Abstract:Bayes' Theorem confers inherent limitations on the accuracy of screening tests as a function of disease prevalence. We have shown in previous work that a testing system can tolerate significant drops in prevalence, up until a certain well-defined point known as the $prevalence$ $threshold$, below which the reliability of a positive screening test drops precipitously. Herein, we establish a mathematical model to determine whether sequential testing overcomes the aforementioned Bayesian limitations and thus improves the reliability of screening tests. We show that for a desired positive predictive value of $\rho$ that approaches $k$, the number of positive test iterations $n_i$ needed is: $ n_i =\lim_{\rho \to k}\left\lceil\frac{ln\left[\frac{\rho(\phi-1)}{\phi(\rho-1)}\right]}{ln\left[\frac{a}{1-b}\right]}\right\rceil$ where $n_i$ = number of testing iterations necessary to achieve $\rho$, the desired positive predictive value, a = sensitivity, b = specificity, $\phi$ = disease prevalence and $k$ = constant. Based on the aforementioned derivation, we provide reference tables for the number of test iterations needed to obtain a $\rho(\phi)$ of 50, 75, 95 and 99$\%$ as a function of various levels of sensitivity, specificity and disease prevalence.

Abstract:In the realm of medical diagnostic testing, diagnostic tests can assume either binary forms, distinguishing between diseased and non-diseased states, or ordinal forms, categorizing states from non-diseased to various stages (1 to K). Another significant classification scheme for multi-class scenarios is tree or umbrella ordering, which entails several unordered sub-classes (subtypes) within a biomarker. Within tree or umbrella ordering, the classifier assesses whether the marker measurement for one class surpasses or falls below those for the other classes. Although Receiver Operating Characteristic (ROC) curves and summary measures have been adapted to accommodate tree and umbrella ordering, these approaches often yield cut-off points that generate highly sensitive tests for certain disease subtypes while compromising specificity for others. This may not be ideal for all diseases. Hence, in this investigation, we explore diverse measures of diagnostic test accuracy and optimal cut-off point selection procedures under tree or umbrella ordering to foster more specific tests. We present numerical examples and simulation studies and demonstrate the approach using real data on lung cancer.

This is because there are a wide variety of tests available. Abstract:Quality control programs rely on continuous monitoring which may generate large volume of complex data. Assessing the precision of a biological assay using quality control processes is essential to evaluate the daily variations in a testing system. Variation can be introduced by reagents, instruments and operators, as well as biological changes in the populations screened. We have proposed a statistical analysis framework which combines novel statistical analysis and visualization techniques to determine the functional relationship between the quality control and blood donor’s negative results. Flexible semiparametric regression techniques were used to determine the functional relationships between blood donor’s hepatitis B surface antigen and anti-HIV test results and the reactivity of quality control samples over a period of 9 months. We demonstrated that the use of semiparametric regression models in conjunction with the probabilistic approaches may bring comprehensive insight into understanding the significant temporal features of the data and its impact on patient’s test results. In the absence of clinically relevant cut-point(s), data-driven methodologies, such as the one described in this study may potentially have significant benefits and widespread applications.

Abstract:Purpose The purpose of this research is to address the need for a robust system to accurately determine a cutoff score by using the Angoff method and leveraging the Rasch infit and outfit statistics of item response theory by detecting and removing misfitting items in a test. Design/methodology/approach Researchers in educational evaluation support the use of grades as a complete measure to evaluate students' overall academic performance. In higher education, determining cutoff scores is crucial for assessing academic progress and achievement. Higher education institutions worldwide set predefined minimal standards of success, sometimes referred to as cutoff thresholds. These thresholds serve as benchmarks for assessing performance quality and determining eligibility for academic credit or advancement. Findings The proposed approach combines the Angoff method with Rasch item fit statistics to precisely determine the cutoff score by excluding items that do not fit well (misfitting items) in a test. This methodology holds the potential to enhance the fairness and accuracy of grading practices, ensuring that assessments align with educational objectives and provide meaningful feedback to students. Originality/value The value of this research extends to various fields where cutoff scores are used for decision-making purposes. By using Rasch item fit statistics and the Angoff method, practitioners and researchers can enhance the quality and validity of their assessments.
On the use of receiver operating characteristic curve analysis to determine the most appropriate p value significance threshold

