Abstract:Objectives:To investigate the surgical outcome and complications of growing rods in the treatment of early onset scoliosis(EOS) with type 1 neurofibromatosis.Methods:Thirteen cases of EOS with type 1 neurofibromatosis treated with growing rods were studied retrospectively.There were 8 boys and 5 girls,aged from 5 to 10 years old(average,7.5±2.1 ys).The average follow-up was 5.1±2.8 years(2-10 years).Exptending procedure was carried out every 6 months after first time internal fixation.The Cobb angle,kyphosis angle and trunk height were measured,and the complications were also evaluated.Results:The mean Cobb angle was corrected from 69.0°±21.2°(43°-90°) preoperatively to 25.3°±15.0°(12°-45°) at the final follow-up,the correction rate was 63.3％.The trunk height was corrected from 323.0±29.1mm(288-355mm) to 400.5±25.7mm (361-430mm),and spine grew an average of 11.7±3.3mm per year.There were 16 complications in 9 cases (69.2％),with implant failures in 9 cases(56.3％),junctional kyphosis in 5 cases(31.3％),and wound dehiscence and deep wound infection in 1 case(6.2％).Among 9 cases with kyphosis angle less than 50°,complication was found in 6 cases (66.7％);among 4 cases with kyphosis angle more than 50°,complication was found in 3 cases (75％).There was no significant difference of complication rate between kyphosis more than 50° group and that less than 50°group(x2=0.123,P=-0.726).Conclusions:The use of growing rods in patients with EOS associated with type 1 neurofibromatosis provides curve correction and allows the growth of spine.Kyphosis more than 50° does not increase complications.
A Preliminary Study on a Novel Growth Guidance Rod System for Early-Onset Scoliosis in a Sheep Model

Abstract:Study design: An animal study with immature sheep to evaluate the effects of a multisegment growth guidance rod (MSGGR) on spine growth. Objective: To determine whether the spine of the immature sheep can still grow after MSGGR fixation. Summary of background data: The disadvantages for current growing rod techniques are that they can partially correct only the spinal curve and have little control to the apex of the curve. The rigidity of the spine after a growing rod procedure may also interfere with the final correction. Current systems are complex and not always affordable, especially in the developing world. Newer, more inexpensive techniques that provide 3-dimensional deformity correction while allowing normal spinal growth without surgical lengthening are still desired. Methods: The MSGGR is a rod consisting of segments. Spinal deformation in scoliosis is corrected and maintained by the rods without fusion. The system allows the growth of the fixed spinal segments. It is stable when twisted and bent but extendable when stretched. Rod extension occurs through sliding between the segments along the sockets in accordance to the growth of the spine. Ten 3-month-old immature sheep were used in this study. Dual MSGGRs were implanted to fix the lumber and low thoracic spine. Radiographs, magnetic resonance image, and computed tomographic scan of the spine were obtained to evaluate the fixation, rod extension, and spine health. Results: All of the sheep spines grew with the implants in position. The spine segments within the instruments were 12.5 ± 0.8 cm and grew by 10.9% (range: 6%-18.4%) from their original length in 4 months. None of the implants failed. No MSGGR-related complications were observed. Magnetic resonance imaging showed normal disc within the instrumented segments. Motion of the instrumented spinal segments was conserved. Conclusion: Growth guidance with this novel MSGGR allowed for continued growth in this sheep model, and repeated surgical lengthening of the system is not needed. Level of evidence: N/A.

Abstract:The application of a growing rod technique can retain the growth and development potential of the spine and thorax while controlling the progression of scoliosis deformity. Theoretically, convex side short fusion combined with a concave side single growing rod technique can significantly reduce the asymmetric growth of the spine in the vertex region in most patients. However, the final clinical outcome of various techniques is yet to be clearly determined and compared between studies. Therefore, we compared the efficacy of these two growing rod techniques in treating early onset scoliosis.

