Abstract:AIM To study the effects of intrathecal magnesium sulfate on anesthesia in severe pre-eclampsia patients undergoing cesarean section. METHODS Sixty severe pre- eclampsia primiparas undergoing caesarean section were included and randomly divided into two groups(n = 30). The patients in the magnesium group were received spinal anesthesia with 0.5% bupivacaine 2 m L + sufentanil 2.5 μg + magnesium sulfate 50 mg and the other patients in the control group received bupivacaine and sufentanil only. Intra- operative pain was rescued by intravenous fentanyl 1 μg·kg-1. Postoperative pain was rescued by patient control intravenous analgesia(PCIA). Onset, duration of sensory and motor block, duration of spinal anesthesia and postoperative analgesia requirements were studied. The adverse drug reactions and the Apgar score of infants were observed.RESULTS The duration of spinal anesthesia((233.4 ± 17.2) min vs.(192.4 ± 12.4) min) and motor block((204.8 ± 20.1) min vs.(173.6 ± 17.5) min) were significantly longer in the magnesium group compared with the control group(P < 0.05). The VAS at 4 h time point after cesarean delivery in the magnesium group was significantly lower than that in the control group(P < 0.05). Fentanyl requirement for 24 h following surgery was significantly lower in the magnesium group than in the control group(( 157.3 ± 45.5) μg vs.( 243.5 ±48.7) μg, P < 0.05). The incidence rate of adverse drug reactions, such as hypotension, bradycardia,nausea, vomiting and pruritus, was similar between the two groups(P > 0.05). The Apgar score at 1 min and5 min after babies’ delivery was above seven points in the two groups. CONCLUSION In parturients with severe pre- eclampsia undergoing caesarean delivery, the addition of intrathecal magnesium sulfate to bupivacaine and sufentanil prolongs the duration of analgesia and reduces postoperative analgesic requirements without additional adverse drug reactions.
A Preliminary Study of Fetal Intelligent Navigation Echocardiography in the Normal Fetuses During Second and Third Trimester

Abstract:Objective To investigate the value of fetal intelligent navigation echocardiography (FINE,5D Heart)in displaying the normal fetal heart structure during 20 to 38 weeks'gestation.Methods Three-dimensional data of fetal heart during 20 to 38 weeks'gestation were acquired using volume transducer prospectively.The STIC volume images were post-processed with 5D heart software by three physicians with different levels of echocardiographic examination and the quality of the section images were analyzed.The intra-and interobserver repeatability was analyzed for the qualification of section images.The correlation between the quality of image and gestational age,body mass index,the placenta position,fetal initial plane position were evaluated using univariate and multivariable logistic regression analysis.Results Two hundreds and five cases of pregnant women were included,and 9 cases were excluded.Volume data acquisition was completed in 180 pregnant women,and the image acquisition rate was 91.8%.The data were assessed twice by three observers and qualified 9 section images were gotten in 156-180 cases(degree of qualification 86.7%-100%).The screening images were satisfied in 152 cases(84.4%),dissatisfied with 28 cases(15.6%).The intra-observer agreement by three observers was absolute concord in four chamber view and abdomen、stomach view;excellent in three vessel trachea view(3VT),five chamber view, left ventricular outflow tract view and superior and inferior vena cava view;good in right ventricular outflow tract view,ductal arch view and aortic arch view.The inter-observer agreement by each of three observers was absolute concord in four chamber view and abdomen、stomach view;excellent in superior and inferior vena cava view;excellent to good in five chamber view,left ventricular outflow tract view,right ventricular outflow tract view;good in 3VT,ductal arch view and aortic arch view.In a univariate analysis,the quality of the image was associated with obesity(χ2=15.338,P =0.000),gestational age(χ2=23.356,P =0.000).Multivariable analysis showed that the obesity(OR=5.587,P =0.001)and gestational age (OR=8.843,P=0.000)were factors affecting the quality of the image.The average time spent in post-processing the images for first time by three observers was(2.77±1.39)min,and there was no significant difference among observers(P > 0.05).Conclusions Fetal intelligent navigation echocardiography (FINE,5D Heart)based on STIC volumes can successfully generate 9 standard fetal echocardiography views in 86.7%-100% of fetuses in the 2nd and 3rd trimesters.This method can simplify examination of the fetal heart and reduce operator dependency.

