Abstract:Background: Malignant peripheral nerve sheath tumors (MPNST) are rare neoplasms, usually arising from peripheral nerves or showing a nerve sheath differentiation. Primary MPNSTs of the scalp is exceptionally rare, and only sporadic cases have been reported recently. Objectives: Report a rare case of giant malignant peripheral nerve sheath tumor (MPNST) beneath occipital scalp, and discuss how to-treat with this kind of tumor. Methods: Descriptive study of a rare case of giant peripheral nerve sheath tumors of occipital scalp without adjuvant treatment with nine months follow up. Results: In a 52-year-old man with MPNSTs beneath occipital scalp, the tumor was treated with complete surgical resection. Histological examination proved that the lesion was a scalp MPNST. The patient was followed up asymptomatic for the following nine months after surgical resection without adjuvant radiotherapy. Conclusion: MPNSTs beneath the occipital scalp should be treated individually, for those well-circumscribed MPNSTs without bone destruction or brain invasion (low-grade tumors), complete surgical resection with clear margins (if possible) is recommended. Otherwise, adjuvant postoperative radiotherapy is necessary.
Malignant Proliferative Ependymoma of the Neck with Lymph Node Metastasis: A Case Report.

Abstract:BACKGROUND:Malignant proliferating trichilemmal tumor (MPTT) is an infrequent malignant neoplasm originating from cutaneous appendages, with only a handful of documented cases. This report delineates a unique instance of MPTT situated in the neck, accompanied by lymph node metastasis. A comprehensive exposition of its clinical trajectory and imaging manifestation is presented, aiming to enhance comprehension and management of this atypical ailment.CASE SUMMARY:Patient concerns: A 79-year-old male presented with a longstanding right neck mass persisting for over six decades, exhibiting recent enlargement over the past year. Diagnoses: Enhanced magnetic resonance imaging of the neck unveiled an elliptical mass on the right neck side, characterized by an ill-defined border and a heterogeneous signal pattern. The mass exhibited subdued signal intensity on T1-weighted imaging (T1WI) and a heterogeneous high signal on T2-weighted imaging (T2WI), interspersed with a lengthy T1 and T2 cystic signal motif. Close anatomical association with the submandibular gland joint was noted, and intravenous gadolinium diethylene triamine pentaacetic acid administration facilitated conspicuous enhancement. Substantial enhancement of the solid segment prompted an initial preoperative diagnosis of malignant nerve sheath tumor. However, post-surgery histopathological and immunohistochemical analysis conclusively confirmed the diagnosis as malignant hyperplastic external hair root sheath tumor. Intervention: Complete excision of the tumor was successfully executed. Outcomes: The patient experienced a favorable postoperative recovery.CONCLUSION:Malignant proliferative trichilemmal tumor external hair root sheath tumor is a cystic-solid lesion, appearing as low signal on T1WI images or high signal on T2WI with enhancement of the solid component. Suspicions of malignancy are heightened when the tumor border is indistinct, tissue planes are breached, or when linear or patchy high signals are observed in the subcutaneous tissue on T1 liver acquisition with volume acceleration enhanced images along with intermediate signal on T2WI and restricted diffusion on diffusion-weighted imaging images. Strong consideration for malignancy should arise if there are signs of compromised adjacent tissue relationships or direct invasion evident on imaging. We have incorporated the above-mentioned content into the entire manuscript.

Abstract:A 69 year-old female patient presented with a reddish-brown, enlarging lesion on her right lower eyelid. Physical exam revealed a 4-5mm, oval-shaped, reddish-brown, shiny, exophytic papule on the right lower eyelid. Shave biopsy of the lower eyelid papule was performed, which revealed nests of cells with prominent cribriform arrangements, focally surrounded by small pools of blue stringy mucinous material and papillary architecture. Cells stained positive for EMA, synaptophysin, and chromogranin, which confirmed the diagnosis of endocrine mucin-producing sweat gland carcinoma (EMPSGC). The lesion was fully removed with negative margins via Mohs micrographic surgery. Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is a rare, low-grade, cutaneous adenocarcinoma that typically arises on the eyelids of elderly women. EMPSGC may be associated with invasive mucinous carcinoma.

