Learn more.Copy URL Share a linkShare onFacebookTwitterLinkedInRedditWechat To the Editor: Until now, 13 RARA fusion variants, including ZBTB16, NPM1, NUMA1, STAT5B, PRKAR1A, FIP1L1, BCOR, NABP1, TBLR1, GTF2I, IRF2BP2, FNDC3B and STAT3, have been reported in acute promyelocytic leukemia (APL).1 Here, we report another novel NUP98-RARA fusion in an APL patient lacking the t(15,17)(q22;q12)/PML-RARA fusion, which represents the 14th variant of APL. A 31-year-old man was admitted due to having a fever and feeling weak, and his blood count showed a white blood cell count of 2.4 × 109/L, including 29.5% promyelocytes, a hemoglobin level of 124 g/L, and a platelet count of 123 × 109/L. His prothrombin time, fibrinogen, fibrin degradation products and D-dimer levels were 16.3 seconds (ref.: 9.4-12.5 seconds), 85 mg/dL (ref.: 200-400 mg/dL), 82.4 μg/mL (ref.: 0-5 μg/mL) and 19 690 ng/mL (ref.: 0-243 ng/mL), respectively. His activated partial thromboplastin time was within the normal range. A bone marrow (BM) smear showed hypercellularity, with 78.4% hypergranular promyelocytes and 9.8% blasts (Figure 1A, top). The BM histochemistry showed a typical pattern for APL (Figure 1A, bottom). The leukemia cells expressed myeloperoxidase, CD117, CD123, CD33, CD13, CD9, CD64, and HLA-DR and partially expressed CD34. However, these cells lacked the expression of CD11b, CD38, CD14, CD15, CD16, CD19, cCD79a, CD7, cCD3, and TdT. The presumptive diagnosis of this patient was the microgranular variant of APL. There were few Auer Rods and they are occasionally seen in a few APL cells. However, both the reverse transcription-polymerase chain reaction (RT-PCR) and fluorescence in situ hybridization (FISH) failed to detect the PML-RARA transcript in the BM. An unpaired two-sided Student ttest was used to compare two groups. *P < .05, ***P < .001 On the first day after having a suspected diagnosis of APL, which was based on the morphological and immunophenotypical findings, the patient started an all-trans retinoic acid (ATRA) treatment. The white blood cell counts increased from 2.4 × 109/L to 12.2 × 109/L after 9 days. Because there was no evidence of a PML-RARA rearrangement with t(15;17), ATRA was stopped. The patient started induction chemotherapy composed of idarubicin and cytarabine on the 10th day. The patient achieved complete remission (CR) after one cycle of induction chemotherapy. He then received consolidation therapy, which included a high-dose of cytarabine for three cycles and "3 + 7"-based regimens for three cycles. To date, he has maintained a CR status for 44 months. To search for a potential fusion gene, we used the patient's BM samples collected at diagnosis to perform RNA sequencing using HiSeq (llumina, Inc., San Diego, CA, USA). After analyzing the data by STAR-Fusion and adjusting using FusionInspector software, the patient was shown to have a novel fusion transcript within chromosome 11, in which NUP98 was fused to RARA. To confirm this fusion, the following pair of primers was designed to perform PCR using cDNA: forward (at NUP98 exon 2): 5′-ggatttaatactacgacagccactttg-3′ and reverse (at RARA exon 3): 5′-ctctaccccgcatctacaagc-3′. An expected band of approximately 136 bp was visualized by gel electrophoresis, and the PCR product was analyzed by Sanger sequencing. The expected protein sequences were translated from the NUP98-RARA transcript (Figure 1B). Self-binding to form homodimers and binding to RXR to form heterodimers are key features of many leukemia fusion proteins in mediating transcriptional deregulation and leukemogenesis.2 To examine whether NUP98-RARA can form homodimers and heterodimers, coimmunoprecipitation (Co-IP) assays were performed with differentially tagged proteins in human 293 T cells. Similar to other RARA fusion proteins, Myc-tagged NUP98-RARA could bind with His-tagged NUP98-RARA (Figure 1C), which indicated that NUP98-RARA could self-associate