Clinical analysis of endoscopic resection for submucosal tumors in the esophagogastric junction

Abstract:Objective: To investigate the clinical application of endoscopic dissection technique for submucosal tumors (SMTs) in the esophagogastric junction (EGJ). Methods: A total of 43 cases of SMTs in the EGJ were treated by endoscopic resection in the Endoscopy Center of the First Affiliated Hospital, College of Medicine, Zhejiang University between January 2010 and October 2013. The clinical and histopathologic features, surgical approach, en bloc resection rate, complications, and postoperative follow-up information were evaluated. Results: There were 19 males and 24 females with a mean age of 52.0±11.0 years old. The en bloc resection rate was 100%. The mean lesion size was 27.81±19.51 mm. The mean procedure time was 66.97±30.53 minutes and the mean bleeding volume during operation was 4.65±12.12 mL. Perforations occurred in 4 patients, pneumoperitoneum in 2 patients, and esophago-mediastinal fistula in 1 patient. All of them were successfully managed with conservational therapy. One patient with severe pneumothorax was treated with thoracic drainage. Pathologic examination showed that the lesions were leiomyoma (n = 19), gastrointestinal stromal tumor (n = 14), unknown pathological type of spindle cell tumor (n = 5) and intermuscular lipoma (n = 3), and mucinous cystadenoma (n = 1). No local recurrence and distant metastasis were noted during the follow-up. Conclusion: Various endoscopic resection methods based on endoscopic submucosal dissection seem to be safe and effective for SMTs in the EGJ. DOI:10.3781/j.issn.1000-7431.2014.04.008

Abstract:Esophageal subepithelial lesions (ESELs) encompass a range of diseases. Especially <2 cm in diameter, ESELs often remain asymptomatic and are usually discovered incidentally during medical examinations or endoscopies. It is reported that ESELs are found in approximately 1 out of every 300 patients undergoing routine endoscopy. [ 1 ] The most common benign histological types of ESELs include leiomyomas, granular cell tumors (GCTs), hemangiomas, lipomas, and esophageal gastrointestinal stromal tumors (GISTs), with leiomyomas being the most prevalent, accounting for approximately 90% of benign tumors. [ 2 , 3 ] The typical layer of origin of ESELs are illustrated in Figure 1.

Abstract:A 62-year-old man with a history of heartburn and regurgitation was referred for the management of a black lesion in the middle esophagus. Our Esophagogastroduodenoscopy (EGD) showed a 13 mm, black protruding lesion located 29cm from the incisor teeth. Endoscopic ultrasonography (EUS) showed a 11.7 mm×7.2 mm hyperechoic mass in the esophageal wall arising in layer 2 and 3. Biopsy of the lesion showed a black and elastic hard appearance. Pathological examination showed chronic inflammatory with coal dust underneath the mucosal epithelium.

Abstract:A 64-year-old male having experienced upper abdominal discomfort for 2 weeks underwent esophagogastroduodenoscopy (EGD) at a local hospital, which revealed a cardiac mass in the stomach. The pathological findings of the biopsy showed marked atypia and spindle cell proliferation (Fig. 1a),indicating a malignant tumor. Endoscopy reexamination at our hospital revealed a protuberant polypoid mass, with a squamous epithelium covering the oral side, and a papillary villous appearance on the anal side (Fig. 1b–d). The patient underwent endoscopic submucosal dissection (ESD) diagnostic resection.

Abstract:Esophageal submucosal gland duct adenoma (ESGDA) is a rare tumor. The clinicopathological features of the ESGDA and its precursor lesion have not been comprehensively evaluated. In this study, we aimed at delineating the clinicopathological features of the ESGDA and cyst formation of the esophageal submucosal gland duct (ESGD), as well as their correlations and clinical implications. We identified three cases of ESGDA and 16 cases of cyst formation of the ESGD among 786 endoscopic mucosal resection specimens over a 7-year period. The median patient age was 58 years with a male predominance. These lesions were small submucosal bulges locating at the lower esophagus with a size no more than 1 cm. The main microscopic changes of these lesions included content retention, multilayered epithelium or papillary folds of the ESGD and inflammatory cell infiltration, acidophilic degeneration, hyperplasia or atrophy of the acini. The included cases generally showed moderate to severe microscopic esophagitis. The ESGDA was mainly consisted by multiple glandular cysts covered by two layers of cells. Immunohistochemical results showed that the luminal duct lining cells and basal cells were positive for CK7 and p63, respectively. Both of the two layer cells were positive for HMWCK and negative for CK20, p53, CDX2, MUC5AC, MUC6, MUC2 and MUC1. The proliferation index was very low (1%). The diagnostic criteria of the ESGDA were proposed and, the differential diagnosis was discussed. Cyst formation of the ESGD is considered to be the precursor lesion of the ESGDA, because they have overlapping clinicopathological features with progressive relationship. In addition, the ESGDA have close connection with advance of the GERD and, probably, an increased risk of carcinoma.
An Esophageal Submucosal Tumor Treated with Submucosal Tunneling Endoscopic Resection: an Unexpected Result