Abstract:A proliferating trichilemmal tumor (PTT) is a rare, benign, exophytic tumor originating from the isthmus region of the outer root sheath of the hair follicle. Clinically, PTTs manifest as isolated, exophytic, firm nodules that have the potential to ulcerate. These tumors may occasionally originate from a pre-existing trichilemmal cyst, or they can emerge spontaneously. Most exclusively these lesions are seen on the scalp. However, rarely these tumors can be found in other anatomical areas. Our patient had a protruding mass in her shoulder for 20 years, and this is a rare site for the occurrence of these lesions; it could be the first case to document such a site, as far as we found in the literature. The mainstay treatment of the PTT is surgical excision of the tumor, assessing the histological margins to ensure sufficient resection was made, close monitoring, and follow-up with the patient.

Abstract:Malignant proliferating pilar tumors (MPPTs) are rare, unique cutaneous adnexal tumors. Sarcomatous transformation in MPPTs is even rarer (4 previous cases reported). Here, we report an extraordinary case of a MPPT with sarcomatous transformation occurring on the scalp of a 63-year-old man with an in-depth molecular profile along with histologic, immunohistochemical, and follow-up data. Shared mutations in the epithelial and sarcomatous components included a loss-of-function TP53 mutation. An inactivating TP53 mutation was only identified in the epithelial component, and an inactivating CDKN2A mutation was only identified in the sarcomatous component. Copy number variations previously reported in MPPT were also identified, including 6p21.1 loss, 6q arm loss, and 15q21.1-q26.3 gain [epithelial], and 6p22.2-p22.3 loss [sarcoma]. Histologically, the tumor demonstrated juxtaposed areas of proliferating pilar tumor, carcinoma with clear cell change, and sarcomatous areas that did not stain for AE1/AE3, p40, CD34, S100 protein, and smooth muscle actin by immunohistochemistry. The patient is alive at 2 years without evidence of recurrence or metastasis.
Exophytic Squamous Cell Carcinoma of the Scalp: A Case Report

Abstract:Squamous cell carcinoma (SCC) is the second-most common clinical presentation of non-melanoma skin cancer. Despite its prevalence, the rate of growth and development of SCC lesions is low. We present a case report of an exophytic SCC of the midline scalp. Over approximately 18 months, the exophytic portion of this lesion grew to a size of 8.5 x 7 cm due to the fact that the patient did not seek medical attention. The patient suffered from many predisposing factors including active smoking status, type II diabetes, and significant previous sun exposure. In addition to these predisposing factors, the patient did not have comprehensive health insurance to cover outpatient medical care. This case highlights the importance of early intervention in the management of head and neck skin cancers and the negative impact of delayed treatment.

Abstract:Pleomorphic dermal sarcoma (PDS) is a rare mesenchymal tissue tumor. Distinguishing PDS from similar conditions, such as atypical fibroxanthoma (AFX), its less aggressive tumor counterpart, is difficult, as they are clinically and histologically similar. We present a case of a 77-year-old man presenting with a large nodular scalp lesion of three weeks duration. Pathology revealed a 3.3 cm invasive pleomorphic dermal sarcoma. Surgical excision with 2 cm margins was performed with successful healing of the graft. This case highlights a rare case of a large pleomorphic dermal sarcoma and discusses the histological features and management of PDS.

Abstract:Pilomatrixoma is a benign cutaneous adnexal neoplasm that arises from the hair matrix. This lesion is typically observed in the first two decades of life, with most cases occurring before the age of 20. However, it is rare in the elderly. While aggressive and malignant variants of pilomatrixoma exist, they are rarely reported compared to their benign counterparts. In this case, we present an instance of aggressive pilomatrixoma in a 90-year-old female. The patient exhibited a large ulceroproliferative growth on the scalp. A preliminary biopsy was performed, followed by wide local excision. Both the biopsy and excision specimens exhibited characteristics of pilomatrixoma, with a focal infiltrative growth pattern and moderate nuclear atypia. Features indicative of malignancy, such as geographic necrosis, brisk mitosis, atypical mitosis, vascular invasion, and perineural invasion, were not observed. Therefore, the diagnosis of aggressive pilomatrixoma was made. Wide local excision and subsequent skin grafting were conducted, with no involvement of deeper subcutaneous tissue and intact pericranium. The patient is currently faring well during follow-up. This case underscores the importance of meticulous histopathological examination for distinguishing aggressive variants, as they carry a higher risk of recurrence and aid in distinguishing aggressive pilomatrixoma from carcinoma. Failure to recognise the aggressive variant may result in inadequate management.

