'A stroke of luck': a case report of a right-sided cerebellar cerebrovascular accident in a young man

Abstract:A 27-year-old man with a history of migraines, epilepsy and pulmonary stenosis presented to the emergency department with symptoms of vomiting, headache, visual disturbance and problems with balance. The team considered the possibility of intracranial pathology and an urgent CT head with contrast showed what appeared to be a large posterior fossa mass with an appearance suggestive of a primary haemangioblastoma, which was causing considerable mass effect. The patient had neurosurgery to relieve the obstruction and a biopsy of the area showed the mass to be an ischaemic infarct rather than a tumour. Further investigations following the stroke confirmed that the cause was due to having antiphospholipid syndrome and a patent foramen ovale. The patient made a good recovery following the operation and remains well.

Abstract:This paper analyses two key aspects of the life and work of Sir James Young Simpson: his evangelical Christianity and his reaction to criticism following his use of anaesthesia in obstetrics. Simpson's personal religious struggle is placed in the context of the devastating events surrounding the Disruption of the established Church of Scotland in 1843. Whatever his involvement in the events of that year, the development of his faith demonstrates his simple evangelical conviction in the atonement of Christ, very much in keeping with that of many believers in Victorian Britain. There has arisen the notion (still current as is clear from publications in medical journals within recent years) that there was a savage religious response, especially in Presbyterian Scotland, to his use of chloroform - in reality the attack on Simpson's enthusiastic promotion of chloroform was brief, sporadic and of little moment. Simpson's carefully constructed counter to criticism of anaesthesia, drawing on considerable theological and linguistic expertise, reveals a complexity at odds with the simplicity of his faith. The contrast is so great and the reaction so elaborate that it is proposed that Simpson deliberately exaggerated the affair, believing the publicity could only be valuable.

Subcortical arteriosclerotic encephalopathy: Binswanger's disease

Abstract:Binswanger, in his 1894 dissertation on the differential diagnosis of general paresis of the insane, described a slowly progressive dementia associated with macroscopic loss of white matter. In recent years interest in Binswanger's disease was rekindled with CT demonstration of extensive white matter low densities in some patients. To define the clinical spectrum, we reviewed 22 consecutive cases in which the CT appearances suggested a diagnosis of Binswanger's disease. Two patients had focal neurological deficits at presentation, but recent anoxic or hypoglycaemic insults could not be excluded as the cause of the CT abnormalities. The 20 remaining patients were demented and showed variable combinations of corticobulbar dysfunction and gait dyspraxia. The duration of symptoms ranged from a few months to several years. Sixty per cent of this group gave a history of discrete stroke events and focal cortical and/or lacunar infarcts were a frequent CT finding. Binswanger's disease is probably due to chronic or acute-on-chronic white matter ischaemia. The association with lacunar and cortical infarctions suggests that a combination of large and small vessel disease produces diffuse ischaemia maximal in white matter watershed zones. Binswanger's disease is clinically differentiated from multi-infarct dementia by its time course.

Abstract:Abstract‘Churchill’ has been used as a powerful lure in US collective memory on questions of military intervention and defiance. While the history of Winston Churchill is extensive and complicated the image of him in US collective memory has been reduced to a narrower image and understanding of him set around resolution, defiance, individual heroism as an antidote to the discourses and memories of appeasement. US presidents have a proclivity to reach for a memorable phrase or quotation from Churchill when considering questions of defiance and intervention. While the common aphorism suggests that lessons from the past that are unlearned will be repeated, it does not engage the issue of memory. Collective memory, as opposed to history, provides a consciousness that can be used by speechwriters and presidents to galvanise, define and motivate public opinion under particular circumstances.

Abstract:Jonathan Swift is one of the most celebrated satirist writers in literature. His well-known "Gulliver's Travels", apart from being a serious human being criticism, contains many interesting ana unrecognized comments about medical and other scientific facts. Swift made what seems to be the first account of a dementing illness in the elderly; his description fits well with what we now know as Alzheimer's disease. He also described a condition now called synaesthesia, a very interesting phenomenon that may contribute to creativity. In this paper, we review aspects of his life and reveal these amazing descriptions as samples of Swift's extraordinary power of observation.

