Abstract:Gingival enlargement is quite a common pathology in pediatric patients and may be inflammatory, noninflammatory, or a combination of both. Idiopathic gingival fibromatosis, although rare, is a slowly progressive benign enlargement that affects the marginal gingiva, attached gingival, and interdental papilla. The fibromatosis may potentially cover the exposed tooth surfaces, causing esthetic and functional problems. The treatment of gingival fibromatosis is essential because it causes difficulties with mastication, speech problems, mispositioning of teeth, esthetic effects, and psychological difficulties for the patient. Traditional gingivectomy procedures have been a challenge for dentists who confront issues of patient cooperation and discomfort. In the last decade, laser procedures in oral cavity had shown many optimum effects in both hard and soft tissue procedures. Laser soft-tissue surgery has been shown to be well accepted by children. The following case report describes a laser-assisted gingivectomy procedure performed on a 13-year-old female.
Laser Surgery versus Non-laser Surgery in Periodontitis: A Comparative Study

Abstract:Objective: The purpose of this research is to estimate the effectiveness of non-laser surgery and laser surgery in the management of periodontitis. Methods: One hundred participants with a chronic periodontitis diagnosis participated in a randomized controlled experiment. Two cohorts of patients were created: Cohort B underwent non-laser surgery and Cohort A underwent laser surgery. Pre- and postoperative assessments were conducted at 3, 6, and 12 months for clinical criteria such as "pocket depth (PD), gingival index (GI), and clinical attachment level (CAL)". Results: There was a noticeable improvement in clinical parameters following both laser and non-laser operations. However, laser surgery showed superior results in terms of decreased PD and raised CAL. Conclusion: In conclusion, compared to non-laser surgery, laser surgery seems to be a more successful treatment option for periodontitis. Larger sample sizes and longstanding follow-up are required for future research in order to validate these results and evaluate the longstanding sustainability of the treatment effects.
Comparative evaluation of healing after gingivectomy with electrocautery and laser

Abstract:Purpose of the study: To evaluate whether laser has got any advantage over electrocautery in performing gingivectomy procedure. Method: This was randomized split mouth prospective study where 17 patients having symmetrical gingival hyperplasia were treated with electrocautery and laser on each side. Intra- and postoperative parameters assessed were duration of surgery, bleeding, tissue sticking, postoperative pain and healing. The results were statistically analyzed and compared. Result: There were 7 Females and 10 Males with a mean age of 28.2 ± 11.6 years. The mean time taken on the laser and electrocautery was 16 (±8.05) and 16.7 min (±9.86) respectively and the difference was not significant. On laser side, there was no bleeding in 3 (17.6%) and self limiting in 14 patients (82.4%) whereas electrocautery side, there was no bleeding in 2 (11.8%) and self limiting in 15 patients (88.2%). Both electrocautery and laser were comparable regarding ease of use during gingivectomy procedure. Charring was observed on both sides although there was more charring on the laser side. Fibrinous slough was present on both sides at 24 h. Difference in healing was not significant at various time intervals. There were no significant difference in postoperative pain experienced by the patients among the laser and cautery group at different time intervals. Conclusion: Both techniques with proper adhesion to safeguards, can be used to remove gingival overgrowth with equal efficiency and wound healing capacity. There is no advantage of diode laser over electrocautery in performing gingivectomy.

Abstract:After dental extractions, patients may experience post-operative pain, with varying degree of severity among patients. The use of Er, Cr: YSGG laser in oral wound healing has garnered considerable interest and attention in recent years. The present study aims to evaluate and assess the wound healing and post operative pain after extraction of teeth in pediatric patients with application of Er, Cr: YSGG laser. Children aged 7–14 years of age with alteast one grossly decayed permanent molar indicated for extraction were included in the study. 60 teeth were randomly divided in two group– one receiving conventional treatment with post extraction pressure pack gauze and one receiving Er, Cr: YSGG laser bandage treatment. A post operative assessment was done by WBFPRS and Wound healing index for post operative pain and wound healing respectively on 24 h, 3rd, 7th and 14th day follow up. Er, Cr: YSGG laser bandage group showed statistically significant difference in both post operative pain and wound healing on follow ups. The study shows that application of laser bandage can be done in post operative extraction sockets to alleviate wound healing, decreasing post operative pain and discomfort.

