A Novel Multiple Screw Distraction Reducer System in the Treatment of Scoliosis with a Severe Rib Hump

Abstract:Objectives: The treatment of scoliosis with a severe rib hump remains a major challenge. Traditional vertebral rotation techniques are not satisfactory, and thoracoplasty has many pulmonary complications that limit its application. A novel surgical device, the multiple screw distraction reducer (MSDR) system, provides longitudinal distraction during the corrective operation while at the same time providing lateral translation and axial derotation, which may facilitate the correction of a rib hump. This study was performed to investigate the effectiveness of the MSDR system for adolescent idiopathic scoliosis (AIS) with a severe rib hump. Methods: This was a case-matched study of patients with retrospectively collected data from our hospital between January 2017 and December 2021. Sixty-eight patients who underwent one-stage posterior pedicle screw-instrumented spinal fusion were matched by the Cobb angle of the main curve and rib hump. All patients underwent a minimum of 2 years of follow-up. The patients were divided into two groups: the MSDR group (using the MSDR system, n = 34) and the DVR group (using direct vertebral derotation, n = 34). The patients were evaluated for the height of the rib hump, deformity correction, complications, and SRS-30 scores. The unpaired Student's t-test and Pearson's χ2 -test were used to compare the outcome measures between the two groups. Multiple linear regression analysis was used to examine the variables that affected the correction of a rib hump. Results: The rib hump was 30.21 ± 6.21 mm versus 29.35 ± 6.52 mm (p = 0.583) preoperatively and 9.18 ± 4.06 mm versus 13.82 ± 5.54 mm (p < 0.001) at the last follow-up in the MSDR and DVR groups, respectively. The correction rates were 70.83% and 53.56%, respectively (p < 0.001). Preoperatively, the main thoracic curve was 58.43° ± 7.97° and 57.84° ± 6.32° (p = 0.736) and was corrected to 10.92° ± 5.47° and 19.14° ± 5.32° (p < 0.001) at last follow-up in the MSDR and DVR group, respectively. Thoracic kyphosis was restored from 18.24° ± 5.19° and 17.98° ± 5.28° (p = 0.836) in the MSDR and DVR group to 24.59° ± 4.41° and 19.32° ± 4.96° (p < 0.001), respectively. Correction of apical vertebra rotation and translation in the main thoracic curve were significantly better in the MSDR group than in the DVR group (p < 0.05). There was no significant difference in the Lenke type, implant density, estimated blood loss, or follow-up duration between the two groups, whereas the operation time in the DVR group was significantly less than that in the MSDR group. There were only two minor pulmonary complications in the MSDR group. At the last follow-up, the MSDR group scored higher in terms of appearance and satisfaction (p < 0.05). Conclusion: The MSDR system, enabling better coronal alignment, thoracic kyphosis, and axial derotation, could be a safe and effective technique for severe rib hump correction in AIS.

Abstract:BackgroundThe purpose of the animal study is to introduce a newly designed hinged plate and screw system for correction of angular deformities of the lower limbs. The technique was compared with the use of a conventional tension-band plate and screw system.MethodsThis is a randomized controlled animal trial. Eight 3-month-old Bama miniature pigs were used to establish animal models. In each animal, one leg was randomly allocated into study group and another leg into control group. Legs of the study group were corrected with a hinged plate and screw system, and legs of the control group were corrected with a conventional tension-band plate and screw system. The corrective rates of medial slope angle, medial proximal tibial angle, and angle of the two arms of the hinged plate were measured. Residual stress on the implants was also evaluated. A P < 0.05 was statistical significant.ResultsAt the final measurements of 18 weeks, the mean corrective rates of medial slope angle, medial proximal tibial angle, and angle of the two arms of the study group were 0.71°/week, 0.85°/week, and 2.18°/week, respectively; the data in the control group were 0.84°/week, 0.89°/week, and 2.13°/week, respectively. No significant difference was found between the groups regarding the mean corrective rates of the angles (P < 0.05). The mean residual stress in the study group was 643.35 MPa, and measurement in the control group was 1,273.63 MPa, with a significant difference (P < 0.05).ConclusionsCompared to the conventional tension-band plate and screw system, the hinged plate and screw system may be more reliable for correction of angular deformities of the lower limb.

