Abstract:Objectives: The frenum is a mucous membrane fold that attaches the lip and the cheek to the alveolar mucosa, the gingiva, and the underlying periosteum. Frenectomy is the surgical removal of the whole frenum, including the area connected to the bones. The purpose of this study was to compare the healing period and postsurgical pain experienced by patients operated with diode and Er:YAG lasers. Method and materials: Twenty referred patients requiring excision of the abnormal upper labial frenum were included in the study. Patients were randomly assigned into two groups: diode group (810 nm, 2W, continuous emission, initiated tip) and Er:YAG group (2,940 nm, 2W, 200 mJ, 10 Hz). Both lasers were applied in contact mode. Postoperative pain was assessed using a numerical rating scale at 3 hours postoperatively and every day during the first postoperative week. The epithelialization process of the wound surface was evaluated using hydrogen peroxide solution applied to the wound on postoperative days 7, 14, 30, 60, and 90. Results: The results showed the mean values of Pain Index after 3 hours (diode group 2.1 ± 2.0, Er:YAG group 2.6 ± 1.4), day 1 (diode group 1.1 ± 1.1, Er:YAG group 1.9 ± 1.4), and day 2 (diode group 0.0 ± 0.0, Er:YAG group 0.9 ± 1.1), with no significant difference after 3 to 7 days (P = 1.00). For the Healing Index there was a significant difference between the diode group and the Er:YAG group (7 days, P = .029; 14 days, P = .001), with no significant difference after 30/60/90 days (P = 1.00). Conclusions: The Er:YAG laser had better clinical results in healing wounds, whereas the diode laser resulted in better decreasing pain levels after frenectomy during the follow-up periods.
Evaluation of Patient's Perceptions, Healing, and Reattachment After Conventional and Diode Laser Frenectomy: A Three-Arm Randomized Clinical Trial

Abstract:Objective: The aim of this study is to compare the conventional and diode laser techniques in terms of patient's perceptions, epithelization, reattachment, and periodontal clinical parameters in the treatment of abnormal papillary frenum. Materials and methods: Forty-eight patients with abnormal papillary frenum were enrolled in the study. Patients were randomly assigned into three groups; conventional frenectomy operation (C group), diode laser-assisted frenectomy (L group), and diode laser-assisted frenectomy with conventional horizontal incision on the periosteum (L + P group). Postoperative pain, discomfort in speaking, and chewing scores were assessed with visual analogue scale (VAS) at postoperative 3rd hour and on days 1, 7, 14, 21, and 45. Epithelialization process of the wound surface was evaluated by hydrogen peroxide solution applied to the wound on days 7, 14, 21, and 45 following operations. The distance between the frenum attachment point and mucogingival junction (FMGJ) was recorded at baseline, postoperative 45th day, and 6th month to assess the reattachment of the frenum. Plaque index, gingival index, bleeding on probing, and probing depth were recorded at baseline and postoperative 7th, 14th, 21st, and 45th days. Results: On the 1st and 7th day after operation, VAS pain score in the C group was significantly higher than in the L and L + P groups (p < 0.017). Difficulty in speaking and chewing scores were significantly lower in the L and L + P groups compared to the C group at postoperative 3rd hour and 7th day (p < 0.017). The FMGJ and epithelization period demonstrated no difference among the groups at any time point (p > 0.05). Conclusions: Our results suggest that diode laser provides better postoperative patient's perceptions than the conventional technique in frenectomy operation. In addition, both conventional and laser-assisted frenectomy surgeries prevent the frenum reattachment regardless of periosteal horizontal incision.
Comparative study of upper lip frenectomy with the CO2 laser versus the Er, Cr:YSGG laser

