Abstract:BACKGROUND: The aim was to retrospectively analyze the ultrasonographic and clinical characteristics of focal inflammatory masses and malignant masses of salivary gland by using B-mode ultrasound (US) and contrast-enhanced ultrasound (CEUS) for differential analysis. METHODS: The features of US and CEUS were retrospectively analyzed for 19 cases of focal salivary inflammatory masses and 45 cases of malignant salivary masses. All cases were confirmed by pathohistological examination. RESULTS: On B-mode US, the incidence of expansive growth patterns of malignant salivary masses (44.4%, 20/45) was significantly higher than that of focal salivary inflammatory masses (15.8%, 3/19) (p = 0.029). The rate of lymphadenopathy surrounding salivary glands of malignant salivary masses (42.2%, 19/45) was significantly higher than that of focal salivary inflammatory masses (15.8%, 3/19) (p = 0.042). On CEUS, clear enhancement margins were more common in malignant salivary masses (44.4%, 20/45) compared to focal salivary inflammatory masses (15.8%, 3/19) (p = 0.029); Rapid washout was more common in malignant salivary masses (82.2%, 37/45) than focal salivary inflammatory masses (31.6%, 6/19) (p < 0.001). Rapid washout on CEUS and craniocaudal diameter were independent predictive factors in differentiating salivary inflammatory masses and malignant masses according to binary logistic regression analysis. US and CEUS achieved a sensitivity of 80.0%, a specificity of 78.9% and an accuracy of 80.0% for discrimination between salivary inflammatory masses and malignant masses. CONCLUSION: Therefore, a multimodal ultrasonographic pathway combining clinical manifestations, B-mode US and CEUS was needed to differentiate between salivary focal inflammatory masses and malignancies to avoid unnecessary biopsies.

Abstract:OBJECTIVE:To investigate the value of the signal intensity on T2-weighted magnetic resonance (MR) imaging using quantitative analysis in the differentiation of parotid tumors.MATERIALS AND METHODS:MR data of 80 pleomorphic adenomas (PAs), 68 Warthin tumors (WTs), and 34 malignant tumors (MTs) confirmed by surgery and histology were retrospectively analyzed. The signal intensities of tumor, normal parotid gland, spinal cord, and buccal subcutaneous fat were measured, and the signal intensity ratios (SIRs) between the tumor and the three references were calculated. Receiver operating characteristic curve was used to determine the optimal threshold and diagnostic efficiency of SIR for differentiating PAs, WTs, and MTs.RESULTS:The area under the curve (AUC) of tumor to parotid gland SIR (SIRP), tumor to spinal cord SIR (SIRC), and tumor to buccal subcutaneous fat SIR (SIRF) for differentiating PAs and WTs was 0.922, 0.918, and 0.934, respectively. The sensitivity and specificity at an optimal SIR threshold were 86.3% and 91.2%, 80.0% and 97.1%, and 85.0% and 94.1%, respectively. The AUC of SIRP, SIRC, and SIRF for distinguishing PAs from MTs was 0.793, 0.802, and 0.774, respectively. The sensitivity and specificity at an optimal SIR threshold was 86.3% and 61.8%, 80.0% and 73.5%, and 82.5% and 73.5%, respectively. The AUC of SIRP, SIRC, and SIRF for distinguishing WTs from MTs was 0.716, 0.709, and 0.759, respectively. The sensitivity and specificity at an optimal SIR threshold were 61.8% and 82.4%, 55.9% and 82.4%, and 64.7% and 86.8%, respectively.CONCLUSION:SIRP, SIRC, and SIRF on T2-weighted MR images had high diagnostic efficiency for differentiating between PAs and WTs, while SIRP and SIRC for differentiating between PAs and MTs, and SIRF for differentiating between WTs and MTs had relatively high diagnostic efficiency.
Malignant or benign salivary gland lesions: predicting with MR imaging

