Abstract:BACKGROUND: The aim was to retrospectively analyze the ultrasonographic and clinical characteristics of focal inflammatory masses and malignant masses of salivary gland by using B-mode ultrasound (US) and contrast-enhanced ultrasound (CEUS) for differential analysis. METHODS: The features of US and CEUS were retrospectively analyzed for 19 cases of focal salivary inflammatory masses and 45 cases of malignant salivary masses. All cases were confirmed by pathohistological examination. RESULTS: On B-mode US, the incidence of expansive growth patterns of malignant salivary masses (44.4%, 20/45) was significantly higher than that of focal salivary inflammatory masses (15.8%, 3/19) (p = 0.029). The rate of lymphadenopathy surrounding salivary glands of malignant salivary masses (42.2%, 19/45) was significantly higher than that of focal salivary inflammatory masses (15.8%, 3/19) (p = 0.042). On CEUS, clear enhancement margins were more common in malignant salivary masses (44.4%, 20/45) compared to focal salivary inflammatory masses (15.8%, 3/19) (p = 0.029); Rapid washout was more common in malignant salivary masses (82.2%, 37/45) than focal salivary inflammatory masses (31.6%, 6/19) (p < 0.001). Rapid washout on CEUS and craniocaudal diameter were independent predictive factors in differentiating salivary inflammatory masses and malignant masses according to binary logistic regression analysis. US and CEUS achieved a sensitivity of 80.0%, a specificity of 78.9% and an accuracy of 80.0% for discrimination between salivary inflammatory masses and malignant masses. CONCLUSION: Therefore, a multimodal ultrasonographic pathway combining clinical manifestations, B-mode US and CEUS was needed to differentiate between salivary focal inflammatory masses and malignancies to avoid unnecessary biopsies.
Diagnostic Value of CT- and MRI-Based Texture Analysis and Imaging Findings for Grading Cartilaginous Tumors in Long Bones.

Abstract:OBJECTIVE:To confirm the diagnostic performance of computed tomography (CT)-based texture analysis (CTTA) and magnetic resonance imaging (MRI)-based texture analysis for grading cartilaginous tumors in long bones and to compare these findings to radiological features.MATERIALS AND METHODS:Twenty-nine patients with enchondromas, 20 with low-grade chondrosarcomas and 16 with high-grade chondrosarcomas were included retrospectively. Clinical and radiological information and 9 histogram features extracted from CT, T1WI, and T2WI were evaluated. Binary logistic regression analysis was performed to determine predictive factors for grading cartilaginous tumors and to establish diagnostic models. Another 26 patients were included to validate each model. Receiver operating characteristic (ROC) curves were generated, and accuracy rate, sensitivity, specificity and positive/negative predictive values (PPV/NPV) were calculated.RESULTS:On imaging, endosteal scalloping, cortical destruction and calcification shape were predictive for grading cartilaginous tumors. For texture analysis, variance, mean, perc.01%, perc.10%, perc.99% and kurtosis were extracted after multivariate analysis. To differentiate benign cartilaginous tumors from low-grade chondrosarcomas, the imaging features model reached the highest accuracy rate (83.7%) and AUC (0.841), with a sensitivity of 75% and specificity of 93.1%. The CTTA feature model best distinguished low-grade and high-grade chondrosarcomas, with accuracies of 71.9%, and 80% in the training and validation groups, respectively; T1-TA and T2-TA could not distinguish them well. We found that the imaging feature model best differentiated benign and malignant cartilaginous tumors, with an accuracy rate of 89.2%, followed by the T1-TA feature model (80.4%).CONCLUSIONS:The imaging feature model and CTTA- or MRI-based texture analysis have the potential to differentiate cartilaginous tumors in long bones by grade. MRI-based texture analysis failed to grade chondrosarcomas.
Texture analysis of diffusion kurtosis imaging for differentiating malignant from benign sinonasal lesions: added value to conventional imaging features

