Abstract:Research flow diagram Aim To investigate the independent influences affecting the global score of the Hammersmith Infant Neurological Examination (HINE) in the early life of high‐risk infants and to provide evidence for early effective screening and for evaluating interventions. Method We conducted a prospective cohort study of 258 high‐risk infants assessed by the HINE and Gesell Developmental Diagnosis Schedule at 3, 6, 9, and 12 months corrected age. A multiple linear regression model was developed to investigate independent influences on HINE global score at 3 months corrected age. Abstract:Journal of Child Neurology, Ahead of Print. The Hammersmith Neonatal (HNNE) and Infant (HINE) Neurological Examinations are increasingly used to evaluate developing neuromotor control in infants at risk for physical disability, but there is no global consensus on score interpretation across the first 6 months after birth. We report scores for typically developing, full-term infants aged 1 month for the HNNE and aged 2-6 months for the HINE. The median HNNE and HINE scores are consistent with previously published data. These normative data can be used to aid in the interpretation of HNNE and HINE scores from infants at risk for neuromotor impairment.

Abstract:OBJECTIVE: Cognition is an essential human function, and its development in infancy is crucial. Traditionally, pediatricians used clinical observation or medical imaging to assess infants' current cognitive development (CD) status. The object of pediatricians' greater concern is however their future outcomes, because high-risk infants can be identified early in life for intervention. However, this opportunity has not yet been realized. Fortunately, some recent studies have shown that the general movement (GM) performance of infants around 3-4 months after birth might reflect their future CD status, which gives us an opportunity to achieve this goal by cameras and artificial intelligence.METHODS: First, infants' GM videos were recorded by cameras, from which a series of features reflecting their bilateral movement symmetry (BMS) were extracted. Then, after at least eight months of natural growth, the infants' CD status was evaluated by the Bayley Infant Development Scale, and they were divided into high-risk and low-risk groups. Finally, the BMS features extracted from the early recorded GM videos were fed into the classifiers, using late infant CD risk assessment as the prediction target.RESULTS: The area under the curve, recall and precision values reached 0.830, 0.832, and 0.823 for two-group classification, respectively.CONCLUSION: This pilot study demonstrates that it is possible to automatically predict the CD of infants around the age of one year based on their GMs recorded early in life.SIGNIFICANCE: This study not only helps clinicians better understand infant CD mechanisms, but also provides an economical, portable and non-invasive way to screen infants at high-risk early to facilitate their recovery.

Abstract:Introduction: The testing of visuocognitive development in preterm infants shows strong interactions between perinatal characteristics and cognition, learning and overall neurodevelopment evolution. The assessment of anticipatory gaze data of object-location bindings via eye-tracking can predict the neurodevelopment of preterm infants at the age of 3 years; little is known, however, about the early cognitive function and its assessment methods during the first year of life. Methods: The current study presents data from a novel assessment tool, a Delayed Match Retrieval (DMR) paradigm via eye-tracking was used to measure visual working memory (VWM) and attention skills. The eye-tracking task that was designed to measure infants' ability to actively localize objects and to make online predictions of object-location bindings. 63 infants participated in the study, 39 preterm infants and 24 healthy full term infants – at a corrected age of 8–9 months for premature infants and similar chronological age for full term infants. Infants were also administered the Bayley Scales of Infant and Toddler Development. Results: The analysis of the Bayley scores showed no significant difference between the two groups while the eye-tracking data showed a significant group effect on all measurements. Moreover, preterm infants' VWM performance was significantly lower than full term's. Birth weight affected the gaze time on all Areas Of Interest (AOIs), overall VWM performance and the scores at the Cognitive Bayley subscale. Furthermore, preterm infants with fetal growth restriction (FGR) showed significant performance effects in the eye-tracking measurements but not on their Bayley scores verifying the high discriminatory value of the eye gaze data. Conclusion: Visual working memory and attention as measured via eye-tracking is a non-intrusive, painless, short duration procedure (approx. 4-min) was found to be a significant tool for identifying prematurity and FGR effects on the development of cognition during the first year of life. Bayley Scales alone may not pick up these deficits. Identifying tools for early neurodevelopmental assessments and cognitive function is important in order to enable earlier support and intervention in the vulnerable group of premature infants, given the associations between foundational executive functional skills and later cognitive and academic ability.

