Abstract:Background: Overseeing the quality of community-based, home-care services is a subject of concern in most jurisdictions confronted with population aging and the rise of chronic conditions. Although various quality management strategies have been used in different health care settings, continuous quality improvement (CQI) is still in the early stages of development among home-care service providers. What is more, some authors have raised questions as to whether CQI is suitable to the unique character of home-care and can be adequately applied to a diverse and varied range of agencies, each featuring a unique organizational culture, professional mix, and mode of operation. Purposes: The article reports on how differing organizational cultures--as found in a set of public and private home-care providers--appear to affect agency receptivity to CQI during program implementation. Methodology/approach: The research methodology is characterized by a qualitative, multiple case study approach. Data were gathered from a purposive sample of four home-care agencies in Quebec, Canada, belonging to the public, private for-profit, and not-for-profit sectors. Findings and practice implications: It is concluded that a core set of cultural attributes play a decisive role in determining agency receptivity to CQI, even when its effect is mediated by several contingent variables. Further, some of the levers and barriers to implementation identified in previous research seem less relevant to home-care agencies. A number of policy/management implications are discussed, which may enhance receptivity to CQI by home-care agencies and prevent implementation failure.
Healthcare workers’ perceptions of organisational culture and the impact on the delivery of compassionate quality care

Abstract:This paper explores the qualitative outcomes as part of a larger feasibility study which was developed to test a Cultural Health Check toolkit designed to assist healthcare workers and organisations in the provision of safe, dignified and compassionate quality care for older people. For the qualitative part of this project, 11 healthcare workers agreed to participate in a one-to-one semi-structured interview. The primary aim of the research was to identify the experiences of staff in relation to the different factors they feel contribute to the culture of the organisation. These data were analysed using Attride-Stirling thematic analysis. The data were coded and four global themes were evident: professional practice, support, workforce and service delivery. Professional practice included organisational themes of quality, communication and collaboration and how these impact on a health worker’s ability to carry out their role and the impact on the delivery of compassionate quality care. Support included organisation themes of personal and professional development, and facilitation by leaders and managers to access the available support. Workforce centred on the importance of having adequate levels of all staffing groups to be able to deliver safe, quality person-centred care and services. Service delivery identified the organisational themes of care, person-centred care and communication where staff have the desire, determination and enthusiasm to deliver safe, high quality compassionate person-centred care and services. These themes are key in the impact analysis relating to the delivery of compassionate quality care.
Revisiting organizational culture in healthcare: Heterogeneity as a resource

Abstract:Aligning culture to be similar across work units is a common organizational tactic, but its appropriateness for the multidisciplinary context of healthcare is far from certain. Variation in perceptions of culture across large health systems may serve a functional purpose in delivering high quality care and ameliorating job stress; however, past research in healthcare has focused on culture as the average set of values and norms (i.e., cultural content) rather than on (dis)agreement about values and norms among organizational members (i.e., cultural structure). This survey-based study examines both cultural content (averages among individuals) and structure (distances between individuals) in departments of a large U.S. healthcare organization (total sample = 26,314 workers, response rate = 84%). We used linear models to associate four commonly used culture measures with outcome measures (perceived care quality, intent to stay, and manageable job stress). We found substantial heterogeneity in perceptions for multiple culture types. We found curvilinear relationships between heterogeneity for all culture types and outcomes, suggesting that heterogeneity promotes positive outcomes up to a certain point after which the positive effect declines. For research, our findings point to the importance of studying culture in healthcare with greater focus on heterogeneity; for practice, this study highlights how culturally-focused efforts to improve care quality and worker experience in healthcare should be more precise about balancing cultural alignment and heterogeneity.

Abstract:Purpose The purpose of this paper is to explore the connection between organisational cultures and the employee's resistance to change at five hospital wards in Western Sweden. Staff had experienced extensive change during a research project implementing person‐centred care (PCC) for patients with chronic heart failure. Design/methodology/approach Surveys were sent out to 170 nurses. The survey included two instruments – the Organisational Values Questionnaire (OVQ) and the Resistance to Change Scale (RTC). Findings The results indicate that a culture with a dominating focus on social competence decreases “routine seeking behaviour”, i.e. tendencies to uphold stable routines and a reluctance to give up old habits. The results indicate that a culture of flexibility, cohesion and trust negatively covariate with the overall need for a stable and well‐defined framework. Practical implications An instrument that pinpoints the conditions of a particular healthcare setting can improve the results of a change project. Managers can use instruments such as the ones used in this study to investigate and plan for change processes. Originality/value Earlier studies of organisational culture and its impact on the performance of healthcare organisations have often investigated culture at the highest level of the organisation. In this study, the culture of the production units – i.e. the health workers in different hospital wards – was described. Hospital wards develop their own culture and the cultures of different wards are mirrored in the hospital.