Finite Element Analysis of an Improved Correction System for Spinal Deformity

Abstract:Background/aim: Surgical treatment for spinal deformity aims to correct malformation, release the nerves, and reconstruct spinal stability. To explore and develop a new improved spinal correction system (ISCS) for clinical application, we studied the stability and biomechanical characteristics of the ISCS through finite element analysis and comparison of the ISCS with the pedicle screw and rod system (PSRS). Patients and methods: Using L1-L3 CT image data of a normal adult male lumbar spine for establishment of L1-L3 finite element model, we established posterior internal fixation models for a comparative finite element analysis of PSRS and ISCS. An axial load of 500 N and a moment of 10 N•m were applied to L1 to simulate flexion, extension, lateral bending, and axial rotation. Stress distribution characteristics, load sharing, strain bending stiffness and strain angle change of the models were measured. Results: In flection and extension directions, the maximum stress of the L2 vertebral body and the L1/2 and L2/3 discs in PSRS was less than that of ISCS. In lateral bending and axial rotation directions, the maximum stress between PSRS and ISCS was similar. However, the stress shielding rate of L2, L1/2, and L2/3 intervertebral discs in ISCS was significantly lower than that of PSRS. We also found that both models had similar angular displacement and maximum displacement in lateral bending direction, but PSRS had a lower angular displacement and maximum displacement in flection and extension directions. Finally, we showed that PSRS had similar angular displacement and a lower maximum displacement compared with ISCS in axial rotation, whereas ISCS had lower bending stiffness than PSRS in different directions. Conclusion: ISCS can effectively fix spinal deformities compared to PSRS. ISCS provides a new option for orthopedic surgery treatment of scoliosis and, therefore, warrants further clinical studies in patients with other spinal deformities.
Reassessment System and Staged Surgical Strategy with Minimally Invasive Techniques for Treatment of Severe Adult Spinal Deformities

Abstract:OBJECTIVE: We determined the feasibility of minimally invasive surgery for severe adult spinal deformities and proposed a reassessment system and staged minimally invasive surgical strategy. METHODS: We treated 53 patients from June 2016 to August 2017 using a reassessment system and a staged surgical strategy with minimally invasive techniques for minimally invasive spinal deformity surgery, class III. Patients with scoliotic apex vertebrae above L1-L2 or spontaneous fusion of the facet joints were excluded. The reassessment system was applied. The first stage involved multisegmental lateral lumbar interbody fusion and anterior column realignment (ACR), and the second stage involved posterior minimally invasive surgery. The pre- and postoperative visual analog scale scores, Oswestry disability index, surgical data, and radiographic images were collected. The major and minor complications were recorded. RESULTS: All the patients were followed up for 11.5 months (range, 6-20). Lateral lumbar interbody fusion was performed in 168 segments, of which 113 had ACR. The average sagittal correction angle of each ACR segment was 15.6 degrees +/- 6.3 degrees (range, 7 degrees-28 degrees). The correction rate of the Cobb angle for lumbar scoliosis after first stage was 55.4%, and the total correction rate was 75.6%. The lumbar lordosise-pelvic index mismatch was improved from -32.8 degrees +/- 14.9 degrees to -2.5 degrees +/- 9.4 degrees, reaching the sagittal matching state of the lumbar spine. CONCLUSIONS: A reassessment system and minimally invasive staged surgery for severe adult spinal deformity can achieve good clinical outcomes and deformity correction and might have the advantage of decreasing unnecessary iatrogenic injury.
Three-dimensional correction of scoliosis by a double spring reduction system as a dynamic internal brace: a pre-clinical study in Göttingen minipigs