Abstract:Objectives: To compare upper lip frenulum reinsertion, bleeding, surgical time and surgical wound healing in frenectomies performed with the CO2 laser versus the Er, Cr:YSGG laser. Study design: A prospective study was carried out on 50 randomized pediatric patients who underwent rhomboidal resection of the upper lip frenulum with either the CO2 laser or the Er,Cr:YSGG laser. Twenty-five patients were assigned to each laser system. All patients were examined at 7, 14, 21 days and 4 months after the operation in order to assess the surgical wound healing. Results: Insertion of the frenulum, which was preoperatively located between the upper central incisors, migrated to the mucogingival junction as a result of using both laser systems in all patients. Only two patients required a single dose of 650 mg of paracetamol, one of either study group. CO2 laser registered improved intraoperative bleeding control results and shorter surgical times. On the other hand, the Er,Cr:YSGG laser achieved faster healing. Conclusions: Upper lip laser frenectomy is a simple technique that results in minimum or no postoperative swelling or pain, and which involves upper lip frenulum reinsertion at the mucogingival junction. The CO2 laser offers a bloodless field and shorter surgical times compared with the Er,Cr:YSGG laser. On the other hand, the Er,Cr:YSGG laser achieved faster wound healing.
Effects of topically administered 0.6% hyaluronic acid on the healing of labial frenectomy in conventional and 940-nm indium gallium arsenide phosphide (InGaAsP) diode laser techniques in pediatric patients: a randomized, placebo-controlled clinical study

Abstract:This study aimed to investigate the effects of 0.6% hyaluronic acid (HA) gel on the healing process and postoperative pain levels after diode laser-assisted labial frenectomy in pediatric patients. Ninety-six pediatric patients (females, 50 and males, 46) aged 8–14 years were randomly divided into four groups as follows: (1) conventional frenectomy with 0.6% topically administered HA (CFH, n = 24); (2) conventional frenectomy with placebo gel (CFP, n = 24); (3) frenectomy performed by diode laser with 0.6% topically administered HA (DLH, n = 24); and (4) frenectomy performed by diode laser with placebo gel (DLP, n = 24). HA application was continued for 1 week thrice daily after the frenectomy. Visual analog scale forms were collected from patients 1 week after the operation. In addition, the plaque index, gingival index, periodontal probing depth, and keratinized tissue width and thickness were recorded. This process was repeated 1 and 3 months after the first visit. The DLH group revealed significant differences in the probing depth, bleeding on probing, keratinized gingiva width, and attached gingiva width according to dual comparisons of the initial, first, and third-month values ( p = 0.010, p = 0.007, p <0.001, and p = 0.001, respectively). Significant differences were observed between the CFP and CFH groups according to the initial and initial third-month values with regard to the bleeding on probing ( p =0.019 and p = 0.019, respectively). The attached gingival thickness revealed significant differences between the CFP and CFH groups for the initial and initial-third-month comparisons ( p = 0.005 and p = 0.007, respectively). The mean values of the initial and initial-third-month differences were significantly higher in the CFH group than those in the CFP group. HA- and laser-assisted labial frenectomies revealed better outcomes in terms of the probing depth, attached gingiva width, keratinized gingiva width, healing process, and postoperative comfort.

Abstract:Aim: The labial frenulum is a mucosal fold that connects the upper lip to the alveolar ridge. Frenectomy involves the surgical removal of a mucosal fold. The traditional surgical technique utilizing a scalpel remains popular for performing a frenectomy. The study aims to examine the effectiveness of applying Hyaluronic Acid (HA) Gel after conventional frenectomy. Methods: This research comprised 48 young dental patients aged 8 to 14. Out of all the patients, there are 25 females and 23 males. Participants were randomly divided into Group 1: Conventional Scalpel Surgery Sterile Saline (SS) (n=24) and Group 2: Conventional Scalpel Surgery+HA (Aftamed, Child Gel 24% HA) (n=24). After measuring the participants' periodontal parameters, the frenectomy was performed. Postoperative pain values were assessed after the first week using a 10-point Visual Analog Scale. Before frenectomy, Plaque Index (PI), Gingival Index (GI), Pocket Depth (PD), Bleeding On Probing (BOP), Keratinized Gingival Width (KGW), Attached Gingival Thickness (AGT) values were measured and recorded. The periodontal parameters were assessed and analyzed after a 3-month follow-up period. Results: VAS (Visual Analogue Scale) was gathered and assessed. The application of 0.24% HA gel did not result in a significant reduction in pain levels. Although the HA gel caused less pain from days 1-3.6, there was no significant difference in pain levels between the HA gel and control groups (p>0.05). On the 7th day, the level of pain was equal. No significant changes in PI, GI, PD, BOP, KGW, AGT values after three months of follow-up (p>0.05). Conclusions: During the first week after a frenectomy performed with classic scalpel surgery, the application of 0.24% HA gel did not significantly reduce pain levels. The intervention was deemed ineffective as it did not result in notable changes in other measured parameters after three months of follow-up.
The applications of diode and Er:YAG lasers in labial frenectomy in infant patients