Abstract:A 66-year-old female patient underwent esophagogastroduodenoscopy (EGD) in our center because of occasional dysphagia, and a large submucosal tumor (SMT) was identified 32–36 cm from the incisors, occupying nearly half of the luminal circumference (Figure 1A). Mini-probe endoscopic ultrasonography (EUS) revealed a hypoechoic submucosal lesion, measuring 20 15 mm and originating from the superficial muscularis propria layer (Figure 1B). It was covered by normal-appearing mucosa. The patient underwent EUS (SU-9000, Fujifilm, Japan) for further investigation (Figure 1C). EUS elastography showed high tissue stiffness with a predominantly blue pattern (Figure 1D) and there was no enhancement after injection of intravenous contrast. Overall, these findings were consistent with those of a submucosal tumor (SMT) of the lower esophagus originating in the muscularis propria, possibly a leiomyoma. The patient underwent en bloc resection of the tumor by submucosal tunneling endoscopic resection (STER). The first step of the procedure consisted of a submucosal injection 5 cm proximally to the lesion. A longitudinal mucosal incision and subsequent submucosal tunneling were performed using the Hybrid Knife (ERBE, Germany). Endoscopic en bloc resection was then carried out using both the Hybrid Knife and the IT Knife (Olympus, Japan). After retrieval of the endoscopic specimen measuring 40 x 20 mm (Figure 1E) and adequate hemostasis of the tunnel, the incision site was closed using hemostatic clips. However, histology revealed a poorly differentiated neuroendocrine carcinoma (small-cell carcinoma) invading the submucosal layer with lymphovascular invasion (Figure 1F). Immunohistochemistry staining was positive for CD56, synaptophysin, CgA and CK-PAN and negative for SSTR2, SSTR5, and p53. The Ki-67 score exceeded 60%. The patient was suggested to receive further surgical treatment. However, she did not seek any treatment. On a 3-month follow-up, investigation revealed distant metastases to both liver lobes and the left chest wall so the patient was referred for palliative care.

Abstract:Heterotopic gastric mucosa in the upper esophagus, commonly known as a cervical inlet patch (IP), is found in 1%-10% of the population. Inlet patches are commonly asymptomatic and are typically identified incidentally during endoscopy. The detection rate of inlet patches is dependent on the awareness of the endoscopist about the inlet patch during the procedure. In current literature there is a limited understanding of the malignant transformation potential of esophageal inlet patches.

