Abstract:2020 marks the 25th anniversary of the “digital divide.” Although a quarter century has passed, legacy digital inequalities continue, and emergent digital inequalities are proliferating. Many of the initial schisms identified in 1995 are still relevant today. Twenty-five years later, foundational access inequalities continue to separate the digital haves and the digital have-nots within and across countries. In addition, even ubiquitous-access populations are riven with skill inequalities and differentiated usage. Indeed, legacy digital inequalities persist vis-à-vis economic class, gender, sexuality, race and ethnicity, aging, disability, healthcare, education, rural residency, networks, and global geographies. At the same time, emergent forms of inequality now appear alongside legacy inequalities such that notions of digital inequalities must be continually expanded to become more nuanced. We capture the increasingly complex and interrelated nature of digital inequalities by introducing the concept of the “digital inequality stack.” The concept of the digital inequality stack encompasses access to connectivity networks, devices, and software, as well as collective access to network infrastructure. Other layers of the digital inequality stack include differentiated use and consumption, literacies and skills, production and programming, etc. When inequality exists at foundational layers of the digital inequality stack, this often translates into inequalities at higher levels. As we show across these many thematic foci, layers in the digital inequality stack may move in tandem with one another such that all layers of the digital inequality stack reinforce disadvantage.

Abstract:Marking the 25th anniversary of the “digital divide,” we continue our metaphor of the digital inequality stack by mapping out the rapidly evolving nature of digital inequality using a broad lens. We tackle complex, and often unseen, inequalities spawned by the platform economy, automation, big data, algorithms, cybercrime, cybersafety, gaming, emotional well-being, assistive technologies, civic engagement, and mobility. These inequalities are woven throughout the digital inequality stack in many ways including differentiated access, use, consumption, literacies, skills, and production. While many users are competent prosumers who nimbly work within different layers of the stack, very few individuals are “full stack engineers” able to create or recreate digital devices, networks, and software platforms as pure producers. This new frontier of digital inequalities further differentiates digitally skilled creators from mere users. Therefore, we document emergent forms of inequality that radically diminish individuals’ agency and augment the power of technology creators, big tech, and other already powerful social actors whose dominance is increasing.
Digital inequality beyond the digital divide: conceptualizing adverse digital incorporation in the global South

Abstract:Digital systems are significantly associated with inequality in the global South. That association has traditionally been understood in terms of the digital divide or related terminologies whose core conceptualization is the exclusion of some groups from the benefits of digital systems. However, with the growing breadth and depth of digital engagement in the global South, an exclusion worldview is no longer sufficient. What is also needed is an understanding of how inequalities are created for some groups that are included in digital systems. This paper creates such an understanding, drawing from ideas in the development studies literature on chronic poverty to inductively build a model of a new concept: 'adverse digital incorporation', meaning inclusion in a digital system that enables a more-advantaged group to extract disproportionate value from the work or resources of another, less-advantaged group. This new model will enable those involved with digital development to understand why, how and for whom inequality can emerge from the growing use of digital systems in the global South. It creates a systematic framework incorporating the processes, the drivers, and the causes of adverse digital incorporation that will provide detailed new insights. The paper concludes with implications for both digital development researchers and practitioners that derive from the model and its exposure to the broader components of power that shape the inclusionary connection between digital and inequality.

Abstract:As its usage has grown, social media has positively and negatively impacted public health. Although social media presents known harms to mental health and spreads misinformation, it also offers rapid dissemination of public health information, expanded access to health resources, and a robust source of health information. However, these benefits are not equitably shared due in part to the "digital divide" of unequal access and use of information and communication technologies. Measurable inequalities in digital access exist among regions, with an eight-fold difference between Central Africa (9.8% social media penetration) and Northern Europe (80.2% social media penetration). Digital inequalities also differ by gender, age, and socioeconomic classes within countries. Increased digital access has been positively associated with improved health information and outcomes related to chronic diseases, infectious disease outbreaks, and reproductive health. Given the expanding role of social media in public health and the emerging evidence on the health benefits of digital access, we argue for reducing digital inequities by (1) creating an enabling government policy and regulatory environment that views digital health access as a social determinant of health; (2) targeting public and private investments to expand digital access for underserved regions and populations; (3) making digital access and use affordable to low-resource populations; and (4) improving digital competency among these groups through digital literacy programs.