Abstract:This paper describes a clinical case of labial frenectomy using different high power lasers: diode (810 nm) and Er:YAG (2940 nm). Considerations are made about specific indications, surgery techniques, and advantages of labial frenectomy using these laser wavelengths. The diode laser has high absorbance by pigmented tissues with hemoglobin, melanin, and collagen chromophores. For this reason, this wavelength is well indicated for surgery in soft tissue (vaporization, incision, coagulation, hemostasis). It is not properly absorbed, however, and should never be used in contact with hard tissues (bone). The Er:YAG laser has high absorbance to water and mineral apatite, making this wavelength useful and safe for the ablation of hard tissues. In the labial frenectomy clinical procedure, a combined technique is suggested: using the diode laser in soft tissues and the Er:YAG laser in periosteal bone tissues and for removal of final collagen fibers. It is important for the professional to understand the physical characteristics of the different laser wavelengths and their interaction with biological tissues to assure that they are used in a safe way, and that the benefits of this technology can be provided to infant patients.

Abstract:After dental extractions, patients may experience post-operative pain, with varying degree of severity among patients. The use of Er, Cr: YSGG laser in oral wound healing has garnered considerable interest and attention in recent years. The present study aims to evaluate and assess the wound healing and post operative pain after extraction of teeth in pediatric patients with application of Er, Cr: YSGG laser. Children aged 7–14 years of age with alteast one grossly decayed permanent molar indicated for extraction were included in the study. 60 teeth were randomly divided in two group– one receiving conventional treatment with post extraction pressure pack gauze and one receiving Er, Cr: YSGG laser bandage treatment. A post operative assessment was done by WBFPRS and Wound healing index for post operative pain and wound healing respectively on 24 h, 3rd, 7th and 14th day follow up. Er, Cr: YSGG laser bandage group showed statistically significant difference in both post operative pain and wound healing on follow ups. The study shows that application of laser bandage can be done in post operative extraction sockets to alleviate wound healing, decreasing post operative pain and discomfort.

Abstract:Median diastema is a physiological occurrence that is frequently seen in the maxillary jaw. Therefore, a median diastema has been associated with a wide range of etiological variables, including thumb sucking, supernumerary teeth, tongue thrusting, some dentoalveolar discrepancy, and hypodontia. Patient esthetic and function demands are both negated by the labial frenum's abnormal location in relation to the maxillary anterior teeth, which results in diastema and gingival recession. Lasers are now being used in several fields of dentistry as an alternative to conventional scalpel operations. Frenectomy can be done through the use of electrosurgery, laser surgery, or the classic scalpel technique. This pathological frenum can be very well excised with a diode laser. Due to its applicability, sufficient coagulation, lack of suture requirements, and reduced discomfort and inflammation, the diode laser can be utilized in pediatric dentistry. High-connected midline diastema has remained a subject of debate when it comes to management and the right time to intervene and treat it. Both orthodontists and pediatric dentists agree that frenectomy should not be done after the closure of the orthodontist gap or before the appearance of the permanent canine teeth. However, several conditions, including the child's psychological status, parents' concerns, the closure's unpredictable effects on the future, and the expense of combined therapies, may lead to an early intervention for therapy during the primary or mixed dentition. In this specific scenario, a child who was eight years old underwent a diode laser frenectomy. After seven days, a follow-up examination showed normal position and attachment of the frenum and no signs of infection at the site of surgery.
Effect of laser corticotomy on canine retraction rate: a split-mouth randomized clinical trial