Abstract:Background context: Adolescent idiopathic scoliosis (AIS) is a major skeletal deformity that is characterized by a combination of apical rotation, lateral bending and apical lordosis. To provide full 3D correction, all these deformations should be addressed. We developed the Double Spring Reduction (DSR) system, a (growth-friendly) concept that continuously corrects the deformity through two different elements: A posterior convex Torsional Spring Implant (TSI) that provides a derotational torque at the apex, and a concave Spring Distraction System (SDS), which provides posterior, concave distraction to restore thoracic kyphosis. Purpose: To determine whether the DSR components are able to correct an induced idiopathic-like scoliosis and to compare correction realized by the TSI alone to correction enforced by the complete DSR implant. Study design/setting: Preclinical randomized animal cohort study. Patient sample: Twelve growing Göttingen minipigs. Outcome measures: Coronal Cobb angle, T10-L3 lordosis/kyphosis, apical axial rotation, relative anterior lengthening. Methods: All mini-pigs received the TSI with a contralateral tether to induce an idiopathic-like scoliosis with apical rotation (mean Cobb: 20.4°; mean axial apical rotation: 13.1°, mean lordosis: 4.9°). After induction, the animals were divided into two groups: One group (N=6) was corrected by TSI only (TSI only-group), another group (N=6) was corrected by a combination of TSI and SDS (DSR-group). 3D spinal morphology on CT was compared between groups over time. After 2 months of correction, animals were euthanized. Results: Both intervention groups showed excellent apical derotation (TSI only-group: 15.0° to 5.4°; DSR-group: 11.2° to 3.5°). The TSI only-group showed coronal Cobb improvement from 22.5° to 6.0°, while the DSR-group overcorrected the 18.3° Cobb to -9.2°. Lordosis was converted to kyphosis in both groups (TSI only-group: -4.6° to 4.3°; DSR-group: -5.2° to 25.0°) which was significantly larger in the DSR-group (p<.001). Conclusions: The TSI alone realized strong apical derotation and moderate correction in the coronal and sagittal plane. The addition of distraction on the posterior concavity resulted in more coronal correction and reversal of induced lordosis into physiological kyphosis. Clinical significance: This study shows that dynamic spring forces could be a viable method to guide the spine towards healthy alignment, without fusing it or inhibiting its growth.
Posterior Double Vertebral Column Resections Combined with Satellite Rod Technique to Correct Severe Congenital Angular Kyphosis.

Abstract:This paper presents a highly challenging technique involving posterior double vertebral column resections (VCRs) and satellite rods placement. This was a young adult case with severe angular thoracolumbar kyphosis of 101 degrees, secondary to anterior segmentation failure from T11 to L1 . There were hemivertebrae at T11 and T12 , and a wedged vertebra at L1 . He received double VCRs at T12 and T11 and instrumented fusion from T6 to L4 via a posterior only approach. Autologous grafts and a cage were placed between the bony surfaces of the osteotomy gap. Once closure of osteotomy was achieved, bilateral permanent CoCr rods were placed with addition of satellite rods. Postoperative X-ray demonstrated marked correction of kyphosis. On the 10(th) days after surgery, the patient was able to walk without assistance. In conclusion, double VCRs are effective to correct severe angular kyphosis, and addition of satellite rods may be imperative to enhance instrumentation strength and thus prevent correction loss.
Unilateral Posterior Vertebral Column Resection For Severe Thoracolumbar Kyphotic Deformity Caused By Old Compressive Vertebrae Fracture: A Technical Improvement

Abstract:Severe thoracolumbar kyphotic deformity caused by old compressive vertebrae fracture remains a big challenge for spine surgeons. When symptoms related to significant deformities cannot be adequately managed conservatively, posterior vertebral column resection (PVCR) is required, but with long operating time and severe blood loss. We develop a UPVCR technique, which is done through a unilateral approach instead of a bilateral approach, vertebral body resection advancing to cross the midline in an abrasive way from an extreme oblique orientation enable the resection of most contralateral vertebral body. In the present study, the effects of UPVCR for severe thoracolumbar kyphotic deformity were investigated. We did find that satisfactory correction of sagittal deformity, functional improvement and pain relief can be achieved by UPVCR, and it has the advantage of shortening surgery time, reducing blood loss and incidence of nerve root impingement over PVCR.
Single-stage Anterior-Posterior or Posterior-Alone Hemivertebra Resection for Congenital Scoliosis

Abstract:Objective:To evaluate the efficacy and safety of single-stage anterior-posterior or posterior-alone hemivertebra resection for congenital scoliosis retrospectively.Method:From April 2003 to August 2007,32 congenital scoliosis patients undergoing single-stage anterior-posterior or posterior-alone hemivertebra resection were included in this study.The mean age at time of surgery was 12.8 years old (range,9 -22 years).13 patients had hemivertebre at T9 -L2 with the bottom of hemivertebra orientated ventrally and underwent single-stage anterior-posterior hemivertebra resection(anterior-posterior group),the other 19 patients had single hemivertebra at T10 -L3 with the bottom of hemivertebra orientated dorsally experienced single posterior surgery.The operation time,blood loss,correction rate of main curve and complication were reviewed and compared between 2 groups.Result:The mean operation time and blood loss for anterior-posterior group and posterior group was 5.4h/580ml and 4.3h/470ml respectively,which had significant difference (P 0.05).The average correction rate of main curve for anterior-posterior group and posterior group was 64.1% and 63.7% (P 0.05) respectively.Follow-up period averaged 18 months.No significant loss of correction was found. Complication in 2 groups was rare.Conclusion:Hemivertebra resection in thoracolumbar region is effective for congenital scoliosis.Preoperative evaluation of hemivertebra and careful maneuver can ensure the procedure safe and successful.

