Abstract:Arterial stiffness can be estimated noninvasively with various methods. Pulse pressure (PP), pulse wave velocity (PWV), stiffness index (SI), stroke volume (SV)/PP, proximal compliance (C1) and distal compliance (C2) are reported to be associated with arterial stiffness and can be used as predictors of cardiovascular events. 452 subjects were involved in the present study, mentioned six indices were obtained and compared to verify if they are suitable for the arterial stiffness estimation and which one is the most sensitive marker of arterial stiffness in hypertension. It is found that PWV has a good correlation with PP, SI and C2, and it is an effective index to distinguish hypertensive from normotensive. The receiver operating characteristic (ROC) analysis also evidenced the good performance of PWV in the diagnosis of hypertension. Abstract:Information regarding regional arterial stiffness assessment in osteoarthritis (OA) was scarce and sometimes contradictory. We aimed to investigate the aortic, lower limb peripheral arterial stiffness and their associations with knee OA. Patients with primary knee OA and matched non-OA controls were prospectively enrolled from two medical centers in China. The carotid-femoral pulse wave velocity (cfPWV) and femoral-ankle pulse wave velocity (faPWV) were measured using a novel ultrasound technique. A total of 238 participants (including 128 patients with knee OA and 110 controls) were included. In OA patients, cfPWV was significantly higher than that of non-OA controls (9.40 ± 1.92 vs 8.25 ± 1.26 m/s, P < 0.0001). However, faPWV measurements in OA patients (12.10 ± 2.09 m/s) showed no significant difference compared with that of the controls (11.67 ± 2.52 m/s, P = 0.130). Multiple regression analysis revealed that cfPWV was independently associated with knee OA ( P < 0.0001) after adjusting for the confounding factors including age, gender, smoking, mean blood pressure, body mass index, heart rate, high-sensitivity C-reactive protein and lipids profiles. In contrast, faPWV did not show independent association with knee OA ( P = 0.372) when after adjusting for confounding factors. In addition, Spearman's correlation analysis showed cfPWV had a significant correlation with Kellgren-Lawrence score (r s = 0.2333, P = 0.008), but no correlation was founded between faPWV with Kellgren-Lawrence score (r s = 0.1624, P = 0.067) in OA patients. This study demonstrated that stiffening of aorta, but not lower limb arteries, was independently associated with knee OA. Our findings may call for further implementation of routine aortic stiffness assessments so as to evaluate cardiovascular risk in patients with OA.

Abstract:The aim of this study was to investigate how frequent ambulatory blood pressure (ABP) readings need to be obtained to reproduce the ambulatory arterial stiffness index (AASI) and pulse pressure (PP) without loss of information. We compared concordance from full and reduced ABP recordings. We recorded 24-h ABP at 30-min intervals in 1542 residents of Ohasama, Japan (baseline age, 40–93 years; 63.4% women). We randomly excluded up to 16 readings per recording or we selected readings at fixed 1- or 2-h intervals. Using full recordings as reference, we computed for the reduced recordings repeatability coefficient by Bland and Altman's approach. By Cox regression, we also calculated multivariate-adjusted hazard ratios for cardiovascular mortality. The median number of ABP readings per recording was 46. Randomly excluding more readings reduced the concordance of AASI, but not PP. Selecting blood pressure readings at 1- or 2-h intervals produced mean values of AASI and PP, which significantly differed from those in full recordings. During follow-up (median, 13.3 years) 126 cardiovascular deaths occurred. Across quartiles, AASI significantly predicted cardiovascular mortality in a U-shaped manner. AASI lost its prognostic significance when the number of randomly excluded readings increased from 8 to 16 or when the interval between readings was 1 h or longer. Compared with PP, AASI is less reproducible when the number of readings in ABP decreases, but this does not affect the predictive accuracy of AASI for cardiovascular mortality, until the median number of readings per ABP recording is less than ∼35.