Abstract:Pilar cysts/trichilemmal cysts are benign lesions that arise from the hair follicle epithelium. The commonest area of occurrence is the scalp but it can also occur in the head and neck region. The pilar cyst grows at a prolonged rate. They are relatively rare and occur in about 10% of the population. They appear in the region of high concentrations of hair follicles. A 75-year-old male patient came to the Department of Oral and Maxillofacial Surgery with a chief complaint of swelling on the left side of the face for the past one year. Cytological examination revealed an infected cystic lesion. Computed tomography (CT) showed a well-defined lesion in the left temporal region. After surgical excision of the lesion, it was sent for histopathological examination. Excisional biopsy revealed a pilar cyst. We report a rare case of pilar cyst in the left temporal region in a patient who was previously operated on for osteomyelitis of the left side of the mandible up to the coronoid process. These cysts may mimic temporal space infection and lead to an incorrect treatment plan.

Abstract:Introduction: Basal cell carcinoma and squamous cell carcinoma are malignant epithelial tumors that frequently occur on the scalp. The reconstruction of large surgical breaches in which the scalp was removed leaving the bone bare has always been a difficult problem to solve. Case presentation: An 84-year-old patient in good general condition with an extensive surgical breach in the scalp comes to our observation where a sessile squamous cell carcinoma was removed 2 months ago in another hospital; The surgeon had removed the aponeurotic galea with bone exposure. Clinical discussion: There are numerous surgical solutions proposed: reconstruction by a secondary intention, grafts of dermal matrix, transposition flaps and finally an old technique that involves the perforation of the cranial theca. Dermal matrices cannot be used on bone as they still need blood support to facilitate the repair process. Local flaps could not be used as the skin was seriously photodamaged and treatment of this would further delay the repair. In our case the solutions adopted with other patients were not applicable, therefore we evaluated the literature to determine which solution could be adopted. We had experience with tissue perforation in large ulcers and position punch grafting to facilitate re-epithelialization therefore we also drew inspiration from this method. Conclusion: The technique adopted allowed an immediate reconstruction limiting the discomfort to the patient with few dressings and complete healing in about 1 month. Scalp, the graft is completely rooted in 3 month.
A Rare Hybrid Skin Cyst of the Scalp With Pilar and Apocrine Features

Abstract:Benign epithelial skin cysts containing multiple components of the folliculo-sebaceous apocrine unit are only rarely reported in the literature. Here, we describe a 12-year-old girl who presented with a cystic mass on the vertex of her scalp. Upon resection, it showed a hybrid benign skin cyst with interesting histological features of both pilar and apocrine differentiation. The clinicopathological and imaging findings of this unusual skin cyst, successfully managed by a plastic surgeon and neurosurgeon, are described. Pathologists and clinicians should be aware of this type of skin cyst rarely encountered in their clinical practice.
Cutaneous myoepithelioma on the head: a case report

Abstract:A case of cutaneous myoepithelioma on the head is reported. The 42-year-old male patient presented with a tumor on the scalp for two years without any symptom. Skin examination showed that a red, hemispherical, well-circumscribed tumor on the scalp, in a size of 1 cm x 1 cm, with telangectasis on the smooth surface, no ulceration and hemorrage were noted. The mass was hard without tenderness, it was completely excised. Pathologically, the epidermis was normal, a well-circumscribed tubercle was seen in the dermis. The tumor cells in the tubercle consisted of epithelioid and plasmacytoid cells, and the intercellular matrix showed hyaloid degeneration. These cells were separated as cord-like, trabecular shapes and masses by the denaturedmatrix. Immunohistochemically, the tumor cells were strongly positive for CK, focally for S-100, and diffusely strongly positive for P63. Combined with clinical manifestations, histopathologic features and immunohistologic findings, the patient was diagnosed as cutaneous myoepithelioma. No recurrence had been found for one year follow-up after operation. The patient is still being followed-up.
Pilomatricoma: A Case Report