Abstract:Surgical treatment of early-onset scoliosis (EOS) remains challenging as no definitive surgical technique has emerged as the single best option in this varied patient population1-3. Although the available surgical techniques may differ substantially, they all share the same goals of achieving and maintaining deformity correction, allowing physiological spinal growth, and reducing the number of operations and complications. Herein, we present a modified self-growing rod technique that represents a valid alternative to the existing surgical procedures for EOS. Description: The patient is positioned prone on a radiolucent table, and the spine is prepared and draped in a standard fashion. A posterior midline skin incision is made from the upper to the lower instrumented level. Subperiosteal exposure of the spine is carried out, ensuring that capsules of the facet joints are spared. Pedicle screws are inserted bilaterally at the cranial and caudal ends of the instrumentation. Fixation with pedicle screws of at least 3 levels at the top and bottom end is usually advised; in nonambulatory patients with pelvic obliquity, caudal fixation can be extended to the pelvis with bilateral iliac screws. Sublaminar wires are positioned bilaterally at every level between the cranial and caudal ends of the instrumentation and are passed as medially as possible to avoid damage to the facet joints. Four 5-mm cobalt-chromium rods are cut, contoured, and inserted at each end of the construct. Ipsilateral rods are secured with use of sublaminar wires, making sure that they overlap over a sufficient length to allow for the remaining spinal growth. Correction of the deformity is achieved with use of a combination of cantilever maneuvers and apical translation by progressive and sequential tightening of the sublaminar wires. The wound is closed in layers over a subfascial drain. The patient is allowed free mobilization after surgery. No postoperative brace is required. Alternatives: Nonoperative alternative treatment for EOS includes serial cast immobilization and bracing4. Alternative surgical treatments include traditional growing rods5, magnetically controlled growing rods6, the vertical expandable prosthetic titanium rib-expansion technique7, and the Shilla technique8. The use of compression-based systems (i.e., staples or tether)9 or early limited fusion has also been reported by other authors. Rationale: The main advantage of our technique is that it relies on physiological spinal growth and does not require surgery or external devices for rod lengthening, which is particularly beneficial in frail patients with a neuromuscular disease in whom repeated surgery is not advised. Segmental fixation by sublaminar wires allows good control of the deformity apex during growth. Concerns regarding early fusion of the spine have not been confirmed in our mid-term follow-up study10. Expected outcomes: This technique allows correction of the deformity and continuous spinal growth in the years following surgery. At 6.0 years postoperatively, the average main curve correction was reported to be 61% and the average pelvic obliquity correction was 69%. The spine was reported to lengthen an average of 40.9 mm (range, 14.0 to 84.0 mm) immediately postoperatively, and the T1-S1 segment was reported to continue growing at 10.5 mm/year (range, 3.6 to 16.5 mm/year) thereafter10. The most common complication is rod breakage at the thoracolumbar junction, which seems to be more common in patients with idiopathic or cerebral palsy EOS and during the pubertal growth spurt10. Important tips: Subperiosteal exposure of the spine should be carried out, making sure to preserve facet joints in the unfused area of the spine.Achieve segmental fixation with use of sublaminar wires at every level and pedicle screws at the top and bottom ends of the instrumentation.If pelvic imbalance is present and the patient is nonambulatory, pelvic fixation with iliac screws is advised.First round correction of the deformity is achieved with a cantilever technique; correction fine-tuning can be performed by tightening sublaminar wires.Consider utilizing thicker rods in cases of idiopathic or cerebral palsy EOS. Abstract:Study Design: An animal study. Objective: To create a reliable porcine scoliosis model representative of early-onset scoliosis (EOS) without violation of the vertebral elements along the curve. Summary of the Background Data: To develop new nonfusion techniques for the treatment of EOS, a reliable large animal model with remarkable growth potential is required. However, a long tethering period which consumed the majority of the rapid growth phase or violation of the vertebral elements was thought to be essential in most of the previous models. Therefore, these models may be suboptimal for mimics of human EOS which was usually idiopathic type without vertebral anomalies. Materials and Methods: This study included 12 female Yorkshire pigs (aged, 5-6 wk; weight, 5-7 kg) in which scoliosis was created by posterior asymmetric tethering from T5 to L3. At the index surgery, 3 separate incisions were preformed, and ipsilateral rib tethering from the 10th to the 13th rib was performed while maintaining the vertebral elements along the maximal curve in a pristine state. Progressive deformity was documented with monthly radiographs. Frontal and sagittal profiles were assessed using the Cobb method. After an 8-week tethering period, the whole instrumentations were removed, and the pigs were observed for an additional 8-week period with serial radiographs to document the progression of the deformity. Results: Of the 12 pigs enrolled in this study, 2 encountered substantial complications (1 developed a postoperative in-fection, and the other experienced prolonged postoperative weakness). Of the 10 available for analysis, all pigs developed rapidly progressive, structurally 3-dimensional, idiopathic-type curves with convex to the right in the lower thoracic spine. The mean coronal Cobb angle was 29 degrees immediately post-operatively and progressed to 65 degrees after the 8-week tethering period. Eight weeks after removal of the tether, the scoliosis continued to progress and averaged 68 degrees (range, 58-78 degrees). On the sagittal plane, a mean lordosis of 32 degrees at the thoracic spine and a thoracolumbar kyphosis of 63 degrees were observed at study completion. Conclusions: A 3-dimensional rapidly progressive scoliosis model, that is closely approximate to human EOS, can be successfully created in pigs by unilaterally tethering the thoracolumbar spine and the ribcage. This model provides an equivalent EOS-like deformity and leaves adequate skeletal growth potential for biomechanical research as well as validation of fusionless scoliosis correction systems.
Hybrid Growing Rod Technique of Osteotomy with Short Fusion and Spinal Distraction: An Alternative Solution for Long-spanned Congenital Scoliosis.

