Abstract:Skin resurfacing for cutaneous rejuvenation has evolved with the development of a plethora of nonsurgical and minimally invasive modalities. We have highlighted the advances in laser therapy, chemical peels, radiofrequency, microneedling, and platelet-rich plasma therapy. We have also included studies providing head-to-head comparisons between procedures and discussed relevant debates in the field. We have examined additionally combination treatments and resurfacing in acne scars, melasma, and skin of color.

Abstract:With increased demands for medical aesthetics procedures and the sudden profusion of newly licensed, and unlicensed, providers who are performing these medical aesthetics procedures also comes the responsibility to shift to collective competence. Collective competence refers to what occurs among professionals in action, emphasizing the sharing of experiences, knowledge, and perceptions among those who are providing services to the medical aesthetics client. Registered nurses and medical students are not taught to perform cosmetic procedures in basic nursing or medical programs and thus require a post-entry-level education to validate their competency. The current medical aesthetics apprenticeship training approach of see one, do one, and teach one focuses on teaching technical skills and thus does not sufficiently address the ever-changing health care context and the ambiguity in practitioner role. Recent scholars highlight that when health care failed or an error has been identified, it is rarely adduced to an individual's competence but rather is more likely to be a failure of the collective team. In this article, we are advocating for a change in how medical aesthetics practitioners are trained. In particular, it advocates creating opportunities within the curricula to train practitioners as a collective body, as opposed to providing training that focuses on the individual's competence and technical skills alone.
Aesthetic breast surgery: putting in context-a narrative review

Abstract:Objective: This review article summarises the latest evidence for commonly undertaken procedures in aesthetic breast surgery with a focus on key principles of breast augmentation, reduction, and mastopexy. The paper also outlines various approaches and controversies as well as complications such as breast implant associated anaplastic large cell lymphoma (BIA-ALCL) and breast implant illness (BII) which are increasingly being recognised and becoming a challenge to manage. Background: Changing trends of aesthetic breast surgery over the decades has warranted a continuous evolution of this field. The ability to deliver safe and appropriate care is dependent upon sound reconstructive principles and proper training. The lack of uniformity in either is a cause of concern. The impact of social media and changing perception of body image can also no longer be overlooked in the field of aesthetics and reconstruction. Methods: Review of literature including recent journals, textbook chapters, online databases like PubMed, and current government and surgical society guidelines. Conclusions: Breast reconstruction is based on sound surgical principles and it is imperative to follow these for the practice of this speciality. There are two important issues that revolve around this aspect of surgery. First relates to the urgent need to invest time and effort in improving regulations and outcomes in the cosmetic surgery industry. Second, it is crucial to promote and prioritize the development and training in this field as the principles of aesthetic breast surgery underpins oncoplastic breast surgery for management of cancer.

Abstract:Medical aesthetics is the use of a procedure or product for a therapeutic indication which is conventionally used for aesthetics. Several medical conditions are now being treated with products, procedures or equipment that are conventionally used for aesthetic indications. This has widened the scope of treatment modalities available for dermatologists to treat various indications that fall outside the purview of aesthetic dermatology. The authors present aesthetic treatment modalities and procedures which can be used for medical aesthetics, their present-day status and usefulness in field of therapeutics with a review of published literature from "Medline" (via "PubMed"), "Cochrane," the Virtual Health Library, and Google Scholar.

