Abstract:Application of a neodymium:yttrium-aluminum-garnet (Nd:YAG) laser was compared to conventional scalpel in dental soft tissue surgery. Two surgery sites on 29 patients were randomly selected and treated. An additional 41 patients were exclusively treated with the Nd:YAG laser. The surgical technique was then evaluated for periodontal pocket depths, degree of pain perceived, bleeding, inflammation, procedure time, and anesthesia. Surgical prognosis was made at the time of surgery and compared to actual healing 1 week and 1 month after surgery. No differences were observed between laser and scalpel surgery in terms of pocket depth reduction, postoperative pain, post-operative inflammation, and treatment time. However, operative and postoperative bleeding with laser surgery were significantly less than with conventional surgery. Anesthesia is required for scalpel surgery, the majority of laser-treated sites evoked minimal pain without anesthesia. These results indicate that the Nd:YAG laser can be used successfully for intraoral soft tissue applications are well tolerated without anesthesia and minimal bleeding compared to scalpel surgery.

Abstract:Severe compression of the left renal vein produces a pressure gradient between it and the inferior vena cava and results in changes in haemodynamics. The cause of the narrowing is usually the aorta, less commonly the superior mesenteric artery. Compression of the left renal vein may be responsible for a number of abnormalities such as primary varicoceles, primary varices of the ovarian, renal, pelvic and ureteric veins on the left, the more frequent occurrence of unilateral renal vein thrombosis on the left and the development of renovascular hypertension. One hundred and twenty-three selective phlebograms of the left renal vein and CT examinations of this structure in a further 87 patients acting as a control group were carried out. The significance of compression of the left renal vein as an aetiological factor in the development of the above mentioned abnormalities is discussed.
The applications of diode and Er:YAG lasers in labial frenectomy in infant patients

Abstract:This paper describes a clinical case of labial frenectomy using different high power lasers: diode (810 nm) and Er:YAG (2940 nm). Considerations are made about specific indications, surgery techniques, and advantages of labial frenectomy using these laser wavelengths. The diode laser has high absorbance by pigmented tissues with hemoglobin, melanin, and collagen chromophores. For this reason, this wavelength is well indicated for surgery in soft tissue (vaporization, incision, coagulation, hemostasis). It is not properly absorbed, however, and should never be used in contact with hard tissues (bone). The Er:YAG laser has high absorbance to water and mineral apatite, making this wavelength useful and safe for the ablation of hard tissues. In the labial frenectomy clinical procedure, a combined technique is suggested: using the diode laser in soft tissues and the Er:YAG laser in periosteal bone tissues and for removal of final collagen fibers. It is important for the professional to understand the physical characteristics of the different laser wavelengths and their interaction with biological tissues to assure that they are used in a safe way, and that the benefits of this technology can be provided to infant patients.
Analysis of early failure of tunneled hemodialysis catheters.

Abstract:OBJECTIVE Tunneled hemodialysis catheters are often placed by the interventional radiology service using sonographic guidance and fluoroscopy for safe and optimal placement. The aim of this study was to determine the causes of early failure (<or=7 days) of these catheters in our practice. SUBJECTS AND METHODS Data were prospectively collected for 639 radiologically placed tunneled hemodialysis catheters. The reason for catheter removal was recorded in each case. Tips of removed catheters were routinely sent for microbial culture. RESULTS Fifty-two (8.1%) of 639 catheters were removed within 7 days of insertion. Six (0.9%) of these had completed their purpose and had not failed; these were not included in the study. Of the 46 catheters having early failure, six (0.9%) were clotted and 12 (1.9%) were suspected of being infected, only three of which had a proven catheter-related infection. Twenty-eight catheters (4.4%) were removed for other reasons. In this group, the most common reasons were poor tip position (n = 9) and catheter replacement over a guidewire into a preexisting fibrin sheath (n = 8). Only two failed because of poor tip orientation. Other reasons for failure were kinked or pinched catheters (n = 4) and bleeding (n = 3), including one exsanguination, and two unknown reasons. CONCLUSION By paying careful attention to catheter tip position, searching diligently for the presence of a fibrin sheath when catheter exchanges are made over a wire, and better investigating presumed catheter infection, we could reduce the early failure rate by more than half, from 46 cases to 20 cases (nine cases of suspected infection were in fact not infected).

