Abstract:A 24-year-old female patient presented to the neurological department after a seizure that lasted for 10 minutes. Magnetic resonance imaging revealed a cystic and heterogeneously enhanced giant mass in the right frontal lobe mimicking parasagittal meningioma. Surgery via a single frontal craniotomy confirmed the tumor was attached to the falx cerebri and sagittal sinus. The histological diagnosis was schwannoma. Since total resection of the tumor, the patient was seizure free. Twelve months of follow-up revealed good outcome.
Multiple Warty Dyskeratoma: A Case Report And Review Of The Literature

Abstract:Warty dyskeratoma is an uncommon entity usually characterized by a single solitary papule or nodule on the head or neck with a comedo-like plug. Multiple warty dyskeratoma is an extremely rare variant. To our knowledge, there have been only 8 cases reported in English. We report a 62-year-old Chinese male presented with a 2-year history of multiple papules on the scalp and forehead. Physical examination revealed numerous firm flesh-colored or grayish hyperkeratotic papules and nodules with an umbilicated center located on the right parietal, forehead, and right temporal region. Histopathology showed cup-shaped invagination of the epidermis with acanthosis, suprabasal clefting, zones of acantholysis, and dyskeratosis with corps ronds and corps grains extending into the papillary dermis. Multiple warty dyskeratoma should be differentiated with verruca, seborrheic keratosis, sebaceous hyperplasia, and actinic keratosis. The histopathologic differential diagnoses of warty dyskeratoma include Darier's disease, Grover's disease, Hailey-Hailey disease, acantholytic acanthoma, familial dyskeratotic comedones, acantholytic actinic keratosis, and acantholytic squamous-cell carcinoma.
Giant malignant peripheral nerve sheath tumors of the occipital scalp

Abstract:Background: Malignant peripheral nerve sheath tumors (MPNST) are rare neoplasms, usually arising from peripheral nerves or showing a nerve sheath differentiation. Primary MPNSTs of the scalp is exceptionally rare, and only sporadic cases have been reported recently. Objectives: Report a rare case of giant malignant peripheral nerve sheath tumor (MPNST) beneath occipital scalp, and discuss how to-treat with this kind of tumor. Methods: Descriptive study of a rare case of giant peripheral nerve sheath tumors of occipital scalp without adjuvant treatment with nine months follow up. Results: In a 52-year-old man with MPNSTs beneath occipital scalp, the tumor was treated with complete surgical resection. Histological examination proved that the lesion was a scalp MPNST. The patient was followed up asymptomatic for the following nine months after surgical resection without adjuvant radiotherapy. Conclusion: MPNSTs beneath the occipital scalp should be treated individually, for those well-circumscribed MPNSTs without bone destruction or brain invasion (low-grade tumors), complete surgical resection with clear margins (if possible) is recommended. Otherwise, adjuvant postoperative radiotherapy is necessary.

Abstract:We describe 4 cases of pseudocyst of the scalp, which still is only being described in the Japanese literature. The tumor is characterized as follows: localized between the top and the forehead area of the scalp, it first appears as a solitary reddish, painful small nodule or papule; it then gradually increases in size, protruding into a dome-shaped mass, and becomes associated with alopecia limited to the lesion area. This report describes typical cases of pseudocyst of the scalp, reviews the reported Japanese cases and introduces this tumor to western dermatology.

Abstract:Schwannoma of the larynx is a rare entity, accounting for less than 0.1% of all benign laryngeal neoplasms. In particular, schwannoma of the epiglottis is much more rare. We report a case of schwannoma located on the laryngeal surface of the epiglottis which mimicked epiglottic cyst. A 39-year-old female presented with a 5-month history of dry cough. Laryngostroboscopy examination revealed a smooth, predunculated, measuring approximately 1.0 x 1.0 cm mass located on the laryngeal surface of the epiglottis. The patient had suspension laryngoscopy under general anesthesia, with the mass removed by electrotome excision. Based on the histological examination, a final diagnosis of laryngeal schwannoma was made. The patient was free of postoperative complications and had no sign of recurrence. Her dry cough had disappeared immediately after surgery.