Abstract:Study Design. Retrospective study. Objective. The current study aimed to evaluate the effectiveness of the hybrid growing rod (GR) technique for the treatment of young children with congenital scoliosis (CS). Summary of Background Data. Congenital vertebral anomalies within long-spanned kyphoscoliosis were difficult to be managed by either definitive spinal fusion or standard GR technique. Hybrid technique of one-stage posterior osteotomy with short fusion and GR distraction was proposed in previous studies. There is, however, paucity of data regarding its effectiveness. Methods. Thirteen patients (mean age, 5.4 +/- 2.0 yr) who had undergone hybrid GR treatment were retrospectively reviewed, including 8 patients treated with single GR and 5 patients with dual GR. All of their radiographic data were measured, and surgical complications were recorded. Results. On average, the current cohort had 4.1 +/- 1.7 lengthening procedures. After the index surgery, the major curve improved remarkably from 86.48 +/- 11.98 to 37.38 +/- 12.48 (P<0.001). The global kyphosis significantly decreased from 66.88 +/- 16.18 to 33.38 +/- 3.58 postoperatively. Both value remained steady during follow-up. There were no significant difference between single GR and dual GR groups in terms of the coronal deformity correction, whereas correction loss of global kyphosis in single GR group was significantly higher than that in dual GR group (P = 0.039). After the index surgery, T1-S1 height averagely gained 4.4 +/- 0.8cm and it grew at an average rate of 1.31 +/- 0.24 cm/yr during the follow-up. Two complications were identified in two patients, including one with rod fracture and one with proximal junctional kyphosis. Conclusion. The hybrid GR technique was effective in correcting spinal deformity and allowing continuous spinal growth. The optimal indications were young children with apical vertebrae anomalies and a long-spanned kyphoscoliosis.
Outcome of Traditional Growing Rods for Correction of Apical Vertebra Rotation in Early-Onset Scoliosis

Abstract:Objective To evaluate the correction result of traditional dual growing rods on apical vertebral rotation. Methods This study recruited 19 early-onset scoliosis patients(6 boys and 13 girls) who had received traditional dual growing rods treatment at Department of Spine Surgery,Nanjing Drum Tower Hospital from January 2009 to July 2015. The age at initial surgery was(5.7±1.7)years(range,3 to 9 years). Measurements of primary curve magnitude,height of T1-S1,apical vertebral translation(AVR), apical vertebral body-rib ratio,apical vertebral rotation,thoracic rotation and rib hump were compared between pre-operatively,post-operatively,and at latest follow-up,through a paired-t test. Pearson correlation test was used for correlation analysis between parameters. Results All patients had a follow-up of(49.5± 12.8)months(range,24 to 71 months). A total of 111 operative procedures were performed,among which there were 92 lengthening procedures,averagely 4.8 lengthening procedures per patient. The average interval for each lengthening procedure was 10 months. The Cobb angle of primary curve was notably decreased from (66.5±13.2)° to(35.2±10.9)°(t=24.013,P<0.01),and no significant correction loss was found at the latest follow-up((36.7 ± 10.7)°)(t=-1.324,P=0.202). In addition,significant correction of AVR, thoracic rotation,apical vertebral translation,apical vertebra body-rib ratio,and rib hump were noted after initial surgery. Whereas,these parameters significant increased during follow-up(all P <0.05)except for thoracic rotation. Pearson correlation analysis showed that the increase of AVR during follow-up significantly correlated with change of apical vertebra translation,apical vertebral body-rib ratio,and rib hump(r=0.652, 0.814,0.695;all P<0.05). Conclusions Significant correction of AVR can be achieved after initial surgery in early-onset scoliosis patients treated with traditional dual growing rods. However,such a technique can hardly prevent the deterioration of AVR during follow-up.
Comparative analysis between shape memory alloy-based correction and traditional correction technique in pedicle screws constructs for treating severe scoliosis