Abstract:Intra-ocular pressure, arterial blood pressure and central venous pressure were recorded before, during and after open-heart surgery in 12 patients. In contrast to previous findings by others, no increase in intra-ocular pressure was observed during the perfusion period. The discrepancy may be explained by differences in composition of the priming solution and the degree of haemodilution during perfusion. Attention should be paid to any sudden increase in central venous pressure after the end of extracorporeal circulation, as this may produce a corresponding increase in intracranial pressure.
In vitro and in vivo effect of different fat preparations on blood coagulation.

Abstract:LIPIDs have been shown to be capable of either accelerating or inhibiting thromboplastin formation during blood coagulation. Phosphatidyl ethanolamine has been reported to be active in thromboplastin formation,1 2 while phosphatidyl serine' 2 and sphingosine3 have been considered to be inhibitors. As recent investigations have shown that phosphatidyl ethanolamine loses its accelerating activity when composed of saturated fatty acids,2 it would be reasonable to expect that dietary fats may also vary in their in vitro and in vivo effect on blood coagulation. An attempt was therefore made to assess the in vitro activity of butter, margarine, corn oil, and cod liver oil in the thromboplastin generation test. The fats were then fed to selected subjects in order to find out whether there was any correlation between the in vitro activity of the fats, and the changes in the coagulation mechanism which occur in some subjects during alimentary lipaemia.4 Only subjects who showed definite changes in platelet numbers and antihaemophilic globulin (AHG) and Christmas-factor activity during alimentary lip2emia were selected for this study. Changes in clotting factors were determined as previously described.4 Butter, which was found to have a noticeable effect on blood coagulation both in vitro and in vivo, was altered by the addition of a sufficient quantity of Folch fraction III5 (mainly phosphatidyl serine) to block its activity in the thromboplastin generation test. The brand of margarine tested was found to be comparatively inactive in the in vitro tests. Similarly, the margarine was rendered as active as butter in clotting by the addition of Folch fraction V5 (mainly phosphatidyl ethanolamine). The butter and margarine mixtures were fed to the subjects and the effect on their thromboplastin mechanism was studied. The results of these studies are reported in this communication.
Comparative Evaluation of Influence of Nd:YAG Laser (1064 nm) and 980 nm Diode Laser on Enamel around Orthodontic Brackets: An In Vitro Study

Abstract:(1) Background: The prevention of demineralizing lesions at the enamel structure level continues to represent a challenge in daily dental practice. When bacteria influence the pH level, this will decrease below the threshold for remineralization and the dissociation of hydroxyapatite will occur with a high percentage of phosphate and calcium loss. These elements continue to be studied by many authors in order to obtain a working protocol that will lead to their stabilization at the level of the enamel structure, thus preventing the demineralization process. The aim of this study is to evaluate and compare the influence of two types of laser wavelengths on the surface morphology and mineral components of the enamel through an examination with scanning electron microscopy (SEM) and energy-dispersive X-ray spectrometry (EDX). (2) Methods: Thirty permanent human incisors extracted for periodontal reasons from patients aged 25−40 years old were selected for this study. Metallic brackets (SS Standard 022 Slot, OC Orthodontics, McMinnville, OR, 97128, USA) were bonded onto each tooth. The buccal surface was randomly assigned three sections: Section A—negative control (no treatment), section B—treated with 980 nm Gallium−Aluminum−Arsenide diode laser (a 300 µm optic fiber was used with 0.8 W output power, energy density of 5.33 J/mm2, in continuous mode, for 30 s, oriented perpendicularly to the enamel surface in contact mode) (KaVo GENTLEray 980 Diode Laser, Kaltenbach & Voigt GmbH, Biberach, Germany), and section C—treated with Nd:YAG laser (a 300 µm fiber was carried out at a 1 mm distance from the enamel surface with 0.75 W power, 75 mJ pulse energy, pulse repetition rate of 10 Hz, 5 J/mm2 fluency, average exposure time of 30 s, and water cooling assisted) (LIGHTWALKER AT S, M021-5AF/1 S, Fotona d.o.o, Ljubljana, Slovenia). The elements evaluated in this study were calcium (Ca), phosphate (P), oxygen (O), and carbon (C). A one-way analysis of variance, paired t-tests, and independent t-tests were carried out to evaluate the results using the SPSS 19 IBM Statistical package software for Microsoft. (3) Results: The evaluation of the data indicated that both wavelengths produced an increase in Ca wt% (for diode laser the mean of Ca wt% before irradiation was 21.06, while that after treatment reached 28.24; and for Nd:YAG laser, the mean of Ca wt% before irradiation was 21.31, while that after treatment reached 33.88); as well, the 980 nm diode laser decreased P wt% (from 17.20 before irradiation to 16.92 after irradiation) and the Nd:YAG laser increased P wt% (from 17.46 before irradiation to 18.28 after irradiation). These results showed a statistically significant difference at the p < 0.05 level. (4) Conclusions: It can be concluded that the best improvement of enamel chemical composition was obtained with Nd:YAG irradiation.

