Abstract:Background: Facial acne scarring is a prevalent disease with both physical and psychosocial sequelae. Aims: This study aims to evaluate an innovative solid state dual wavelength 1,319 and 589 nm laser, which does not require consumable dye, for the treatment of acne scars. Patients/methods: A total of 12 patients (11 female, 1 man - Fitzpatrick skin phototypes II & III) with acne scar for more than one year, were treated with 1319 nm and subsequently by 589 nm, all having four-sessions, one every other week. A full face was covered in approximately 30 minutes. Acne scars were scored by one physician evaluator using the ECCA grading scale before, 2 weeks after each treatment and 1 month and 6 months after the 4th treatment. Safety was measured by recording subject discomfort scores and adverse effects. Results: 12 subjects were enrolled into the study, 10 completed all 4 treatments and 2 were lost to follow up. Fluence used was 28 J/cm² ± 2.4 J/cm² at 1,319 nm and 16 ± 2.9 J/cm² at 589 nm. At baseline, mean ECCA score was 98 ± 23. This score was reduced to 88 ± 30 (p<0.02), after one session, to 68 ± 21 (p<0.01) after 2 sessions, to 58 ± 17 (p<0.01) after 3 sessions to reach 58 ± 15 (p<0.01) 1 month after the 4th and finally 66 ± 11 (p<0.01) at 6 month follow up. This observation corresponds respectively to 14%, 33%, 42%, 40% and 30% reduction of the ECCA score. Only one patient (ECCA score: 120) did not improve after 3 sessions. Slight to moderate erythema was sometimes observed without dryness or bruising. No or minimal burning or stinging was reported. No crust was observed. Conclusion: Improvement in scarring was noted in almost all patients with minimal discomfort and minimal downtime. Combining both minimal side effects with effective acne scar reduction, this laser appears to be highly effective. Long-term evaluation remains necessary to confirm the efficacy of this new laser.

Abstract:BACKGROUND Patients commonly complain about the appearance of acne scars. While various lasers and energy-based devices can offer clinical improvement, many of them are limited in treating patients with skin of color. Concerns exist regarding efficacy and safety, including risks of scarring, prolonged dyspigmentation, necrosis, and burns. OBJECTIVE To examine the utility of a 755-nm picosecond laser with fractionated lens array in improving the clinical appearance of acne scars in patients with Fitzpatrick skin types V and VI. MATERIALS AND METHODS A prospective clinical study investigated this laser using up to 5 monthly treatments. RESULTS Twenty-four subjects were enrolled, while 17 subjects completed a 3-month follow-up. The mean age was 33.9 years, and 95.8% of the subjects were women. Fitzpatrick skin types V and VI were represented. Assessments compared baseline to 3-month follow-up. Two of the 3 blinded reviewers agreed on identifying pretreatment and post-treatment photographs for 82.4% of the cases. For physician Global Aesthetic Improvement Scale, 100% of the subjects had clinical improvement. Overall, 94.1% of subjects were satisfied with their treatment. No serious or unanticipated adverse events occurred. CONCLUSION A 755-nm picosecond laser with fractionated lens array can safely and effectively improve acne scars in patients with Fitzpatrick skin types V and VI.

Abstract:Key point The 1064‐nm picosecond laser with holographic optics demonstrated significant efficacy in treating atrophic acne scars. Background Picosecond lasers with fractionated optics have enabled the development of a breakthrough skin rejuvenation method. The authors compared the fractionated, non‐ablative neodymium‐doped yttrium aluminum garnet 1064‐nm picosecond laser with holographic optics and the fractional CO2 laser in treating atrophic acne scars. Methods One side of each patient's face was randomly allocated and treated with three sessions of the 1064‐nm picosecond laser with holographic optics at 2‐month intervals. In contrast, the other side was treated with the fractional CO2 laser. Participants were followed up 3 months after the final session. The primary outcome included the physicians' evaluation using the ECCA grading scale and a four‐point scale to assess improvement. The patients' assessment of progress, their overall satisfaction and preferences, and the side effects were also evaluated. Results No significant difference was observed between the two lasers in terms of the mean ECCA scores after treatments (P = 0.209). The physicians' improvement assessment was more significant for the fractional CO2 laser (P = 0.001). Resurfacing of atrophic facial acne scars with a multimodality CO2 and 1570 nm laser system