Abstract:Aim To develop a short version of the original Hammersmith Infant Neurological Examination (HINE) to be used as a screening tool (Brief‐HINE) and to establish if the short examination maintains good accuracy and predictive power for detecting infants with cerebral palsy (CP). Method Eleven items were selected from the original HINE ('visual response'; 'trunk posture'; 'movement quantity'; 'movement quality'; 'scarf sign'; 'hip adductor angles'; 'popliteal angle'; 'pull to sit'; 'lateral tilting'; 'forward parachute reaction'; 'tendon reflexes') identifying those items previously found to be more predictive of CP in both low‐ and high‐risk infants. In order to establish the sensitivity of the new module, the selected items were applied to existing data, previously obtained using the full HINE at 3, 6, 9, and 12 months, in 228 infants with typical development at 2 years and in 82 infants who developed CP. Results Brief‐HINE scores showed good sensitivity and specificity, at each age of assessment, for detecting infants with CP. At 3 months, a score of less than 22 was associated with CP with a sensitivity of 0.88 and a specificity of 0.92; at 6, 9, and 12 months, the cut‐off scores were less than 25 (sensitivity 0.93; specificity 0.87), less than 27 (sensitivity 0.95; specificity 0.81), and less than 27 (sensitivity 1; specificity 0.86) respectively. The presence of more than one warning sign, or items that are not optimal for the age of assessment, imply the need for a full examination reassessment. Interpretation These findings support the validity of the Brief‐HINE as a routine screening method and the possibility of its use in clinical practice.

Abstract:Background: The increasing clinical use of combining structural MRI (sMRI) with General Movements Assessment (GMA) or Hammersmith Infant Neurological Exam (HINE) before five months corrected age (CA) for early diagnosis of cerebral palsy (CP) lacks sufficient prognostic data for children with CP, especially those with Gross Motor Function Classification System (GMFCS) I. Objective: Evaluate the predictive value of sMRI, GMA, and HINE individually and in combination for early CP diagnosis and assess accuracy across varying GMFCS levels in a regional cohort of preterm infants. Methods: We performed sMRI between 39-44 weeks postmenstrual age and GMA and HINE between 12-18 weeks CA in 395 preterm infants before 32 weeks of gestation across five NICUs in Greater Cincinnati. Brain abnormalities on sMRI included white matter injuries, cortical and deep gray matter lesions, or extensive cerebellar hemorrhage. Absent fidgety movements constituted abnormal GMA; abnormal HINEs were scores below 56. The primary outcome was CP diagnosis at 22-26 months CA, classified by the GMFCS. We calculated sensitivity, specificity, positive predictive value, negative predictive value, and likelihood ratios for individual tests and combinations. Results: Of 338 (86%) infants with complete follow-up, 48 (14.2%) showed sMRI abnormalities, 15 (4.6%) had abnormal GMA, and 69 (20.9%) had abnormal HINE. Thirty-nine children (11.5%) developed CP at age 2, of which 28 had GMFCS level I and 11 had GMFCS II-V. The combination of sMRI and GMA achieved 100% specificity but only 22% sensitivity while the combination of abnormal sMRI and HINE demonstrated sensitivity of 32% and specificity of 98% for prediction of CP. Individual or combined tests showed far higher sensitivity (78-100%) for predicting CP in children with GMFCS levels II-V. Conclusions: The combination of sMRI with GMA or HINE demonstrated high specificity but low sensitivity for early CP diagnosis in a regional cohort of preterm infants. This approach appears effective for early detection of CP levels II-V but not for level I cases, the most prevalent type, underscoring the need for continued developmental follow-up for all very preterm infants and need for more sensitive diagnostic tools for early detection of CP.
Predictors Of Neurodevelopmental Outcomes At 12 Months In Term Newborns With Hypoxic Ischemic Encephalopathy

