Effect of Initiative Corrected Eyeball Rotation in LASIK by Iris Localization on Remnant Degrees of Astigmatism and Higher-Order Aberrations in Patients with Myopic Astigmatism

Abstract:Objective To investigate the effect of initiative corrected rotation of eyes in laser in situ keratomileusis(LASIK) by iris localization on remnant degrees of astigmatism and higher-order aberrations in patients with myopic astigmatism.Methods One hundred and fourteen subjects(228 eyes) with LASIK were divided into 2 groups according to the way of surgery,60 cases(120 eyes) were treated with iris localization wavefront-guided laser in situ keratomileusis(IR-LASIK) as IR-LASIK group and 54 cases(108 eyes) were treated with conventional LASIK as LASIK group.The angle of eyeball rotation in surgery in IR-LASIK group was recorded,which was initiatively corrected by iris localization.The differences of naked visual acuity,remnant degrees of astigmatism and higher-order aberrations between the two groups were compared at 1 month,3 months,6 months,12 months after surgery.Results The naked visual acuity in IR-LASIK group at 1 month,3 months,6 months,12 months after surgery were better than those in LASIK group,the eyes whose naked visual acuity better than 1.0 in IR-LASIK group were 100 eyes,108 eyes,108 eyes and 107 eyes,which in LASIK group were 78 eyes,79 eyes,77 eyes and 75 eyes,respectively,there were statistical differences between the two groups(all P0.05).The remnant degrees of astigmatism in IR-LASIK group at 1 month,3 months,6 months,12 months after surgery were (-0.75±0.61)D,(-0.69±0.44)D,(-0.61±0.38)D,and (-0.59±0.49)D,which in LASIK group were (-0.99±0.57)D,(-0.87±0.67)D,(-0.83±0.61)D,(-0.81±0.68)D,respectively,the former was smaller than the latter,there were statistical differences(all P0.05).The root mean square values of total higher-order aberrations in IR-LASIK group at 1 month,3 months,6 months,12 months after surgery were 0.612±0.061,0.598±0.054,0.540±0.051 and 0.512±0.037,which in LASIK group were 0.836±0.056,0.801±0.076,0.706±0.062 and 0.679±0.078,the former was smaller than the latter,there were statistical differences(all P0.05).The rate of eyeball rotation be and less than 6° in IR-LASIK group was 78.4%,more than 6° was 21.6%.Average rotation degrees of right eye was 4.60°±3.04°,of left eye was 3.25°±2.49°.Both eyes were almost with extorsion,and there was significant difference on rotation degrees between right eye and left eye(P0.01).Conclusion IR-LASIK can initiatively correct the eyeball rotation in surgery,and the remnant degrees of astigmatism and higher-order aberrations of IR-LASIK are smaller than those of LASIK.