Abstract:Objectives: The current study aim was to evaluate the effect of Bioactive Hydrogel versus Advanced-PRF on bone regeneration following impacted mandibular third molar surgery. Material & Methods: sixty patients who had been scheduled for surgical removal of mesio-angular or horizontal impacted mandibular 3rd molar teeth were randomly divided into three groups. Group A involved 20 patients received A-PRF-Xenograft at the surgical site. Group B involves 20 patients received Xenograft-Hydrogel at the surgical site. While group C involved 20 patients left to heal without graft as control. Clinical and radiographic evaluation were performed immediate and at 6 months postoperatively. Data was collected and analysed statistically. Results: At 6-month postoperative evaluation, the current study found that A-PRF-Xenograft group showed significant shorter mean pain duration (5.1±1.4 days) compared to Hydrogel-Xenograft group (5.3±1.5 days) and control group (5.8±1.4 days). A-PRF-Xenograft group showed the lowest mean values of PPD (3.7±0.6) followed by Hydrogel-Xenograft group (5±0.8) and followed by control group (6.6±0.6). A-PRF-Xenograft group showed the highest mean values of bone density (605.3±85.5) followed by Hydrogel-Xenograft group (522.4±83.5) followed by control group (286.4±44.7) Conclusion: According to the results of the present study, there was significant improvement regarding pain duration, mouth opening limitation, periodontal pocket depth and bone density at the surgical site where A-PRF-Xenograft was applied. Hydrogel-Xenograft Group was more superior to the control group regarding pain duration, PPD, and bone density.
Case series: laser treatments for soft tissue problems in children

Abstract:Background: Many soft tissue pathologies in children can be treated by paediatric dentists. New technologies such as laser surgery enable simpler treatments to be carried out than with conventional techniques. Case reports: A case series of 7 children aged 6 to 14 years old were treated for various soft tissue problems including crown lengthening, exposure of an unerupted molar, lingual and maxillary fraenectomies, gingivectomy, excision of a pyogenic granuloma and for a pulpotomy. Treatments: All children were treated using a Er,Cr:YSGG laser for removal of appropriate soft tissues. FOLLOWUP: All cases healed satisfactorily and were followed up for 3 to 4 years. Conclusion: It is essential to have a good knowledge of laser operation and of which type of laser is most appropriate for each lesion.
"Comparative Evaluation of Three Different Microabrasion Techniques in Esthetic Management of Fluorosis": An In Vivo Study

Abstract:Aim: "Comparative evaluation of three different microabrasion techniques in esthetic management of fluorosis"-an in vivo study. Materials and methods: A total of 48 permanent incisors in children between the age-groups of 8-12 years with Dean's fluorosis index modified criteria 1, 2, and 3 were included. The patients were randomly divided into three groups; each group included 16 samples. Group I-phosphoric acid and pumice microabrasion (37% phosphoric acid and pumice as abrasive), group II-opalustre microabrasion [6.6% hydrochloric (HCL) acid and silicon dicarbide (SiC2)], and group III-icon etch microabrasion (15% HCL acid gel as icon etch, pumice, and resin infiltrate). Preoperative sensitivity and pulp vitality of each tooth were evaluated. Standardized preoperative photographs were captured for the evaluation of color parameters L1, a1, and b1 by Adobe Photoshop 7 software and an assessment of color enhancement was carried out. Preoperative surface roughness was evaluated with two methods like, cellulose acetate replicating tape and confocal microscope, and Epoxy resin replica obtained from the preoperative impression of teeth using additional silicone and contact profilometer. Microabrasion was done accordingly. Postoperative values of all the parameters were evaluated. Results: Phosphoric acid and pumice showed the best color change, followed by icon etch resin infiltrate. Opalustre (6.6% HCL acid and SiC2) and was unsuccessful as it had less concentration and was more abrasive. Conclusion: Phosphoric acid and pumice showed the best color enhancement, followed by icon etch resin infiltrate and opalustre. Mandibular third molar removal in young patients: an evaluation of 2 different flap designs

