Abstract:The article discusses the problems of obstetrics from different perspectives - science, practice, teaching - based on a review of modern technologies for reducing maternal and perinatal mortality, as well as current aspects of treatment of gynecological patients.The purpose of the study is to show that the use of modern scientific achievements and their application in practice and the educational process will improve the quality of education of students and interns at the departments of obstetrics and gynecology. We applied the results of scientific research of the department staff in the educational process. Modern, highly effective approaches to lectures with the introduction of discussion elements are presented. The structure of question-and-answer methods, their use in practical classes, which contribute to the emergence of professional competence of interns and significantly increase the efficiency of their perception of academic material, are described in detail.

Abstract:Importance: Citation analysis is a bibliometric method that uses citation rates to evaluate research performance. This type of analysis can identify the articles that have shaped the modern history of obstetrics and gynecology (OBGYN).Objectives: To identify and characterize top-cited OBGYN articles in the Institute for Scientific Information Web of Science's Science Citation Index Expanded and to compare top-cited OBGYN articles published in specialty OBGYN journals with those published in nonspecialty journals.Design, Setting, and Participants: Cross-sectional bibliometric analysis of top-cited articles that were indexed in the Science Citation Index Expanded from 1980 to 2018. The Science Citation Index Expanded was queried using search terms from the American Board of Obstetrics and Gynecology's 2018 certifying examination topics list. The top 100 articles from all journals and the top 100 articles from OBGYN journals were evaluated for specific characteristics. Data were analyzed in March 2019.Main Outcomes and Measures: The articles were characterized by citation number, publication year, topic, study design, and authorship. After excluding articles that featured on both lists, top-cited articles were compared.Results: The query identified 3 767 874 articles, of which 278 846 (7.4%) were published in OBGYN journals. The top-cited article was published by Rossouw and colleagues in JAMA (2002). Top-cited articles published in nonspecialty journals were more frequently cited than those in OBGYN journals (median [interquartile range], 1738 [1490-2077] citations vs 666 [580-843] citations, respectively; P < .001) and were more likely to be randomized trials (25.0% vs 2.2%, respectively; difference, 22.8%; 95% CI, 13.5%-32.2%; P < .001). Whereas articles from nonspecialty journals focused on broad topics like osteoporosis, articles from OBGYN journal focused on topics like preeclampsia and endometriosis.Conclusions and Relevance: This study found substantial differences between top-cited OBGYN articles published in nonspecialty vs OBGYN journals. These differences may reflect the different goals of the journals, which work together to ensure optimal dissemination of impactful articles.

The article traces the history of the Journal, its development and state of the art, while reflecting amazing facts, sometimes filled with drama. The Journal of Obstetrics and Womens Diseases has become a prominent publication, thanks to which practitioners, even in the remotest corners of Russia, can be aware of advanced medical technologies and important scientific events, and acquainted with the latest achievements in the field of obstetrics and gynecology, as is evidenced by leading articles published in the Journal.
The knowledge domain and emerging trends in the infertility field: A 67-year retrospective study

Abstract:Infertility is a significant problem influencing many couples. Our purpose was to assess the field of infertility in Obstetrics and Gynecology from 1955 to 2022 reviewing 3575 documents found in the Web of Science database. Most articles were in the areas of Reproductive Biology, Fertility, Endometriosis & Hysterectomy, and Chromosome Disorders. We found publication has increased dramatically since 1989. Agarwal, Thomas, and Sharma; United States, England, and Canada; Fertility and Sterility, Human Reproduction, and AJOG were the most-cited authors, countries, and journals, respectively. We discovered five substantive clusters: male infertility factors, female infertility factors, causes and treatment of infertility, the consequence of infertility, and assisted reproductive techniques. Using bibliometric review (Co-citation analysis) six research areas were found: semen analysis and sperm morphology, regional differences in the psychological effects of infertility, unexplained infertility, endometriosis, diagnosis and treatment of infertility, and polycystic ovary syndrome. Despite advances in understanding infertility, further research is needed.

