Abstract:Aims/Purpose: Oguchi disease is characterized by night blindness and a striking golden retinal sheen in the light‐adapted state which disappears following prolonged dark adaptation (Mizuo‐Nakamura phenomenon). The sheen is observable clinically and on colour and pseudocolour (Optos plc) imaging. We observed a localized loss of sheen in a patient on colour/pseudocolour imaging in an area corresponding to the 30 degree square illuminated by short wavelength (488 nm) autofluorescence (AF) imaging (Heidelberg Spectralis). We explored whether other cases showed this phenomenon. Methods: Patients in our service with Oguchi disease (diagnosed phenotypically and/or with genetic confirmation) who had undergone colour or pseudocolour imaging were identified. In these patients, note was made of whether prior 30 degree short wavelength autofluorescence images had been undertaken and whether a localized square‐shaped loss of sheen was observed. Results: We identified 13 patients who had undergone colour or pseudocolour imaging. Of these, 4 patients (2 males, 2 females) had undergone prior 30 degree (and not 55 degree) short wavelength AF imaging at the same visit (ages ranging from 12 to 67). In 2 patients (a 12 year old, with variants in GRK1 , and a 67 year old, not yet genotyped), a clear central square in which the sheen was absent was observed in the colour or pseudocolour images. Conclusions: The phenomenon of disappearance of the sheen in Oguchi disease following prolonged darkness is well‐described. Paradoxically, we observed loss of the sheen in 2 patients in a defined area corresponding to the field of illumination by bright blue light in 30 degree AF imaging. The blue light is preferentially absorbed by rhodopsin, which becomes bleached. It is possible that the sheen (at least in some patients) is dependent on there being a large proportion of unbleached rhodopsin as well as deficient shut‐off of activated rhodopsin.

Abstract:The Mizuo-Nakamura phenomenon describes a golden-yellow sheen of the retina [1], which disappears upon prolonged dark adaptation. The underlying mechanism is unclear. Oguchi disease arises from impaired inactivation of light-activated rhodopsin [2, 3]. In CSR, it is possible that impaired retinoid recycling in the area of neurosensory detachment reduces the pool of available rhodopsin, which might lead to localised loss of the sheen. Alternative mechanisms might include altered retinal pigment epithelium ion transport [4] potentially affecting potassium levels (some authors have postulated elevated extracellular potassium underlying the Mizuo phenomenon) [5].

Abstract:In recent years, concerns have escalated regarding eye health problems arising from Light-emitting diode (LED), which emits high-energy blue light (BL), potentially causing corneal epithelial dysfunctions (CEpD). Nevertheless, the mechanisms underlying this damage remain poorly comprehended. This study endeavors to explore the specific mechanisms through which BL exposure induces CEpD. The study carried out diverse assays and treatments to investigate the toxicological effects of BL exposure. 48 hours (h) of 440 nm of BL exposure decreased the migration of human corneal epithelial cells (hCEpCs) while augmenting reactive oxygen species (ROS) production and apoptosis. RNA-Sequencing and bioinformatic analysis indicated that cellular oxidation and reduction equilibrium, wound healing, the positive regulation of the apoptotic process, and the Phosphoinositide 3-kinase (PI3K)/AKT pathway were significantly influenced by BL exposure. Treatment with N-acetylcysteine (NAC), a ROS scavenger, restored cell viability and AKT/S6 kinase (S6K) activation, suggesting the involvement of ROS in BL-induced damage. NAC also reversed BL-induced apoptosis and migration. Blocking AKT/S6K replicated detrimental effects, while pre-treatment with SC79 (SC), an AKT activator, alleviated the changes caused by BL exposure in hCEpCs. Furthermore, in mice, the combination of AKT inhibition and BL exposure led to CEpD. However, treatment with SC and NAC restored CEpD caused by BL exposure. These results imply that the regulation of the ROS/PI3K/AKT/S6K axis is implicated in BL-induced CEpD. Collectively, this study offers insights into the molecular mechanisms of BL-induced CEpD and proposes targeting the ROS/PI3K/AKT/S6K cascade as a potential therapeutic approach. The findings contribute to ocular health knowledge and establish the basis for developing interventions to safeguard the cornea from the detrimental effects of excessive BL exposure.
Exogenous Ochronosis: Characterizing a Rare Disorder in Skin of Color