Abstract:Research on digital health equity has developed in important ways especially since the onset of the COVID-19 pandemic, with a series of clear recommendations now established for policy and practice. However, research and policy addressing the health system dimensions of digital health equity is needed to examine the appropriate roles of digital technologies in enabling access to care. We use a highly cited framework by Levesque et al on patient-centered access to care and the World Health Organization's framework on digitally enabled health systems to generate insights into the ways that digital solutions can support access to needed health care for structurally marginalized communities. Specifically, we mapped the frameworks to identify where applications of digital health do and do not support access to care, documenting which dimensions of access are under-addressed by digital health. Our analysis suggests that digital health has disproportionately focused on downstream enablers of access to care, which are low-yield when equity is the goal. We identify important opportunities for policy makers, funders and other stakeholders to attend more to digital solutions that support upstream enablement of peoples' abilities to understand, perceive, and seek out care. These areas are an important focal point for digital interventions and have the potential to be more equity-enhancing than downstream interventions at the time that care is accessed. Overall, we highlight the importance of taking a health system perspective when considering the roles of digital technologies in enhancing or inhibiting equitable access to needed health care.
Digital Literacy in Africa: Exploring Its Relationship with Infrastructure, Policy, and Social Inequality

Abstract:Digital literacy has become indispensable for individuals in the contemporary techno-centric world and imperative for governments to promote equitable growth. Using the fixed effects models to analyze a panel sample of 25 African countries from 2017 to 2021, the authors describe the overall situation of digital literacy in Africa and examine its relationship with ICT infrastructure, policy, and social inequality. The results show that the African countries exhibit a substantial and stable disparity in digital literacy. Availability of ICT infrastructure increases digital literacy, while social inequality decreases it. They also find that digital literacy and digital inclusion policies can overcome the barriers of availability and affordability of ICT infrastructure. In addition, digital literacy and digital inclusion policies have the potential to be mutually supportive, reinforcing each other's role in promoting social equality.

Abstract:Purpose: The general objective of this study was to examine digital divide and social inequality. Methodology: The study adopted a desktop research methodology. Desk research refers to secondary data or that which can be collected without fieldwork. Desk research is basically involved in collecting data from existing resources hence it is often considered a low cost technique as compared to field research, as the main cost is involved in executive’s time, telephone charges and directories. Thus, the study relied on already published studies, reports and statistics. This secondary data was easily accessed through the online journals and library. Findings: The findings reveal that there exists a contextual and methodological gap relating to digital divide and social inequality. Preliminary empirical review revealed that the digital divide exacerbated social inequalities in education, employment, and healthcare, primarily due to disparities in digital access and literacy. Despite technological advancements, marginalized groups continued to face significant barriers in using digital technologies effectively. The COVID-19 pandemic further highlighted these issues, underscoring the necessity for comprehensive strategies that included both digital literacy programs and improved access. The study emphasized the importance of targeted interventions and collaborative efforts to bridge the digital divide and promote social equity. Unique Contribution to Theory, Practice and Policy: The study made significant contributions to theory, practice, and policy. It advanced theoretical frameworks by integrating Knowledge Gap Theory, Social Capital Theory, and Structuration Theory to better understand digital disparities. Practically, it highlighted the need for targeted digital literacy programs and community-based digital hubs. Policy recommendations included prioritizing investments in digital infrastructure, subsidizing digital access for low-income households, and integrating digital literacy into education curricula. The study also emphasized bridging the urban-rural digital divide, enhancing digital equity in education, and developing inclusive digital policies with stakeholder engagement and continuous monitoring.

Abstract:Extant literature has increased our understanding of the multifaceted nature of the digital divide, showing that it entails more than access to information and communication resources. Research indicates that digital inequality mirrors to a significant extent offline inequality related to socioeconomic resources. Bridging digital divides is critical for sustainable digitalized societies. Ιn this paper, we present a literature review of Information Systems research on the digital divide within settings with advanced technological infrastructures and economies over the last decade (2010–2020). The review results are organized in a concept matrix mapping contributing factors and measures for crossing the divides. Building on the results, we elaborate a research agenda that proposes [1] extending established models of digital inequalities with new variables and use of theory, [2] critically examining the effects of digital divide interventions, and [3] better linking digital divide research with research on sustainability.

Abstract:Abstract The pandemic affected more than 1.5 billion students and youth, and the most vulnerable learners were hit hardest, making digital inequality in educational settings impossible to overlook. Given this reality, we, all educators, came together to find ways to understand and address some of these inequalities. As a product of this collaboration, we propose a methodological toolkit: a theoretical kaleidoscope to examine and critique the constitutive elements and dimensions of digital inequalities. We argue that such a tool is helpful when a critical attitude to examine ‘the ideology of digitalism’, its concomitant inequalities, and the huge losses it entails for human flourishing seems urgent. In the paper, we describe different theoretical approaches that can be used for the kaleidoscope. We give relevant examples of each theory. We argue that the postdigital does not mean that the digital is over, rather that it has mutated into new power structures that are less evident but no less insidious as they continue to govern socio-technical infrastructures, geopolitics, and markets. In this sense, it is vital to find tools that allow us to shed light on such invisible and pervasive power structures and the consequences in the daily lives of so many.