Abstract:Abstract Background Scarring is one of the most prevalent long‐term complications of acne vulgaris and has cosmetic, psychological, and social burdens. Contemporary management programs integrate multiple modalities to best address the multiple factors underlying their development and persistence. This work assessed the impact of sequential multimodal laser therapy on acne scar geometrics and texture. Methods Adult patients ( n = 16) with Fitzpatrick skin type II–IV and presenting with facial acne scars, underwent three combination ablative (CO 2 ), and nonablative (1570 nm) laser treatment sessions at two‐month intervals. Treatment was delivered using a ProScan Hybrid applicator, with each regimen including illumination with both ablative and a nonablative lasers applied in a grid mode sequence. Scar microtopography was assessed at baseline and 6 months after the last treatment session. Results At baseline, all patients had both box and rolling scars, while only three had icepick scars. Six months following treatment, mean scar volume improved from 5.7 ± 5.2 mm 3 at baseline to 3.1 ± 3.0 mm 3 and mean affected area improved from 165.6 ± 134.0 mm 2 94.0 ± 80.1 mm 2 , translating to 47.0 ± 7.9% and 43.2 ± 8.6% reductions from baseline, respectively. Patients were highly satisfied with treatment outcomes, and no serious adverse reactions were documented during the course of treatment or follow‐up. Conclusion Multimodal CO 2 and 1570‐nm laser treatment improved the surface profilometry of patients with atrophic facial acne scars. Customization of both treatment intervals and laser settings to cosmetic regions, scar profiles and skin phototypes may further enhance treatment outcomes and expand its applicability to additional skin deformities.

Abstract:Background: Laser treatment of acne scars is common, but quality evidence on its efficacy is still needed. Our study aimed to compare picosecond laser and non-ablative fractional laser's efficacy and safety in treating acne atrophic scars. Methods: This was a randomized, split-face double-blind trial recruiting patients with acne atrophic scars. Facial halves were randomly divided and treated with fractionated frequency-doubled 1,064/532 nm picosecond Nd:YAG laser or non-ablative fractional 1,540 nm Er: glass laser. ECCA score (echelle d'evaluation clinique des cicatrices d'acne) and skin flatness measured with a non-invasive phaseshift rapid in vivo measurement of skin (PRIMOS) system were evaluated one month after the last treatment. Results: Twenty-two Fitzpatrick skin type IV patients were included in this study, with an average age of 29.68 years, an average duration of acne scars of 8.8 years. Picosecond laser impacted all acne scar types (before and after treatment; P=0.000 for all types, P<0.001 for V-type, P=0.002 for U-type, and P=0.021 for M-type) and more pronounced effect on ECCA score than non-ablative laser for V-type and U-type acne scars. After treatment, each treatment site's height was significantly lower than that before treatment (P=0.041) in the picosecond group but not in the non-ablative group (P=0.785). The reported erythema rate was higher in patients treated with a picosecond laser, while edema, exudation, purpura, pain, and long-term AEs were similar between the groups. Conclusions: Fractionated frequency-doubled 1,064/532 nm Picosecond Nd: YAG laser showed better efficacy in treating acne atrophic scars than the alternative and provided satisfactory safety with added improvement in pores and the glossiness of the skin. Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background and Objective</h3><p>Fractional 1064‐nm picosecond‐domain laser has recently been utilized for the treatment of atrophic acne scars and showed promising results. However, data on the safety and efficacy of this procedure in dark‐skinned patients are limited. This prospective, self‐controlled study was conducted to objectively evaluate the safety and efficacy of a 1064‐nm picosecond laser coupled with a microlens array (MLA) for the treatment of atrophic acne scars on Asian skin.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Study Design/Materials and Methods</h3><p>Twenty‐six subjects of Fitzpatrick skin types (FSTs) III and IV with atrophic acne scars were enrolled. All subjects were treated with a 1064‐nm picosecond laser (spot size of 8 mm, fluence of 1.0 J/cm<sup>2</sup>, a repetition rate of 10 Hz) in combination with the MLA handpiece for an average of three passes, for 6 monthly sessions. Objective (measurement of scar volume using three‐dimensional (3D) photography and skin roughness analysis using ultraviolet A‐light video camera) and subjective (clinical evaluation by two blinded dermatologists) assessments were obtained at baseline and at 1, 3, and 6 months after the final treatment. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Statistically significant reduction of the scar volume from baseline at 1, 3, and 6 months after the final treatment were observed by 3D photography and ultraviolet A‐light video camera. At the 6‐month follow‐up, 50% (13 of 26) of the subjects were rated as having at least 50% improvement of the scars. The rate of improvement significantly increased from the 1‐month follow‐up to the 6‐month follow‐up (<i>P</i> = 0.013). Similarly, at the 6‐month follow‐up, the scar volume (<i>P</i> = 0.024) and skin roughness (<i>P</i> = 0.001) also significantly improved, in comparison with the baseline. Mild postinflammatory hyperpigmentation (PIH) was observed to develop in approximately 18% of all the treatment sessions. All cases of PIH were temporary and resolved within 4 weeks on average. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>The 1064‐nm picosecond laser with MLA is a safe therapeutic alternative for the treatment of atrophic acne scars in dark‐skinned individuals. Abstract:The treatment of acne and acne scarring with lasers and light-based and energy-based technologies has become an integral component of our therapeutic arsenal. Lasers including infrared wavelengths and pulsed dye lasers; light devices including blue light, red light, and broadband light; and photodynamic therapy with aminolevulinic acid and methylaminolevulinic acid have been shown to be effective in the treatment of acne vulgaris. The optimal outcomes are achieved with photodynamic therapy combined with medical therapy. Acne scarring has been best treated with lasers, including nonablative infrared lasers, fractional nonablative and ablative laser resurfacing, and most recently needle-based radiofrequency devices.
A Single Center, Prospective, Randomized, Blinded Study to Evaluate the Efficacy and Safety of a Topical Tripeptide/Hexapeptide Anhydrous Gel When Used Pre- and Post- Hybrid Fractional Laser for the Treatment of Acne Scars

