Abstract:In September 2020, the Children's Commissioner for England published a report entitled 'Unregulated: Children in care living in semi-independent accommodation', which concluded that some of the country's most vulnerable looked after children were living in accommodation unfit for human habitation. This observation encapsulates my experience of residing in such a placement between 2009 and 2011. In this article, I refer to diary entries I made during this period, which are photographed and categorised into three areas — challenges I encountered at a micro, meso, and macro level. Following each section, I provide reflection, contextual information, and propose what may have ameliorated these issues. The photographed diary extracts are personal; they were never intended to be published. However, given that the future of England's care system hangs in the balance in the context of the Independent Care Review, they are offered as insights into the impact of unregulated placements on the physical and mental wellbeing of children. The article proposes that such settings are unsuitable for children and young people who have not yet reached adulthood and should be abandoned in favour of nurturing homes which enable them not just to barely survive, but truly thrive.

Methodological Challenges to the Evaluation of Interventions for Foster/Kinship Carers and Children: A Systematic Review

Abstract:Caregivers of children in alternative care face a complex range of challenges that can result in placement disruption and poor long-term outcomes. Interventions designed to help carers meet these challenges report positive outcomes. Nevertheless, several reviewers have reported these positive results may be mitigated by limitations in trial methodology. This review aims to systematically review these methodological challenges and limitations, to provide an analysis of the current state of the evidence base for these interventions. A systematic review was conducted into the methods used to evaluate the effectiveness of psychosocial interventions for foster and kinship carers. Limitations relating to internal validity, external validity and clinical heterogeneity were identified and synthesised. Seventeen studies met inclusion criteria. The quality of methods used in the included studies is mixed, with high and unknown levels of bias in the majority of trials. Heterogeneity in participant characteristics, intervention aims and outcome measures across interventions reflect the diversity of carer and child needs and make it difficult to generalise results or compare and synthesise the efficacy of different interventions. These factors limit the application of trial results to evidence-based clinical practice. The diverse and complex needs of this population present significant challenges to robustly evaluating interventions for foster/kinship families. Participant needs, theoretical approaches, intervention aims and outcome measures need to be better coordinated, both within trials and across the field. Exploratory research should be used to generate focussed and concrete hypotheses that can be robustly tested in high-quality randomised controlled trials. Abstract:Children with disabilities in out‐of‐home care (OOHC) are an overrepresented group in Australia, yet little is known about their circumstances, needs, and experiences within OOHC. Utilising a systematic scoping review methodology, we explored the state of knowledge about the experiences of children and young people with a disability in out‐of‐home care in Australia. Findings in this review speak to the unmet needs and challanges that children with disabilities face in child welfare systems and how systemic failures can lead to institutional pipelines of further maltreatment, adversity, and marginalisation. The review discusses the key themes in the literature, including (i) compounding trauma and placement failures, (ii) concerns of safety, stability, and neglect, (iii) limited control and choice, and (iv) disability and multisystem involvement.

Placement Matching of Children and Young People within Out-of-Home Residential Care: A Qualitative Analysis

Abstract:Residential care for children and young people is typically regarded as the least preferred option in out-of-home care by child welfare and protection practitioners. Increasingly, residential care is only used to place young people after multiple unsuccessful placements in kinship or foster care. Consequently, these young people often come into residential care with complex needs and entrenched emotional and behavioural difficulties. Coplacement of these young people in residential care has raised significant and persistent concern of peer victimisation while in residential care. Improved matching of peers within residential care may enhance safety and stability, a proposal for which young people themselves have expressed support. However, the matching of young people in residential care has received little attention in research. This article reports the qualitative findings of a multiinformant study, drawing on the first-hand experiences of 34 participants comprising eight young people and 26 practitioners in residential care, to identify the considerations they perceived as important when matching young people in residential care. Framework analysis of young people's and practitioners’ perspectives identified three matching strategies that may more effectively address the concern of peer victimisation in residential care including (1) balancing risks and strengths; (2) steering into collective duty of care; and (3) involving young people in planning and transition. Implications of the findings directly point to the need to prioritise young people’s rights to protection and participation, and to provide independent oversight to assist residential care organisations and the broader child welfare system to navigate the complexity in the practice of matching young people in residential care.