Abstract:Objectives: To evaluate the performance of texture anal-ysis (TA) of diffusion kurtosis imaging (DKI) in differen-tiating malignant from benign sinonasal lesions, and its added value to the conventional imaging features.Methods: Fifty -eight patients with malignant and 40 patients with benign sinonasal lesions were retro-spectively enrolled. Conventional CT and MRI features were reviewed. Texture parameters were obtained and compared between two groups. Multivariate logistic regression analysis was used to identify the most valu-able variables. Receiver operating characteristic curves were performed to assess the differentiating perfor-mance of independent variables and their combination.Results: There were significant differences in tumor necrosis, bone erosion and soft tissue invasion between the two groups (all p < 0.05). There were significant differences in the 10th and entropy of Apparent diffu-sion coefficient map, the mean, 10th and entropy of D map, the mean and 90th of K map between the two groups (all p < 0.002). The bone erosion, entropy of D, and mean of K were independent variables associated with malignant tumors. Receiver operating character-istic analyses indicated that the combination of three features possessed better differentiating performance than bone erosion alone (p = 0.003).Conclusion: TA of DKI could supply incremental value to conventional imaging features for pre-operative differ-ential diagnosis between benign and malignant sinon-asal lesions. Advances in knowledge: The present study is the first to combine conventional imaging features and the TA of DKI in the differential diagnosis between benign and malignant sinonasal lesions. Our findings suggest that TA of DKI could supply incremental value to conven-tional imaging features.
Machine Learning on Ultrasound Texture Analysis Data for Characterizing of Salivary Glandular Tumors: A Feasibility Study

Abstract:Background: Objective quantitative texture characteristics may be helpful in salivary glandular tumor differential diagnosis. This study uses machine learning (ML) to explore and validate the performance of ultrasound (US) texture features in diagnosing salivary glandular tumors. Material and methods: 122 patients with salivary glandular tumors, including 71 benign and 51 malignant tumors, are enrolled. Representative brightness mode US pictures are selected for further Gray Level Co-occurrence Matrix (GLCM) texture analysis. We use a t-test to test the significance and use the receiver operating characteristic curve method to find the optimal cut-point for these significant features. After splitting 80% of the data into a training set and 20% data into a testing set, we use five machine learning models, k-nearest Neighbors (kNN), Naïve Bayes, Logistic regression, Artificial Neural Networks (ANNs) and supportive vector machine (SVM), to explore and validate the performance of US GLCM texture features in diagnosing salivary glandular tumors. Results: This study includes 49 female and 73 male patients, with a mean age of 53 years old, ranging from 21 to 93. We find that six GLCM texture features (contrast, inverse difference movement, entropy, dissimilarity, inverse difference and difference entropy) are significantly different between benign and malignant tumors (p < 0.05). In ML, the overall accuracy rates are 74.3% (95%CI: 59.8-88.8%), 94.3% (86.6-100%), 72% (54-89%), 84% (69.5-97.3%) and 73.5% (58.7-88.4%) for kNN, Naïve Bayes, Logistic regression, a one-node ANN and SVM, respectively. Conclusions: US texture analysis with ML has potential as an objective and valuable tool to make a differential diagnosis between benign and malignant salivary gland tumors.
Characterization of Texture Features Between Carcinomatous and Normal Wall Tissue in MR Bladder Imaging

Abstract:Objective: To distinguish bladder wall tissue from bladder cancer tissue using the texture analysis in MR images. Method: with collected MRI scans for bladder imaging, the ROI (region of interest) area was first selected manually. Second, the texture features of bladder cancer and bladder wall tissue, such as entropy, uniformity et al, were calculated within the ROI. Third, statistical analysis was applied to these features to reflect their significance. Results: All the texture features were analyzed by t-test. Entropy, uniformity, smoothness, relation and contrast show significant differences between the two groups, while the differences for others are not significant. Conclusion: The preliminary results demonstrate significant differences in some texture features of bladder cancer compared to normal wall tissue. Additionally, more properties of bladder cancer might be obtained directly from patient's MRI scanning.
Quantitative Analysis and Pathological Basis of Signal Intensity on T2-Weighted MR Images in Benign and Malignant Parotid Tumors.