Abstract:Gynecological cancer originating in the female reproductive tract is one of the most common cancers among women, and its incidence is increasing every year in India. This results in high mortality, poor survival outcomes, and psychological effects. Several theoretical and empirical investigations examined gynecological cancer and its varied influences. However, no bibliometric attempts were performed to screen these publications to comprehend the most recent developments and trends in gynecological cancer research in India. Thus, this study aimed to examine the recent development of gynecological cancer research using bibliometric analysis of 3378 research articles collected from the Web of Science database from 2003 to 2022. The study used the Bibliometrix and VOSviewer software to inspect the performance and the science mapping analysis. The performance analysis findings indicate an increase in publication trends after 2008 in India. In scientific production, India collaborated with 116 countries worldwide. The most productive journal, author, and institution are Plos One, Rengaswamy Sankaranarayanan, and All India Institute of Medical Sciences, respectively. By using keywords, intellectual structure, and conceptual structure, the analysis indicates that future studies could focus on cervical carcinoma, ovarian cancer, human papillomavirus, and epithelial ovarian cancer. In light of the overall findings, it is recommended that academicians and practitioners interested in gynecological cancer-based research deliver an overview of the field by providing readers with essential papers, authors, universities, concepts, and sources. These outcomes will also help scholars in gaining a better understanding of current developments, trends, and issues in gynecological cancer research.

Abstract:During the French Revolution, obstetrics underwent substantial transformations in practice, teaching, and the physical spaces where it was conducted. The revolutionary authorities implemented reforms in French medical institutions that promoted an instrument-centred style and the dissemination of novel surgical techniques in obstetrics. The selection of professors for the obstetrics chair at the newly established École de santé and the appointment of chiefs for the new maternity ward in Paris favoured proponents of a mechanistic approach to labour assistance. This essay explores the theoretical principles and societal pressures that guided these transformative reforms and the remarkable changes they introduced in healthcare and in the practise of medicine and surgery. Furthermore, it examines the consolidation of new epistemological, ethical, and professional boundaries within the context of late eighteenth-century French obstetrics. A critical section of this study focuses on the debate ignited by the contemporaries who voiced concerns that the rise of surgical interventions on pregnant women's bodies might result in unwarranted violence, in a diminishing of midwives' roles, and in a departure from the tradition of natural childbirth. These controversies among obstetricians highlight significant contradictions within the Revolutionary medical reforms.
Safety of Minimally Invasive Surgery in Gynecologic Oncology

Abstract:Patients with gynecological malignant tumor are relatively older than patients with benign gynecological diseases. The operation process for gynecological cancers is more complicated and need longer operation time than for benign gynecological diseases. Malignant tumor has possibility of wound planting and recurrence. Many doctors worry about peri-operation complications and the prognosis of patients with gynecologic cancers received minimally invasive operation. At present, the minimally invasive operation in gynecological malignant tumor was far less than in the benign gynecological diseases. But a lot of literature shows that in cervical cancer, endometrial cancer, ovarian cancer and other gynecological malignant tumors, the prognosis and peri-operation complication rate in patients treated with laparoscopic radical operation are similar or even better than patients treated with traditional open operation. In conclusion, minimally invasive treatment of gynecologic malignant tumor by doctors with laparoscopic operation skills is feasible, reasonable and safe.

Abstract:Nine years after the first edition of The Guideline for Gynecological Practice, which was jointly edited by The Japan Society of Obstetrics and Gynecology and The Japan Association of Obstetricians and Gynecologists, the 4th Revised Edition was published in 2020. The 2020 Guidelines includes 4 additional clinical questions (CQ), which brings the total to 99 CQ (12 on infectious disease, 29 on oncology and benign tumors, 29 on endocrinology and infertility and 29 on healthcare for women). Currently, a consensus has been reached on the Guidelines, and therefore, the objective of this report is to present the general policies regarding diagnostic and treatment methods used in standard gynecological outpatient care that are considered appropriate. At the end of each answer, the corresponding Recommendation Level (A, B, C) is indicated.