Abstract:Purpose: To evaluate the influence of 2 different flap designs on periodontal healing and postoperative complications, after inferior third molar removal in young patients. Patients and methods: Twenty-four mandibular third molars were extracted from 12 patients with an average age of 16 years. Patients were included in the study when radiographs at the time of surgery showed that only the crown of the germ was formed. Each patient underwent 2 extractions, using a triangular flap on one side and an envelope flap on the other. Periodontal probing was recorded at the preoperative visit, and 7 days, 3 months, and 6 months after extraction. Postoperative complications were recorded using a questionnaire completed by the patient for the week after the extraction. Results: The examination performed 7 days after the extraction demonstrated a deeper probing depth in all teeth examined. This increase was statistically greater (P < .05) for the first and second molars when an envelope flap was used. After 3 months, the probing depths returned to preoperative values. No significant differences were seen between the 2 flap designs when postoperative complications were considered. The average operating time was 30.66 minutes with the triangular flap, versus 35.66 minutes with the envelope flap. This difference was not significant. After 6 months, the 2 flap designs resulted in no difference in periodontal healing or complications, but 30% of the surgical extractions resulted in a debilitating postoperative period for the patients treated. Conclusions: Although we observed statistically significant differences in probing depth between triangular and envelope flaps 7 days after the extraction of third molars with no root development, this was not important from a clinical perspective, because periodontal healing at 3 and 6 months was comparable. We believe that this is also the case with the extraction of third molars with fully formed roots. Another important finding was the presence of a debilitating postoperative period in most of the patients who underwent extraction, contrary to the beliefs of many surgeons.
Comparison of 3-dimensional postoperative dental movement in Class III surgical correction with and without presurgical orthodontic treatment

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Surgery-first approach (SFA) is an emerging concept that surgically reposition the jaw bones without presurgical orthodontic treatment phase. The study investigated 3D dental movement in the postoperative orthodontic phase with orthodontic-first (OF) and SFA in orthognathic surgery (OGS).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Material and Methods</h3><p>This study included consecutive 40 patients (20, SF group; 20, OF group) skeletal Class III who underwent 2-jaw OGS correction. The data of cone-beam computed tomography were acquired at 3 stages with the scan of dental models to replace the dentition of the craniofacial images; at before OGS (T0), 1 week after OGS (T1) and at the completion of treatment (T2). The skeletal changes were obtained by overall superimposition. The post-operative dental movement was measured by 3D regional superimposition between T1 and T2.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>There were no significant difference in the postsurgical orthodontic movement in both groups except significant upper and lower molars extrusion by 2 mm in the SF group. Both groups exhibited no significant difference in mandibular stability in sagittal and vertical directions. The amount of extrusion in the molars was correlated with a postoperative sagittal mandibular forward movement. The total treatment duration was significantly shorter 230 days in the SF group.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>The completion of the orthodontic treatment after OGS in the SFA was mainly accomplished through molar extrusive movement in both arches. The surgical setup of dental occlusion with 4 mm posterior open bite could be corrected during the postsurgical orthodontics in SFA through molar extrusion. The dental occlusion outcome was no different between OF and SFA.</p>

Evaluation of Pain Perception and Wound Healing After Laser-Assisted Frenectomy in Pediatric Patients: A Retrospective Comparative Study