Temperature Rise and Pain Following the Use of 810 and 980 nm Diode Lasers for Second-Stage Dental Implant Surgery: A Clinical Trial

Abstract:Introduction: Many surgical procedures in a soft tissue are performed using diodes lasers. This study aimed to assess the temperature rise and pain following the use of 810 and 980 nm diode lasers for second-stage dental implant surgery. Methods: This clinical trial was conducted on 24 osseointegrated dental implants that were randomly divided into two groups of 810 nm and 980 nm diode lasers. The temperature rise in each group was measured right after uncovering by the laser and 15 minutes later by a thermocouple, compared with the baseline temperature of gingival tissue. The level of pain was also measured at 24 hours postoperatively by using a visual analog scale. Data were analyzed by ANOVA, Tukey's test, and t test (alpha=0.05). Results: Within-group comparisons by ANOVA showed a significant difference in tissue temperature between the three time points in both groups (P<0.0001). Pairwise comparisons by Tukey's test showed that the temperature at baseline (P<0.0001) and 15 seconds after uncovering was significantly lower than that immediately after uncovering in both groups (P<0.0001). The mean tissue temperature and the mean pain score in the 980 nm laser group were significantly higher than the corresponding values in the 810 nm laser group (P<0.05). Conclusion: According to the results, temperature rise in the use of the 980 nm laser was higher than the 810 nm laser. The use of 810 nm diode laser was associated with lower temperature rise and significantly lower pain score after 24 hours.
Wound healing after therapy of oral potentially malignant disorders with a 445-nm semiconductor laser: a randomized clinical trial

Abstract:Objectives: Oral potentially malignant disorders (OPMDs) are the most clinically relevant precursor lesions of the oral squamous cell carcinoma (OSCC). OSCC is one of the 15 most common cancers worldwide. OSCC is with its high rate of mortality an important cause of death worldwide. The diagnosis and therapy of clinically relevant precursor lesions of the OSCC is one of the main parts of prevention of this malignant disease. Targeted therapy is one of the main challenges concerning an oncologically safe tissue removal without overwhelming functional and aesthetic impairment. Materials and methods: In this randomized controlled trial, a newly introduced intraoral 445-nm semiconductor laser (2W; cw-mode; SIROLaser Blue, Dentsply Sirona, Bensheim, Germany) was used in the therapy of OPMDs. Duration and course of wound healing, pain, and scar tissue formation were compared to classical cold blade removal with primary suture by measuring remaining wound area, tissue colorimetry, and visual analogue scale. The study includes 40 patients randomized using a random spreadsheet sequence in two groups (n1 = 20; n2 = 20). Results: This comparative analysis revealed a significantly reduced remaining wound area after 1, 2, and 4 weeks in the laser group compared to the cold blade group (p < 0.05). In the laser group, a significantly reduced postoperative pain after 1 week was measured (p < 0.05). Conclusion: Laser coagulation of OPMDs with the investigated 445-nm semiconductor laser is a safe, gentle, and predictable surgical procedure with beneficial wound healing and reduced postoperative discomfort. Clinical relevance: Compared to the more invasive and bloody cold blade removal with scalpel, the 445-nm semiconductor laser could be a new functional less traumatic tool in the therapy of OPMDs. The method should be further investigated with regard to the identification of further possible indications. Trail registration: German Clinical Trials Register No: DRKS00032626.
Ambulatory laser-assisted surgery: a multicenter application and experience