Abstract:The aim of this study was to evaluate the validity of different predictors and establish an effective prediction of the prognosis of hypoxic ischemic encephalopathy (HIE). The general information, cerebral computed tomography (CT) imaging, Apgar scores, and NBNA score were collected from 42 term newborn infants with HIE and 80 normal infants. The Bayley Scale of Infant Development was used to evaluate the levels of mental, motor and behavior of infants. The mental developmental index (MDI) and behavioral rating of normal infants were significantly higher than those of HIE group (P<0.05), while such differences were not noted in psychomotor developmental index (PDI). Significant correlations were found between MDI and PDI scores and some items of the behavioral rating. The HIE infants with severe CT degree were associated with markedly lower PDIs when compared with the normal infants and the infants with mild CT degree (P<0.05). For the NBNA scores, significant differences of MDI were noted both in the case and control groups (P<0.01). HIE significantly influenced the progression of mental, psychomotor and behavioral in infants. Cranial CT could be considered as a reliable index for helping to establish the prognosis of HIE.
Automated assessment of EEG background for neurodevelopmental prediction in neonatal encephalopathy

Abstract:Objective Assess the capacity of brain state of the newborn (BSN) to predict neurodevelopment outcomes in neonatal encephalopathy. Methods Trends of BSN, a deep learning‐based measure translating EEG background to a continuous trend, were studied from a three‐channel montage long‐term EEG monitoring from a prospective cohort of 92 infants with neonatal encephalopathy and neurodevelopmental outcomes assessed by Bayley Scales of Infant Development, 3rd edition (Bayley‐III) at 18 months. Outcome prediction used categories "Severe impairment" (Bayley‐III composite score ≤70 or death) or "Any impairment" (score ≤85 or death). Results "Severe impairment" was predicted best for motor outcomes (24 h area under the curve (AUC) = 0.97), followed by cognitive (36 h AUC = 0.90), overall (24 h AUC = 0.84), and language (24 h AUC = 0.82). "Any impairment" was best predicted for motor outcomes (12 h AUC = 0.95), followed by cognitive (24 h AUC = 0.85), overall (12 h AUC = 0.75), and language (12 and 24 h AUC = 0.68). Optimal BSN cutoffs for outcome predictions evolved with the postnatal age. Low BSN scores reached a 100% positive prediction of poor outcomes at 24 h of age. Interpretation BSN is an excellent predictor of adverse neurodevelopmental outcomes in survivors of neonatal encephalopathy after therapeutic hypothermia, even at 24 h of life. The trend provides a fully automated, objective, quantified, and reliable interpretation of EEG background. The high temporal resolution supports continuous bedside brain assessment and early prognostication during the initial dynamic recovery phase.

The predictive value of early oral motor assessments for neurodevelopmental outcomes of moderately and late preterm infants

Abstract:Oral motor assessment is used to identify abnormal sucking patterns which may reflect neurodevelopmental problems in preterm infants, but few studies have focused on moderately and late preterm infants. We enrolled 118 moderately and late preterm infants (mean gestational age, 35.04 weeks; mean birth weight, 2347.59 g) and analyzed the relationship between the Neonatal Oral-Motor Assessment Scale scores of these infants and the Chinese revision of Bayley Scales of Infant Development outcomes at 6 months corrected age. And the infants with abnormal sucking pattern had significantly lower Mental Development Index and Psychomotor Development Index and showed a higher rate of below average scores than control group (P = .003, P = .029, P = .022). The incoordination of suck-swallow-respiration was a risk factor for adverse neurodevelopment (RR = 3.67, 95% CI: 1.42-9.45). These indicate that abnormal sucking patterns in moderately and late preterm infants might provide some predictive value for short-term neurodevelopmental outcomes, but the clinical predictive value for developmental delay need to be determined in a longer term follow-up. This finding may offer a basis for early intervention.
Early Diagnosis of Mild Cognitive Impairment Based on Eye Movement Parameters in an Aging Chinese Population