and form homodimers. In addition, NUP98-RARA could interact with RXR and form heterodimers (Figure 1D). To further assess the impact of retinoid/rexinoid on the homodimer formation and NUP98-RARA/RXR interaction, Co-IP assay was performed by using these protein complexes in the absence or presence of 1 μM ATRA. Both the interactions of NUP98-RARA/NUP98-RARA and NUP98-RARA/RXR could be suppressed by treating with ATRA (Figure 1E,F). To further investigate the cellular localization of the NUP98-RARA fusion protein, His-tagged versions of these proteins were expressed in 293 T cells (Figure 1G). Using immunofluorescence and immunoblotting, we found that NUP98-RARA was predominantly located in the nucleus, and a small amount of it was in the cytoplasm (Figure 1H). NUP98-RARA fusion proteins have acquired an intracellular localization pattern that is distinct from that of the fusion partners (NUP98 and RARA). To further determine the domain within RARA needed for mediating the aberrant localization, we selectively deleted the DBD or LBD of RARA to obtain mutants of NUP98-RARA (ΔDBD) and NUP98-RARA (ΔLBD). While NUP98-RARA (ΔLBD) had a similar cellular distribution as the wild-type NUP98-RARA protein (Figure 1H), NUP98-RARA (ΔDBD) exhibited a decreased distribution of intranuclear proteins (Figure 1H), indicating a critical function of the DBD in determining the subcellular localization of the NUP98-RARA fusion protein. We next found that NUP98-RARA was downregulated both in the nucleus and cytoplasm by treating with ATRA (1 μM) but not by treating with daunorubicin (100 nM) (Figure 1I). Surprisingly, combined treatment of ATRA (1 μM) with daunorubicin (100 nM) almost diminished the expression of NUP98-RARA (Figure 1I). More specifically, the expression of the NUP98-RARA fusion protein began to decrease significantly after 8 hours of treatment with 1 μM ATRA, but showed no difference in the expression after 12 hours of treatment with 100 nM daunorubicin (Figure 1J). Consistent with the observation of the downregulated expression in nucleus and cytoplasm, and 2 hours of the combined treatment of ATRA and daunorubicin significantly reduced the total protein level of NUP98-RARA (Figure 1J). Furthermore, non-APL U937 leukemia cells stably transfected with NUP98-RARA were employed to detect the potential efficacy of ATRA and/or daunorubicin treatments. Note, NUP98-RARA-transformed U937 cells treated with 1 μM ATRA alone underwent differentiation-associated changes with lobulation of nuclei, and an increase in CD11b expression concurred with the morphological changes (Figure 1K and L). Furthermore, coadministration of ATRA and daunorubicin induced typical apoptotic morphological changes (eg, nuclear condensation) (Figure 1K). While wild-type U937 cells showed a negligible differentiation ratio after ATRA treatment at a concentration of 1 μM, NUP98-RARA-transformed U937 cells were extremely sensitive to the coadministration of ATRA and daunorubicin (Figure 1L). These data revealed that the combined therapy of ATRA and daunorubicin may have potential therapeutic effects for leukemia patients with NUP98-RARA. The NUP98 gene encodes a protein component of the nuclear pore complex (NPC) that regulates the nucleocytoplasmic transport of proteins and mRNAs.2, 3 The NUP98 protein contains a domain with a GLFG repeat that can activate transcription. Chimeric transcripts formed by the NUP98 N-terminal GLFG repeats fused to the C-terminus of partner proteins are expressed in all reported NUP98 fusions, suggesting that the NUP98 N-terminus may be important for leukemogenesis.3 Here, we report the first case of RARA as a new NUP98 partner that has a similar cellular localization to other NUP98-fusion proteins.2 Moreover, we demonstrated that NUP98-RARA could self-associate and form homodimers, and was able to coimmunoprecipitate with RXR and form heterodimers, which may suppress