Abstract:BACKGROUND AND OBJECTIVES: Subepithelial lesions (SELs) of the upper part of the digestive tract are rare, and it can be difficult to characterize them. Recently, contrast-enhanced endosonography (EUS) and elastometry have been reported as useful adjuncts to EUS and EUS-guided fine needle aspiration (EUS-FNA) in cases of pancreatic mass and lymph node involvement. The aim of this retrospective analysis was to evaluate whether contrast-enhanced EUS can discriminate benign submucosal lesions from malignant ones. We describe our retrospective experience using the contrast agent SonoVue® (Bracco Imaging, Milan, Italy) in an attempt to increase the diagnostic yield.PATIENTS AND METHODS: Between May 2011 and September 2014, 14 patients (5 men, 9 women; median age 64 years, range 31-80 years) with SELs of the stomach or esophagus underwent EUS with SonoVue® (low mechanical index). There were 3 esophageal lesions and 11 gastric lesions. Mean size of the lesions was 30 mm (range 11-50 mm). They were discovered after anemia (n = 5), dysphagia (n = 1), and pain (n = 4) and during follow-up for resected gastrointestinal stromal tumors (GISTs) (n = 1) and a standard upper gastrointestinal endoscopy (n = 3). On endoscopic sonograms, 10 of these lesions were hypoechoic and located in the fourth layer (muscularis), and 4 were in the second or third layer (mucosa and submucosa). Contrast enhancement was assessed in the early phase (after several seconds) and late phase (>30 seconds); a final diagnosis was made based on the findings of EUS-FNA using a 19-gauge ProCore (Cook Medical, Bloomington, IN) (n = 9) or 22-gauge FNA system (Cook Medical) (n = 1), the resected specimen (n = 3), or deep biopsy (n = 1). Different immunostaining was used in the pathologic studies (RNA was analyzed later using the C-kit, CD-117, CD-34, desmin, DOG-1, α-smooth actin, caldesmon, PS-100, and Ki-67 antibodies).RESULTS: Final diagnoses were leiomyoma (n = 4), GIST (n = 5), schwannoma (n = 1), inflammatory tumor of Helvig (n = 1), pancreas rest (n = 2), and fibrosis (n = 1). No complications occurred. All 5 GISTs showed enhancement in the early and late phases, whereas the 8 remaining lesions did not show any enhancement. Only 1 leiomyoma showed heterogeneous enhancement.LIMITATIONS: The monocentric and retrospective study design and small number of patients.CONCLUSIONS: In cases of SELs of the stomach or esophagus, SonoVue® could be a complementary tool to endosonography to differentiate GISTs (early and clear enhancement) from other SELs (few or no enhancement), such as leiomyomas or pancreatic rest. These results are similar to those of the few, small studies published on this topic, but more studies with a larger number of patients are needed to confirm these findings.

An Unusual Cause of Dysphagia and Melena.

Abstract:Question: A 51-year-old man complained of a dry cough, dysphagia, odynophagia, and melena for 10 months. He had a >30 pack-years smoking history, but did not consume an excessive amount of alcohol. He had no history of dyspnea, respiratory tract bleeding, or massive hemorrhage of the gastrointestinal tract. No clinically important signs were found during physical examination. Laboratory results exhibited a positive hemoccult stool test and mild anemia (hemoglobin, 107 g/L). The patient was found to have a large, soft tissue density mass in the upper esophagus on a computed tomography scan (Figure A, white arrow). Esophagogastroduodenoscopy then revealed a large, thick-pedunculated, nonpulsatile, partially erosive and necrotic subepithelial tumor (SET) 15–23 cm from the incisors (Figure B). The lesion was diagnosed as a hypopharyngeal SET because its root arose from the lateral wall of the right hypopharynx on esophagogastroduodenoscopy (Figure C, white arrow). The large body of the tumor extended into the upper esophageal lumen and led to significant narrow of the esophageal inlet. The patient’s melena and chronic blood loss anemia were attributed to erosion and necrosis of the tumor (Figure D). Based on these findings, what is the most likely diagnosis? Pharyngeal endoscopic submucosal dissection was attempted under general anesthesia (Supplementary Video). The tumor root was carefully exposed, and submucosal injection was performed around the thick peduncle. Mucosal incision was performed at the oral side of the peduncle. The submucosal layer beneath the tumor was then dissected directly from the oral side to the anal side, and saline solution was injected repeatedly during dissection when necessary (Figure E). The tumor was en bloc resected and removed with a snare. The specimen was 4 × 8 cm and weighed 16 g (Figure F). Pathologic examination showed an angiofibroma with no evidence of malignancy (Figure G; H&E × 100). The patient had no procedure-related adverse events, and his symptoms completely resolved. Abstract:A 59-year-old man experienced epigastric pain and an upper gastrointestinal endoscopy revealed a bulging lesio n 0.5×0.6cm in size in the middle esophagus. EUS showed a homogenous hypoechoic lesion, which interrupted the five layers structure of esophageal wall. Biopsies revealed epithelioid granuloma with central caseous necrosis and several acid-fast bacilli.
Oral Regurgitation of a Large Esophageal Mass

