Abstract:Upper blepharoplasty is the most popular procedures in China and East and Southeast Asia. A variety of technical methods are used because the mechanism of the formation of double eyelid in the Asians is still controversial. Sayoc's theory that double eyelid is formed by the fusion of fibers from the levator aponeurosis and the pretarsal skin gains a majority support, but has never been proved in Asian eyelid. We carried on an anatomic study about the inferior part of the upper eyelid using both electron microscope and light microscope, and found direct evidence of the existence of this connection, which confirmed Sayoc's theory. This anatomic structure and motor system is then confirmed in clinical attempting of reapproximating them in upper blepharoplasty in thousands of Chinese patients by perfect satisfied results in terms of zero postoperative blepharoptosis rate, naturally looked appearance, and significantly shortened recovery time.

Abstract:Background“Sunken eyelid” is a term used to describe a deeply sunken area between the upper eyelid and upper edge of the orbital bone. The condition frequently is accompanied by blepharoptosis. Correction of a “sunken eyelid” generally is performed with a fat graft. However, surgical limitations such as a movement dysfunction of the upper eyelid or irregular skin surface often are encountered using the grafting method previously reported. An open technique is believed to be more precise than a closed procedure, and satisfactory results may be achieved by repositioning the upper eyelid fat and applying a fat graft to the retro-orbicularis orbital fat (ROOF).MethodsFat was harvested from the medial thigh area using a 10-ml syringe with minimal pressure. During the upper eyelid surgery, the orbital fat was relocated between the conjoined tendon of the levator aponeurosis and the lower orbicularis oculi muscle and skin flap. The ROOF was verified, and fat grafting then was performed from the upper medial side to the lower lateral side and from the medial aspect to the lateral direction using an 18-gauge Coleman needle. The sunken eyelids were classified into grades 1 to 4 according to the sunken depth (i.e., the distance between the most inferior orbital rim and the site of the most sunken area). The amount of fat graft and the technique of incorporating the fat were different according to sunken depth.ResultsFrom January 2006 to June 2008, 50 Korean and Chinese patients (48 women and 2 men) underwent fat repositioning and ROOF fat grafting using an open technique. The patients ranged in age from 24 to 67 years. The mean observation period was 4.7 months. The amount of fat graft for each palpebra was 0.3 to 3.3 ml, and the mean graft amount was 1.4 ml. Satisfactory results were obtained for most of the patients, but four patients required additional surgery. The reasons for the secondary surgery were a translocation of the fat graft to a lower position forming a lump (2 patients) and undercorrection (2 patients). The rate of fat absorption was relatively low and the complications very few compared with surgical cases involving a noninvasive method.ConclusionsMore precise treatment is possible by dividing the depressed area of the upper eyelid according to the stage. Satisfactory results were achieved by relocating the orbital fat using an open method and adjusting the ROOF fat graft according to the stage.

Abstract:Objective To study the precise indication,reasonable procedure,and stable effect of autologous fat graft injection in the correction of the sunken upper eyelid.Methods There were 124 cases in our study.Ninety-five cases were physiological and bilateral in origin.Twenty-nine cases were resulted from double eyelid surgery.The fat graft was sucked from the lower abdomen,fine particle processed,and then injected within the supraorbital rim,between the orbicularis and orbital septum (ROOF) without anesthetics.Results All patients were followed up for 3-22 months,with average 7.8 months.Satisfactory correction was attained in 104 cases.In cases of double eyelid over-correction were partly rectified.Nine of ptosis were corrected,although three were not improved.20 cases had had undercorrection,and 15 obtained satisfactory outcome after second procedure.Except 16 patients had ecchymoses which subsided within 7-10 days,there were no major complications.Conclusions Lipofilling in ROOF of sunken upper eyelid has no obvious scar adhesion or loose skin effectively.It can also correct over-corrected double eyelid and pseudoptosis at the same time.Autologous fat graft injection is an effective,safe and minimally invasive operation for the sunken upper eyelid correction.

