Abstract:purpose To investigate the diagnostic value of endometrial polyp. Metheds 102 cases of ultrasonogram with endometrial polyp comfirmed by pathology were retrospectively analysed, and compared with hysteroscope. Results 186 cases were diagnosed suspect-edly by elinic, 126 cases were examined by hysteroscope, 102 cases were diagnosed by transvaginal ultrasound. The coincidence rate of transvaginal ultrasound was 80.95%. Conclusion Transvaginal ultrasound have high clinical value to diagnose endometrial polyp.

Clinical Characteristics of Endometrial Polyps in Postmenopausal Women

Abstract:Objective To investigate clinical features of endometrial polyps(EP) in postmenopausal patients and to provide clues for clinical diagnosis and treatment.Methods Based on the pathologic diagnosis, there were 403 postmenopausal EP between June 2005 and October 2014 in our hospital.The clinical data were retrospectively analyzed.Results The percentage of EP accounted for 51.7%(403/779) of endometrial lesions in menopausal patients during the same period.The percentage of clinical symptoms, such as menopausal bleeding, was 43.4%(175/403).Under ultrasound examinations, there were 77.9% (314/403) of cases with inhomogeneous echo and the average thick of endometrium was (0.8±0.4) cm.About 48.4%(195/403) of cases had occupying lesions in the uterine cavity.The coincidence rate between hysteroscopy diagnosis of EP and pathology was 88.6%(357/403).Based on pathology, the percentage of benign and malignant polyps was 93.3%(376/403) and 6.7% (27/403), respectively.Accompanied with EP, the percentages of endometrial simple hyperplasia and complex hyperplasia, atypical hyperplasia, and endometrial carcinoma were 20.3%(61/300), 2.7%(8/300), and 1.0%(3/300), respectively.Conclusions In postmenopausal women, EP is the most common intrauterine lesions.Hysteroscopy is the effective method for EP diagnosis and treatment.It is noted that simultaneous endometrial biopsy is required to avoid misdiagnosis.

Abstract:Endometrial polyps (EPs) are common gynecological diseases among women of childbearing age. They are formed by the proliferation of blood vessels and connective tissue in a portion of the endometrium, resulting in the formation of a polyp-like mass that protrudes into the uterine cavity and varies in size and number. Typically, the body of the uterus is the site of implantation of these polyp-like growths, which are attached to the medial wall of the uterine cavity by a long, thin tip. Irregular vaginal bleeding is the first clinical manifestation of endometrial polyps, and it can often lead to infertility. Studies have shown that the prevalence of endometrial polyps in the female population is approximately 25.0%. With the widespread use of hysteroscopic techniques in clinical practice, knowledge and diagnosis of endometrial polyps have significantly improved. However, pathological examination remains the “gold standard” for final diagnosis in clinical practice. This article aims to review the epidemiology, pathological histology, and pathogenesis of endometrial polyps to reveal their mechanisms and characteristics more accurately and guide further clinical research and treatment. The relevant progress in recent years in this field, both domestically and internationally, is reviewed below.

Abstract:Purpose: Endometrial polyps(EP) are one of the most frequently encountered endometrial abnormalities. This study was designed to use transvaginal sonohysterography(SHG)in diagnosing EP. Materials and Methods: SHG was performed in 39 women with abnormal vaginal bleeding. The results of SHG were compared with that of transvaginal sonography (TVS), and with the pathologic results of uterine explorative curette or hysteroscopy. Results: Pathology confirmed 34 endometial polys, 3 submucous myomas and one endometrial hyperplasia. SHG revealed endometial polys in 32 patients by clearly displaying the shape of uterine cavity and the outline of the lesions. SHG displayed distinctively the extent of endometrium and the sites of lesion attachment in 30 cases, and the blood flow inside the lesion or pedicel in 23. TVS diagnosed endometial polys in 26 cases as hyperechoic nodules of endometrium. Out of 26 cases, 19 had distinctive outlines, 6 showed lesion attachment sites, and 8 had flow signs. TVS could not visualized uterine cavity. The detection rates of SHG and TVS in uterine cavity shape and lesion attachment site were 94.1%, 88.2%, 0% and 17.6% respectively. SHG correctly diagnosed 3 cases of submucous myomas. Conclusion: SHG markedly improved the detection rates of EP lesions, their relations to endometrium and perfusion characteristics of edometrial polyps.

