Abstract:Although inflammation plays a central role in the pathogenesis of acute lung injury, the molecular mechanisms underlying inflammatory responses in acute lung injury are poorly understood, and therapeutic options remain limited. CCAAT/enhancer-binding proteins, C/EBPβ and C/EBPδ, are expressed in the lung and have been implicated in the regulation of inflammatory mediators. However, their functions in lung pathobiological characteristics are not well characterized. Herein, we show that C/EBPβ and C/EBPδ are activated in mouse lung after intrapulmonary deposition of lipopolysaccharide (LPS). Mice carrying a targeted deletion of the C/EBPδ gene displayed significant attenuation of the lung permeability index (lung vascular leak of albumin), lung neutrophil accumulation (myeloperoxidase activity), and neutrophils in bronchial alveolar lavage fluids compared with wild-type mice. These phenotypes were consistent with morphological evaluation of lung, which showed reduced inflammatory cell influx and minimal intra-alveolar hemorrhage. Moreover, mutant mice expressed considerably less tumor necrosis factor-α, IL-6, and macrophage inflammatory protein-2 in bronchial alveolar lavage fluids in LPS-injured lung compared with wild-type mice. In contrast, C/EBPβ deficiency had no effect on LPS-induced lung injury. By using small-interfering RNA-mediated knockdown for C/EBPδ, we demonstrate, for the first time to our knowledge, that C/EBPδ plays a critical role for the tumor necrosis factor-α, IL-6, and macrophage inflammatory protein-2 production in LPS-stimulated alveolar macrophages. These findings demonstrate that C/EBPδ, but not C/EBPβ, plays an important role in LPS-induced lung inflammatory responses and injury.

Abstract:The role of Toll-like receptors (TLRs) in pathogen recognition has been expeditiously advanced in recent years. However, investigations into the function of TLRs in non-infectious tissue injury have just begun. Previously, we and others have demonstrated that fragmented hyaluronan (HA) accumulates during tissue injury. CD44 is required to clear HA during tissue injury, and impaired clearance of HA results in unremitting inflammation. Additionally, fragmented HA stimulates the expression of inflammatory genes by inflammatory cells at the injury site. Recently, we identified that HA fragments require both TLR2 and TLR4 to stimulate mouse macrophages to produce inflammatory chemokines and cytokines. In a non-infectious lung injury model, mice deficient in both TLR2 and TLR4 show an impaired transepithelial migration of inflammatory cells, increased tissue injury, elevated lung epithelial cell apoptosis, and decreased survival. Lung epithelial cell overexpression of high molecular mass HA protected mice against acute lung injury and apoptosis, in part through TLR-dependent basal activation of NF-kappaB. The exaggerated injury in TLR2 and TLR4 deficient mice appears to be due to impaired HA-TLR interactions on epithelial cells. These studies identify that host matrix component HA and TLR interactions provide signals that initiate inflammatory responses, maintain epithelial cell integrity, and promote recovery from acute lung injury.

Abstract:Background Acute lung injury (ALI) is a syndrome of interstitial pulmonary and alveolar diffuse edema caused by direct or indirect factors,which could further develop into acute respiratory distress syndrome(ARDS).Sepsis is the critical cause of ALI/ARDS.A lot of studies had been performed in the pathogenesis of ALI/ARDS,particularly the role of signaling pathway mediated by Toll-like receptor.Recently,the signaling pathway mediated by non-Toll-like receptors has attracted a lot of attentions.Objective The aim of this article was to summary the roles of non-Toll-like receptors in the pathogenesis and pathophysiological process of ALI induced by sepsis,thus to provide the early molecular targets for the diagnosis and treatment of acute lung injury.Content The role of non-Toll-like receptors signaling pathways in mechanisms of the acute lung injury induced by sepsis was reviewed in present article.Trend Non-Toll-like receptors played critical role in the pathogenesis of ALI induced by sepsis,which will be the focus issue of research field.
Protective Role of Ilc2 in Sepsis-Induced Acute Lung Injury

