Abstract:Summary Transplantation is a life-saving medical intervention that unfortunately is constrained by scarcity of available organs. An ideal system for allocating organs should seek to achieve the greatest good for the greatest number of people. It also must be fair and not disadvantage certain populations. However, policies aimed at reducing disparities also must be balanced with considerations of utility (graft outcomes), cost, efficiency, and any adverse effects on organ utilization. Here, we discuss the ethical challenges of creating a fair and equitable organ allocation system, focusing on the principles governing deceased donor kidney transplant waitlists around the world. The kidney organ allocation systems in the United States, Australia, and Hong Kong are used as illustrations.

Fairness, Efficiency, and Flexibility in Organ Allocation for Kidney Transplantation

Abstract:We propose a scalable, data-driven method for designing national policies for the allocation of deceased donor kidneys to patients on a waiting list in a fair and efficient way. We focus on policies that have the same form as the one currently used in the United States. In particular, we consider policies that are based on a point system that ranks patients according to some priority criteria, e.g., waiting time, medical urgency, etc., or a combination thereof. Rather than making specific assumptions about fairness principles or priority criteria, our method offers the designer the flexibility to select his desired criteria and fairness constraints from a broad class of allowable constraints. The method then designs a point system that is based on the selected priority criteria and approximately maximizes medical efficiency—i.e., life-year gains from transplant—while simultaneously enforcing selected fairness constraints. Among the several case studies we present employing our method, one case study designs a point system that has the same form, uses the same criteria, and satisfies the same fairness constraints as the point system that was recently proposed by U.S. policy makers. In addition, the point system we design delivers an 8% increase in extra life-year gains. We evaluate the performance of all policies under consideration using the same statistical and simulation tools and data as the U.S. policy makers use. Other case studies perform a sensitivity analysis (for instance, demonstrating that the increase in extra life-year gains by relaxing certain fairness constraints can be as high as 30%) and also pursue the design of policies targeted specifically at remedying criticisms leveled at the recent point system proposed by U.S. policy makers.

A critical assessment on kidney allocation systems

Abstract:The kidney allocation system that took effect on December 4, 2014 represents a significant improvement over the prior approach. It seeks to improve outcomes by longevity matching - pairing kidneys expected to function the longest with recipients expected to live the longest. It addresses the biological barriers faced by highly sensitized patients in an evidence based fashion and it begins to introduce the concept of medical need into kidney allocation by crediting time from the starting dialysis to a patient's waiting time. Additionally, it adds a more granular and continuous approach to classifying deceased donor kidneys through the kidney donor profile index and moves away from the dichotomous and flawed, standard criteria/extended criteria approach to allocating kidneys. Despite these changes, access to kidney transplantation across the age spectrum has remained intact and equitable. However even with these numerous positive improvements the system is not without its flaws. The increased sharing and by extension shipping of kidneys have created logistical challenges for organ procurement organizations and transplant centers. Early results seem to indicate that there have been an increase in cold ischemic time, an increase in delayed graft function and an increase in organ discard rate. There is also a reduced offer rate for children and while not a statistically significant decline in the number of transplants, it is a trend that requires close monitoring. Finally, the new kidney allocation system has done nothing to address the glaring deficiencies in the multi-organ allocation practices, all of which include a kidney, in the United States. Therefore despite the improvements made in kidney allocation, there is work yet to be done to ensure that the allocation of life saving and life prolonging organs for transplantation is done in a fashion consistent with ethical principles, based on science and free from local self interest so that this national resource is used for the betterment of the population it is meant to serve.
A novel allocation scheme for deceased donor kidneys to balance equity and utility