[Admission with freedom restriction of minors to child and adolescent psychiatrics clinics--medical decisions without regard for psychodynamic sequelae?]

Abstract:The placement of minors who have been given a custodial sentence is barely mentioned in child psychiatric literature. The aim of this contribution is to present the statutory rules in civil law and public law regulating placement and to discuss the similarities and differences between them. The legislation on the placement of minors, which is concurrent in most of the German states, and the resulting problems for legal laypersons (such as doctors) are explained. Uncertainties among family and guardianship courts in regard to application of the law are examined. Case reports are presented to show the relevance to clinical practice. The conclusion can be drawn that the competing legislative regimes must be taken into account in each individual case when placing minors who have been given a custodial sentence. The application of legislation should take into account psychodynamic effects on the minor and his or her parents or guardians. Under certain circumstances, an insufficiently considered legal decision can have a counterproductive effect on the course of the disease. It is therefore recommended to proceed according to the principle of "priority to the better form of placement" when placing minors who have been given a custodial sentence.

Abstract:This is a perspective from an Independent Advocate in England, United Kingdom on the importance of equality in the involuntary treatment of children and young people (CYP). The article highlights the need for safeguards when CYP require detention as part of their mental health care. The paper raises concern that CYP and their families who are less empowered to advocate for optimal care plans may be at risk of less satisfactory outcomes from mental health detention. It notes that CYP in the care system may be particularly vulnerable to such outcomes due to their lower levels empowerment. To mitigate this risk, services need to be proactive in reducing inequity arising from differential levels of empowerment among service users. This could be achieved by adopting strong participation and coproduction activities and ensuring access to Advocacy services for all CYP.

Abstract:Parents caring for children and young people with disabilities typically have extensive additional demands on their time and resources. This added pressure can significantly impact well-being and mental health. In extreme circumstances, parents may seek an out-of-home placement for their child. Previous research has looked into factors that influence decisions for families to place their child into out-of-home care but little is known about outcomes for these young people and their families. The Supporting Families study aimed to explore the impact of a voluntary out-of-home placement on young people with disabilities, and consequences for their families. Fourteen parents/carers, twenty six case managers, six accommodation services' managers, and four young people with disabilities participated in face-to-face and telephone interviews and focus groups. Participants reported a range of outcomes for young people in care. Positive outcomes included increased levels of respect for themselves and others, an improvement in independent living skills, and reductions in challenging behaviours. Negative outcomes centred on their experiences of grief, loss and rejection, as well as behavioural problems. Positive and negative outcomes were also found for families. For many parents/carers there was a reduction in perceived stress and caring load, as well as improved mental health and wellbeing for them and the child's siblings. However, parents/carers often experienced ongoing feelings of guilt, grief and loss. The study adds to knowledge about outcomes of being in voluntary out-of-home care for this small but vulnerable group of young people in care and their families.

Abstract:Objectives. The goal of this review is to identify the experiences of the adoption assessment process for LGBTQ + adoptive parents. The intention is to highlight the scope of current literature, identify any research gaps, and from these, make recommendations for policy, practice, and research. Design. A scoping review. Methods. Social Policy and Practice, Medline, PsychINFO, ASSIA, British Education Index, International Bibliography of the Social Sciences, Scopus, Social Services s, and Google Scholar databases were searched. Articles were screened at the title and abstract level and at full text by two reviewers. The PAGER framework for scoping reviews was utilised. Results. A total of 413 articles were screened at the title and abstract level, of which 74 were also assessed at full text for eligibility. The 16 studies identified for inclusion originated from 6 different countries, with the most prevalent being the United States (10). Conclusions. Adoption processes are heterocentric which creates difficulty for LGBTQ + people in navigating them effectively. Examples of inclusive practice are evident in the literature. Process change and inclusion need to occur at organizational and policy levels rather than being the sole responsibility of social care practitioners. Future research is needed with underrepresented groups within the LGBTQ + community.

