Abstract:Objective To study the clinical significance of measurement of serum cystatin C(Cysc),lipoprotein α(LPα) and high sensitivity C-reactive protein(hs-CRP) in type 2 diabetic mellitus(T2DM)patients with early renal impairment.Methods One hundred and twenty six patients were divided into three groups: 40 cases in simple diabetic group(DM),44 cases in early diabetic nephropathy group(EDN) and 42 cases in clinical diabetic nephropathy group(DN),and 70 healthy persons were chosen as normal control group.Serum levels of Cysc,LP(α) and hs-CRP were detected by BECKMAN UniCel Dxc600 automatic biochemistry analyzer.Results Serum levels of Cysc,LP(α)and hs-CRP in T2DM patients were higher than those of normal control group(P0.01),they were also significantly higher in DN group than in DM group and EDN group(P0.01).The serum levels of Cysc,LP(α)and hs-CRP were positively correlated with urinary mALB in T2DM patients.Conclusion Serum levels of Cysc,LP(α)and hs-CRP are sensitive indicators for detection of early renal impairment in patients with type 2 diabetes mellitus.
Study on the Clinical Significance of Serum Cystatin C in Different Stage of Renal Impairment in Type 2 Diabetic Patients

Abstract:Objective To investigate the difference of serum cystatin C(CysC) in different stage of renal impairment in type 2 diabetic patients and compare the relationship of cystatin C with glomerular filtration rate and UAER.Methods The serum levels of cystatin C was detected by particle-enhanced immunonephelometry in 102 diabetic patients .(including 66 diabetic nephropathy) and the glomerular filtration rate and UAER was detected at the same time.Results The difference among the group of normal albuminuria,the group of microalbuminuria and the group of macro-albuminuria was significant(P0.01).In the group of microalbuminuria and the group of macro-albuminuria has the serum cystatin C significant correlation with the glomerular filtration rate(P0.05).Conclusions Serum cystatin C can be used as a marker in valuation of glomerular filtration function .It is helpful for early diagnosis of diabetic nephropathy.

Serum creatinine level is the most commonly used biochemical parameter to estimate GFR in routine practice. However, 50% of GFR can be lost before significant elevation of serum creatinine. Cystatin C is found to be a new promising marker for early detection of renal diseases. Objective of the Study: The aim of this study was to determine the value of serum cystatin C and serum creatinine levels for early detection of renal disease in patients with type 2 diabetes mellitus. Methodology: A hospital-based comparative cross-sectional study was conducted with a sample size of 120. For early detection of renal disease in patients with type 2 diabetes mellitus, serum creatinine and cystatin C levels were measured and compared. Result and Discussion: Serum creatinine and cystatin C levels were significantly increased in patients with type 2 diabetes mellitus compared to healthy controls. The mean±SD value of serum creatinine was found to be 0.87± 0.44 mg/dL in patients and 0.63± 0.27 mg/dL in control. Serum cystatin C level was also found to be significantly ( P =0.0001) higher in patients (0.92± 0.38 mg/L) compared to controls (0.52± 0.20 mg/L). The mean±SD of eGFR in three equations (Creatinine Equation, Cystatin C Equation, and Creatinine–Cystatin C Equation) were 105.7± 27.5 mL/min/m 2 , 90.4± 28.2 mL/min/m 2 , and 100± 29.5 mL/min/m 2 , respectively. Conclusion: Cystatin C-based GFR estimation equations detect renal impairment in patients with type 2 diabetes mellitus earlier than creatinine-based GFR estimation equations. Correlation of cystatin C and diabetic retinopathy in type 2 diabetic patients

Abstract:[Summary] Serum cystatin C levels were measured by microparticle enhanced turbidimetry in 114 type 2diabetic patients without retinopathy (group A) and 105 type 2 diabetic patients with retinopathy (group B),as well as 47 normal healthy individuals as controls (group NC).The results showed that the serum cystatin C level of group B was much higher than those of group NC and group A (P＜0.05).However,there was no difference in serum cystatin C level between group NC and group A.Multivariate logistic regression analysis revealed that serum levels of cystatin C and cholesterol,and systemic blood pressure were independent risk factors of diabetic retinopathy in type 2 diabetic patients (all P＜0.05),suggesting that detection of serum cystatin C level may play an important role in early diagnosis and prevention of diabetic retinopathy.