Abstract:OBJECTIVE:To investigate the value of the signal intensity on T2-weighted magnetic resonance (MR) imaging using quantitative analysis in the differentiation of parotid tumors.MATERIALS AND METHODS:MR data of 80 pleomorphic adenomas (PAs), 68 Warthin tumors (WTs), and 34 malignant tumors (MTs) confirmed by surgery and histology were retrospectively analyzed. The signal intensities of tumor, normal parotid gland, spinal cord, and buccal subcutaneous fat were measured, and the signal intensity ratios (SIRs) between the tumor and the three references were calculated. Receiver operating characteristic curve was used to determine the optimal threshold and diagnostic efficiency of SIR for differentiating PAs, WTs, and MTs.RESULTS:The area under the curve (AUC) of tumor to parotid gland SIR (SIRP), tumor to spinal cord SIR (SIRC), and tumor to buccal subcutaneous fat SIR (SIRF) for differentiating PAs and WTs was 0.922, 0.918, and 0.934, respectively. The sensitivity and specificity at an optimal SIR threshold were 86.3% and 91.2%, 80.0% and 97.1%, and 85.0% and 94.1%, respectively. The AUC of SIRP, SIRC, and SIRF for distinguishing PAs from MTs was 0.793, 0.802, and 0.774, respectively. The sensitivity and specificity at an optimal SIR threshold was 86.3% and 61.8%, 80.0% and 73.5%, and 82.5% and 73.5%, respectively. The AUC of SIRP, SIRC, and SIRF for distinguishing WTs from MTs was 0.716, 0.709, and 0.759, respectively. The sensitivity and specificity at an optimal SIR threshold were 61.8% and 82.4%, 55.9% and 82.4%, and 64.7% and 86.8%, respectively.CONCLUSION:SIRP, SIRC, and SIRF on T2-weighted MR images had high diagnostic efficiency for differentiating between PAs and WTs, while SIRP and SIRC for differentiating between PAs and MTs, and SIRF for differentiating between WTs and MTs had relatively high diagnostic efficiency.

Abstract:PURPOSE:To assess the utility of texture analysis for predicting the pathological degree of differentiation of pancreatic carcinoma (PC).METHODS:Eighty-three patients with PC who went through postoperative pathology diagnose and CT examination were selected at Anhui Provincial Hospital. Among them, 34 cases were moderately differentiated, 13 cases were poorly differentiated, and 36 cases were moderately poorly differentiated. The images in the arterial and venous phase (VP) with the lesions at their largest cross section were selected to manually outline the region of interest (ROI) to delineate lesions using open-source software. A total of 396 features were extracted from the ROI using AK software. Spearman correlation analysis and random forest selection by filter (rfSBF) in the caret package of R studio were used to select the discriminating features. The receiver operating characteristic ROC analysis was used to evaluate their discriminative performance.RESULTS:Twelve and six features were selected in the arterial and VPs, respectively. The areas under the ROC curve (AUC) in the arterial phase (AP) for diagnosing poorly differentiated, moderately differentiated and moderate-poorly differentiated cases were 0.80, 1, and 0.80 in the training group and 0.77, 1, and 0.77 in the test group; in the VP, the values were 0.81, 1, and 0.82 in the training group and 0.74, 1, and 0.74 in the test group.CONCLUSION:Texture analysis based on contrast-enhanced CT images can be used as an adjunct for the preoperative assessment of the pathological degrees of differentiation of PC.
[Diffusion-Weighted Imaging Texture Features in Differentiation of Malignant from Benign Nonpalpable Breast Lesions for Patients with Microcalcifications-Only in Mammography].