Abstract:Background: Patients with sensitization and blood type O experience increased waiting times for deceased-donor kidney transplantation (DDKT). While allocation benefits are needed to resolve inequity in DDKT opportunity, whether DDKT has comparable outcomes in this disadvantaged population requires further study. This study assessed these outcomes and developed a new allocation system that balances equity and utility. Methods: Patients from national and hospital cohorts from two centers in Korea were categorized as B1 to B4 (according to panel reactive antibody [PRA] positivity and ABO blood type) and A1 to A4 (based on the maximal PRA% and blood type), respectively. Competing risk and Cox regression analyses were performed to assess the effects of PRA and blood type on graft failure and mortality, respectively. Based on DDKT opportunities and posttransplant outcomes, a new scoring system for kidney allocation was developed. Results: The national and hospital cohorts included 3,311 and 819 patients, respectively, who underwent DDKT. Despite the disparities in DDKT opportunities, the graft failure rates and mortality did not differ among the different PRA and blood type groups. Furthermore, posttransplantation outcomes did not differ according to the categories with different DDKT opportunities. A new scoring system to provide additional points to disadvantaged populations was developed based on the hazard ratios for DDKT. Conclusion: A new allocation approach based on PRA and ABO blood types offers benefits to disadvantaged patients with fewer DDKT opportunities and could enhance equity without sacrificing utility in Korea, which has a long waiting time for DDKT.

Abstract:In this work, we close an open theoretical problem regarding the price of fairness in modern kidney exchanges. We then propose a hybrid fairness rule that balances a lexicographic preference ordering over agents, with a utilitarian objective. This rule has one parameter which controls a bound on the price of fairness. We apply this rule to real data from a large kidney exchange and show that our hybrid rule produces more reliable outcomes than other fairness rules.

Abstract:Given potential disparity and limited allocation of deceased donor kidneys for transplantation, a new federal kidney allocation system was implemented in 2014. Donor organ function and estimated recipient survival in this system has implications for perioperative management of kidney transplant recipients. Early analysis suggests that many of the anticipated goals are being attained. For anesthesiologists, implications of increased dialysis duration and burdens of end-stage renal disease include increased cardiopulmonary disease, challenging fluid, hemodynamic management, and central vein access. With no recent evidence to guide anesthesia care within this new system, we describe the kidney allocation system, summarize initial data, and briefly review organ systems of interest to anesthesiologists. As additional invasive and echocardiographic monitoring may be indicated, one consideration may be development of a dedicated anesthesiology team experienced in management and monitoring of complex patients, in a similar manner as has been done for liver transplant recipients.
Improving Policy-Constrained Kidney Exchange via Pre-Screening

Abstract:In barter exchanges, participants swap goods with one another without exchanging money; exchanges are often facilitated by a central clearinghouse, with the goal of maximizing the aggregate quality (or number) of swaps. Barter exchanges are subject to many forms of uncertainty--in participant preferences, the feasibility and quality of various swaps, and so on. Our work is motivated by kidney exchange, a real-world barter market in which patients in need of a kidney transplant swap their willing living donors, in order to find a better match. Modern exchanges include 2- and 3-way swaps, making the kidney exchange clearing problem NP-hard. Planned transplants often fail for a variety of reasons--if the donor organ is refused by the recipient's medical team, or if the donor and recipient are found to be medically incompatible. Due to 2- and 3-way swaps, failed transplants can "cascade" through an exchange; one US-based exchange estimated that about 85% of planned transplants failed in 2019. Many optimization-based approaches have been designed to avoid these failures; however most exchanges cannot implement these methods due to legal and policy constraints. Instead we consider a setting where exchanges can query the preferences of certain donors and recipients--asking whether they would accept a particular transplant. We characterize this as a two-stage decision problem, in which the exchange program (a) queries a small number of transplants before committing to a matching, and (b) constructs a matching according to fixed policy. We show that selecting these edges is a challenging combinatorial problem, which is non-monotonic and non-submodular, in addition to being NP-hard. We propose both a greedy heuristic and a Monte Carlo tree search, which outperforms previous approaches, using experiments on both synthetic data and real kidney exchange data from the United Network for Organ Sharing.