Abstract:Background: Acne scarring remains a significant problem. Laser therapy has produced varying results with deeper ablative therapies occasionally associated with side effects including delayed healing, infection, scarring, erythema, acne, milia, edema and dyspigmentation. Objective: Can adjuvant topical therapy impact the healing process and outcome of patients treated with fractional laser for acne scarring? Methods and materials: Ten patients were randomized to receive either Regenerating Skin Nectar with TriHex Technology®- RSN or a bland moisturizer. Patients underwent two laser procedures one month apart with Hybrid Fractional Laser. The topical was applied twice daily for 2 weeks prior to the first laser procedure, and through completion of the study. Seven study visits occurred over a 90-day period. Measurements were conducted in lesional and non-lesional areas - transepidermal water loss (TEWL), erythema, photography, Goodman and Baron qualitative scale, Global Aesthetic Improvement Scale and patient questionnaires - to assess functional recovery and aesthetic outcomes in the scarred areas. One patient from each cohort consented to biopsy before the procedures and 90 days after the first procedure. Results: Reduced TEWL scores in the RSN group were evident at all time points with statistically significant reductions occurring 4 days after first and second procedures indicating more efficient fluid conservation at a critical point in the healing trajectory. Erythema index demonstrated a consistent decrease in the RSN cohort over the control from day 4 through day 90 on lesional and non-lesional skin. Acne scar assessment scores improved in the RSN cohort compared with the control at all time points. Biopsy results showed early elastin regeneration in the RSN biopsy with controlled non-hypertrophic collagen formation evident. Conclusion: The use of RSN pre- and post- laser resurfacing significantly decreased postprocedural TEWL and erythema, and increased aesthetic improvement in acne scars and patient satisfaction, when compared with bland moisturizer.
Evaluation of the Safety and Efficacy of a Picosecond Alexandrite Laser With DLA for Acne Scars in Chinese Patients