Abstract:Background: The pathogenesis of dementia often starts several years prior to clinical onset during which the individual is asymptomatic. Existing strategies for the accurate diagnosis of early dementia are limited by high cost and the invasive nature of the procedures. Eye movement parameters associated with cognitive functions may be helpful in the early identification of dementia and in the development and evaluation of preventive and therapeutic strategies.Objective: We aimed to assess differences in eye movement parameters between healthy elderly individuals and patients with mild cognitive impairment (MCI). Furthermore, we examined the correlations between eye movement parameters with cognitive functions and specific hemispheric region and neural structures in individuals with MCI.Method: Eighty individuals with MCI without dementia (based on DSM-IV criteria) identified by community screening and 170 healthy controls were administered Chinese versions of MoCA and NTB, and a long (20 min) or short (5 min) version of a visual paired comparison (VPC) task. Two weeks later, 44 MCI patients and 107 healthy controls completed a retest of the VPC task, 44 MCI patients and 43 healthy controls among them administered a MRI. At the end of 1-year follow-up, a subset of 26 individuals with MCI and 57 healthy controls were administered the long version of VPC task and MoCA test again. Eye movement parameters and the relationship of eye movement parameters with cognitive functions and with changes in neural structures were compared between groups.Results: Patients with MCI were older, had less education, and had lower scores on cognitive tests than healthy controls. After adjustment for age and level of education, patients with MCI had lower novelty preference scores on the VPC than healthy controls. Using the logistic regression model, the amount of time that subjects focused on these novel images could predict MCI patients from normal elderly with an out of sample area under the receiver operator characteristic curve of 0.62. Furthermore, the cognition score of subjects whose novelty preference score was low decreased more remarkably in 1 year. For both the patient and control groups, VPC novelty preference was significantly correlated with verbal fluency and delayed and short-term memory function. Novelty preference score was also significantly correlated with the cortical thickness of several structures in the right hemisphere.Conclusion: Eye movement parameters are stable indicators to distinguish patients with MCI and cognitively normal subjects and are not affected by different testing versions and numbers. Additionally, the patients' cognitive deficits and eye movement indices were correlated. Future longitudinal studies should further explore the clinical utility of eye movement parameters as early markers of MCI.

Abstract:The HEALthy Brain and Child Development (HBCD) study, a multi-site prospective longitudinal cohort study, will examine human brain, cognitive, behavioral, social, and emotional development beginning prenatally and planned through early childhood. The study plans enrolling over 7000 families across 27 sites. This manuscript presents the measures from the Neurocognition and Language Workgroup. Constructs were selected for their importance in normative development, evidence for altered trajectories associated with environmental influences, and predictive validity for child outcomes. Evaluation of measures considered psychometric properties, brevity, and developmental and cultural appropriateness. Both performance measures and caregiver report were used wherever possible. A balance of norm-referenced global measures of development (e.g., Bayley Scales of Infant Development-4) and more specific laboratory measures (e.g., deferred imitation) are included in the HBCD study battery. Domains of assessment include sensory processing, visual-spatial reasoning, expressive and receptive language, executive function, memory, numeracy, adaptive behavior, and neuromotor. Strategies for staff training and quality control procedures, as well as anticipated measures to be added as the cohort ages, are reviewed. The HBCD study presents a unique opportunity to examine early brain and neurodevelopment in young children through a lens that accounts for prenatal exposures, health and socio-economic disparities.
A Stable and Scalable Digital Composite Neurocognitive Test for Early Dementia Screening Based on Machine Learning: Model Development and Validation Study