Abstract:Background: The widespread use of surgical lasers has found place in dermatologic and aesthetic surgery largely because they are well tolerated with a reduced incidence of postoperative haemorrhage and oedema, few associated adverse events and a high rate of patient satisfaction. Patients and methods: A retrospective analysis of 1232 consecutive patients was performed on patients undergoing a range of laser treatments between January 2005 and January 2010. A mixed variety of indications for laser use included dermatologic surgeries for the removal of fibromas, angiomas and naevi, aesthetic surgeries for acne, superficial vascular conditions, facial rejuvenations and remodelling and tattoo removals as well as many miscellaneous conditions. A range of lasers were employed including CO2, Erbium: Yttrium Aluminum Garnet (Er:YAG), diodes, Alexandrite, Ruby and Neodymium:Yttrium Aluminum Garnet (Nd:YAG) lasers with variable use for different indications in combination with a selective protocol of topical anaesthesia and local cooling systems. Results: Patient satisfaction was high overall (92.4%) with most reported failures amongst those treated for general surgical conditions where there were the highest recorded complication rates. The main failures occurred in those with small cutaneous telangiectases (36.2% incomplete treatment response and 31.9% dissatisfaction rate). In those patients where lasers were used for aesthetic reasons,(most notably in those undergoing tattoo or scar removal and in those with hypertrichosis and dermatofolliculitis), incomplete results occurred in 7.2% with an 11.8% dissatisfaction rate and rare complications (0.9%). In this group, the highest reported incomplete results occurred in patients undergoing tattoo removal (13.9%), followed by scar removal (12.5%) and then by those undergoing treatment for hypertrichosis and dermatofolliculitis (8.8%). Conclusions: Laser use in general surgery is associated with a high success rate for a wide variety of conditions with high patient satisfaction and a low incidence of adverse events in experienced hands. The advantages and disadvantages as well as the specific recommendations for different laser types are presented with clinical advances resulting from the development of non-ablative laser systems designed for dermal remodeling.
Soft Tissue Applications of Er,Cr:YSGG Laser in Pediatric Dentistry

Abstract:Soft tissue laser, such as diode and Nd:YAG lasers were initially used in soft tissue lesions because of its increased success rate. It was because of the fact that these lasers were well-absorbed by chromophores, such as hemoglobin and melanin which are found abundant in the oral mucosa. The introduction of erbium family in 1990 comprising the Er:YAG and Er,Cr:YSGG lasers made the hard tissue laser a boon for dentistry. Erbium, chromium-doped yttrium, scandium, gallium and garnet (Er,Cr:YSGG) was introduced in 1997 for the surgical needs of clinical dentistry in general practice. The erbium belongs to the rare earth which is embedded in a host crystal. The actual lasing process takes place in the Er ion Er3+. Two host crystals consisting of yttrium, aluminum, and garnet (Y3A5O12) and yttrium, scandium, gallium, and garnet (Y3Sc2Ga3O12) are added to the erbium. The interest to use these hard tissue laser in the treatment of soft tissue lesion was because of the properties of these lasers which are well-absorbed by chromophore water apart from hydroxy appetite crystals. Erbium laser energy is absorbed by collagen, hydroxyapatite, and water components. It allows the laser to cut soft tissue, tooth structure, and bone. In the noncontact mode, the incision is scalpel-like, with very little hemostasis. In contact mode, it performs soft tissue sculpting with adequate hemostasis. The Er,Cr:YSGG is the world's most advanced dental laser, which is ideal all-tissue laser because all dental tissues contain water, for the multidisciplinary dentist who performs a broad spectrum of procedures. It delivers the highest level of clinician control, operating efficiency, flexibility in tip, and accessory selection. For optimal clinical results and patient comfort in hard and soft tissue procedures, the erbium lasers have set a new standard of clinical performance. The present case series aims to highlight the various soft tissue applications of Er,Cr:YSGG (Waterlase Biolase®, Biolase, Inc, San Clemente, California, USA) in pediatric patients. Efficacy of a single Nd:YAG laser dose in reducing pain after mandibular third molar extraction: a prospective randomized controlled clinical trial