Abstract:OBJECTIVE:To evaluate the application of MR diffusion-weighted imaging(DWI) texture features in differentiation of malignant from benign nonpalpable breast lesion for patients with microcalcifications-only in mammography.METHODS:The clinical and MR-DWI data of 61 patients with microcalcifications, who underwent three-dimensional positioning of breast X-ray wire from October 2012 to December 2015 in Zhejiang Cancer Hospital, were retrospectively analyzed, including 38 patients with malignant lesions and 23 patients with benign lesions. Two radiologists independently drew the regions of interest (ROI) on DWI for image segmentation, and 6 histogram features and 16 grayscale symbiosis matrix (GLCM) texture features were extracted on each ROI. The random forest algorithm was applied to select the features and built the classification model. The leave-one-out cross-validation (LOOCV) was used to validate the classifier, and the performance of the classifier was evaluated by ROC curve.RESULTS:Six features were selected, including histogram features of mean, variance, skewness, entropy, as well as contrast (0°) and correlation (45°) in GLCM. The histogram features of mean, variance, skewness and entropy were significantly different between the benign and malignant breast lesions (all P<0.05). The AUC of the model was 0.76, and the diagnostic accuracy, sensitivity and specificity were 77.05%, 84.21% and 65.21%, respectively.CONCLUSIONS:The texture feature analysis of DWI can improve the diagnostic accuracy of differentiating benign and malignant breast nonpalpable lesions with microcalcifications-only in mammography. Histogram features of mean, variance, skewness, entropy of DWI may be used as important imaging markers.
Malignant or benign salivary gland lesions: predicting with MR imaging

Abstract:Objective To evaluate if MR imaging can be used to diagnosis the nature of the salivary gland lesions, and the accuracy of this modality. Methods Sixty patients with salivary gland diseases that included 7 cases of inflammatory lesions were analyzed respectively. All cases were diagnosed pathologically. A logistic regression model was used to determine which MR imaging features were significant factor for predicting the lesion being benign or malignant for each patient. Results Logistic regression model analysis in 60 cases revealed that metastasis was only significant MR characteristics for determining a lesion that was benign or malignant (P0.001). Its accuracy, sensitivity and specificity for predicting malignancy were 97.7%, 43.8%, 83.3%, respectively. However, the analysis in 53 cases (excluding inflammatory cases) demonstrated that metastases (P0.001) and infiltration (P0.001) were significant factors in predicting a malignant lesion, respectively. Their accuracy, sensitivity and specificity for predicting malignancy were 100%, 43.8%, 83.0% and 100%, 50%, 84.9%, respectively. When combining these two predictors to diagnosis malignancy, their accuracy, sensitivity and specificity could improve to 92.5%, 75%, and 100%. Conclusion Lymph node swelling and infiltration of tumor are useful factors to differentiate the malignant or benign lesions.
Texture Analysis of CT Images in Predicting Malignancy Risk of Gastrointestinal Stromal Tumours

Abstract:AIM: To explore the role of texture analysis of computed tomography (CT) images in predicting the malignancy risk of gastrointestinal stromal tumours (GISTs). MATERIALS AND METHODS: Seventy-eight patients with histopathologically confirmed GISTs underwent preoperative CT. Texture analysis was performed on unenhanced and contrast-enhanced CT images, respectively. Fourteen CT texture parameters were obtained and compared among GISTs at different malignancy risks with one-way analysis of variance or independent-samples Kruskal-Wallis test. Correlations between CT texture parameters and malignancy risk were analysed with Spearman's correlation test. Diagnostic performance of CT texture parameters in differentiating GISTs at low/very low malignancy risk was tested with receiver operating characteristic (ROC) analysis. RESULTS: Three parameters on unenhanced images (r=-0.268-0.506), four parameters on arterial phase (r=-0.365-0.508), and six parameters on venous phase (r=-0.343-0.481) imaging correlated significantly with malignancy risk of GISTs, respectively (all p<0.05). For identifying GISTs at low/very low malignancy risk, three parameters on unenhanced images (area under ROC curve [AUC], 0.676-0.802), four parameters on arterial phase (AUC, 0.637-0.811), and six parameters on venous phase (AUC, 0.636-0.791) imaging showed significant diagnostic performance, respectively (all p<0.05), especially maximum frequency on both unenhanced and contrast-enhanced images (AUC, 0.791-0.811). Abstract:To evaluate the value of texture analysis (TA) of conventional magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI) in the differential diagnosis between sinonasal non-Hodgkin’s lymphoma (NHL) and squamous cell carcinoma (SCC). Forty-two patients with sinonasal SCC and 30 patients with NHL were retrospectively enrolled. TAs were performed on T2-weighted image (T2WI), apparent diffusion coefficient (ADC) and contrast-enhanced T1-weighted image (T1WI). Texture parameters, including mean value, skewness, kurtosis, entropy and uniformity were obtained and compared between sinonasal SCC and NHL groups. Receiver-operating characteristic (ROC) curves and logistic regression analyses were used to evaluate the diagnostic value and identify the independent TA parameters. The mean value and entropy of ADC, and mean value of contrast-enhanced T1WI were significantly lower in the sinonasal NHL group than those in the SCC group (all P < 0.05). ROC analysis indicated that the entropy of ADC had the best diagnostic performance (AUC 0.832; Sensitivity 0.95; Specificity 0.67; Cutoff value 6.522). Logistic regression analysis showed that the entropy of ADC (P = 0.002, OR = 26.990) was the independent parameter for differentiating sinonasal NHL from SCC. TA parameters of conventional MRI and DWI, particularly the entropy value of ADC, might be useful in the differentiating diagnosis between sinonasal NHL and SCC.