Abstract:Exogenous ochronosis is a rare dyschromia that primarily impacts those with skin of color. It is characterized by blue-black pigmentation and is associated with the long-term application of skin-lightening creams containing hydroquinone. Commonly confused with other dyschromias, the use of skin lightening topicals can cause paradoxical skin darkening in patients with known exogenous ochronosis. This is highly distressing to patients, often worsening the underlying dyschromia and making treatment more difficult. A 10-year retrospective analysis was conducted that revealed 25 patients with exogenous ochronosis. The average patient used a skin lightening cream for 9.2 years, with exogenous ochronosis most commonly arising on the cheeks (68%), forehead (24%), and temples (20%). Furthermore, this study identified that patients with exogenous ochronosis may respond well to treatment with Q-switched Alexandrite laser and microneedling. The incidence of exogenous ochronosis is likely to increase as demographics shift and access to a wide range of over-the-counter topicals becomes more available, both in the United States and worldwide. Therefore, it is imperative to better characterize exogenous ochronosis to identify best treatment practices for all patients.
UV light and the ocular lens: a review of exposure models and resulting biomolecular changes

Abstract:UV light is known to cause damage to biomolecules in living tissue. Tissues of the eye that play highly specialised roles in forming our sense of sight are uniquely exposed to light of all wavelengths. While these tissues have evolved protective mechanisms to resist damage from UV wavelengths, prolonged exposure is thought to lead to pathological changes. In the lens, UV light exposure is a risk factor for the development of cataract, which is a condition that is characterised by opacity that impairs its function as a focusing element in the eye. Cataract can affect spatially distinct regions of the lens. Age-related nuclear cataract is the most prevalent form of cataract and is strongly associated with oxidative stress and a decrease in the antioxidant capacity of the central lens region. Since UV light can generate reactive oxygen species to induce oxidative stress, its effects on lens structure, transparency, and biochemistry have been extensively investigated in animal models in order to better understand human cataract aetiology. A review of the different light exposure models and the advances in mechanistic understanding gained from these models is presented.

Abstract:BackgroundOchronosis/Alkaptonuria is a tyrosine metabolism disorder where accumulation of homogentisic acid, in eye, skin, cartilage and several other connective tissues leads to a black pigmentation of the affected tissues. It is autosomal-recessive inherited in men with a frequency of 1-9/1,000,000. While it is clear that pigment deposits lead to joint destruction, renal stone formation and cardiac valvulopathy respectively, the significance of ocular findings is still unclear. We therefore aim to evaluate the frequency and clinical significance of ocular findings in ochronosis and discuss possible therapeutic options.MethodsSystematic review of literature via Medline and Web of Science. Only case reports in English, German, French, Spanish or Italian documenting detailed ophthalmologic examination were included.ResultsOur search revealed 36 case reports including 40 patients. Average age at the onset of ocular signs was 40.6 years. The most frequent sign was symmetric brown sclera pigmentation present in 82.5 percent of the patients. “Oil-drops”, brown pigment spots in the limbus are generally considered pathognomonic but were a little less frequent (75 percent). Vermiform pigment deposits at the level of the conjunctiva or increased conjunctival vessel diameter is also frequent. We found an increased incidence of central vein occlusion and elevated intraocular pressure going along with chamber angle hyperpigmentation. Another condition observed twice is rapid progressive astigmatism attributable to corneoscleral pigment accumulation.ConclusionOur observations suggest that ocular findings are of double relevance. First, characteristic ocular findings can anticipate the time of diagnosis and second, ocular findings may complicate to various conditions putting sight at risk. Opthalmologists and general physicians should be aware of both. Therapeutic options include protein restriction, administration of high dose vitamin C or nitisonone. Evidence for all of them is limited.

The effect of visual blue light on mitochondrial function associated with retinal ganglions cells

Abstract:The retina is the only part of the central nervous system that is exposed to light radiation between 400 and 780 nm. Short wavelength light (SWL) ranging between 400 and 480 nm are absorbed maximally by chromophores located in mitochondria. An abundance of mitochondria are located in retinal ganglion cell (RGC) intraocular axons and photoreceptor inner segments and as a consequence SWL will impact these organelles. The purpose of this article is to summarise the experimental evidence for the possible negative effects of SWL on RGC mitochondria, in situ. The threat of damage to photoreceptor mitochondria may be less than to RGCs, since macular carotenoid, located chiefly in Henle's layer of the photoreceptor inner segment absorbs SWL. The article underlines the hypothesis that SWL contributes to RGC death when these neurones are not in an optimum homoeostatic state as is likely to occur in conditions such as glaucoma and possibly other types of pathologies and even old age. A case therefore exists for the idea that shielding RGCs to slow down visual loss in certain circumstances. This can theoretically be achieved with lenses that reduce transmission of SWL but specifically allow for maximal transmission of 479 nm light to stimulate melanopsin and maintain an optimum sleep/wake cycle.
One‐Year Ocular Safety Observation of Workers and Estimations of Microorganism Inactivation Efficacy in the Room Irradiated with 222‐Nm far Ultraviolet‐C Lamps