Abstract:Objective To evaluate if MR imaging can be used to diagnosis the nature of the salivary gland lesions, and the accuracy of this modality. Methods Sixty patients with salivary gland diseases that included 7 cases of inflammatory lesions were analyzed respectively. All cases were diagnosed pathologically. A logistic regression model was used to determine which MR imaging features were significant factor for predicting the lesion being benign or malignant for each patient. Results Logistic regression model analysis in 60 cases revealed that metastasis was only significant MR characteristics for determining a lesion that was benign or malignant (P0.001). Its accuracy, sensitivity and specificity for predicting malignancy were 97.7%, 43.8%, 83.3%, respectively. However, the analysis in 53 cases (excluding inflammatory cases) demonstrated that metastases (P0.001) and infiltration (P0.001) were significant factors in predicting a malignant lesion, respectively. Their accuracy, sensitivity and specificity for predicting malignancy were 100%, 43.8%, 83.0% and 100%, 50%, 84.9%, respectively. When combining these two predictors to diagnosis malignancy, their accuracy, sensitivity and specificity could improve to 92.5%, 75%, and 100%. Conclusion Lymph node swelling and infiltration of tumor are useful factors to differentiate the malignant or benign lesions.
Use of Computed Tomography-Based Texture Analysis to Differentiate Benign From Malignant Salivary Gland Lesions

Abstract:Objective: Salivary gland lesions show overlapping morphological findings and types of time/intensity curves. This research aimed to evaluate the role of 2-phase multislice spiral computed tomography (MSCT) texture analysis in differentiating between benign and malignant salivary gland lesions. Methods: In this prospective study, MSCT was carried out on 90 patients. Each lesion was segmented on axial computed tomography (CT) images manually, and 33 texture features and morphological CT features were assessed. Logistic regression analysis was used to confirm predictors of malignancy ( P < 0.05 was considered to be statistically significant), followed by receiver operating characteristics analysis to assess the diagnostic performance. Results: Univariate logistic regression analysis revealed that morphological CT features (shape, size, and invasion of adjacent tissues) and 17 CT texture parameters had significant differences between benign and malignant lesions ( P < 0.05). Multivariate binary logistic regression demonstrated that shape, invasion of adjacent tissues, entropy, and inverse difference moment were independent factors for malignant tumors. The diagnostic accuracy values of multivariate binary logistic models based on morphological parameters, CT texture features, and a combination of both were 87.8%, 90%, and 93.3%, respectively. Conclusions: Two-phase MSCT texture analysis was conducive to differentiating between malignant and benign neoplasms in the salivary gland, especially when combined with morphological CT features.

Abstract:Objective: To determine the diagnostic value of combining conventional MRI, diffusion-weighted imaging (DWI) and dynamic contrast enhanced MRI (DCE-MRI) in salivary gland tumors. Methods: 45 patients with salivary gland tumors were evaluated with conventional MRI, DWI and DCE-MRI prior to surgery and confirmed by pathologic findings. The apparent diffusion coefficient (ADC) was calculated from DWI that was obtained with a factor of 0 and 1000 s mm(-2). A time-intensity curve (TIC) was obtained from DCE-MRI. Results: In conventional MRI, benign tumors often showed well-defined and clear margins, malignant tumors showed irregular margins or infiltration into the surrounding tissue. There were significant differences with regard to the ADC values between pleomorphic adenoma (1.72 +/- 0.29 x 10(-3) mm(2) s(-1)) and malignant tumors (0.95 +/- 0.09 x 10(-3) mm(2) s(-1) p < 0.05) and between adenolymphoma (0.74 +/- 0.05 x 10(-3) mm(2 )s(-1) and malignant tumors (p < 0.05). However, there was no significant differences in term of the ADC values between benign tumors (1.33 +/- 0.52 x 10(-3) mm(2) s(-1)) and malignant tumors. DCE-MRI showed benign tumors with A-type, B-type and D-type of TICs, and the malignant tumors with C-type TICs. A combination of all of these parameters yielded sensitivity, specificity, accuracy, and positive and negative predictive values of 90%, 97%, 95%, 90 and 97%, respectively. Conclusion: An evaluation combining MRI morphologic findings and functional MRI (ADCs and TIC) appears to be useful in differentiating benign from malignant tumors in salivary gland tumors. Advances in knowledge: The study firstly dealt with the combination of conventional MRI, DWI-MRI with DCE-MRI in salivary gland tumors.
Synthetic MRI in differentiating benign from metastatic retropharyngeal lymph node: combination with diffusion-weighted imaging