Abstract:Objective: To determine the diagnostic accuracy and cut-off values of serum cystatin C as early diagnostic biomarker of diabetic kidney disease. Study design: Cross-sectional analytical study. Place and duration of study: Department of Pathology, Army Medical College, Rawalpindi in collaboration with Endocrinology Department, Military Hospital (MH), Rawalpindi from November 2015 to November 2016. Methodology: One hundred and nineteen diagnosed patients of type 2 diabetes mellitus were enrolled in the study from the outpatient Endocrinology Department of the MH Rawalpindi. Fifty disease-free controls were also included. Fasting blood samples of the patients and controls were analysed for creatinine by Jaffé's kinetic method and estimated GFR was calculated using MDRD-based equation for GFR. Serum cystatin C was estimated by quantitative turbidimetric method. Results: Serum cystatin C was higher in the diabetic group (mean = 1.022 ±0.33 mg/dl) as compared to the control group (mean = 0.63 ±0.14 mg/dl). ROC curve analysis, keeping less than 60 ml/min/1.73 m2 GFR (CKD-MDRD based) as reference value of the stat variable/gold standard; revealed an area under the curve of 0.914 (95% CI 0.85-0.98) and at optimal sensitivity of 88.2% and specificity of 84.8% the established cut-off of serum cystatin C was 1.26 mg/L. Conclusion: Cystatin C is an accurate biomarker of diabetic kidney disease with good sensitivity and specificity.

Abstract:Chronic kidney disease (CKD) is a microvascular complication that frequently affects numerous patients diagnosed with diabetes. For the diagnosis of CKD, the guidelines recommend the identification of the urinary albumin/creatinine ratio and the determination of serum creatinine, based on which the estimated rate of glomerular filtration (eGFR) is calculated. Serum creatinine is routinely measured in clinical practice and reported as creatinine-based estimated glomerular filtration rate (eGFRcr). It has enormous importance in numerous clinical decisions, including the detection and management of CKD, the interpretation of symptoms potentially related to this pathology and the determination of drug dosage. The equations based on cystatin C involve smaller differences between race groups compared to GFR estimates based solely on creatinine. The cystatin C-based estimated glomerular filtration rate (eGFRcys) or its combination with creatinine (eGFRcr-cys) are suggested as confirmatory tests in cases where creatinine is known to be less precise or where a more valid GFR estimate is necessary for medical decisions. Serum creatinine is influenced by numerous factors: age, gender, race, muscle mass, high-protein diet, including protein supplements, and the use of medications that decrease tubular creatinine excretion (H2 blockers, trimethoprim, fenofibrate, ritonavir, and other HIV drugs). The low levels of creatinine stemming from a vegetarian diet, limb amputation, and conditions associated with sarcopenia such as cirrhosis, malnutrition, and malignancies may lead to inaccurately lower eGFRcr values. Therefore, determining the GFR based on serum creatinine is not very precise. This review aims to identify a new perspective in monitoring renal function, considering the disadvantages of determining the GFR based exclusively on serum creatinine.
Serum cystatin C is associated with subclinical atherosclerosis in patients with type 2 diabetes: A retrospective study

Abstract:Background: The aim of this study was to investigate the association between the serum cystatin C level and cardiovascular disease risk in patients with type 2 diabetes mellitus. Methods: We studied 523 patients with type 2 diabetes mellitus and calculated estimated 10-year risk of atherosclerotic cardiovascular disease (%). Subclinical atherosclerosis was defined as brachial-ankle pulse wave velocity ⩾1700 ms, indicating the presence of arterial stiffness. Results: Cystatin C level was significantly higher in the subclinical atherosclerosis group (brachial-ankle pulse wave velocity ⩾ 1700 ms) than in the non-subclinical atherosclerosis group (brachial-ankle pulse wave velocity < 1700 ms) (7.54 ± 3.15 mg/L vs 10.04 ± 5.12 mg/L, p < 0.001). Subclinical atherosclerosis was mainly determined by age, duration of diabetes and cystatin C level, but not by serum creatinine, 10-year risk of atherosclerotic cardiovascular disease score and estimated glomerular filtration rate in the multiple linear regression analysis. In addition, an increase in cystatin C level was independently associated with the risk of subclinical atherosclerosis after adjusting for age, sex, duration of diabetes, smoking, hypertension, 10-year risk of atherosclerotic cardiovascular disease risk score, serum creatinine level, total cholesterol, high-density lipoprotein cholesterol and haemoglobin A1c (odds ratio = 1.200, 95% confidence interval: 1.04-1.38, p = 0.011). Conclusion: Serum cystatin C level was significantly associated with subclinical atherosclerosis. This result suggests that an increase in cystatin C level could be a valuable surrogate marker for the risk of cardiovascular disease in patients with type 2 diabetes mellitus.