Abstract:Background and objectives: Recently, picosecond laser treatment has been used as an effective treatment for acne scars. To evaluate the efficacy and safety of a picosecond alexandrite laser with a diffractive lens array in the treatment of acne scars in Chinese patients. Study design/materials and methods: Patients with facial acne scars were treated with a picosecond alexandrite laser in three sessions at 4- to 6-week intervals and followed up for 2 months. Primary outcomes were measured by physicians' blinded evaluation of the acne scar using the ECCA (échelle d'éva physicians'luation clinique des cicatrices d'acné) grading scale. The secondary outcomes included the investigator global assessment (IGA) on the improvement of post-inflammatory erythema (PIE), patients' assessment of improvement on a 4-point scale and of satisfaction on a 5-point scale. Pain scores and adverse effects were also evaluated. Result: Twenty patients with Fitzpatrick skin types III and IV were enrolled in the study and completed all treatment and follow-up visits. The mean ECCA scores fell from 197.75 ± 35.26 to 142.00 ± 35.92 (a 28% improvement), and the change was significant (P = 0.000). The mean IGA score of PIE improvement was 3.03 ± 0.75 (0 = no improvement and 4 = 76-100% improvement). On the basis of the patients' self-assessment, the average improvement scores were 2.30 ± 0.98 (0 indicating 0-25% improvement and 3 indicating >75% improvement). In total, 50% and 30% of the patients were "satisfied" and "very satisfied," respectively, with the treatment. The mean pain score was 3.20 ± 0.50 (0 = no pain, 10 = maximum pain) with topical anesthesia. The adverse effects included transient and mild erythema, edema, and scabbing. [Application of Fractional Laser Resurfacing in the Treatment of Superficial Scar].

Abstract:OBJECTIVE:To study the therapeutic effect, safety and risk of fractional resurfacing with ablative laser in the treatment of superficial scar.METHODS:88 cases of superficial scar, including 66 cases of acne scar, 12 cases of burn scar and 10 cases of other scar, were treated. All the patients were treated with Pixel (Er:YAG 2940 nm, > or = 3 times), or Encore (Ultrapulse CO2 10600 nm, > or = 2 times), or a combination of Pixel and Encore( > or = 3 times).RESULTS:The therapeutic effect of treatment was graded as medium when the scar was improved > 25%, as good when the scar was improved > 50%. The effective result was achieved in 80% of the patients. Good effect was achieved in 50% of the patients. There were 244 treatments of Pixel and 86 treatments of Encore. Persistent hyperpigmentation was happened in one case with Encore treatment, which relieved four months later. No other complication was happened.CONCLUSIONS:The fractional resurfacing with ablative laser is an effective method for superficial scar, especially acne scar. It is mini-invasive with low risk. The key mechanism is the ability of enhancing the tissue remodeling induced by heat effect of laser.
The Efficacy and Safety of Fractional 1064 nm Nd:YAG Picosecond Laser Combined With Intense Pulsed Light in the Treatment of Atrophic Acne Scar: A Split-Face Study

Abstract:Background and objectives: To evaluate the efficacy and safety of fractional 1064 nm Nd:YAG picosecond laser combined with intense pulsed light (IPL) in the treatment of atrophic acne scar with post-inflammatory erythema (PIE). © 2021 Wiley Periodicals LLC. Study design/materials and methods: Seventeen patients received five sessions of treatment at weeks 0, 4, 8, 12, 16 and were followed up at week 28. One half of the face was randomly treated by fractional 1064 nm Nd:YAG picosecond laser combined with IPL (FxPico + IPL), and the other by IPL alone as a control. Results: For the 15 patients who completed the study, the FxPico + IPL side demonstrated significant median Échelle D'évaluation clinique des cicatrices D'acné (ECCA) score improvement (P < 0.01), while IPL alone side did not (P = 0.1250). The pore counts for both sides decreased but more pore count reduction was seen on the FxPico + IPL side (P < 0.05). Better scar improvement was observed on the FxPico + IPL-treated side (P < 0.05) while no difference in erythema improvement was seen between the two sides. There was no difference between the two treatments in terms of overall satisfaction. Pain, erythema, edema, petechiae, crusting, reactive acneiform eruptions, and pruritus were common adverse effects and were mild to moderate. Conclusion: FxPico + IPL is an effective, efficient, and safe treatment regimen for atrophic acne scars complicated by PIE.
Modulated dual mode erbium/CO2 lasers for the treatment of acne scars