Abstract:Objectives: This study aimed to evaluate the synthetic MRI (syMRI), its combination with diffusion-weighted imaging (DWI), and morphological features for discriminating benign from metastatic retropharyngeal lymph nodes (RLNs). Methods: Fifty-eight patients with a total of 63 RLNs (21 benign and 42 metastatic) were enrolled. The mean and standard deviation of syMRI-derived relaxometry parameters (T1, T2, PD; T1SD, T2SD, PDSD) were obtained from two different regions of interest (namely, partial-lesion and full-lesion ROI). The parameters derived from benign and metastatic RLNs were compared using Student's t or chi-square tests. Logistic regression analysis was used to construct a multi-parameter model of syMRI, syMRI + DWI, and syMRI + DWI + morphological features. Areas under the curve (AUC) were compared using the DeLong test to determine the best diagnostic approach. Results: Benign RLNs had significantly higher T1, T2, PD, and T1SD values compared with metastatic RLNs in both partial-lesion and full-lesion ROI (all p < 0.05). The T1SD obtained from full-lesion ROI showed the best diagnostic performance among all syMRI-derived single parameters. The AUC of combined syMRI multiple parameters (T1, T2, PD, T1SD) were higher than those of any single parameter from syMRI. The combination of synthetic MRI and DWI can improve the AUC regardless of ROI delineation. Furthermore, the combination of synthetic MRI, DWI-derived quantitative parameters, and morphological features can significantly improve the overall diagnostic performance. Conclusions: The value of syMRI has been validated in differential diagnosis of benign and metastatic RLNs, and syMRI + DWI + morphological features can further improve the diagnostic efficiency for discriminating these two entities. Abstract:The algorithm for imaging the salivary glands depends on the clinical scenario with which the patient presents to the clinician. Because of the importance of identifying small calculi in the gland or salivary duct as the cause of the symptom complex, nonenhanced computed tomography is often the best initial study for the evaluation of the painful gland. If an infiltrative neoplasm is highly suspected, nonenhanced and enhanced magnetic resonance (MR) imaging may be superior in demonstrating perineural, meningeal, and skull base invasion. Sialography is reserved for the evaluation of chronic sialadenitides unrelated to sialolithiasis. Thin-section MR techniques for MR sialography may soon replace conventional sialography.

Abstract:Purpose: To assess the diagnostic potential of the synthetic MRI (SyMRI) for differentiating Warthin's tumors (WT) from pleomorphic adenomas (PA). Materials and methods: Forty-nine individuals with parotid gland tumors (PA, n = 23; WT, n = 26) were recruited. Using two distinct regions of interest (ROI), SyMRI quantitative parameters of lesions were calculated, including mean and standard deviation (T1, T2, PD, T1sd, T2sd, and PDsd). Meanwhile, T1ratio, T2ratio, and PDratio (lesion/masseter muscle) were calculated based on the mean SyMRI quantitative parameters of masseter muscle (T1, T2, PD). Using the independent samples t test, we compared PA and WT parameters, while comparing the areas under the curve (AUC) using the DeLong's test. A multi-parameter SyMRI model was constructed using logistic regression analysis. Results: In PA, the T1, T1sd, T2, PD, T1ratio, T2ratio, and PDratio derived from full and partial lesion ROIs were significantly higher than in WT. According to the receiver operating curve analysis, the AUC of the quantitative parameters derived from full-lesion and partial-lesion ROIs ranged from 0.722 to 0.983 for differentiating PA from WT. T1 values derived from partial-lesion ROI delineation demonstrated the best diagnostic performance among all single parameters, achieving an AUC of 0.983. Using 1322 ms as a cutoff value, the sensitivity, specificity, and accuracy were 88.46%, 100% and 93.88%, respectively. Conclusion: The SyMRI-derived quantitative parameters demonstrated excellent performance for discriminating PA from WT in the parotid gland.