Abstract:BACKGROUND: Sunken upper eyelid deformity results in an aged and unnatural appearance and is common among Asians along with the single eyelid. The upper eyelid volume restoration is as important as the creation of a supratarsal fold for patients with sunken eyelids. In the present study, the combination of orbital fat pad repositioning and orbicularis oculi muscle flap folding was established to correct the upper eyelid depression during blepharoplasty.METHODS: A retrospective study was conducted on patients who underwent the orbital fat pad repositioning and orbicularis oculi muscle flap folding to correct the upper eyelid depression between July 2020 and October 2021. The preoperative and postoperative sunken depths were calculated and compared, and overall patient satisfaction was performed to evaluate the operation efficiency.RESULTS: There were 40 patients, including 39 females and 1 male, who underwent surgery with an average age of 25.4 years. During the mean follow-up period of 12.7 months, the sunken depth improved from 3.2 mm preoperatively to 1.5 mm postoperatively with a statistical significance. Satisfactory results were achieved without major complication.CONCLUSIONS: For patients with mild to moderate sunken upper eyelids, the combination of orbital fat pad repositioning and orbicularis oculi muscle folding has been proved efficient and consistent to restore the upper eyelid volume and helpful to build esthetic double eyelids.

Abstract:Background: Women commonly experience upper eyelid dermatochalasis and upper eyelid depression with advancing age. Blepharoplasty is a suitable method for treating dermatochalasis, but not sunken eyelid. This study proposed a novel technique for eyelid rejuvenation by simultaneously correcting dermatochalasis and sunken upper eyelids in middle-aged women. Methods: Forty patients underwent subbrow blepharoplasty combined with brow fat pad transfer. Ellipse-shaped skin and subcutaneous tissue underneath the eyebrow were measured, demarcated, and excised. The orbicularis oculi muscle beneath the subcutaneous tissue was exposed and dissected in the upper-third area. The brow fat pad was turned downward using the lower edge as the pedicle and was fixed in the layer of retro-orbicularis oculi fat to fill the depressed area in the upper eyelid. The lower muscle flap was fixed to the supraorbital rim periosteum and upper musculocutaneous flaps, thereby forming a cross flap for interlocking fixation. The surgical outcomes were evaluated using a three-dimensional imaging device and the Global Aesthetic Improvement Scale. Results: The depth and volume of upper eyelid depression decreased significantly 3 months after the surgery and stabilized within 6 months. Global Aesthetic Improvement Scale scores significantly improved after the surgery, and the postoperative outcomes were acceptable. Conclusions: The novel technique is simple and effective for simultaneously correcting dermatochalasis and sunken upper eyelids in middle-aged women. The surgical outcomes are predictable and acceptable to most patients. Clinical question/level of evidence: Therapeutic, IV.

Abstract:Background Nowadays, people's pace of life continues to rapid up, and many bad habits will accelerate the aging of the eye periphery, and patients with sunken upper eyelids are to be found in younger people. In young Asians, single eyelids are often accompanied by upper eyelid depression, so correcting the upper eyelid depression during blepharoplasty becomes a higher challenge for plastic surgeons. Current surgical methods for upper eyelid depression include three major categories: tissue repositioning, injection and filling, and combined use. According to grades 1 and 2 are mild or moderate upper eyelid sunken. The sunken can be well corrected only by repositioning the orbital fat pad, while the correction effect for severe upper eyelid sunken in grades 3 and 4 is Poor, need to be used in combination to achieve the desired effect. Purpose The authors sought to determine whether, for patients with single eyelids and severe upper eyelid depression of grades 3 and 4, combined with orbital fat pad repositioning and autologous fat transplantation during blepharoplasty, an aesthetic and youthful blepharoplasty can be achieved. Methods This study included 79 patients with single eyelids and severe upper eyelid depression of grades 3 and 4 who received treatment between June 2020 and July 2022. All patients underwent double eyelid surgery plus orbital fat repositioning and autologous fat grafting. Results After a minimum follow-up period of 1 year, overall patient satisfaction was 92%. The recurrence rate within the first year was 6% and the complication rate was 5%. Conclusion This combined surgery may be an option for young Asians with single eyelids and severe upper eyelid depression. In this study, the surgery resulted in natural-looking double eyelids and younger-looking eye sockets in most patients. A combination of different surgical methods based on the patient's preoperative condition is critical to achieving long-term correction.