Abstract:Question: A 57-year-old man presented to the outpatient clinic with a 1-month history of worsening globus sensation and intermittent transoral mass regurgitation. The patient reported that the mass had developed gradually over the past 2 weeks. He had no significant dysphagia and had mild transient dyspnea when the mass regurgitated. He could induce nausea and freely vomit a smooth mass of 3-cm diameter into his mouth and swallow it back (Figure A, Video). A contrast-enhanced computed tomography scan of the neck revealed a 10-cm hypodense lesion originating in the cervical esophagus at the C6 level and extending distally to T3 (Figure B, C). Esophagogastroduodenoscopy showed a large, broad-based, pedunculated polypoid mass rooted at the meatus of the esophagus and extending up to 28 cm distal to the incisor (Figure D). The mass partially obstructed the laryngeal opening while prolapsing from the esophagus (Figure E). Transoral resection was successfully performed under general anesthesia with endotracheal intubation. The patient recovered uneventfully after surgery and had complete resolution of his presenting symptoms. He remained asymptomatic, and upper gastrointestinal endoscopy performed 3 months after the surgery showed no evidence of recurrence. What is the diagnosis? Examination after surgical removal showed a sausage-shaped smooth mass measuring 10 × 2.2 × 1 cm (Figure F). Histological examination of a sample of the mass revealed an admixture of massive mature adipose tissue and minimal fibrous and vascular components covered with normal squamous epithelium, characteristic of esophageal fibrovascular polyps (Figure G). Endoscopic submucosal dissection of intramucosal adenocarcinoma on Barrett's esophagus

Abstract:A 63-year-old man was investigated because of long-standing pathological gastroesophageal reflux. Diagnostic endoscopy showed long-segment Barrett's esophagus associated with a flat lesion compatible with a granular laterally spreading tumor (LST-G), and endoscopic submucosal dissection was decided upon [<a href="#JR2412-1">1</a>].A Fujinon EG-590-ZW diagnostic endoscope was used for the procedure. A Fujinon transparent conical cup and Fujifilm 1.5-mm FlushKnife BT were used as the dissecting instruments.The endoscope was advanced to the distal esophagus, where long-segment Barrett's esophagus (6 cm in length) was confirmed. In addition, a homogeneous flat lesion compatible with a LST-G of 4 cm maximum diameter was shown ([<a href="#FI2412-1">Fig. 1</a>]).Flexible spectral imaging color enhancement (FICE) and magnification were used for exhaustive assessment of the surface and margins of the lesion, which presented a granular flat segment on its edges.The edges of the lesion were marked with a safety margin of 5 mm ([<a href="#FI2412-2">Fig. 2</a>]). The lesion was then raised by submucosal injection with a solution made up of 500 mL Voluven, 2.5 mL methylene blue, and 1 mg epinephrine. A complete perimeter mucotomy, external to the marking, was performed ([<a href="#FI2412-3">Fig. 3</a>]). Careful hemostasis was carried out, followed by endoscopic dissection of the submucosal layer adjacent to the muscularis propria ([<a href="#FI2412-4">Fig. 4</a>]) [<a href="#JR2412-2">2</a>]. The entire submucosal layer of the lesion in the dissected specimen was included. The surgical bed was undamaged, with no signs of perforation and correct hemostasis ([<a href="#FI2412-5">Fig. 5</a>]; [<a href="#ME2412-1">Video 1</a>]).Video 1 Endoscopic submucosal dissection of the distal esophagus.At 6-month post-procedure follow-up the wound had healed. Re-epithelialization with squamous mucosa without evidence of esophageal stenosis was demonstrated. On this occasion, the remaining Barrett's esophagus was ablated using radiofrequency.The pathological report was of well-differentiated, intramucosal adenocarcinoma without compromise of lateral or deep margins.Carrying out ESD for incipient neoplastic lesions in Barrett's esophagus is feasible and safe [<a href="#JR2412-3">3</a>] and achieves good oncological results. Abstract:ABSTRACT Background and Objectives: Endoscopic ultrasound guided fine needle aspiration (EUS-FNA) is a minimally invasive and reliable non-surgical technique for the diagnosis of gastrointestinal lesions. The present study aimed to evaluate the spectrum of lesions encountered in the gastric subepithelium on EUS-FNA at a tertiary care center. Materials and Methods: Archival data of all patients undergoing EUS-FNA for gastric submucosal lesions over a period of 5 years was retrieved. Patient demographics, clinical presentation, and EUS findings were recorded along with the FNA results. Results: A total of 78 EUS-FNA samples were analyzed. Material was adequate in 62 cases (79.48%) and inadequate in 16 cases (12.82%) patients due to scant cellularity. Of the adequate samples, 34 (43.5%) were reported as neoplastic while 20 (25.64%) were non-neoplastic, and 8 (10.25%) were reported as suspicious of a neoplasm. In the neoplastic category, the predominant diagnosis was of spindle cell neoplasm comprising gastrointestinal stromal tumor (13), benign neural tumor (03), leiomyoma (02), and spindle cell tumors (03). The latter could not be categorized further due to a lack of IHC material. The next common diagnosis was adenocarcinoma (06) followed by neuroendocrine tumor (02) and poorly differentiated carcinoma (01). The non-neoplastic lesions included non-specific pathology (15), inflammatory lesions (08), and one case each of tuberculosis, pancreatic rest, and Brunner gland hamartoma. Cell blocks for ancillary testing were available in 54 cases (65.23%) and follow-up was available in 42 cases (53.84%). Conclusion: EUS-FNA is a good modality for the diagnosis of gastric submucosal lesions with a high diagnostic yield.

