It is a diverse and beautiful part of the world that includes not only 7 of the world's 10 most populous countries but also some of the world's smallest island nations. The AP region celebrates many cultures and people, and over the last several decades, it has seen industrial and technological developments with major economic growth. Such extraordinary achievements in productivity have accelerated the emergence of environmental challenges and increased the prevalence of diseases that are associated with pollution in industrialized economies. This cost of economic success has increased the need for environmental preservation and sustainable, healthy living environments. Foremost among such aspirations are the universal right to breathe clean air and the collective moral obligation to provide children with the opportunity to grow and develop healthy lungs. The Asian Pacific Society of Respirology (APSR) and the American Thoracic Society (ATS) are two leading professional respiratory societies. The APSR was established in 1986 to encourage research, improve clinical practice through teaching, increase awareness of respiratory health problems and promote the exchange of knowledge among pulmonologists (i.e. respirologists) in the AP region. The 11 countries of the APSR comprise 9% of the world's population and include Australia, Cambodia, Indonesia, Japan, Kyrgyz Republic, Malaysia, New Zealand, the Philippines, Republic of Korea, Sri Lanka, Taiwan and Vietnam. Approximately 15 700 healthcare professionals have membership in the APSR. The ATS was founded in 1905 as the American Sanatorium Association with an initial mission to prevent, control and treat tuberculosis. It was renamed the American Trudeau Society in 1938 and then the American Thoracic Society in 1960; with the renaming, its core mission was broadened to improve health worldwide by advancing research, clinical care and public health in respiratory disease, critical illness and sleep disorders. The ATS has a membership of more than 16 000 physicians, research scientists, nurses and other healthcare professionals. Given the complementary mandates of the APSR and ATS, the societies formed a partnership to focus on the need for clean air, with the fundamental aim of promoting lung health. This pulmonary perspective is intended as an assertive declaration of the public health importance of clean air and the deleterious implications of environmental pollution. It is our hope that it will serve as a catalyst for additional activities in the AP region to ensure that people in all parts of the world, including the AP region, breathe clean, healthy air. METHODS Participants were identified by the leadership of the APSR and ATS on the basis of their expertise in the effects of air pollution and/or the development of professional society statements. An initial search was performed using PubMed to search MEDLINE and using Google Scholar. Synonyms were used to look for articles that address the AP region (i.e. ‘Asia’, ‘Pacific’ and ‘Asia Pacific’), air pollution (i.e. ‘air’, ‘air pollution’, ‘atmosphere’, ‘environment’, ‘outdoor’, ‘outdoor air pollution’, ‘particulate matter’ and ‘PM2.5’) and potential respiratory consequences of air pollution (i.e. ‘respiratory’, ‘asthma’, ‘COPD’, ‘hospitalizations’, ‘influenza’, ‘lower respiratory tract infection’, ‘lung cancer’, ‘lung function’, ‘obstructive lung disease’, ‘spirometry’, ‘tuberculosis’ and ‘wheeze’). An initial literature search yielded 68 000 studies, and a more specific repeat search yielded 1418 studies, including 791 studies related to sources of air pollution, 1249 studies related to the impact of air pollution, 896 studies related to the impact of air pollution on vulnerable populations and 845 studies related to the burden of air pollution. Each author was assigned a section of the manuscript. Authors selected the relevant studies that they wanted to inform their sections of the manuscript. Many authors conducted individual literature searches to identify relevant publications. The bibliographies of selected studies were also searched for additional relevant references. Completed sections were returned to the project chair (K.M.F.), who collated the sections into a single manuscript. All authors reviewed and provided feedback about the draft manuscript. Multiple cycles of review and revision ensued until there was consensus among the authors that the manuscript was ready for submission to the journal. SOURCES OF AIR POLLUTION IN THE AP REGION Air pollution originates from both natural and human-related activities that involve combustion or dissemination of dust1, 2 (Table 1). Natural events that contribute to the pollution burden include volcanoes, forest fires and dust storms.3-11 The leading anthropogenic sources of air pollution are driven by industrialization and urbanization, and they include emissions from vehicles, factories and power plants. In addition, despite continued economic development, solid fuels are still used for cooking and heating in the homes of an estimated 2.5 billion people worldwide (e.g. fossil fuels (charcoal) and biomass (wood, animal dung and crop wastes)) on open fires