Abstract:Two krypton‐chloride germicidal excimer lamp units (Care222 TRT‐104C11‐UI‐U3, USHIO Inc.) were installed in the examination room of an ophthalmology department. The irradiation dose was set not to exceed the former (i.e., before 2022) threshold limit value (TLV) (22 mJ/cm2/8 hours) recommended by the ACGIH. Section 1: The eyes and lids of the 6 ophthalmologists (5 wore glasses for myopic correction) who worked in the room for a mean stay of 6.7 hours/week were prospectively observed for 12 months. Slitlamp examinations revealed neither acute adverse events such as corneal erosion, conjunctival hyperemia, lid skin erythema nor chronic adverse events such as pterygium, cataract, or lid tumor. The visual acuity, refractive error, corneal endothelial cell density remained unchanged during the study. Section 2: The irradiation of samples placed on the table or floor using the same fixtures in the room (5‐7.5 mJ/cm2) was associated with >99% inhibition of φX174 phage and >90% inhibition of S. aureus. In conclusion, no acute or chronic health effects in human participants was observed in a clinical setting of full‐room ultraviolet germicidal irradiation by 222‐nm lamp units and high efficacy in deactivation of microorganisms was determined in the same setting.

The spectrum of oculocutaneous disease: Part II. Neoplastic and drug-related causes of oculocutaneous disease

Abstract:There are a multitude of diseases that commonly affect both the skin and the eye. Part II of this 2-part series reviews the oculocutaneous manifestations of neoplasms, both benign and malignant, and adverse drug reactions affecting the skin and the eye. Though rare, a number of neoplasms that primarily involve the skin, such as melanoma and basal cell carcinoma, can metastasize to the eye, leading to permanent damage if not properly treated. In addition, periocular neoplasms can irritate the conjunctiva and lid, reducing a patient's ability to see clearly. Neoplastic diseases, such as xeroderma pigmentosum, Sturge-Weber syndrome, and multiple myeloma, can also lead to permanent changes in the eye if not discovered and managed promptly. Furthermore, there are a multitude of drugs, including those commonly used by dermatologists, which can result in permanent damage to the eye. With proper knowledge of the ocular manifestations and treatment recommendations described in this 2-part series, dermatologists with the assistance of their ophthalmology colleagues can help avoid the complications, including permanent blindness, associated with infectious, inflammatory, genetic, neoplastic, and drug-related conditions.
The susceptibility of the retina to photochemical damage from visible light

Abstract:The photoreceptor/RPE complex must maintain a delicate balance between maximizing the absorption of photons for vision and retinal image quality while simultaneously minimizing the risk of photodamage when exposed to bright light. We review the recent discovery of two new effects of light exposure on the photoreceptor/RPE complex in the context of current thinking about the causes of retinal phototoxicity. These effects are autofluorescence photobleaching in which exposure to bright light reduces lipofuscin autofluorescence and, at higher light levels, RPE disruption in which the pattern of autofluorescence is permanently altered following light exposure. Both effects occur following exposure to visible light at irradiances that were previously thought to be safe. Photopigment, retinoids involved in the visual cycle, and bisretinoids in lipofuscin have been implicated as possible photosensitizers for photochemical damage. The mechanism of RPE disruption may follow either of these paths. On the other hand, autofluorescence photobleaching is likely an indicator of photooxidation of lipofuscin. The permanent changes inherent in RPE disruption might require modification of the light safety standards. AF photobleaching recovers after several hours although the mechanisms by which this occurs are not yet clear. Understanding the mechanisms of phototoxicity is all the more important given the potential for increased susceptibility in the presence of ocular diseases that affect either the visual cycle and/or lipofuscin accumulation. In addition, knowledge of photochemical mechanisms can improve our understanding of some disease processes that may be influenced by light exposure, such as some forms of Leber's congenital amaurosis, and aid in the development of new therapies. Such treatment prior to intentional light exposures, as in ophthalmic examinations or surgeries, could provide an effective preventative strategy.
Delayed loss of cone and remaining rod photoreceptor cells due to impairment of choroidal circulation after acute light exposure in rats