Abstract:Cystatin C has been shown to be a reliable and accurate marker of kidney function across diverse populations. The 2012 Kidney Disease Improving Global Outcomes (KDIGO) guidelines recommended using cystatin C to confirm the diagnosis of chronic kidney disease (CKD) determined by creatinine-based estimated glomerular filtration rate (eGFR) and to estimate kidney function when accurate eGFR estimates are needed for clinical decision-making. In the efforts to remove race from eGFR calculations in the United States, the National Kidney Foundation (NKF) and American Society of Nephrology (ASN) Joint Task Force recommended increasing availability and clinical adoption of cystatin C to assess kidney function. This review summarizes the key advantages and limitations of cystatin C use in clinical practice. Our goals were to review and discuss the literature on cystatin C; understand the evidence behind the recommendations for its use as a marker of kidney function to diagnose CKD and risk stratify patients for adverse outcomes; discuss the challenges of its use in clinical practice; and guide clinicians on its interpretation.
Cystatin C in Prediction of Acute Kidney Injury: A Systemic Review and Meta-analysis

Abstract:Background: Cystatin C (CysC) has been proposed as a filtration marker for the early detection of acute kidney injury (AKI); however, a wide range of its predictive accuracy has been reported.Study Design: Meta-analysis of diagnostic test studies.Setting & Population: Various clinical settings of AKI, including patients after cardiac surgery, pediatric patients, and critically ill patients.Selection Criteria: Computerized search of PubMed, Current Contents, CINAHL, and EMBASE from inception until November 15, 2010, was performed to identify potentially relevant articles. Inclusion criteria were studies investigating the diagnostic accuracy of CysC level to predict AKI. There were no language restrictions in the search.Index Tests: Increasing or increased serum CysC level or urinary CysC excretion.Reference Tests: The outcome was the development of AKI, primarily based on serum creatinine level (definition varied across studies).Results: We analyzed data from 19 studies and 11 countries involving 3,336 patients. Of these studies, 13 could be included in the meta-analysis. Across all settings, the diagnostic OR for serum CysC level to predict AKI was 23.5 (95% CI, 14.2-38.9), with sensitivity and specificity of 0.84 and 0.82, respectively. The area under the receiver operating characteristic curve (AUROC) of serum CysC level to predict AKI was 0.96 (95% CI, 0.95-0.97). Subgroup analysis showed that serum CysC was of diagnostic value when measured early (within 24 hours after renal insult or intensive care unit admission). For the diagnostic value of urinary CysC excretion, the diagnostic OR was 2.60 (95% CI, 2.01-3.35), with sensitivity and specificity of 0.52 and 0.70, respectively. The AUROC of urinary CysC excretion to predict AKI was 0.64 (95% CI, 0.62-0.66).Limitations: Variation in criteria for definitions of index and reference tests, absence of measured glomerular filtration rate in most studies.Conclusion: Serum CysC appears to be a good biomarker in the prediction of AKI, whereas urinary CysC excretion has only moderate diagnostic value. Abstract:This study was designed to evaluate the usefulness of serum cystatin C (CysC) as a marker of renal function in 83 patients with diabetic nephropathy, considering multiple factors including tubular function and body mass index. Serum CysC was assayed using a particle-enhanced nephelometric immunoassay and the glomerular filtration rate (GFR) was obtained by measuring the plasma disappearance of the isotope (99m)Tc-diethylenetriamine penta-acetic acid. By comparing the correlation of CysC and serum creatinine (SCr) with GFR, it was concluded that CysC may be a better indicator of GFR than SCr in diabetic patients, in both the early hypertransfusion stage and in the late renal dysfunction stage. CysC showed a slightly higher sensitivity for renal function evaluation than SCr in patients with renal tubular dysfunction and moderate to severe proteinuria. In addition, CysC was not affected by the metabolic index. Thus, CysC may serve as an ideal endogenous marker of GFR in patients with diabetic nephropathy.
Evaluation of Renal Function in Patients with Diabetic Nephropathy Using Cystatin C