Abstract:Background: Treatment of acne scarring is a challenging problem for dermatologists and cosmetic surgeons. Combining skin resurfacing with other procedures, especially the use of filling agents, has proven to be superior to the use of a single modality. Carbon dioxide laser resurfacing has been popularized for the treatment of acne scarring, but post operative morbidity with stimulation of active acne has been problematic. Conventional erbium resurfacing is effective when sufficient fluences are used, particularly when treating relatively superficial acne scars. Deeper resurfacing for acne scars is difficult with erbium lasers, owing to bleeding and heavy exudation, which decreases ablation efficiency and makes 'end points' difficult to visualize. The newer modulated hybrid erbium/CO2 lasers (Derma K, ESC Sharplan, Yokneam, Israel) produce efficient ablation by the reduction of bleeding, yet at the same time incur little post operative morbidity. This is suitable for treating deeper acne scars, and may be combined with other procedures, e.g. fat grafting. Of particular importance is the lack of acne stimulation associated with the use of the erbium/CO2 hybrid lasers. Results: In my series of 78 patients, improvement in acne scarring was > 70% in the majority of patients. Post operative morbidity and complications were minor compared to conventional CO2 laser resurfacing.
Treatment of Post-acne Scarring with Long-pulsed and Q-switched 1,064nm Nd:YAG Laser

Abstract:Background: Acne scarring is one of the most dramatic consequences of inflammatory acne. It can lead to physical disfigurement and psychological burden on the affected individuals. Many treatment options for post-acne scarring are used, with variable results. Nonablative lasers, such as the 1,064nm neodymium-doped yttrium aluminum garnet (Nd:YAG) laser, are known to ameliorate acne scar appearance by stimulating collagen production and dermal remodeling. Objectives: We sought to evaluate the clinical efficacy, safety, and long-term effects of long-pulsed and Q-switched 1,064nm Nd:YAG lasers in the treatment of acne scars. Methods: From March to December 2019, a total of 25 patients with different skin types with acne scars were treated. Patients were divided into two groups. In Group I, 12 patients received a combination of Q-switched 1,064nm Nd:YAG laser, then long-pulsed 1,064nm Nd:YAG laser. In Group II, 13 patients received a combination of long-pulsed 1,064nm Nd:YAG laser, then Q-switched 1,064nm Nd:YAG laser. All patients received a total of six sessions at two-week intervals. Results: There were no statistically significant differences between the studied groups in skin type, lesions, or scar type. A positive response with either good or excellent results was documented in 43 patients, corresponding to 86. Six percent of the patients included in this study. Excellent response was observed in a total of 17 patients (26.6%). Twenty-six patients (60%) showed a moderate-to-good response, while seven patients (13.4 %) showed a fair response. The majority of patients in this study had an excellent-to-good response, with an 86.6% improvement of post-acne scars after laser sessions. Conclusion: Q-switched and long-pulsed 1,064nm Nd:YAG lasers are considered an efficient and safe modality for the treatment of mild and moderate post-acne scars. Both lasers can enhance dermal collagen remodeling and spare the epidermis with minimal downtime after the procedure.
Laser and Energy Treatments for Acne Scarring: A Review of Clinical Trials