or traditional stoves.12 These energy-inefficient fuels produce high concentrations of pollutants that can reach concentrations over 100 times higher than international standards considered safe for health in poorly ventilated dwellings. Together, these natural and human-related activities release numerous pollutants into the atmosphere that include metals, salts and organic constituents of particulate matter (PM), gases (e.g. carbon monoxide, nitric oxides, ozone and sulphur dioxide (SO2)) and volatile organic compounds (e.g. polycyclic aromatic hydrocarbons, toluenes, benzenes and quinones). The composition and concentration of these constituent pollutants vary by geographic region, season, climate and local activities.2 Table 1. Its composition can vary considerably by source and region. Particulate matter can be emitted directly from indoor and outdoor sources and can also form in secondary reactions of chemicals such as sulfur dioxide, nitrogen oxides and volatile organic compounds. ‡ Ozone is a secondary pollutant that is formed from secondary reactions of anthropogenic and biogenic precursors (volatile organic compounds and nitrogen oxides) in the presence of sunlight. Pollutant concentrations are substantially higher in the AP region than in much of the developed world (Fig. 1), a consequence of rapid regional urbanization and industrialization together with less stringent air quality and emissions regulations and lagging enforcement. The leading sources of ambient and household air pollution vary on the basis of regional economic development. In more developed urban regions, such as Beijing and Hong Kong, ambient air pollution is due largely to vehicle- and industry-related emissions,13-19 whereas household air pollution results mostly from the infiltration of ambient PM. In less developed rural regions, levels of both ambient and household air pollution are driven mostly by biomass fuel burning.10, 20, 21 Secondhand smoke contributes to household air pollution at all levels of economic development.22, 23 A recent analysis of the Global Burden of Disease data found that coal burning from both industrial activities and home heating is the main source of ambient pollution in China.24 Numerous studies have identified geographically distant mixtures and sources of PM in several cities throughout the AP region.25-28 However, in most cases, it is not possible to parse differential health effects on the basis of individual constituent pollutants. PM is frequently used as an indicator pollutant because combustion-related pollution from different sources (e.g. from wildfires, indoor and outdoor biomass burning and fuel combustion for power and transportation) generally has similar health effects. The Asia-Pacific region has some of the highest annual PM2.5 concentrations in the world.100 THE IMPACT OF AIR POLLUTION IN THE AP REGION Population health burden Air pollution is the leading environmental cause of death and disability globally, and mortality rates are increasing at an alarming pace.29, 30 According to the 2016 Global Burden of Disease report, air pollution is responsible for an estimated 6.1 million deaths annually and 163 million disability-adjusted life-years (DALY) globally.12, 29 Over 90% of this burden occurs in low- and middle-income countries, principally in the countries of the AP region owing to the region's large population and poor air quality. For example, China is home to 416 million people exposed to household air pollution from solid fuel burning, second only to India, and bears 26% of the global burden of ambient pollution-associated mortality.24 The World Health Organization estimates that household air pollution alone causes 946 000 chronic obstructive pulmonary disease (COPD)-related deaths and a loss of 8. 5 million COPD-related DALY in the AP region each year.31 According to the most recent Global Burden of Disease estimates, the highest burden of COPD-related DALYs in the world occurs in India, Nepal, Bangladesh, Bhutan and Papua New Guinea (>2 000 age-adjusted DALYs per 100 000 people), whereas China, Myanmar, Laos, Vietnam and the Philippines also experience extremely high COPD disability in the global context.32 Women and young children are at particularly high risk of household air pollution-related health effects because of sex-based domestic responsibilities.33 Consistent with the aforementioned global estimates, studies conducted within the AP region confirm that both ambient and household air pollution are associated with numerous negative health effects among both adults and children. On the basis of absolute pollution concentrations alone, inhabitants of AP countries may be at much higher risk of air pollution-related morbidity and mortality than people in less polluted regions of the world.34-36 Adult lung health Studies conducted in the AP region have found that, among adults, acute increases in ambient air pollution lead to increases in hospital and doctor visits for respiratory infections37-39 and exacerbations of chronic respiratory conditions such as asthma40-43 and COPD.37, 44-47 Respiratory-related death rates are higher after periods of poor ambient