Organizational Culture and Quality Service Delivery of Health Information Managers in Teaching Hospitals, North-West Nigeria.

Abstract:The study examined organizational culture and quality service delivery in teaching hospitals in North-West Nigeria. The provision of adequate treatment, training and research, which are the functions of teaching hospitals, will be difficult to attain without a well-organized, effective and efficient health information management department. The study adopted the survey research design to investigate six teaching hospitals in North-West, Nigeria. The proportionate stratified sampling technique was used to administer copies of questionnaire to 243 health Information Managers (HIMs) and 443 patients, but 194 and 407 were returned respectively for data analysis. Data obtained were analyzed using descriptive statistics (frequency counts, percentage, mean and standard deviation). Findings revealed that the extent of quality service delivery in the teaching hospitals, was high (x̅=3.30), on a scale of 4. All the indicators of quality service delivery showed high values: assurance (x̅=3.32), empathy (x̅=3.31), tangibles (x̅=3.30), responsiveness (x̅=3.29) and reliability (x̅=3.26). The prevalent organizational culture in the teaching hospitals were mission (x̅=3.46), involvement (x̅=3.45), adaptability (x̅=3.41) and consistency (x̅=3.41). Organizational culture had significant influence on quality service delivery of HIMs in the teaching hospitals (R2= 0.043, β = 0.208, t (231) = 3.239, p<0.05). It was, therefore, recommended that management of the teaching hospitals should make continual efforts to sustain key indicators of quality service delivery such as security of patients’ files, protection of request forms, easy retrieval of patients’ records and availability of physical infrastructure. Abstract:Objective. To measure perceptions of organizational culture among employees of public hospitals in China and to determine whether perceptions are associated with hospital performance.Data Sources. Hospital, employee, and patient surveys from 87 Chinese public hospitals conducted during 2009.Study Design. Developed and administered a tool to assess organizational culture in Chinese public hospitals. Used factor analysis to create measures of organizational culture. Analyzed the relationships between employee type and perceptions of culture and between perceptions of culture and hospital performance using multivariate models.Principal Findings. Employees perceived the culture of Chinese public hospitals as stronger in internal rules and regulations, and weaker in empowerment. Hospitals in which employees perceived that the culture emphasized cost control were more profitable and had higher rates of outpatient visits and bed days per physician per day but also had lower levels of patient satisfaction. Hospitals with cultures perceived as customer-focused had longer length of stay but lower patient satisfaction.Conclusions. Managers in Chinese public hospitals should consider whether the culture of their organization will enable them to respond effectively to their changing environment.

Abstract:This article shares the experiences of a new institute for patient safety and quality in Asia in building an organization culture of patient safety. Since its launch in June 2017, the SingHealth Duke-NUS Institute for Patient Safety & Quality in Singapore has been active in integrating cluster-wide efforts in patient safety and quality within the SingHealth Duke-NUS Academic Medical Centre. Institute for Patient Safety & Quality focuses on eliminating preventable errors and harm, and improving patient outcomes by building and cultivating a strong patient safety culture. A culture where healthcare professionals strive for implementation of strong safety management systems requires strong leadership, which prioritizes safety above financial and operational goals; encourages the identification, communication and resolution of safety issues and provides appropriate resources, structure and accountability to maintain effective safety systems. A strong patient safety and quality culture requires a virtuous patient safety and quality ecosystem that takes time to develop. We believe that it is necessary when building this ecosystem to promote the various contributing constituent cultures, including Speak Up Culture, Reporting Culture, Learning Culture, Patient Centric Culture and Just Culture. Leadership commitment is paramount for developing this ecosystem. This entails co-creation of organizational strategies and co-development of a professional education framework to enhance capability and capacity for patient safety and quality.
Promoting quality: the health-care organization from a management perspective