Abstract:Objective: Foster family care is associated with adverse short- and long-term consequences for the child. A systematic review was conducted on interventions for foster children and foster careers. Method: A comprehensive search process was used to find eligible interventions evaluated in randomized controlled trials or quasi-experimental studies. The quality of studies was assessed with GRADE, and effects were synthesized using meta-analytic methods. Results: In all, 28 publications of 18 interventions, including 5,357 children, were identified. Only three specific interventions had sufficient confidence of evidence. No study had examined tools for foster parent selection nor had evaluated preservice programs related to outcomes. Discussion: These analyses provide new insights and hope into the field of systematic interventions in foster care. The overall results indicate that it is possible to improve eight outcomes but cannot point out which programs are superior. Ethically, social care organizations should systematically collect knowledge about effects and side effects.

Continuous caring relationships are crucial to the wellbeing of children living in out-of-home care, with the family environment of long-term foster care usually the preferred placement for achieving relational permanence. Some children, however, experience a cycle of instability with the impact of placement disruption exacerbating emotional and behavioural difficulties and thereby undermining subsequent foster relationships. This paper reports on a service evaluation of a residential facility specialising in short-term placements for children who have experienced disruption, with the aim of interrupting the cycle of placement instability. Overview of 34 children's placement trajectories show that most were enabled to re-enter foster care and of those who are now aged over 18 years (n = 29), over half (n = 16) remained in family placements until adulthood. For four children, detailed individualised assessment indicated residential care as the most appropriate option for avoiding future disruption. The paper gives cause for optimism that relationships in the residential setting have the potential to build emotional resilience and capacity for connection. There is paucity of information on how residential staff achieve these relationships in practice. Focus groups and interviews with ten staff and managers illuminate relational practices in the residential setting that are simultaneously family-like and trauma-informed.Plain Language SummaryLong-term caring relationships in stable placements are important to the wellbeing of children who are not living with their parents. However, some children experience repeated instability and disruption when successive foster care placements end prematurely, sometimes under difficult circumstances. This can be a very upsetting experience and may have a serious impact on children. Emotions and behaviours that they struggle with can worsen, which may make things harder for them with their next foster family, leading to a cycle of instability, with fewer opportunities for continuous long-term caring relationships.This paper looks at an approach used by a residential facility, Children's House, where children who have experienced many foster placement endings can temporarily live. The goal is to stop the cycle of premature placement endings and to offer an experience of stability. The paper notes the journeys of 34 children who spent a period living at this specialist residential unit. Many (29 children) are now aged over 18 years, and over half of these (16 children) were able to stay in family placements until reaching adulthood. For four children, assessments recommended that residential care was the best option for avoiding future disruption in their lives.The findings of this paper suggest that relationships with staff in residential settings may offer children opportunities for genuine connection, as well as building emotional resilience. There is limited information available on how staff develop caring relationships with children in residential settings. This paper includes data from focus groups and interviews with ten staff and managers from Children's House. This helps to explain how relationships are built, in a way that is similar to family life but also shaped by staff's knowledge about how trauma can affect children's ability to form relationships.

Abstract:Private law childrens proceedings typically involve court disputes between parents who have separated and disagree about child arrangements, and are asking the court to make orders that determine where a child should live and with whom they should spend time. Children involved in private law, who potentially represent a vulnerable group, commonly receive less attention in policy than those in public law cases. The aim of this review was to shine a light on the wellbeing and other important characteristics or outcomes of children who are currently, or have been, involved in family law proceedings due to parental separation, to identify the support needs of these children who are often overlooked in policy. This rapid review is intended for policymakers who are responsible for policy concerning children and families as well as for family law professionals and families in private law childrens proceedings. The literature searches were conducted between June and August 2024. The included literature was published between 2001 and 2022. 22 studies reported in 25 documents were identified (8 published in academic journals and 17 in reports produced by organisations). Originated in England and Wales (n=13), Australia (n=7), Canada (n=1), New Zealand (n=1). Most studies aimed to describe the characteristics of children who are or have been involved in private family law proceedings, whilst only one compared the outcomes of such children to those in the general population. Almost all of the studies addressed mental health and emotional wellbeing. Written accounts of children, parents, and professionals described children as having anxiety, depression, anger, post-traumatic stress disorder symptoms, and eating disorders, and experiencing self-harm and suicide attempts. In Wales, children with a history of involvement in private law proceedings had higher incidence of depression and anxiety than children in the general population. From the evidence, it was unclear whether the poor mental health was associated with parental separation, the court proceedings, court orders, or some other factors, but some participants attributed difficulties to unwanted court orders. Other key areas of evidence included engagement with mental health services, behaviour, development, social relationships, learning and education, and physical health.
[Interest in periodic health examinations for young people in the judicial system]