Optimizing Hard-to-Place Kidney Allocation: A Machine Learning Approach to Center Ranking

Abstract:Kidney transplantation is the preferred treatment for end-stage renal disease, yet the scarcity of donors and inefficiencies in allocation systems create major bottlenecks, resulting in prolonged wait times and alarming mortality rates. Despite their severe scarcity, timely and effective interventions to prevent non-utilization of life-saving organs remain inadequate. Expedited out-of-sequence placement of hard-to-place kidneys to centers with the highest likelihood of utilizing them has been recommended in the literature as an effective strategy to improve placement success. Nevertheless, current attempts towards this practice is non-standardized and heavily rely on the subjective judgment of the decision-makers. This paper proposes a novel data-driven, machine learning-based ranking system for allocating hard-to-place kidneys to centers with a higher likelihood of accepting and successfully transplanting them. Using the national deceased donor kidney offer and transplant datasets, we construct a unique dataset with donor-, center-, and patient-specific features. We propose a data-driven out-of-sequence placement policy that utilizes machine learning models to predict the acceptance probability of a given kidney by a set of transplant centers, ranking them accordingly based on their likelihood of acceptance. Our experiments demonstrate that the proposed policy can reduce the average number of centers considered before placement by fourfold for all kidneys and tenfold for hard-to-place kidneys. This significant reduction indicates that our method can improve the utilization of hard-to-place kidneys and accelerate their acceptance, ultimately reducing patient mortality and the risk of graft failure. Further, we utilize machine learning interpretability tools to provide insights into factors influencing the kidney allocation decisions.

Operations Research to Solve Kidney Allocation Problems: A Systematic Review

Abstract:Background: Operations research techniques enable health care administrators to optimize resource allocation and to find solutions to staff and patient scheduling problems. We aimed to conduct the first systematic review of the international literature on the use of operations research for allocating deceased-donor kidneys. Methods: We searched the MEDLINE, EMBASE, and PubMed databases from inception to February 2023. Two reviewers independently screened the title/abstract and subsequently the full text of potentially eligible articles and abstracted the data. Quality assessment of the final set of studies was conducted using Subben's checklist. Results: Of the 302 citations identified, 5 studies were included. These studies covered three themes, including (1) provider-facing decision aids to determine the timing of transplant for single or multiple patients; (2) system-level planning on kidney allocation based on blood type matching rules; and (3) patient-facilitated wait times estimation using incomplete information. Markov models, sequential stochastic assignment models, and queuing models were amongst the most used techniques. Although we found all included studies to meet Subben's criteria, we believe the checklist in its current form lacks items to assess the validity of model inferences. As such, we ended this review with a set of practical recommendations. Conclusions: Our review demonstrated the utility of operations research techniques in assisting the system, healthcare providers, and patients in the transplantation process. More research is needed to reach a consensus on a model that can be used to support the decision-making of different stakeholders for efficient kidney allocation, with the ultimate goal of reducing the gap between kidney supply and demand and enhancing the population's well-being.

Fair Matching in Dynamic Kidney Exchange

Abstract:Kidney transplants are sharply overdemanded in the United States. A recent innovation to address organ shortages is a kidney exchange, in which willing but medically incompatible patient-donor pairs swap donors so that two successful transplants occur. Proposed rules for matching such pairs include static fair matching rules, which improve matching for a particular group, such as highly-sensitized patients. However, in dynamic environments, it seems intuitively fair to prioritize time-critical pairs. We consider the tradeoff between established sensitization fairness and time fairness in dynamic environments. We design two algorithms, SENS and TIME, and study their patient loss. We show that the there is a theoretical advantage to prioritizing time-critical patients (around 9.18% tradeoff on U.S. data) rather than sensitized patients. Our results suggest that time fairness needs to be considered in kidney exchange. We then propose a batching algorithm for current branch-and-price solvers that balances both fairness needs.
Equilibrium Allocations under Alternative Waitlist Designs: Evidence from Deceased Donor Kidneys Kidneys

Abstract:Waitlists are often used to ration scarce resources, but the trade-offs in designing these mechanisms depend on agents' preferences. We study equilibrium allocations under alternative designs for the deceased donor kidney waitlist. We model the decision to accept an organ or wait for a preferable one as an optimal stopping problem and estimate preferences using administrative data from the New York City area. Our estimates show that while some kidney types are desirable for all patients, there is substantial match-specific heterogeneity in values. We then develop methods to evaluate alternative mechanisms, comparing their effects on patient welfare to an equivalent change in donor supply. Past reforms to the kidney waitlist primarily resulted in redistribution, with similar welfare and organ discard rates to the benchmark first come first served mechanism. These mechanisms and other commonly studied theoretical benchmarks remain far from optimal. We design a mechanism that increases patient welfare by the equivalent of an 18.2 percent increase in donor supply.