the transcription of downstream genes. How to treat rare variants of APL is an important issue.4-6 Although we demonstrated that NUP98-RARA showed sensitivity to ATRA ex vivo, we could not assess this treatment in this patient because of early discontinuation of ATRA. Compared with PML-RARA, NUP98-RARA also harbors the main functional domains of DBD and LBD in the fusion protein, which partially explains their similar biological properties. Combination of daunorubicin and ATRA obviously decreased NUP98-RARA expression and showed sensitivity to U937 cells expressing NUP98-RARA. This may partially explain the favorable outcome of this patient who received an anthracycline-based chemotherapy treatment. In conclusion, we describe here a novel NUP98-RARA gene rearrangement as the 14th variant of APL, which exhibits morphological and immunophenotypical features of APL. The favorable outcome of this patient with a standard 3 + 7 chemotherapy did not allow us to ascertain the sensitivity of NUP98-RARA to ATRA, due to the early discontinuation of the drug. Further studies are needed to better understand the biological properties, especially its sensitivity to ATRA, of this new NUP98-RARA fusion protein. Abstract:The NUP98::NSD1 fusion, frequently associated with FLT3-ITD mutation, is often detected in cytogenetically normal acute myeloid leukaemia (CN-AML) patients and predicts poor outcomes in children.1, 2 However, the clinical features of the NUP98::NSD1 fusion in adult AML remain unclear. We retrospectively investigated 11 adult patients with the NUP98::NSD1 fusion gene at the Institute of Hematology and Blood Diseases Hospital of the Chinese Academy of Medical Sciences from 2020 to 2021. Informed consent was obtained from the patients, and the study was conducted according to the Declaration of Helsinki. Five patients were male, and six were female, with a median age of 30 years (range, 14–59 years). Based on the 5th WHO classification, all patients were classified as acute myeloid leukaemia with defining genetic abnormalities. Table 1 shows the clinical characteristics of the patients. Chromosomes were identified from bone marrow (BM) cells using the reverse banding (R-banding) method, and karyotypes were classified according to the International System for Human Cytogenetic Nomenclature (2016) (ISCN2016).3 The chromosomal analysis at initial diagnosis showed a normal karyotype in most patients (cases 1–4, 6–10). Cases 5 and 11 showed trisomy 8 and del(9q13), respectively. With disease progression, a complex karyotype (CK) was observed in two patients (case 3, 7) who were CN-AML at diagnosis. This phenomenon suggests that NUP98::NSD1 positive patients have highly unstable chromosomes and genomes, resulting in disease progression.1, 4, 5 Trisomy 8 has also appeared frequently from disease onset to progression in our cohort. The relationship between an extra copy of the 8th chromosome and NUP98::NSD1 fusion remains to be further studied. Table 2 shows the karyotype data. CN Consistent with previous studies,4, 6 the FLT3-ITD mutation appeared the most in our cohort (63.6%), followed by the WT1 mutation (27.3%) and the CEBPA single mutation (27.3%). Five of seven patients harboring the FLT3-ITD mutation had an allelic ratio (AR) of ≥0.5. It is worth noting that none of the 11 patients acquired an NPM1 mutation. In the early stage of AML, this should prompt us to look for an NUP98::NSD1 fusion when an FLT3-ITD mutation exists or an NPM1 mutation is absent. All patients in our cohort completed at least one cycle of induction therapy, including daunorubicin (DNR) and cytarabine (Ara-C) (DA), idarubicin (IDA), Ara-C and cyclophosphamide (CTX) (IAC), homoharringtonine (HHT), Ara-C and aclacinomycin (Acla) (HAA) and a therapeutic regimen of Ara-C, azacitidine (AZA) plus a targeted drugs combination, and the clinical curative effect was evaluated. Consolidation therapy included