Abstract:Defining the exact histological features of salivary gland malignancies before treatment remains an unsolved problem that compromises the ability to tailor further therapeutic steps individually. Radiomics, a new methodology to extract quantitative information from medical images, could contribute to characterizing the individual cancer phenotype already before treatment in a fast and non-invasive way. Consequently, the standardization and implementation of radiomic analysis in the clinical routine work to predict histology of salivary gland cancer (SGC) could also provide improvements in clinical decision-making. In this study, we aimed to investigate the potential of radiomic features as imaging biomarker to distinguish between high grade and low-grade salivary gland malignancies. We have also investigated the effect of image and feature level harmonization on the performance of radiomic models. For this study, our dual center cohort consisted of 126 patients, with histologically proven SGC, who underwent curative-intent treatment in two tertiary oncology centers. We extracted and analyzed the radiomics features of 120 pre-therapeutic MRI images with gadolinium (T1 sequences), and correlated those with the definitive post-operative histology. In our study the best radiomic model achieved average AUC of 0.66 and balanced accuracy of 0.63. According to the results, there is significant difference between the performance of models based on MRI intensity normalized images + harmonized features and other models ( p value < 0.05) which indicates that in case of dealing with heterogeneous dataset, applying the harmonization methods is beneficial. Among radiomic features minimum intensity from first order, and gray level-variance from texture category were frequently selected during multivariate analysis which indicate the potential of these features as being used as imaging biomarker. The present bicentric study presents for the first time the feasibility of implementing MR-based, handcrafted radiomics, based on T1 contrast-enhanced sequences and the ComBat harmonization method in an effort to predict the formal grading of salivary gland carcinoma with satisfactory performance.

Abstract:Magnetic resonance imaging is most frequently used for pre-surgical diagnosis and follow-up in salivary gland neoplasms. As for traditional techniques, dynamic contrast-enhanced and diffusion-weighted imaging are two magnetic resonance sequences commonly used in clinic, in spite of their relatively lower sensitivity and specificity. Recently, a novel magnetic resonance technique, amide proton transfer-MRI (APT-MRI), gets a lot of attention due to its special imaging principle and higher sensitivity and specificity for many human tumors diagnosis. Moreover, some studies applied this new MRI technique successfully to the diagnosis and differential diagnosis of salivary gland neoplasms. In this review, we are going to focus on the basic principle of APT-MRI and its clinical applications in salivary gland neoplasms, with a comparison of traditional MRI sequences. We will also give some prospects based on these preliminary applications in APT-MRI diagnosis for salivary gland neoplasms.
Differentiation of salivary gland tumors using diffusion-weighted image-based virtual MR elastography: A pilot study

Abstract:Abstract Objectives Differentiation among benign salivary gland tumors, Warthin tumors, and malignant salivary gland tumors is crucial to treatment planning and predicting patient prognosis. However, differentiation of those tumors using imaging findings remains difficult. This study evaluated the usefulness of elasticity determined from Diffusion-weighted image (DWI)-based virtual MR elastography (MRE) compared with conventional MRI findings in differentiating the tumors. Methods This study included 17 benign salivary gland tumors, six Warthin tumors, and 11 malignant salivary gland tumors scanned on neck MRI. The long and short diameters, T1 and T2 signal intensities, tumor margins, ADC values, and elasticity from DWI-based virtual MRE of the tumors were evaluated. The interobserver agreement in measuring tumor elasticity and the ROC curves were also assessed. Results The long and short diameters and the T1 and T2 signal intensities showed no significant difference among the three tumor groups. Tumor margins and the mean ADC values showed significant differences among some tumor groups. The elasticity from virtual MRE showed significant differences among all three tumor groups and the interobserver agreement was excellent. The area under the ROC curves of the elasticity were higher than those of tumor margins and mean ADC values. Conclusion Elasticity values based on DWI-based virtual MRE of benign salivary gland tumors, Warthin tumors, and malignant salivary gland tumors were significantly different. The elasticity of Warthin tumors was the highest and that of benign tumors was the lowest. The elasticity from DWI-based virtual MRE may aid in the differential diagnosis of salivary gland tumors.