Abstract:AI algorithms increasingly make decisions that impact entire groups of humans. Since humans tend to hold varying and even conflicting preferences, AI algorithms responsible for making decisions on behalf of such groups encounter the problem of preference aggregation: combining inconsistent and sometimes contradictory individual preferences into a representative aggregate. In this paper, we address this problem in a real-world public health context: kidney exchange. The algorithms that allocate kidneys from living donors to patients needing transplants in kidney exchange matching markets should prioritize patients in a way that aligns with the values of the community they serve, but allocation preferences vary widely across individuals. In this paper, we propose, implement and evaluate a methodology for prioritizing patients based on such heterogeneous moral preferences. Instead of selecting a single static set of patient weights, we learn a distribution over preference functions based on human subject responses to allocation dilemmas, then sample from this distribution to dynamically determine patient weights during matching. We find that this methodology increases the average rank of matched patients in the sampled preference ordering, indicating better satisfaction of group preferences. We hope that this work will suggest a roadmap for future automated moral decision making on behalf of heterogeneous groups.

Abstract:The deceased donor kidney allocation system in the United States has undergone several rounds of iterative changes, but these changes were not explicitly designed to address the geographic variation in access to transplantation. The new allocation system, expected to start in December 2020, changes the definition of "local allocation" from the Donation Service Area to 250 nautical mile circles originating from the donor hospital. While other solid organs have adopted a similar approach, the larger number of both kidney transplant centers and transplant candidates is likely to have different consequences. Here, we discuss the incredible increase in complexity in allocation, discuss some of the likely intended and unintended consequences, and propose metrics to monitor the new system.

Inconsistent values and algorithmic fairness: a review of organ allocation priority systems in the United States

Abstract:Background: The Organ Procurement and Transplant Network (OPTN) Final Rule guides national organ transplantation policies, mandating equitable organ allocation and organ-specific priority stratification systems. Current allocation scores rely on mortality predictions. Methods: We examined the alignment between the ethical priorities across organ prioritization systems and the statistical design of the risk models in question. We searched PubMed for literature on organ allocation history, policy, and ethics in the United States. Results: We identified 127 relevant articles, covering kidney (19), liver (60), lung (24), and heart transplants (23), and transplant accessibility (1). Current risk scores emphasize model performance and overlook ethical concerns in variable selection. The inclusion of race, sex, and geographical limits as categorical variables lacks biological basis; therefore, blurring the line between evidence-based models and discrimination. Comprehensive ethical and equity evaluation of risk scores is lacking, with only limited discussion of the algorithmic fairness of the Model for End-Stage Liver Disease (MELD) and the Kidney Donor Risk Index (KDRI) in some literature. We uncovered the inconsistent ethical standards underlying organ allocation scores in the United States. Specifically, we highlighted the exception points in MELD, the inclusion of race in KDRI, the geographical limit in the Lung Allocation Score, and the inadequacy of risk stratification in the Heart Tier system, creating obstacles for medically underserved populations. Conclusions: We encourage efforts to address statistical and ethical concerns in organ allocation models and urge standardization and transparency in policy development to ensure fairness, equitability, and evidence-based risk predictions.
Penalties and Rewards for Fair Learning in Paired Kidney Exchange Programs

Abstract:A kidney exchange program, also called a kidney paired donation program, can be viewed as a repeated, dynamic trading and allocation mechanism. This suggests that a dynamic algorithm for transplant exchange selection may have superior performance in comparison to the repeated use of a static algorithm. We confirm this hypothesis using a full scale simulation of the Canadian Kidney Paired Donation Program: learning algorithms, that attempt to learn optimal patient-donor weights in advance via dynamic simulations, do lead to improved outcomes. Specifically, our learning algorithms, designed with the objective of fairness (that is, equity in terms of transplant accessibility across cPRA groups), also lead to an increased number of transplants and shorter average waiting times. Indeed, our highest performing learning algorithm improves egalitarian fairness by 10% whilst also increasing the number of transplants by 6% and decreasing waiting times by 24%. However, our main result is much more surprising. We find that the most critical factor in determining the performance of a kidney exchange program is not the judicious assignment of positive weights (rewards) to patient-donor pairs. Rather, the key factor in increasing the number of transplants, decreasing waiting times and improving group fairness is the judicious assignment of a negative weight (penalty) to the small number of non-directed donors in the kidney exchange program.