Abstract:Objective To investigate the clinical manifestations,diagnosis and treatment options of polypoid adenomyosis of the uterus. Methods Review pathological diagnosis of polypoid adenomyosis of the clinical and pathological data by department of pathology at our hospital from January 2006 to December 2009. Results Endometrial polypoid adenomyosis is a special kind of benign intrauterine lesions,histological into typical polypoid adenomyosis of endometrium and atypical polypoid adenomyosis. Endometrial polypoid adenomyosis have a malignant tendency. The probability of occurrence of malignant in women aged 50 and above with polypoid adenomyosis is higher than in women under 50 years old. Abnor-mal vaginal bleeding and discharge liquid are the main clinical manifestations. Hysteroscopy has an advantage in the diag-nosis and treatment for polypoid adenomyosis of the uterus. Conclusion Although polypoid adenomyosis is not common in clinical practice,however,tend to have malignant rate,malignant rate in elderly patients with polypoid adenomyoma rather than younger. For uterine cavity with occupation or irregular vaginal bleeding,particularly in patients with postmenopausal vaginal bleeding,positive hysteroscopic examination and pathological examination should be done.

Abstract:Background Endometrial polyps (EPs) occur in approximately 34.9% of infertile women. Transvaginal sonography (TVS) is a routine, non-invasive component of fertility evaluation. Most ultrasonographic studies of EPs have focused on abnormal uterine bleeding; few have assessed EPs in infertile women. Furthermore, no studies have explored endometrial thickness and its correlation with EPs in infertile women. This study aimed to assess transvaginal sonographic assessment of endometrial thickness and its value in diagnosis and prediction of EPs in infertile women.Methods A retrospective study on 314 infertile women was conducted from June to December 2010. After TVS, endometrial biopsies were obtained by hysteroscopy. Pathologically confirmed EPs were taken as the gold standard.Results Based on recognized criteria, TVS had a sensitivity of 37.04%, specificity of 98.71%, positive predictive value of 90.91%, negative predictive value of 81.85%, and accuracy of 82.80% for diagnosing EPs. Mean endometrial thickness was significantly different in patients with and without EPs (P=0.0001). In women in the mid and late-proliferative phase, the endometrial thickness was significantly greater in those with EPs than in those without them (P=0.0001 and 0.024). Receiver operating characteristic analysis showed that endometrial thickness had a sensitivity of 85.2% and specificity of 38% in the diagnosis of EPs, the area under the curve being 0.64. In the mid-proliferative phase, sensitivity was up to 90.9%, the area under the curve being 0.70.Conclusions TVS is poor at detecting EPs in infertile women; however, transvaginal sonographic measurement of endometrial thickness is helpful. It is suggested that the diagnostic value of TVS for EPs in infertile women could be improved by adding the measurement of endometrial thickness to the variables that are routinely assessed. Abstract:Purpose Polypoid endometriosis is an uncommon and distinctive variant of endometriosis, and its detailed histopathological features have been investigated and analysed. Methods A retrospective analysis was performed in 10 cases with polypoid endometrio-sis by comparing clinical data, histopathologic and immunohistochemical changes. Results The patients were 27 to 54 years ( mean 39. 4 years) of age. The most common clinical presentations were a pelvic mass. The intraoperative findings suggested a neoplasm. Sites of involvement in order of frequency included ovary, uterine serosa and cervix. Polypoid, pink, gray or tan masses ranged up to 9 cm in maximal dimension. On microscopic examination, the polypoid masses were composed of an admixture of endometriotic glands and stroma. Most commonly glandular architectural patterns were simple hyperplasia. Tubal epithelial metaplasia was common. The stroma was fibrous, resembling to some extent the stroma of the endometrial polyp and atypical stromal cells can be seen. Nine cases were associated with usual endometriosis. Follow-up data in nine patients indicated that all of patients were alive without evidence of re-sidual disease. The main lesion in the differential diagnosis is a mullerian adenosarcoma. Conclusion Polypoid endometriosis is a rare manifestation of endometriosis, and probably is overgrowth of local ectopic endometrium based on endometriosis. Understanding of such lesions is avoided misdiagnosing for other benign or malignant tumors on clincal, intraoperative or pathologic assesment, and influ-ences the clinical treatment.