Abstract:The evolving relationship between Siegfried Sassoon and Wilfred Owen serves as a focus for this history of Craiglockhart Military Hospital and the treatment of shell-shocked officers. The experiences of other soldier patients, such as Max Plowman and George Bonner, are also depicted, together with the methods of their therapists, W.H.R. Rivers, Arthur Brock and William Brown. However, the subject matter is not novel as biographies have been written on both poets and Rivers. Much has been published on the nature of shell shock, its relationship to PTSD and how doctors in World War One sought to return those traumatised by combat to frontline duty. Craiglockhart Military Hospital, though small compared with the Maudsley or Royal Victoria Hospital at Netley, has attracted disproportionate attention, being the subject of two commercial films, Regeneration and Benediction . What then does Charles Glass add to what appears to be a topic exhaustively explored? First, he provides an insightful and carefully researched narrative into the battlefield experiences of Owen and Sassoon, their breakdown, treatment and use of poetry to understand the trauma of conflict. Glass also brings to life the regime at Craiglockhart, and the challenges faced by the doctors who were subject to competing military and therapeutic imperatives. The title of the book is a reference to the evidence given by Robert Graves to Sassoon's medical board called to assess his mental state in the light of an anti-war declaration published in The Times . Because of his public profile and Military Cross, the War Office could have managed his protest by finding him insane and committing him to an asylum. Swayed by Graves' testimony of mental and physical exhaustion, the board preferred a diagnosis of shell shock and sent Sassoon to Craiglockhart. This is not a book about the representative soldier passing through the standard medical system; Glass describes exceptional patients in an experimental institution. Sassoon, a recognised poet with political connections, was given latitude, choosing to remain aloof from much of the hospital's activities apart from regular sessions with Rivers. Owen, then unknown, was fortunate to be referred to Craiglockhart. There he was allocated to Arthur Brock, whose treatment regime – derived from tuberculosis rehabilitation – dovetailed with Owen's vulnerabilities. Glass contrasts the 758 officers (42%) returned to duty from Craiglockhart with the 7.1% from D Block at Netley. Yet this is not a like-for-like comparison as D Block was a secure unit that treated soldiers diagnosed with severe psychoses, whilst Craiglockhart admitted officers suffering from post-traumatic illnesses, some with established coping skills. One point of detail: Rivers and Brock are both described as psychiatrists, though neither had formal qualifications in the speciality, nor had they acquired clinical experience in a mental health unit before the war. Brock was a general practitioner who had worked in a tuberculosis sanitorium and Rivers, a Cambridge academic, had research interests in neurology, experimental psychology and anthropology. Yet they undoubtedly developed expertise in the treatment of post-traumatic illnesses, earning the respect of their officer patients. This book is recommended not only to psychiatrists but also to those with an interest in the complex relationships created by war and the management of trauma.

Acute cerebral infarction. Optimal management in older patients

Abstract:The optimal management of acute cerebral infarction requires consideration of the diagnosis, aetiology, identification of problems, general and specific aspects of care, and prevention of further vascular events. Stroke is a clinical diagnosis but cranial computed tomography (CT) scanning is invaluable to exclude the possibility of cerebral haemorrhage or where the diagnosis is uncertain. Good general care under a specialist multidisciplinary team can reduce mortality and the need for institutional care. Despite promising results from experimental studies, no routine drug therapies have yet shown clinical benefit in acute stroke. Several large trials are currently evaluating anticoagulant, antiplatelet, thrombolytic and neuroprotective agents. Many other proposed therapies have been subject to limited evaluation. Aspirin has a proven role in the prevention of further vascular events after a stroke or transient ischaemic attack. Warfarin, and to a lesser extent aspirin, can prevent recurrent events in patients with nonrheumatic atrial fibrillation. Concerns remain about the safety of warfarin in routine geriatric medical practice. The risk of recurrent stroke in patients with a symptomatic severe carotid artery stenosis is greatly reduced by endarterectomy.
Sir Alexander Morison and The Physiognomy of Mental Diseases: Part 2

Abstract:Sir Alexander Morison's The Physiognomy of Mental Diseases and the original art work that formed the basis of the book have not had the scholarly attention they deserve. The published book and the commissioned portraits have not been studied in any detail. Historians have tended to offer cursory assessments that have reflected their own preconceived ideas rather than properly engaging with the material. This is a pity because Morison's work is a rich source that tells us much about the history of psychiatry. The pictures and text give us a glimpse into the world of the asylum and that of the patient. Although we see the patient through the eyes of the artist and Dr Morison, they do emerge as individuals. The accompanying texts reflect the psychiatric approach of the time and reveal contemporary notions of diagnosis, aetiology and treatment. Morison's work can also be located in the history of ideas about physiognomy. He himself was particularly influenced by Jean-Étienne-Dominique Esquirol, and Morison's work, in turn, influenced WAF Browne. These papers will outline Morison's career and consider in detail his book on The Physiognomy of Mental Diseases.
Unexpected Strikes: Sjögren Syndrome Triggering Multiple Evolving Cerebral Strokes in a Young Patient