Abstract:SummaryBackgroundAmbulatory arterial stiffness index (AASI) has been proposed as a new measure of arterial stiffness for predicting cardio–cerebro–vascular morbidity and mortality. However, there has been no research on the direct relationships between AASI and arterial stiffness‐determining factors.MethodsWe utilized beat‐to‐beat intra‐aortic blood pressure (BP) telemetry to characterize AASI in Wistar–Kyoto (WKY) and spontaneously hypertensive rats (SHR). By determination of aortic structural components and analysis of their correlations with AASI, we provided the first direct evidence for the associations between AASI and arterial stiffness‐determining factors including the collagen content and collagen/elastin.ResultsAmbulatory arterial stiffness index was positively correlated with pulse pressure in both WKY and SHR, less dependent on BP and BP variability than pulse pressure, and relatively stable, especially the number of BP readings not less than ~36. The correlations between AASI and aortic components were comparable for various AASI values derived from BP readings not less than ~36. Not only AASI but also BP variability and pulse pressure demonstrated a direct relationship with arterial stiffness.ConclusionsThese findings indicate AASI may become a routine measure in human arterial stiffness assessment. It is recommended to use a cluster of parameters such as AASI, BP variability, and pulse pressure for evaluating arterial stiffness.

Abstract:Background: Arterial stiffness-typically assessed from non-invasive measurement of pulse wave velocity along a straight portion of the vascular tree between the right common carotid and femoral arteries-is a reliable predictor of cardiovascular risk in patients with essential hypertension. Methods: We reviewed how carotid-femoral pulse wave velocity increases with age and is significantly higher in hypertension (than in age- and gender-matched individuals without hypertension), particularly when hypertension is associated with diabetes mellitus. Results: From the elastic aorta to the muscular peripheral arteries of young healthy individuals, there is a gradual but significant increase in stiffness, with a specific gradient. This moderates the transmission of pulsatile pressure towards the periphery, thus protecting the microcirculatory network. The heterogeneity of stiffness between the elastic and muscular arteries causes the gradient to disappear or be inversed with aging, particularly in long-standing hypertension. Conclusions: In hypertension therefore, pulsatile pressure transmission to the microcirculation is augmented, increasing the potential risk of damage to the brain, the heart, and the kidney. Furthermore, elevated pulse pressure exacerbates end-stage renal disease, particularly in older hypertensive individuals. With increasing age, the elastin content of vessel walls declines throughout the arterial network, and arterial stiffening increases further due to the presence of rigid wall material such as collagen, but also fibronectin, proteoglycans, and vascular calcification. Certain genes, mainly related to angiotensin and/or aldosterone, affect this aging process and contribute to the extent of arterial stiffness, which can independently affect both forward and reflected pressure waves.
Arterial stiffness as a screening tool for cardiovascular risk in health and disease

Abstract:Background: Cardiovascular diseases (CVD) account for approximately one-third of all deaths worldwide. The incidence of cardiovascular events such as myocardial infraction has been reported to be progressively increasing with age, especially with existing comorbidities such as hypertension, diabetes and obesity. Assessing arterial stiffness indices may serve as a screening tool in identification of population at risk of cardiovascular diseases and assist in implementation of preventive measures and early treatment in this population. Objectives: To measure and compare the arterial stiffness indices in healthy adults with diabetes, hypertension and obesity. Methods: A total of 184 adults in the age group of 30-50 years were included in the study who were divided into 4 groups: Group I (n = 64) (diabetic), group II (n = 40) (hypertensives), group III (n = 40) (obese) and group IV (n = 40) (control). The arterial stiffness indices were measured by using a certified oscillometric device in all the participants. Results: The arterial stiffness indices were assessed by using a certified oscillometric device in all the participants. The mean values of right baPWV and left baPWV are found to be significantly higher in hypertensive subjects compared with obese, diabetic and healthy controls. Conclusion: The pulse wave velocity, ASI and pulse pressure serve as independent predictors of cardiovascular mortality and outcomes in hypertension, diabetes and obesity as well as healthy individuals.
Aortic-brachial stiffness mismatch as potential marker of subclinical arterial damage in patients with rheumatoid arthritis