Abstract:Background: Acne scarring can have a detrimental impact on quality of life, making early and effective treatment essential. Lasers and energy-based treatment are the preferred treatment modality, yet a clear consensus on the ideal laser selection is lacking in the literature. Aims: This review article aims to describe the therapeutic potential of lasers for acne scarring with a focus on clinical trials involving atrophic facial acne scars. Methods: A search was conducted in PubMed with keywords such as "acne scars", "laser resurfacing", "fractional laser", "nonfractional laser", "ablative laser", "nonablative laser", with attention to clinical trials completed between 2018 to 2022; results were summarized in narrative format. Results: Improvement in acne scarring is seen from a variety of lasers, with ablative lasers offering the most significant improvement combined with a longer downtime as compared to nonablative lasers. Laser treatment can be combined with a variety of other therapies, including platelet-rich plasma, to increase efficacy. The choice of laser is highly individualized and relies on shared decision-making between the patient and physician. Conclusion: This study presents an overview of clinical trials in laser treatment for acne scarring between 2018 through 2022. Overall, ablative lasers offered the most significant improvement in acne scars. Laser selection is a patient-centered process that requires attention to the type of scarring present, the patient's goals, and the advantages and disadvantages unique to each laser. Abstract:Background and objective: The utilization of lasers in dermatology has greatly expanded in recent decades. Acne scarring is a common indication in which lasers play an important therapeutic role. Study design/materials and methods: Available lasers include traditional ablative lasers, such as carbon dioxide and erbium lasers, traditional non-ablative lasers, such as neodymium, diode, alexandrite, pulsed dye lasers and intense pulse light, as well as both ablative, and non-ablative fractional laser systems. Conclusion: We sought to provide a framework for understanding the various types of lasers available to treat acne scars and review the primary literature pertaining to the efficacy, safety, and advantages of each laser discussed.
Selective nonablative treatment of acne scarring with 585 nm flashlamp pulsed dye laser

Abstract:Background: Selective nonablative wrinkle reduction with low-fluence pulsed dye laser has been shown to provide cosmetic benefits by stimulating the production of dermal collagen. The clinical efficacy for improving the appearance of acne scarring using selective nonablative laser treatments has yet to be established. Objective: To evaluate the improvement in the appearance and topography of acne scarring following application of a 585 nm pulsed dye laser with a temporal profile and pulse duration designed specifically to target healthy microvasculature in the dermis. Methods: Ten patients (mean age 34.8 years) with Fitzpatrick skin types I-IV and shallow to moderately deep, saucerized facial acne scars were enrolled in a prospective trial to receive a single laser treatment of both cheeks. Patients were evaluated at 30, 60, 90, and 120 days to assess the degree of clinical improvement. The evaluation process included assessment of pre- and posttreatment photography by two independent observers, patient assessment surveys, and surface profilometry using silicone imprints in order to quantify the degree of clinical improvement. Results: All 10 patients reported visible cosmetic improvement in the treated areas while surface profilometry showed that, on average, the depth of the acne scars was reduced by 47.8%. No adverse effects of this treatment were reported. Conclusion: The treatment of acne scars utilizing a 585 nm pulsed dye laser with a temporal profile and pulse duration designed specifically to target healthy microvasculature in the dermis may be a safe and effective noninvasive alternative for a natural result.
Management of acne scarring, part II: a comparative review of non-laser-based, minimally invasive approaches

Abstract:Acne scarring is a commonly encountered yet extremely challenging problem to treat for the dermatologist. As acne scarring can lead to significant psychological distress and low self-esteem, it is of utmost importance to have effective and satisfying treatments in the physician's armamentarium. However, many treatments are unsatisfying, leading to patient disappointment and frustration. Although early treatment of acne lesions and inflammation with isotretinoin is beneficial in preventing acne scarring, many patients still present with troubling noticeable scars. Despite the advances in pharmacology and technology, scar treatment still remains suboptimal and is tainted with several adverse effects. However, some treatments can provide benefits. This review article exhaustively discusses and analyzes the various minimally invasive approaches to the treatment of acne scarring with an emphasis on pharmacologic agents, such as isotretinoin for atrophic acne scars and corticosteroids and chemotherapeutic drugs for hypertrophic scars. Intralesional injections of corticosteroids are efficacious in reducing keloid scar formation in addition to preventing recurrence following surgical excision. In-office and minimally invasive procedural management, including chemical peels, dermabrasion, tissue augmentation, and punch excision is also discussed. Superficial chemical peels are efficacious in treating atrophic scars with relatively few adverse effects and complications. Although dermabrasion is used less often with the advent of laser resurfacing, this technique remains as a viable option for those with atrophic scars. Post-inflammatory hyperpigmentation can be managed successfully with topical agents such as azelaic acid and hydroquinone. The efficacy of various treatment modalities is highlighted with a focus on choosing the correct modalities for specific scar types.