Abstract:Background: The maxillary labial frenulum is an attachment that connects the upper lip to the gingival tissue, and when it is too closely attached to gingival margin due to inadequate plaque control, it may put gingival health at risk. Objective: This study aimed to assess postoperative pain perception and wound healing after laser-assisted frenectomy in a pediatric population retrospectively. Methods: This study is based on the clinical records of the patients who received laser-assisted frenectomy treatment due to mucogingival problems at the pediatric dentistry department. Twenty-two patients were treated with either 2780 nm Er,Cr:YSGG laser (Waterlase iPlus; Biolase Technology, Irvine, CA, USA) or 940 nm diode laser (Epic10; Biolase Technology). The analyzed data included age, gender, frenulum insertion type, type of dental laser, postoperative pain perception, and wound healing. Postoperative pain evaluation was performed using Wong-Baker Faces Pain Rating Scale after 3 h, 1 day, 1 week, and 2 weeks. Wound surface healing was assessed through photography. Images were uploaded to the software, and changes in the wound surface area were measured in square millimeters on the day of frenectomy and on postoperative first day, first week, and second week. Results: Er,Cr:YSGG group had statistically significant better wound healing results after 1 and 2 weeks, respectively (p < 0.05). No significant difference was found between 2780 nm Er,Cr:YSGG laser and 940 nm diode laser groups in terms of pain perception. Conclusions: It can be concluded that both laser wavelengths are a safe and useful tool for frenectomies in pediatric patients with less pain. Er,Cr:YSGG laser achieved faster healing than 940 nm diode laser. Clinical Registration number: Clinical Trials gov ID NCT04368715.
Outcome Assessment of Three Different Methods of Root-end Preparation and Filling Materials in Endodontic Surgery: A Comparative Clinical Prospective Study

Abstract:Introduction: The technique of endodontic surgery had evolved tremendously in the recent years with introduction of new instruments and materials. Aim: This study aims to compare the clinical outcome of endodontic microsurgery using three different techniques with three different root-end filling materials for 16 months. Materials and methods: A total of 45 maxillary incisors indicated for root-end surgery were selected for the study. They were enrolled into three groups, i.e. Group A (traditional [TRS]/heat burnished gutta-percha), Group B (concave [CON]/Retroplast), and Group C (cavity/DiaRoot BioAggregate), of 15 teeth each. The clinical and radiographic outcome was recorded at 1, 6, 12, and 16 months using various criteria. Statistical analysis: The data were analyzed by one-way analysis of variance and Tukey's post hoc test using SPSS V. 21 software (IBM Corp., Somers, NY, USA). Results: All patients had uneventful healing at the final follow-up. Radiological intratime analysis concluded a highly high significant (P < 0.0001) decrease in the size of radiolucency between the three groups at the third recall visit. Intertime analysis recorded no significant decrease in radiolucency between Groups A and B, a significant decrease in B and C, and a highly significant decrease between Groups A and C at 12 months. Conclusions: There was no significant difference in the clinical outcome after endodontic surgery when comparing TRS/heat burnished gutta-percha, CON/Retroplast, and cavity/DiaRoot BioAggregate techniques at 16 months. However, cavity/DiaRoot BioAggregate resulted in significantly rapid and predictable healing at 12 months.

Abstract:Germectomy is a surgical method most typically performed in young adults. The indications for treatment are controversial. The purpose of this review was to determine the correct indications for germectomy and to discuss the advantages and disadvantages of the procedure. We reviewed the surgical techniques (anesthetic methods, patient preparation, and flap designs) and complications. Germectomy for orthodontic purposes is a common indication for surgical removal of tooth germ among young patients. Several studies have supported removal at an earlier age to produce fewer surgical complications. Several surgical techniques have been described in the literature.
THREE-YEAR OUTCOME OF DIODE LASER PULPOTOMY OF PRIMARY MOLARS USING THREE PULP CAPPING AGENTS: A SPLIT-MOUTH RANDOMIZED CLINICAL TRIAL