Abstract:Objective:To evaluate the Cystatin C(Cys-C)as a marker of renal function in patients with diabetic nephropathy(DN).Methodology:Eight three patients with DN were enrolled in this study.Serum Cys-C was tested by immunoturbidimetry,and serum creatinine(SCr)was measured at the same day.Radionuclide 99mTc-DTPA was used to evaluate glomerular filtration rate(ECT-GFR).The tubular markers(NAG enzyme,retinol binding protein,urine osmotic pressure),24-hour urine proteinuria excretion,serum albumin,and uric acid were also measured.The correlation between Cys-C,SCr,MDRD-GFR and ECT-GFR was evaluated in the different renal stages.Further more,the correlation of Cys-C and GFR was detected in different level of tubular function,proteinuria,serum albumin,uric acid and BMI.Receiver operator characteristic curve(ROC curve)of Cys-C and SCr was made by ECT-GFR as standard,and calculated the area under the curve(AUC).Results:(1)In the whole study population:the Pearson correlation coefficient of Cys-C,SCr and ECT-GFR was-0.740,-0.663,respectively(P0.01).Cys-C correlated with GFR better than SCr.When GFR was less than 60ml/min,the correlation coefficient of Cys-C,MDRD-GFR,SCr and GFR was-0.798,0.760,-0.716,respectively(P0.01).Cys-C correlated with GFR better than SCr or MDRD formula.When GFR was more than 90 ml/min,the correlation coefficient of Cys-C,SCr and GFR was-0.561,-0.465,respectively(P0.05).Cys-C was also correlated with GFR better than SCr.(2)In the patients with tubular impairment:when NAG enzyme was more than 20 u/g·cr,the correlation coefficient of Cys-C,SCr and GFR was-0.660,-0.595,respectively(P0.01);when RBP was more than 2 mg/L,the correlation coefficient was-0.672,-0.635,respectively(P0.01);and when Urine osmolarity was less than 600 mOsm/kg·H_2O,the correlation coefficient was-0.696,-0.663,respectively(P0.01).Cys-C was alsocorrelated with GFR better than SCr when tubular damage occurred.(3)When proteinuria was more than 1 g/24hr,the correlation coefficient of Cys-C,SCr and GFR was-0.704,0.649,respectively(P0.01).When the concentration of serum albumin was less than 35g/L,the correlation coefficient of Cys-C,SCr and GFR was-0.635,0.6581,respectively(P0.01).Cys-C was also correlated with GFR better than SCr in cases with heavy proteinuria.(4)In different levels of serum uric acid or BMI,the correlation was not affected.In the ROC,the AUC of Cys-C and SCr was 0.915 vs 0.902,respectively(P0.01)(GFR 90ml/min as cut off value),Cys-C was more sensitive and specific.Conclusion:Cys-C may be correlated with GFR better than SCr in type 2 diabetic patients with different renal stages,including early hypertransfusion period or overt renal dysfunction state.In patients with renal tubular dysfunction or medium to massive urine protein,renal function could be evaluated by Cys-C better.Cys-C is not affected by metabolic index.Thus,Cys-C could be an ideal endogenous marker of GFR in DN patients.
Cystatin C: A Strong Marker For Lower Limb Ischemia In Chinese Type 2 Diabetic Patients?

Abstract:Objective: Cystatin C is growing to be an ideal indicator for renal function and cardiovascular events. The aim of this study was to investigate the relationship between serum cystatin C levels and peripheral arterial disease and to explore its diagnostic value for lower limb ischemia (LLI) in type 2 diabetic population.Methods: A total of 1609 T2DM patients were included in this cross-sectional study. Their clinical and biochemical characteristics, ankle-brachial index (ABI), carotid and lower extremity arterial ultrasound were detected. LLI was defined by ABI <0.9 and lower extremity arterial stenosis >50% by ultrasound examination. Patients were divided to two groups: with LLI and without. The risk factors of LLI were explored by binary logistic regression analysis.Results: The serum concentrations of cystatin C were 1.53 +/- 0.60 and 1.08 +/- 0.30 mg/L in patients with and without LLI, respectively. Binary logistic regression analysis showed that the significant risk factors were cystatin C (P = 0.007, OR = 5.081), the presence of hypertension (P = 0.011, OR = 3.527), age (P<0.001, OR = 1.181), GA (P = 0.002, OR = 1.089) and diabetes duration (P = 0.008, OR = 1.074). The prevalence of coronary artery disease, cerebral infarction and LLI increased with cystatin C (P<0.01), and the prevalence of LLI in patients with cystatin C>1.2 mg/L was much higher than other three quartile groups. Receiver operating characteristic curve analysis revealed the cut point of cystatin C for LLI was 1.2 mg/L. The risk of LLI dramatically increased in patients with cystatin C>1.2 mg/L (OR = 21.793, 95% confidence interval 10.046-47.280, P<0.001). After adjusting for sex, age, duration, HbA1c, GA and hypertension, its OR still remained 3.395 (95% confidence interval 1.335-8.634).Conclusions: There was a strong and independent association between cystatin C and limb arterial disease in diabetic population, and cystatin C>1.2 mg/L indicated a great increased risk of LLI.