Abstract:Study Design: An animal study. Objective: To create a reliable porcine scoliosis model representative of early-onset scoliosis (EOS) without violation of the vertebral elements along the curve. Summary of the Background Data: To develop new nonfusion techniques for the treatment of EOS, a reliable large animal model with remarkable growth potential is required. However, a long tethering period which consumed the majority of the rapid growth phase or violation of the vertebral elements was thought to be essential in most of the previous models. Therefore, these models may be suboptimal for mimics of human EOS which was usually idiopathic type without vertebral anomalies. Materials and Methods: This study included 12 female Yorkshire pigs (aged, 5-6 wk; weight, 5-7 kg) in which scoliosis was created by posterior asymmetric tethering from T5 to L3. At the index surgery, 3 separate incisions were preformed, and ipsilateral rib tethering from the 10th to the 13th rib was performed while maintaining the vertebral elements along the maximal curve in a pristine state. Progressive deformity was documented with monthly radiographs. Frontal and sagittal profiles were assessed using the Cobb method. After an 8-week tethering period, the whole instrumentations were removed, and the pigs were observed for an additional 8-week period with serial radiographs to document the progression of the deformity. Results: Of the 12 pigs enrolled in this study, 2 encountered substantial complications (1 developed a postoperative in-fection, and the other experienced prolonged postoperative weakness). Of the 10 available for analysis, all pigs developed rapidly progressive, structurally 3-dimensional, idiopathic-type curves with convex to the right in the lower thoracic spine. The mean coronal Cobb angle was 29 degrees immediately post-operatively and progressed to 65 degrees after the 8-week tethering period. Eight weeks after removal of the tether, the scoliosis continued to progress and averaged 68 degrees (range, 58-78 degrees). On the sagittal plane, a mean lordosis of 32 degrees at the thoracic spine and a thoracolumbar kyphosis of 63 degrees were observed at study completion. Conclusions: A 3-dimensional rapidly progressive scoliosis model, that is closely approximate to human EOS, can be successfully created in pigs by unilaterally tethering the thoracolumbar spine and the ribcage. This model provides an equivalent EOS-like deformity and leaves adequate skeletal growth potential for biomechanical research as well as validation of fusionless scoliosis correction systems.
Sequential Correction: a Reliable and Simple Technique for Complex Spine Deformity

Abstract:Based on the coronal balance classification for adult spinal deformity established by our center, a new surgical technique for adult spinal deformity was further proposed , namely Sequential Correction. Spine deformity was classified as thora?columbar/lumbar type and lumbosacral type according to the driver of the deformity. A short rod was firstly installed to correct the driver, followed by another short rod installed on the other side, and two long rods were installed at last. The incidence of postoper?ative coronal imbalance was significantly reduced using sequential correction, and the correction rate was significantly improved compared with the traditional technique.

Vertebral Coplanar Alignment for Correction of Thoracic Scoliosis: Techniques and Results

Abstract:Objective To prospectively explore the techniques and correction results of the method of vertebral coplanar alignment (VCA) in posterior correction for thoracic scoliosis. Methods Between June 2008 to March 2009, 27 patients with idiopathic thoracic scoliosis were selected to undergo posterior pedicle screw fixation with assistance of Coplanar system. There were 26 females and 1 male with the average age of 15.9 years(ranged 11-23 years). There were type 1 in 25 cases and type 2 in two cases. The levels of fusion were established according to the Lenke criteria. The average preoperative coronal Cobb angle was 49°(ranged 40°-70° ). During the procedure, the pedicle screws were inserted in each involved levels on the convex side, an extended Coplanar tube was screwed in line with screw axis to each screws. Then two rigid bars were inserted through the uppermost part of the slotted tube sequentially. As the bar was gently driven down toward the bottom end, the pedicle screws axis started to converge in the straight line, correcting translation and rotation. Spacers were inserted into the slots of the tubes to achieve the ideal physiologic thoracic kyphosis. Once the rod was fixated in the concave side of the main curve, the Coplanar system could be removed. Results The mean postoperative Cobb angle was 14° (ranged 6°-25°), representing the correction rate of 70.7%. The average duration of surgery was 245 min (ranged 210-300 min) and a mean estimated blood loss was 1500 ml (ranged 600-2100 ml). The duration of follow-up averaged 15 (12-18) months. The loss of correction at last follow-up was 3° with the loss of correction rate of 6.1%. In the sagittal plane, the thoracic kyphosis was restored from 18° to 25°. No coronal or sagittal plane decompensation occurred. No thoraeoplasties were necessary to correct the residual rib hump. There was no death, infection and neurological complications. Conclusion The Coplanar is a new technique for the correction of scoliosis, its advantage lies in excellent and modulatable three-dimensional correction with a single and standardized maneuver.
Remodelling of vertebral endplate subchondral bone in scoliosis: A micro-CT analysis in a porcine model