Abstract:Objectives: Pulpotomy is the most commonly performed treatment for asymptomatic primary molars with exposed dental pulp. This study aimed to assess the clinical /radiographic success of diode laser pulpotomy with mineral trioxide aggregate (MTA), calcium hydroxide (CH), and calcium-enriched mixture (CEM) cement as pulp capping agents. Methods: This split-mouth randomized clinical trial was conducted initially on 34 children aged 3-8 years but 4 patients left the study before the first follow-up visit and the study was accomplished and analyzed with 30 cases. The patients had at least 3 first/second molars with deep caries that in radiographic evaluation revealed that they required pulpotomy. Following pulpotomy, the pulp stump was irradiated with diode laser (noncontact mode, 632 nm, 30 mW power) as photobiomodulation mode. Pulp tissue was then capped with MTA, CH, or CEM cement (n = 30 in each group). Reinforced zinc oxide eugenol was applied over the capping agent, and the teeth were restored with stainless steel crowns. Teeth were clinically/radiographically assessed at 6, 12, 18, and 36 months, after treatment. Data were analyzed by Cochran and McNemar tests. Results: All 30 patients showed up for clinical/radiographic follow-ups for up to 36 months. Regarding clinical outcomes, the 6-, 12-, 18-, and 36-month success rates of all experimental groups were nearly similar with no significant difference (p > .05). Regarding radiographic outcomes, the 6-month success rates were similar among the groups (p > .05); however, the 12-, 18-, and 36-month outcomes of CEM and MTA groups were similar but significantly superior to that of CH group (p < .05). Conclusion: Diode laser irradiation and subsequent capping of pulp tissue with MTA or CEM cement can be employed for pulpotomy of primary molars.
[Three-Dimensional Analysis of the Early Correction of Anterior Crossbite Using Eruption Guidance Appliance].

Abstract:OBJECTIVE:To investigate the clinical effect of eruption guidance appliance in the treatment of anterior cross bite in mixed-dentition children.METHODS:In the study,10 mixed-dentition children with anterior cross bite, totally 12 incisors, were selected. Alginate was used to take upper and lower dentition impression and make a hard plaster model,which served as the eruption guidance appliance for treatment. The pre- and post-operative dental casts were digitized with SmartOptics Activity 880 scanner,and the three-dimensional overlapping models were obtained by reverse engineering software,Geomagic Studio 2012,then the three-dimensional movements of the upper and lower incisors were analyzed using Imageware 13.2 software. The overbite and overjet were analyzed using the same methods. Measurement with copper wire was used to analyze the upper and lower arch length. Space analysis was the result by the sum of crown width minus the arch length. The crown width of unerupted permanent teeth was according to X-ray method to predict. The SPSS 17.0 software was used to analyze the pre- and post-operative measurements of the same child. The normality test of the measured data showed that it conformed to the normal distribution. Therefore,the t test and double side test were used,and the significance level was 0.05.RESULTS:The course of treatment was (5.6±2.7) months. During orthodontic treatment, the upper incisors moved mainly labially (P<0.001) in three-dimensional displacement, and the lower incisors moved mainly the same direction (P=0.025). During the treatment of eruption guidance appliance,the average overbite decreased (1.01±0.9) mm (t=-3.531, P=0.006), and the difference was statistically significant. There was no statistically significant difference between the pre- and post-operative average overjet (t=0.771, P=0.460). The severity of crowding in upper arch decreased (1.9±0.99) mm (t=-6.042, P<0.001),and that in lower arch decreased (1.9±0.74) mm (t=-8.143, P<0.001), both of the differences were statistically significant.CONCLUSION:The anterior cross bite in mixed dentition could be corrected by eruption guidance appliance, and at the same time, the normal overjet and overbite were established, and the teeth were aligned.
Effectiveness of laser-assisted gingival troughing and conventional gingival displacement methods in fixed prosthodontics: A systematic review