Abstract:Abstract Background p value is the most common statistic reported in scientific research articles. Choosing the conventional threshold of 0.05 commonly used for the p value in research articles, is unfounded. Many researchers have tried to provide a reasonable threshold for the p value; some proposed a lower threshold, eg , 0.005. However, none of the proposals has gained universal acceptance. Using the analogy between the diagnostic tests with continuous results and statistical inference tests of hypothesis, I wish to present a method to calculate the most appropriate p value significance threshold using the receiver operating characteristic curve (ROC) analysis. Results As with diagnostic tests where the most appropriate cut-off values are different depending on the situation, there is no unique cut-off for the p significance threshold. Unlike the previous proposals, which mostly suggest lowering the threshold to a fixed value ( eg , from 0.05 to 0.005), the most appropriate p significance threshold proposed here, in most instances, is much less than the conventional cut-off of 0.05 and varies from study to study and from statistical test to test, even within a single study. The proposed method provides the minimum weighted sum of type I and type II errors. Conclusions Given the perplexity involved in using the frequentist statistics in a correct way (dealing with different p significance thresholds, even in a single study), it seems that the p value is no longer a proper statistic to be used in our research; it should be replaced by alternative methods, eg , Bayesian methods.

Annual Review of Statistics and Its Application

Abstract:In this review, we present an overview of the main aspects related to the statistical evaluation of medical tests for diagnosis and prognosis. Measures of diagnostic performance for binary tests, such as sensitivity, specificity, and predictive values, are introduced, and extensions to the case of continuous-outcome tests are detailed. Special focus is placed on the receiver operating characteristic (ROC) curve and its estimation, with emphasis on the topic of covariate adjustment. The extension to the case of time-dependent ROC curves for evaluating prognostic accuracy is also touched upon. We apply several of the approaches described to a data set derived from a study aimed to evaluate the ability of homeostasis model assessment of insulin resistance (HOMA-IR) levels to identify individuals at high cardio-metabolic risk and how such discriminatory ability might be influenced by age and gender. We also outline software available for the implementation of the methods.

Abstract:BACKGROUND: The misinterpretation of the results of multiple statistical tests is an error commonly made in scientific literature. When testing several outcome variables simultaneously, many researchers declare a statistically significant result for each test having a P value of <0.05, for example. This approach ignores the fact that, based on a probability result called the Bonferroni inequality, the risk of incorrectly declaring as significant > or =1 test result increases with the number of tests conducted. The implication of this practice is that many scientific results are presented as statistically significant when the underlying data do not adequately support such a claim (sometimes referred to as false-positive results). Although the sequentially rejective Bonferroni test is well known among statisticians, it is not used routinely in scientific literature.OBJECTIVE: The intent of this article was to increase the awareness and understanding of the sequentially rejective Bonferroni test, thereby expanding its use.METHODS: This article describes the statistical problem and demonstrates how the use of the sequentially rejective Bonferroni test ensures that incorrect declarations of statistical significance for > or =1 test result are bounded by 0.05, for example.CONCLUSION: The sequentially rejective Bonferroni test is an easily applied, versatile statistical tool that enables researchers to make simultaneous inferences from their data without risking an unacceptably high overall type I error rate.

Abstract:BackgroundIn clinical and epidemiological researches, continuous predictors are often discretized into categorical variables for classification of patients. When the relationship between a continuous predictor and log relative hazards is U-shaped in survival data, there is a lack of a satisfying solution to find optimal cut-points to discretize the continuous predictor. In this study, we propose a novel approach named optimal equal-HR method to discretize a continuous variable that has a U-shaped relationship with log relative hazards in survival data.MethodsThe main idea of the optimal equal-HR method is to find two optimal cut-points that have equal log relative hazard values and result in Cox models with minimum AIC value. An R package 'CutpointsOEHR' has been developed for easy implementation of the optimal equal-HR method. A Monte Carlo simulation study was carried out to investigate the performance of the optimal equal-HR method. In the simulation process, different censoring proportions, baseline hazard functions and asymmetry levels of U-shaped relationships were chosen. To compare the optimal equal-HR method with other common approaches, the predictive performance of Cox models with variables discretized by different cut-points was assessed.ResultsSimulation results showed that in asymmetric U-shape scenarios the optimal equal-HR method had better performance than the median split method, the upper and lower quantiles method, and the minimum p-value method regarding discrimination ability and overall performance of Cox models. The optimal equal-HR method was applied to a real dataset of small cell lung cancer. The real data example demonstrated that the optimal equal-HR method could provide clinical meaningful cut-points and had good predictive performance in Cox models.ConclusionsIn general, the optimal equal-HR method is recommended to discretize a continuous predictor with right-censored outcomes if the predictor has an asymmetric U-shaped relationship with log relative hazards based on Cox regression models.