Abstract:At the international transplantation meeting in Paris 2 years ago, 1 we reported our experience with clinical intestinal transplantation under a new immunosuppressant, FK 506. At the time of the report, 19 of the 23 recipients, or 83%, were alive with functioning grafts with a minimum follow-up of 2 months. We concluded that intestinal transplantation in humans is approaching clinical reality.1,2 Since then, we have performed 39 additional intestinal transplantations, but with worsening outcome. This article describes the results of 62 intestinal transplantations at the University of Pittsburgh.
CT Texture Analysis for Differentiating Bronchiolar Adenoma, Adenocarcinoma In Situ, and Minimally Invasive Adenocarcinoma of the Lung

Abstract:Purpose: This study aimed to investigate the potential of computed tomography (CT) imaging features and texture analysis to distinguish bronchiolar adenoma (BA) from adenocarcinoma in situ (AIS)/minimally invasive adenocarcinoma (MIA). Materials and Methods: Fifteen patients with BA, 38 patients with AIS, and 36 patients with MIA were included in this study. Clinical data and CT imaging features of the three lesions were evaluated. Texture features were extracted from the thin-section unenhanced CT images using Artificial Intelligence Kit software. Then, multivariate logistic regression analysis based on selected texture features was employed to distinguish BA from AIS/MIA. Receiver operating characteristics curves were performed to determine the diagnostic performance of the features. Results: By comparison with AIS/MIA, significantly different CT imaging features of BA included nodule type, tumor size, and pseudo-cavitation sign. Among them, pseudo-cavitation sign had a moderate diagnostic value for distinguishing BA and AIS/MIA (AUC: 0.741 and 0.708, respectively). Further, a total of 396 quantitative texture features were extracted. After comparation, the top six texture features showing the most significant difference between BA and AIS or MIA were chosen. The ROC results showed that these key texture features had a high diagnostic value for differentiating BA from AIS or MIA, among which the value of a comprehensive model with six selected texture features was the highest (AUC: 0.977 or 0.976, respectively) for BA and AIS or MIA. These results indicated that texture analyses can effectively improve the efficacy of thin-section unenhanced CT for discriminating BA from AIS/MIA. Conclusion: CT texture analysis can effectively improve the efficacy of thin-section unenhanced CT for discriminating BA from AIS/MIA, which has a potential clinical value and helps pathologist and clinicians to make diagnostic and therapeutic strategies.

Value of the Texture Feature for Solitary Pulmonary Nodules and Mass Lesions Based on Pet/Ct

Abstract:Objective The aim of this study was to explore the clinical value of the combination of prior knowledge of physicians and texture features for differentiation of malignant from benign solitary pulmonary nodules(SPNs) and mass lesions based on 18F-FDG PET/CT. Methods 143 patients with known solitary pulmonary nodule or mass were enrolled in the study. All the patients underwent 18F-FDG PET/CT examination. Two physicians judged the benign and malignant lesions of lung and the skewness, Angular Second Moment(ASM) and coarseness of PET/CT fusion images were measured. All patients were verified by pathological results or follow-up imagings after treatments.Results 106 patients were found to have malignant tumors and 37 patients were found with benign lesions.The sensitivity of prior knowledge, texture feature and combination of the two methods for diagnosis of pulmonary lesions were 88.7, 84.9, and 96.2%,respectively. Conclusion The result implies that combined texture feature of PET/ CT images may has potential to discriminate benign and malignant lesions and make up for deficiencies in individual parameters,and that the combination of the two methods generates better discrimination results than using one modality alone and plays a positive role for discrimination between benign and malignant lesions at present situation.
Machine learning-based CT texture analysis in the differentiation of testicular masses