Abstract:STATEMENT OF PROBLEM: High-level evidence that gingival troughing using dental lasers has better outcomes for the management of gingival tissue surrounding prepared teeth compared with conventional gingival displacement methods is lacking.PURPOSE: The purpose of this systematic review was to review in vivo studies on gingival troughing using dental lasers to compare achieved gingival displacement, influence on periodontal health, and other relevant clinical parameters with different existing gingival displacement methods.MATERIAL AND METHODS: The study followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement and identified studies through January 2022. An electronic search was conducted on the digital databases PubMed/MEDLINE, Web of Science, Scopus, and Embase to identify relevant studies with no limit on publication year.RESULTS: Nine studies were included in this systematic review. Five studies measured achieved horizontal gingival displacement, where lasers show the highest pooled mean of 0.53 mm, among all other displacement methods. Six of the 9 included studies measured periodontal health parameters, with a follow-up time up to 1 year, and other clinically relevant parameters: hemorrhagic control, time needed, ease of use, tooth mobility, pain, and tooth hypersensitivity.CONCLUSIONS: Gingival displacement and bleeding control appeared to be better achieved using lasers. The time needed to achieve gingival displacement was shortest and easiest with lasers. Lasers resulted in either less or comparable gingival recession, lower impact on tooth hypersensitivity, and less pain during the procedure than other methods.

Abstract:We present three cases for the management of paediatric tongue tie (ankyloglossia) through laser-assisted frenectomy and postoperative oral myofunctional exercises. Ankyloglossia, characterised by a restricted lingual frenulum, poses challenges in tongue movement and is traditionally treated with surgical interventions. Laser technology, particularly diode lasers, has shown promise in paediatric cases due to reduced bleeding, minimal discomfort and faster recovery. The case series presents three instances where patients underwent laser-assisted frenectomy, demonstrating improved tongue function and speech outcomes. The comprehensive approach includes adherence to safety protocols, meticulous surgical techniques and the integration of postoperative exercises. Results indicate significant improvement in speech and tongue mobility, with no reported pain or complications. The study underscores the efficacy, safety and patient-centric nature of the combined laser and exercise strategy in addressing ankyloglossia in paediatric dentistry, emphasising the need for further research and long-term follow-up studies to validate its sustained effectiveness.
Comparative Evaluation of Amniotic Membrane Derivative, Chitosan with Mineral Trioxide Aggregate, Diode Laser, and Ferric Sulfate as Pulpotomy Agents in Human Primary Molars: An In Vivo Study

Abstract:Aim: The aim of the present study was to compare the clinical and radiographic success of amniotic membrane derivative (AMD), chitosan with mineral trioxide aggregate (C-MTA), diode laser (DL), and ferric sulfate (FS) as pulpotomy agents in human primary molars. Materials and methods: In this present study, pulpotomies were performed on 48 primary molars in 30 children aged between 4 and 8 years (12 teeth in each group). Following the pulpotomy procedure, teeth were evaluated clinically and radiographically at 1st, 3rd, 6th, and 9 monthly intervals. Results: After 9 months of follow-up, the clinical success was 91.6% for AMD and C-MTA and 83.3% for DL and FS. Radiographic success was 91.6, 91.6, 75, and 83.3% for AMD, C-MTA, DL, and FS groups, respectively. There is no statistically significant difference between the four groups (p > 0.05). Interpretation and conclusion: Results of our study showed that both AMD and C-MTA were equally successful compared to traditional agents like laser and ferric sulfate as pulpotomy agents. Clinical significance: Amniotic membrane derivative (AMD) and C-MTA are alternative biomimetic pulpotomy agents that can be used in pediatric primary tooth pulpotomies. Abstract:Introduction: The study was performed to evaluate and compare the clinical and radiographic efficacy of chlorhexidine (CHX) polymer scaffold, 3Mixtatin, and formocresol for vital primary pulp therapy-a randomized clinical study.Materials and methods: A total of 120 primary molars were included from children aged between 6 and 8 years in this randomized clinical study based on inclusion and exclusion criteria and were randomly allocated into three groups (group I-CHX polymer scaffold, group II-3Mixtatin, and group III-formocresol. Pulpotomy was performed in a vital cariously exposed primary tooth with healthy periodontium where their retention is more beneficial than extraction. Subjects were followed up at 1, 3, and 6 months for clinical and radiographic evaluations.Results: At 6 months of follow-up, the overall success rate of pulpotomy in groups I, II, and III was 56.41, 71.05, and 60.52% in each group, respectively. Nonsignificant difference (<i>p</i> > 0.05) was seen during intergroup comparison.Conclusion: However, among the three materials used in this study, 3mixtatin comparatively had better results.How to cite this article: Goel N. [Evaluation of Clinical Outcomes for Er:YAG Laser Application in Caries Therapy of Children].

