Abstract:OBJECTIVE To determine accurately the incidence of heterotopic gastric mucosa in the upper esophagus (HGMUE) in China, and to study the macroscopic and microscopic aspects of the lesions and to evaluate the clinical importance of HGMUE. METHODS A prospective study was made among a total of 15,228 consecutive patients, 8,573 male and 6,655 female, aged 54 (8-95), undergoing gastroscopy. Disease histories of all patients were carefully inquired, especially those regarding possible complaints including discomfort of throat and swallowing pain and so on. Special care was taken in the upper esophageal sphincter area to make sure whether the area was adequately inspected. Biopsy specimens from aberrant mucosa were obtained and the sections were stained with haematoxylin and eosin, and Giemsa stain for Helicobacter pylori. RESULTS HGMUE was found in 39 patients (0.26%) with an average age of 50. Five patients with H. pylori infection in heterotopic gastric mucosa also presented the infection in the stomach. The gastric mucosa was gastric body type in 8 patients, transitional type in 11 patients, and antral pattern in 7 patients. Intestinal metaplasia was found in 5 patients, and mild atypical hyperplasia in 2 patients. An impressive finding was coexistent erosive gastritis in 14 patients (35.9%), Barrett's esophagus in one patient (2.6%), peptic ulcer in 8 patients (20.5%), and a patient had the complication of constriction in the upper esophagus. CONCLUSION HGMUE is not rare in China. The presence of inlet patches is possibly correlated with specific symptoms. There are some severe complications in HGMUE, especially esophageal constriction. Close surveillance should be taken for rare cases with metaplasia or dysplasia in HGMUE.
Cervical inlet patch: case series and literature review

Abstract:Heterotopic gastric mucosa patch (HGMP) has been reported to occur in various parts of the gastrointestinal system from mouth to anus. Extra-gastrointestinal locations have also been reported. Presence of ectopic gastric mucosa has been associated with complications such as ulceration, bleeding, perforations and malignant transformations. Most complications are probably related to acid production. Meckel diverticulum is the most commonly reported ectopic location. Similarly, esophageal HGMP, also known as cervical inlet patch (CIP), has been increasingly reported. Nonspecific oropharyngeal symptoms are common and thought to be due to laryngopharyngeal reflux. CIP is often missed due to its location in the esophagus, just distal to the upper esophageal sphincter, making endoscopic evaluation difficult. Hence the condition is most likely to be under-reported. This article presents a series of five cases of CIP and discusses the pathogenesis, clinical presentations and management of this interesting entity.
Large Heterotopic Gastric Mucosa and a Concomitant Diverticulum in the Rectum: Clinical Experience and Endoscopic Management.

Abstract:Heterotopic gastric mucosa (HGM) in the rectum is an extremely rare clinical entity which may be missed or misdiagnosed due to a lack of knowledge. In the present study, a 14-year-old girl visited our hospital due to a 5-year history of repeated hematochezia. Colonoscopy showed a solitary superficial depressed lesion approximately 5 cm in size and a concomitant 1.5 cm deep diverticulum in the rectum. Histological examination of the endoscopic biopsy showed typical ectopic gastric mucosa in the depressed lesion and inside the diverticulum. Narrow band imaging further confirmed the histological results. Endoscopic ultrasound indicated that the lesion originated from the mucosal layer, and partially involved the submucosal layer. Endoscopic submucosal dissection was performed in this patient due to the large size and shape of the lesion. No bleeding, perforation or other adverse events were observed. The presence of HGM in the diverticular cavity greatly increased the surgical difficulty. A literature review was also carried out in our study.
A Novel Submucosa Nodule of the Rectum: A Case of the Ectopic Prostatic Tissue Outside the Urinary Tract.