Abstract:Purpose: To examine the long-term effects of acute photooxidative stress in the retina, retinal pigment epithelium (RPE), and choroid. Methods: Albino rats injected with either the protective antioxidant phenyl-N-tert-butylnitrone (PBN) or saline 30 minutes before exposure to 5 klx white fluorescent light for 6 hours were kept for up to 3 months in 5 lux cyclic light. Electroretinograms were recorded, and the outer nuclear layer (ONL) and the choroidal thickness and area were measured after hematoxylin-eosin (H&E) staining. The expression of rod, cone, and RPE cell markers was detected by Western blotting, and apoptosis was analyzed by TUNEL staining. Oxidative stress was analyzed by immunohistochemistry against 4-hydroxynonenal (4-HNE)-modified proteins. Retinal and choroidal ultrastructures were observed by transmission electron microscopy (TEM). Choroidal circulation was analyzed by in vivo staining of the choroidal layer by trypan blue. Results: In the saline-injected animals, TUNEL- and 4-HNE-labeling in the ONL, RPE, and choroid were higher 24 hours and 7 days after light exposure, and ERG amplitude, ONL and choroidal thickness and area, and rhodopsin and RPE65 expression were lower 7 or more days after light exposure than in phenyl-N-tert-butylnitrone (PBN)-injected animals. In the saline-injected animals, the expression of mid-wavelength opsin and the presence of cone cells in the ONL and the choroidal circulation were preserved for 7 days after light exposure but started to decrease by 1 month and continued to decrease for 3 months after light exposure. An increase in TUNEL-positive cells was observed in the ONL at the inferior peripheral retina, just behind the iris, by 3 months after light exposure. Delayed loss of cone cells, remaining rod cells, and choroidal circulation were counteracted by PBN treatment. Conclusions: Although cone cells are resistant to cell damage induced by acute photooxidative stress, progressive loss of cone cells continued for up to 3 months after light exposure. Impaired choroidal circulation is likely to be involved in the mechanism of delayed photoreceptor cell death after light exposure. Preserving choroidal circulation may provide a novel target for preserving the cone and the remaining rod cells in patients with retinal degeneration such as retinitis pigmentosa.
UV Light Is Not a Toy—Recreational Use and Eye Harm

Abstract:In their retrospective case series, Chan et al detail the clinical course of 8 patients who sustained photokeratitis after exposure to a decorative display of UV lights during an outdoor public event. The authors describe the presenting symptoms of photokeratitis, including pain, photophobia, redness, and tearing. Fortunately, visual acuity was minimally affected, and all patients fully recovered after supportive treatment. This report demonstrates the critical need to increase public awareness of and regulation for the potential deleterious effects of UV light exposure on the eye. Patients have been exposed to UV light during activities such as prolonged outdoor recreation, use of germicidal lamps, and exposure to decorative UV bulbs.

Temporary Myopic Shift in a Patient with Multiple Parallel-Line Endotheliitis

Abstract:Here, we report the case of a patient with multiple parallel-line endotheliitis with myopic shift, which has not been previously reported. A 36-year-old man visited our clinic with blurred vision in his left eye. A slit lamp examination revealed an interesting pattern of multiple parallel lines of keratic precipitates, along with subtle corneal edema. The spherical equivalents measured in the right and left eyes were -9.25 and -11.875 diopter, respectively, with the left eye showing more myopic progression. On specular microscopy, endothelial cell loss was found in the left eye. After administration of a weak topical steroid, keratic precipitates and corneal edema resolved completely within 2 days. The myopic shift in his left eye recovered to -10.0 diopters. Multiple parallel-line endotheliitis can be easily treated with a weak topical steroid in a short period. A temporary myopic shift may occur, which in this case resolved upon disease recovery. However, corneal endothelial cell loss is inevitable; therefore, careful monitoring is needed.
Topical nutraceutical Optixcare EH ameliorates experimental ocular oxidative stress in rats

