Abstract:Background: Short-term blood pressure variability (BPV) is associated with arterial stiffness in patients with hypertension. Few studies have examined associations between arterial stiffness and digital home BPV over a mid- to long-term time span, irrespective of underlying hypertension. Objective: This study aims to investigate if arterial stiffness traits were associated with subsequent mid- to long-term home BPV in the electronic Framingham Heart Study (eFHS). We hypothesized that higher arterial stiffness was associated with higher home BPV over up to 1-year follow-up. Methods: At a Framingham Heart Study research examination (2016-2019), participants underwent arterial tonometry to acquire measures of arterial stiffness (carotid-femoral pulse wave velocity [CFPWV]; forward pressure wave amplitude [FWA]) and wave reflection (reflection coefficient [RC]). Participants who agreed to enroll in eFHS were provided with a digital blood pressure (BP) cuff to measure home BP weekly over up to 1-year follow-up. Participants with less than 3 weeks of BP readings were excluded. Linear regression models were used to examine associations of arterial measures with average real variability (ARV) of week-to-week home systolic (SBP) and diastolic (DBP) BP adjusting for important covariates. We obtained ARV as an average of the absolute differences of consecutive home BP measurements. ARV considers not only the dispersion of the BP readings around the mean but also the order of BP readings. In addition, ARV is more sensitive to measurement-to-measurement BPV compared with traditional BPV measures. Results: Among 857 eFHS participants (mean age 54, SD 9 years; 508/857, 59% women; mean SBP/DBP 119/76 mm Hg; 405/857, 47% hypertension), 1 SD increment in FWA was associated with 0.16 (95% CI 0.09-0.23) SD increments in ARV of home SBP and 0.08 (95% CI 0.01-0.15) SD increments in ARV of home DBP; 1 SD increment in RC was associated with 0.14 (95% CI 0.07-0.22) SD increments in ARV of home SBP and 0.11 (95% CI 0.04-0.19) SD increments in ARV of home DBP. After adjusting for important covariates, there was no significant association between CFPWV and ARV of home SBP, and similarly, no significant association existed between CFPWV and ARV of home DBP (P>.05). Conclusions: In eFHS, higher FWA and RC were associated with higher mid- to long-term ARV of week-to-week home SBP and DBP over 1-year follow-up in individuals across the BP spectrum. Our findings suggest that higher aortic stiffness and wave reflection are associated with higher week-to-week variation of BP in a home-based setting over a mid- to long-term time span.

Abstract:Abstract Arterial blood pressure (BP) is a continuous variable, with a physiology characterized by significant variability stemming from the complex interaction among haemodynamic factors, neuronal reflexes, as well as hormonal, behavioural, and environmental stimuli. The homoeostatic response accounts for the physiologic variability in BP in normotensive individuals, which is more evident in hypertensive patients. Blood pressure variability is a complex phenomenon, which could be classified in various types: very short term (beat to beat), short term (during 24 h), mid-term (day by day), long term (<5 years), and very long term (>5 years). Accurate measurement of BP variability represents a complex and often controversial endeavour, despite several methodological approaches are available. Albeit a prognostic significance has been demonstrated for some indicators of BP variability, the clinical significance of this measurement is still uncertain. In fact, none of the indicators presently available for BP variability, including early morning BP rise, substantially affects, and redefines, the cardiovascular risk of the hypertensive patient, over and beyond the mere BP values. Accordingly, in defining the cardiovascular risk, the focus should be on the absolute BP values, which remain the most relevant risk factor, and the one more susceptible to modification with both non-pharmacologic and pharmacologic treatment.