Abstract:The Centre for Health Examinations (CES) in Roche sur Yon has experience with the Periodic Health Examinations (EPS) on population groups in fragile or disadvantaged situations and young people in the process of integration. Minors followed by the Judicial Protection of the Youth (PJJ) are in a preoccupying state of health. Professionals led a working group for reflection from two institutions working in health management. This study aims to explore the representations of young people's health under the care of the judicial system in order to evaluate the pertinence of EPS in the health course of youth in the judicial correctional system. 23 semi-directed interviews allowed the team to show that if the youth have a somatic definition of health, they are nevertheless open to a comprehensive approach to health. Their parents are unavoidable reference points. The readability of the speakers in terms of mental health remains average. The resources in health documentation are under-utilised. The treating doctor remains a special partner for health. The knowledge of social rights is insufficient. The notion of risk and the need for more information concerns the areas of drunk driving, sexuality and road rage. The EPS very logically places itself in the health course of the youth. The partnership between the health and the justice systems should be constructed in the framework of a convention between the two institutions.
[Violence in contact and custody proceedings-A case of child endangerment in the family court-Where are efficient child protection measures?]

Abstract:Against the background of 30 years of discussion in medicine and law about unfounded accusations and the existence of reasonable suspicion of intra-family violence in contact and custody proceedings, this article emphasises the importance of focusing on child protection by all professionals.Based on a case report in connection with the Federal Ministry of Justice's handout "Dealing with violence in custody and contact proceedings", the article describes the relevance of child protection in Austrian case law and the means by which it is undermined. In the name of science, ideological diagnoses are used instead of evidence-based treatment diagnoses [1]. "Professionals from the healthcare system, youth welfare, justice and education should cooperate in child protection with the aim of recognising, identifying and ending child abuse, maltreatment and/or neglect as such (cf. § 3 KKG) [2]."In legal practice, however, practitioners in the psychosocial, psychosomatic and psychotherapeutic fields are often not recognised as witnesses because of the assumption they are influenced by parents or patients themselves. In Austria, the evaluation of evidence is the responsibility of the judge and it is often assumed that practitioners cannot take an objective position towards their patients and their relatives. Specialist medical or psychological experts are therefore of crucial importance. When taking their oath, they have committed themselves to objectivity and impartiality in the recording of findings and orientation towards scientific principles and standards ("state of the art") and their application, as well as to continuous further training and education in the preparation of expert opinions [3].The specific case in question is a child with a diagnosis of post-traumatic stress disorder following suspected sexualised violence, enuresis and enkopresis in connection with the traumatic experiences.The connection between violent situations and child and adolescent psychiatric disorders and the course of the custody and contact proceedings are described. Relevant legal interests such as the will of the child, the best interests of the child, attachment tolerance and protection against violence are addressed and put up for discussion. Topics such as domestic, psychological, sexualised, institutional violence and intimate terror are presented.The case report is an example of how child psychiatrists deal with points relevant to the Federal Ministry of Justice's handbook "Dealing with violence in custody and contact proceedings", which was published in January 2024. It represents a guideline for the area of court proceedings, assessment and treatment.
At the Edge of Care: A Systematic Review and Thematic Synthesis of Parent and Practitioner Views and Experiences of Support for Parents with Mental Health Needs and Children’s Social Service Involvement