Abstract:The three-dimensional correction of severe rigid scoliosis has been improved by segmental pedicle screw instrumentation. However, there can be significant difficulty related to the use of a rigid rod, especially in the apex region of severe scoliosis. This study is a retrospective matched cohort study to evaluate the advantages of Nitinol shape memory alloy (SMA) rod-based correction by comparing the clinical and radiographic results obtained from using a temporary SMA rod and those from a standard rod in the correction of severe scoliosis. From May 2004 to September 2006, patients with matched curve type, ages at surgery, operative methods and fusion levels in our institute and instrumented with either SMA rods ( n = 14) or traditional correction techniques ( n = 16) were reviewed. In SMA group, the SMA rods served as a temporary intraoperative tool for deformity correction and were replaced by standard rods. The blood loss at surgery averaged 778 ± 285 ml in the traditional group and 585 ± 188 ml in the SMA group ( P < 0.05). Operative time averaged 284 ± 53 min in the SMA group and 324 ± 41 min in the traditional group ( P < 0.05). In the SMA group, the preoperative major curve was 92.6° ± 13.7° with a flexibility of 25.5 ± 7.3% was corrected to 29.4° ± 5.7° demonstrating a 68.4% immediate postoperative correction. In the traditional group, the preoperative major curve was 88.6° ± 14.6° with a flexibility of 29.3 ± 6.6% was corrected to 37.2° ± 7.3° demonstrating a 57.8% immediate postoperative correction. There was a statistic difference between the SMA group and traditional group in correction rate of the major thoracic curve. In the SMA group, one case suffered from deep infection 2 months postoperatively. In the traditional group, 6 of 16 cases suffered pedicle screw pull out or loosening during placement of the standard rod at the apex vertebrae on the concave side. In three cases, the mono-axial pedicle screws near the apex were abandoned and in five cases replaced with poly-axial pedicle screws. This study shows that the temporary use of SMA rod may reduce the operative time, blood loss, while improve the correction of the coronal plane when compared with standard techniques.
Minimally Invasive Controlled Growing Rods for the Surgical Treatment of Early-Onset Scoliosis—A Surgical Technique Video

Abstract:Background: Spinal deformities in children and adolescents can be easily divided into those occurring and diagnosed before the age of 10—early-onset scoliosis—and those occurring and diagnosed after the age of 10—late-onset scoliosis. When the curvature continues to progress and exceeds a Cobb angle of more than 60–65 degrees, surgical treatment should be considered. The most common treatment procedure for EOS is the surgical correction of the deformity using standard growing rods (SGRs), and in the case of congenital defects with additional hemivertebrae, it is the resection of the hemivertebra and short fusion. Minimally invasive controlled growing rods (MICGRs) need to be distracted every 6–9 months through a minimally invasive approach that involves sedation and neuromonitoring to obtain the best possible correction while minimizing complications. The aim of our study is to present a less-invasive surgical technique for MICGR implantation based on a two-case presentation—early-onset idiopathic scoliosis and congenital kyphosis. The surgical technique is the less-invasive percutaneous and subfascial implantation of MICGRs without long incisions in the back. Conclusions: The use of MICGRs is an alternative and safe surgical technique for patients undergoing surgical treatment for EOS. Without the risk of metallosis, like in other implant systems, and the need for replacement after 2 years of use, like in using magnetically controlled growing rods (MCGRs), the MICGR system can be used as a less-invasive procedure, allowing for the avoidance of many periodic invasive procedures in children with a wider opening of the spine (like in using standard growing rods), minimizing the number of planned hospitalizations, reducing the length of hospital stays, and reducing the physical and mental burdens on young patients, parents, and families.