Abstract:Benign epithelial skin cysts containing multiple components of the folliculo-sebaceous apocrine unit are only rarely reported in the literature. Here, we describe a 12-year-old girl who presented with a cystic mass on the vertex of her scalp. Upon resection, it showed a hybrid benign skin cyst with interesting histological features of both pilar and apocrine differentiation. The clinicopathological and imaging findings of this unusual skin cyst, successfully managed by a plastic surgeon and neurosurgeon, are described. Pathologists and clinicians should be aware of this type of skin cyst rarely encountered in their clinical practice.

Abstract:Proliferating pilar tumors are rare, benign, exophytic neoplasms, which can closely resemble a squamous cell carcinoma. We describe a patient with a large benign exophytic tumor on the scalp that had been slowly growing over 10 years. While this class of benign follicular tumors is rare, the standard of care is typically excision with clear histologic margins. In this case, this large scalp tumor was surgically excised with clear margins/permanent section margin control using "Slow Mohs" technique, with subsequent repair using a skin substitute dressing, followed by a delayed skin graft.

Abstract:Background: Schwannomas are well-encapsulated, solitary tumors that grow slowly from the nerve sheath. Sellar schwannomas tend to be mistaken for other sellar/parasellar lesions due to similar clinical and radiological findings. The present article describes the clinical presentation, radiological findings, histopathological features, and outcome of a patient with sellar schwannoma. Case Description: A 23-year-old female, known to have hypothyroidism secondary to Hashimoto's thyroiditis, presented with multiple episodes of galactorrhea, weight gain, and irregular menstrual cycle for 8 months. It was associated with decreased visual acuity and episodic headaches. Neurological examination revealed no focal deficits. Brain magnetic resonance imaging (MRI) showed a well-defined lobulated lesion in the sellar region, compressing the right optic nerve and optic chiasm. The patient underwent craniotomy and tumor resection. The histopathological sections were diagnostic of schwannoma. Postoperatively, the patient noted a subjective improvement in her visual acuity. She was discharged in stable condition with regular follow-ups at neurosurgery, endocrine, and ophthalmology clinics. Conclusion: Schwannoma of the sellar region is rare and can be misdiagnosed as pituitary adenomas. Preoperative hormonal profile and meticulous neuroradiological assessment narrow down the differential diagnosis for patients with sellar lesions. The diagnosis of sellar schwannomas is established with histopathology and immunohistochemistry results.

Abstract:Synovial sarcoma (SS) arising primarily in the scalp was scarcely reported. Here we described a subcutaneous mass in the right parietal region of the scalp of a 39year-old male patient. It has been present for 10years and enlarged recently. It was initially suspected as hemangioma by magnetic resonance imaging (MRI), but the histological examination revealed a sarcoma rich of spindle cells reminiscent of SS. Therefore, immunohistochemistry, florescence in site hybridization (FISH), reverse transcription-polymerase chain reaction (RT-PCR) and DNA sequencing were performed to determine the final diagnosis as biphasic SS. The detailed description of primary scalp SS might help differential diagnosis of scalp soft tumors.

Abstract:Pilar cysts/trichilemmal cysts are benign lesions that arise from the hair follicle epithelium. The commonest area of occurrence is the scalp but it can also occur in the head and neck region. The pilar cyst grows at a prolonged rate. They are relatively rare and occur in about 10% of the population. They appear in the region of high concentrations of hair follicles. A 75-year-old male patient came to the Department of Oral and Maxillofacial Surgery with a chief complaint of swelling on the left side of the face for the past one year. Cytological examination revealed an infected cystic lesion. Computed tomography (CT) showed a well-defined lesion in the left temporal region. After surgical excision of the lesion, it was sent for histopathological examination. Excisional biopsy revealed a pilar cyst. We report a rare case of pilar cyst in the left temporal region in a patient who was previously operated on for osteomyelitis of the left side of the mandible up to the coronoid process. These cysts may mimic temporal space infection and lead to an incorrect treatment plan.

