Abstract:Objective To introduce the characteristics of T lymphoblastic lymphoma/acute lymphoblastic leukemia(T-LBL/ALL)from blast crisis of chronic myelogenous leukemia(CML),a case report and review of lite- rature.Methods The clinical features,changes of cytogenetics and follow-up data were analyzed from a patient with T-LBL/ALL from blast crisis of CML.The clinical characteristics,diagnosis and differential diagnosis,as well as the therapy of this disorder were discussed by reviewing the relevent literature.Results A patient with T-LBL/ALL from extramedullary blast crisis of CML was verified by fluorescence in situ hybridization(FISH) analysis.On all the reviewing literatures,47 cases of this disorder had been reported,male gender was predomi- nant,with male to female ratio 2.6: 1.All were adults with a median age of 41 years(range: 19-72).About 90% cases with history of CML,but yet,did not rule out having no such history.Extramedullary lesions occurred in more than 75% of cases,the main sites involved being the lymph nodes,liver,spleen and mediastinum,while me- dullary involvement occurred only in less than 50% of the cases.All cases were diagnosed according to the findings of cy- tomorphology,immunohistopathology,as well as cytogenetic and molecular analyses such as karyotype analysis and/or Southern blot analysis and/or RT-PCR.However,only a few cases were further verified with FISH analysis.It is neces- sary to discriminate between T-LBL/ALL from blast crisis of CML and secondary Ph(-)T-LBL/ALL after CML while simultaneously occurring with CML,and de novo Ph(+)T-LBL/ALL.However,it was very difficult to distinguish be- tween them clinically,and it was mainly dependent upon the presence or absence of bcr-abl rearrangement in lym- phoid tumor cells and/or normal blood cells shown by FISH analysis.At present,there has been no standard ther- apy for this disorder,and it has responded poorly to the commonly used chemotherapy with a serious prognosis. Imatinib in combination with chemotherapy might improve the prognosis of some patients.Conclusion T-LBL/ ALL from blast crisis of CML is a rare disorder,FISH analysis is essential for its diagnosis and differential diagno- sis.Moreover,differentiation between it and its mimicking diseases might have important clinical relevance in the imatinib therapy era.