Abstract:BACKGROUND: Disc degeneration has been correlated with alteration of bone density of adjacent vertebral bodies. Abnormal mechanical loading appears in scoliosis as compared to normal spines. How vertebral endplate was remodelled in scoliosis is not well understood.METHODS: We conducted a micro-CT analysis of subchondral bone of the vertebral endplate at the curve apex in a porcine scoliosis model. Two adjacent thoracic T(5)-T(6) and lumbar L(1)-L(2) levels were instrumented in six four-week-old pigs with a custom offset implant connected by a flexible stainless steel wire. Two months after implantation, three cylindrical specimens were harvested into the vertebral endplate of each of the scoliosis levels: centre, convexity and concavity, and from the dorsal T(9)-T(10) vertebral units obtained from nine three-month-old non-instrumented pigs used as controls. Micro-CT analysis was carried out on each specimen.FINDINGS: In the concavity of the scoliotic spine, bone volume fraction, trabecular thickness, and trabecular separation significantly increased whereas in the convexity, only trabecular separation increased. Connectivity index and trabecular number decreased significantly.INTERPRETATION: This was the first micro-CT study of subchondral bone microarchitecture of the scoliotic vertebral end plate. At the curve apex, increased compression in the concavity induced an osteogenic process. In the convexity, diminished compression caused an osteolytic process with a local resorption. Clinically, the unbalanced tissue remodelling could play a role in the convective and diffusive transports into the end plate, which is of prime importance for the segment homeostasis in scoliosis treatment with or without surgery.

Abstract:ObjectiveAnterior cervical corpectomy and fusion (ACCF) has been widely used in the treatment of cervical spondylotic myelopathy (CSM) but is accompanied by unavoidable motion loss and destruction of vertebra. We aim to evaluate the range of motion (ROM) of caprine cervical spine constructs implanted with cervical artificial disc and vertebra system (ADVS). The purpose of this study was to investigate the biomechanical properties of the ADVS from an in vivo caprine cervical spine non-fusion model.MethodsTwelve goats were randomly divided into ADVS or control group, with 6 animals in each group. The animals in the ADVS group were implanted with ADVS at the C4 level. The cervical spine constructs were harvested 6 months after the operation. The ROM of cervical spine specimens in the ADVS group was recorded. Biomechanical testing of the specimens in the control group were conducted to evaluate the ROM of the cervical spine specimens under intact and fixed condition (C3-C5) by an anterior plate, respectively.ResultsThe biomechanical outcomes showed that the ROM of the levels (C3-C5) implanted with ADVS was maintained. The ROM in the adjacent level (C2-3) did not increase significantly comparing with intact group.ConclusionsIn general, ADVS could preserve the ROM of operative levels and could reconstruct the height of the vertebra. ADVS did not increase the ROM of upper adjacent level. This device provides a non-fusion method for the treatment of patients suffering from CSM. However, improvements on the design of ADVS are still needed.Translational potential statementThis study introduced a novel cervical spinal implant, which was designed to have the ability of motion preservation and vertebra construction. Our study provided a non-fusion procedure in the treatment of CSM after ACCF.