Three-dimensional correction of scoliosis by a double spring reduction system as a dynamic internal brace: a pre-clinical study in Göttingen minipigs

Abstract:Background context: Adolescent idiopathic scoliosis (AIS) is a major skeletal deformity that is characterized by a combination of apical rotation, lateral bending and apical lordosis. To provide full 3D correction, all these deformations should be addressed. We developed the Double Spring Reduction (DSR) system, a (growth-friendly) concept that continuously corrects the deformity through two different elements: A posterior convex Torsional Spring Implant (TSI) that provides a derotational torque at the apex, and a concave Spring Distraction System (SDS), which provides posterior, concave distraction to restore thoracic kyphosis. Purpose: To determine whether the DSR components are able to correct an induced idiopathic-like scoliosis and to compare correction realized by the TSI alone to correction enforced by the complete DSR implant. Study design/setting: Preclinical randomized animal cohort study. Patient sample: Twelve growing Göttingen minipigs. Outcome measures: Coronal Cobb angle, T10-L3 lordosis/kyphosis, apical axial rotation, relative anterior lengthening. Methods: All mini-pigs received the TSI with a contralateral tether to induce an idiopathic-like scoliosis with apical rotation (mean Cobb: 20.4°; mean axial apical rotation: 13.1°, mean lordosis: 4.9°). After induction, the animals were divided into two groups: One group (N=6) was corrected by TSI only (TSI only-group), another group (N=6) was corrected by a combination of TSI and SDS (DSR-group). 3D spinal morphology on CT was compared between groups over time. After 2 months of correction, animals were euthanized. Results: Both intervention groups showed excellent apical derotation (TSI only-group: 15.0° to 5.4°; DSR-group: 11.2° to 3.5°). The TSI only-group showed coronal Cobb improvement from 22.5° to 6.0°, while the DSR-group overcorrected the 18.3° Cobb to -9.2°. Lordosis was converted to kyphosis in both groups (TSI only-group: -4.6° to 4.3°; DSR-group: -5.2° to 25.0°) which was significantly larger in the DSR-group (p<.001). Conclusions: The TSI alone realized strong apical derotation and moderate correction in the coronal and sagittal plane. The addition of distraction on the posterior concavity resulted in more coronal correction and reversal of induced lordosis into physiological kyphosis. Clinical significance: This study shows that dynamic spring forces could be a viable method to guide the spine towards healthy alignment, without fusing it or inhibiting its growth.

Abstract:Purpose: Growing rods are the gold-standard for treatment of early onset scoliosis (EOS). However, these implanted rods experience frequent fractures, requiring additional surgery. A recent study by the U.S. Food and Drug Administration (FDA) identified four common rod fracture locations. Leveraging this data, Agarwal et al. were able to correlate these fractures to high-stress regions using a novel finite element analysis (FEA) framework for one patient. The current study aims to further validate this framework through FEA modeling extended to multiple patients. Methods: Three patient-specific FEA models were developed to match the pre-operative patient data taken from both registry and biplanar radiographs. The surgical procedure was then simulated to match the post-operative deformity. Body weight and flexion bending (1 Nm) loads were then applied and the output stress data on the rods were analyzed. Results: Radiographic data showed fracture locations at the mid-construct, adjacent to the distal and tandem connector across the patients. Stress analysis from the FEA showed these failure locations matched local high-stress regions for all fractures observed. These results qualitatively validate the efficacy of the FEA framework by showing a decent correlation between localized high-stress regions and the actual fracture sites in the patients. Conclusions: This patient-specific, in-silico framework has huge potential to be used as a surgical tool to predict sites prone to fracture in growing rod implants. This prospective information would therefore be vital for surgical planning, besides helping optimize implant design for reducing rod failures.
The Minimally Invasive Bipolar Fixation for Pediatric Spinal Deformities: A Narrative Review

Abstract:Growing rod techniques are increasingly used for early-onset scoliosis in children. Unfortunately, they are associated with many complications, particularly neuromuscular scoliosis, favored by the poor general condition of these patients and the fragility of their osteoporotic bones. Furthermore, these interventions are often iterative and usually followed by vertebral fusion at the end of growth. This is a review of the literature on a recent fusionless technique, minimally invasive bipolar fixation, which is more stable than the traditional growing rod techniques and less aggressive than vertebral arthrodesis. It allows the avoidance of arthrodesis, owing to the solidity of the construct and the stability of the results, leading to progressive spinal stiffening that occurs over time. The results of this technique have been published with a long follow-up period and have confirmed that it can completely replace posterior vertebral arthrodesis, especially in the most complicated scoliosis. Because it preserves growth, this technique should be recommended for early-onset scoliosis before the age of 10 years. The use of a self-expanding rod can avoid the need for repeated surgery, thereby reducing the risk of complications and the overall cost of treatment.