Abstract:Abstract Objective Group 2 innate lymphoid cells (ILC2), a recently identified population of innate immune cells, were suggested to be protective in infection. However, the underlying mechanism remains unknown. The current study aimed to determine the protective role of ILC2 in sepsis-induced acute lung inflammation and the underlying mechanism. Methods WT and IL-33 knockout mice were subjected to cecal ligation and puncture (CLP) to induce sepsis. The number of ILC2 and the concentration of cytokines including IL-4, IL-9 and IL-13 in the lung tissue were measured by flow cytometry and ELISA, respectively, at different time after CLP. In the in vitro studies, lung-derived ILC2 were co-cultured with mouse lung endothelial cells (LEC) and macrophages (MΦ), respectively, and challenged with LPS and/or TNF-α for up to 48 h. The LEC apoptosis and the ability of MΦ phagocytosis of bacteria were then detected by confocal fluorescence microscopy. Results Sepsis induced IL-33-dependent recruitment of ILC2 in the lung after CLP. The levels of ILC2-derived IL-9 and IL-13 significantly increased in the lung tissue of WT septic mice and in the supernatant of cultured ILC2 after LPS stimulation. IL-9 protects LPS/TNFα-induced LEC apoptosis, whereas, IL-13 decreased MΦ apoptosis in response to LPS/TNF-α. In addition, both IL-9 and IL-13 promote MΦ phagocytosis of bacteria. Conclusions IL-33-mediated ILC2 recruitment in the lung following sepsis protects lung injury through reducing apoptosis of LEC and MΦ and promotes bacteria clearance by MΦ. Increasing ILC2 and/or ILC2-derived cytokines in the lung may present a new therapeutic strategy for acute lung injury in sepsis.

Abstract:The present study aimed to investigate the effects of the Toll-like receptor (TLR)4/myeloid differentiation primary response (MyD)88 signaling pathway on sepsis-associated acute respiratory distress syndrome (ARDS) in rats, and the involvement of macrophage activation and the inflammatory response. A total of 36 specific pathogen-free male Sprague-Dawley rats were selected to establish the rat model of sepsis-associated ARDS using cecal ligation and puncture (CLP). Rats were assigned into the Ab (anti-TLR4 monoclonal antibody)-CLP, CLP and Sham groups. Arterial partial pressure of oxygen (PaO2) was detected using blood gas analysis. Bronchoalveolar lavage fluid (BALF) and alveolar macrophages were collected. The pathological structure of lung tissue was observed following hematoxylin-eosin staining. The ultrastructural alterations of alveolar epithelial cells were observed under transmission electron microscope. The ratios of wet/dry weight of lung tissue and total protein content in BALF were measured. The concentration of tumor necrosis factor (TNF)-α and interleukin (IL)-1β in BALF and peripheral blood was determined by enzyme-linked immunosorbent assay. The TLR4, TLR9, MyD88 and nuclear factor (NF)-κΒ mRNA and protein expression levels in alveolar macrophages were measured by reverse transcription-quantitative polymerase chain reaction and western blotting. Compared with the Sham group, the rats in the CLP group demonstrated significantly increased respiratory frequency, lung permeability, lung edema, inflammatory infiltration, TNF-α and IL-1β expression levels in BALF and peripheral blood and TLR4, TLR9, MyD88 and NF-κΒ expression levels in macrophages, however decreased arterial PaO2. Following pretreatment with anti-TLR4 monoclonal antibody, rats exhibited decreased lung injury, inflammatory infiltration, lung edema, TNF-α and IL-1β expressions in BALF and peripheral blood, and TLR4, TLR9, MyD88 and NF-κΒ expression levels in macrophages, with increased arterial PaO2. These results suggested that the inhibition of TLR4/MyD88 signaling pathway may relieve sepsis-associated ARDS in rats through regulating macrophage activation and the inflammatory response.

Abstract:Objective To study the dynamic expression of Toll-like receptor 4 in septic acute lung injury in rats.Methods Thirty-two Wistar rats were randomly divided into control group(n=12) and septic acute lung injury group(n=20).The rats were sacrificed after 6,12,18 and 24 hours.The expression of Toll-like receptor 4 mRNA in lung was measured with real-time PCR.The expression of Toll-like receptor 4 protein in lung was analyzed with immunohistochemistry.Results The expressions of Toll-like receptor 4 mRNA and Toll-like receptor 4 protein increased in septic acute lung injury group compared with the control group (P<0.01),reached the peak 18 hours after cecal ligation puncture and then declined gradually.Conclusion The increase of Toll-like receptor 4 has an important effect on the course of septic acute lung injury.