Abstract:A range of professionals and services are often involved in supporting parents with mental health needs where there are child protection concerns. However, they do not always meet the needs of this population who tend to experience inadequate support and mistrust of services. This review aimed to synthesize parent and practitioner experiences of support for parents with both mental health needs and children’s social services involvement. We performed electronic searches of the following databases: PsycINFO, CINAHL, HMIC, MEDLINE, Embase, Social Policy and Practice, Social Services Abstracts, Social Science Citation Index, OpenGrey, Social Care Online, and ProQuest. Following searching and screening, 41 studies were identified including 359 parents and 1370 practitioners. We worked with a Lived Experience Advisory Group to develop the following themes: (1) a downward spiral of service intervention; (2) working with parents, not against them; (3) support wanted versus support provided; and (4) constrained by service rigidity. We found that families were often parenting amidst trauma and adversity. However, service involvement could trigger a “downward spiral” of stressful processes over which parents felt they lacked control. Instead of improving their situations, support sometimes added to families’ difficulties, worsening parents’ mental health and making them feel marginalised, criticised, and retraumatised. There were, however, also examples of positive practice, where practitioners and parents developed trusting, open, and mutually respectful relationships. Practitioners often felt that they were limited in their ability to offer collaborative, holistic care because services were fragmented, underfunded, crisis driven, and inflexible. Difficulties mentioned most often by parents, such as financial issues, tended not to be a focus of available interventions. We conclude that the key issues identified must be targeted to improve support.

A modified video-feedback intervention for carers of foster children aged 6 years and under with reactive attachment disorder: a feasibility study and pilot RCT

Abstract:Background: Looked-after children are at risk of suboptimal attachment patterns and reactive attachment disorder. However, access to interventions varies widely and there are no evidence-based interventions for this disorder. Objectives: (1) To adapt an existing video-feedback intervention to meet the specific needs of foster children in the UK with reactive attachment symptoms, (2) to conduct a case series to road-test the treatment manual and study procedures, (3) to conduct a scoping study of the key hurdles in a pilot trial and (4) to conduct a pilot randomised controlled trial of the adapted intervention to determine the feasibility of a future full-scale trial. Design: This was a mixed-methods study. The adapted treatment manual was developed with expert input and tested on a small case series. Qualitative interviews with key stakeholders were used in the scoping study in preparation for the trial and later with foster carers who received the new intervention. The final stage was a feasibility and pilot randomised controlled trial of the new intervention, compared with usual care. Researchers assessing the outcomes were blinded to group assignment. Setting: The study was set in outpatient child and adolescent mental health services and partner social services departments. Sites included urban and rural/semirural areas. Participants: Participants were foster carers with children aged ≤ 6 years presenting with difficulties in the domain of reactive attachment disorder. Key stakeholders included children's services managers and mental health service practitioners in the scoping study. Foster carers who received the modified intervention participated in qualitative interviews. Intervention: The video-feedback intervention to promote positive parenting and sensitive discipline is an extensively evaluated and effective treatment approach. This intervention was modified (based on the adapted version for foster care in the Netherlands) to suit the needs of young children with reactive attachment symptoms in foster care in the UK and was delivered to improve the sensitive responding of foster carers, foster carer-child relationships and child outcomes. The modified intervention was delivered in-home by trained mental health professionals over a period of 4-6 months. Main outcome measure: The main outcome was reactive attachment symptom scores on the Disturbances of Attachment Interview. Results: A series of minor changes to the intervention programme were introduced, which focused on improving its suitability for the UK foster care context. Challenges in recruitment meant that, despite numerous modifications to the protocol and the inclusion of additional sites, only 30 families (target, n = 40) were recruited to the randomised controlled trial (15 allocated to each group). However, most other trial parameters were deemed feasible and acceptable, particularly the high levels of data and treatment completeness. All randomised families were available for baseline analyses, but two in the treatment arm were not available for post-treatment analyses. The revised intervention was positively received by practitioners and foster carers. Limitations: Only three-quarters of the target sample size was recruited. Furthermore, the sites' own exclusion of potential participants and the low return rates of screening questionnaires raise the possibility of non-randomness of non-responses. Conclusion: A larger-scale trial may be feasible, but only if recruitment barriers can be overcome. Kinship Care and Child Protection in High-Income Countries: A Scoping Review