Abstract:Abnormal uterine bleeding (AUB), common in a cohort of women in the reproductive period, is undoubtedly an extremely important factor reducing quality of life. Persistent endometrial inflammation is a pathogenetic link associated with endometrial dysfunction. The article reflects the modern paradigm of etiology, pathogenesis and management of patients with AUB on the background of chronic endometritis according to the data of domestic and foreign literature.
Global research trends in labor analgesia: A bibliometric analysis from 2013 to 2023

Abstract:Objective: In the past decade, thousands of original articles focus on labor analgesia have published. However, little has been published in the literature that includes a bibliometric analysis of labor analgesia. Therefore, this study is designed to analyze the current status of research on labor analgesia over the past ten years and explore potential directions for the future. Methods: We retrieved the Web of Science Core Collection (WOSCC) for labor analgesia articles published from 2013 to 2023 and extracted the data from the literature. To perform the bibliometric analysis, we used CiteSpace (6.2. R5) and VOS viewer (1.6.19) as our primary analysis tools. Results: A total of 2406 articles were included, and the number of publications has increased steadily in the last ten years. The United States was the leading contributor to the area, and Harvard University was the most productive institution. The American Journal of Obstetrics and Gynecology was the most cited and influential journal. The most co-cited reference was "Epidural versus non-epidural or no analgesia for pain management in labour". The labels of the co-citation cluster have identified the characteristics of 5major clusters, such as "postpartum depression", "programmed intermittent epidural bolus", "childbirth experience" "intrapartum maternal fever" and "dural puncture", which play important roles in this field. Keywords co-occurrence and keywords burst detection showed that "vaginal birth", "postpartum depression", "maternal fever", "inflammation", "systematic review", "guidelines", "decreased risk" and "scale" were the most recent and most prominent topics of labor analgesia. Discussion: This study provided a global review of labor analgesia using bibliometric and visual techniques to provide an intuitive understanding of this topic and identify hotspots and research trends. Notably, intrapartum fever and postpartum depression have emerged as hotpots and trends in labor analgesia research, reflecting the current research landscape.

Abstract:3D printing technology is a novel method of utilizing computer-generated three-dimensional models for drawing, assembling special bioinks, and manufacturing artificial organs and biomedical products. In recent years, it has evolved into a relatively mature therapeutic approach and has been widely used in clinical and basic research. In the field of obstetrics and gynecology, 3D printing technology has been applied for various purposes, including disease diagnosis, treatment, pathogenesis research, and medical education. Notably, researchers have gained significant application experience in common gynecological and obstetrical disorders, such as intrauterine adhesions, uterine tumors, congenital malformations, and fetal congenital abnormalities. This review aims to provide a systematical summary of current research on the application of 3D bioprinting technology in the field of obstetrics and gynecology.

Abstract:" Those who cannot remember the past are condemned to repeat it. " This maxim underscores the importance of historical awareness in medicine, particularly for obstetricians and gynecologists (ObGyns). ObGyns significantly impact societal health through their care for pregnant women, fetuses, and newborns, uniquely positioning them to advocate for health initiatives with lasting societal benefits. Despite its importance, the history of medicine is underrepresented in medical curricula, missing opportunities to foster critical thinking and ethical decision-making. In today's climate of threatened reproductive rights, vaccine misinformation, and harmful ideologies, it is imperative for ObGyns to champion comprehensive historical education. The history of medicine, particularly in relation to societal issues – such as racism, discrimination, genocides, pandemics, and wars – provides valuable context for addressing challenges like maternal mortality, reproductive rights, vaccine hesitancy, and ethical issues. Understanding historical milestones and notable ethical breaches, such as the Tuskegee Study and the thalidomide tragedy, informs better practices and safeguards patient rights. Technological advancements in hygiene, antibiotics, vaccines, and prenatal care have revolutionized the field, yet contemporary ObGyns must remain vigilant about lessons learned from past challenges and successes. Integrating historical knowledge into medical training enhances clinical proficiency and ethical responsibility, fostering innovation and improving health outcomes. By reflecting on historical achievements and their impacts, current and future ObGyns can advance the field, ensuring comprehensive and ethically sound approaches to patient care. This paper highlights the crucial role of historical knowledge in shaping modern ObGyn practices, advocating for its integration into medical education to address contemporary health challenges and ethical considerations.