Abstract:The purpose is to explore the analgesic effect of a single Nd:YAG laser dose after mandibular third molar extraction. This was a prospective randomized controlled clinical trial. Subjects were enrolled according to the inclusion and exclusion criteria and randomly divided into the experimental and control groups. In the experimental group, the wound was irradiated with the Nd:YAG laser (wavelength, 1064 nm; output power, 1.5 W; energy density, 45 J/cm 2 ; and power density, 1.5/cm 2 , pulsed mode) immediately after mandibular third molar extraction for 120 s (30 s at each site). In the control group, the laser working tip was placed near the extraction site but not activated. The primary outcome was the visual analog scale (VAS) pain scores in both groups at 2, 4, 12, 24, 48, and 72 h and 7 days after surgery. Secondary outcomes included wound healing scores and adverse reactions. The VAS score was significantly lower in the experimental group than in the control group at 2 and 4 h after surgery, while there was no significant difference in the VAS score between the two groups at 12, 24, or 48 h or 7 days after surgery. There were no significant differences in the wound healing scores between the two groups on postoperative day 7. No adverse reactions were observed in any of the laser-irradiated areas. A single Nd:YAG laser dose was effective in reducing pain at 2 and 4 h after mandibular third molar extraction. China Clinical Trial Registry: ChiCTR2000033870 (Registration Date: 2020–6-15).

Abstract:It was previously shown that during the acute‐phase response‐induced elevated transcription of the rat haptoglobin gene, protein p55 and the lamins mediate the increased binding of restriction fragment II (−541/−146) via a 38 bp adenine tract lying 147bp upstream from the haptoglobin gene cis element (−165/−56), to scaffold type II‐like nuclear matrices. Here we show that the fragment II binding pattern to ≥40 kDa proteins of nuclear matrices analogous to type I scaffolds is more complex. Fragment II bound conspicuously to a 40 and less so to p55, a 60 kDa protein and the lamins of control matrices. During increased gene transcription, it bound prominently to p55, the lamins, and less so to 45 and 52 kDa proteins. This was accompanied by the tenacious binding of the DNA to isolated nuclear matrices in vitro and post‐translational modifications of certain matrix proteins. The lamins and p55 demonstrated a greater N‐acetylglucosamine/sialic acid content and p55 an increased in vitro phosphorylation by a nuclear matrix‐associated cyclic‐nucleotide‐independent kinase. The acute‐phase response also caused an increased partitioning of p55 with the nuclear matrix. It was concluded that, as a result of a molecular remodelling of the nuclear matrix at the point of contact with chromatin, the nature of its association with region II DNA changed during elevated haptoglobin gene expression.
Nd:YAG laser therapy on postoperative pain, swelling, and trismus after mandibular third molar surgery: a randomized double-blinded clinical study