Abstract:The Final Rule mandates that organ allocation not be based on the transplant candidate's place of residence or listing, except as required by sound medical judgment and best use of donated organs, to avoid wasting organs and futile transplants, and to promote access and efficiency. Current Organ Procurement and Transplantation Network (OPTN) policies use donation service areas and OPTN regions to distribute and allocate organs for transplant. These policies have recently been called into question as not meeting the requirements of the Final Rule. Therefore, we propose using borderless allocation scores that combine medical priority scores with geographic feasibility scores. Medical priority scores are currently used in OPTN allocation policy, for example, the model for end-stage liver disease and the lung allocation score. Geographic feasibility scores can be developed to account for the effects of ischemia due to travel times, donor characteristics that modify the feasibility of traveling due to organ outcomes, and the costs of shipping organs over long distances. A borderless distribution and allocation system could address the goals of equity and utility, while fulfilling the mandates of the Final Rule and providing optimal use of a scare resource.

Abstract:Disparities that impact equity in access to kidney transplantation for patients with kidney failure have been well described. Many robust clinical trials have tested the effectiveness of interventions to reduce disparities and equilibrate access to kidney transplantation. Moreover, policy changes have been enacted to achieve the same aims. Despite these efforts, rates of kidney transplant wait listing within the first year of end‐stage kidney disease have remained unchanged over the past two decades, while incident rates of end‐stage kidney disease have climbed. Since prior interventions have not durably increased transplant access, disruptive change is clearly needed. The Advancing American Kidney Health Executive Order sets bold goals to transform kidney care for patients and caregivers. In this spirit, we discuss an Opt Out for Transplant Referral Model as a compelling solution to improve equity in access to kidney transplantation.

A Data Envelopment Analysis Approach for Assessing Fairness in Resource Allocation: Application to Kidney Exchange Programs

Abstract:Kidney exchange programs have significantly increased transplantation rates but raise pressing questions about fairness in organ allocation. We present a novel framework leveraging Data Envelopment Analysis (DEA) to evaluate multiple fairness criteria--Priority, Access, and Outcome--within a single model, capturing complexities that may be overlooked in single-metric analyses. Using data from the United Network for Organ Sharing, we analyze these criteria individually, measuring Priority fairness through waitlist durations, Access fairness through Kidney Donor Profile Index scores, and Outcome fairness through graft lifespan. We then apply our DEA model to demonstrate significant disparities in kidney allocation efficiency across ethnic groups. To quantify uncertainty, we employ conformal prediction within the DEA framework, yielding group-conditional prediction intervals with finite sample coverage guarantees. Our findings show notable differences in efficiency distributions between ethnic groups. Our study provides a rigorous framework for evaluating fairness in complex resource allocation systems, where resource scarcity and mutual compatibility constraints exist. Matching Kidneys with Priority in Kidney Exchange Programs

Abstract:Kidney exchanges were developed to match kidney failure patients with willing but incompatible donors to other donor-patient pairs. Finding a match in a large candidate pool can be modeled as an integer program. However, these exchanges accumulate participants with characteristics that increase the difficulty of finding a match and, therefore, increase patients' waiting time. Therefore, we sought to fine-tune the formulation of the integer program by more accurately assigning priorities to patients based on their difficulty of matching. We provide a detailed formulation of prioritized kidney exchange and propose a novel prioritization algorithm. Our approach takes advantage of the global knowledge of the donor-patient compatibility within a pool of pairs and calculates an iterative, paired match power (iPMP) to represent the donor-patient pairs' abilities to match. Monte Carlo simulation shows that an algorithm using the iPMP reduces the waiting time more than using paired match power (PMP) for the difficult-to-match pairs with hazard ratios of 1.3480 and 1.1100, respectively.