Abstract:Objective To investigate the expression of Tlr4 and CD14 mRNA in the lung and hepatic tissues in sepsis rats.Methods The sepsis model was made by cecal ligation and puncture (CLP).Fifty male SD rats were randomly divided into 5 groups:normal control (n=10),CLP (subdivided into 24,48,72,96 h post-CLP,n=10 in each).At serial time points in each group,animals were sacrificed,and the tissues of lung,liver,,kidney and intestine were harvested to detect the expression of Tlr4 and CD14 mRNA by using reverse transcription polymerase chain reaction (RT-PCR).Results The expression levels of Tlr4 and CD14 mRNA were significantly increased in lung and hepatic tissues during 24-96 h after CLP as compared with control group (P 0.05), while in kidney and intestine there was no statistically significant difference (P 0.05). Conclusion Tlr4 and CD14 may be a key factor in the development of excessive inflammatory response.

Abstract:Resolution of the acute respiratory distress syndrome (ARDS) from pneumonia requires repair of the injured lung endothelium and alveolar epithelium, removal of neutrophils from the distal airspaces of the lung, and clearance of the pathogen. Previous studies have demonstrated the importance of specialized proresolving mediators (SPMs) in the regulation of host responses during inflammation. Although ARDS is commonly caused by Streptococcus pneumoniae, the role of lipoxin A4 (LXA4) and resolvin D1 (RvD1) in pneumococcal pneumonia is not well understood. In the present experimental study, we tested the hypothesis that endogenous SPMs play a role in the resolution of lung injury in a clinically relevant model of bacterial pneumonia. Blockade of formyl peptide receptor 2 (ALX/FPR2), the receptor for LXA4 and RvD1, with the peptide WRW4 resulted in more pulmonary edema, greater protein accumulation in the air spaces, and increased bacteria accumulation in the air spaces and the blood. Inhibition of this receptor was also associated with decreased levels of proinflammatory cytokines. Even in the presence of antibiotic treatment, WRW4 inhibited the resolution of lung injury. In summary, these experiments demonstrated two novel findings: LXA4 and RvD1 contribute to the resolution of lung injury due to pneumococcal pneumonia, and the mechanism of their benefit likely includes augmenting bacterial clearance and reducing pulmonary edema via the restoration of lung alveolar-capillary barrier permeability.

Abstract:TLRs are important for the recognition of conserved motifs expressed by invading bacteria. TLR4 is the signaling receptor for LPS, the major proinflammatory component of the Gram-negative cell wall, whereas CD14 serves as the ligand-binding part of the LPS receptor complex. Triggering of TLR4 results in the activation of two distinct intracellular pathways, one that relies on the common TLR adaptor MyD88 and one that is mediated by Toll/IL-1R domain-containing adaptor-inducing IFN-beta (TRIF). Nontypeable Haemophilus influenzae (NTHi) is a common Gram-negative respiratory pathogen that expresses both TLR4 (LPS and lipooligosaccharide) and TLR2 (lipoproteins) ligands. To determine the roles of CD14, TLR4, and TLR2 during NTHi pneumonia, the following studies were performed: 1) Alveolar macrophages from CD14 and TLR4 knockout (KO) mice were virtually unresponsive to NTHi in vitro, whereas TLR2 KO macrophages displayed a reduced NTHi responsiveness. 2) After intranasal infection with NTHi, CD14 and TLR4 KO mice showed an attenuated early inflammatory response in their lungs, which was associated with a strongly reduced clearance of NTHi from the respiratory tract; in contrast, in TLR2 KO mice, lung inflammation was unchanged, and the number of NTHi CFU was only modestly increased at the end of the 10-day observation period. 3) MyD88 KO, but not TRIF mutant mice showed an increased bacterial load in their lungs upon infection with NTHi. These data suggest that the MyD88-dependent pathway of TLR4 is important for an effective innate immune response to respiratory tract infection caused by NTHi.