Abstract:Background: Preoperative classification of head and neck (HN) tumors remains challenging, especially distinguishing early cancerogenic masses from benign lesions. Synthetic MRI offers a new way for quantitative analysis of tumors. The present study investigated the application of synthetic MRI and stimulus and fast spin echo diffusion-weighted imaging with periodically rotated overlapping parallel lines with enhanced reconstruction (FSE-PROPELLER DWI) to differentiate malignant from benign HN tumors. Materials and methods: Forty-eight patients with pathologically confirmed HN tumors were retrospectively recruited between August 2022 and October 2022. The patients were divided into malignant (n = 28) and benign (n = 20) groups. All patients were scanned using synthetic MRI and FSE-PROPELLER DWI. T1, T2, and proton density (PD) values were acquired on the synthetic MRI and ADC values on the FSE-PROPELLER DWI. Results: Benign tumors (ADC: 2.03 ± 0.31 × 10 -3 mm 2 /s, T1: 1741.13 ± 662.64 ms, T2: 157.43 ± 72.23 ms) showed higher ADC, T1, and T2 values compared to malignant tumors (ADC: 1.46 ± 0.37 × 10 -3 mm 2 /s, T1: 1390.06 ± 241.09 ms, T2: 97.64 ± 14.91 ms) (all P 0.55; AUC, 0.950) had optimal diagnostic performance vs. T1 (cut-off value, ≤ 1675.84 ms; AUC, 0.698), T2 (cut-off value, ≤ 113.24 ms; AUC, 0.855) and PD (cut off value, > 80.67 pu; AUC, 0.568) alone in differentiating malignant from benign lesions (all P<0.05); yet, the difference in AUC between ADC and T2+ADC or T2 did not reach statistical significance. Conclusion: Synthetic MRI and FSE-PROPELLER DWI can quantitatively differentiate malignant from benign HN tumors. Abstract:The parotid glands are the largest of the major salivary glands. They can harbour both benign and malignant tumours. Preoperative work-up relies on MR images and fine needle aspiration biopsy, but these diagnostic tools have low sensitivity and specificity, often leading to surgery for diagnostic purposes. The aim of this paper is (1) to develop a machine learning algorithm based on MR images characteristics to automatically classify parotid gland tumours and (2) compare its results with the diagnoses of junior and senior radiologists in order to evaluate its utility in routine practice. While automatic algorithms applied to parotid tumours classification have been developed in the past, we believe that our study is one of the first to leverage four different MRI sequences and propose a comparison with clinicians. In this study, we leverage data coming from a cohort of 134 patients treated for benign or malignant parotid tumours. Using radiomics extracted from the MR images of the gland, we train a random forest and a logistic regression to predict the corresponding histopathological subtypes. On the test set, the best results are given by the random forest: we obtain a 0.720 accuracy, a 0.860 specificity, and a 0.720 sensitivity over all histopathological subtypes, with an average AUC of 0.838. When considering the discrimination between benign and malignant tumours, the algorithm results in a 0.760 accuracy and a 0.769 AUC, both on test set. Moreover, the clinical experiment shows that our model helps to improve diagnostic abilities of junior radiologists as their sensitivity and accuracy raised by 6 % when using our proposed method. This algorithm may be useful for training of physicians. Radiomics with a machine learning algorithm may help improve discrimination between benign and malignant parotid tumours, decreasing the need for diagnostic surgery. Further studies are warranted to validate our algorithm for routine use.
Diagnostic efficacy of diffusion-weighted imaging and semiquantitative and quantitative dynamic contrast-enhanced magnetic resonance imaging in salivary gland tumors