Abstract:Background: This study assessed the effect of corticotomy with Er: YAG (erbium-doped yttrium aluminium garnet) laser on the rate of canine retraction. Methods: This randomized split-mouth controlled clinical trial was conducted on 12 patients undergoing orthodontic treatment with extraction of maxillary first premolars. Following initial leveling and alignment, an alginate impression was made from the maxillary arch, and Er: YAG laser corticotomy was performed in one of the maxillary quadrants of each patient. Canine retraction was started immediately after corticotomy by placement of nickel-titanium (NiTi) closed coil springs at both sides. At the end of each month, alginate records were repeated for 4 months. Study models were scanned, and the anteroposterior movement of canine was quantified bilaterally. Pain was also measured by a visual analog scale (VAS). Probing depth (PPD) of canines and two adjacent teeth was also evaluated and pulp vitality was assessed by performing the cold test. Data were analyzed by paired and independent t-test and one-way ANOVA (alpha = 0.05). Results: The rate of canine retraction was significantly greater in the laser-assisted corticotomy quadrant than the control (P < 0.05). No significant difference existed in posterior anchorage loss, canine rotation angle, PPD, pulp vitality, or pain score between two groups (P > 0.05). Conclusions: Flapless Er: YAG laser corticotomy significantly enhanced canine retraction rate with no adverse effect on other parameters.
Assessment of healing following low-level laser irradiation after gingivectomy operations using a novel soft tissue healing index: A randomized, double-blind, split-mouth clinical pilot study

Abstract:Background: Lasers have become a part of modern dentistry since the past three decades. A wide-ranging assortment of lasers is being used in periodontology for both soft and hard tissue surgical procedures. Regardless of the frequent practice of using these well-known surgical lasers, there is another lesser familiar class of lasers called the low-level lasers. The main doctrine behind using low-level laser therapy (LLLT) is centered on the biostimulation, or the biomodulation effect, which relies on the dexterity of low-intensity laser energy when irradiated at a specific wavelength, is able to modify cellular activities (increase in cell metabolism and fibroblast and keratinocyte proliferation). Aim: The aim of the present study was to investigate the adjunctive use of LLLT on gingival healing after gingivectomy procedures. Materials and methods: Ten systemically healthy patients requiring gingivectomy or gingivoplasty procedures bilaterally in either the maxillary or mandibular teeth were included in the study. After surgical intervention, a diode laser (810 nm) was randomly activated to one side of the operated area while other side did not receive any treatment and served as the control. The healing index given by Landry et al. was used to evaluate the soft tissue healing immediately postsurgery and at the 3rd day, 1 week, and 2 weeks postoperatively. After the follow-up period, results were analyzed using appropriate statistical tests. Results: There was no statistically significant difference observed in the surface epithelialization between both groups on the 3rd and 7th-postoperative days, but highly significant differences were observed on the 14th day. Conclusion: Within the confines of this pilot study, the outcomes have indicated that gingival wound healing may be improved using LLLT after gingivectomy and gingivoplasty operations.
Laser-assisted gingivectomy in pediatric patients: a novel alternative treatment

Abstract:Gingival enlargement is quite a common pathology in pediatric patients and may be inflammatory, noninflammatory, or a combination of both. Idiopathic gingival fibromatosis, although rare, is a slowly progressive benign enlargement that affects the marginal gingiva, attached gingival, and interdental papilla. The fibromatosis may potentially cover the exposed tooth surfaces, causing esthetic and functional problems. The treatment of gingival fibromatosis is essential because it causes difficulties with mastication, speech problems, mispositioning of teeth, esthetic effects, and psychological difficulties for the patient. Traditional gingivectomy procedures have been a challenge for dentists who confront issues of patient cooperation and discomfort. In the last decade, laser procedures in oral cavity had shown many optimum effects in both hard and soft tissue procedures. Laser soft-tissue surgery has been shown to be well accepted by children. The following case report describes a laser-assisted gingivectomy procedure performed on a 13-year-old female.
Postoperative pain following laser fistula closure versus ligation of the intersphincteric fistula tract: A prospective double‐blinded randomized controlled trial