Abstract:Background: Although agreement about the need for quality improvement in health care is almost universal, the means of achieving effective improvement in overall care is not well understood. Avedis Donabedian developed the structure-process-outcome framework in which to think about quality-improvement efforts. Issue: There is now a robust evidence-base in the quality-improvement literature on process and outcomes, but structure has received considerably less attention. The health-care field would benefit from expanding the current interpretation of structure to include broader perspectives on organizational attributes as primary determinants of process change and quality improvement. Solutions: We highlight and discuss the following key elements of organizational attributes from a management perspective: (i) executive management, including senior leadership and board responsibilities (ii) culture, (iii) organizational design, (iv) incentive structures and (v) information management and technology. We discuss the relevant contributions from the business and medical literature for each element, and provide this framework as a roadmap for future research in an effort to develop the optimal definition of 'structure' for transforming quality-improvement initiatives.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>The notion of culture is increasingly invoked in the medical education literature as a key influence on how educational strategies unfold, and culture change is frequently identified as a necessary precursor to progress. A meaningful perspective on what culture means is often missing from these discussions, however. Without a theoretically grounded notion of culture, calls for culture change are challenging to interpret and to act upon.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>In this cross‐cutting edge paper, we explore how culture has been defined and theorised using three lenses: the organisational perspective; the identity perspective, and the practice perspective. We consider what each perspective might offer to medical education researchers.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Each of these perspectives draws on a range of disciplinary influences, and none represents a singular theory of culture. Broadly, the organisational perspective directs our attention to the shared assumptions and values that bind individuals within an organisation. It tends to view culture through a strategic lens; culture may be either a barrier to or a facilitator of the changes that are inevitably required of an organisation if it is to maintain its relevance. The identity perspective, particularly the notion of figured worlds, alerts us to the power of communal narratives to shape how individuals see themselves within particular cultural worlds. The practice perspective emphasises what actually occurs in practice, avoiding symbolic ideas about culture and shared values and instead privileging activity and human‐material networks or arrangements.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>These diverse perspectives share a common thread— they shift our research gaze beyond the individual, allowing us instead to see how those individuals form organisations, inhabit cultural worlds and constitute practices. They afford substance and direction for explorations of culture, and thus offer the promise of a more nuanced understanding of some of medical education's most challenging problems.</p></section>

From assurance to coproduction: a century of improving the quality of health-care service

Abstract:Background: Over the last century, the invitation to improve health-care service quality has taken many different forms: questions, observations, methods, tools and actions have emerged and evolved to create relevant 'improvement work.' In this paper we present three phases of this work. The basic frameworks used in these phases have not supplanted each other, but they have been layered one upon the next over time. Each brought important new thinking, new change opportunities and a new set of limits. The important messages of each need to be carried together into the future, as must the sense of curiosity and possibility about the commonalities that has driven this evolution. Methods: Literature, personal experience and other artifacts were reviewed to develop this description of how the focus on quality work has evolved (and continues to evolve) over the last century. Results: We describe three phases. Quality 1.0 seeks to answer the question 'How might we establish thresholds for good healthcare services?' It described certain 'basic' standards that should be used to certify acceptable performance and capability. This led to the formation of formal processes for review, documentation and external audits and a system for public notice and recognition. Over time, the limits and risks of this approach also became more visible: a 'micro-accounting compliance' sometimes triumphed over what might be of even greater strategic importance in the development and operations of effective systems of disease prevention and management to improve outcomes for patients and families. Quality 2.0 asked 'How might we use enterprise-wide systems for disease management?' It added a focus on the processes and systems of production, reduction of unwanted variation, the intrinsic motivation to take pride in work, outcome measurement and collaborative work practices as ways to improve quality, modeled on experiences in other industries. Quality 3.0 asks 'How might we improve the value of the contribution that healthcare service makes to health?' It requires careful consideration of the meaning of 'service' and 'value', service-creating logic, and prompts us to consider both relationships and activities in the context of the coproduction of health-care services. Conclusion: Efforts to improve the quality and value of health-care services have evolved over the last century. With each success have come new challenges and questions, requiring the addition of new frames and approaches.

Abstract:The International Council on Harmonisation E8 Guidance Revision 1 (ICH E8(R1)) calls for creating a Culture of Quality that "values and rewards critical thinking and open, proactive dialogue about what is critical to quality." Across the biopharma landscape, clinical sites, sponsors, and service providers are working to translate this far-reaching guideline into working practices. This manuscript deconstructs key elements that comprise the critical thinking and open, proactive Culture of Quality "enablers." In addition, maturity models are provided so readers can visualize what a Culture of Quality looks like in their clinical research organization. These provide examples of high performing cultures of quality and useful tools for teams or organizations to measure and evolve their respective quality cultures.