Evaluation and Comparation of a Novel Surgical Technique and Hemivertebra Resection to the Correction of Congenital Cervical Scoliosis in Lower Cervical and Cervicothoracic Spine

Abstract:Objective To report concave-side distraction technique to treat congenital cervical scoliosis in lower cervical and cervicothoracic spine. To evaluate and compare clinical and radiographic results of this procedure with classic hemivertebra resection procedure. Methods This study reviewed 29 patients in last 13 years. These patients were divided into convex-side resection group (group R) and concave-side distraction group (group D). Radiographic assessment was based on parameter changes preoperatively, postoperatively and at last follow-up. Demographic data, surgical data and complications were also evaluated and compared between the 2 groups. Results In group R, mean age was 8.9 ± 3.3 years and follow-up was 46 ± 18 months. Operation time and blood loss averaged 500 ± 100 minutes, 703 ± 367 mL. In group D, mean age was 9.9 ± 2.8 years and follow-up was 34 ± 14 months. Operation time and blood loss averaged 501 ± 112 minutes, 374 ± 181 mL. Structural Cobb angle was corrected from 29.4° ± 12.5° to 5.3° (2.1°–18.1°) (p = 0.001) and 33.7° ± 14.1° to 12.8° ± 11.4° (p < 0.001) in groups R and D. Compensatory Cobb angle had a spontaneous correction rate of 59.6% (40.0%–80.8%) and 59.7% ± 23.0% in groups R and D. Mandibular incline, clavicle angle and spine coronal balance were significantly improved at last follow-up in both groups. All correction rates were not statistically different between groups. However, group D had significant less blood loss (p < 0.001) and operation time (p = 0.004) per vertebra than group R. Seven patients developed C5 nerve root palsy and recovered by 6 months of follow-up. Conclusion Both surgical procedures are safe and effective in correcting congenital cervical scoliosis. But concave-side distraction technique has less blood loss and time-consuming during surgery, which provides a better option for the treatment of congenital cervical scoliosis.
Posterior Multisegment Apical Convex plus Concave Intervertebral Release Combined with Posterior Column Osteotomy for the Treatment of Rigid Thoracic/Thoracolumbar Scoliosis.

Abstract:BACKGROUND:Intervertebral release (IVR) in the apical region is critical for full release of a rigid spine. Previous studies have mainly reported IVR techniques using an anterior approach or posterior apical convex IVR. We first report the surgical procedure of posterior multisegment apical convex plus concave IVR combined with posterior column osteotomy (PCO) for treating rigid thoracic/thoracolumbar scoliosis. METHODS:This study retrospectively analyzed clinical, radiologic outcomes and technique notes of 18 patients with rigid scoliosis treated with posterior multisegment convex plus concave IVR combined with PCO. RESULTS:The preoperative, postoperative, and final follow-up mean sagittal Cobb angles of the main curve were 75.2° (58.7°-110.2°), 18.4° (9°-35.1°), and 19.0° (8.2°-36.3°), respectively. The mean correction rate was 75.3% (66.7%-86.7%). In cases of thoracolumbar kyphosis, the preoperative, postoperative, and final follow-up mean sagittal Cobb angles were 45.7° (40.5°-52.6°), 18.8° (10.2°-27.5°), and 19.8° (11.1°-29°), respectively. The mean correction rate was 57% (42.1%-72.6%). The mean axial vertebral rotation (AVR) in the IVR region was 24.4° (14.3°-46.3°) preoperatively and was corrected to 10.9° (10.9°-26.6°) postoperatively. The mean correction rate for AVR was 55.9% (41.1%-78.6%). The coronal and sagittal Cobb angles and AVR postoperatively were significantly lower than those preoperatively (P < 0.001). This case series reported 2 cases of pleural effusion and 1 case of wound infection. CONCLUSIONS:Single posterior multilevel apical convex plus concave IVR combined with PCO is a safe and effective surgical method for treating rigid thoracic/thoracolumbar scoliosis that does not need 3-column osteotomy.

Abstract:Little research has been reported on evaluating the safety of the fixation construct in cervical kyphosis correction. In this study, we proposed a principal-strain criterion to evaluate the safety of the fixation construct and validated the modeling method against a retrospective case of anterior cervical discectomy fusion (ACDF). From C2 to T2 vertebra bodies, fixation instruments were reconstructed and positioned as per postoperative computed tomography (CT) scans. Head weight (HW) and various moments estimated from isometric strength data were imposed onto the C2. The postoperative stability of non-surgical segments, deformations surrounding the screw trajectories, and contact slipping on zygapophysial joints were analyzed. The model was validated against the reality that the patient had a good fusion and deformity correction. The ACDF restricted the range of motions (ROMs) of cervical segments and lent stability to vertebra fusion, no failure was found in the finite element (FE) model of cervical vertebrae. The deformation surrounding the screw trajectories were concentrated to the lateral sides of trajectories, recommending that the shape of the anterior cervical plate conforming to the curvature of the vertebra and screws fully inserted into vertebrae reduced the deformation concentration around the screw trajectories.
