Abstract:Objective To define the values of E/Em in healthy singleton fetuses during the second and third trimester of gestation,with which and with traditional ultrasound and tissue Doppler imaging(TDI)diastolic function was analyzed integrately.Hence,to assess the evolution throughout gestation.Methods Fetal echocardiography were performed in 327 normal fetals within 18-42 weeks by two-dimensional,M-mode,pulsed-wave Doppler(PWD)and TDI.The atrioventricularvalves inflow velocity(including E wave and A wave)by PWD and the atrioventricularvalves annulus motion(including Em wave,Am wave and Sm wave)by TDI were measured in all fetuses.E/A,Em/Am and E/Em were calculated.The differences in variables and the evolution of E/Em with gestation were analyzed.Results There was negative correlation between E/Em and other diastolic indices.The values of the E/Em in right ventricules were lower than those of left ventricules.The values of other indices in right ventricules were greater than those of left ventricules.The E/Em of both ventricules decreases significantly throughout pregnancy.And the decreasing amplitude of E/Em was greater in the second than in the third trimester.Conclusions The E/Em relates strongly to filling pressure,which can reflects the cardiac diastolic function.Therefore,it is a simple,sensitive and reliable indicator.Reference charts for E/Em were constructed,in the meantime,the evolution of E/Em during gestation was studied.These will allow identification of fetuses with an abnormal diastolic function.E/Em plays an important role in prenatal management.