Abstract:Background and objectives: Subepithelial lesions (SELs) of the gastrointestinal (GI) tract present a diagnostic challenge due to their heterogeneous nature and varied clinical manifestations. Usually, SELs are small and asymptomatic; generally discovered during routine endoscopy or radiological examinations. Currently, endoscopic ultrasound (EUS) is the best tool to characterize gastric SELs. Materials and methods: For this review, the research and the study selection were conducted using the PubMed database. Articles in English language were reviewed from August 2019 to July 2024. Results: This review aims to summarize the international literature to examine and illustrate the progress in the last five years of endosonographic diagnostics and treatment of gastric SELs. Conclusions: Endoscopic ultrasound is the preferred option for the diagnosis of sub-epithelial lesions. In most of the cases, EUS-guided tissue sampling is mandatory; however, ancillary techniques (elastography, CEH-EUS, AI) may help in both diagnosis and prognostic assessment.

Abstract:Purpose of Review The study aimed to discuss novel endoscopic approaches to resection of submucosal lesions. Recent Findings Endoscopic resection by excavation, full-thickness resection, and submucosal tunneling are by now well-documented techniques for submucosal tumor resection. Combined approaches with laparoscopy such as several variations of laparoscopic endoscopic cooperation surgery (LECS) and non-exposed wall-inversion surgery (NEWS) are also possible now. Reports of these approaches show good results. However, lesion size is the main limitation to these approaches. Summary Endoscopic submucosal tumor resection can be effectively done endoscopically or by endoscopy combined with laparoscopy. These techniques allow localized resection of lesions at difficult locations such as in the gastroesophageal junction with limited influence on the normal anatomy. Variations in techniques are being practiced worldwide striving to achieve complete resection with minimal disturbance of function or contamination. The choice between approaches depends largely on the size and site of the lesion and expertise available.

Abstract:This study delineates a distinct subtype of esophageal submucosal benign adenoma characterised by multiple lobulated cystic proliferations of benign epithelial layers within the submucosa. BRAF V600E mutations were present in this tumour, which indicated that it might be an unrecognised esophageal counterpart of the minor salivary gland tumour. Aims Esophageal submucosal gland duct adenoma is an extremely rare benign tumour, with only a few reported cases. We conducted the largest single‐centre clinical study of esophageal submucosal gland duct adenoma, examining its molecular mechanisms and clinicopathological features. Methods and Results Between 2018 and 2023, seven cases of esophageal submucosal gland duct adenoma were identified at a tertiary medical centre; two were female and five were male, aged between 51 and 75 years (mean = 63.8 years). Comprehensive evaluations of clinicopathological, immunohistochemical and molecular characteristics were conducted. Histologically, tumours showed papillotubular and cystic patterns lined with double layers of cells arranged in ducts, papillary folds and microcysts. The inner luminal tall columnar cells had eosinophilic cytoplasm and did not show mucin production and the basal cells showed myoepithelial differentiation. Immunohistochemically, inner luminal layer cells were positive for CK7, CK19 and CK5/6 and outer basal layer cells were positive for SMA and P40. Both layers were negative for CK20, CDX2, MUC5AC, MUC6, MUC2, GCDFP15 and Alcian blue‐periodic acid–Schiff (AB‐PAS). Genomic analyses revealed the presence of BRAF V600E mutations in five of seven tumours (71.43%). Conclusions This study delineates a distinct subtype of benign adenoma arising from the esophageal submucosal gland duct, characterised by multiple lobulated cystic proliferation of benign epithelial layers within the submucosa. BRAF V600E mutations were present, similar to in sialadenoma papilliferum. We determined the genetic mutation present in esophageal submucosal gland duct adenoma, providing further evidence that it is an esophageal counterpart of minor salivary gland tumours.