Abstract:Kinship care is a global phenomenon with a long history, which in high-income countries (HICs) at least, is being increasingly formalized through legislation and policy. There are many benefits to kinship care, including improved child mental health and well-being when compared to other types of out-of-home care. Despite this, kinship care is not without its risks with a lack of support and training for kinship carers putting children at an increased risk of abuse and neglect. This scoping review was conducted across 11 databases to explore the breadth and depth of the literature about abuse and neglect within kinship care in HICs and to provide initial indications about the relationship between kinship care and abuse. Of the 2,308 studies initially identified, 26 met the inclusion criteria. A majority of studies were from the United States, and most used case review methods. From the included studies, rates of re-abuse, and particularly rates of physical and sexual abuse, appear to be lower in kinship care settings when compared to other out-of-home care settings, but rates of neglect are often higher. This review has demonstrated that a small but significant number of children living in kinship care experience neglect or abuse.
Program Components of Psychosocial Interventions in Foster and Kinship Care: A Systematic Review

Abstract:Foster children frequently experience early trauma that significantly impacts their neurobiological, psychological and social development. This systematic review examines the comparative effectiveness of foster and kinship care interventions. It examines the components within each intervention, exploring their potential to benefit child and carer well-being, particularly focussing on child behaviour problems, and relational functioning. Systematic searches of electronic databases included PsycINFO, MEDLINE, Web of Science Core Collection, the Cochrane Collaborations Register of Controlled Trials (CENTRAL) and Scopus to identify randomised or quasi-randomised trials of psychosocial foster/kinship care interventions, published between 1990 and 2016. Seventeen studies describing 14 interventions were included. Eleven studies reported comparative benefit compared to control. Overall, effective interventions had clearly defined aims, targeted specific domains and developmental stages, provided coaching or role play, and were developed to ameliorate the effects of maltreatment and relationship disruption. Interventions effective in reducing behaviour problems included consistent discipline and positive reinforcement components, trauma psychoeducation, problem-solving and parent-related components. Interventions effective in improving parent-child relationships included components focussed on developing empathic, sensitive and attuned parental responses to children's needs. Given the prevalence of both behaviour problems and relational difficulties in foster families, targeting these needs is essential. However, interventions have tended to measure outcomes in either behavioural or relational terms. A more coordinated and collaborative research approach would provide a better understanding of the association between parent-child relationships and child behaviour problems. Professionals' decision-making about out-of-home placements following instances of child abuse

Abstract:Objective: The goal of this research was to study how different groups of child welfare professionals prioritize and use information to make placement decisions following instances of child abuse. Method: A total of 90 juvenile court judges and guardians ad litem, Court Appointed Special Advocates (CASA), social workers, and mental health professionals responded to a detailed questionnaire describing four case studies of child physical abuse in which a parent was the perpetrator, the child was either 2 years or 6 years of age, and the abuse was either first time or chronic. Participants rated the impact of specific pieces of information regarding child, family, and system-level characteristics on their decision-making process. Results: Analyses of reactions to these vignettes demonstrate that professional groups use different kinds of information when making decisions about foster care placements. Social workers and mental health providers rely on information about the severity and pattern of abuse and on information about services offered in the past and parental responses to those services. Judges and guardians ad litem rely more heavily on information about the likelihood of a reoccurrence of abuse and the child's ability to recount the abuse, whereas CASA volunteers rely on information about the stability of the family. Conclusions: Professional group membership, rather than factors such as age or ethnicity of the child or chronicity of abuse, accounts for different patterns of prioritizing and using information when making decisions about whether a child should remain in the home or be placed in foster care. The implications for community-based training and intervention efforts are discussed.