Abstract:Background: Tear trough deformity is one of the most common complaints in clinical settings. The correction of this groove is challenging in facial rejuvenation. The lower eyelid blepharoplasty varies with different conditions. A novel approach of using orbital fat in the lower eyelid to increase the volume of the infraorbital rim with granule fat injection has been applied in our institution for more than 5 years. Objectives: This article aims to describe the detailed steps of our technique and verify its effectiveness by a cadaveric head dissection after surgical simulation. Methods: In this study, a total of 172 patients with tear trough deformity underwent lower eyelid orbital rim augmentation with fat filling in the sub-periosteum pocket. According to Barton's grades, 152 patients underwent lower eyelid orbital rim augmentation with orbital fat filling, 12 patients had it combined with autologous granule fat from other body parts, and 8 patients received only transconjunctival fat removal to correct tear trough. Results: The modified Goldberg score system was used to compare preoperative and postoperative photographs. Patients were satisfied with the cosmetic results. Excessive protruding fat was released, and the tear trough groove was flattened by using autologous orbital fat transplantation. The lower eyelid sulcus deformities were well-corrected. To further illustrate the anatomical structure of the lower eyelid area and injection layers, six cadaveric heads were used for surgical simulation and demonstrated the effectiveness of our technique. Conclusions: This study indicated that the infraorbital rim could be increased by transplanting orbital fat to the pocket, which was dissected under the periosteum, and the procedure has been verified as reliable and effective. Reconstruction of Eyelid Defect Using Periosteal Stripe from Lateral Orbital Rim As Substitute of Tarsus

Abstract:OBJECTIVE:To evaluate the results of eyelid defect reconstruction using periosteal stripe of the lateral orbital rim.METHODS:This was a retrospective analysis of eyelid defects reconstruction in 13 patients with lateral canthus eyelid defect following the excision of the eyelid tumors including: seven patients with basal cell carcinoma, four patients with sebaceous cell carcinoma and two patients with squamous cell carcinoma. All eyelid defects were reconstructed with zygoma periosteal stripe as the posterior layer of the eyelid. The anterior layer of the eyelid was reconstructed by myocutaneous flap.RESULTS:After follow-up at 8 to 56 months, all reconstructed eyelids showed a normal appearance with good function and without deformities, e.g. eyelid notch, entropion and ectropion. There was no relapse of the eyelid tumor. All patients were satisfied with the results.CONCLUSIONS:This method is good for repairing lateral eyelid defect involving 1/3 to 2/3 eyelids and without the tumor invasion of lateral orbit rim. This procedure obtains satisfactory results, especially for simultaneous reconstruction of upper and lower eyelids defects.