Abstract:Aortic-brachial stiffness mismatch is a potential new marker of a subclinical vascular damage that has never been studied in patients with rheumatic diseases. The aim of the study was to assess the frequency of arterial stiffness mismatch in rheumatoid arthritis (RA) and to evaluate its clinical associations. Materials and Methods. The study group included 85 patients with RA (males 22.4 %, aged 59.7 ± 14.3 years, hypertension in 65 %, mean disease activity score (DAS-28 (C-reactive protein) 3.7 ± 1.1), and the control group included 40 subjects matched by gender, age and risk factors. The study methods included measurements of clinical and ambulatory brachial and aortic blood pressure (BP) (BPLab-Vasotens), arterial stiffness parameters parameters (applanation tonometry, SphygmoCorAtCor), cardio-ankle vascular index (VaSera) and cardio-vascular risk assessments using the SCORE, American College of Cardiology/American Heart Association (ACC/AHA) 2013 pooled cohort equations and QRisk2 scoring systems. The arterial stiffness gradient was calculated as a ratio between carotid-femoral (cf) and carotid-radial (cr) pulse wave velocity, and its elevation of ≥ 1 was considered as arterial stiffness mismatch. A p-value of 0.05 was considered significant. Results and Discussion. The mean stiffness gradient in RA patients without and with hypertension was 1.1 ± 0.1 and 1.4 ± 0.4, respectively (р 0.001); in controls, 0.99 ± 0.2 and 1.3 ± 0.3, respectively (р 0.001). The frequency of stiffness mismatch in the RA group was significantly higher compared to the controls in the whole study population (88.2 % vs 65 % (р = 0.002)) and in both normotensive and hypertensive subgroups (76.7 % vs 43.8 % (p = 0.03), and 94.5 % vs 79.2 % (p = 0.04), respectively). The same trend was observed in the subgroups with normal carotid-femoral pulse wave velocity: arterial stiffness mismatch was present in 82.1 % of RA patients vs. 51.9 % control subjects (p = 0.004). The stiffness gradient was associated with age (r = 0.63), hypertension duration (r = 0.56), cardio-vascular risk by the ACC/AHA 2013 (r = 0.69) and Qrisk2 (r = 0.7) scoring systems, nocturnal aortic systolic BP (r = 0.53), cardio-ankle vascular index (r = 0.60) and diurnal index of brachial systolic BP (r = -0.4). Significant differences in stiffness gradient values were observed in the subgroups based on elevation of aortic systolic BP and pulse wave velocity above individual reference values, aortic pulse pressure 50 mmHg, cardio-ankle vascular index 9, presence of high cardio-vascular risk, masked and nocturnal hypertension, and non-dipping. Conclusion. Patients with RA are characterized by higher frequency of arterial stiffness mismatch compared to controls, irrespective of the history of hypertension or the values of carotid-femoral pulse wave velocity. Arterial stiffness mismatch is associated with unfavorable 24-h BP profile, higher frequency of nocturnal hypertension and cardio-vascular risk.

Theoretical Method and Clinical Experiments for Estimating Arterial Stiffness Based on Upper-Arm Cuff Oscillometric Wave

Abstract:Objective To examine the validity of an upper-arm cuff-based pulse wave device (PASESA AVE-1500) on assessing central arterial stiffness based on oscillometric waves, and analyze the related influential factors. Methods A computational model of the cardiovascular system coupled with an upper-arm oscillometric cuff was constructed and applied to simulate the measurement process of the pulse wave device. With the model, a theoretical study for assessing central arterial stiffness based on the characteristics of cuff oscillometric wave was performed, followed by a series of numerical experiments aimed to quantify the sensitivity of the assessment to various cardiovascular parameters. Furthermore, the validity of the pulse wave device was examined by clinical experiments. Results The computational model-based theoretical study revealed that the ratio of the second valley to the first peak (|V2|/|P1|) on the time-differentiated cuff wave correlated positively with the stiffness of central arteries. Subsequent numerical experiments showed that |V2|/|P1| increased with the impairment of cardiac systolic function, but was relatively less influenced by the total peripheral vascular resistance and the stiffness of the brachial artery. The clinical study demonstrated that the measured pulse velocity index (AVI) differed significantly between the patient group with cardiovascular diseases (n=68) and the healthy subject group (n=170) (P<0.0001), which was consistent with the inter-group difference observed for brachial-ankle pulse wave velocity (baPWV) (P<0.0001). In addition, AVI was found to correlate significantly with both baPWV (r=0.656, P<0.0001) and age (r=0.525, P<0.0001). Conclusion The mechanical property of central arteries can be obtained by analyzing the oscillometric wave of an upper-arm cuff, which makes it possible to noninvasively and rapidly assess central arterial stiffness.
Aortic Stiffness Is Associated with Coronary Microvascular Dysfunction in Patients with Non-obstructive Coronary Artery Disease