Abstract:Background Prior studies focus primarily on surgical outcomes of anal fistula treatment, such as healing rates, rather than patient‐reported outcomes, such as postoperative pain, which could influence surgical choice. Objective To compare pain scores at 6 and 24 h postoperatively between laser closure and ligation of the intersphincteric tract for anal fistula. Design Prospective, double‐blinded randomized controlled trial. Settings A quaternary hospital in Malaysia. Patients Patients aged 18–75 years with high transsphincteric fistulas. Intervention Fistula laser closure versus ligation of the fistula tract (LIFT) treatment. Main Outcome Measures Pain scores, continence, quality of life (QOL), operative time, and treatment failure were compared using chi‐square, Fisher's exact test, student t‐test, or Mann–Whitney with p 0.05). Limitations Patients with prior fistula surgery (approximately 20%) led to heterogeneity. The total laser energy delivered varied depending on fistula anatomy. Conclusion Laser fistula closure is an alternative to LIFT, with similar postoperative pain and shorter operative time despite more complex fistula anatomy in the laser arm, with a greater improvement in QOL. Clinical Evaluation of 532-Nm Green Laser on Dentin Hypersensitivity: A Randomized Double-Blind Clinical Trial

Abstract:Objective: The aim of this study was to evaluate the therapies of low-level green laser and chemical desensitizer in the treatment of dentin hypersensitivity (DH). Methods: Forty-eight patients with 96 sensitive teeth were invited to participate in this clinical trial and were randomly divided into three groups. One group was treated with low-level green laser, the second group was treated with desensitizer [sodium fluoride (NaF)], and the third group acted as the placebo group and was treated with distilled water and placebo laser. The wavelength of green laser was 532 nm and the irradiance was 15 J/cm2 per treatment site. Hypersensitivity was assessed by visual analogue scale (VAS) according to cold test and probing at baseline. Immediately, 2 weeks, and 3 months after the application of green laser, NaF, and placebo, the participants' sensitivity level was accessed by new VAS analysis. Results: Forty-five patients with 90 teeth (n = 15 patients/group; 30 teeth/group) were followed up for 2 weeks and 3 months after treatment. There were significant differences in VAS scores between the placebo group and intervention group (green laser group and NaF group; analysis of variance, p < 0.05) at all three time points. The mean pain scores in DH reduced significantly immediately after treatment in the green laser group and NaF group when stimulated by cold and probing, whereas no significant difference was observed with these two therapies after 2 weeks (p > 0.05). After 3 months, mean VAS scores of the NaF group were higher than those of the green laser group (p < 0.01). Conclusions: Therefore, the green laser displayed similar effectiveness as NaF in treatment of DH and could be a promising new therapy to reduce DH.
A Short-Term Clinical Evaluation of Periodontal Treatment with an Er:YAG Laser for Patients with Chronic Periodontitis: a Split-Mouth Controlled Study

Abstract:OBJECTIVE:To compare the short-term clinical effects following non-surgical periodontal treatment with Er:YAG laser or with combination of ultrasonic subgingival scaling and root planing with hand instrument (SRP) for patients with chronic periodontitis.METHODS:In the study, 17 patients with chronic periodontitis were randomly treated in a split-mouth design with Er:YAG laser (test group) or a combination of ultrasonic subgingival scaling and root planing with hand instrument (control group). The degree of discomfort experienced during the treatment was graded by the patient using visual analogue scale (VAS) immediately after the completion of test and control treatment procedures. The following clinical parameters were recorded by a calibrated and blinded examiner: plaque index (PLI), bleeding index (BI), probing depth (PD) and attachment loss (AL).RESULTS:The mean VAS score of Er:YAG laser treatment [3 (2, 4.5)] was significantly lower than that of control treatment [5 (4, 6), P=0.013]. Both the groups showed significant reduction of PLI, PD, AL and BI values 2 months and 4 months after treatment. For sites with PD≥4 mm at baseline, the sites treated with Er:YAG laser demonstrated mean PD change from (5.6±1.1) mm to (3.6±1.1) mm and to (3.4±1.0) mm at the end of 2 months and 4 months respectively and demonstrated mean AL change from (5.1±1.5) mm to (3.9±1.6) mm and to (3.8±1.7) mm at the end of 2 months and 4 months respectively, meanwhile the BI value showed significant decrease, P=0.000; the sites treated with conventional SRP demonstrated mean PD change from (5.6±1.1) mm to (3.8±1.1) mm and (3.5± 1.0) mm at the end of 2 months and 4 months respectively and demonstrated mean AL change from (4.8±1.6) mm to (3.8±1.6) mm and (3.6±1.8) mm at the end of 2 months and 4 months respectively, and the BI value also showed significant improvement. No statistical difference for all clinical parameters were found between the two treatment groups.CONCLUSION:The present results indicate that non-surgical periodontal therapy with Er:YAG laser is safe and effective, and Er:YAG laser therapy could be used for patients who was sensitive to pain.
The Efficacy of Lingual Laser Frenectomy in Pediatric OSAS: A Randomized Double-Blinded and Controlled Clinical Study