Abstract:Rural and marginalized communities in Kenya live in socio-economic backwardness and information poverty, yet they can be empowered digitally to improve their lives on their own by providing access to information and knowledge on using digital tools. Individual-level microdata collected between 2005 and 2010 indicates differences in socio-economic and demographic patterns of technology adoption, usage, and skills over time. Inequality in ICT access, use and skills reflects pre-existing inequality in other areas of economy and society. The ICT sector in Kenya is more active in urban areas, resulting in wide regional disparities in the distribution of ICT facilities. Despite the high overall diffusion rates of ICT in Kenya, there remains a clear digital empowerment gap in access and use between various demographic groups. Household income, education and gender are the key determinants of existing digital inequality.

Abstract:The rapid development in information and communications technologies (ICTs) has created a wealth of opportunities for businesses and societies around the world. Yet, the disparity in the ICT adoption between developed and developing countries, often referred to as the Digital Divide, continues to widen. As a result, the digital divide has remained an issue of significant importance to policy-makers and scholars. In an effort to measure the magnitude of the digital divide and monitor how the disparity evolves over time, the United Nations commissioned the development of a comprehensive ICT Development Index (IDI) in 2009. The objective of this paper is to extend the methodology used in the IDI project and other scientific results presented in previous research to measure the digital divide. Using data mining techniques, we analyze ICT profiles from 154 countries to provide a rigorous quantitative assessment of the digital divide. In addition to analyzing the digital divide at the global level, we present our results at a regional level by identifying countries that are leaders and followers in their respective geographical area. Moreover, our analysis found that between 2002 and 2007, nine countries have made a significant progress in ICT adoption such that they have transitioned into a group previously consisting primarily of developed countries.

Abstract:Purpose Digital inequality is considered one of the leading causes of socioeconomic disparities nowadays and a barrier to sustainable development. However, a dearth of empirical research has examined the impact of digital inequality in attaining sustainable development. This study aims to systematically review the scientific publications on the impact of digital inequality in achieving sustainable development. Design/methodology/approach The preferred reporting items for systematic reviews and meta-analyses (PRISMA, 2020) guidelines were followed to carry out the systematic literature review (SLR) using Scopus, Web of Science, ProQuest and Google Scholar electronic databases. Numerous inclusion/exclusion criteria were employed to obtain the most relevant literature. Finally, 54 articles were included to prepare the final database and qualitative synthesis was performed using 12 variables. Findings While the findings show that there has been a substantial expansion of scientific publications on the focused area in recent years, there is still a lack of empirical and comparative studies; less focus on the offline benefits of online activities were also demonstrated by the results. Moreover, SDGs 04 and 05 were identified as the predominant goals in the literature. Findings further highlighted the importance of an accurate conceptualization of digital inequality. Originality/value In general, this study investigates the level of impact of digital inequality on the United Nations' Sustainable Development Goals. Moreover, it shows the evolution of scientific publications on digital inequality in terms of its contribution when achieving sustainable development.

Abstract:Digital transformation in health care has a lot of opportunities to improve access and quality of care. However, in reality not all individuals and communities are benefiting equally from these innovations. People in vulnerable conditions, already in need of more care and support, are often not participating in digital health programs. Fortunately, numerous initiatives worldwide are committed to make digital health accessible to all citizens, stimulating the long-cherished global pursuit of universal health coverage. Unfortunately initiatives are not always familiar with each other and miss connection to jointly make a significant positive impact. To reach universal health coverage via digital health it is necessary to facilitate mutual knowledge exchange, both globally and locally, to link initiatives and apply academic knowledge into practice. This will support policymakers, health care providers and other stakeholders to ensure that digital innovations can increase access to care for everyone, leading towards Digital health for all.

Abstract:The healthcare industry is constantly evolving to bridge the inequality gap and provide precision care to its diverse population. One of these approaches is the integration of digital health tools into healthcare delivery. Significant milestones such as reduced maternal mortality, rising and rapidly proliferating health tech start-ups, and the use of drones and smart devices for remote health service delivery, among others, have been reported. However, limited access to family planning, migration of health professionals, climate change, gender inequity, increased urbanization, and poor integration of private health firms into healthcare delivery rubrics continue to impair the attainment of universal health coverage and health equity. Health policy development for an integrated health system without stigma, addressing inequalities of all forms, should be implemented. Telehealth promotion, increased access to infrastructure, international collaborations, and investment in health interventions should be continuously advocated to upscale the current health landscape and achieve health equity.