Abstract:AbstractBlood pressure (BP) variability may have its effect on the development of vascular disease. The authors aimed to examine the association between the visit‐to‐visit variability (VVV) of BP and arterial stiffness in Chinese adults. The authors included 1407 participants from a prospective cohort study of community residents who were ≥40 years, without a history of myocardial infarction or stroke, and with data at the baseline, the second and the third visits in 2008, 2009, and 2013. The VVV of BP was defined as the standard deviation (SD), the coefficient of variation (CV), the average successive variability (ASV), and the variability independent of the mean (VIM) in BP levels at the 3 visits. Arterial stiffness was measured by brachial‐ankle pulse wave velocity (ba‐PWV) at the 2nd and the 3rd visits. Levels of ba‐PWV change and the occurrence of an elevated ba‐PWV increased significantly in the highest tertile of VVV measures of systolic BP (SBP) and pulse pressure (PP) compared with the lowest tertile, respectively. The multivariable regression analysis revealed that VVV measures of SBP and PP were significantly associated with levels of ba‐PWV change and the risks of developing an elevated ba‐PWV. The odds ratios (ORs) and 95% confidence intervals (CIs) for the risk were 2.12 (1.57–3.12) and 1.92 (1.38–2.68) in participants with the highest versus the lowest tertile of SBP‐SD and PP‐SD, respectively. No significant association was found for diastolic BP variability measures. The increased long‐term variabilities of SBP and PP were associated with an increased risk of arterial stiffness.

Abstract:Aging is characterized by alterations in neuro-cardiovascular regulatory mechanisms, leading to impaired physiological variability patterns. Repeated evidence has shown that increased Blood Pressure Variability (BPV) is associated with organ damage and exerts independent predictive value on several health outcomes: cardiovascular events, neurocognitive impairment, metabolic disorders and typical geriatric syndromes such as sarcopenia and frailty. Accordingly, it may constitute the epiphenomenon of the alterations in homeostatic mechanisms, typical of late life.Aging and altered BPV share the same molecular mechanisms, in particular the clinical state of subclinical inflammation has been widely ascertained in advanced age and it is also related to BP dysregulation through altered endothelial function and increased production of ROS. Arterial stiffness and autonomic dysfunction have been associated to impairment in BPV and also represent key features in elderly patients. Furthermore, accumulating evidence in the field of Geroscience has reported that several molecular changes described in cardiovascular aging and altered BPV also relate with the majority of the 9 identified hallmarks of aging. Indeed, BPV may be linked to genomic instability, epigenetic modification and mitochondrial oxidative damage, which represent milestones of aging process.The aim of the present paper is to analyse the interplay between BPV and the pathophysiology of the ageing process, in order to stimulate discussion about the potential role of BPV as a new marker of aging.

Temporal and Bidirectional Association Between Blood Pressure Variability and Arterial Stiffness: Cross-Lagged Cohort Study

Abstract:Background The causal relationship between blood pressure variability (BPV) and arterial stiffness remains debated. Objective This study aimed to explore the temporal and bidirectional associations between long-term BPV and arterial stiffness using a cohort design with multiple surveys. Methods Participants from the Beijing Health Management Cohort who underwent health examinations from visit 1 (2010-2011) to visit 5 (2018-2019) were enrolled in this study. Long-term BPV was defined as intraindividual variation using the coefficient of variation (CV) and SD. Arterial stiffness was measured by brachial-ankle pulse wave velocity (baPWV). The bidirectional relationship between BPV and arterial stiffness was explored using cross-lagged analysis and linear regression models, with records before and after visit 3 categorized as phase 1 and phase 2, respectively. Results Of the 1506 participants, who were a mean of 56.11 (SD 8.57) years old, 1148 (76.2%) were male. The cross-lagged analysis indicated that the standardized coefficients of BPV at phase 1 directing to the baPWV level at phase 2 were statistically significant but not vice-versa. The adjusted regression coefficients of the CV were 4.708 (95% CI 0.946-8.470) for systolic blood pressure, 3.119 (95% 0.166-6.073) for diastolic pressure, and 2.205 (95% CI 0.300-4.110) for pulse pressure. The coefficients of the SD were 4.208 (95% CI 0.177-8.239) for diastolic pressure and 4.247 (95% CI 0.448-8.046) for pulse pressure. The associations were predominant in the subgroup with hypertension, but we did not observe any significant association of baPWV level with subsequent BPV indices. Conclusions The findings supported a temporal relationship between long-term BPV and arterial stiffness level, especially among people with hypertension.