Abstract:Objective: To record and extract features of fetal cardiac activities with a semi-rigid prototype optically-pumped magnetometers (OPM) sensor array. Methods: Fetal magnetocardiography (fMCG) data were collected from 15 pregnant women between 28 and 40 weeks gestation. Mothers were lying flat in a customized bed with sensors touching their abdomen from below using a prototype grid. fMCG was extracted to perform standard fetal heart rate variability (FHRV) analysis. Results: fMCG was observed in 13 of the 15 pregnant women. OPM FHRV indicators were in the range of previous SQUID studies. Conclusion: Semi-rigid prototype OPM system has the ability to record quality fMCG. fMCG is capable of identifying lethal cardiac rhythm disturbances in the fetus. Our novel application of OPM technology may lower costs and increase maternal comfort, thus expanding fMCG's generalizability.

Examining the effect of MgSO4 on sharp wave transient activity in the hypoxic-ischemic fetal sheep model

Abstract:Hypoxic-ischemic encephalopathy (HIE) due to lack of oxygen is a debilitating disorder experienced by a significant number of preterm infants during birth. Studies show that the brain undergoes different phases of injury following hypoxic insult, but the first 6-8 hours (known as a latent phase) are the key to treatment efficacy. Cerebral hypothermia is one known treatment, and for it to be effective it must be started during the latent phase and continued for several days. In order to determine the effectiveness of treatment it is important to pinpoint the time of insult. Monitoring of sharp wave transient activity in the hypoxic-ischemic (HI) electroencephalogram (EEG) could be a predictor for time of hypoxic insult. Due to practicality, it is optimal if this monitoring is performed automatically. Further, MgSO4 is a drug given to an increasing number of women in labor, due to its neuroprotective properties. This drug may influence transient activity in the HI fetal sheep EEG, leading to further complications in predicting hypoxic insult. This paper explores the effect of MgSO4 on sharp wave transient activity in the EEG of a HI fetal sheep. Demonstrated in this paper is the usage of a Wavelet-Type-II Fuzzy classifier to detect sharp wave transients during the latent phase of a control group fetal sheep and an MgSO4-treated fetal sheep. This detection was performed with an average overall performance of 93.21%±5.49 over 660 minutes of latent phase, post occlusion. There were no significant differences in number of sharp wave transients in the early- and mid-latent phases of injury for both fetal sheep. However, in the late-latent phase the MgSO4-treated fetal sheep had significantly fewer sharp wave transients than the control fetal sheep.

Abstract:Developing innovative informatics approaches aimed to enhance fetal monitoring is a burgeoning field of study in reproductive medicine. Several reviews have been conducted regarding Artificial intelligence (AI) techniques to improve pregnancy outcomes. They are limited by focusing on specific data such as mother's care during pregnancy. This systematic survey aims to explore how artificial intelligence (AI) can assist with fetal growth monitoring via Ultrasound (US) image. We used eight medical and computer science bibliographic databases, including PubMed, Embase, PsycINFO, ScienceDirect, IEEE explore, ACM Library, Google Scholar, and the Web of Science. We retrieved studies published between 2010 to 2021. Data extracted from studies were synthesized using a narrative approach. Out of 1269 retrieved studies, we included 107 distinct studies from queries that were relevant to the topic in the survey. We found that 2D ultrasound images were more popular (n=88) than 3D and 4D ultrasound images (n=19). Classification is the most used method (n=42), followed by segmentation (n=31), classification integrated with segmentation (n=16) and other miscellaneous such as object-detection, regression and reinforcement learning (n=18). The most common areas within the pregnancy domain were the fetus head (n=43), then fetus body (n=31), fetus heart (n=13), fetus abdomen (n=10), and lastly the fetus face (n=10). In the most recent studies, deep learning techniques were primarily used (n=81), followed by machine learning (n=16), artificial neural network (n=7), and reinforcement learning (n=2). AI techniques played a crucial role in predicting fetal diseases and identifying fetus anatomy structures during pregnancy. More research is required to validate this technology from a physician's perspective, such as pilot studies and randomized controlled trials on AI and its applications in a hospital setting.

Abstract:Electronic fetal monitoring is used to evaluate fetal well-being by assessing fetal heart activity. The signals produced by the fetal heart carry valuable information about fetal health, but due to non-stationarity and present interference, their processing, analysis and interpretation is considered to be very challenging. Therefore, medical technologies equipped with Artificial Intelligence algorithms are rapidly evolving into clinical practice and provide solutions in the key application areas: noise suppression, feature detection and fetal state classification. The use of artificial intelligence and machine learning in the field of electronic fetal monitoring has demonstrated the efficiency and superiority of such techniques compared to conventional algorithms, especially due to their ability to predict, learn and efficiently handle dynamic Big data. Combining multiple algorithms and optimizing them for given purpose enables timely and accurate diagnosis of fetal health state. This review summarizes the currently used algorithms based on artificial intelligence and machine learning in the field of electronic fetal monitoring, outlines its advantages and limitations, as well as future challenges which remain to be solved.