Abstract:Ectopic prostatic tissue is an underreported entity, which is found most commonly in the tower male genitourinary tract, and ectopic prostate tissue outside the urinary tract is even rarer. Our patient was a unique case of ectopic prostatic tissue within submucosa of the rectum. The patient presented with rectal bleeding, and a firm, round solid submucosa nodule found in the anterior rectum at digital rectal examination, it was 1cm in diameter and 5cm above the anal verge. The size and submucosa location of this nodule were confirmed by the colonoscopy and MRI. After being removed surgically, the histopathology of the specimen sections possessed typical prostatic acini and stroma, meanwhile the immunohistochemical staining for prostate specific antigen confirmed its' prostatic nature. It is the first case to date, which involves the mural of rectum. We hypothesizes that the etiologies of ectopic prostatic tissue within the submucosa rectum attribute to embryogenetic abnormality.
Heterotopic gastric mucosa in the upper esophagus: a prospective study of 33 cases and review of literature

Abstract:Background and study aims: The prevalence of endoscopically diagnosed heterotopic gastric mucosa in the upper esophagus (HGMUE) has been reported in a few studies, and varies from 0.1 to 10%. Clinical relevance and possible association with other pathological conditions remain a matter of debate. A prospective study was carried out to determine the prevalence of HGMUE, the influence on it of age and sex, and to study the macroscopic and microscopic aspects of the lesion, its clinical relevance and possible association with other pathological conditions. Patients and methods: A total of 674 new patients with upper digestive complaints or alteration of their state of health underwent upper gastrointestinal endoscopy, with special attention paid to the proximal esophagus when withdrawing the gastroscope. They had been carefully questioned, especially regarding possible complaints, which could have drawn attention to the upper esophagus. Results: Heterotopic columnar epithelium in the proximal esophagus was found in 4.9 % of patients. No difference was observed according to age or sex. A mild to moderate chronic inflammatory infiltration of the heterotopic patch was observed in most cases, not related to the presence in the lesion of Helicobacter pylori, which was found in only one case. Pathological conditions of the gastroesophageal junction, especially esophagitis, were slightly more frequent in patients with HGMUE. Mild complaints, possibly related to the presence of the lesion, were observed in three out of the 33 cases. Conclusions: On the basis of our prospective study we consider that heterotopic columnar epithelium in the proximal esophagus is a rather common, generally asymptomatic, benign congenital anomaly. Malignant transformation of heterotopic gastric mucosa in the upper esophagus and other severe complications are rare. The need for surveillance should be reserved for the rare cases with metaplasia or dysplasia in the heterotopic columnar mucosa.
The outlet patch: gastric heterotopia of the colorectum and anus

Abstract:Aims: Gastric heterotopia (GH) has been described throughout the gastrointestinal tract. However, the colorectal region is an extremely rare location for it. We describe the clinicopathological features of GH of the colon, rectum and anus. Methods and results: We identified 33 cases in 20 males and 13 females (median age = 54 years; range = 4 months-73 years). Sites included the rectum (n = 26), anus (n = 4), ileocaecal junction (n = 1), ascending colon (n = 1) and descending colon (n = 1). Presenting symptoms (n = 27) included haematochezia (41%) and altered bowel habits (4%); 15 patients (55%) were asymptomatic. On colonoscopy (n = 31), all appeared as solitary lesions (median size = 6.5 mm, range = 2-55 mm), either as polyps (61%), raised erythematous patches (23%), an ulcer (10%), within a rectal diverticulum (3%) or a haemorrhoid (3%). Patients were managed by polypectomy. One with an associated carcinoma in the area of GH underwent resection. No morbidity related to GH itself was reported following excision. Histologically, heterotopic gastric mucosa was oxyntic (85%), mixed oxyntic and non-oxyntic (12%) and not specified (3%) types. In five patients a pyloric gland adenoma (PGA) arose from heterotopic gastric mucosa, two of which contained a focus of invasive adenocarcinoma. One case had associated surface foveolar-type low-grade dysplasia. Another had associated adenocarcinoma arising from the heterotopic mucosa. One example harboured Helicobacter pylori organisms. Conclusions: We highlight the features of GH in the distal GIT - the 'outlet patch'. Association with PGA, surface dysplasia and adenocarcinoma suggests that lower tract GH can undergo neoplastic transformation.
Heterotopic And Metaplastic Gastric Mucosa In The Duodenum: A Case Report