air quality in Asian cities, particularly among the elderly.48-53 For example, dust from the Mongolian desert has been associated with respiratory morbidity and mortality in East Asia51, 54, 55 and vegetation fires in equatorial Asia have been associated with emergency department visits in Australia and increased mortality in Indonesia, Malaysia and Singapore.56, 57 Chronic exposure to air pollution, including ambient and household air pollution, has been associated with higher all-cause mortality,58 increased lower respiratory tract infections,59 lower adult lung function,60-63 accelerated pulmonary function decline64 and increased risk of developing COPD or asthma.63, 65-67 Indeed, evidence of the association between biomass smoke exposure and COPD led to its inclusion in the ATS's position statement on novel risk factors for COPD among non-smokers.68 Lung cancer incidence and mortality are also elevated among those more heavily exposed to ambient and household air pollution, particularly among non-smokers.69-74 Children's lung health Children are among the most vulnerable to the health effects of air pollution exposure. Lung development continues from approximately 4 weeks of gestation until 25 years of age, so both in utero and early childhood air pollution exposures can harm children's lungs. Air pollution exposure during pregnancy is associated with lower average infant birth weight and increased risk for low-birthweight infants.75 Studies conducted in the AP region have found that children who are more heavily exposed to ambient or household air pollution have, on average, more chronic respiratory symptoms (including cough and wheeze), lower lung function and an increased risk of asthma.76, 77 Children with asthma are more likely to experience persistent asthmatic symptoms and seek outpatient care or hospitalization when pollution levels are higher.42, 78-81 Lower respiratory tract infections are the leading cause of death for children under 5 years of age in the AP region, and both ambient and household air pollution exposure have been found to increase the overall risk and mortality of lower respiratory tract infections among the youngest children in Asian countries.82-90 IMPROVING AIR QUALITY IN THE AP REGION The evidence that air pollution harms health is irrefutable, and regional leaders have responded with legislative action. Over the last decade, the Chinese, Japanese and Korean governments have mandated lower fuel emissions from motor vehicles, power plants and factories; promoted cleaner-burning fuels to replace coal; increased support for solar, wind and other clean energies; and tightened annual national air quality regulations. Furthermore, for the first time in Chinese history, China's 5-year plan (2016–2020) now includes a nationwide total energy cap.91 In Hong Kong and China, for example, air quality improvement is being achieved through a multimodality approach that includes a comprehensive motor vehicle emission control programme, reducing marine vessel emissions with fuel regulations, instituting emissions caps for power plants and government subsidies and other financial support for the transition from coal to cleaner fuels and renewable energy. Through these efforts, regional air quality in the Pearl River Delta region has significantly improved: Concentrations of SO2, nitrogen dioxide (NO2) and PM have decreased by 62%, 13% and 15%, respectively, since 2006.92 These governments, with the recent addition of Macao, have jointly created the Guangdong-Hong Kong-Macao Pearl River Delta Regional Air Quality Monitoring Network, which is providing vital data as ongoing air pollution reduction interventions are enacted. RISK REDUCTION IMPROVES RESPIRATORY HEALTH IN THE AP REGION Pollution reduction strategies in the AP region have had important health benefits across a range of pollutant concentrations among adults, children and infants. In preparation for the 2008 Olympics in Beijing, China, the Chinese government enacted several emissions- and travel-related restrictions that resulted in pollutant concentrations decreasing by up to 62% of pre-Olympic levels.93 The reduction in air pollution was associated with reduction in systemic inflammation among healthy young adults, improvement in lung function among healthy adults and adults with asthma, and reduction in asthma-related physician visits by 46%.53, 93-96 The Japanese government passed legislation to limit transportation-related emissions in 2001, with a subsequent decrease in average PM ≤2.5 μm in aerodynamic diameter (PM2.5) levels from 38 to 26 μg/m3 and of NO2 from 30 to 21 ppb as of 2009. This improvement in air quality was linked to 0.6% and 1.1% lower prevalence of pediatric asthma attributable to PM2.5 and NO2, respectively.97 Improved air quality has also been shown to improve birthweight in Chinese newborns.98 Measures to improve household air quality through cleaner-burning fuels and better kitchen ventilation reduced the subsequent yearly decline in lung function in a 9-year prospective study of Chinese adults.64 Together, these findings add to the evidence that pollution reduction strategies confer health benefits