Abstract:Chondroid syringomas are similar to mixed parotid tumors and present in both benign and malignant forms. Malignant chondroid syringoma (MCS) is a rare skin tumor that has a predilection for extremities, particularly in young women. It is even rarer to present as a scalp tumor with very few reported cases in the literature. We present a middle-aged woman, with a history of increasing fatigability of her right arm for the past 3 months who, on examination, was found to have scalp swelling and matted right posterior triangle lymph nodes. The working diagnosis on her was a large sebaceous cyst with secondary in the neck from an occult primary/non-Hodgkin’s lymphoma. Preliminary fine-needle aspiration was inconclusive. Imaging followed by wide excision of the tumor and the nearby occipital node was done. The final histopathological diagnosis was MCS with secondary in the lymph node. These tumors are aggressive and metastasize early. Radical surgery is the only hope of cure as adjuvant treatment is yet to be standardized.

Abstract:Spitz nevus (SN) is a benign melanocytic lesion with cytologic and architectural atypia. It is sometimes difficult to distinguish SNs from atypical Spitz tumor (AST), Spitz melanoma, or conventional melanoma. SNs frequently develop in Caucasians and appear on the skin of the head and lower extremities. Lesions on the ear in Asian populations are rare. Here, we report a “red Spitz tumor” on the ear of a Chinese 18year-old boy. Dermoscopic examination revealed possibly malignant features presented as polymorphous vessels along with central white area, pseudo-network depigmentation and atypical peripheral globular pattern. The results of histopathological examination strongly suggested that the neoplasm was a compound SN and no recurrences or metastases occurred during 1-year follow-up post-surgery. Further, we review the literature on 4 previously reported cases of SN on the ear and summarize themain points of SN diagnosis and differential diagnosis with atypical Spitz tumors and melanoma.
Pancreatic schwannoma mimicking pancreatic cystadenoma: A case report and literature review of the imaging features

Abstract:Introduction: Schwannomas, also known as neurilemmoma, are benign neoplasms that originating from Schwann cells in peripheral nerve sheaths. The head, neck, and extremities are the most common sites; however, pancreatic schwannomas are rare neoplasms. Accurate preoperative diagnosis of these tumors is very tough because of pancreatic schwannomas usually mimicking other cystic tumors. Here we present a case of pancreatic schwannoma misdiagnosed as pancreatic cystadenoma. Patient concerns: We presented a rare case of a 55-year-old female admitted to our hospital for abdominal distension. The physical examination and results of laboratory testing reveal no abnormalities. Diagnosis: A computed tomography (CT) scan detected a hypodense 2.4 cm × 2.6 cm mass with a clear margin at the neck of the pancreas. Pancreatic cystadenoma was strongly suspected. Interventions: The patient underwent robotic distal pancreatectomy with splenectomy. The gross specimen showed a pale and solid mass with a capsule. Outcomes: Histological examination of the surgical specimen demonstrated a pancreatic schwannoma. Immunohistochemistry results were as follows: S-100 (+), CD117 (-), SMA (-), and Desmin (-). She was discharged on postoperative day 6 and no recurrence of the tumor happened during the 12-month follow-up. Conclusion: Precise preoperative diagnosis of pancreatic schwannomas is very difficult despite the application of multiple imaging modalities. Surgery is the most effective treatment for this rare disease and the final diagnosis usually relies on pathology. Following complete tumor removal, patients with pancreatic schwannomas generally have a good prognosis.

Abstract:A 39-year-old female had been subject to headache, and intermittent seizures for 9 years and decreasing memory for one year, without obvious neurological signs. An MRI revealed a 2x2 cm contrast-enhanced lesion in the frontal lobe, with a cyst and peritumoral edema, which was not attached to the dura or falx. Preoperatively, it was diagnosed as a glioma. Total surgical removal of the lesion led to a favorable result. Post-operative histo-pathological examination showed characteristic Antoni A and B areas consistent with intraparenchymal schwannoma. Intraparenchymal schwannoma is an extremely uncommon lesion, which is seen mostly in young adults and children. The main clinical symptoms include rising-intracranial-pressure-related manifestations and associated seizure disorders. The possible developmental origins, histological, imaging features, and protocols of treatment for this entity are discussed.