Abstract:This randomized, double-blind and controlled clinical trial investigates how a diode laser lingual frenectomy can improve obstructive sleep apnea syndrome (OSAS) in pediatric patients. Background: Several authors have shown that a short lingual frenulum causes a reduction in incoming air flow and the relationship between OSAS and a short lingual frenulum. Methods: Thirty-two pediatric patients were equally randomly divided into a Study Group (SG) and a Control Group (CG). On each SG patient a polysomnography 1 (PSG1) and a lingual frenectomy were performed using a diode laser via Doctor Smile Wiser technology, power 7 W. After three months, a new polysomnography (PSG2) was performed to evaluate the lingual frenectomy efficacy in pediatric patients. The pain was assessed by a numerical rating scale (NRS) before and after surgery. The CG followed the same protocol without a lingual frenectomy but myofunctional and speech therapy were conducted to qualitatively and quantitatively improve the lingual functionality. In the SG, eight subjects (50%) had severe OSAS and eight had moderate (50%) while in the CG, three subjects had severe OSAS (18.8%) and thirteen had moderate (81.2%). Results: In the SG, 93.8% were classified as mild OSAS and 6.2% as moderate. In contrast, in the CG, 18.75% were classified as mild OSAS, 62.5% as moderate and 18.75% as severe. Conclusion: The study demonstrates how a lingual laser frenectomy can improve OSAS in pediatric patients.

Nd:YAG and diode laser in the surgical management of soft tissues related to orthodontic treatment

Abstract:Objective: The aim of this clinical study was to observe and evaluate the surgical management efficiency of soft tissues during orthodontic treatment. Materials and methods: Thirty-seven young patients were selected and treated by laser-assisted surgery before or during orthodontic treatment and were classified by sex, age, and type of surgical management. Three different wavelengths were used (diode 810 nm, diode 980 nm, and Nd:YAG 1064 nm) in different surgical situations: maxillary vestibular and lingual frenectomies, surgical exposure and alignment of ectopic or retained teeth, and re-contouring gingival overgrowth. Results: In all evaluated patients, the laser treatment was performed without local anesthesia or sutures. Only topical anesthetic was needed. Conclusion: These use of these wavelengths of laser energy was a noticeable aid in the surgical management of soft tissues before or during orthodontic treatment. The benefits of laser treatment include reduced bleeding during surgery with consequent reduced operating time and rapid postoperative hemostasis, thus eliminating the need for sutures. The lack of need for anesthetics and sutures, as well as improved postoperative comfort and healing, make this technique particularly useful for very young patients.
All done procedure by laser in free gingival graft treatment: A case series study

Abstract:Aim: This case series study evaluated the application of Er;Cr:YSGG laser for a free gingival graft procedure and the effect of low level laser therapy on post surgical pain and wound healing. Cases: 6 cases with mucogingival problems needing free gingival grafts were selected. First, the recipient site was anesthetized and de-epithelialization was done with a Er;Cr:YSGG laser. After tracing the palatal donor site with the laser, the graft was harvested. For root modification of the recipient tooth, conditioning of the dentin was done by the same laser. The inner side of graft was trimmed by the laser before suturing at the recipient site. After final adaptation of the grafts and suturing process, the recipient and donor sites were irradiated by therapeutic laser for reduction of post surgical complications. Results: All patients reported the reduction in pain level after day 2 on donor and recipient site. Also, the wound healing was achieved after 14 days in all cases in donor site. Conclusion: According to high surface absorbance, better incision, coagulation and application on both soft and hard tissues, Er;Cr:YSGG laser can be safe, minimally invasive manner and useful for all steps of the free gingival graft procedure.