Abstract:Neurological complications are observed less frequently with primary Sjögren syndrome (SS). The central nervous system (CNS) has seldom been shown to exhibit symptoms of SS, making the diagnosis of SS with neurological involvement difficult. We present a rare case scenario in which a young 23-year-old male presenting with an acute history of fever, headache, vomiting, altered sensorium, and seizures was admitted and diagnosed as a sub-acute infarct in the right frontal-parietal-temporal lobes on a computed tomography (CT) scan. Upon further examination, laboratory investigations were suggestive of viral encephalitis. The patient was treated accordingly with antiviral drugs, and the patient improved. The patient took "discharge against medical advice" after 12 days, only to return to the hospital with similar complaints within 15 days. Magnetic resonance imaging (MRI) was done, which suggested an acute evolving infarct in the right frontal and parietal lobe, and further evaluation yielded a diagnosis of SS. The patient was treated with high-dose steroids for seven days. A repeat MRI showed new acute infarcts with dilatation of the ventricular system with periventricular ooze. The patient could not be revived and succumbed after one week of steroid therapy.
[Tributes to Macdonald Critchley and his achievements in neurolinguistics]

Abstract:Macdonald Critchley, a world renowned neurologist, is best remembered as the person who challenged to solve the mystery and enigma of higher cerebral functions. He was the author of over 200 published articles and 20 books on neurology including his pioneer work on developmental dyslexia, the parietal lobe, cerebral hemisphere dominance and aphasiology. He also published articles on migraine, epilepsy, dementia, visual perseveration, anterior cerebral artery syndromes, indifference to pain, sleep disorders, movement disorders, and myotonia. He was very erudite and talented in linguistics; his interests were broad and eclectic. He wrote extensively on non-medical topics, such as plays, horror stories, mythology, and biographies of distinguished neuroscientists, philosophers or artists. His articles were written in the elegant prose style with thorough reference citations and extensive reviews of the literature worldwide. He was born in Bristol in 1900. From an early age, he was fascinated with the functioning brain and retained a life-long interest in languages. He was appointed to the Consultant staff of the National Hospital, Queen Square, and to King's College Hospital at the age of 27, and was elected to become FRCP at 30. He was Dean of the Institute of Neurology from 1948 to 1953. When I was studying advanced neurology at Queen Square in 1960-1961, I was fascinated by his outstanding lectures on Huntington's Disease and Non-verbal Communication. Although he did not read the manuscripts and used only a few slides for illustrations in his lectures, he could describe the details of historical accounts and relevant figures. He was an altruistic, instructive teacher with keen attention to responses from the audience. His clinical demonstrations, teaching rounds and tutorials were hugely popular among postgraduate students from all over the world. He emphasized the importance of shrewd observation and careful history taking in neurological practice. Macdonald Critchley was President of the World Federation of Neurology (1965-1973) and Editor of Journal of the Neurological Sciences. I met him at the 2nd Asian and Oceanian Congress of Neurology held in Melbourne in 1967. He was interested to know more about aphasia and dyslexia among people speaking and reading Chinese. He kindly invited me to speak on 'Linguistic Implications of Chinese Characters' at the meeting of the Research Group on Developmental Dyslexia in Dallas. Critchley's prominent achievements in teaching, writings and research on neurology derived from his creative thinking, powers of shrewd observation, wide knowledge of literature, and indefatigable assiduity in scientific pursuits. He died peacefully at home at the age of 97 and entered the pantheon of distinguished neuroscientists of the 20th century.