Abstract:Purpose: To evaluate the ability of texture features for distinguishing between benign and malignant testicular masses, and furthermore, for identifying primary testicular lymphoma in malignant tumors and identifying seminoma in testicular germ cell tumors, respectively. Methods: We retrospectively collected 77 patients with an abdominal and pelvic enhanced computed tomography (CT) examination and a histopathologically confirmed testicular mass from a single center. The ROI of each mass was split into two parts by the largest cross-sectional slice and deemed to be two samples. After all processing steps, three-dimensional texture features were extracted from unenhanced and contrast-enhanced CT images. Excellent reproducibility of texture features was defined as intra-class correlation coefficient ≥0.8 (ICC ≥0.8). All the groups were balanced via the synthetic minority over-sampling technique (SMOTE) method. Dimension reduction was based on pearson correlation coefficient (PCC). Before model building, minimum-redundancy maximum-relevance (mRMR) selection and recursive feature elimination (RFE) were used for further feature selection. At last, three ML classifiers with the highest cross validation with 5-fold were selected: autoencoder (AE), support vector machine(SVM), linear discriminant analysis (LAD). Logistics regression (LR) and LR-LASSO were also constructed to compare with the ML classifiers. Results: 985 texture features with ICC ≥0.8 were extracted for further feature selection process. With the highest AUC of 0.946 (P <0.01), logistics regression was proved to be the best model for the identification of benign or malignant testicular masses. Besides, LR also had the best performance in identifying primary testicular lymphoma in malignant testicular tumors and in identifying seminoma in testicular germ cell tumors, with the AUC of 0.982 (P <0.01) and 0.928 (P <0.01), respectively. Conclusion: Until now, this is the first study that applied CT texture analysis (CTTA) to assess the heterogeneity of testicular tumors. LR model based on CTTA might be a promising non-invasive tool for the diagnosis and differentiation of testicular masses. The accurate diagnosis of testicular masses would assist urologists in correct preoperative and perioperative decision making.

Abstract:SIR, Generalized pustular psoriasis (von Zumbusch type) is often induced by abrupt termination of systemic corticosteroid therapy, bacterial infections, pregnancy, oral contraceptives and drugs such as b-blockers or antimalarials. It is characterized by groups of sterile pustules arising on erythematous skin with episodes of fever and subsequent waves of new pustules. Erythroderma, weight loss, muscle weakness, leucocytosis, hypocalcaemia and an increased erythrocyte sedimentation rate are associated with the disease. Besides elimination of provoking factors, first-line treatment is with systemic retinoids, occasionally combined with photochemotherapy. However, without adequate treatment, death may occur in one-third of cases. In 1996, a 76-year-old woman experienced a first manifestation of skin lesions which were histologically shown to be psoriasis vulgaris. The skin lesions responded to local treatment. Six years later, she developed a sudden eruption of severe generalized pustular psoriasis. This was treated with acitretin (up to 1 mg kg), with only partial response. Due to sudden agranulocytosis this therapy was terminated, resulting in immediate relapse. As subsequent therapy with methotrexate (up to 15 mg weekly) led to only insufficient control, ciclosporin (up to 3 mg kg) was added. Again, severe leucopenia occurred. Discontinuation of methotrexate resulted in granulocyte recovery, but ciclosporin alone was unable to suppress new eruptions and was stopped because of increased blood pressure. Dapsone 1 mg kg also caused immediate agranulocytosis, accompanied by an impressive clearance of cutaneous symptoms. However, leucocyte recovery was paralleled by a drastic deterioration of skin conditions. Following reports of improvement of plaque-type psoriasis in a number of patients by antitumour necrosis factor (TNF)-a antibody treatment, we initiated therapy with infliximab 5 mg kg as a single dose. Obvious improvement of the skin lesions was observed within 12 h, with complete disappearance of pustules and erythema within 48 h. After 1 week, signs of pustular psoriasis recurred and infliximab was given again. Again, total remission of skin lesions occurred within 48 h. This time, remission was lasting, with erythema and tiny pustules reappearing only 3 months later. A third course of infliximab was given and complete remission was observed within the following days. After 3 months a few erythematous macules bearing some pustules recurred, but were easily controlled by local therapy. The treatment was well tolerated and no adverse effects, including concomitant infections, decreased blood granulocytes or other laboratory changes, have been recorded. The central role of neutrophils in pustular psoriasis is deduced not only from the clinical picture of multiple sterile pustules on inflamed skin, but also from the obvious clearing of skin lesions during episodes of agranulocytosis in this case. The repeated critical decrease in granulocytes in our patient is ascribed mainly to toxic effects of the various drugs. Cyclical neutropenia can be excluded by the fact that blood cell counts showed no abnormalities during the last 6 months. TNF-a is a decisive mediator for the activation and tissue invasion of neutrophil granulocytes, e.g. by the induction of adhesion molecules on vascular endothelial cells. Thus TNF-a-neutralizing antibodies should provide a rational tool in the treatment of psoriasis, not only for chronic plaque type, but also for possibly life-threatening generalized pustular psoriasis, as has recently also been reported by others. This may especially apply if other treatment modalities are contraindicated or prove to be ineffective.
Analysis of the value of enhanced CT combined with texture analysis in the differential diagnosis of pulmonary sclerosing pneumocytoma and atypical peripheral lung cancer: a feasibility study