Abstract:Aim: To evaluate and compare the effect of impregnated retraction cord vs Laser on gingival attachment level and pain perception following retraction for subgingival margins. Settings and design: Many methods for achieving and measuring the amount of gingival retraction in fixed prosthodontic work have been advocated. Though the gingival attachment level is crucial in Periodontology, the literature available regarding the effect of these retraction methods on the same is scarce. Hence, this clinical study was designed to compare the pain perception and amount of gingival recession when impregnated cord and laser were used for retraction. Materials and methods: In 40 subjects (age range of 20 to 40 years) with single missing maxillary incisor, the abutments were prepared with subgingival margins, to receive a full coverage metal-ceramic fixed dental prosthesis. The gingiva was retracted on one of the abutments with impregnated retraction cord and on the other with diode laser. Gingival attachment levels were compared at six sites per abutment using superimposition of digital scans, preoperative and four weeks after cementation of final prosthesis. Statistical analysis used: Statistical analysis of the data for gingival recession was done using t-test. Pain perception was analysed with Chi-square test. Pain perception by patients following retraction was compared with VAS scale. Results: The average values of gingival recession on buccal side were 0.61 mm and 0.38 mm and on the palatal side were 0.58 mm and 0.35 mm for impregnated retraction cord and laser respectively. The P values of <0.01 indicated a highly significant difference between the two groups. Intragroup comparison did not show significant differences between various sites. Pain and discomfort produced by cord method was moderate in comparison with mild/no pain with diode laser and the difference was highly significant.Conclusion: Retraction cord produced more gingival recession than the diode laser, which was statistically highly significant on both buccal and palatal aspects of the teeth. Patients experience with diode laser technique was less painful in comparison with retraction cord method.

Abstract:Postoperative pain is a common concern following root canal treatments (RCT), impacting both patients and oral health practitioners. This systematic review and meta-analysis aimed to evaluate the effectiveness of laser treatment modalities in reducing postoperative pain compared to conventional methods after primary and secondary RCT in permanent mature teeth. A search of three electronic databases (PubMed, ScienceDirect, and The Cochrane Library) was conducted, using a broad range of keywords and terms. Gray literature and manual searches were conducted to complement the search. The inclusion criteria included randomized clinical trials based on the objective of the secondary study. A minimum sample size of 10 participants per group and a clearly defined criterion for postoperative pain assessment were required. The characteristics of the included studies were presented as tables. The Cochrane collaboration tool RoB 2.0 was used to assess the risk of bias within each study. Two reviewers extracted the data and assessed the studies independently, and discrepancies were resolved through consultation with a third reviewer. A random-effects model was employed for meta-analysis to estimate the overall effect measure. Heterogeneity was evaluated using Cochran's Q test and the I 2 index. Publication bias was explored via Funnel plots and Egger's test. Subgroup analyses and meta-regression were conducted to assess variations among laser methods and examine the influence of independent factors. The significance threshold for all analyses was set at 5% (α = 0.05). Intraoral laser therapy demonstrated no significant advantage over conventional treatments but consistently outperformed placebo, particularly from 4 to 72 h post-treatment. Low-level laser therapy provided slight pain reduction in the first 8 h, though its effectiveness diminished in retreatment scenarios. Photodynamic therapy and laser disinfection showed marginal benefits, especially shortly after treatment, with reduced efficacy in longer-term or retreatment contexts. Further research is needed to explore different applications of laser modalities and assess distinct prognostic factors in more detail.

Use of the pulsed Nd:YAG laser for intraoral soft tissue surgery

Abstract:Application of a neodymium:yttrium-aluminum-garnet (Nd:YAG) laser was compared to conventional scalpel in dental soft tissue surgery. Two surgery sites on 29 patients were randomly selected and treated. An additional 41 patients were exclusively treated with the Nd:YAG laser. The surgical technique was then evaluated for periodontal pocket depths, degree of pain perceived, bleeding, inflammation, procedure time, and anesthesia. Surgical prognosis was made at the time of surgery and compared to actual healing 1 week and 1 month after surgery. No differences were observed between laser and scalpel surgery in terms of pocket depth reduction, postoperative pain, post-operative inflammation, and treatment time. However, operative and postoperative bleeding with laser surgery were significantly less than with conventional surgery. Anesthesia is required for scalpel surgery, the majority of laser-treated sites evoked minimal pain without anesthesia. These results indicate that the Nd:YAG laser can be used successfully for intraoral soft tissue applications are well tolerated without anesthesia and minimal bleeding compared to scalpel surgery.