Abstract:Aims To investigate the relationship between retinal vascular parameters and cystatin C in patients with type 2 diabetes in northwestern China. Methods This was a cross-sectional study of 1689 patients with type 2 diabetes. A validated fully automated computer program was used to extract retinal vascular parameters from the entire vascular tree. Multiple logistic regression analyses were conducted to investigate the relationship between these vascular measurements and cystatin C. Results For retinal vascular geometrical measurements, smaller arteriolar fractal dimension was related to high cystatin C after adjusting for multiple variables (odds ratio [OR] 0.149, 95% CI 0.042–0.532). For retinal vascular caliber measurements, narrower central and middle arteriolar calibers were related to high cystatin C after adjusting for multiple variables (central: OR 0.922, 95% CI 0.886–0.960; middle: OR 0.940, 95% CI 0.901–0.981). Wider central, middle and peripheral venular calibers were associated with high cystatin C after adjusting for multiple variables (central: OR 1.058, 95% CI 1.003–1.117; middle: OR 1.094, 95% CI 1.040–1.150; peripheral: OR 1.075, 95% CI 1.023–1.130). Conclusions Multiple retinal vascular geometrical and caliber measurements are associated with cystatin C in type 2 diabetic patients. Further studies are needed to explore whether these retinal vascular changes can predict the incidence and progress of diabetic nephropathy.

Abstract:Purpose of review In this report, we summarize why the availability of cystatin C is important across a variety of clinical scenarios, the recent literature on when, why and in whom cystatin C testing should be considered, and how nephrologists can take practical steps to incorporate cystatin C testing into their practice. Recent findings Large intra-individual discrepancies between estimated glomerular filtration rate by creatinine (eGFRcr) and estimated glomerular filtration rate by creatinine eGFRcys (known as eGFRdiff) are observed in at least 1 in 4 people. These differences are seen more commonly among more vulnerable individuals: older adults, females, non-White individuals and those living with multiple medical conditions. A large eGFRdiff, where eGFRcys is lower than eGFRcr, is associated with a plethora of adverse outcomes, including medication-associated adverse events, acute kidney injury, cardiovascular disease, kidney failure and all-cause mortality. Among studies that have measured GFR, eGFRcr-cys usually provides the most accurate estimation of kidney function compared to mGFR, including among participants with large discrepancies between eGFRcr and eGFRcys. Summary Cystatin C improves sensitivity and specificity of chronic kidney disease diagnosis, improves detection of harmful acute and chronic changes in kidney function, improves precision of treatment eligibility and safety, and may reduce healthcare inequalities. Better education, curiosity, and motivation among nephrologists could substantially improve the availability and utilization of cystatin C.

Abstract:Introduction : Simple renal cysts (SRCs) are the most common acquired cystic kidney disease, but the relationship between SRCs and renal function has not been clarified in patients with type 2 diabetes mellitus (T2DM). Methods : A retrospective study was conducted to analyze the clinical features of renal cysts and ultrasound data of the kidney in 4,304 patients with T2DM. Results : The prevalence of SRCs in patients with T2DM was 21.1%. Compared to patients with no SRCs, patients with SRCs had worse renal function (estimated glomerular filtration rate: 108.65 ± 40.93 vs. 92.38 ± 42.1 ml/min/1.73 m 2 , p < 0.05). After adjusting the confounders, SRC was related to estimated glomerular filtration rate in patients with T2DM [odds ratio = 1.49, 95% confidence interval (1.24, 1.79), p < 0.01]. Age, gout, proteinuria, cerebrovascular disease (CVD), and increased serum phosphorus levels were associated with SRCs in patients with T2DM. Conclusion : SRCs are associated with worse renal function in patients with T2DM. More attention should be paid to gout, proteinuria, CVD, serum phosphorus levels, and renal function in T2DM patients with SRCs.