Abstract:Schwannomas (SCHs) are benign neural tumors originating from Schwann cells of the peripheral nerve sheaths. These neoplasms typically exhibit hyalinized vessels with impaired vascular permeability; however, angioma-like features are rare. We report an intriguing case of a cutaneous SCH with unusual vascular changes in a 60-year-old female who presented with a tender nodular lesion on her lower back. Histopathological examination of the excised lesion revealed a schwannoma with a central area of thrombosis and a vascular proliferative lesion consistent with Masson's hemangioma (MH). MH, also known as intravascular papillary endothelial hyperplasia (IPEH), is a rare benign vascular lesion characterized by papillary endothelial hyperplasia and obliterative changes within vascular lumens. Immunohistochemical staining confirmed S100 positivity in the SCH component and highlighted the papillary endothelial lining by ERG (erythroblast transformation-specific regulated gene 1). To our knowledge, this is the first report of a schwannoma harboring MH. This unique case underscores the potential for rare vascular proliferation to arise within otherwise typical SCHs.

Abstract:Psammomatous melanotic schwannoma (PMS) represents a rare type of schwannoma; its occurrence in the spinal canal is even rarer. In the present study, we report a case of pathologically confirmed PMS with cystic changes from old hemorrhages in the cervical spinal canal. The patient underwent surgical resection of the tumor with postoperative radiotherapy, and had a smooth recovery. Follow-up X-rays and MRI at 16 months after the operation revealed no evidence of tumor recurrence.

Abstract:Pleomorphic dermal sarcoma (PDS) is an undifferentiated high-grade tumour and is considered the most common primary soft tissue tumour in adults. The most common locations of PDS are the trunk, extremities and retroperitoneal regions. Skin involvement of PDS is rare, and involvement of the scalp is less common. Most cases of PDS appear as a gradually growing lesion for 1 to 2 years and are associated with ulceration and bleeding. The definitive treatment for PDS is usually surgical resection. We present an unusual case of a primary PDS involving the scalp of a 78-year-old male patient and discuss its unusual clinical presentation, dermoscopic features, histopathological correlation and its management.

Aggressive Pilomatrixoma of the Scalp in a Geriatric Patient: A Rare Occurrence

Abstract:Pilomatrixoma is a benign cutaneous adnexal neoplasm that arises from the hair matrix. This lesion is typically observed in the first two decades of life, with most cases occurring before the age of 20. However, it is rare in the elderly. While aggressive and malignant variants of pilomatrixoma exist, they are rarely reported compared to their benign counterparts. In this case, we present an instance of aggressive pilomatrixoma in a 90-year-old female. The patient exhibited a large ulceroproliferative growth on the scalp. A preliminary biopsy was performed, followed by wide local excision. Both the biopsy and excision specimens exhibited characteristics of pilomatrixoma, with a focal infiltrative growth pattern and moderate nuclear atypia. Features indicative of malignancy, such as geographic necrosis, brisk mitosis, atypical mitosis, vascular invasion, and perineural invasion, were not observed. Therefore, the diagnosis of aggressive pilomatrixoma was made. Wide local excision and subsequent skin grafting were conducted, with no involvement of deeper subcutaneous tissue and intact pericranium. The patient is currently faring well during follow-up. This case underscores the importance of meticulous histopathological examination for distinguishing aggressive variants, as they carry a higher risk of recurrence and aid in distinguishing aggressive pilomatrixoma from carcinoma. Failure to recognise the aggressive variant may result in inadequate management.

Abstract:Hypopharyngeal schwannoma is quite rare, with only nine cases presented in the literature to our knowledge. The presentation itself requires a distinction from malignant hypopharyngeal disease, which is usually the initial diagnosis. A transcervical approach and laryngomicrosurgery are two possible operating methods for the treatment of hypopharyngeal schwannoma. The selection of the surgical method depends on the size of the tumor. For this case, we preferred the transcervical approach without tracheotomy. In contrast to previous case reports, we were unable to trace the original nerve.