Abstract:Background: As a rare benign lung tumour, pulmonary sclerosing pneumocytoma (PSP) is often misdiagnosed as atypical peripheral lung cancer (APLC) on routine imaging examinations. This study explored the value of enhanced CT combined with texture analysis to differentiate between PSP and APLC. Methods: Forty-eight patients with PSP and fifty patients with APLC were retrospectively enrolled. The CT image features of the two groups of lesions were analysed, and MaZda software was used to evaluate the texture of CT venous phase thin-layer images. Independent sample t-test, Mann-Whitney U tests or χ2 tests were used to compare between groups. The intra-class correlation coefficient (ICC) was used to analyse the consistency of the selected texture parameters. Spearman correlation analysis was used to evaluate the differences in texture parameters between the two groups. Based on the statistically significant CT image features and CT texture parameters, the independent influencing factors between PSP and APLC were analysed by multivariate logistic regression. Extremely randomized trees (ERT) was used as the classifier to build models, and the models were evaluated by the five-fold cross-validation method. Results: Logistic regression analysis based on CT image features showed that calcification and arterial phase CT values were independent factors for distinguishing PSP from APLC. The results of logistic regression analysis based on CT texture parameters showed that WavEnHL_s-1 and Perc.01% were independent influencing factors to distinguish the two. Compared with the single-factor model (models A and B), the classification accuracy of the model based on image features combined with texture parameters was 0.84 ± 0.04, the AUC was 0.84 ± 0.03, and the sensitivity and specificity were 0.82 ± 0.13 and 0.87 ± 0.12, respectively. Conclusion: Enhanced CT combined with texture analysis showed good diagnostic value for distinguishing PSP and APLC, which may contribute to clinical decision-making and prognosis evaluation.

Abstract:Objective To establish a diagnostic model by combining imaging features with enhanced CT texture analysis to differentiate pancreatic serous cystadenomas (SCNs) from pancreatic mucinous cystadenomas (MCNs). Materials and Methods Fifty-seven and 43 patients with pathology-confirmed SCNs and MCNs, respectively, from one center were analyzed and divided into a training cohort (n = 72) and an internal validation cohort (n = 28). An external validation cohort (n = 28) from another center was allocated. Demographic and radiological information were collected. The least absolute shrinkage and selection operator (LASSO) and recursive feature elimination linear support vector machine (RFE_LinearSVC) were implemented to select significant features. Multivariable logistic regression algorithms were conducted for model construction. Receiver operating characteristic (ROC) curves for the models were evaluated, and their prediction efficiency was quantified by the area under the curve (AUC), 95% confidence interval (95% CI), sensitivity and specificity. Results Following multivariable logistic regression analysis, the AUC was 0.932 and 0.887, the sensitivity was 87.5% and 90%, and the specificity was 82.4% and 84.6% with the training and validation cohorts, respectively, for the model combining radiological features and CT texture features. For the model based on radiological features alone, the AUC was 0.84 and 0.91, the sensitivity was 75% and 66.7%, and the specificity was 82.4% and 77% with the training and validation cohorts, respectively. Conclusion This study showed that a logistic model combining radiological features and CT texture features is more effective in distinguishing SCNs from MCNs of the pancreas than a model based on radiological features alone.