Abstract:In senile patients with sunken superior sulcus, involutional ptosis, and higher eyelid crease, a single operation to correct depression or ptosis cannot achieve good results. We demonstrated the anatomy of periorbital septum fibers, which may contribute to the levator muscle's volume depletion and dynamic power transmission disorder, and described a procedure for correcting upper-eyelid depression and blepharoptosis in senile patients. The fibrous webs in these patients connected the posterior aspect of the orbicularis and the orbital septum and extended to the orbital fat and levator aponeurosis. These fibers were dissected to release the periorbital septal fibers, and the orbital septal fat flap was transferred to the depressed region. Advancement or plication of the levator aponeurosis was performed in patients with uncorrected blepharoptosis after the procedures described above. The technique was applied to 13 Chinese patients (25 eyes) between May 2021 and April 2022. Postoperative magnetic resonance imaging revealed that the preaponeurotic fat was displaced forward and down to the upper margin of the tarsus, and the curvature of the upper-eyelid depression was significantly improved. Moreover, the superior sulcus deformity improved, the ptosis was corrected, and the uppermost crease decreased in all patients. No recurrence of ptosis or abnormal adhesion was observed. We believe this is the first study using magnetic resonance imaging to evaluate eyelid anatomy and the effects of surgery in this patient group. Releasing periorbital septum fibers is crucial for correcting a portion of the sunken eyelid and ptosis in Asians.

Abstract:Asian eyelids are characteristically puffy, with a fold, and have bulging of the orbital fat and septum. Moreover, with the aging process, the orbital septum and skin lose elasticity and orbital fat atrophy proceeds, thus resulting in dermatochalasis and a sunken eyelid. We introduce a modified technique of infrabrow excision with septo-myocutaneous flap repositioning. An upper incision was made along the lower margin of the eyebrow and the placement of the lower incision was decided according to the amount of skin excision needed. After skin removal, the orbicularis oculi muscle (OOM) was incised 2 mm above the lower margin and dissected to expose the orbital septum. Additional dissection was performed along the preseptal plane to the supraorbital rim periosteum. Then, the lower flap (septo-myocutaneous flap) was anchored to the supraorbital rim periosteum. The overlapping OOM and subcutaneous layers were sutured separately. This technique makes it possible to correct the cause of an aging eyelid appearance. Additionally, an overlapping OOM aids in correcting sunken eyelids, and by tensioning the orbital septum, a puffy eyelid is corrected. As an advantage over the conventional infrabrow excision, our method prevents recurrence and corrects orbital septum laxity. Therefore, it can achieve more natural outcomes and lower recurrence rates.

Abstract:This study aimed to evaluate the effectiveness and safety of a double-suspension technique using the lateral orbital thickening (LOT) and the lateral brow thickening (LBT) in patients with lower eyelid defects. A total of 5 cadavers (4 males and 1 female) ranging in age from 50 to 78 years was subjected to observe the distribution of the orbicularis retaining ligament (ORL) and its characteristics. The ORL is a closed-loop structure that forms the LBT outside the supraorbital rim and forms the LOT in the lateral canthal region. A total of 17 patients (12 females and 5 males) with an average age of 12.3 years who underwent the ORL-assisted lower eyelid reconstruction were included in this retrospective study. After the ORL was completely released on the infraorbital rim for reducing down traction, the subocularis oculi fat and the orbicularis oculi were fixed to the LBT and the LOT for increasing the upward traction. The lower eyelid defects in 17 patients were reduced in different degrees. The defects were closed directly in 3 patients and were resurfaced by skin graft or flap in the other 14 patients. Physicians and patients were all satisfied with the final cosmetic results. Thus, the ORL is a rigid and restrictive internal connected structure, and the ORL-assisted technique decreases the defect size and reconstructs vertical force balance. The LOT and the LBT are strong and safe enough as new fixation choices for lower eyelid suspension.
Surgical correction of sunken upper eyelid with upper arcus marginalis release and precision fat distribution technique