Abstract:Objective:To investigate the cardio ankle vascular index arterial stiffness in patients with hypertension index (CAVI) and blood pressure(BP) variability (BPV) and its influencing factors correlation.Methods:133 cases of patients with primary hypertension were monitored by 24hAMBP and CAVI.According to the systolic blood pressure(SBP) variability(SPV),groups were divided into three groups:groupA SBPV ＜0％,group B:SBPV 0％～ 10％ and group C:SBPV 10％～20％.The correlation between CAVI and BPV and related risk factors were analyzed.Results:The CAVI and diastolic blood pressure (DBP) variability (DPV) was significantly negatively correlated with SPV in hypertensive patients (P ＜ 0.05).SPV ＜ 0％,CAVI increases with the absolute value of SPV,SPV is 0％～ 20％,then decreases with the absolute value of SPV (P ＜0.05).The difference was significant between the groups (P ＜0.01) in the night time average (SBP,DBP,BP and pulse pressure).Correlation analysis showed that CAVI was significantly correlated with serum creatinine,serum uric acid,NSP,DSP,N-MAP,24h (SBP,BP,pulse pressure),respectively (P ＜ 0.05).Multivariate linear regression analysis showed that age,body mass index,index (nocturnal pulse pressure,daytime pulse pressure,SPV,DPV) is a relatively independent risk factors of arterial stiffness in hypertensive patients.Conclusion:Patients with hypertension BPV maintained at 10％～20％ level of arterial stiffness is lowest,BPV ＜0％ arterial stiffness is highest,at the center of 0％～ 10％.NocturnalBP and pulse pressure are the main factors that affect the BPV.

Abstract:Blood pressure variability is an emerging risk factor for Alzheimer's disease in older adults, independent of average blood pressure levels. Growing evidence suggests increased blood pressure variability is linked to Alzheimer's disease pathophysiology indexed by cerebrospinal fluid and positron emission tomography markers, but relationships with plasma Alzheimer's disease markers have not been investigated. In this cross-sectional study of 54 community-dwelling older adults (aged 55-88, mean age 69.9 [8.2 SD]), elevated blood pressure variability over 5 min was associated with lower levels of plasma Aβ1-42 (standardized ß = - 0.36 [95% CI - 0.61, - 0.12]; p = 0.005; adjusted R2 = 0.28) and Aβ1-42: Aβ1-40 ratio (ß = - 0.49 [95% CI - 0.71, - 0.22]; p < 0.001; adjusted R2 = 0.28), and higher levels of total tau (ß = 0.27 [95% CI 0.01, 0.54]; p = 0.04; adjusted R2 = 0.19) and Ptau181:Aβ1-42 ratio (ß = 0.26 [95% CI 0.02, 0.51]; p = 0.04; adjusted R2 = 0.22). Findings suggest higher blood pressure variability is linked to plasma biomarkers of increased Alzheimer's disease pathophysiology.

Abstract:Objective Short time blood pressure variability (BPV), such as 24-hour ambulatory BPV, was an independent predictor for adverse cardiovascular outcome. We hypothesized that arterial stiffness, as a surrogate of cardiovascular endpoints, could be influenced by BP level as well as by BPV. Methods We performed 24-hour ambulatory BP monitoring in 439 subjects (47.4% male, mean age 53 years) who were not taking antihypertensive drugs. Standard deviation (SD) of 24-hour BP, day-night weighted average SD of daytime and nighttime BP (SDdn), and reading-to-reading average real variability of BP (ARV) were calculated as 24-hour short BPV measures. Carotid-femoral pulse wave velocity (cf-PWV) was obtained by using SphygmoCor system (Australia). Brachial-ankle PWV (ba-PWV) was measured by Omron Colin VP-1000 device (Japan). Results Pearson simple correlation analyses showed that 3 measures of systolic BPV were all positively correlated with cf-PWV and ba-PWV (r = 0.18–0.37, P < 0.001). SDdn and ARV of diastolic BP were significantly correlated with cf-PWV only (P < 0.05). Multivariate regression analyses showed that after adjustment for age, sex, body mass index, 24-hour pulse rat, current smoking and corresponding 24-hour BP level, only ARV of systolic BP was significantly associated with cf-PWV (P = 0.016) and ba-PWV(P = 0.049), whereas other systolic and diastolic BPV measures were not associated with arterial stiffness indexes (P ≥ 0.11). With 1-SD elevation in ARV of systolic BP (2.4mmHg), cf-PWV and ba-PWV increased by 0.17m/s and 0.21m/s, respectively. However, ARV added only 1% to the explained variance of cf-PWV or ba-PWV while accounting for conventional risk factors including 24-hour systolic blood pressure level. Conclusions Increased 24-hour systolic BPV, if measured as ARV, was an independent risk factor for large arterial stiffness, but its contribution was small. It indicated that in clinical practice we should mainly focus on blood pressure level, but at the same time not to ignore BPV.