Abstract:Pilomatricoma is one of the rare skin benign neoplasms arising from the matrix cells of hair follicles. We report the case of a 17-year-old female who had a swelling over the medial surface of the right ear lobule, which was determined to be an epidermal inclusion cyst by radiological and clinical examination. Over the period of eight days, it grew to 2 × 2 cm. A well-defined encapsulated lesion with thick cellular debris was found on an ultrasonography of the local region. Under local anesthesia, the patient had a cyst excision. Early detection by ultrasonography is beneficial in skin lesions to confirm if they are benign or malignant. Complete excision of the cyst is the treatment in order to limit the morbidity and lower the aggressive behavior. In this case, we came to a diagnosis after a histopathological examination, confirming it as a pilomatricoma. Due to its rarity, it is often misdiagnosed.
Eccrine poroma over the scalp: an atypical presentation

Abstract:An eccrine poroma is a solitary tumor arising from the eccrine duct epithelium in the epidermis. The lesions commonly occur on palm and soles, and occasionally on the nose, eyelids, neck, and chest. We report a patient who presented with a slow-growing nodular lesion over his scalp, dermoscopic examination showed lobular appearance composed a structureless white areas and pigmentation at periphery with erosion in centre, vascular pattern with interlacing white cords separating structure-less islands with central ulceration. Histopathology examination showed a nodular infiltrate composed of poroid cells extending from epidermis to dermis and finding suggestive of an eccrine poroma. We are presenting this case for its unusual site and early onset of presentation.

Abstract:Abstract A 73-year-old man was referred from a secondary hospital to the dermatology department with a 2-year history of a biopsy-reported recurrent basal cell carcinoma on the left nasal ala. Full-skin examination revealed a solitary 20 × 11-mm, slightly tender, pink-coloured nodule. His past medical history was significant for multiple nonmelanoma skin cancers and colon cancer. No regular medications were prescribed. Mohs micrographic surgery was performed, with two layers required for margin clearance. The Mohs debulk described a carcinoma with features suggestive of a cylindrocarcinoma with necrosis and a high mitotic rate. On immunohistochemical profiling, tumour cells were strongly positive for S0X10, p63 and p53; had multifocal moderate positive staining for S100; had membranous staining for BerEP4; and were focally positive for CD117. Tumour cells were negative for epithelial membrane antigen, calponin, smooth muscle actin, keratin 20, chromogranin, GATA3 and p16. A PET-CT scan was performed, which ruled out any metastatic disease. No clinical recurrence has been observed 8 months after excision. Cylindromas are benign adnexal neoplasms, usually arising on the head and neck. Malignant transformation of these tumours has been reported only rarely, with fewer than 50 cases in the literature. A female predominance has been noted in both benign and malignant cylindromas. Nineteen cases of metastatic disease have been described, typically in the high-grade form of this malignant tumour, with spread to local lymph nodes or distant viscera. Histopathologically, cylindrocarcinomas may show a salivary gland-type basal cell adenocarcinoma-like pattern. Depending on the grade of the tumour, basaloid cells vary in size and degree of nuclear pleomorphism, and are surrounded by a hyaline eosinophilic sheath. Loss of the hyaline sheath network in a jigsaw pattern can be recognized by partial or complete loss of periodic acid–Schiff positivity in malignant cases. Focal to extensive tumour necrosis is frequently seen. Lymphovascular or perineural invasion is not typically identified. The similar clinical characteristics of cylindrocarcinomas to basal cell carcinomas prompt an important differential diagnosis for dermatology clinicians to be aware of, given the metastatic potential. The rapid growth over 2 years in our case is atypical for basal cell carcinomas and more in keeping with cylindrocarcinomas, as documented in previous case reports of this rare tumour. This case also highlights the merits of reviewing histology slides from the original referring centre when performing Mohs micrographic surgery.

Abstract:Pleomorphic dermal sarcoma (PDS) is an undifferentiated high-grade tumour and is considered the most common primary soft tissue tumour in adults. The most common locations of PDS are the trunk, extremities and retroperitoneal regions. Skin involvement of PDS is rare, and involvement of the scalp is less common. Most cases of PDS appear as a gradually growing lesion for 1 to 2 years and are associated with ulceration and bleeding. The definitive treatment for PDS is usually surgical resection. We present an unusual case of a primary PDS involving the scalp of a 78-year-old male patient and discuss its unusual clinical presentation, dermoscopic features, histopathological correlation and its management.
