Abstract:Introduction Aortic Regurgitation(AR) is common in patients with continuous flow Left Ventricular Assist Devices(LVADs). The severity of AR can often be underestimated. Here we report the case of a 71-year-old male with end-stage ischemic cardiomyopathy on destination therapy with an LVAD found to have moderate continuous AR. Given the patient was not a candidate for a third sternotomy, we describe a case on the feasibility of Transcatheter Aortic Valve Replacement(TAVR) for the treatment of this common comorbidity in patients with LVADs. Case Presentation A 71-year-old male on destination therapy with LVAD presented for refractory dyspnea and fatigue with physical exam findings of rales and peripheral edema. Diuresis and increasing LVAD speed were attempted, however, failed to improve symptoms. Transesophageal Echocardiogram(TEE) demonstrated moderate AR on doppler imaging(Vena contracta 0.4cm, jet width 27% of LVOT) which was continuous through the cardiac cycle. Given this would be the patient's third sternotomy, the patient was prepared for TAVR. Hemodynamic evaluation demonstrated a high-to-normal cardiac index, Right Atrial Pressure of 23 mmHg, Pulmonary capillary wedge pressure of 21 mmHg, and a mean pulmonary artery pressure of 32 mmHg. Prior to TAVR, gated CT angiography, and 3D TEE were performed for planning and annular sizing. Perioperative hemodynamic assessment revealed equalization of aortic and ventricular diastolic pressures consistent with severe AR. A wire was subsequently looped in the apical gutter adjacent to the LVAD inflow cannula. After briefly slowing the LVAD to 2000rpm, minimizing the chance of valve diving during deployment. The valve was successfully deployed and post-implantation TEE(Figure) demonstrated a well-placed valve with no leakage, with improvement in the diastolic pressure with concurrent improvement in the patient's symptoms. Discussion This case demonstrates not only the feasibility of TAVR in LVAD patients with AR who are not surgical candidates but also the importance of both systolic as well as diastolic regurgitation in the complete assessment of disease severity in these patients. It was hypothesized that this continuous AR created a parallel circuit resulting in a shunt-like physiology leading to high output as well as right-sided heart failure. This was proven with the resolution of symptoms with the correction of the underlying continuous AR and interruption of the hypothesized shunt. Our case demonstrates the viability and effectiveness of TAVR in LVAD patients with continuous AR. We hope to promote further prospective studies to evaluate the use of TAVR in similar situations.