Abstract:Objective: Cardiovascular regulation is affected by ageing through different mechanisms, including arterial baroreflex dysfunction and increased arterial stiffness, with an impact on blood pressure (BP) and heart rate (HR) variability (V), established risk factors for cardiovascular morbidity and mortality. The aim of this study was to compare BPV and HRV in older vs. younger individuals. Design and method: 1080 hypertensive patients (704 younger, 40-55yrs, YH and 376 older, >=75yrs, OH) underwent 24h BP monitoring (ABPM) from 2018 to 2023. Patients with known dysautonomy, Parkinson or other neurodegenerative diseases were excluded. Short-term BPV indexes including standard deviation (SD), weighted standard deviation (wSD), average real variability (ARV), coefficient of variation (CV), and variability independent of mean (VIM) were estimated over 24h. HRV was assessed by SD and CV of heart rate measured during ABPM. The same analysis was replicated in a sample of 343 untreated individuals from the general population (309 YH; 34 OH). Results: As compared to YH, (mean age: 49.5±4.5yrs), OH (mean age: 80.2±4.5yrs) had significantly higher 24h systolic BPV (SD: 15.7±4.6 vs.13.6±4.0; wSD: 15.5±4.6 vs.13.5±4.0; ARV: 12.3±2.9 vs.10.0±2.7; VIM: 15.8±4.5 vs.13.9±4.0 mm Hg; CV: 12.2±3.5 vs.10.8±3.1%; p<0.001). In contrast, 24h HRV was significantly lower in OH than in YH group (SD: 7.2±3.0 vs. 9.1±3.4 beats/min; CV: 11.5±4.5 vs. 12.8.1±4.5%; p<0.001). Higher tertiles of BPV were associated with higher mean pulse pressure and higher pulsatility (pulse pressure normalized for mean arterial pressure), in both YH and OH. Similar results were obtained when comparing OH vs. YH individuals in the general population sample, both for 24h systolic BPV (SD: 14.7±3.1 vs.12.2±2.8; wSD: 14.9±3.1 vs.12.7 ±3.0; ARV: 11.5±2.1 vs.9.2±2.0; VIM: 13.6±2.5 vs.12.3±2.4 mmHg; CV: 11.4±2.1 vs.10.1±2.0%; p<0.01) and HRV (SD: 8.8±3.2 vs. 11.3±3.4 beats/min; CV: 12.7±4.5 vs. 15.1±4.3%; p<0.01). Conclusions: Increased BPV was associated with decreased HRV in older compared to younger individuals, both in a hypertensive cohort and in a general population sample. The respective contribution to these changes of age-related dysfunction of autonomic cardiovascular modulation and increased arterial stiffness and their prognostic implications deserve further investigation.

Abstract:Augmented blood pressure variability has emerged as a quantity predictive of adverse cardiovascular outcomes. Among the range of intrinsic and extrinsic factors shown to increase night-time, circadian, short-term, and long-term blood pressure variations, the presence and severity of obstructive sleep apnea have emerged as one of the most prevalent and potent. Obstructive sleep apnea alters acutely the normal nocturnal equilibrium between sympathetic and parasympathetic tone, magnifying nocturnal blood pressure oscillations, and induces sustained autonomic aftereffects with the capacity to amplify short-term and intersessional blood pressure variabilities. The object of this brief review is to synthesize the current understanding of the potential interrelations between obstructive sleep apnea, the acute and sustained autonomic disturbances that it elicits, and beat-to-beat blood pressure fluctuation during sleep, nocturnal dipping status, and day-to-day blood pressure variability and the consequences of these perturbations for cardiovascular risk.