A Study of Patients Undergoing Abdominal Hysterectomy in Tertiary Care Institute

Abstract:Background Hysterectomy is one of the most common surgical procedures performed.Patient education, compliance, and socioeconomic status are important determinants in choosing the mode of treatment; however, medical treatment is usually inadequate.Thus, in the present study, we tried to study the various profiles of patients undergoing abdominal hysterectomies reported in tertiary care centers. Materials and Methods Brief clinical data were noted from the case records, which include age, parity, presenting symptoms, past surgical and medical history, hemoglobin status, preoperative dilatation and curettage, and indications of hysterectomy. Results Most hysterectomies were performed on women between the ages of 41 and 45, with the average parity being 3.2 and the majority of cases having three children. Abnormal vaginal bleeding associated with various forms of menstrual irregularities was the most common complaint for which patients sought advice, and the incidence of patients undergoing tubal ligation was the most common previous surgery procedure, with anemia being the most common preoperative-associated condition. The fibroid was the most common demonstration in the current study, which brought forth abdominal hysterectomy for the chosen participants. Conclusion Findings from the current study suggest abdominal hysterectomy to be the most preferred route of surgery commonly in the women of age group 40 to 45 years or higher, precisely due to the occurrence of a higher parity rate in this age category. Fibroid uterus, dysfunctional uterine bleeding, and the presence of other menstrual complaints were the most probable indications for abdominal hysterectomy demonstrated by the individuals in the present study. Moreover, anemia was the most predominant complication associated pre-operatively, followed by other clinical manifestations such as hypertension, diabetes mellitus, and urinary tract infection.
Contextualizing ovarian pain in the late 19th century-Part 1: Women with "hysteria" and "hystero-epilepsy"

Abstract:"Hysteria" and "hystero-epilepsy" were common medical diagnoses among physicians during the nineteenth century. In Paris, L'Hôpital de la Salpêtrière-originally a hospice for the poor and a prison for prostitutes and other female inmates-became a center of great interest for the possible role of neurological diseases in these conditions. At the same time in the Americas and Europe, gynecologists were removing women's ovaries in cases with the same clinical conditions, which emphasized the role of the ovaries in contemporary hysteria studies in France, Great Britain, and the United States. The objective of this article is to explore nineteenth-century conceptualizations of ovarian pain as an organ-pathological substrate for a portion of these diagnoses. The theoretical role of the pelvic organs in these diagnoses has waxed and waned over the centuries, but there have not been many detailed explorations of the associated clinical phenomena. Suggesting an organic basis (le substratum organique) for the diagnoses remains a precarious notion, given the universally repudiated role of the uterus and decreasing interest in the ovary. In contemporary literature, the potential role of the ovary has not been addressed from a detailed medical perspective, however.
Factors affecting marriage and pregnancy/childbirth among women physicians in Japan

Abstract:Background Previous studies have not elucidated the factors affecting marriage and pregnancy/childbirth. This study identifies whether work characteristics and conditions affect marriage and pregnancy/childbirth among women physicians. Methods This nationwide survey was conducted on women members of three major Japanese medical societies: internal medicine, obstetrics and gynecology, and surgery. Results A total of 7747 women participated in this study. Although the mean age at marriage was the same, the age at the first childbirth in all specialties, especially in surgery, was significantly higher than that for the general Japanese population. In addition, being in a junior position, being board‐certified, and working in a workplace other than a university hospital were positively associated with the experience of pregnancy/childbirth, whereas working hours showed no impact. Conclusions We should change the mindset regarding pregnancy/childbirth and build effective strategies in academic institutions to improve the environment for safer pregnancy/childbirth while advancing careers.

Episiotomy: history, present and future – a review

Abstract:Childbirth has always carried traumatic stress to the woman's body. To deliver with less perineal trauma, obstetricians have used episiotomies. Episiotomy is still a common practice despite the controversy regarding its use. Weighing the risks and benefits, the scientific literature supports its selective use. With the worldwide trend to reduce the rate of episiotomy, several techniques have been proposed to achieve that. However, further research is still needed to prove their efficacy. This review will shed light on the historical background of episiotomy, its different techniques, indications, and the future of its practice.

Abstract:The fields of Obstetrics and Gynecology and Critical Care often share medically and surgically complex patients. Peripartum anatomic and physiologic changes can predispose or exacerbate certain conditions and rapid action is often needed. This review discusses some of the most common conditions responsible for the admission of obstetrical and gynecological patients to the critical care unit. We will consider both obstetrical and gynecologic concepts including postpartum hemorrhage, antepartum hemorrhage, abnormal uterine bleeding, preeclampsia and eclampsia, venous thromboembolism, amniotic fluid embolism, sepsis and septic shock, obstetrical trauma, acute abdomen, malignancies, peripartum cardiomyopathy, and substance abuse. This article aims to be a primer for the Critical Care provider.