Abstract:BackgroundDementia has become a major public health concern due to its heavy disease burden. Mild cognitive impairment (MCI) is a transitional stage between healthy aging and dementia. Early identification of MCI is an essential step in dementia prevention. ObjectiveBased on machine learning (ML) methods, this study aimed to develop and validate a stable and scalable panel of cognitive tests for the early detection of MCI and dementia based on the Chinese Neuropsychological Consensus Battery (CNCB) in the Chinese Neuropsychological Normative Project (CN-NORM) cohort. MethodsCN-NORM was a nationwide, multicenter study conducted in China with 871 participants, including an MCI group (n=327, 37.5%), a dementia group (n=186, 21.4%), and a cognitively normal (CN) group (n=358, 41.1%). We used the following 4 algorithms to select candidate variables: the F-score according to the SelectKBest method, the area under the curve (AUC) from logistic regression (LR), P values from the logit method, and backward stepwise elimination. Different models were constructed after considering the administration duration and complexity of combinations of various tests. Receiver operating characteristic curve and AUC metrics were used to evaluate the discriminative ability of the models via stratified sampling cross-validation and LR and support vector classification (SVC) algorithms. This model was further validated in the Alzheimer’s Disease Neuroimaging Initiative phase 3 (ADNI-3) cohort (N=743), which included 416 (56%) CN subjects, 237 (31.9%) patients with MCI, and 90 (12.1%) patients with dementia. ResultsExcept for social cognition, all other domains in the CNCB differed between the MCI and CN groups (P<.008). In feature selection results regarding discrimination between the MCI and CN groups, the Hopkins Verbal Learning Test-5 minutes Recall had the best performance, with the highest mean AUC of up to 0.80 (SD 0.02) and an F-score of up to 258.70. The scalability of model 5 (Hopkins Verbal Learning Test-5 minutes Recall and Trail Making Test-B) was the lowest. Model 5 achieved a higher level of discrimination than the Hong Kong Brief Cognitive test score in distinguishing between the MCI and CN groups (P<.05). Model 5 also provided the highest sensitivity of up to 0.82 (range 0.72-0.92) and 0.83 (range 0.75-0.91) according to LR and SVC, respectively. This model yielded a similar robust discriminative performance in the ADNI-3 cohort regarding differentiation between the MCI and CN groups, with a mean AUC of up to 0.81 (SD 0) according to both LR and SVC algorithms. ConclusionsWe developed a stable and scalable composite neurocognitive test based on ML that could differentiate not only between patients with MCI and controls but also between patients with different stages of cognitive impairment. This composite neurocognitive test is a feasible and practical digital biomarker that can potentially be used in large-scale cognitive screening and intervention studies.

Abstract:Background A risk factor is described as a condition related to a possible negative or unfavorable outcome, which may be environmental, physical, or biological. Prematurity, a biological risk factor, is the major cause of neonatal mortality, associated with neurological sequelae, and has occurred in 1 out of 10 live births in São Carlos in 2019. Thus, access to scales with high sensitivity and predictability is pivotal to early detection (in the first four months of age) of delays or neuromotor impairments. A systematic review showed the predictive value of the General Movement Assessment (GMA) and Hammersmith Infant Neurological Examination (HINE) instruments associated with magnetic resonance imaging for early detection up to five months of age. Objectives To early detect motor impairments and delays during the first four months of life in infants with biological risk. Methods It is an observational, cross-sectional, and case-control study. Five infants from the biological risk group and five healthy full-term infants from the control group were assessed. Parents and legal guardians had to assign the informed and the image consent form. The identification form was used to characterize the personal and environmental factors. GMA and HINE instruments were performed to evaluate the neuromotor development. The data collection occurred in the home environment or at the Movement Analyses Research Lab (NENEM/UFSCar). Results The infants from the control group presented a mean chronological age of two months and 12 days, and the infants from the biological risk group presented a mean corrected age of one month and two days. The majority of the sample was from the female sex (90%), born at eutocyte birth (80%), with adequate weight for the gestational age at birth (90%), born from multiparous mothers (100%), and with gestational difficulties (60%). The most frequent sociodemographic characteristics were single parents (60%), both with complete high school education (80% for mothers and 70% for fathers), the mother's mean age of 30.3 years and the father's 27.2 years. Regarding the GMA results, all infants from the biological risk group were evaluated during the writhing movements period, in which 80% scored as moderately abnormal and 20% as definitely abnormal; only 20% of infants from the control group presented abnormal general movements. At HINE evaluation, 60% of the infants from the risk group presented resistance to shoulder passive movement and absence of alternate kicks in vertical suspension; 60% were unable to follow an object with their eyes; 80% had no auditory response; 100% had persistently fisted hands and 100% presented good suction. The tremors and cramped synchronized movements presence were observed in 60% of the infants from the biological risk group during the assessment throughout both instruments. Conclusion Infants from the biological risk group presented signs of neuromotor deficits at two months of chronological age. Implications Performing early detection before four months old might allow more efficient physiotherapeutic intervention.