Abstract:Objective: To investigate the relationship between arterial stiffness and variability of blood pressure (BPV) in senile patients with hypertension. <br> Methods: A total of 234 senile patients with hypertension were enrolled. The patients were examined by 24h ambulatory blood pressure monitoring (ABPM), BPV was identified by the standard deviation (SD) of blood pressure value. Based on increased percentage of baPWV, the patients were divided into 4 groups: Normal artery group, n=44, Mild arterial stiff group, n=72, Moderate arterial stiff group, n=78 and Severe arterial stiff group, n=40. The 24h ABPM results, BPV, biochemical indexes and their relationship to baPWV were compared among different groups. <br> Results:①The HDL-C level and 24hDBP, dDBP, 24hSBPSD, 24hDBPSD, dSBPSD, dDBPSD, nDBPSD were significantly different among each group, all P<0.05.②The increased percentage of baPWV was positively related to the age and 24hSBPSD, 24hDBPSD, dSBPSD, dDBPSD, nDBPSD, while negatively related to HDL-C level, all P<0.05.③The age and 24hSBPSD, 24hDBPSD, dSBPSD were the independent risk factors for baPWV elevation. <br> Conclusion: The arterial stiffness was closely related to BPV in senile patients with hypertension.
Systemic Endothelial Function, Plasma Xanthine Oxidoreductase Activity, and Blood Pressure Variability in Patients with Stable Coronary Artery Disease

Abstract:Background and Objectives: Although previous studies showed that an activity of xanthine oxidoreductase (XOR), a rate-limiting enzyme in purine metabolism, beyond the serum uric acid level, was associated with the development of coronary artery disease (CAD), the underlying mechanisms are unclear. Because endothelial dysfunction and a greater blood pressure (BP) variability may play a role, we investigated the relations among the endothelial function, XOR, and BP variability. Materials and Methods: This was a post-hoc study using pooled data of patients with a stable CAD from two prospective investigations, in which the systemic endothelial function was assessed with the reactive hyperemia index (RHI) and the XOR activity was measured. The BP variability was evaluated using BP measurements during the three- and four-day hospitalization. Results: A total of 106 patients with a stable CAD undergoing a percutaneous coronary intervention were included. Of the 106 patients, 46 (43.4%) had a systemic endothelial dysfunction (RHI < 1.67). The multivariable analysis identified a higher body mass index (BMI), female gender, and diabetes as factors associated with an endothelial dysfunction. A higher BMI was also related to an elevated XOR activity, in addition to current smoking. No significant correlation was observed between the RHI and XOR activity. Similarly, the in-hospital BP variability was associated with neither the endothelial function nor XOR. Conclusions: Among patients with a stable CAD, several factors were identified as being associated with a systemic endothelial dysfunction or an elevated XOR activity. However, no direct relations between the endothelial function, XOR, and BP variability were found.

Abstract:Journal of the American Heart Association, Ahead of Print. Recent advances in wearable technology through convenient and cuffless systems will enable continuous, noninvasive monitoring of blood pressure (BP), heart rate, and heart rhythm on both longitudinal 24‐hour measurement scales and high‐frequency beat‐to‐beat BP variability and synchronous heart rate variability and changes in underlying heart rhythm. Clinically, BP variability is classified into 4 main types on the basis of the duration of monitoring time: very‐short‐term (beat to beat), short‐term (within 24 hours), medium‐term (within days), and long‐term (over months and years). BP variability is a strong risk factor for cardiovascular diseases, chronic kidney disease, cognitive decline, and mental illness. The diagnostic and therapeutic value of measuring and controlling BP variability may offer critical targets in addition to lowering mean BP in hypertensive populations.