The Role of Quality Improvement Process in Improving the Culture of Information among Health Staff in Ghana

Abstract:Background . Over the past decades, knowledge and understanding have grown regarding the role that health information systems play in improving global health. Even so, using data to make evidence-informed decisions is still weak in most low- and middle-income countries. People do not always act on what they are told to do but act on sharing what is important and valued in an organization. Shared principles related to information systems are alluded to as a pre-existing culture of data collection or “culture of information” without specifying how these values originate and sustain themselves. They work in an organizational environment, which ultimately impacts them through organizational directives, principles, and practices. The objective of the study was to determines the role of quality improvement process in improving culture of information among health staff in Ghana, particularly in the Ejisu Juaben Health Service over time. Methods . A quasi-non-experimental pre- and post-intervention study was conducted in 26 health facilities in the Ejisu Juaben municipal health service of Ghana. The study involved assessment of perceived culture of information of staff coupled with training of 141 core staff selected from 26 facilities who were involved in data collection and use of information through application of data quality improvement training module over a twelve-month period. Results . Overall perceived promotion of culture of information improved from 71 percent in the baseline to 81 percent in the endline. Test-retest analysis suggested that the mean levels of the indices measuring promotion of a perceived culture of information, was significantly higher in endline compared to the situation in baseline. Conclusions . The study concluded that the improvement in staff perceived culture of information improved significantly overtime and this might have been contributed by the application and adoption of quality improvement training.

Abstract:Abstract The concept of quality culture has gained increased attention in health professions education, drawing on insights that quality management processes and positive work-related attitudes of staff in synergy lead to continuous improvement. However, the directions that guide institutions from quality culture theory to educational practice have been missing so far. A prospective qualitative case study of three health professions education programmes was conducted to explore how a quality culture can be enhanced according to the experiences and perspectives of educational leaders. The data collection was structured by an appreciative inquiry approach, supported with vignette-based interviews. A total of 25 participants (a selection of course coordinators, bachelor coordinators and directors of education) reflected on quality culture themes to learn about the best of what is (Discover), envision positive future developments (Dream), identify actions to reach the desired future (Design), and determine how to support and sustain improvement actions (Destiny) within their own educational setting. The results are presented as themes subsumed under these four phases. The experiences and perspectives of educational leaders reveal that peer learning in teams and communities, attention to professional development, and embedding support- and innovation networks, are at the heart of quality culture enhancement. An emphasis on human resources, (inter)relations and contextual awareness of leaders stood out as quality culture catalysts. Educational leaders are therefore encouraged to especially fuel their networking, communication, coalition building, and reflection competencies.

From Reporting to Improving: How Root Cause Analysis in Teams Shape Patient Safety Culture

Abstract:Background: Given the increasing focus on patient safety in healthcare systems worldwide, understanding the impact of Continuous Quality Improvement Programs (QIPs) is crucial. QIPs, including Morbidity and Mortality Conferences (MMCs) and Experience Feedback Committees (EFCs), have been identified as effective strategies for enhancing patient safety culture. These programs engage healthcare professionals in the identification and analysis of adverse events to foster a culture of safety (ie the product of individual and group value, attitudes, and perceptions about quality and safety). This study aimed to determine whether patient safety culture differed regarding care provider participation in MMCs and EFCs activities. Methods: A cross-sectional web-only survey was conducted in 2022 using the Hospital Survey on Patient Safety Culture (HSOPS) among 4780 employees at an 1836-bed, university-affiliated hospital in France. We quantified the mean differences in the 12 HSOPS dimension scores according to MMCs and EFCs participation, using Cohen d effect size. We performed a multivariate analysis of variance to examine differences in dimension scores after adjusting for background characteristics. Results: Of 4780 eligible employees, 1457 (30.5%) participated in the study. Among the respondents, 571 (39.2%) participated in MMCs or EFCs activities. Participants engaged in MMCs or EFCs reported significantly higher scores in six out of twelve HSOPS dimensions, particularly in "Nonpunitive response to error", "Feedback and communication about error", and "Organizational learning" (Overall effect size = 0.14, 95% confidence interval = 0.11 to 0.17, P<0.001). Notably, involvement in both MMCs and EFCs was associated with higher improvements in patient safety culture compared to non-participation or singular involvement in either program. However, certain dimensions such as "Staffing", "Hospital management support", and "Hospital handoffs and transition" showed no significant association with MMCs or EFCs participation, highlighting broader systemic challenges. Conclusion: The study confirms the positive association between participation in MMCs or EFCs and an enhanced culture of patient safety, emphasizing the importance of such programs in fostering an environment conducive to learning, communication, and nonpunitive responses to errors. While MMCs or EFCs are effective in promoting certain aspects of patient safety culture, addressing broader systemic challenges remains crucial for comprehensive improvements in patient safety.
Inside the black box of comparative national healthcare performance in 35 OECD countries: Issues of culture, systems performance and sustainability