Abstract:Objective To analyze the clinicopathological characteristics of upper gastrointestinal submucosal tumors ( SMTs ) . Methods Clinicopathological data of 1 743 patients with 1 775 upper gastrointestinal SMTs in our department from January 2008 to December 2012 were retrospectively analyzed. Results The first finding was that in 702 esophagus cases,leiomyoma(92?59%,650/702) was the most common type of esophageal SMTs. Second, in 1 045 gastric cases, there were 405 lesions at gastric fundus, the most common type of SMTs were 249 ( 61?48%) GISTs and 144 ( 35?56%) leiomyoma. In 307 lesions located at body,the most common type of SMTs were 143( 46?58%) GISTs and 90( 29?32%) leiomyoma. In 191 lesions located at antrum, the most common type of SMTs were 83( 43?46%) heterotopia pancreas and 45(23?56%) hamartoma, followed by 28(14?66%) lipoma and 20(10?47%) GISTs. In 142 lesions located at cardia, the most common type of SMTs was 110 ( 77?46%) leiomyoma. Third, in 28 duodenum cases, there were 19 lesions at duodenal bulb, the most common type of SMTs was 10 heterotopia pancreas, 4 Brunner gland adenoma and 3 GISTs. In 9 lesions located at descending duodenum,the most common type of SMTs was 4 lipomyoma, followed by 2 ectopic pancreas,1 GISTs and 2 others. Conclusion Leiomyoma is the most common type of esophageal SMTs. In gastric fundus and body, the most common type of SMTs are GISTs and leiomyoma. In antrum, the most common type of SMTs are heterotopia pancreas and hamartoma, but in cardia, that is leiomyoma.In duodenal bulb, the most common type of SMTs are heterotopia pancreas, Brunner gland adenoma and GISTs,and in descending duodenum, is lipomyoma.

Abstract:Introduction: Complete excision of upper gastrointestinal sub-epithelial lesions (U-SELs) eliminates diagnostic uncertainty, obviates the need for surveillance and may be necessary for definitive diagnosis and management. Current guidelines lack precision and cohesion, and surgery is associated with significant morbidity. Herein, we describe and report on the outcomes of our SEL algorithm for endoscopic (SAFE) resection. Methods: U-SELs were enrolled prospectively over 115 months until March 2023. All underwent axial (CT) imaging and endoscopic-ultrasonography (EUS) to exclude a large exophytic component or invasion into local structures, and assess for muscularis propria involvement. Results: 106 U-SELs were resected (mean age 60.6±13.4, 51.9% male; 41 oesophageal, 65 gastric). Oesophageal U-SELs underwent ESD (n=22) or STER if MP involvement was suspected (n=19). Gastric U-SELs underwent STER (n=6 at cardia), ESD (n=47) or exposing-EFTR (n=12). Technical success was 97.6% and 92.3% respectively. Five (7.7%) were completed laparoscopically due to deep and broad full-thickness involvement. Five (7.7%) required laparoscopic gastrotomy and surgical retrieval after successful resection and closure due to a large lesion size (mean 47 mm). There was no delayed bleeding, perforation or recurrence at 13 months. Conclusion: U-SELs may be effectively and safely treated by endoscopic resection. The SAFE approach provides a framework that facilitates structured decision-making. Oesophageal U-SELs suspected to involve the MP should undergo STER. Gastric SELs can be best managed by ESD with a view to proceed to exposing-EFTR. A laparoscopic UGI surgeon should be available in case surgical retrieval of the specimen or laparoscopic completion is required.