Abstract:Background: Increased use of functional magnetic resonance imaging (MRI) methods such as diffusion-weighted imaging (DWI) and dynamic contrast-enhanced (DCE) MRI consisting of sequential contrast series, allows us to obtain more information on the microstructure, cellularity, interstitial distance, and vascularity of tumors, which has increased the discrimination power for benign and malignant salivary gland tumors (SGTs). In the last few years, quantitative DCE MRI data containing T1 perfusion parameters (Ktrans, Kep and Ve), were reported to contribute to the differentiation of benign or malignant subtypes in SGTs. Aim: To evaluate the diagnostic efficacy of DWI and semiquantitative and quantitative perfusion MRI parameters in SGTs. Methods: Diffusion MRI [apparent diffusion coefficient (ADC) value] with a 1.5 T MR machine, semiquantitative perfusion MRI [time intensity curve (TIC) pattern], and quantitative perfusion MRI examinations (Ktrans, Kep and Ve) of 73 tumors in 67 patients with histopathological diagnosis performed from 2017 to 2021 were retrospectively evaluated. In the ADC value and semiquantitative perfusion MRI measurements, cystic components of the tumors were not considered, and the region of interest (ROI) was manually placed through the widest axial section of the tumor. TIC patterns were divided into four groups: Type A = Tpeak > 120 s; type B = Tpeak ≤ 120 s, washout ratio (WR) ≥ 30%; type C = Tpeak ≤ 120 s, WR < 30%; and type D = flat TIC. For the quantitative perfusion MRI analysis, a 3D ROI was placed in the largest solid component of the tumor, and the Ktrans, Kep and Ve values were automatically generated. Results: The majority of SGTs were located in the parotid glands (86.3%). Of all the SGTs, 68.5% were benign and 31.5% were malignant. Significant differences were found for ADC values among pleomorphic adenomas (PMAs), Warthin's tumors (WTs), and malignant tumors (MTs) (P < 0.001). PMAs had type A and WTs had type B TIC pattern while the vast majority of MTs and other benign tumors (OBTs) (54.5% and 45.5%, respectively) displayed type C TIC pattern. PMAs showed no washout, while the highest mean WR was observed in WTs (59% ± 11%). Ktrans values of PMAs, WTs, OBTs, and MTs were not significantly different. Kep values of PMAs and WTs were significantly different from those of OBTs and MTs. Mean Ve value of WTs was significantly different from those of PMAs, OBTs, and MTs (P < 0.001). Conclusion: The use of quantitative DCE parameters along with diffusion MRI and semiquantitative contrast-enhanced MRI in SGTs could improve the diagnostic accuracy.
Mucosa-associated lymphoid tissue lymphoma of the salivary glands: MR imaging findings including diffusion-weighted imaging

Abstract:Purpose: The purpose of this study was to describe MR findings including diffusion-weighted (DW) imaging findings in patients with mucosa-associated lymphoid tissue (MALT) lymphoma of the salivary glands. Materials and methods: Ten patients with histologically proven MALT lymphoma of the salivary glands were included. All patients underwent 1.5-T MR imaging, six of the ten underwent DW imaging, nine underwent CT, and eight (18)F-fluorodeoxyglucose (FDG) PET/CT. MR images were reviewed for numbers, locations, sizes, MR imaging characteristics, and apparent diffusion coefficients (ADCs). Calcium deposition and maximum standardized uptake values (SUVmax) were also assessed. Results: Twenty-six tumors, ranging in number from 1 to 5 (mean, 2.6), were identified. Nine patients had tumors in the parotid glands and one in the submandibular glands. Tumors were found bilaterally in 7 patients and unilaterally in three. Tumors ranged in size from 0.6 to 5.5cm (mean, 1.8cm). Ten (38%) tumors had intratumoral cystic formations and 8 (31%) had ill-demarcated margins. DW images showed hyperintensity with extremely low ADCs (range, 0.48-0.82 [×10(-3)mm(2)/s]; mean, 0.64) for solid components of all 19 tumors in the 6 examined patients. Calcium deposition was found in one (4%) tumor on CT. SUVmax variously ranged from 1.3 to 17.7 (mean, 6.3). Conclusion: Salivary gland MALT lymphomas were often found bilaterally and were occasionally accompanied by intratumoral cystic formations and ill-demarcated margins. DW imaging may play a supplementary role in the diagnosis of lymphoma, because it showed restricted water molecule diffusion, whereas PET/CT showed indeterminate findings.