Abstract:OBJECTIVE This study aimed to evaluate the correction results of traditional dual growing rods (DGRs) on axial rotation using CT scans and to further explore the relationships between axial and torso deformities in patients with early-onset scoliosis (EOS). METHODS Patients with EOS who were treated with traditional DGRs between January 2006 and December 2014 were retrospectively reviewed. Plain radiographs were used to assess the degree of coronal and sagittal deformity. The apical vertebral rotation (AVR) and rib hump (RH) were measured on CT scans at the apical vertebra. Pearson or Spearman rank correlation analyses were used to analyze the associations between spinal and torso deformities. RESULTS A total of 27 patients (10 boys and 17 girls, average age 6.5 +/- 1.7 years) were enrolled in this study. The average number of lengthenings per patient was 5.0 +/- 1.9, with a mean follow-up duration of 52.9 +/- 18.2 months. The apical vertebral translation, apical vertebral body-rib ratio (AVB-R), AVR, and RH parameters were significantly decreased after the initial surgery (p < 0.05) but showed notable progression at the latest follow-up evaluation (p < 0.05). The pre-operative AVR and its correction after index surgery were significantly correlated with the preoperative values as well as with the corrections of the major Cobb angle, AVB-R, and RH. During the follow-up period, significant correlations were found between the deterioration of AVR and the AVB-R and also between the deterioration of AVR and the RH from the initial surgery to the latest follow-up. CONCLUSIONS Significant AVR correction can be achieved by DGR techniques after the initial surgery. However, this technique weakly prevents the deterioration of AVR during the follow-up period.
Temporary use of shape memory spinal rod in the treatment of scoliosis

Abstract:NiTinol shape memory alloy is characterized by its malleability at low temperatures and its ability to return to a preconfigured shape above its activation temperature. This process can be utilized to assist in scoliosis correction. The goal of this retrospective study was to evaluate the clinical and radiographic results of intraoperative use of shape memory alloy rod in the correction of scoliosis. From May 2002 to September 2006, 38 scoliosis patients (ranging from 50° to 120°; 22 cases over 70°) who underwent shape memory alloy-assisted correction in our institute were reviewed. During the operation, a shape memory alloy rod served as a temporary correction tool. Following correction, the rod was replaced by a rigid rod. The mean blood loss at surgery was 680 ± 584 ml; the mean operative time was 278 ± 62 min. The major Cobb angle improved from an average 78.4° preoperatively to 24.3° postoperatively (total percent correction 71.4%). In 16 patients with a major curve <70° and flexibility of 52.7%, the deformity improved from 58.4° preoperatively to 12.3° postoperatively (percent correction, 78.9%). In 22 patients with a major curve >70° and flexibility of 25.6%, the deformity improved from 94.1° preoperatively to 30.1° postoperatively (percent correction, 68.1%). Only one case had a deep infection. There were no neurologic, vascular or correction-related complications such as screw pullout or metal fracture. The study shows that the intraoperative use of a shape memory rod is a safe and effective method to correct scoliosis.

Abstract:Purpose: Describe the surgical technique and experience using the LSZ growing system in skeletally immature patients for two-stage surgical treatment of adolescent idiopathic scoliosis (AIS). Methods: Eleven skeletally immature patients who underwent two-stage surgical treatment of AIS in our center were retrospectively analyzed. Patients initially treated with the LSZ growing system were observed for an average of 40.5 months (range 23-64 months) and for 4 years after planned replacement of the LSZ growing system with a standard pedicle screw system. Results: The average preoperative angle of the main thoracic curve was 64.55 ± 3.21°, and that of the lumbar curve was 46 ± 5.52°. After instrumentation using the LSZ growing system, the thoracic curve was corrected to 17.63 ± 5.14°, and the lumbar curve to 9.2 ± 5.45°. The correction percentages were 72.52 ± 8.35 and 80.59 ± 10.77, respectively. After planned replacement of the LSZ system with a pedicle screw system, the angle of the thoracic curve changed to 11.45 ± 4.84°, and the lumbar curve to 6.4 ± 4.72°. The percentages of final correction were 82,1 ± 7,91 and 85,64 ± 5,47, respectively. The difference in the "LSZ growth coefficient" was 0.063 ± 0.037, which indicates continued growth of the spine along the instrumented region. None of the patients had neurological or infectious complications. Conclusion: Our study demonstrates the safety and effectiveness of 2-stage treatment of AIS using the LSZ system, which allows for spine growth during the period between stages.