Abstract:Objective Associations between aortic stiffness and cardiovascular disease events are mediated in part by pathways that include coronary microvascular dysfunction (CMD) and remodeling. However, the relationship between aortic stiffness and CMD remains unclear. The present study aimed to determine whether aortic stiffness causes CMD as evaluated by the hyperemic microvascular resistance index (hMVRI) in patients with non-obstructive coronary artery disease (CAD). Methods The intracoronary physiological variables in 209 coronary arteries were evaluated in 121 patients with non-obstructive CAD (fractional flow reserve >0.80) or reference vessels. The cardio-ankle vascular index (CAVI) as a measure of aortic stiffness and atherosclerotic risk factors were also measured. Results Univariate analyses showed that hMVRI correlated with age (β=0.24, p=0.007), eicosapentaenoic acid (EPA; β=-0.18, p=0.048), EPA/arachidonic acid (AA) (EPA/AA) ratio (β=-0.22, p=0.014) and CAVI (β=0.30, p=0.001). A multivariate regression analysis identified CAVI (β=0.25, p=0.007) and EPA/AA ratio (β=-0.26, SE=0.211, p=0.003) as independent determinants of hMVRI. Conclusion Aortic stiffness may cause CMD in patients with non-obstructive CAD via increased coronary microvascular resistance. Aortic stiffness is associated with CMD which is evaluated as hyperemic microvascular resistance in patients with non-obstructive CAD.

Abstract:Both cardio-ankle vascular index (CAVI) and brachial-ankle pulse wave velocity (baPWV) are noninvasive methods to estimate arterial stiffness. The purpose of this study is to determine whether CAVI or baPWV is superior as an index of arterial stiffness. One hundred and thirty patients with chest pain syndrome who underwent coronary angiography (CAG) were included in this study. We obtained intima-media-thickness (IMT) and the stiffness parameter β of the carotid artery by carotid ultrasounds (CU). The peak early diastolic velocity (E), deceleration time of E (EDCT), peak atrial systolic velocity (A) of transmitral flow and left ventricular mass index (LVMI) were obtained by echocardiography. CAVI, baPWV, total cholesterol (T-C), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) and triglycerides (TG) were measured before CAG. There was a significant correlation between CAVI and baPWV (r=0.64, p <0.01). Both CAVI and baPWV were significantly correlated with age, IMT and β (age: r = 0.64, p<0.01 for CAVI, and r=0.48, p<0.01 for baPWV; IMT: r=0.40, p<0.01, and r=0.31, p<0.01; β: r=0.36, p<0.01 and r=0.25, p<0.01). However, only CAVI was correlated with the parameters of left ventricular diastolic indices from echocardiography (E/A: r=0.44, p<0.01; EDCT: r=0.36, p<0.01). Additionally, LDL-C and T-C/HDL-C were also associated with only CAVI (LDL-C: r=0.26, p<0.02; T-C/HDL-C: r=0.30, p<0.01), not baPWV. Finally, only CAVI was significantly higher in the group with angina pectoris than in the normal group (9.708±1.423 vs. 9.102±1.412; p=0.0178). All parameters associated with atherosclerosis suggested that CAVI was superior to baPWV as a parameter of arterial stiffness.