Abstract:We report a rare case of heterotopic and metaplastic gastric mucosa in the duodenum. Observations through histology, immunochemical analysis as well as special staining methods showed mural cells, advocate cells, endocrine cells, and mucus cells which are different in the duodenal bulb compared to the stomach glands. This results in the secretion of gastric acid, pepsin, and chymotrypsin, as well as the possession of gastric tissue structure and function, which can lead to local tissue digestion. Therefore, this report suggests that the base of the duodenum in the duodenal bulb may be one of the underlying causes of duodenal ulcers and patients with chronic inflammation of the duodenal bulb may have ectopic gastrocnemius. Due to this reason, pathological biopsy should be considered as an appropriate, clear etiology and timely ectopic fundic gland tissue local resection to prevent the occurrence of duodenal ulcer.

Abstract:Heterotopic gastric mucosa in the upper esophagus, commonly known as a cervical inlet patch (IP), is found in 1%-10% of the population. Inlet patches are commonly asymptomatic and are typically identified incidentally during endoscopy. The detection rate of inlet patches is dependent on the awareness of the endoscopist about the inlet patch during the procedure. In current literature there is a limited understanding of the malignant transformation potential of esophageal inlet patches.

Esophageal Stricture: An Uncommon Complication of Cervical Inlet Patch

Abstract:Esophageal heterotopic gastric mucosal patches (HGMP), also known as cervical inlet patches (CIP), is a rare but underdiagnosed condition characterized by the presence of salmon-colored, velvety mucosa located in the proximal esophagus, distal to the upper esophageal sphincter. The incidence of CIP ranges from 3% to 10% in adults, and its endoscopic appearance is characterized by a flat or slightly raised salmon-colored patch. In this case, we report a 78-year-old man who presented with symptoms of laryngopharyngeal reflux and dysphagia. An esophagogastroduodenoscopy showed a flat area of salmon-colored patch between 17 and 20 cm from the incisors, suggestive of a CIP complicated by stricture. The biopsy results showed an oxyntic-type mucosa lined with columnar cells consistent with an inlet patch. Esophageal dilation was done with a savory dilator with no resistance at 18 mm. The patient was placed on maintenance acid suppression therapy with proton-pump inhibitors and reported complete resolution of symptoms at the 1-month follow-up.

Abstract:Heterotopic pancreas (HP) is a congenital anomaly defined as pancreatic tissue that has no contact with the orthotopic pancreas and its own duct system and vascular supply. The most common locations of HP are the upper gastrointestinal tract, specifically, the stomach, duodenum, and proximal jejunum. Involvement of the mesentery is rare. Here, we describe a rare case of mesenteric heterotopic pancreas (MHP) in a 12-year-old girl who presented with acute abdomen. The patient underwent emergency laparotomy, and the mass and adjacent small bowel were resected. Results of the postoperative histopathologic examination confirmed the diagnosis of MHP. Observation of the patient for 12 mo postoperatively showed no evidence of recurrence. Preoperative diagnosis of HP is difficult, even in a symptomatic patient. Increased awareness and understanding of the image characteristics of MHP will aid in correct preoperative diagnosis and appropriate patient management.

Abstract:Gastric inlet patch is a rare disorder that is seen between < 0.1% to 10% of adults. It is usually found in the proximal esophagus but can be found in other parts of the gastrointestinal tract. Typically, these patches appear localized and non-circumferential and can be incidental findings. However, the inlet patch can secrete acid leading to complications such as strictures, ulceration, perforation, and malignancy. Here, we present a rare case of esophageal stenosis caused by a circumferential gastric inlet patch.