[Early Screening of Cerebral Palsy During Infancy Using 'infant Motor Malfunction Profile' in the Communities of Two Cities].

Abstract:OBJECTIVE:To evaluate the prospective validity and predictive value of cerebral palsy during infancy, using 'Infant Motor Malfunction Profile'.METHODS:Items of motor milestone in the profile was used as the first step to screen cerebral palsy on 8137 infants who were screened at 3, 6, 9, 12 months of age. The positive cases who had one of 7 developmental abnormal motor milestone items were examined using the items of primitive reflexes or postural reaction as the secondary step of cerebral palsy screening. The cases who had at least one abnormal item of primitive reflex or postural reaction received neurological examination by doctors to diagnose cerebral palsy.RESULTS:According to our data, sensitivity, specificity, positive prospective value, positive and negative likelihood ratio of the profile in screening cerebral during infancy were 94.1%, 99.8%, 48.5%, 449.6 and 0.06, respectively.CONCLUSION:The profile seemed to be an acceptable instrument for early identification of cerebral palsy.
Reliability and Concurrent Validity of a Chinese Version of the Alberta Infant Motor Scale Administered to High-Risk Infants in China

Abstract:The Alberta Infant Motor Scale (AIMS) is widely used to screen for delays in motor development in high-risk infants, but its reliability and validity in Chinese infants have not been investigated. To examine the reliability and concurrent validity of AIMS in high-risk infants aged 0-9 months in China, this single-center study enrolled 50 high-risk infants aged 0-9 months (range, 0.17-9.27; average, 4.14±2.02), who were divided into two groups: 0-3 months (n=23) and 4-9 months (n=27). A physical therapist evaluated the infants with AIMS, with each evaluation video-recorded. To examine interrater reliability, two other evaluators calculated AIMS scores by observing the videos. To measure intrarater reliability, the two evaluators rescored AIMS after >1 month, using the videos. Concurrent validity was assessed by comparing results between AIMS and the Peabody Developmental Motor Scale-2 (PDMS-2). For all age groups analyzed (0-3, 4-9, and 0-9 months), intraclass correlation coefficients (ICCs) for AIMS total score were high for both intrarater comparisons (0.811-0.995) and interrater comparisons (0.982-0.997). AIMS total scores were well correlated with all PDMS-2 subtest scores (ICC=0.751-0.977 for reflexes, stationary, locomotion, grasping, and visual-motor integration subsets). However, the fifth percentile of AIMS total score was only moderately correlated with the gross motor quotient, fine motor quotient, and total motor quotient subtests of PDMS-2 (kappa=0.580, 0.601, and 0.724, respectively). AIMS has acceptable reliability and concurrent validity for screening of motor developmental delay in high-risk infants in China.