Abstract:Testing has developed into the fundamental statistical framework for falsifying hypotheses. Unfortunately, tests are binary in nature: a test either rejects a hypothesis or not. Such binary decisions do not reflect the reality of many scientific studies, which often aim to present the evidence against a hypothesis and do not necessarily intend to establish a definitive conclusion. We propose the continuous generalization of a test, which we use to measure the evidence against a hypothesis. Such a continuous test can be viewed as a continuous and non-randomized interpretation of the classical 'randomized test'. This offers the benefits of a randomized test, without the downsides of external randomization. Another interpretation is as a literal measure, which measures the amount of binary tests that reject the hypothesis. Our work completes the bridge between classical tests and the recently proposed $e$-values: $e$-values bounded to $[0, 1/\alpha]$ are continuously interpreted size $\alpha$ randomized tests. Taking $\alpha$ to 0 yields the regular $e$-value: a 'level 0' continuous test. Moreover, we generalize the traditional notion of power by using generalized means. This produces a unified framework that contains both classical Neyman-Pearson optimal testing and log-optimal $e$-values, as well as a continuum of other options. The traditional $p$-value appears as the reciprocal of generally invalid continuous test. In an illustration in a Gaussian location model, we find that optimal continuous tests are of a beautifully simple form.

Abstract:Traditionally, common testing problems are formalized in terms of a precise null hypothesis representing an idealized situation such as absence of a certain "treatment effect". However, in most applications the real purpose of the analysis is to assess evidence in favor of a practically relevant effect, rather than simply determining its presence/absence. This discrepancy leads to erroneous inferential conclusions, especially in case of moderate or large sample size. In particular, statistical significance, as commonly evaluated on the basis of a precise hypothesis low p value, bears little or no information on practical significance. This paper presents an innovative approach to the problem of testing the practical relevance of effects. This relies upon the proposal of a general method for modifying standard tests by making them suitable to deal with appropriate interval null hypotheses containing all practically irrelevant effect sizes. In addition, when it is difficult to specify exactly which effect sizes are irrelevant we provide the researcher with a benchmark value. Acceptance/rejection can be established purely by deciding on the (ir)relevance of this value. We illustrate our proposal in the context of many important testing setups, and we apply the proposed methods to two case studies in clinical medicine. First, we consider data on the evaluation of systolic blood pressure in a sample of adult participants at risk for nutritional deficit. Second, we focus on a study of the effects of remdesivir on patients hospitalized with COVID-19.

Abstract:The Receiver Operating Characteristic (ROC) curve stands as a cornerstone in assessing the efficacy of biomarkers for disease diagnosis. Beyond merely evaluating performance, it provides with an optimal cutoff for biomarker values, crucial for disease categorization. While diverse methodologies exist for threshold estimation, less attention has been paid to integrating covariate impact into this process. Covariates can strongly impact diagnostic summaries, leading to variations across different covariate levels. Therefore, a tailored covariate-based framework is imperative for outlining covariate-specific optimal cutoffs. Moreover, recent investigations into cutoff estimators have overlooked the influence of ROC curve estimation methodologies. This study endeavors to bridge this gap by addressing the research void. Extensive simulation studies are conducted to scrutinize the performance of ROC curve estimation models in estimating different cutoffs in varying scenarios, encompassing diverse data-generating mechanisms and covariate effects. Additionally, leveraging the Alzheimer's Disease Neuroimaging Initiative (ADNI) dataset, the research assesses the performance of different biomarkers in diagnosing Alzheimer's disease and determines the suitable optimal cutoffs.

Abstract:Most textbooks for biostatistics only explain each individual statistical test with its mathematical formula. However, it is crucial to understand the relationships among the statistical methods and to properly integrate the individual methods to effectively apply them to real clinical research settings. The choice for valid statistical tests greatly depends on the dependency of the sample and the number of independent variables in the analyses as well as the measurement scale of dependent variables and independent variables. In this report, many statistical tests such as the two sample t-test, ANOVA, non-parametric tests, chi-square test, log-rank test, multiple linear regression, logistic regression, mixed model, and Cox regression model are addressed through hypothetical examples. The key for a successful analysis of a clinical experiment is to adopt suitable statistical tests. This study presents a guideline to clinical researchers for selecting valid and powerful statistical tests in their study design. The choice of suitable statistical tests increases the reliability of analytical results and therefore the possibility of accepting a researcher's clinical hypothesis. The proposed flowchart of appropriate tests of statistical inference will be of help to many clinical researchers to their study.

Abstract:Biomarkers are critically important tools in modern clinical diagnosis, prognosis, and classification/prediction. However, there are fiscal and analytical barriers to biomarker research. Selective Genotyping is an approach to increasing study power and efficiency where individuals with the most extreme phenotype (response) are chosen for genotyping (exposure) in order to maximize the information in the sample. In this article, we describe an analogous procedure in the biomarker testing landscape where both response and biomarker (exposure) are continuous. We propose an intuitive reverse-regression least squares estimator for the parameters relating biomarker value to response. An expression for robust standard error and corresponding confidence interval are derived. A simulation study is used to demonstrate that this method is unbiased and efficient relative to estimates from random sampling when the joint normal distribution assumption is met, and to compare the estimator to an alternative under a related sampling design. We illustrate application of proposed methods on data from a chronic pain clinical trial.