Abstract:Purpose: Based on the hypothesis that oral nutraceuticals do not adequately reach all ocular tissues in the anterior segment, we evaluated the ability of a 3% concentration of the ingredients in a topical nutraceutical antioxidant formulation called Optixcare Eye Health (Optixcare EH) to ameliorate oxidative stress in rat models of age-related ocular diseases. Methods: Diabetes was induced by tail-vein injection of streptozotocin, and the development of cataracts was monitored by slit lamp. Young rats were exposed to ultraviolet (UV) light, and the reduction in lens glutathione (GSH) levels and increase in 4-hydroxynonenol (4-HNE) were measured. Oxidative stress in the neural retina was generated by exposure of dark-adapted rats to 1,000 lx of light, and oxidative stress markers were measured. Dry eye was induced in rats by twice daily (b.i.d.) subcutaneous scopolamine injections. Topical Optixcare EH was administered b.i.d. and compared in select experiments to the multifunctional antioxidant JHX-4, the topical aldose reductase inhibitor (ARI) Kinostat™, oral Ocu-GLO™, and the topical ocular comfort agents Optixcare Eye Lube, Optixcare Eye Lube + Hyaluron, and Idrop Vet Plus hyaluronic acid. Results: In diabetic rats, topical ARI treatment prevented cataract formation while the nutraceuticals delayed their development with Optixcare EH>Ocu-GLO. In UV-exposed rats, the reduction of GSH and increase in 4-HNE in the lens were normalized in order JHX-4>Optixcare EH>Ocu-GLO. In the retina, oxidative stress markers were reduced better by oral JHX-4 compared with topical Optixcare EH. In the scopolamine-induced dry-eye rats, tear flow was maintained by Optixcare EH treatment, while none of the comfort agents examined altered tear flow. Conclusions: Topical administration of a 3% concentration of the ingredients in Optixcare EH reduces experimentally induced reactive oxygen species in rats exposed to several sources of ocular oxidative stress. In addition, Optixcare EH maintains tear volume in scopolamine-induced dry eye. This suggests that in the anterior segment, the ingredients in Optixcare EH may have clinical potential against ocular oxidative stress.
Chronic Retinal Injury Induced by White LED Light with Different Correlated Color Temperatures As Determined by Microarray Analyses of Genome-Wide Expression Patterns in Mice.

Abstract:Widely used white light-emitting diodes (LEDs) currently deliver higher levels of blue light than conventional domestic light sources. The high intensity of the blue component is the main source of concern regarding possible health risks of LED to chronic light toxicity to the retina. Therefore, we analyzed retinal injury and genome-wide changes in gene expression induced by white LED light with different correlated color temperatures (CCTs) in a mouse model. Balb/c mice (10 weeks old) were exposed to LED light with CCTs of 2954, 5624, and 7378 K, at different illuminance levels (250, 500, 1000, and 3000 lx) and for different exposure times (7, 14, and 28 days). Hematoxylin and eosin staining revealed that exposure to 7378 K light at 250 lx for 28 days resulted in a significant reduction of outer nuclear layer (ONL) nuclei, whereas 2954 K light at < 3000 lx led to only a mild reduction in the number of ONL nuclei. In addition, 5624 and 7378 K light at 3000 lx resulted in a significant increase in TUNEL-positive apoptotic nuclei, which was not found at an illuminance of 1000 lx. Genome-wide expression analyses showed that, compared to a control group, there were 121 upregulated differentially expressed genes (DEGs) and 458 downregulated DEGs found in the 7378 K group, and 59 upregulated and only 4 downregulated DEGs in the 2954 K group. Gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses showed that the DEGs were involved in 341 GO terms and 16 related pathways for the 7378 K group and in 12 GO terms and 7 related pathways for the 2954 K group. Signal pathways related to ubiquitin potentially played an important role in light-induced retinal degeneration. Furthermore, retinal immunohistochemistry (IHC) indicated downregulation of ubiquitin and autophagy function caused by 7378 K light. Taken together, these results indicate that retinal injury in the mice induced by white LED light occurred in a CCT-dependent manner, and that light with a higher CCT was more likely to reduce ONL nuclei; however, the apoptosis pathway may not be the only mechanism involved. Based on genomewide expression analyses and retinal IHC, the ubiquitin-mediated proteolysis signal pathway may have participated in the induction retinal degeneration.

Abstract:There has been an increased understanding of the protective effect of two or more hours in high lux light on the development and progression of myopia. The aim of myopia management is to reduce the incidence of high myopia and sight-threatening myopic complications. Equally important are the sight-threatening complications of ultraviolet radiation (UVR) on the eye and adnexal structures. This review will analyze the literature for both these epidemics to help guide public health policy. Whilst increasing childhood high lux light exposure is important, consideration of a holistic eye health policy should ensure that UV eye diseases are also prevented. The advent of ultraviolet (UV) fluorescence photography has increased our understanding that significant UV eye damage occurs in childhood, with 81% of children aged 12-15 years having signs of UV eye damage. Hence, the need to reduce myopia and protect from UV-related eye diseases needs simultaneous consideration. Advocating for eye protection is important, particularly as the natural squint reflex is disabled with dark sunglasses lenses. The pathways UV reaches the eye need to be considered and addressed to ensure that sunglasses offer optimum UV eye protection. The design of protective sunglasses that simultaneously allow high lux light exposure and protect from UVR is critical in combating both these epidemics.