Abstract:Defining the exact histological features of salivary gland malignancies before treatment remains an unsolved problem that compromises the ability to tailor further therapeutic steps individually. Radiomics, a new methodology to extract quantitative information from medical images, could contribute to characterizing the individual cancer phenotype already before treatment in a fast and non-invasive way. Consequently, the standardization and implementation of radiomic analysis in the clinical routine work to predict histology of salivary gland cancer (SGC) could also provide improvements in clinical decision-making. In this study, we aimed to investigate the potential of radiomic features as imaging biomarker to distinguish between high grade and low-grade salivary gland malignancies. We have also investigated the effect of image and feature level harmonization on the performance of radiomic models. For this study, our dual center cohort consisted of 126 patients, with histologically proven SGC, who underwent curative-intent treatment in two tertiary oncology centers. We extracted and analyzed the radiomics features of 120 pre-therapeutic MRI images with gadolinium (T1 sequences), and correlated those with the definitive post-operative histology. In our study the best radiomic model achieved average AUC of 0.66 and balanced accuracy of 0.63. According to the results, there is significant difference between the performance of models based on MRI intensity normalized images + harmonized features and other models ( p value < 0.05) which indicates that in case of dealing with heterogeneous dataset, applying the harmonization methods is beneficial. Among radiomic features minimum intensity from first order, and gray level-variance from texture category were frequently selected during multivariate analysis which indicate the potential of these features as being used as imaging biomarker. The present bicentric study presents for the first time the feasibility of implementing MR-based, handcrafted radiomics, based on T1 contrast-enhanced sequences and the ComBat harmonization method in an effort to predict the formal grading of salivary gland carcinoma with satisfactory performance.

Abstract:Objective: To determine the diagnostic value of combining conventional MRI, diffusion-weighted imaging (DWI) and dynamic contrast enhanced MRI (DCE-MRI) in salivary gland tumors. Methods: 45 patients with salivary gland tumors were evaluated with conventional MRI, DWI and DCE-MRI prior to surgery and confirmed by pathologic findings. The apparent diffusion coefficient (ADC) was calculated from DWI that was obtained with a factor of 0 and 1000 s mm(-2). A time-intensity curve (TIC) was obtained from DCE-MRI. Results: In conventional MRI, benign tumors often showed well-defined and clear margins, malignant tumors showed irregular margins or infiltration into the surrounding tissue. There were significant differences with regard to the ADC values between pleomorphic adenoma (1.72 +/- 0.29 x 10(-3) mm(2) s(-1)) and malignant tumors (0.95 +/- 0.09 x 10(-3) mm(2) s(-1) p < 0.05) and between adenolymphoma (0.74 +/- 0.05 x 10(-3) mm(2 )s(-1) and malignant tumors (p < 0.05). However, there was no significant differences in term of the ADC values between benign tumors (1.33 +/- 0.52 x 10(-3) mm(2) s(-1)) and malignant tumors. DCE-MRI showed benign tumors with A-type, B-type and D-type of TICs, and the malignant tumors with C-type TICs. A combination of all of these parameters yielded sensitivity, specificity, accuracy, and positive and negative predictive values of 90%, 97%, 95%, 90 and 97%, respectively. Conclusion: An evaluation combining MRI morphologic findings and functional MRI (ADCs and TIC) appears to be useful in differentiating benign from malignant tumors in salivary gland tumors. Advances in knowledge: The study firstly dealt with the combination of conventional MRI, DWI-MRI with DCE-MRI in salivary gland tumors.