Abstract:Digital competence has been recognized in Europe as one of the 8 key competencies for lifelong learning. The problem of the digital divide as a binary division of the world population into those with and without access to the Internet is still relevant. However, even among the population provided with Internet access, significant differences persist, the so-called second-level digital divide. Digitalization brings enormous opportunities for the education sector with the constructive use of ICT, freeing up time, the most valuable resource in strategizing, from routine tasks to creative, innovative tasks. However, there are also certain threats when the power of this resource is “wasted” on tasks not related to acquiring knowledge and training skills. It seems appropriate, firstly, to develop ICT tools that can reduce the time spent on routine tasks by both teachers and students. Secondly, students need to be encouraged to use ICT regularly, as their intensive use for educational purposes (searching for bibliographic references, using translation software, etc.) leads to improved skills. Thirdly, special programs are needed to reduce the digital divide between students, overcoming both physical inequality (providing schoolchildren from low-income families with access to ICT, high-speed access to educational resources on the Internet) and inequality in skills of ICT usage in the learning process. Both opportunities and threats must be taken into account when strategizing the development of education according to the OTSW methodology proposed by Vladimir L’vovich Quint. At the same time, the human must be at the center of the educational strategizing process.

Abstract:This editorial discusses the transformative potential of digital health literacy and the critical role of electronic medical records (EMRs) in promoting patient empowerment in the healthcare landscape of developing countries. It examines the impact of digital media in healthcare, noting its ability to both democratize access to information and services and pose risks of misinformation among populations with limited health literacy. The discussion includes an overview of key literacy components critical for effectively navigating the digital healthcare ecosystem. Our article highlights the critical role of EMR in facilitating a patient-centered care (PCC) model, with a special emphasis on making EMR systems accessible and user-friendly for vulnerable groups in developing countries. The core aim of our study is twofold: First, it sheds light on the significant challenges - be they technical, financial, or infrastructural - that obstruct the adoption of sophisticated EMR systems in these areas. Second, it explores the essential aspect of digital health literacy, advocating for its improvement as a vital step toward enabling patients to effectively engage with their medical records. By addressing these key issues, our study seeks to illustrate how enhancing digital health literacy, alongside increasing the accessibility of EMR systems, can empower patients in the developing world to actively participate in their healthcare processes. This dual focus aims to contribute to the broader discourse on improving healthcare outcomes through more inclusive and patient-centered approaches, particularly in settings that are currently underserved by modern healthcare technologies. In conclusion, the editorial advocates for a concerted effort toward creating a more inclusive and empowered healthcare paradigm. It suggests integrating PCC principles, tailoring EMR systems to diverse needs, and enhancing digital health literacy as strategies to harness digital health innovations for better healthcare outcomes and equity. Abstract:The disparity in access to essential healthcare resources and services is exacerbated by the digital divide, which presents a significant obstacle to health education. Effective tactics to advance digital equity and provide equitable access to resources for telehealth and digital health are needed to close this gap. Digital databases such as PubMed, MEDLINE, Scopus, Web of Science, and Google Scholar were used to conduct a systematic review. Keywords and Boolean operators including "digital divide," "health education," "digital equity," "telehealth," "digital health literacy," and "strategies" were used in the literature search process. Only peer-reviewed English-language papers that addressed methods for bridging the digital divide in health education were accepted after being screened in accordance with the preset inclusion and exclusion criteria. The results were compiled using a narrative synthesis method after data were retrieved and synthesized with the aid of suitable quality assessment tools. After satisfying the inclusion criteria, seven papers were added to the systematic review. The results emphasized the complexity of the digital divide in health education and provided a range of approaches to mitigate disparities in access to digital health technologies and resources. The importance of digital equality and universal design, continuous intervention evaluation and monitoring, and enduring obstacles to Internet access and healthcare technology availability were among the major themes. This systematic study emphasizes how critical it is to put evidence-based tactics into practice in order to close the digital divide in health education. Through the promotion of universal design principles, ongoing evaluation of treatments, and digital equity, stakeholders can mitigate health disparities, improve population health, and guarantee equitable access to telehealth and digital health services.

Abstract:This research investigates the digital divide in Nepal, a country witnessing rapid growth in digital adoption. Despite impressive internet penetration rates, disparities persist, rooted in socio-economic factors such as income inequality, gender imbalances, educational disparities, and rural-urban distinctions. Drawing on international literature and employing logistic regression analysis on data from the 2022 National Demographic and Health Survey, the study reveals a significant correlation between socio-economic status (SES) and internet access. Results indicate that individuals with higher SES have a 3.92 times greater likelihood of internet access. Additionally, gender disparities emerge, with females exhibiting higher access probabilities. Urban residents and those with higher education levels also show an increased likelihood of internet access. The findings highlight the persistent digital divide in Nepal and emphasize the need for targeted interventions to promote digital inclusivity. This research contributes to international discussions on digital disparities, offering insights into the multifaceted nature of the digital divide and urging for equitable digital opportunities.