Abstract:Background: Survivors of stroke are often concerned about cognitive problems, and information on the risk of cognitive problems often comes from small studies. We aimed to estimate years of cognitive ageing associated with stroke compared with transient ischaemic attack, myocardial infarction, and other hospitalisations in a large population. Methods: Using data from six randomised controlled trials (ORIGIN, ONTARGET, TRANSCEND, COMPASS, HOPE-3, and NAVIGATE ESUS), we completed an individual participant data meta-analysis using data requested from the Public Health Research Institute to estimate the association of stroke (by type and severity), transient ischaemic attack, myocardial infarction, and other hospitalisations with cognitive performance measured at the end of each trial. We included participants in any of these randomised controlled trials with a cognitive assessment at baseline and at least one other timepoint. Cognitive performance was measured with the Mini-Mental State Examination or the Montreal Cognitive Assessment, transformed into Z scores. We estimated Z score differences in end of trial cognitive performance between people with and without events and calculated corresponding years of cognitive ageing in these trials, and additionally calculated using a population representative cohort-the Cognitive Function and Ageing Study. Findings: In 64 106 participants from 55 countries, compared with no event, stroke was associated with 18 years of cognitive ageing (1487 strokes included in the model, 95% CI 10 to 28; p<0·0001) and transient ischaemic attack with 3 years (660 transient ischaemic attacks included in the model, 0 to 6; p=0·021). Myocardial infarction (p=0·60) and other hospitalisations (p=0·26) were not associated with cognitive ageing. The mean difference in SD compared with people without an event was -0·84 (95% CI -0·91 to -0·76; p<0·0001) for disabling stroke, and -0·12 (-0·19 to -0·05; p=0·0012) for non-disabling stroke. Haemorrhagic stroke was associated with worse cognition (-0·75, -0·95 to -0·55; p<0·0001) than ischaemic stroke (-0·42, -0·48 to -0·36; p <0·0001). Interpretation: Stroke has a substantial effect on cognition. The effects of transient ischaemic attack were small, whereas myocardial infarction and hospitalisation had a neutral effect. Prevention of stroke could lead to a reduction in cognitive ageing in those at greatest risk. Funding: Population Health Research Institute and Chief Scientist Office of Scotland.

Abstract:A growing awareness about brain health and Alzheimer's disease in the general population is leading to an increasing number of cognitively unimpaired individuals, who are concerned that they have reduced cognitive function, to approach the medical system for help. The term subjective cognitive decline (SCD) was conceived in 2014 to describe this condition. Epidemiological data provide evidence that the risk for mild cognitive impairment and dementia is increased in individuals with SCD. However, the majority of individuals with SCD will not show progressive cognitive decline. An individually tailored diagnostic process might be reasonable to identify or exclude underlying medical conditions in an individual with SCD who actively seeks medical help. An increasing number of studies are investigating the link between SCD and the very early stages of Alzheimer's disease and other neurodegenerative diseases.

Abstract:Biographical accounts of the British writer W. Somerset Maugham have often focused on his sexuality. These accounts may be complemented by a disability studies perspective, in which Maugham's stammer is not seen as simply an impediment to be overcome, but an enabling force in the narrative framing and timing that his writing displays. Maugham's mild cognitive impairment and then dementia in the last few years of his life have proved a formidable obstacle to biographers who have taken his self-made public persona at face value. Drawing on preliminary biographical work on Maugham, this essay suggests that the writer's dementia, and the manner in which it was narrativized by others, may tell one less about Maugham himself than the relationships readers construct between literary texts and authors’ lives. Such a realization may also prompt reflection on the auto|biographical elements in academic writing which scholars in literary studies often occlude.

Abstract:Introduction: Associations between stroke subtype and post-stroke cognitive decline are unclear. We determined if post-stroke cognitive trajectories differed between 1) hemorrhagic vs. ischemic stroke and 2) ischemic stroke subtypes. Methods: We identified 1,150 eligible dementia-free individuals with incident stroke (93% ischemic, 7% hemorrhagic, 50% women, 31% Black, median age, 65) from 4 cohort studies from 1971-2019: Atherosclerosis Risk in Communities Study, Cardiovascular Health Study, Framingham Offspring Study, and Reasons for Geographic and Racial Differences in Stroke. Of the ischemic strokes, 22% were small-vessel occlusion, 8% large artery atherosclerosis, 21% cardioembolic, and 49% cryptogenic/other determined etiology. Linear mixed-effects models were used to estimate longitudinal changes in cognition after incident stroke. Global cognition (primary outcome), executive function, and memory were harmonized across studies and standardized as T-scores (mean [SD], 50 [10]); a 1-point difference represents a 0.1-SD difference in cognition. Median (IQR) follow-up was 5.9 (3.2, 9.2) years. Results: Survivors of ischemic stroke had significant declines in global cognition, executive function, and memory (Table). For example, global cognition declined -0.35 points/year (95%CI, -0.43, -0.26); P <0.001. Initial post-stroke cognitive scores and post-stroke cognitive trajectories did not differ between hemorrhagic and ischemic stroke, except for lower initial post-stroke memory scores (adjusted difference, -1.39 points; 95% CI, -2.77, -0.01; P =0.049). Survivors of small-vessel occlusions had significant declines in global cognition, executive function, and memory (Table). Initial post-stroke cognitive scores and post-stroke cognitive trajectories did not differ by ischemic stroke subtype. Conclusions: Our results suggest that stroke survivors experience long-term post-stroke cognitive decline regardless of subtype.