Abstract:Abstract The aesthetic injectables and nonsurgical facial aesthetics industry, marked by its rapid growth, faces critical challenges concerning regulatory standards and ethical conduct. This manuscript contributes to the discourse on these issues through perspectives from professionals across disciplines captured in a survey. This study explores the multifaceted concerns related to the lack of stringent regulations and their impact on patient safety, practitioner credibility, and field integrity, and therefore aims to foster informed discussions, encourage responsible practices, and advocate for a robust framework ensuring the wellbeing of both practitioners and patients. Consensus surveys were distributed at a conference in 2023 attended by registered healthcare professionals (RHPs); students, including medical, dental and nursing students; and other allied healthcare professionals working in the aesthetics industry. The surveys were designed using Google Forms and featured multiple-choice questions assessing regulatory standards and professional ethics in nonsurgical facial aesthetics. Responses were anonymized, aggregated and analysed using Microsoft Excel. Consent was obtained from participants and no patients were involved. This study involved 82 participants encompassing various roles within the healthcare industry. Of all the participants, 67% were employed, of whom 67% were working within the NHS. A high percentage (99%) of respondents agreed on the need for a minimum educational standard for RHPs practising injectable aesthetics. Opinions were divided on healthcare practitioners overseeing and prescribing for nonhealthcare practitioners in aesthetics, with 68% of participants disagreeing and 30% agreeing. There was strong support (88%) for specialist training in cosmetic injectables to involve a faculty of trainers with expertise across different areas, and almost all respondents (96%) believed that those teaching cosmetic injectables should possess high-level certified expertise. In the realm of social media, a significant majority (90%) deemed it acceptable for RHPs to use these platforms for advertising. A substantial majority (90%) agreed that it is acceptable for RHPs to use social media to advertise their nonsurgical facial aesthetics qualifications, private practice or skills. In conclusion, there is a clear consensus on the importance of setting minimum educational standards for healthcare professionals in injectable aesthetics. The majority of respondents believe in stringent regulations for those overseeing and assisting nonhealthcare practitioners in aesthetics. Interestingly, social media is seen as an acceptable platform for healthcare practitioners to showcase their qualifications and skills in nonsurgical facial aesthetics. Strong support exists for the protection of professional titles, emphasizing the importance of qualifications and training in the field of cosmetic and aesthetic medicine.

Abstract:Skin of color in dermatology encompasses individuals of various ethnic backgrounds including Black or those of African descent, Hispanic or Latino, Asian, Native American, Pacific Islander, and those of mixed ethnicities. Because these populations continue to expand, more patients of color (POC) are seeking out cosmetic enhancements and treatments. Aside from cosmeceuticals, nonsurgical cosmetic rejuvenation options, such laser and light-based treatments, neurotoxins, soft tissue augmentation, and more recently body contouring and skin tightening, are becoming increasingly popular worldwide. This article examines risks of cosmetic enhancement procedures in POC and best practices to prevent adverse events.

Abstract:The future promises more increasingly sophisticated formulations for cosmetics and skin care products. The trend toward therapeutic cosmetics is sure to result in the need for the dermatologist to obtain a better understanding of modern ingredients and assessment techniques. Examples of products presently on the market representing new technology or formulation advances are listed in Table 3. New challenges will also be presented to government regulatory agencies as more chemicals with true biological activity are invented and tested. Claim substantiation and pre-marketing testing must also evolve to accurately assess efficacy and safety issues with important implications for total body health. Finally, new vehicles and delivery systems combined with established ingredients will alter percutaneous absorption, requiring reevaluation of substances with an assumed good safety profile. In the end, this new research will result in cosmetics and skin care products of superior quality and efficacy to offer patients.

Abstract:Objective To summarize and update the terminology of the lip and perioral morphological aesthetic subunits and aesthetic landmarks in the domestic medical aesthetics field. Methods 36 English literatures, 13 Chinese literatures, 3 foreign academic compositions, 2 domestic academic compositions, 1 domestic medical aesthetics textbook, and 14 websites of the medical aesthetic field have been included. We summarized the commonly seen terminology of the lip and perioral morphological aesthetic subunits and aesthetic landmarks. Moreover, for the aesthetic subunits and aesthetic landmarks which are commonly used in clinical practice but have not yet been named in Chinese term are named following the current Chinese terminology rule. We summarized not only the terminology of lips but also the anatomical acknowledge of the commonly used aesthetic subunits. Results There are 17 standard nomenclatures for aesthetic subunits, 8 standard nomenclatures with abbreviations for aesthetic surface landmarks,20 standard terms for aesthetic measurement and distance. Moreover, we named 5 aesthetic subunits and 6 aesthetic surface landmarks for which there are no standard Chinese medical terms. Conclusions The standard terminology for the aesthetic subunits and landmarks is the foundation for the lip and perioral measurement and evaluation. Besides, it also provides a reference basis for the development of lip cosmetic and perioral rejuvenation treatment programs.
Transatlantic Innovations: a new approach to international ideas and technology

