Abstract:Sir: We read with great interest the article by Rohrich et al. entitled “The Six-Step Lower Blepharoplasty: Using Fractionated Fat to Enhance Blending of the Lid-Cheek Junction” published in the June of 2017 issue of the Journal.1 Previously, Rohrich et al. described a “five-step” clinical approach2 based on anatomical principles for lower lid blepharoplasty, as follows: step 1, fat augmentation to the deep malar fat compartment; step 2, conservative fat removal; step 3, orbicularis retaining ligament selective release; step 4, lateral retinacular canthopexy; and step 5, skin pinch and excision. By addressing the root cause of lower lid shape and aging, the aesthetic outcomes are improved. Now, the authors added a sixth step: inject fractionated fat above the periosteum and below the orbicularis oculi muscle with 50 percent overcorrection with the purpose of blending the lid-cheek junction even more. Here, we want to discuss the possibly right anatomical plane and volume for injecting the fractionated fat, which thus makes the most anatomical and physiologic sense, for surgical intervention. According to Haddock’s fresh cadaver lower lid and midface dissections,3 we know that one anatomical causative factor contributing to external visibility of the tear trough and lid-cheek junction landmark is the fact that the eyelid skin above the landmark and the cheek skin below the landmark have distinctly different textures and thicknesses. The ideal and most physiologic treatment for this factor would appear to be in the superficial plane between the skin and the underlying muscle. In this article, the author mechanically emulsified the harvested fat particles and injected them in a deeper plane. We know that the obtained fractionated fat, also called nanofat,4 can be smoothly injected with a sharp fine needle into the superficial intradermal and subdermal plane with the skin rejuvenation effect to improve skin quality, and will not leave contour irregularities or be visible like microfat or filler when injected superficially. In addition, the adipocytes were destroyed during the emulsification process, and the fractionated fat does not have the capacity to build up a significant fat volume, which means that the volumetric effect is obviously very limited. Maybe it is not necessary to inject too much volume. Thus, we think that injecting the fractionated fat into the superficial intradermal and subdermal plane with no overcorrection could possibly achieve a better result.
