 30+ brain tumor diagnoses, over 66,000 biobanked samples and 150 preclinical models. Longitudinal clinical data is also collected for every subject that is enrolled in the observational protocol. Through large scale data generation efforts funded by the NCI and foundational support, CBTN has whole genome, RNA-seq and other molecular characterization for over half of the enrolled patient population. Additionally, efforts have been underway to collect all pathology and radiology imaging and reporting for the subjects. With sequencing being done by multiple vendors, imaging protocols being different across multiple hospitals, and complex clinical treatment and longitudinal follow up data being translated from EHR systems, CBTN has created a rich, but complex, data landscape that is the largest of its kind in the world. In order to accelerate the process of going from data to cures, the data needs to be centralized, organized, and easily distributable. To do this, CBTN has built a first of its kind data workflow that acts as the inventory system for its various data assets. Using a modern data stack including dbt, PostgresSQL, Meltano and AWS, combined with utilization of FHIR as an interchange standard, data from multiple disparate sources such as REDcap, EHR systems, and PAC systems flow in near “real-time” to be utilized as integrated data resources The result of this modern, multimodal, and multi-institutional warehouse allows CBTN to distribute data quickly and accurately to translational researchers around the world and contribute data to key research efforts such as AACR Project Genie, Kids First Data Resource Center, NCI Childhood Cancer Data Initiative, and the NCI’s Open Targets Platform.