Abstract:This study aims to evaluate the effect of Nd:YAG laser therapy (NdLT) on postoperative pain, swelling, and trismus after mandibular third molar (M3) surgery. Three hundred patients were randomly divided into the Nd group ( n = 100), medication group ( n = 100), and Nd+medication (Nd+m) group ( n = 100). The WHARFE classification system was used to assess surgical difficulty. After surgery, the Nd group was irradiated by the Nd:YAG laser in very long-pulsed mode (VLP, pulse duration 1 ms, 20 Hz, 4 W, R21-C3) in 6 regions of the extraction socket with a total energy of 300 J. For the medication group, dexamethasone 0.75 mg and loxoprofen 60 mg were prescribed immediately and every 12 h thereafter for 3 days. The Nd+m group received both treatments mentioned above. Pain assessment was performed at 6, 24, 48, and 72 h postoperatively using the visual analog scale (VAS). Swelling was evaluated by changes in the distance from (1) the tragus to the labial commissure, (2) the tragus to the pogonion, and (3) the mandibular angle to the lateral canthus preoperatively and 72 h postoperatively. Trismus was assessed by the change in maximum mouth opening. Groups Nd and Nd+m had lower VAS scores at 6 h, 24 h, and 48 h ( F = 13.80, p = 0.00), but the difference between the two groups was not significant ( F = 1.34, p = 0.11). However, no significant difference was observed at 72 h ( p = 0.10). There was no significant difference in swelling or trismus among the three groups ( p > 0.05). NdLT is an effective approach to improve complications after M3 surgery.

Abstract:Lasers are widely used in oral and maxillofacial surgery. Laser therapy is effective in sterilizing the surgical wound, resecting various types of lesions and tissues, promoting blood coagulation and hemostasis. In addition, the use of the laser tends to decrease postoperative swelling or scar formation and reduce operation time. ErYAG laser uses light in the range of 5~40nm wavelength. With its high absorption rate, the Er-YAG laser can be safely used on thin soft mucosal layers, with minimal thermal damage to adjacent tissues. We hereby report 4 cases of oral mucosal diseases which were successfully managed with Er-YAG laser under local anesthesia, and also introduce the advantages and clinical utility of the Er-YAG laser.

Abstract:A modification of photoreflectance spectroscopy, which improves the precision and efficiency of measuring time-evolving surface electric fields, is proposed. This modification is explored for studying the band bending evolution under cesium adsorption on the reconstructed GaAs(001) surface and relaxation processes in the non-equilibrium adsorbate overlayer. Observation of several distinct maxima and minima in the coverage dependence of the band bending can be explained by the formation of adatom-induced surface states with a quasi-discrete spectrum.

Abstract:Objectives This study assessed alterations in sensitivity among symptomatic non-carious cervical lesions (NCCLs) following the application of three low-level diode laser wavelengths before composite restoration. It analyzed the changes in dentin topography using a scanning electron microscope (SEM). Materials and methods Nine patients with 36 NCCLs were randomly assigned intra-individually to four groups based on the laser wavelength: laser simulation, 445 nm, 660 nm, and 970 nm. Cavity preparation, irradiation, and composite restoration were performed for each lesion. Sensitivity to cold stimuli was recorded using a visual analog scale (VAS) before the intervention (baseline) and at 1 day, 14 days, 1-, 3-, and 6-month. Pulp sensibility was recorded using an electrical pulp tester (EPT) at baseline, before treatment, and at 3- and 6-month. Additionally, an in vitro examination was performed on 12 extracted human molars to yield 12 dentin discs. Each disc was randomly divided into four quadrants to receive the same laser wavelengths to determine the diameters of the tubules using SEM. Results were analyzed statistically for clinical studies by the Friedman test, while ANOVA (RM-ANOVA) was conducted in-vitro, followed by the Bonferroni test in the case of significance (p < 0.05). Results VAS readings decreased across all groups, with a significant decrease observed for 660 nm and 970 nm from 14 days to 6-month, while at 445 nm there was a significant decrease at 6-month compared to the control (p < 0.05). EPT showed a significant decrease in pain threshold levels at 660 nm and 970 nm at 3- and 6-month, while 445 nm showed a significant decrease at 6-month compared to the control (p < 0.05). The mean tubular diameter at 445 nm decreased, with no significant difference from the control, whereas a significant decrease was found at 660 nm and 970 nm compared to the control (p < 0.05). Conclusions Prior to composite restoration in symptomatic NCCLs, diode lasers with a wavelength of 660 nm showed the highest reduction in sensitivity, followed by 970 nm, whereas 445 nm diode lasers showed the least reduction. Additionally, diode lasers with wavelengths of 660 and 970 nm reduced the width of the dentinal tubules (DT) without inducing melting, as viewed under SEM.