Abstract:Background: High Intensity Macro and Micro Focused Ultrasound ( HIFU) is a safe and effective method for the treatment of skin laxity. However, the application of high-intensity focused ultrasound energy on eyelids has been associated with potential ocular complications including traumatic cataract, iridocyclitis, and conjunctival hemorrhage, among others. Case presentation: A 40-year-old female developed blurred vision in her left eye after receiving HIFU treatment on binocular eyelids, and her left far vision was 20/66. The examination revealed left eye iris depigmentation and conjunctival hemorrhage. Both eyes exhibited multiple white streaking or tadpole-shaped opacities in the lenses. Conclusion: Excessive ultrasonic energy generated by HIFU can cause protein denaturation, leading to conditions such as traumatic cataract, visual impairment, injuries to the iris and conjunctiva when applied to the eyes. We recommend that individuals undergoing cosmetic treatment in the periorbital region should be highly aware of the possible ocular side effects.

Abstract:Fuchs endothelial corneal dystrophy is a heterogenous disease with multifactorial etiology, and genetic, epigenetic, and exogenous factors contributing to its pathogenesis. DNA damage plays a significant role, with ultraviolet-A (UV-A) emerging as a key contributing factor. We investigate the potential application of neuropeptide α-melanocyte stimulating hormone (α-MSH) in mitigating oxidative stress induced endothelial damage. First, we examined the effects of α-MSH on a cultured human corneal endothelial cell line (HCEnC-21T) exposed to hydrogen peroxide (H2O2) induced oxidative DNA damage. We performed immunofluorescence and flow cytometry to assess DNA damage and cell death in the cultured cells. Additionally, we used an established mouse model that utilizes ultraviolet light to induce corneal endothelial cell damage resulting in decreased CEnC number, increased cell size variability, and decreased percentage of hexagonal cells. This endothelial decompensation leads to an increase in corneal thickness. Following UV-A exposure, the mice were systemically treated with α-MSH, either immediately after exposure (early treatment) or beginning two weeks post-exposure (delayed treatment). To evaluate treatment efficacy, we analyzed CEnC density and morphology using in vivo confocal microscopy, and central corneal thickness using anterior segment optical coherence tomography. Our findings demonstrated that α-MSH treatment effectively protects HCEnC-21T from free-radical induced oxidative DNA damage and subsequent cell death. In vivo, α-MSH treatment, mitigated the loss of CEnC density, deterioration of cell morphology and suppression of the resultant corneal swelling. These results underline the potential application of α-MSH as a therapeutic agent for mitigating corneal endothelial damage.
Transcriptomic analyses reveal the molecular mechanisms underlying the ocular surface damage in treated glaucoma patients

Abstract:Aims/Purpose: Topical glaucoma drugs have a high prevalence of ocular surface disease (OSD), affecting to its compliance. The clinical signs of OSD are very similar to those observed in patients with dry eye syndrome; however, the mechanisms mediating the onset of symptoms are unknown. The purpose of this study is to identify conjunctival transcriptomic changes in patients treated with topical medication and patients with dry eye and compare them to healthy controls. Methods: Impression cytology was performed, and samples were selected for RNA sequencing; 33 patients treated for glaucoma, 9 with dry eye and 14 healthy controls. After a quality check, the reads were aligned, and gene expression was quantified by counting exonic regions. Ingenuity Pathway Analysis identified the routes and biologic processes associated with transcriptomic changes. Results: In treated patients, up‐regulation of immune‐related genes represented the 79% of the altered pathways. In dry eye samples, immune system and inflammation represented 39% of the altered routes; increased expression of genes related to mitochondrial metabolism and oxidative stress represented 16% and decreased expression of genes involved in maintaining conjunctival integrity and cell differentiation represented 15%. Conclusions: According to our results, an exacerbation of the immune system is observed in patients with glaucoma treatment as the main pathway involved in ocular surface damage. However, in the case of patients with dry eye, the damage is the result of multiple pathways. Knowledge of the mechanisms involved in ocular surface damage is essential in the search for effective treatments for these patients.