Abstract:The test used most commonly in biomedical research to compare means when measurements have been made on a continuous scale is Student's t-test, followed closely by various forms of analysis of variance. These tests require that defined populations have been randomly sampled, but there are other assumptions about populations and samples that must be satisfied. These include: (i) normality of the population distributions; (ii) equal variance in those normal populations; and (iii) statistical independence of the samples. This review offers advice to investigators on how to recognize breaches of the assumptions of normality and equality of variance, and how to deal with them by modifying the usual t-test or by transforming the experimental data. The sample-size also has an important bearing on statistical inferences: (i) if it is too small, the risk of Type II error is inflated; and (ii) inequality of sample size exaggerates the effects of inequality of variance. The assumption of independence is breached if repeated measurements are made serially rather than in random order, but adjustments to analysis of variance can be made to correct for the inflated risk of Type I error. The review also considers the problem of making multiple comparisons of means, and recommends solutions.

Abstract:This paper considers the problem of inference in observational studies with time-varying adoption of treatment. In addition to an unconfoundedness assumption that the potential outcomes are independent of the times at which units adopt treatment conditional on the units' observed characteristics, our analysis assumes that the time at which each unit adopts treatment follows a Cox proportional hazards model. This assumption permits the time at which each unit adopts treatment to depend on the observed characteristics of the unit, but imposes the restriction that the probability of multiple units adopting treatment at the same time is zero. In this context, we study Fisher-style randomization tests of a null hypothesis that specifies that there is no treatment effect for all units and all time periods in a distributional sense. We first show that an infeasible test that treats the parameters of the Cox model as known has rejection probability no greater than the nominal level in finite samples. We then establish that the feasible test that replaces these parameters with consistent estimators has limiting rejection probability no greater than the nominal level. In a simulation study, we examine the practical relevance of our theoretical results, including robustness to misspecification of the model for the time at which each unit adopts treatment. Finally, we provide an empirical application of our methodology using the synthetic control-based test statistic and tobacco legislation data found in Abadie et. al. (2010).

Abstract:We present methodology for constructing exact significance tests for cross tabulated data for "difficult" composite alternative hypotheses that have no natural test statistic. We construct a test for discovering Simpson's Paradox and a general test for discovering positive dependence between two ordinal variables. Our tests are Bayesian extensions of the likelihood ratio test, they are optimal with respect to the prior distribution, and are also closely related to Bayes factors and Bayesian FDR controlling testing procedures.

Abstract:Many statisticians resort to using the asymptotic normal approximation method to carry out statistical inference for many statistical tests, especially nonparametric ones. In this article, the saddlepoint approximation method is proposed as an alternative to the asymptotic normal approximation method to carry out statistical inference for a number of nonparametric tests for an important type of data that appears frequently in many clinical studies such as cancer and tumorigenicity studies. In clinical trials, there are many strategies through which treatments are assigned to patients. Equal allocation of both treatments is a largely prevalent approach in clinical trials to eliminate experimental bias and increase power. Accordingly, the statistical analysis is carried out based on the truncated binomial design, which is one of the designs that achieve a perfect balance between the two treatments. To clarify the accuracy of the proposed approximation method, two sets of real data are analyzed, and for the same purpose, a comprehensive simulation study is carried out.

Abstract:Objective: To statistically compare and evaluate commonly used methods of estimating reference intervals and to determine which method is best based on characteristics of the distribution of various data sets. Design and methods: Three approaches for estimating reference intervals, i.e. parametric, non-parametric, and robust, were compared with simulated Gaussian and non-Gaussian data. The hierarchy of the performances of each method was examined based on bias and measures of precision. The findings of the simulation study were illustrated through real data sets. Results: In all Gaussian scenarios, the parametric approach provided the least biased and most precise estimates. In non-Gaussian scenarios, no single method provided the least biased and most precise estimates for both limits of a reference interval across all sample sizes, although the non-parametric approach performed the best for most scenarios. The hierarchy of the performances of the three methods was only impacted by sample size and skewness. Differences between reference interval estimates established by the three methods were inflated by variability. Conclusions: Whenever possible, laboratories should attempt to transform data to a Gaussian distribution and use the parametric approach to obtain the most optimal reference intervals. When this is not possible, laboratories should consider sample size and skewness as factors in their choice of reference interval estimation method. The consequences of false positives or false negatives may also serve as factors in this decision.