Abstract:OBJECTIVETo investigate the children's perception and response to Er:YAG laser application in cavity preparation of caries in the primary teeth and young permanent teeth.METHODSChildren aged 3-15 years old who had two teeth with caries of equivalent degree were selected. One tooth was prepared by mechanical means and the other by Er:YAG laser. Immediately following treatment by laser or mechanical means, each child was asked to answer the questions in the questionnaire including sensitivity during treatment such as discomfort, pain or unpleasant sensations. Each child was asked to indicate which form of preparation, laser or mechanical, they would prefer for treatment in the future. The restorations were evaluated in 3, 6 or 12 months.RESULTSFifty three children with an average age 8.6±3.1 years old (17 children between 3-6 years old, 29 children between 7-12 and 7 children older than 13 years old). One hundred and twenty teeth were restored. 67% of the children considered laser preparation to be more comfortable. 21% considered same sensitive about the two means. The laser treatment was significantly less painful than mechanical treatment (P<0.05) and 72% of the children indicated that they preferred Er:YAG laser preparation for future caries treatment. There were no statistical significant difference in the two groups when compare the restoration in 3, 6 and 12 months.CONCLUSIONEr:YAG laser was considered comfortable and painless compared with mechanical bur preparation for caries therapy in children.
Clinical effectiveness of Eruption Guidance Appliances in treating malocclusion in the mixed dentition: A systematic review and meta‐analysis

Abstract:AIM: To evaluate the clinical effectiveness of the eruption guidance appliances (EGAs) in treating malocclusion in the early mixed dentition.DESIGN: Electronic databases were comprehensively searched for the eligibility literature of the EGA treatment for a period spanning from the earliest available date in each database up to July 2021. Randomized controlled trials, controlled clinical trials, and prospective and retrospective cohort studies were included in the present review. The quality of clinical trials was assessed according to the Cochrane Collaboration's tools (RoB2.0 and ROBINS-I), whereas cohort studies were based on the Newcastle-Ottawa Scale (NOS). The data were gathered and synthesized with the Stata software (version 12).RESULTS: The screen yielded 436 articles, of which 17 papers were potentially eligible, and 7 articles from 3 studies (1 RCT, 1 CCT, and 1 PCS) were qualified for the final review and analysis. The meta-analysis showed both favorable dentoalveolar and skeletal changes in short term. Both overjet and overbite had a significant decrease after treatment (MD = -2.38 mm, 95% CI: -2.82 to -1.94, p < .001, and MD = -2.43 mm, 95% CI: -3.52 to -1.35, p < .001, respectively), and SNB increased significantly by 0.73 degrees (95% CI: 0.17-1.28, p = .01). After the retention period, however, overbite had a significant increase of 0.88mm, which indicated the occurrence of a relapse (95% CI: 0.60-1.16, p < .001).CONCLUSIONS: According to the existing evidence, the EGA treatment is effectively correcting overjet and overbite in the early mixed dentition in short term; furthermore, high-quality and long-term studies are warranted to determine its long-term effectiveness.