Abstract:Data on the impact of arterial stiffness on autonomic function are limited. We sought to investigate whether heart rate recovery (HRR), a predictor of autonomic function, is impaired in patients with increased arterial stiffness. A total of 475 participants (mean age 55.8 ± 11.1 years, 34.3% women) who underwent a treadmill exercise test (TET) for the evaluation of chest pain were retrospectively analyzed. All patients underwent brachial-ankle pulse wave velocity (baPWV) measurement on the same day. HRR was defined as the difference in heart rate from maximal exercise to 1 min of recovery. Participants with the lowest HRR tertile were older and had more cardiovascular risk factors than those with the highest HRR tertile. Simple correlation analysis showed that baPWV was negatively correlated with HRR ( r = − 0.327, P < 0.001). In multiple linear regression analysis, there was a significant association between baPWV and HRR, even after adjusting for potential confounders ( β = − 0.181, P < 0.001). In participants who underwent TET, baPWV was negatively correlated with HRR. The results of our study indicate a potential relationship between arterial stiffness and the autonomic nervous system.

Abstract:Objectives We hypothesized that one minus the slope of diastolic on systolic blood pressure in individual 24-h ambulatory blood pressure recordings (ambulatory arterial stiffness index) might reflect arterial stiffness and predict cardiovascular mortality. Methods In volunteers and a population recruited in China, we studied concordance between ambulatory arterial stiffness index and established indexes of arterial stiffness. We assessed the predictive value of ambulatory arterial stiffness index in relation to cardiovascular outcome in Irish hypertensive patients. Results In 166 healthy volunteers, aged 22–83 years, the correlation coefficient between ambulatory arterial stiffness index and pulse wave velocity was 0.51 (P<0.001). In 348 randomly recruited Chinese, the correlations between ambulatory arterial stiffness index and both the central and peripheral systolic augmentation indexes were significantly stronger than those for 24-h ambulatory pulse pressure, particularly in study participants younger than 40 years. Among normotensive participants, the 95th percentile of the ambulatory arterial stiffness index was 0.55 in 234 Chinese and 0.57 in 1617 Europeans enrolled in the International Database on Ambulatory Blood Pressure Monitoring. The upper boundary of the 95% prediction interval of the ambulatory arterial stiffness index in relation to age ranged from 0.53 at 20 years to 0.72 at 80 years. In 11 291 patients enrolled in the Dublin Outcome Study, both ambulatory arterial stiffness index and 24-h ambulatory pulse pressure significantly predicted cardiovascular mortality. Ambulatory arterial stiffness index was a strong predictor of fatal stroke in normotensive participants, whereas pulse pressure better predicted heart attack in hypertensive patients. Conclusion Ambulatory arterial stiffness index is a novel measure of arterial stiffness, which can be readily determined from ambulatory blood pressure recordings and which independently predicts cardiovascular mortality.

Abstract:Arterial stiffness can be an indicator of the mechanical properties of the elastic arterial wall. Changes in arterial tone and structure lead to increased arterial stiffness. Of the several methods currently available to detect arterial stiffness, pulse wave velocity is used extensively, due to its relative simplicity. A pulse wave is produced by intermittent left ventricular ejection, propagates from the centre to the periphery along the arterial tree, and can be reflected at the points of impedance mismatch. There is an interaction of the increases of arterial stiffness and pulse pressure, which, in turn, leads to a vicious cycle of further increases. Epidemiological studies have proven the prognostic value of arterial stiffness in cardiovascular diseases.