Abstract:Background/aims: To study the features of gastric heterotopic pancreas and to evaluate the feasibility, efficacy, and safety of endoscopic resection for treatment of this condition. Materials and Methods: Between August 2007 and December 2010, 60 gastric heterotopic pancreas patients were treated using endoscopic mucosal resection 1 endoscopic submucosal dissection. The definitive histological diagnosis of heterotopic pancreas was made after the endoscopic treatment. Tumor size, site, layer, complete resection rate, complications, and local recurrence rate were evaluated. Results: The mean tumor diameter was 1.4 +/- 0.1 (0.4-3.5) cm in the 60 gastric heterotopic pancreass patients during this period. Fourteen cases (23.3%, 14160) underwent endoscopic mucosal resection, and the en bloc resection rate was 64.3% (9/14). In 3 cases (21.4%, 3/14), arterial bleeding was controlled with hot biopsy forceps or a metal clip during endoscopic mucosal resection. Forty six cases (76.7%, 46160) underwent endoscopic submucosal dissection procedure, and the en bloc resection rate was 97.8% (45/46). In 6/45 cases (13.3%), arterial bleeding occurred. Pneumoperitoneum developed in 3 cases (6.5%, 3/46) during the operation. The curative resection rate was 98.3% (59/60). There were no recurrences in any cases. Conclusion: Endoscopic mucosal resection 1 endoscopic submucosal dissection is a minimally-invasive technique that allows resection of whole lesions and provides precise histological information, which is particularly suitable for gastric heterotopic pancreas.

Abstract:Gastric inlet patches are an important cause of throat and swallowing symptoms but also common incidental findings during gastroscopy. We report the results of a joined‐up approach to managing them, and introduce a physiological variable, the mucosal impedance gradient, that could help refine patient selection for ablation. Introduction Gastric inlet patches are often incidental, but can also be a treatable cause of laryngo‐esophageal symptoms. Methods We retrospectively reviewed all patients whose gastric inlet patches were diagnosed following assessment for laryngopharyngeal and swallowing symptoms. Improvement following Argon Plasma Coagulation (APC) was assessed using Minimum Clinically‐Important Difference methodology combining voice, throat, and swallowing domains. Correlations between APC response and measures of reflux and mucosal barrier integrity, measured during 24‐h pH‐impedance manometry, were obtained. Proximal and Distal Mean Nocturnal Baseline Impedance (MNBI) values were separately calculated and the novel variable of Mucosal Impedance Gradient was derived as [((Distal MNBI‐Proximal MNBI)/((Distal MNBI + Proximal MMBI)/2)) x 100]. Key Results Inlet patches were detected in 57 of 651 patients who had Transnasal Panendoscopy (8.7 ± 2.2%). There were 34 males. Mean age was 58 years. Mean duration of symptoms was 2 years. The commonest symptoms were hoarseness (n = 33), throat symptoms (n = 24), and dysphagia (n = 21), respectively. APC was used to ablate patches in 34 patients. Treatment response was 71% at a mean followup of 5.5 months. Abstract:Laparoscopic wedge resection is widely accepted as a choice of treatment for gastric submucosal tumors (SMTs). But it cannot easily be applied to tumors located near the esophagogastric junction (EGJ) due to the high risk of causing deformity or stenosis in the gastric inlet. Endoscopic submucosal exavation for the treatment of gastric heterotopic pancreas

Abstract:Objective To evaluate endoscopic submucosal exavation(ESE) in the treatment of heterotopic pancreas in stomach.Methods The clinical data of 60 cases of gastric heterotopic pancreas diagnosed by pathology between August 2007 and December 2010 in Endoscopy Center,Zhongshan Hospital of Fudan University were analyzed retrospectively.The cases were all treated with ESE,and the effects and safety were evaluated.Results Heterotopic pancreas were most commonly found in gastric antrum(51/60,85%) and greater gastric curvature(34/60,56.7%).Twenty-three cases(38.3%)were found a hollow in the top.EUS found that 2 cases(3.3%)were originated from muscular layer of mucosa,42 cases(70%)from submucosa layer,16 cases(26.7%) from muscle propria.En-bloc resection rate was 98.3%(59/60).During ESE,arterial hemorrhage happened in 9 cases(15%),which could be controlled by electric coagulation forceps or metal clips.Pneumoperitoneum was found in 12 cases(20%).There was no patient converted to surgery due to complications.During a median follow-up of 17(4-44) months,no recurrence was found.Conclusion ESE is a method not only to get the exact pathologic diagnosis but also to meet with principle of the local resection for heterotopic pancreas.It is safe,effective and worthy of spreading.
Prevalence of Helicobacter pylori infection rate in heterotopic gastric mucosa in histological analysis of duodenal specimens from patients with duodenal ulcer