across a range of pollutant concentrations, including those at the higher end of the spectrum, with effective implementation demonstrated in high- and low-/middle-income countries within the AP region. The health effects of air pollution do not conform to national or regional borders, and there is no ‘one-size-fits-all’ solution. Mitigating the regional health impacts of air pollution will require attention to the leading sources of pollution within both the region and population of interest, which emphasizes the need to expand ground-level air quality networks, particularly in rural regions, and couple these with publicly available remote-sensing air quality data, including the earth observation data of the National Aeronautics and Space Administration (search.earthdata.nasa.gov). Although the short-term effects of air quality regulations on health described above are encouraging, sustained effort is crucial to ensuring lasting change. For example, the improvements in air quality seen in Beijing during the 2008 Olympics rapidly returned to pre-Olympic levels after regulations were no longer enforced. Furthermore, as the AP region tightens air quality regulations and transitions to cleaner sources of energy, careful attention is needed to ensure that emissions are reduced across all major sources of air pollution (including power, transportation, indoor cooking and heating fuels) to improve air quality and that progress in one sector is not reversed by setbacks in other sectors. COMBATING AIR POLLUTION IN THE AP REGION The global threat from air pollution is clear. In response, in 2015, delegates at the 68th annual World Health Assembly (WHA) meeting approved the first global resolution targeted at combating air pollution, and the roadmap by which to achieve this resolution was approved by the WHA the following year (www.who.int/sustainable-development/news-events/wha69-roadmap-ap/en/). The roadmap for an enhanced global response to the adverse health effects of air pollution outlines courses of action within four areas of influence: Expanding the global knowledge base of the health impacts of air pollution and the effectiveness of intervention strategies Creating networks to monitor progress towards the air pollution-related United Nations Sustainable Development Goals Leveraging coordinated leadership to increase public awareness of air pollution Capacity building within the health sector to address pollution-related health effects To this end, the World Health Organization hosted the first-ever Global Conference on Air Pollution and Health in October 2018, the overarching goal of which was to decrease the number of global air pollution-related deaths by 2030 (www.who.int/airpollution/events/conference/en/). It is our firm belief that clean air is a basic human right and that the ATS and APSR have a responsibility to advocate for clean air to lessen the devastating health consequences of air pollution. Our members can contribute to each of the WHA goals outlined above, such as through (i) our research on the health effects of air pollution in the AP region and around the world, (ii) reporting on air quality trends and sharing of knowledge and technology to monitor air quality in resource-poor settings, (iii) leveraging our expertise and our role as health communicators to demand clean air for our patients’ health and (iv) working within our own healthcare institutions to treat the health consequences of air pollution. Owing to the very high levels of air pollution in the AP region, APSR members in particular must manage and treat the many health consequences of air pollution among their patients. CONCLUSIONS Air pollution is an unintended but harmful consequence of human endeavours aimed at thriving in the modern world, and the health effects of air pollution will continue to worsen without decisive action. The body of evidence highlighted above reinforces the need for global collaboration to improve air quality in the AP region,99 without which the complementary mandates of the ATS and the APSR to ensure respiratory health for all cannot be achieved. In writing the present joint APSR/ATS perspective, we call on our respective society members to act as advocates for clean air as they counsel their patients, hospitals and local and national governments. We also call on our respective professional societies to provide leadership on communicating the health hazards of air pollution, which are growing in many parts of the AP region, and to expand opportunities to share knowledge on health effects of pollution and the effectiveness of interventions to improve air quality. The APSR and the ATS have already started using their combined voices to advocate for clean air. In May 2017, the Forum of International Respiratory Societies (which includes ATS, APSR, the European Respiratory Society, the Pan-African Thoracic Society and others) published and signed the Charter for Lung Health (www.firsnet/org/publications/charter), which is a petition for clean air for the sake of lung health. Through collective action, we can bring about the necessary changes to ensure healthy air for ourselves and future generations.