Abstract:Hypertension prevalence increases with age. Age and high blood pressure are the two main determinants of arterial stiffness. In elderly hypertensives, large arteries stiffen and systolic and pulse pressures increase, due to wave reflections. A major reason for measuring arterial stiffness in clinical practice in elderly hypertensive patients comes from the repeated demonstration that arterial stiffness and wave reflections have a predictive value for CV events. A large body of evidence has been published during the last two decades, concerning the epidemiology, pathophysiology, and pharmacology of large arteries in hypertension in various settings of age. Particularly, two expert consensus documents have reviewed the methodological agreements for measuring arterial stiffness. The concepts of Early Vascular Aging (EVA) and Supernormal Vascular Aging (SUPERNOVA) help to better understand on which determinants of arterial stiffness it is possible to act, in order to limit target organ damage and cardiovascular complications. This review will address the issues of the cellular and molecular mechanisms of arterial stiffening in elderly hypertensives, the consequences of arterial stiffening on central systolic and pulse (systolic <i>minus</i> diastolic, PP) pressures and target organs, the methodology for measuring arterial stiffness, central pulse pressure and wave reflection, the epidemiological determinants of arterial stiffening in elderly hypertensives, the pharmacology of arterial destiffening, and how the concepts of EVA and SUPERNOVA apply to the detection of organ damage and prevention of CV complications.

Arterial Stiffness and Cardiovascular Events: the Artscan Study

Abstract:Objective: To explore the relationship between arterial stiffness and cardiovascular events. Methods: Arterial stiffness indices were measured in essential hypertension, including the carotid–femoral pulse wave velocity (c-f PWV), pulse wave analysis (PWA) and large and small artery elasticity (C1 and C2) and inter-media thickness (IMT). Results: (1) The mean age was 60 years, 64% men. During median follow-up of 24 (1–36)months, 90 of 670 subjects (13.4%) had a total event. (2) Adjusting for age, sex, BMI, MAP, antihypertensive drugs, TC, smoking, each SD (3 m/s) increase in c-f PWV or each SD (0.16 mm) increase in IMT was associated with a 48.7% (95% CI, 1.126–1.965 per SD, P=0.005) or 47.7% (95% CI, 1.056–2.065 per SD, P=0.023) increase in cardiovascular events (CV). Each SD (10.9%) increase in augmentation index ([email protected]) of men was associated with a 41.1% (95% CI, 1.035–1.923 per SD, P=0.029) increase in CV. Each SD (7.6 mm Hg) increase in CAP (central augmentation pressure) of larger than 60 years was associated with a 49.6% (95% CI, 1.079–2.073, per SD, P=0.016) increase in CV. Conclusions: Higher c-f PWV or IMT is associated with increased risk for CV, which can be a surrogate for cardiovascular morbidity and mortality, but for [email protected] or CAP may be influenced by sex or age.

Abstract:OBJECTIVE: Increased resting heart rate as well as increased arterial stiffness are both independent predictors of cardiovascular events and mortality. Results of previous studies have failed to converge concerning the association between heart rate and arterial stiffness, regardless of other potential confounders, such as age, gender and particularly blood pressure (BP). We aimed to investigate: (a) the degree of association (if any) between resting heart rate and carotid-to-femoral pulse wave velocity (PWV), the gold standard index of arterial stiffness, (b) if the relationship between heart rate and PWV is mediated by BP levels and (c) whether their association is affected by the levels of aortic stiffening.APPROACH: Demographic, hemodynamic, laboratory and clinical data of 1566 subjects from the cross-sectional observational 'Corinthia' study were analyzed using univariate and multivariate regression models. Mediation analysis was performed to test whether mean arterial pressure (MAP) is a significant mediator in the heart rate-PWV relationship. The total population was divided in two groups of low and high arterial stiffness according to the median PWV value (8.6 m s-1).MAIN RESULTS: We found that (i) there is a significant association between heart rate and PWV, regardless of other confounding factors. An increase in heart rate by 20 b.p.m. can increase PWV by 0.5 m s-1. However, this association was significant only for subjects with increased aortic stiffness (PWV > 8.6 m s-1) and not for those with PWV ⩽ 8.6 m s-1. Further, (ii) heart rate-PWV association was partially mediated by MAP.SIGNIFICANCE: Increased resting heart rate is related to increased aortic stiffness, only in subjects with stiffer aortas, regardless of BP and other risk factors and subjects' characteristics. The synergistic prognostic effect of increased arterial stiffness and elevated heart rate on target organ damage, cardiovascular events and mortality should be explored in future studies.