Quantitative Investigation Of Solitary Pulmonary Nodules: Dynamic Contrast-Enhanced Mri And Histopathologic Analysis

Abstract:OBJECTIVE. The purposes of this study were to analyze the relation between enhancement patterns on dynamic enhanced MRI and histologic microvessel patterns of solitary pulmonary nodules (SPNs) and to address the topic of false-positive findings in differentiating SPNs with dynamic MRI.SUBJECTS AND METHODS. Sixty-eight patients with 68 pathologically proven SPNs (diameter <= 30 mm) underwent dynamic 1.5-T MRI. On time-signal intensity curves generated after bolus injection of contrast material, steepest slope, peak height, and enhancement ratios of signal intensity at the first, second, and fourth minutes were calculated. The relation between dynamic MRI values and microvessel density was analyzed. The morphologic differences between malignant SPNs and active inflammatory SPNs also were analyzed. Threshold dynamic MRI values for differential diagnosis were determined.RESULTS. The dynamic MRI values of benign SPNs were significantly lower than those of the other SPNs (p < 0.01). The enhancement ratio at the fourth minute for active inflammatory SPNs was significantly higher than that of malignant SPNs (p < 0.01). A high correlation coefficient (r = 0.87, p < 0.001) was found between steepest slope and microvessel density. With steepest slope 1.5%/s or less, benign SPNs were clearly differentiated from other SPNs. With enhancement ratio at the fourth minute 65% or less, malignant SPNs were differentiated from active inflammatory SPNs with high sensitivity (93%) and high specificity (100%).CONCLUSION. Dynamic MRI values reflect the quantitative and morphologic characteristics of microvessels in SPNs and are a useful tool for differentiating SPNs with little overlap.
Evaluation of the differentiation of benign and malignant breast lesions using synthetic relaxometry and the Kaiser score

Abstract:Objective: To investigate whether there is added value of quantitative parameters from synthetic magnetic resonance imaging (SyMRI) as a complement to the Kaiser score (KS) to differentiate benign and malignant breast lesions. Materials and methods: In this single-institution study, 122 patients who underwent breast MRI from March 2020 to May 2021 were retrospectively analyzed. SyMRI and dynamic contrast-enhanced MRI were performed using a 3.0-T system. Two experienced radiologists independently assigned the KS and measured the quantitative values of T1 relaxation time (T1), T2 relaxation time (T2), and proton density (PD) from SyMRI. Pathology was regarded as the gold standard. The diagnostic values were compared using the appropriate statistical tests. Results: There were 122 lesions (86 malignant and 36 benign) in 122 women. The T1 value was identified as the only independent factor for the differentiation of malignant and benign lesions. The diagnostic accuracy of incorporating the T1 into the KS protocol (T1+KS) was 95.1% and 92.1% for all lesions (ALL) and The American College of Radiology (ACR) Breast Imaging Reporting and Data System (BI-RADS) category 4 lesions, respectively, which was significantly higher than that of either T1 (ALL: 82.8%, P = 0.0001; BI-RADS 4: 78.9%, P = 0.002) or KS (ALL: 90.2%, P = 0.031; BI-RADS 4: 84.2%, P = 0.031) alone. The sensitivity and specificity of T1+KS were also higher than those of the T1 or KS alone. The combined diagnosis could have avoided another 15.6% biopsies compared with using KS alone. Conclusions: Incorporating T1 into the KS protocol improved both the sensitivity and specificity to differentiate benign and malignant breast lesions, thus avoiding unnecessary invasive procedures.