high-dose Ara-C and a therapeutic regimen of AZA, fms-like tyrosine kinase 3 (FLT3) inhibitors, and B-cell lymphoma-2 (BCL-2) inhibitors. Salvage therapy included the chemotherapy drugs fludarabine (Flu), Ara-C and granulocyte colony-stimulating factor (G-CSF) (FLAG), Acla, Ara-C and G-CSF (CAG), HHT, Ara-C and etoposide (VP-16) (HAE), HHT, Ara-C and CTX (HAC) and HAA with or without hypomethylating drugs (AZA and decitabine (DCA)) and targeted drugs (FLT3 inhibitors and BCL-2 inhibitors). The complete response (CR) rate for 11 patients was 27.3% (3/11). The CR rate was 9.1% (1/11) in one cycle and 18.2% (2/11) in two cycles. Three patients (cases 1,2,8) who achieved CR after induction therapy were alive at the last follow-up. Of them, one patient (case 1) had a CR after one course of DA, although there were two relapses during the following five courses of treatment. The patient eventually had a durable remission for 4 months after allogeneic hematopoietic stem cell transplantation (allo-HSCT). One patient (case 2) had a sustained remission for 8 months after induction therapy, including one course of HAA followed by a course of Ara-C, AZA, FLT3 inhibitor and BCL-2 inhibitor. Another patient (case 8) had sustained remission for 4 months after DA and IAC. Both cases 2 and 8 underwent four consolidation therapy courses, and one also received allo-HSCT. Eight patients (3–7, 9–11) without CR had a median overall survival (OS) of 10 months. These patients had completed two cycles of induction chemotherapy with DA, followed by IAC. Of them, two patients (case 9, 10) only received induction chemotherapy. Case 9 died 2 months later, while the case 10 was waiting for the next course of therapy. Four patients (case 3, 5–7) received several courses (range 1–3) of salvage therapy before rescue allo-HSCT was done and had already survived more than 6 months, while one patient (case 6) died of pulmonary infection with an OS of 10 months. The remaining two patients both withdrew treatment after one course of salvage therapy, one of them (case 4) died 1 month later, and the other (case 11) was lost to follow-up (Table 2). The median follow-up time of 11 patients was 10 months, and the 1-year OS rate was approximately 54.5%. On the other hand, all seven patients with the FLT3-ITD mutation received chemotherapy agents concomitantly with targeted drugs, namely, FLT3 inhibitors or BCL-2 inhibitors. The median course of therapy was 4 (range, 3–6). Three patients (cases 1–3) received both BCL-2 inhibitors and FLT3 inhibitors, and two achieved CR. One patient (case 5) received BCL-2 inhibitor followed by FLT3 inhibitor during the next course. Cases 6 and 7 received BCL-2 inhibitor and FLT3 inhibitor, respectively. The CR rate was 28.6% (2/7), and the one-year OS rate was 68.6%. Of four patients without FLT3-ITD mutations, none underwent FLT3 inhibitor nor BCL-2 inhibitor therapy. The median course of treatment was 4.5 (range, 2–6). The CR rate was 25% (1/4), and one-year OS could not be determined in newly diagnosed FLT3-ITD-negative patients (Table 2). It has been reported that AML patients positive for NUP98::NSD1 and FLT3-ITD mutations have a worse outcome than those who are FLT3-ITD negative,1 which is contrary to our results. The difference may be due to the small number of cases in this study. All patients positive for NUP98::NSD1 and FLT3-ITD mutations received allo-HSCT. We assume that allo-HSCT played an essential role in their survival. It is worth noting that all patients positive for NUP98::NSD1 and FLT3-ITD mutations received targeted drugs, which might partly explain their better outcomes than those who were FLT3-ITD negative. In conclusion, AML with NUP98::NSD1 fusion is a distinct disease. It is difficult to achieve complete remission in patients with this fusion gene. Our results need to be verified in a larger cohort of patients with a longer duration of follow-up.