Abstract:Introduction Acne scars can play havoc with the social functioning of the patient leading to severe depression with reduced quality of life. Fractional CO 2 laser provides a fine balance between the undesirable side effects of ablative lasers and the limited efficacy of nonablative lasers. Objective The present study was conducted to evaluate the efficacy and safety of fractional CO 2 laser in the treatment of acne scars. Patients and methods Fractional CO 2 laser treatment performed on 30 patients with atrophic acne scar by different protocols depending on the type and its depth. Results were assessed based on serial photographs, clinical improvement, and physician's global assessment scale. Results Of the 30 participants, 27 completed all three sessions. Out of the 27 patients in the final follow-up, three (10%) had excellent improvement of which two had mild acne scar and one had moderate scar before treatment; 16 (59.2%) had good response of which 13 had moderate acne scar and three had mild scar and eight (29.7%) had fair response of which six had moderate scars and two had mild scars. Conclusion Fractional CO 2 laser as monotherapy in the treatment of mild-to-moderate post-acne scars seems to be effective with good patient satisfaction and reduced side effect profile. In case of severe acne scars, fractional CO 2 laser can be used as a combined procedure along with other therapeutic modalities like subcision, platelet-rich plasma, and TCA CROSS (chemical reconstruction of skin scars using trichloroacetic acid) techniques.

Abstract:Background: Acne scars are common and cause cosmetic problems. There is a multitude of treatment options for acne scars, including dermabrasion, chemical peeling, and fillers, but the advent of laser technology has greatly improved the treatment of acne scars. Although several laser systems are available, studies comparing their efficacy are limited. This study compares the results of treatments using resurfacing (carbon dioxide, CO2; erbium-doped yttrium aluminum garnet, Er:YAG) versus fractional (nonablative fractional laser, NAFL; ablative fractional laser, AFL) lasers. Methods: A retrospective photographic analysis of 58 patients who underwent laser treatment for facial atrophic acne scars was performed. Clinical improvement was assessed by six blinded investigators with a scale graded from 0 to 10. Adverse events were also noted. Results: Mean improvement scores of the CO2, Er:YAG, NAFL, and AFL groups were 6.0, 5.8, 2.2, and 5.2, respectively. The NAFL group showed a significantly lower score than the other groups. The mean number of treatments was significantly greater in the fractional laser groups than in the resurfacing laser groups. The resurfacing laser groups had a prolonged recovery period and high risk of complications. The Er:YAG laser caused less erythema or pigmentation compared to the CO2 laser. Conclusions: Although the CO2 laser, Er:YAG laser, and AFL improved the acne scars, the CO2 laser had a greater downtime. Three consecutive AFL treatments are as effective as a single treatment with resurfacing lasers, with shorter social downtime periods and less adverse effects.

Abstract:Traditional medical treatments for acne vulgaris include a variety of topical and oral medications. The combination of poor compliance, lack of durable remission, and potential side effects are common drawbacks to these treatments. The use of lasers and light devices has increased dramatically in recent years due to the overall ease of treatment, predictable clinical efficacy, and minimal adverse effects. A variety of light and laser devices has been used for the treatment of acne, including the potassium titanyl phosphate (KTP) laser, the 585- and 595-nm pulsed dye lasers, the 1450-nm diode laser, radiofrequency devices, intense pulsed light sources, low-intensity light treatment, and photodynamic therapy using 5-aminolevulinic acid and indocyanine green. These devices are thought to target underlying pathogenic factors such as Propionibacterium acnes colonization, increased sebaceous gland activity, and the cutaneous inflammatory response. Lasers in particular also have a central role in the management of acne scarring, which tends to be refractory to medical therapies. Fractional photothermolysis, the 1450-nm diode laser, and pulsed dye lasers have been used in the nonablative treatment of acne scars with significant success. In this article, we review the current status of light- and laser-based treatment of acne and related conditions and briefly review the use of lasers for the treatment of acne scarring.