Abstract:BACKGROUND: Diabetic nephropathy (ND) is one of the main complications of chronic microvascular diabetes mellitus (DM) and the leading cause of the end-stage renal disease (ESRD), accounting for nearly half of all ND cases with ESRD incidence in developed countries. Recently, cystatin C serum has been considered as a new biomarker for the diagnosis of kidney damage. Cystatin-C is an appropriate marker for GFR measurement because it is not affected by age, weight, gender, and protein intake. Early detection of abnormal renal function is essential to slow progression to a further stage of nephropathy or the final stage of kidney disease. AIM: The aim of the study was to analyze the correlation between cystatin-C and ACR to early detection of nephropathy complications. MATERIALS AND METHODS: This study uses an observational analytical research design with a cross-sectional approach. The research was conducted on patients with type 2 diabetes mellitus (T2DM) who were visiting the primary health service in Medan that met the inclusion and exclusion criteria. The sample count was 89 respondents. The sampling method is done through consecutive sampling. The source of this research data is primary data, including the results of cystatin-c examination and albumin. Data analysis used Spearman correlation using SPSS for Windows software. RESULTS: There was a significant relationship between cystatin-C levels and albuminuria (p<0.05). CONCLUSION: Diabetic nephropathy is a clinical syndrome characterized by persistent albuminuria and progressive decline of renal function, cystatin-C and albuminuria levels can be used as early detection of nephropathy complications.

Abstract:Objective To evaluate the value of serum creatinine(Scr),creatinine clearance (Ccr),MDRD equation 7(MDRD 7),~(99)Tc~m-DTPA renal dynamic imaging(gGFR)and cystatin C in screening changed glomerular filtration function in type 2 diabetic patients.Methods The ~(99)Tc~m-Diethylene Triamine Pentaacetic Acid(~(99)Tc~m-DTPA)plasma clearance(rGFR)obtained with the dual plasma sampling method was used as a reference with which Scr,Ccr,MDRD 7,gGFR and Cystatin C were compared.Results Sixty of type 2 diabetic patients were selected,including 35 male and 25 female.The average age was(62.4±11.7)years and the average diabetic history was(10.66±9.35)years.Scr,Ccr, MDRD 7,gGFR were all correlated significantly with rGFR.Correlation coefficients were 0.675 for Ccr, -0.588 for Cystatin C,-0.500 for Scr,O.428 for MDRD 7,0.367 for gGFR(P values all <0.001). The sensitivities of Ccr,MDRD 7 and Cystatin C were higher than that of Scr and gGFR;the specificity of Scr was 100%.The areas under the receiver operating characteristics curve(ROC)of Cystatin C and Ccr were significantly higher than that of Scr,7GFR and gGFR.ROC curves showed an optimum cut off of 1.18 mg/L for Cystatin C and the sensitivity and specificity were 80% and 57.5%.The optimum cut off for Ccr was 62 ml/min and the sensitivity and specificity were 90% and 67.9%.Conclusion Ccr and Cystatin C are better markers to reflect the changes of glomerular filtration rate in type 2 diabetic patients.

Abstract:Background: One of the most well-known diabetic microvascular consequences is diabetic nephropathy, which affects 40 percent of people with diabetes mellitus type 2. It develops into end stage renal disease, and diabetes biomarkers can be used as a predictor. Objectives: This study aimed to determine the concentration levels of serum Beta Trace Protein and serum Cystatin C in all stages of diabetic nephropathy disease. Subjects And Methods: A Case- control study included 120 Persons (30-persons appeared healthy as control and 90 patients proved with T2DM of both gender (64 males and 56 females), split into four groups by using urinary micro-albumin and albumin to creatinine ratio (UACR) as following: group-I: includes 30persons as a healthy control (UACR < 30 mg/g creatinine). Group-II: includes 30patients with type2 DM normoalbuminuria (UACR < 30 mg/g creatinine) as a positive control. Group-III: includes 30 patients with type 2 DM microalbuminuria (UACR 30 – 300 mg/g creatinine). Group-IV: includes 30 patientsا with type 2 DM macroalbuminuria (UACR˃ 300 mg/g creatinine), all groups with ages ranges between (40-69) years. in allا groups, Beta Trace Protein and Cystatin C quantitative enzyme immunoassay (double-antibody sandwich), calculated in serum and both biomarkers using the same methods. Results: Patients with diabetes have statistically substantially increased blood levels of Beta trace protein and Cystatin C. with macro-albuminuria groups in comparison to micro-albuminuria, normo-albuminuria and healthy control in addition, the mean for the micro-albuminuria group was greater than for the healthy control and normo-albuminuria groups. Conclusion: A rise in blood Beta trace protein and Cystatin C levels in the early nephropathy group microalbuminuria, which may be used as a predictor for early diagnosis of diabetic nephropathy.