Abstract:Background Sunken upper eyelids, characterized by hollowing in the upper orbital effects, involving the release of the arcus marginalis of the upper eyelid and the precise distribution of orbital fat. Methods From December 2021 to March 2023, a total of 84 eyelids from 42 patients who underwent surgical correction for sunken upper eyelids, utilizing the upper arcus marginalis release and precision fat distribution technique, were included in this study. Preoperative and postoperative sunken depths were measured and statistically analyzed. Aesthetic satisfaction was assessed through patient questionnaires. Results Preoperative and postoperative sunken depths measured 9.2 ± 2.2 mm and 5.9 ± 2.3 mm, respectively. The mean improvement was 3.3 mm, a change of statistical significance. Aesthetic outcomes and patient satisfaction yielded favorable results. No major complications were observed during the follow‐up period. Conclusion The upper arcus marginalis release and orbital fat distribution technique demonstrated favorable outcomes in correcting sunken upper eyelids. This procedure ensures stable placement of orbital fat at the deepest sunken point, resulting in aesthetically pleasing and enduring results. This technique serves as a valuable alternative for patients with moderate to severe sunken eyelids.

Abstract:Background High eyelid fold is a common complication of upper eyelid surgery. This study proposes a novel technique for correcting high eyelid fold in Asian patients with too little eyelid skin and thick orbital fascia to improve cosmetic outcomes and patient satisfaction. Methods We retrospectively analysed the clinical data of 38 patients with high eyelid fold repaired at the Plastic Surgery Hospital, Chinese Academy of Medical Sciences, from July 2017 to April 2022. All patients were treated with this method of preserving the high eyelid fold and reconstructing the subcutaneous orbital fascia. Postoperative outcomes were reviewed and analysed using objective measures and subjective surgeon and patient evaluations. Results Thirty-eight patients (71 eyes) were included. Postoperative follow-up was 6-25 months. At 6 months postoperatively, the mean anatomic crease height decreased from 10.45 mm to 6.22 mm, the mean pretarsal show decreased from 3.20 mm to 1.61 mm, and the mean margin reflex distance 1 of the 31 patients with ptosis increased from 2.93 mm to 3.87 mm. (P<0.001). The compliance rate between the surgeon's assessment and patient satisfaction was 89.5%, with 31 cases considered good by both the surgeon and patient and one case considered poor by both (undercorrected ptosis), though a satisfactory result was achieved after reoperation. Neither recurrence of the original crease nor serious complications were reported. Conclusion By preserving high eyelid folds and reconstructing the subcutaneous orbital fascia, unnaturally high and deep eyelid folds were converted to lower, nondepressed folds.

Abstract:Background Polyacrylamide hydrogel (PAAG) can cause serious complications during facial augmentation. Previous studies have reported complications following PAAG injections to correct tear troughs.Methods We report a rare case of tear trough deformity following PAAG injections. We developed a comprehensive treatment plan including transcutaneous blepharoplasty and fat grafting to address the presenting complications.Results The patient showed flat lower lids and no more complications 1 year after surgery.Conclusions Blepharoplasty combined with fat grafting is an effective approach to eliminate PAAG injection complications in tear troughs.Level of Evidence IV This journal requires that authors assign a level of evidence to each article. Clinical application of autologous chyle fat transplantation in the correction of sunken upper eyelid

Abstract:Aim: We investigated the clinical application of autologous chyle fat in the correction of sunken upper eyelid. Methods: From November 2020 to October 2021, 89 cases of correction with autologous chyle fat to sunken upper eyelid were performed (with or without other cosmetic procedures related to the eyes). An appropriate amount of fat was extracted from the superficial layer of the patient's thigh and processed to chyle fat. About 0.5-2.8 mL of fat was injected under the orbicularis oculi muscle (roof-retro orbicularis oculi fat) or in the area where the orbital septal fat exists from the outer orbital margin of the upper eyelid. Results: In total, 59 patients were followed up for 1-11 months after surgery. No infection, necrosis, vascular embolism, upper eyelid lumpiness, fat calcification, or liquefaction occurred in all patients, while seven cases showed inadequate correction. The depression basically disappeared, the shape was relatively full, the youthfulness was greatly improved, and the satisfaction of patients was high during the follow-up period. Conclusion: The correction with autologous chyle fat to sunken upper eyelid showed little trauma, short recovery periods, and satisfactory results, and it is worthy of being popularized and applied more widely.