Abstract:Heterotopic gastric mucosa in the duodenal bulb is a rare congenital disorder with varied clinical presentations. The mechanism of formation of a duodenal ulcer is failure of balance of the attack factor and the defense factor, which is the same as the mechanism of formation of a gastric ulcer. However, the true etiology of the duodenal ulcer remains unknown. Gastric mucosa can secrete gastric juice which injures itself, but the duodenal mucosa does not contain cells secreting a digestive enzyme. We assume that duodenal ulcers are caused by the presence of heterotopic gastric mucosa that can secrete gastric acid. This study was designed to assess the prevalence and associations of heterotopic gastric mucosa in duodenal ulcers. The present study included 137 patients who underwent biopsy or resection of duodenal ulcer. We detected gastric foveolar metaplasia due to inflammation from a heterotopic gastric mucosa using immunohistochemical staining. Heterotopic gastric mucosa consists of foveolar epithelium (MUC5AC-positive) and fundic gland (H⁺K⁺ ATPase-positive parietal cells, pepsinogen I-positive chief cells and MUC6-positive mucous neck cells), whereas gastric metaplasia is composed of foveolar epithelium without fundic glands. These specimens were stained with toluidine blue for detection of Helicobacter pylori infection. Among the 137 patients with duodenal ulcer, 76 cases (55%) had heterotopic gastric mucosa in the obtained specimens, and Helicobacter pylori was found in 45 cases (59%,45/76) among those with heterotopic gastric mucosa. Our results suggest that heterotopic gastric mucosa was strongly associated with concurrent duodenal ulcer.

Abstract:The human gastric mucosa is the most active layer of the stomach wall, involved in food digestion, metabolic processes and gastric carcinogenesis. Anatomically, the human stomach is divided into seven regions, but the protein basis for cellular specialization is not well understood. Here we present a global analysis of protein profiles of 82 apparently normal mucosa samples obtained from living individuals by endoscopic stomach biopsy. We identify 6,258 high-confidence proteins and estimate the ranges of protein expression in the seven stomach regions, presenting a region-resolved proteome reference map of the near normal, human stomach. Furthermore, we measure mucosa protein profiles of tumor and tumor nearby tissues (TNT) from 58 gastric cancer patients, enabling comparisons between tumor, TNT, and normal tissue. These datasets provide a rich resource for the gastrointestinal tract research community to investigate the molecular basis for region-specific functions in mucosa physiology and pathology including gastric cancer.

Esophageal Mucosal Metastasis from Adenocarcinoma of the Distal Stomach

Abstract:Dissemination of gastric cancer may usually occur by direct spread through the perigastric tissues to adjacent organ, lymphatic spread, and hematogenous spread. We report a rare case of gastric cancer with mucosal metastastic lesion on the upper esophagus that was diagnosed by endoscopy and endosonography. A biopsy of the esophageal mass was performed and the pathologic findings with immunohistochemical stain for Mucin-5AC are proved to be identical to that of gastric adenocarcinoma, suggesting metastasis from main lesion of the gastric cancer. The lesion could not be explained by lymphatic or hematogenous spread,and its metastasis mechanism is considered to be different from previous studies. We suggest that the gastroesophageal reflux of cancer cells could be one of the possible metastatic pathways for metastasis of esophagus from an adenocarcinoma of the stomach.