Abstract:Neurodevelopmental disorders (NDDs) are mostly diagnosed around the age of 4-5 years, which is too late considering that the brain is most susceptive to interventions during the first two years of life. Currently, diagnosis of NDDs is based on observed behaviors and symptoms, but identification of objective biomarkers would allow for earlier screening. In this longitudinal study, we investigated the relationship between repetition and change detection responses measured using an EEG oddball task during the first year of life and at two years of age, and cognitive abilities and adaptive functioning during preschool years (4 years old). Identification of early biomarkers is challenging given that there is a lot of variability in developmental courses among young infants. Therefore, the second aim of this study is to assess whether brain growth is a factor of interindividual variability that influences repetition and change detection responses. To obtain variability in brain growth beyond the normative range, infants with macrocephaly were included in our sample. Thus, 43 normocephalic children and 20 macrocephalic children were tested. Cognitive abilities at preschool age were assessed with the WPPSI-IV and adaptive functioning was measured with the ABAS-II. Time-frequency analyses were conducted on the EEG data. Results indicated that repetition and change detection responses in the first year of life predict adaptive functioning at 4 years of age, independently of head circumference. Moreover, our findings suggested that brain growth explains variability in neural responses mostly in the first years of life, so that macrocephalic children did not display repetition suppression responses, while normocephalic children did. This longitudinal study demonstrates that the first year of life is an important period for the early screening of children at risk of developing NDDs.

Abstract:As the development of technology in Neonatal Intensive Care Unit,the neurology outcome of premature has become a critical issue for their future life quality.Early electroencephalogram (EEG) test is a noninvasive and convenient method to monitor brain function.Nowadays,EEG monitoring has been used to evaluate the maturity of background activity,to detect subclinical seizures and to predict future neurological outcomes.The guideline of American clinical neurophysiology society in 2011,and the guideline of French clinical neurophysiology society in 2015 have made recommendations for the indication,timing and duration of EEG monitoring.Interpret EEG needs both skill and experience,it is hard for clinicians to master the skill in a short time.Hence,despite the advantages of EEG monitoring,the application of EEG among premature is limited.The amplitude-integrated EEG and other quantitative EEG monitoring method have become the latest research hotspot for evaluation of preterm brain function.In order to improve the recognition of EEG in the area of preterm neurologic evaluation,now the latest researches are reviewed.

Abstract:The aim of this study was to compare the psychometric properties of Alberta Infant Motor Scale (AIMS) with gross motor scale of Peabody Developmental Motor Scale II (PDMS-II) when they were used in high-risk infants. Fifty-eight high-risk infants with the average age 4.46 ± 0.97 months (from 2 to 6 months) were recruited to this study. Three investigators experienced in AIMS and PDMS-II participated in this study. Each investigator administered the AIMS and PDMS-II to the infants and recorded the assessment results. Forty-seven high-risk infants were followed-up till two years old. The diagnosis at two years old and the first scale assessment result were used to investigate the sensitivity, specificity, positive predictive value and negative predictive value of AIMS and PDMS-II. When used in high-risk infants in an early age, the AIMS's sensitivity was 0.857, specificity was 0.731, positive predictive value was 0.720, and negative predictive value was 0.864. For the PDMS-II, the sensitivity was 0.524, specificity was 1.000, positive predictive value was 1.000, and negative predictive value was 0.722. The AIMS is a sensitive monitor tool to evaluate the motor development of high-risk infants at very early age compared with PDMS-II. It needs to dynamic monitor for the high-risk infants.

Abstract:Pediatric professionals are facing two challenges when following up the preterm and young infants.Oneof the challenges is to identify those infants at high risk of neurodevelopmental disorders as early as possible and provide intervention for them.Another one is to identify the infants with normal developmental outcomes to avoid excessive diagnosis and intervention.In resent years,the general movements assessment has been widely used at home and abroad to predict the developmental outcomes of high risk infants with its high sensitivity and specificity,especially for cerebral palsy.It is suitable for application in the basic maternal and child health care institutions and the pediatric clinics at various levels in China.This paper describes the results of clinical studies of general movements assessment at home and abroad as well as its clinical experience in China.