Abstract:Streptococcus pneumoniae is the leading pathogen of community-acquired pneumonia and is a main cause of infectious deaths. However, little is known about host-pathogen interaction in human lung tissue. We tested the hypothesis that human alveolar macrophages (AMs) and alveolar epithelial cells (AECs) are important for initiating the host response against S. pneumoniae, and we evaluated the role of Toll-like receptor (TLR) 2, TLR4, and p38 mitogen-activated protein kinase (MAPK) signaling in the inflammatory response after pneumococcal infection. We established a novel model of acute S. pneumoniae infection using vital human lung specimens. In situ hybridization analysis showed that S. pneumoniae DNA was detected in 80 to 90% of AMs and 15 to 30% of AECs after in vitro infection accompanied by increased expression of inflammatory cytokines. Enhanced phosphorylation of p38 MAPK and increased TLR2 and 4 mRNA expression were observed in infected lung tissue. Thirty to fifty percent of AMs and 10 to 20% of AECs showed evidence of apoptosis 24 hours after pneumococcal infection. After macrophage deactivation with Clodronate/liposomes, infected lung tissue exhibited a significantly decreased release of inflammatory mediators. Inhibition of p38 MAPK signaling markedly reduced inflammatory cytokine release from human lungs, whereas TLR2 blockade revealed only minor effects. AMs are central resident immune cells during S. pneumoniae infection and are the main source of early proinflammatory cytokine release. p38 MAPK holds a major role in pathogen-induced pulmonary cytokine release and is a potential molecular target to modulate overwhelming lung inflammation.

Abstract:Macrophages play a central role in dictating the tissue response to infection and orchestrating subsequent repair of the damage. In this context, macrophages residing in the lungs continuously sense and discriminate among a wide range of insults to initiate the immune responses important to host-defense. Inflammatory tissue injury also leads to activation of proteases, and thereby the coagulation pathway, to optimize injury and repair post-infection. However, long-lasting inflammatory triggers from macrophages can impair the lung's ability to recover from severe injury, leading to increased lung vascular permeability and neutrophilic injury, hallmarks of Acute Lung Injury (ALI). In this review, we discuss the roles of toll-like receptor 4 (TLR4) and protease activating receptor 2 (PAR2) expressed on the macrophage cell-surface in regulating lung vascular inflammatory signaling.

Abstract:Streptococcus pneumoniae is the main cause of bacterial pneumonia, a condition that currently produces significant global morbidity and mortality. The initial immune response to this bacterium occurs when the innate system recognizes common motifs expressed by many pathogens, events driven by pattern recognition receptors like the Toll-like family receptors (TLRs). In this study, lung myeloid-cell populations responsible for the innate immune response (IIR) against S. pneumoniae, and their dependence on the TLR4-signaling axis, were analyzed in TLR4-/- and Myeloid-Differentiation factor-88 deficient (MyD88-/-) mice. Neutrophils and monocyte-derived cells were recruited in infected mice 3-days post-infection. Compared to wild-type mice, there was an increased bacterial load in both these deficient mouse strains and an altered IIR, although TLR4-/- mice were more susceptible to bacterial infection. These mice also developed fewer alveolar macrophages, weaker neutrophil infiltration, less Ly6Chigh monocyte differentiation and a disrupted classical and non-classical monocyte profile. The pro-inflammatory cytokine profile (CXCL1, TNF-α, IL-6, and IL-1β) was also severely affected by the lack of TLR4 and no induction of Th1 was observed in these mice. The respiratory burst (ROS production) after infection was profoundly dampened in TLR4-/- and MyD88-/- mice. These data demonstrate the complex dynamics of myeloid populations and a key role of the TLR4-signaling axis in the IIR to S. pneumoniae, which involves both the MyD88 and TRIF (Toll/IL-1R domain-containing adaptor-inducing IFN-β) dependent pathways.
Implication of TIr4 and CD14 mRNA expression in lung and hepatic tissues in sepsis rats

Abstract:Objective To investigate the expression of Tlr4 and CD14 mRNA in the lung and hepatic tissues in sepsis rats. Methods The sepsis model was made by cecal ligation and puncture（CLP）. Fifty male SD rats were randomly divided into 5 groups： normal control （n = 10）, CLP （subdivided into 24, 48,72,96 h post-CLP, n = 10 in each）. At serial time points in each group, animals were sacrificed, and the tissues of lung, liver,, kidney and intestine were harvested to detect the expression of Tlr4 and CD14 mRNA by using reverse transcription polymerase chain reaction （RT-PCR）. Results The expression levels of Tlr4 and CD14 mRNA were significantly increased in lung and hepatic tissues during 24-96 h after CLP as compared with control group （P〈 0.05）, while in kidney and intestine there was no statistically significant difference （P 〉 0.05）. Conclusion Tlr4 and CD14 may be a key factor in the development of excessive inflammatory response. Key words: Sepsis; Toll like receptor; Lipopolysaccharide