Abstract:Background: In April of this past year, Transatlantic Innovations brought a variety of organizations and industry together for an international exchange of ideas, new technology, and current trends in plastic surgery. Methods: The meeting was highly interactive and included audience response devices. The focus was on 10 major areas: (1) new surgical techniques; (2) composite allografts versus conventional techniques, facing the future; (3) interspecialty collaboration versus competition; (4) the business of plastic surgery, best practices; (5) the image of the plastic surgeon, branding yourself; (6) medical tourism; (7) publicity; (8) the regulation of innovation, U.S. Food and Drug Administration and European Medicines Agency perspective; (9) the future of plastic surgery, cutting edge technologies; and (10) applications and controversies in fat grafting. The meeting concluded with the 8th International Committee for Quality Assurance, Medical Technologies and Devices in Plastic Surgery Consensus Conference with the development of a consensus statement. Results: Through an interactive audience response system, additional questions and attitudes were asked of the audience and, in real time, international differences were identified, which led to further discussions from panelists. Responses were identified in three major groups: European Union, North America, and the Rest of the World. Responses and data are included in this article. Conclusions: The meeting brought participants, industry, regulators, and educators from both sides of the Atlantic. The interaction of these groups in these outlined topics brought a unique perspective to the meeting and, in the end, volumes of data. We have more in common than we believe. It is our anticipation that as we as plastic surgeons move forward, we can use these interactions to help our own practices but more specifically the specialty as a whole.

Abstract:Background: Adipose tissue, which can be readily harvested via a number of liposuction techniques, offers an easily accessible and abundant source of adipose-derived stem cells (ASCs). Consequently, ASCs have become an increasingly popular reconstructive option and a novel means of aesthetic soft tissue augmentation. Objectives: This paper examines recent advances in the aesthetic surgery field, extending beyond traditional review formats to incorporate a comprehensive analysis of current clinical trials, adoption status, and the commercialization pathway. Methods: Keyword searches were carried out on clinical trial databases to search for trials using ASCs for aesthetic indications. An intellectual property landscape was created using commercial software (Thomson Reuters Thomson Innovation, New York, NY). Analysis of who is claiming what in respect of ASC use in aesthetic surgery for commercial purposes was analyzed by reviewing the patent landscape in relation to these techniques. Key international regulatory guidelines were also summarized. Results: Completed clinical trials lacked robust controls, employed small sample sizes, and lacked long-term follow-up data. Ongoing clinical trials still do not address such issues. In recent years, claims to intellectual property ownership have increased in the "aesthetic stem cell" domain, reflecting commercial interest in the area. However, significant translational barriers remain including regulatory challenges and ethical considerations. Conclusions: Further rigorous randomized controlled trials are required to delineate long-term clinical efficacy and safety. Providers should consider the introduction of patient reported outcome metrics to facilitate clinical adoption. Robust regulatory and ethical policies concerning stem cells and aesthetic surgery should be devised to discourage further growth of "stem cell tourism."

Abstract:Our increased knowledge of normal skin physiology has ushered in a subtle revolution in cosmetic science. Originally designed as preparations to enhance personal appearance by direct application on to the skin, cosmetics have now taken on a new role in dermatology, through the support of the management of many skin disorders. This evolving role of cosmetics in skin care is primarily due to scientific and technological advancements that have changed our understanding of normal skin physiology and how cosmetics modify its appearance both physically and biologically. The vast array of techniques currently available to investigate skin responsivity to multiple stimuli has brought about a new era in cosmetic and dermocosmetic development based on a robust understanding of skin physiology and its varied responses to commonly encountered environmental insults. Most cosmetic research is undertaken on reconstructed skin models crucial in dermatological research, given the strict ban imposed by the European Union on animal testing. In addition, the design and conduct of trials evaluating cosmetics now follow rules comparable to those used in the development and evaluation of pharmaceutical products. Cosmetic research should now aim to ensure all trials adhere to strictly reproducible and scientifically sound methodologies. The objective of this review is to provide an overview of the multidisciplinary scientific approach used in formulating dermocosmetics, and to examine the major advances in dermocosmetic development and assessment, the safety and regulatory guidelines governing their production and the exciting future outlook for these dermocosmetic processes following good practice rules.