Abstract:Background: Is national healthcare performance associated with country-level characteristics, and if so what are the implications for international health policy? Methods and findings: We compared Hofstede's six cultural dimensions against relative health systems performance of 35 countries. Hierarchical cluster analysis identified best-matched groupings of countries. Performance was measured by the Organisation for Economic Co-operation and Development's (OECD's) Health at a Glance indicators data framework (five dimensions with 57 indicators) and the United Nations' (UNs') Sustainability Development Goals (SDG) data set (15 indicators). Three country clusters emerged: Collective-Pyramidal (n = 9: comprising Slovak Republic, Mexico, Poland, Greece, Spain, Turkey, Portugal, Chile, and Slovenia); Collaborative-Networked (n = 12: UK, Canada, Australia, USA, Ireland, New Zealand, Netherlands, Finland, Iceland, Norway, Denmark, and Sweden); and Orderly-Future Orientated (n = 14: Korea, Estonia, Latvia, Austria, Israel, Japan, Czech Republic, Hungary, Italy, Belgium, France, Germany, Luxembourg and Switzerland). The Collaborative-Networked cluster had significantly better performing health systems measured by both the Health at a Glance and SDG performance data, followed by the Orderly-Future Orientated cluster, followed by the Collective-Pyramidal cluster. The Collaborative-Networked Cluster was characterized by low power distance (e.g., greater levels of equity), low uncertainty avoidance (e.g., toleration of others' opinions), individualism (e.g., self-reliance) and indulgence (e.g., drives and norms to enjoy life and have fun). Conclusions: National cultures are associated with healthcare performance on two key international measures. In national and international efforts to improve health system performance, cultural characteristics play an important role. This information may be of value to regulators, policymakers, researchers and clinicians examining the practical impact of culture on healthcare performance.

Abstract:Cultural competence is a phenomenon that straddles many disciplines and fields of study. There is no settled definition of the term, and it is argued that this is not necessary to explore or discuss the phenomenon as it is context-dependent across diverse societies. Explorations of cultural competence in clinical education and training are well-established, but there has been less attention towards its expression in the field of developing healthcare leaders. There is a debate about whether cultural competence is best achieved primarily through training-based educational inputs or by being infused in all areas of curriculum development. Using an exploration of selected literature followed by the case of an ambitious set of leadership development programmes in the English National Health Service, this paper explores the balance and interdependencies of cultural competence in healthcare leadership development as knowledge, skills and attitudes. The paper concludes that it is important for educators in this field to provide space for reflection, develop skills of reflexivity and facilitate sensitive discussions of sometimes contested ideas and concepts. A further evaluation of the impact of teaching and learning interventions, while mapping developments in perceptions of knowledge, skill and attitudes would be an area ripe for future research.

[A safety culture in hospitals]

Abstract:Patient safety is currently a central issue in health care. Many principles of patient safety, such as a safety management system, have been copied from high-risk industries. However, without a fundamental understanding of the differences between health care and industry, most incentives and instruments will translate into bureaucracy, control and repression. The necessary risk reduction for the patient can only be achieved through changes in the culture and hierarchical structure within the health care system. This requires breaking through professional and departmental barriers and reshaping the traditional hierarchy.

Abstract:Despite dealing with biomedical practices, infection prevention and control (IPC) is essentially a behavioural science. Human behaviour is influenced by various factors, including culture. Hofstede's model of cultural dimensions proposes that national cultures vary along consistent dimensions which can be grouped and scored as specific constructs. Studies have reported that three Hofstede constructs--power distance, uncertainty avoidance, and masculinity--show significant association with several key performance indicators relevant to IPC and antibiotic stewardship. In addition, national meticillin-resistant Staphylococcus aureus (MRSA) levels within Europe correlate well with general quality-of-care indices, including preventive strategies and patient rights. This suggests that IPC may be simply a microcosm of overall quality and safety standards within hospitals and countries. Effective improvement would therefore need to address underlying and embedded core cultural values relevant to patient safety and quality of care. Successful IPC strategies are likely to be those that are compatible with the cultural background where they are implemented. To this end, content analysis of many current IPC improvement tools identifies elements of strong compatibility with cultures that are low in uncertainty avoidance and power distance, and high in individualism and masculinity. However, this cultural combination is largely restricted to Anglo-Saxon countries, where most of the recent improvements in healthcare-associated infection (HCAI) incidence have taken place. There is a paucity of research on IPC behaviour change in different cultural backgrounds, especially countries that score high for power distance and/or uncertainty avoidance. This information is vital to inform IPC campaigns in these countries, which often show high HCAI prevalence.