Abstract:The magnetically controlled growing rod technique is an effective surgical treatment for children who have early-onset scoliosis. The length of the instrumented growing rods is adjusted regularly to compensate for the normal growth of these patients. Manual measurement of rod length on posteroanterior spine radiographs is subjective and time-consuming. A machine learning (ML) system using a deep learning approach was developed to automatically measure the adjusted rod length. Three ML models—rod model, 58 mm model, and head-piece model—were developed to extract the rod length from radiographs. Three-hundred and eighty-seven radiographs were used for model development, and 60 radiographs with 118 rods were separated for final testing. The average precision (AP), the mean absolute difference (MAD) ± standard deviation (SD), and the inter-method correlation coefficient (ICC [2,1] ) between the manual and artificial intelligence (AI) adjustment measurements were used to evaluate the developed method. The AP of the 3 models were 67.6%, 94.8%, and 86.3%, respectively. The MAD ± SD of the rod length change was 0.98 ± 0.88 mm, and the ICC [2,1] was 0.90. The average time to output a single rod measurement was 6.1 s. The developed AI provided an accurate and reliable method to detect the rod length automatically.

Novel Experimental Scoliosis Model in Immature Rat Using Nickel-Titanium Coil Spring

Abstract:In Brief Study Design. Follow-up of animals after surgically initiated scoliosis. Objective. To develop quantitatively asymmetric loads on rat lumbar to create scoliosis. Summary of Background Data. Current animal models for scoliosis use mostly rigid or flexible posterior asymmetric tethers. The curve progression can only be expected for the growth potential, leading to insufficient growth potential for validation of corrective techniques. Methods. Scoliosis was induced in 55 five-week-old female Sprague-Dawley rats using a nickel-titanium coil spring. The experimental rats were randomly divided into 2 groups: in group A (n = 15), the nickel-titanium coil spring was not removed until these rats reached physical maturity (age, 12 wk). Group B (n = 40) was further randomly subdivided into 5 subgroups (n = 8 for each subgroup): removal of the spring after 1 week (group B1), 2 weeks (group B2), 3 weeks (group B3), 4 weeks (group B4), and 5 weeks (group B5). All rats were followed for a 7-week period with serial radiographs to document change of the deformity. Results. All experimental animals of group A developed progressive, structural scoliotic curves convex to the left in the lumbar segment. In group B, the deformity of the lumbar progressed after the spring load was applied and regressed after the spring was removed. The scoliosis in group B1–B3 (the spring removed before sexual maturity) regressed after spring removal until the rats reached sexual maturity (4 wk after spring implant surgery). The scoliosis in group B4–B5 (the spring removed after sexual maturity) regressed only during the first week after spring removal surgery. The average coronal Cobb angle was 7.8° ± 1.3° (range: 6.0° −10.2°) in group B1 at the final follow-up, and there was only 1 experimental rat that maintained a curve more than 108. The models of group B2–B5 maintained stable scoliotic curves (coronal Cobb angle of L2–L5 > 10°) convex to the left in the lumbar segment at the final follow-up. Conclusion. This study establishes a rat lumbar scoliosis model via asymmetric load. This method develops lumbar scoliosis in a short time and maintains the essential elements along the curve. It is suitable for the investigation of scoliosis. Level of Evidence: N/A Asymmetric load induced by gravity is a critical factor in aggravating scoliosis. Existing scoliosis animal models with rigid or flexible posterior asymmetric tethers cannot efficiently imitate the gravity imposing on lumbar. Nickel-titanium coil springs simulate the asymmetric load that mimics the load on human scoliotic lumbar spines at erect posture.
